RESUMO

The notion of consumerism and that students are customers of pharmacy colleges was explored by proponents and opponents of the idea. First, a working definition of a "customer" in pharmacy education is pondered with respect to the roles and responsibilities of students and schools/colleges of pharmacy. Second, the pros and cons of "student-centered" education are considered in the light of students and their families being consumers of the educational experience. Third, the duality of student-centered education is discussed including student engagement/disengagement in their learning, professional/unprofessional behaviors, and shared/individual responsibilities. Lastly, learning and teaching environment dynamics are discerned when higher education becomes more student-centric and how that may affect the overall outcome of the student and the goals of pharmacy educational programs.

In the aftermath of the COVID-19 pandemic, the intricate relationship between health and the environment has emerged with unparalleled significance. This investigation examines the effect of leisure-time physical activity (LTPA) on health and environmental consciousness and its influence on attitudes towards green/sustainable products among 533 individuals. Utilizing linear structural modeling and regression analysis, the findings reveal that participation in sports and leisure activities significantly mediates the connection between individual well-being and eco-responsible consumer behaviors. Highlighting LTPA's crucial role in enhancing environmental awareness, this study offers invaluable perspectives for the green product sector. It advocates for the development of strategies that align with consumers' environmental values, underscoring the essential function of sports and leisure in fostering sustainable consumer practices. Crucially, this underscores the intertwined nature of environmental sustainability and individual health, highlighting their mutual dependence.

In this article, we discuss the ethics of veganism by looking at the moral implications of having a vegan diet or plant-based diet. Veganism is a way of diet that forbids or avoids consuming any animal products. It has drawn a lot of interest recently due to awareness or trends of vegan diet, human health, and ethical behavior benefits. The aim of the research is to look into the moral values that talk about veganism, such as animal welfare, environmental sustainability, and human welfare. In this study, we conduct a thorough assessment of ethical theories and actual evidence in an effort to know about the ethical reasons for veganism and its larger societal impacts. Through an analysis of existing literature and clinical studies, we discuss the various challenges, advantages, lifestyle modification, and nutritional concerns related to a vegan diet.

Sexual assault rates are increasing with ride-shares. Sexual violence data are often underestimated due to sexual violence cases going unreported and can lead to negative health outcomes. There is no research on the phenomenon of sexual assaults among ride-share users. This manuscript investigates sexual assaults from two ride-share companies and the policies/procedures in place to prevent sexual assaults. The data from two ride-share companies' previous Safety Reports were compared to see an increase in sexual assaults. The researchers looked at policies and regulations that ride-share companies have to prevent sexual assault. There is a call for research to investigate more in-depth the sexual assaults that have occurred including demographic data, geographic location, global positioning system failure, and the effectiveness of the criminal background checks.

In this paper, I raise some doubts about Nicole Hassoun's account of the obligations of states, pharmaceutical firms, and consumers with regard to global health, presented in Global Health Impact. I argue that it is not necessarily the case, as Hassoun claims, that if states are just, and therefore satisfy all of their obligations, then consumers will not have strong moral reasons, and perhaps obligations, to make consumption choices that are informed by principles and requirements of justice. This is because there may be justice-based limits on what states can permissibly and feasibly do both to promote access to existing drugs for all of those who need them, and to promote research and development for new drugs that could treat diseases that primarily affect the global poor. One important upshot of my argument is that there can be reasons for organizations like the Global Health Impact Organization to exist, and to do the kind of work that Hassoun argues is potentially valuable in our deeply unjust world, even in much less unjust worlds in which states and firms largely, or even entirely, comply with their obligations.

Introduction: Value in health care is described as the measured improvement in a patient's health outcomes for the cost of achieving that improvement. In the United States, value-based care has been heralded by providers, payers, and policymakers alike, as a path to addressing the challenges facing the healthcare system and achieving the aspirational goals of the Quadruple Aim of healthcare. Primary care is often viewed as the foundational cornerstone for implementing value-based care. However, primary care is also considered as ground-zero for the rise in healthcare consumerism. Methods: In essence, consumerism refers to increasing expectations from patients (consumers) to be more active participants in decisions related to their healthcare. While much of the literature has portrayed the rise in consumerism as a barrier to the implementation of value-based primary care, some have argued that it may have potential to synergize with and facilitate the implementation of value-based primary care. This paper applies an enhanced stepwise implementation framework for value-based (equitable) care, to examine the potential for conflict and synergy between consumerism and value-based care in the emerging retail model of primary care. The application is based on the potential actions of four key stakeholder groups: (1) retail healthcare entities, (2) primary-care providers, (3) consumers (patients), and (4) healthcare payers. Results: The analysis helps to articulate the responsibilities of each stakeholder group in ensuring synergy between consumerism and value-based primary care. In addition, it helps to identify three drivers of synergy between consumerism and value-based care: (1) trust in the patient-provider relationship, (2) connected consumer-centric technology solutions, and (3) value-based consumer-centric payment models. Discussion: Overall, the application helps to articulate a comprehensive framework for implementing value-based care that incorporates both the principles of consumerism and active consideration for health equity.

BACKGROUND: With the increasing availability of information, patients are becoming more informed about radiology procedures and requesting imaging studies. This qualitative study aims to explore factors that influence general practitioners' (GPs) decisions to fulfil patient requests for imaging studies during clinical consultation. METHODS: Semi-structured interviews were conducted with 10 GPs working across five private medical centres in Northwest Sydney. Conventional content analysis was used with emergent themes to identify GPs perspectives. RESULTS: Six themes stood out from the interviews with GPs fulfilling patient requests for imaging studies. They included four pertaining to patient factors: patient expectations, 'therapeutic scans', 'impressive labels' and entitled. Two further themes pertained to the GP perspective and included defensive medicine, and 'new patients'. Requests are fulfilled from anxious or health-obsessed patients, with GPs worrying about litigation if they refuse. However, GPs decline requests from patients with entitlement attitudes or during first visits. DISCUSSION: The findings suggest that GPs struggle to balance their responsibilities as gatekeepers of imaging with patients' expectations of request fulfilment. Clear guidelines on the appropriate use of diagnostic imaging and its limitations could help patients understand its proper use and ease anxiety. Additionally, education and training for GPs could help them manage patient expectations and provide appropriate care. PATIENT CONTRIBUTIONS: Patients, service users, caregivers, people with lived experiences or members of the public were not directly involved in the design, conduct, analysis or interpretation of the study. However, our study was conducted in primary care facilities where the GPs were interviewed about patients' requests for diagnostic imaging based on their own initiatives. GPs' perspectives in managing patient expectations and healthcare utilisation were explored within the Australian Medicare system, where medical imaging and image-guided procedures come at little to no cost to the individual. The study findings contribute to a better understanding of the challenges faced by GPs in dealing with patient consumerism and requests for diagnostic imaging, as well as factors influencing request fulfilment or denial. Insights gained from this study may inform future research about delivering patient-centred care within a similar context.

Hearing care is expanding accessibility to consumers through new service delivery channels and methods of technology distribution (see Brice et al, this issue). This diversification has the potential to overcome longstanding consumer disparities (e.g., health, socioeconomic, psychological, environmental) in receiving care and provider constraints (e.g., accessibility, geography, direct access) to delivering care that adversely impacts quality of life (e.g., social isolation, depression, anxiety, self-esteem). In this article, the reader is provided with an overview of health outcomes factors (i.e., determinants of health)-in the context of an economic framework (i.e., supply, demand)-and their effect on consumer behavior and provider preferences toward hearing healthcare services. This overview also affords readers with strategic business insights to assess and integrate future hearing care services and technology to consumers in their local markets.

BACKGROUND: Digitization (using novel digital tools and strategies) and consumerism (taking a consumer-oriented approach) are increasingly commonplace in clinical trials, but the implications of these changes are not well described. METHODS: We assembled a group of trial experts from academia, industry, non-profit, and government to discuss implications of this changing trial landscape and provide guidance. RESULTS: Digitization and consumerism can increase the volume and diversity of trial participants and expedite recruitment. However, downstream bottlenecks, challenges with retention, and serious issues with equity, ethics, and security can result. A "click and mortar" approach, combining approaches from novel and traditional trials with the thoughtful use of technology, may optimally balance opportunities and challenges facing many trials. CONCLUSION: We offer expert guidance and three "click and mortar" approaches to digital, consumer-oriented trials. More guidance and research are needed to navigate the associated opportunities and challenges.

Chile is facing an environmental crisis and the territory of the Mapuche people is no exception. This is largely due to extractivism, which refers to the massive extraction and exploitation of natural resources in an indiscriminate manner. The objective of this study was to reveal the implications of extractivism and environmental pollution in Mapuche territories in the Araucanía region. The methodology used was qualitative, based on constructivist grounded theory. In-depth interviews and participant observation were used to collect data. The participants were 46 kimeltuchefes. The main results revealed extensive monocultures of non-native trees: pine and eucalyptus, which consume large amounts of water. They also revealed environmental pollution and indiscriminate forestry extractivism related to these trees, which generate soil degradation and water pollution. These consequences reduce biodiversity and disturb the ngenh (spiritual beings and protectors of nature). They also affect the Mapuche's agricultural activities and, in turn, their health and subsistence. In addition, non-native tree monocultures, environmental pollution and forestry extractivism transgress the az mapu (Mapuche code of ethics and behaviour), which disturbs the ethical, moral and spiritual relationship between the Mapuche and nature. They also have negative implications for the küme mogen (good living of the Mapuche), since they violate the balance and harmony between the Mapuche and all living beings, elements and spiritual beings that are part of nature. This also violates the reciprocity between the Mapuche and nature. It was concluded that there have been violations of the human rights of the Mapuche people, given that they are exposed to harmful environmental conditions that put their health and subsistence at considerable risk. In this sense, the Mapuche are experiencing a spiritual, physical, cognitive, attitudinal, affective and material imbalance. Ultimately, the state of Chile must generate intercultural environmental public and educational policies aimed at generating environmental awareness and creating actions to solve environmental problems in order to protect Mapuche and non-Mapuche territories.

This study set out to better understand virtual consumerism (VC) by applying natural language processing (NLP) methods for sentiment and content analyses. A total of 318 articles related to VC were identified on theguardian.com Web site and analyzed by text mining methodology. A thematic, content analysis using the Leximancer program was performed to explore VC as a concept, and its related concepts and concept associations. For the purposes of "deep-dive insights," further content and sentiment analyses were performed with MonkeyLearn and valence aware dictionary for sentiment reasoning. This triangulation in methodology enabled a comprehensive unstructured qualitative data analysis. The study identified key themes that characterize and define VC. It uncovered that, although there is predominantly positive sentiment toward VC reported in The Guardian online articles, negative sentiment also exists, presenting challenges for the industry to maneuver. The findings reveal that in the context of VC, a virtual experience is also a social experience in a virtual space, which is becoming and evolving. There are certain industries and sectors that are embracing VC, such as marketing, advertising and public relations, software development/IT, art/design, and entertainment, as well as science/technology. Some sectors and industries are experiencing challenges, such as security/law enforcement and medical, and hence display negative sentiment toward VC. Overall, this study presents a working definition of VC, a synopsis of the state of VC, and highlights areas for potential research to further our understanding of this phenomenon. It contributes to an improved understanding of VC for the industry and academia, and provides impetus for future studies focused on the emergent VC-relevant conceptual relationships.

OBJECTIVE: Families affected by asthma report difficulty adhering to care regimens because of high medication costs, coupled with increased cost sharing required by some insurance plans. To inform efforts to support adherence, we conducted a qualitative study to explore how families manage asthma care costs. METHODS: We conducted phone interviews with commercially-insured, US adults (n = 59) who had asthma and/or a child with asthma. Our purposive sample included participants with high- and low/no-deductible health plans. We analyzed data using thematic content analysis to identify strategies for managing asthma care costs and to assess strategies' implications for adherence. RESULTS: Our analysis identified four overarching strategies for managing asthma care costs. First, participants used prevention strategies to avoid costly acute care by minimizing exposure to asthma triggers and adhering strictly to preventive medication regimens. Second, participants used shopping strategies to reduce costs, including by comparing medication prices across pharmacies, using medication coupons or free samples, and switching to lower-cost medications. Third, budgeting strategies involved putting aside funds, including in tax-exempt health savings accounts, or taking on debt to pay for care. Finally, some participants sought to reduce costs by forgoing recommended care, including by skipping medication doses or replacing prescribed medications with alternative therapies. CONCLUSION: Commercially-insured families use a wide range of strategies to manage asthma care costs, with both positive and negative implications for adherence. Our typology of asthma cost management strategies can inform insurance redesign and other interventions to help families safely reduce costs and maximize adherence to recommended care.

Resumen Introducción: Desde el consumo político, las estrategias de comunicación en campañas electorales muestran la importancia de estudiar los mensajes que se propagan a través de distintos medios digitales y sociales con el propósito de influir en la opinión pública para obtener un mayor número de votos. Por lo mismo, es relevante identificar y analizar las estrategias comunicativas racionales y emotivas presentes en los mensajes de campañas electorales que se relacionan con el denominado voto ideológico o de rendimiento. Método: Se realizó un estudio lingüístico descriptivo comparativo de los contenidos publicados por 15 candidatos políticos a elecciones territoriales (2019) de cinco ciudades principales de Colombia, en la red digital Twitter. Resultados: Los resultados muestran que los candidatos políticos comparten el uso de 20 estrategias comunicativas racionales y dos emotivas. No obstante, son los candidatos electos quienes hacen más énfasis en el discurso racional y emotivo en contexto sociopolítico. Conclusiones: El discurso de los candidatos políticos elegidos como alcaldes se distingue por el uso de estrategias comunicativas racionales centradas en el bienestar social con un tono emocional positivo, lo cual desvela que las propuestas electorales ganadoras usaron estrategias de comunicación orientadas a la obtención de un voto por rendimiento más que ideológico.


Abstract Introduction: From the political consumerism, communication strategies in electoral campaigns show the importance of studying the messages that are propagated through different digital and social media with the purpose of influencing public opinion to obtain a greater number of votes. To identify and analyze the rational and emotional communicative strategies present in the messages of electoral campaigns that are related to the so-called ideological or performance vote. Method: A comparative descriptive linguistic descriptive study of the contents published by 15 political candidates to territorial elections (2019) of five principal cities of Colombia, in the digital network Twitter, was carried out. Results: The results show that political candidates share the use of 20 rational and 2 emotional communicative strategies. However, it is the elected candidates who place more emphasis on rational and emotive discourse in socio-political context. Conclusions: The discourse of the political candidates elected as mayors, is distinguished by the use of rational communicative strategies focused on social welfare with a positive emotional tone, which reveals that the victorious electoral proposals used communication strategies oriented to obtaining a vote by performance rather than ideological.

Patient-centered care and healthcare consumerism are the two most dominant ideas about the relationship between patients and providers in the United States. To identify providers' positions between the two perspectives, we analyzed the content of direct-to-consumer healthcare service advertisements. The advertisements were collected in the state of Nevada (N = 323) and their landing pages were analyzed for provider attributes, patient experience features, and terms referring to patients and providers. The results showed that the advertisements fully embraced the notion of patient-centeredness by commonly claiming patient-centered care and frequently using the term "patient." The advertisements also contained multiple indicators of healthcare consumerism, although they avoided using the terms "consumer/customer/client" closely associated with consumerism. Contrary to the prominence of patient experience features, provider attributes were not common. An additional analysis of inter-specialty differences in advertising features confirmed the strong consumerism position of cosmetic surgery providers. Application of the healthcare service advertising analytic scheme developed for this study could help providers and healthcare administrators recognize how their advertising messages may reflect their values.

Consumerism favours the culture of the disposable, excessive extroversion, and superficiality. Data manipulation, the shadow of information technology, harms individual and collective lives. In a world filled with prejudice, intolerance, violence and social inequality, nature is neglected putting our survival at risk. The lack of appreciation for subjectivity and self-knowledge and the demand for greater performance foster physical and emotional problems expressed through anxiety, depression, anguish and burnout. It is thus urgent to develop a pattern of consciousness that allows us to overcome polarization and cope with the opposites by the means of dialogue and an appreciation of the dialectical and symmetrical relationship between the polarities. Carlos Byington, in his symbolic psychology, named this the archetypal pattern of alterity, and considered it the basis of solidarity, respect for difference, democracy and sustainability.


Le consumérisme favorise une culture du jetable, une extraversion excessive, et la superficialité. La manipulation de l'information, l'ombre de la technologie de l'information, font du tort aux vies individuelles et collectives. Dans un monde rempli de préjugés, d'intolérance, de violence, d'inégalité sociale, la nature est victime de négligence, et ceci met en péril notre survie. Le manque de reconnaissance de la subjectivité et de la connaissance de soi ainsi que l'exigence d'une performance plus grande nourrissent l'apparition de problèmes physiques et émotionnels qui s'expriment par de l'angoisse, de la dépression, de la douleur et de l'épuisement (burnout). Ainsi il est urgent de construire un schéma de conscience qui nous permette de surmonter la polarisation et de nous débrouiller des opposés par le moyen du dialogue et d'une revalorisation de la relation dialectique et symétrique entre les polarités. Carlos Byington, dans sa psychologie symbolique, a appelé ceci le schéma archétypal de l'altérité, et considère ceci comme le fondement de la solidarité, du respect de la différence, de la démocratie, et de la durabilité.

This study contributes to the emerging literature on the negative effects over consumption that social media users may develop as a consequence of being engaged on social media platforms. The authors tested materialism's direct and indirect impacts on compulsive, conspicuous, and impulsive buying, adding two novel mediators: attitudes toward social media content (SCM) and social media intensity (SMI). The study uses a convenience sample of 400 Thai social media users analyzed using structural equation modeling. The results confirmed the well-established positive relationships between materialism and each of the three-negative consumption behaviors also in the social media domain. A novel finding showed the important role played by SMI which was found to be a strong predictor of each of the three negative consumption behaviors and it was also found to significantly mediate the relationship between materialism and the three-negative consumption behaviors. An additional contribution of the study was found on the role of attitudes which, contrary to what is commonly believed, were often not significant in predicting any negative behavior.

The phrase 'person-centred care' (PCC) reminds us that the fundamental philosophical goal of caring for people is to uphold or promote their personhood. However, such an idea has translated into promoting individualist notions of autonomy, empowerment and personal responsibility in the context of consumerism and neoliberalism, which is problematic both conceptually and practically. From a conceptual standpoint, it ignores the fact that humans are social, historical and biographical beings, and instead assumes an essentialist or idealized concept of personhood in which a person is viewed as an individual static object. From a practical standpoint, the application of such a concept of personhood can lead to neglect of a person's fundamental care needs and exacerbate the problems of social inequity, in particular for older people and people with dementia. Therefore, we argue that our understanding of PCC must instead be based on a dynamic concept of personhood that integrates the relevant social, relational, temporal and biographical dimensions. We propose that the correct concept of personhood in PCC is one in which persons are understood as socially embedded, relational and temporally extended subjects rather than merely individual, autonomous, asocial and atemporal objects. We then present a reconceptualization of the fundamental philosophical goal of PCC as promoting selfhood rather than personhood. Such a reconceptualization avoids the problems that beset the concept of personhood and its application in PCC, while also providing a philosophical foundation for the growing body of empirical literature that emphasizes the psychosocial, relational, subjective and biographical dimensions of PCC.

Starting with the current pandemic, a potent symbol of death for all, the author explores the need to transform our vision of, and position in, the world. The author describes the world in which we live in terms of accelerated transformation and extreme imbalance: environmental suicide, consumerism, environmental pollution, global warming and intense polarization brought about by techno-populism. She reflects on the enantiodromia of our current times in which the focus has shifted from extreme spirituality to the opposite extreme of materialism. She returns to the concepts of the psychoid archetype and unus mundus, and analogous concepts in quantum physics, examining the need to review the polarities of psyche and matter in the search for a new synthesis. The author then concludes by highlighting the importance of consciousness in the elaboration and construction of a new way of being in the world.


Prenant pour point de départ la pandémie actuelle - un puissant symbole de mort pour chacun - l'auteur explore le besoin de transformer notre vision du monde et notre position dans le monde. What people eat has become a highly political issue, closely intertwined with public health, environmental concerns, and climate change. Individuals' consumption decisions tend to be greatly influenced by the people that surround them, and this seems to be especially true when it comes to food. In recent years, alongside close contacts, such as family and friends, a myriad of social influencers have appeared on the screens, sharing opinions on what (not) to eat. Presenting results from a youth survey conducted in Sweden in 2019 (N = 443), this paper shows that social media have become the primary source of information about food and eating for youths, followed by schools and families. However, primary sources of influence continue to be parents and the family at large. Furthermore, the study shows that it is possible to identify 'central persons', i.e., relatively clear-cut groups of people whose food choices-measured as tendency to eat climate friendly-is mirrored by the youths, both in their everyday food preferences and in their broader political awareness as expressed through political consumerism. A conclusion from this is that certain people can be particularly successful at inspiring larger numbers of other people to engage with healthier and environmentally friendlier (food) consumption in a society.
