RESUMO

BACKGROUND: The continuous advancement in ultrasound technology has given rise to Acoustic Radiation Force Impulse (ARFI) elastography, which boasts non-invasiveness, ease of operation, rapid inspection, and high accuracy. It has been successfully employed in detecting tissue hardness across various diseases. This study aims to investigate the application of acoustic radiation force pulse imaging technology in evaluating the efficacy of calf intermuscular vein thrombosis. METHODS: This study is retrospective in nature, involving a total of 120 patients diagnosed with calf intermuscular venous thrombosis (MCVT) who were admitted to our hospital. These patients were selected retrospectively as the subjects for our research. They were subsequently divided into two groups: the control group and the observation group. The control group received standard nursing care and simple pressure therapy, while the observation group underwent anticoagulant drug treatment. The shear wave elastic hardness of both groups was measured, with the recording of ultrasonic elasticity scores and the average elastic modulus value (E-mean, in kPa). Furthermore, a comparison was made between the two groups regarding thrombus disappearance time, blood flow patency, and the clinical treatment effect. RESULTS: At the 1, 3, and 6-month marks of the treatment period, the ultrasonic elasticity scores in the observation group were consistently higher compared to those in the control group. Additionally, the shear wave elastic hardness in the observation group was consistently lower than that in the control group, and these differences were found to be statistically significant (p < 0.05). The total effective rates for the control and observation groups were 83.33% and 95.00%, respectively. Notably, the clinical total effective rate in the observation group was significantly higher than that in the control group, and this difference was statistically significant (p < 0.05). The thrombus disappearance time in the observation group was significantly shorter than that in the control group, and the blood flow rate was significantly higher than in the control group, with both differences being statistically significant (p < 0.05). CONCLUSION: ARFI plays a crucial role in assessing the efficacy of MCVT by effectively revealing the hardness and location of the patient's thrombus tissue. This technology aids doctors in gaining a more precise understanding of the deep vein thrombosis condition. Notably, ARFI is characterized by high safety levels and exhibits positive effects due to its painless and non-invasive nature.

RESUMO

OBJECTIVE: To observe the efficacy and safety of supramolecular salicylic acid monotherapy and supramolecular nicotinamide in the treatment of chloasma. METHODS: A total of 28 female patients with chloasma diagnosed in the dermatology outpatient department of our hospital were randomly divided into an experimental group and a control group, with 14 cases in each group. All patients were treated with 30% supramolecular salicylic acid every 2 weeks, for a total of 8 treatments. The experimental group was treated with 10% supramolecular nicotinamide once in the morning and once in the evening, and the control group was a blank control. Before each exfoliation treatment, subjects were photographed with a VISIA skin detector, and skin image analysis and modified melasma area and severity index (MASI) score were performed. RESULTS: According to the MMASI decrease rate (%) before and after treatment, the effective rate of the experimental group was 64.29%, and the effective rate of the control group was 14.29%. The results of the GriffithS10 and VISIA were improved. During the whole treatment, there was one mild adverse reaction in both groups. CONCLUSION: Salicylic acid stripping combined with nicotinamide in the treatment of chloasma is safe and effective, can improve skin conditions, and can be widely applied in clinical practice.

RESUMO

OBJECTIVE: To explore the value of applying computed tomography (CT) radiomics based on different CT-enhanced phases to determine the immunotherapeutic efficacy of non-small cell lung cancer (NSCLC). METHODS: 106 patients with NSCLC who underwent immunotherapy are randomly divided into training (74) and validation (32) groups. CT-enhanced arterial and venous phase images of patients before treatment are collected. Region-of-interest (ROI) is segmented on the CT-enhanced images, and the radiomic features are extracted. One-way analysis of variance and least absolute shrinkage and selection operator (LASSO) are used to screen the optimal radiomics features and analyze the association between radiomics features and immunotherapy efficacy. The area under receiver-operated characteristic curves (AUC) along with the sensitivity and specificity are computed to evaluate diagnostic effectiveness. RESULTS: LASSO regression analysis screens and selects 6 and 8 optimal features in the arterial and venous phases images, respectively. Applying to the training group, AUCs based on CT-enhanced arterial and venous phase images are 0.867 (95% CI:0.82-0.94) and 0.880 (95% CI:0.86-0.91) with the sensitivities of 73.91% and 76.19%, and specificities of 66.67% and 72.19%, respectively, while in validation group, AUCs of the arterial and venous phase images are 0.732 (95% CI:0.71-0.78) and 0.832 (95% CI:0.78-0.91) with sensitivities of 75.00% and 76.00%, and specificities of 73.07% and 75.00%, respectively. There are no significant differences between AUC values computed from arterial phases and venous phases images in both training and validation groups (P < 0.05). CONCLUSION: The optimally selected radiomics features computed from CT-enhanced different-phase images can provide new imaging marks to evaluate efficacy of the targeted therapy in NSCLC with a high diagnostic value.

RESUMO

OBJECTIVE: To observe the clinical efficacy of Jujing Decoction combined with lipoic acid in the treatment of asthenospermia and teratospermia. METHODS: Fifty patients with asthenospermia and teratospermia meeting the inclusion criteria were randomly divided into a blank control (n = 10) and a medication group (n = 40), the former provided with fertility guidance and the latter treated with Jujing Decoction combined with lipoic acid. After 12 weeks of treatment, the clinical therapeutic effects were evaluated by comparing the semen volume, sperm concentration, percentages of progressively motile sperm (PMS) and morphologically abnormal sperm (MAS), rate of acrosome integrity, sperm DNA fragment index (DFI) and pregnancy rate between the two groups. RESULTS: Compared with the control group, the medication group achieved a significantly higher overall effectiveness rate (10% ï¼»1/10ï¼½ vs 88.89% ï¼»32/36ï¼½, P < 0.05) and pregnancy rate (0% ï¼»0/10ï¼½ vs 8.33% ï¼»3/36ï¼½, P < 0.05) after treatment. The medication group also showed remarkably increased PMS from (21.04 ± 6.49)% to (32.66 ± 7.05)%, decreased MAS from (98.31 ± 1.28)% to (96.52 ± 1.11)%, elevated acrosome integrity from (42.18 ± 16.67)% to (60.42 ± 11.61)%, and reduced sperm DFI from (21.92 ± 6.96)% to (12.37 ± 3.79)%, all with statistically significant differences compared with the blank control group. CONCLUSION: Jujing Decoction combined with lipoic acid can significantly improve sperm motility, reduce MAS and DFI and increase the pregnancy rate through antioxidant stress, and has a high clinical safety.

RESUMO

BACKGROUND: Stress urinary incontinence is a common and widespread problem among women that can significantly reduce the quality of life of those affected and potentially result in limitations to the lives and activities of their friends and family members. We aimed to observe the effect of floating needle therapy combined with acupuncture point embedding in the treatment of female stress urinary incontinence after childbirth. METHODS: A total of 64 postpartum female stress urinary incontinence patients who were treated and hospitalized in the Urology Clinic of the Ningbo Traditional Chinese Medicine Hospital of the Zhejiang Province from September 2020 to August 2021 were randomly allocated to the experimental group and the control group, each with 32 cases. The control group was treated with the drug midodrine combined with Kegel training, and the experimental group was treated with floating needle therapy combined with acupoint embedding. After 8 weeks of continuous treatment, the pelvic floor muscle strength, urinary incontinence score, and the improvement of urine leakage in the 1 h pad test were observed. We then evaluated the clinical efficacy of the 2 groups and the state of urinary incontinence at 3 months and 6 months as assessed by telephone follow-up. RESULTS: After treatment, the pelvic floor muscle strength of the 2 groups had significantly increased, and both the urinary incontinence score and the urine leakage volume of the 1 h pad test were significantly reduced. The pelvic floor muscle strength of the experimental group was significantly higher than that of the control group after treatment, the urinary incontinence score and the amount of urine leakage of the 1 h pad test were significantly lower than those of the control group. The total effective rate of the experimental group was 90.63% (27/32), which was higher than that of the control group (71.88%, 23/32). CONCLUSIONS: With the benefits of being both convenient and efficient, floating needle therapy combined with press-needle acupoint embedding is effective in treating female stress urinary incontinence after childbirth. RESUMO

Background: Functional dyspepsia (FD) is one of the most critical health problems worldwide. Although there has been an increased intervention to improve FD symptoms, it is difficult to compare the effect of intervention measures with the existing methods of reporting the outcome, and it is a lack of clinical evaluation tools that can be used to evaluate patients' symptoms and treatment. One way of potentially addressing this way is to offer a patient-reported symptom scoring scales, which can be self-reported by patients to highlight interventions' authenticity and reliability. Nevertheless, there is still a lack of validated patient-reported outcome instruments for post-prandial distress syndrome (PDS). This study aims to establish a symptom scoring scale to evaluate the effectiveness of interventions for PDS. Methods: The study consists of two steps. The first step was to formulate the scale. Through a systematic literature review and group discussion, an item pool and scale framework were formed. Then, through the expert consultation and pre-investigation, the formal version of the scale was formed. The second step is to test the reliability and validity of the scale. The scale is tested in the target population to determine whether the reliability and validity of the scale. Discussion: The improvement in patients' self-reported symptoms had a significant impact on the researchers' evaluation of the intervention's authenticity. Therefore, we develop a symptom scoring scale for reporting studies evaluating the effectiveness of PDS interventions. The scale will be used for a more significant comparison to evaluate PDS interventions' effectiveness. The scale also improves trial reporting, reducing research waste by prioritizing the collection and reporting of critical results for all relevant stakeholders. RESUMO

Objective:To investigate the predictive value of quantitative parameters of ultrasound on early neoadjuvant chemotherapy (NAC) in breast cancer. Methods:Between March 2018 and October 2020, breast cancer patients who underwent surgery after NAC treatment in Renmin Hospital of Wuhan University were collected. Multimodal ultrasound examinations were performed before chemotherapy and after NAC in the second cycle. The quantitative parameters of ultrasound included diameter (D), peak enhancement (PE), time to peak (TTP), maximum shear wave velocity (Vmax) and mean shear wave velocity (Vmean). The change rates of corresponding parameters (ΔD, ΔPE, ΔTTP, ΔVmax, ΔVmean) were calculated. According to Miller & Payne (MP) pathological reaction classification, 93 patients were divided into effective group (37 cases) and ineffective group (56 cases). The differences of quantitative parameters between the two groups were compared. Multivariate logistic regression was used to analyze the effective quantitative parameters of NAC response, and ROC curve was plotted to analyze the predictive effect of quantitative parameters on NAC efficacy. Results:Before NAC, there were no significant differences in clinical data and pathological features between the two groups except age (all P>0.05). After the second cycle of chemotherapy, ΔPE and ΔVmax were significantly correlated with the efficacy of NAC ( t=-3.417, P=0.001; Z=-5.173, P<0.001). Multivariate regression analysis showed ΔPE and ΔVmax were effective predictors of NAC ( OR=1.105, 95% CI=1.032-1.184, P=0.004; OR=1.232, 95% CI=1.125-1.349, P<0.001). The cutoff values of ΔPE and ΔVmax in evaluating the efficacy of NAC were 16.37% and 28.52%. ΔPE>16.37%, and ΔVmax>28.52% suggested the pathologically effective response of NAC. The sensitivity and specificity of ΔPE, ΔVmax and ΔPE+ ΔVmax were 83.78%, 57.14%; 78.38%, 85.71%; 75.68%, 91.07%, respectively. Conclusions:The quantitative ultrasound parameters have certain clinical value in early prediction of NAC efficacy of breast cancer, and the combined application of parameters can help to improve the prediction efficiency.

RESUMO

Objective:To explore the effect and mechanism of Xiangshenwan on ulcerative colitis (UC) induced by dextran sulfate sodium (DSS) in mice based on the classic Toll-like receptor (TLR)/nuclear factor kappa B (NF-κB) signaling pathway. Method:The experimental mice were divided into a normal group, a model group, a Xiangshenwan group, and a mesalazine group. The mice, except for those in the normal group, received 3% DSS solution for 7 days to establish the acute UC model and were treated with Xiangshenwan (5 g·kg-1) and mesalazine (300 mg·kg-1) continuously from the 1st day to the 10th day of modeling. The body weight, disease activity index (DAI), colon weight, intestinal weight index, colon length, colon weight per unit length, and pathological changes of mice were evaluated respectively. The protein expression of TLR5, myeloid differentiation factor 88 (MyD88), interleukin-1 receptor-associated kinase 4 (IRAK4), tumor necrosis factor receptor-associated factor 6 (TRAF6), transforming growth factor β-activated kinase 1 (TAK1), p38 mitogen-activated protein kinase (MAPK), NF-κB, IRAK1, TAK1-binding protein 1 (TAB1), TAB2, mitogen-activated protein kinase kinase 3 (MKK3), MKK6 and cyclic adenosine monophosphate response element-binding protein (CREB) in colon tissues of mice was detected by Western blot. Result:Compared with the normal group, the model group showed decreased body weight of mice, increased DAI scores, elevated colon weight, intestinal weight index, and colon weight per unit length, shortened colon length, severe colonic mucosal injury, and up-regulated protein expression of TLR5, MyD88, IRAK4, TRAF6, TAK1, p38 MAPK, NF-κB, IRAK1, TAB1, TAB2, MKK3, MKK6, and CREB in colon tissues (P<0.05, P<0.01). Compared with the model group, the Xiangshenwan group and the mesalazine group displayed increased body weight of mice, decreased DAI scores, declining colon weight, intestinal weight index, and colon weight per unit length, increased colon length, improved colonic mucosal injury, and down-regulated protein expression of TLR5, MyD88, IRAK4, TRAF6, TAK1, p38 MAPK, NF-κB, IRAK1, TAB1, TAB2, MKK3, MKK6, and CREB in colon tissues (P<0.05, P<0.01). Conclusion:Xiangshenwan can effectively treat DSS-induced UC presumedly by the inhibition of TLR/NF-κB signaling pathway.

RESUMO

Electroencephalogram (EEG) has been an important tool for scientists to study epilepsy and evaluate the treatment of epilepsy for half a century, since epilepsy seizures are caused by the diffusion of excessive discharge of brain neurons. This paper reviews the clinical application of scalp EEG in the treatment of intractable epilepsy with vagus nerve stimulation (VNS) in the past 30 years. It mainly introduces the prediction of the therapeutic effect of VNS on intractable epilepsy based on EEG characteristics and the effect of VNS on EEG of patients with intractable epilepsy, and expounds some therapeutic mechanisms of VNS. For predicting the efficacy of VNS based on EEG characteristics, EEG characteristics such as epileptiform discharge, polarity of slow cortical potential changes, changes of EEG symmetry level and changes of EEG power spectrum are described. In view of the influence of VNS treatment on patients' EEG characteristics, the change of epileptiform discharge, power spectrum, synchrony, brain network and amplitude of event-related potential P300 are described. Although no representative EEG markers have been identified for clinical promotion, this review paves the way for prospective studies of larger patient populations in the future to better apply EEG to the clinical treatment of VNS, and provides ideas for predicting VNS efficacy, assessing VNS efficacy, and understanding VNS treatment mechanisms, with broad medical and scientific implications.

RESUMO

BACKGROUND: The clinical treatment of patients with severe diabetic foot (DF) is difficult. Recently, the First Affiliated Hospital of Guangxi Medical University began to apply tibial transverse transport (TTT) in patients with DF. This treatment has achieved significant effects, but its mechanism of action is unclear. Recently, microcirculation and the pathogenesis of diabetes have become the foci of research in this field. The evaluation of the possible mechanism of microcirculation reconstruction requires relevant indicators. The aim of this study was to investigate the value of computed tomography perfusion in evaluations of the curative effects of TTT and establish corresponding quantitative evaluation indicators. METHODS: Twelve patients with DF treated with TTT were recruited as the research participants. All diabetic feet were divided into the transport foot (TF) group and nontransport foot (NTF) group according to whether the patients underwent TTT. All patients underwent CT shuttle scanning preoperatively and 8 weeks after surgery. The shuttle scan data were transferred to Siemens VPCT body software and postprocessed with Customized Tumor2. We chose the TF posterior tibial artery from a distance of approximately 15 cm to the bifurcation of the plantar medial artery and the lateral plantar artery as the input artery. We selected the centre of the bilateral medial plantar muscle group on the coronal and axial regions of interest. We applied a deconvolution approach to obtain data from both sides of the plantar tissue perfusion. Skin temperature (ST) detection was performed with an ST gun to measure the average ST values in the dorsal and plantar areas, the first and fifth heads of the phalanges, and the medial and lateral malleolus points of both feet of patients with DF preoperatively and 8 weeks after surgery. RESULTS: The preoperative and postoperative ST values of the patients in the TF group were 30.73 ± 1.86 °C and 32.22 ± 1.51 °C, respectively. The preoperative and postoperative ST values for the patients in the NTF group were 30.93 ± 2.65 °C and 32.07 ± 2.09 °C, respectively. There were significant differences in the preoperative and postoperative data between the TF (P = 0.001) and NTF (P = 0.013) groups. In the patients with DF who underwent TTT, there were both preoperative and postoperative differences inside the medial plantar muscle group in the relative blood volume (rBV) value and relative mean transit time (rMTT) (P = 0.027, P = 0.026, respectively). The postoperative BV in the NTF group was increased compared with the preoperative BV (P = 0.006). CONCLUSION: There were significant differences in relative BV, relative mean transit time, and ST between the two groups before and after surgery, and the postoperative BV in the NTF group and ST values in the two groups were increased compared with the preoperative values. The BV in the NTF group and the ST values in the two groups were effective indicators in evaluating the changes between preoperative and postoperative perfusion. These results indicate that TTT could increase plantar tissue perfusion as assessed by BV and ST; this increase was among the reasons for the surface healing of severe DF ulcers. TRANSLATIONAL POTENTIAL OF THIS ARTICLE: Preoperative CT perfusion evaluation can provide relevant information of blood vessels and microcirculation for clinical operation, and postoperative CT perfusion evaluation can provide postoperative efficacy evaluation for clinical operation. All patients received information about the study and signed a specific informed consent. Approval for this study was granted by the regional ethics committee [Regional Ethics Committee of the First Affiliated Hospital of Guangxi Medical University, China [(2018-(KY-E-069].

RESUMO

Revaluation of syndrome therapeutic effect of post-marketing Chinese materia medica (CMM) is a necessary complement to pre-ipo syndrome therapeutic effect evaluation. With the transition of research stage and research target, the evaluation method needs a breakthrough and innovation. So considering the academic characteristics of syndrome theory of traditional Chinese medicine (TCM), concerning the key link in the process of design and implementation in syndrome curative effect evaluation, and analyzing the common problems in syndrome curative effect evaluation are the urgent needs to change the current syndrome curative effect evaluation. Through analyzing the type and level of efficacy evaluation in TCM and interpreting the current situation and demand of syndrome therapeutic effect revaluation for post-marketing CMM, here this paper tried to put forward the research of revaluation of syndrome therapeutic effect for post-marketing CMM, which involved in research of symptoms unit, expression of specification symptoms, and symptoms of selection and optimization. Then drawing lessons from study design with effective registration and combining the concept of causality research could list some inspiration for how to establish the syndrome therapeutic effect revaluation for post-marketing CMM.

RESUMO

Objective To evaluate the application of serum procalcitonin(PCT)eetection in patients with severe infection. Methods Seventy-six infectee patients in respiratory intensive care unit(RICU)of the First People′s Hospital of Shangqiu were selectee as our subjects. Patients were eivieee into A group(28 cases of general bacterial infection),B group(36 cases of severe bacterial infection ),C group(12 cases of viral infection). Serum PCT was eetectee at the 1st,3re,5th,7th,9th eays after hospitalization. Results PCT concentration in A,B,C groups were((2. 7 ± 0. 8),(18. 9 ± 5),(0. 6 ± 0. 2))μg/ L respectively,ane there was significant eifference between 3 groups(F = 213. 73,P = 0. 000). The PCT level in B group was higher than of A group,ane PCT in A group was higher than in C group( all P < 0. 05). In B group,PCT concentration at 1st,3re,5th,7th,9th eays after treatment were((18. 9 ± 5),(18. 3 ± 5. 7),(16. 4 ± 5. 5), (12. 2 ± 4. 1),(8. 5 ± 2. 8),(6 ± 2. 1))μg/ L. In A group,PCT concentration at before treatment,1st,3re,5th, 7th,9th eays after treatment were((2. 7 ± 0. 8),(2. 6 ± 0. 7),(2. 3 ± 0. 8),(1. 7 ± 0. 6),(1. 3 ± 0. 5),(0. 9 ± 0. 3))μg/ L respectively. There were significant eifferent between two groups(F between group = 626. 07,F inner group = 75. 21,F interaction = 43. 36,P < 0. 01). After 5 eay antibiotic treatment in two group,PCT concentration at post-treatment were significantly eecreasee comparee with pre-treatment( P < 0. 01). Conclusion The eetection of serum PCT is valuable for eiagnosis ane evaluation of the eegree of clinical bacterial infection. PCT monitor can be usee to evaluate the therapeutic effect,ane assist the clinician to juege the prognosis.
