OBJECTIVE: The purpose of this cross-sectional observational study was to evaluate the effects of SpO2 in a sample of dental health care providers who wear a N95 mask or Filtering Face Piece (FFP2) for four consecutive hours, measured by a pulse oximeter before donning the mask and again after four hours of work and to offer some strategies to minimize discomfort and improve communication with their patients while wearing the mask. MATERIALS AND METHODS: A 17-item questionnaire was sent via Google Drive to various practitioners in Italy and the USA. A sample of 162 questionnaires were returned from dentists, orthodontists, dental hygienists and dental assistants who committed to wearing a FFP2 for 4 consecutive hours during a work day and then measuring the oxygen saturation by way of a pulse oximeter before and after the 4 working hours. The final analysis was performer on 147 viable questionnaires returned. The sample was composed of 62 males and 85 females with an average age of 42.9 ± 12.0 years. RESULTS: For the entire sample population, the baseline saturation was 98.6 ± 1.2 and, after four hours of mask wearing, there was a significant decrease in oxygen saturation to 97.0 ± 2.9 (p < 0.01). No statistical differences in SpO2 were found across specialties or across types of procedures performed during the 4 h. Heart rates were not significantly different before and after the 4 h in all categories. The 3 most frequent reported complaints were: fatigue (64%), headache (36%) and external ear pain (31%). The most common additional personal protective equipment (PPE) was a mask shield (78%) and those who wore the mask continuously reported more communication difficulty with patients, compared with those who took the mask off more often, in fact, 64% of the subjects reported that using the mask influenced their communication with their patients. Based on the results of the questionnaire, a list of breathing and vocal folds health strategies was devised and proposed, along with strategies to augment communication with patients. CONCLUSIONS: This study highlights a significant decrease in oxygen saturation after only 4 h of work (except for smokers) while wearing a FFP2, and confirms the widespread symptoms of fatigue, headache and pain behind the ears that dental professionals experience. But it also highlighted how mask wearing impaired communication with patients and wearing additional masks and a facial shield may add to those communications difficulties. This aspect and the need for better communication can lead the operators to remove the mask to improve breathing and communication, thus putting themselves at a risk of infection. Of all the aspects explored in this study, the most interesting was indeed the impact on fatigue and communication and the strategies proposed in this article can easily be implemented to reduce headache and fatigue by improving breathing efficiency and by aiding communication while donning a mask by improving voice quality and by using augmentative communication tools.

Coronavirus disease 2019 (COVID-19) is an infectious disease that emerged at the end of 2019. On 30 January 2020, the World Health Organization (WHO) classified it as a pandemic. To examine the psychological effects on dental care providers in China in the midst of the COVID-19 outbreak and factors closely associated with those effects, we conducted a cross-sectional study online with 4 widely used self-administered questionnaires: the Patient Health Questionnaire-9, the General Anxiety Disorder-7, the Perceived Stress Scale-10, and the Acute Stress Disorder Scale. Univariate and multivariate analyses were performed to evaluate the variables that potentially affected the mental health of emergency dental care providers. As a result, 969 out of 1035 questionnaires were included in the analysis, with 642 respondents reporting more than 1 symptom (66.3%). The symptom of perceived stress was reported by the largest proportion of the respondents (66.2%, n = 641), and anxiety the least (7.1%, n = 69). After adjustment for confounders, it was found that dental practitioners with preexisting physical health conditions were at higher risk of depression (odds ratio [OR], 1.972; 95% CI, 1.128-3.448; P = .017), and perceived stress (odds ratio, 2.397 95% CI, 1.283-4.478; P = .006). Additionally, feelings of fear, helplessness, or terror resulting from the possibility of contracting COVID-19 were significantly associated with the prevalence of all the 4 psychological symptoms observed (P < .05). In the present study, we found that dental care providers suffered psychological depression, stress, anxiety, and posttraumatic stress disorder (PTSD) during COVID-19, which indicates the importance of psychological support at times of major epidemic outbreaks. Objetivo: Conocer la percepción de los odontólogos clínicos peruanos sobre el uso de la teleodontología para la atención odontológica durante la pandemia de COVID- 19. Métodos: Estudio cualitativo en el que se utilizó el método fenomenográfico y se realizaron entrevistas a dentistas clínicos en el contexto de la cuarentena por la pandemia de la COVID-19 en Lima, Perú. Las entrevistas se efectuaron a través de la plataforma Zoom, y la guía de preguntas claves incluyó: conocimiento previo sobre teleodontología, medios de comunicación utilizados con los pacientes, experiencia en la atención virtual de los pacientes desde el inicio de la cuarentena, calidad del diagnóstico y tratamiento a través de la teleodontología, remuneración de las teleconsultas, beneficios y limitaciones de la teleodontología. Resultados: Once de los 14 odontólogos consideró que es complicado mantener una comunicación a distancia, sobre todo en los casos que requieren control de tratamientos ya realizados. Los odontólogos entrevistados opinan que gran parte de los pacientes se sienten tranquilos al mantener relación con su odontólogo, aun cuando es a distancia, y manifiestan que la teleodontología crea lazos de confianza con los pacientes por el trato constante. Consideran que el contacto por redes sociales puede ser un medio para que aumente la fluidez en la comunicación, lo que ayudaría en los casos de pacientes no muy extrovertidos, al trato persona-persona. Conclusiones: Para los odontólogos entrevistados, la utilización de redes sociales y videollamadas es fundamental, porque permiten mantener el contacto con sus pacientes, consideran que por medio de consejos y recomendaciones a distancia se pueden aliviar molestias y tranquilizar a los pacientes(AU)


Objective: Become acquainted with the perception of Peruvian clinical dentists about the use of teledentistry for dental care during the COVID-19 pandemic. Methods: A qualitative study was conducted based on the phenomenographic method, and interviews were carried out with clinical dentists in the context of the quarantine due to the COVID-19 pandemic in Lima, Peru. The interviews were held on the Zoom platform, and the guide of key questions included the following items: previous knowledge about teledentistry, means of communication used with patients, experience in the virtual care of patients since the start of the quarantine, quality of diagnosis and treatment through teledentistry, teleconsultation fees, and benefits and limitations of teledentistry. Results: Eleven of the 14 dentists considered that it is complicated to maintain distance communication, mainly in cases requiring the control of treatments already performed. The dentists interviewed think that a large number of the patients feel reassured when they maintain contact with their dentist, even if from a distance, reporting that teledentistry creates confidence in patients due to the frequent contact. They think that contact on the social networks may facilitate communication, which would be helpful in the case of introverted patients. Conclusions: The dentists interviewed consider that the use of social networks and video calls is fundamental, for it makes it possible to maintain contact with their patients(AU)

BACKGROUND: A wide range of microorganisms poses a threat to patients and dental care teams. Implementation of safety guidelines is thus essential to prevent infection in dental clinics. AIM: To investigate the level of infection-control practices among dental health-care providers in Jordan. METHODS: A self-administered questionnaire was distributed to dental care providers from hospitals, academic institutions, and dental centres. The questionnaire covered sociodemographic variables, professional characteristics, and recommended guidelines of infection control. RESULTS: A total of 190 questionnaires were obtained out of 500 distributed questionnnaires (response rate: 38%). Females accounted for 62.6% of the sample and 64.7% were dentists. The majority was vaccinated against hepatitis B (82.1%) and wash their hands as usual behavior before (66.3%) and after (83.2%) treatment. Approximately 87.9% wear gloves and 78.9% wear masks while performing dental procedures. Autoclave sterilization and puncture-resistant containers for sharp instruments were used by 90.5% and 88.4%, respectively. The majority (81.0%) had protocols for emergency treatment of needle stick or other sharps accidents. High volume evacuation was used in 28.6% of public hospitals compared to 19.4% in academic institutions (P<0.01). In addition, surface barriers for dental unit surfaces were used by 70.2% of private dental clinics, 50% of public hospitals, and 36.1% of academic institutions (P<0.001). Finally, compared to dentists, dental support staff showed low compliance with infection-control guidelines. CONCLUSION: The overall practice of infection-control measures among the participants is very good. Educational programs and training strategies should be implemented to maximize and enhance the compliance of the dental care providers with infection-control guidelines.

RESUMO

The outbreak of this corona virus disease 19 (COVID-19) was first reported in Wuhan city in China and now it has become a global issue. The total number of confirmed cases of COVID-19 recorded globally has surpassed 100,000. The mode of transmission of COVID-19 is usually from person to person and it could also be through direct transmission such as cough, sneeze, and inhalation of droplets. Dental care providers are at higher risk since they are more prone to cross contamination with the patient's saliva and aerosols that are released during the dental procedures. They can also be transmitted through other routes such as direct contact with blood, oral fluids, and from contaminated instruments. This paper intends to provide primordial-level preventive measures for dental care providers against this life-threatening COVID-19. BACKGROUND: The authors conducted a study to assess recent trends in dental care provider mix (type of dental professionals visited) and service mix (types of dental procedures) use in the United States and to assess rural-urban disparities. METHODS: Data were from the 2000 through 2016 Medical Expenditure Panel Survey. The sample was limited to respondents who reported at least 1 dental visit to a dental professional in the survey year (N = 138,734 adults ≥ 18 years). The authors estimated rates of visiting 3 dental professionals and undergoing 5 dental procedures and assessed the time trends by rural-urban residence and variation within rural areas. Multiple logistic regression was used to assess the association between rural and urban residence and service and provider mix. RESULTS: A decreasing trend was observed in visiting a general dentist, and an increasing trend was observed in visiting a dental hygienist for both urban and rural residents (trend P values < .001). An increasing trend in having preventive procedures and a decreasing trend in having restorative and oral surgery procedures were observed only for urban residents (trend P values < .001). The combined data for 2000 through 2016 showed that rural residents were less likely to receive diagnostic services (adjusted odds ratio [AOR], 0.82; 95% confidence interval [CI], 0.72 to 0.93) and preventive services (AOR, 0.87; 95% CI, 0.78 to 0.96), and more likely to receive restorative (AOR, 1.11; 95% CI, 1.02 to 1.21) and oral surgery services (AOR, 1.23; 95% CI, 1.11 to 1.37). CONCLUSIONS: Although preventive dental services increased while surgical procedures decreased from 2000 through 2016 in the United States, significant oral health care disparities were found between rural and urban residents. PRACTICAL IMPLICATIONS: These results of this study may help inform future initiatives to improve oral health in underserved communities. By understanding the types of providers visited and dental services received, US dentists will be better positioned to meet their patients' oral health needs.

Introducción: El brote de la enfermedad del coronavirus 2019 (COVID-19) ha sido declarado emergencia de salud pública de importancia internacional. Los estomatólogos, entre el personal sanitario, son los más expuestos a la enfermedad porque su área de trabajo es la boca, sitio que se reconoce como el lugar a partir del cual se contagia la enfermedad. Objetivo: Esclarecer elementos relacionados con la COVID-19 y las medidas a tener en cuenta para la prevención del contagio del personal de salud del área estomatológica. Comentarios principales: No se ha descubierto un método único que cumpla con todos los requisitos con respecto a las infecciones asociadas a la atención médica. En el enfrentamiento a la COVID-19 cumplen un papel importante las precauciones estándares, o sea, medidas que se aplican a todos los pacientes, independientemente del diagnóstico o de si se conoce si tiene una infección o está colonizado por un agente. El SARS-CoV-2 es un virus envuelto, y esta característica lo hace más sensible a la acción de las sustancias desinfectantes. Consideraciones globales: La COVID-19 manifiesta síntomas no específicos. Los más comunes son fiebre, anorexia y tos seca. La transmisión probablemente ocurre a través de la propagación del virus en el aire y por contacto. Con la implementación de precauciones estándares entre las que se encuentran la higiene de las manos, uso de equipos de protección personal y el cuidado del medio ambiente, se puede disminuir la posibilidad del contagio y contribuir a controlar la diseminación. Es importante que los proveedores de atención médica, dentro de los que se encuentra el personal de salud del área estomatológica, y el público conozcan este nuevo virus para que las acciones coordinadas, oportunas y efectivas puedan ayudar a prevenir casos adicionales o malos resultados ante el enfrentamiento a esta pandemia(AU)


ABSTRACT Introduction: The outbreak of coronavirus disease 2019 (COVID-19) has been declared a worldwide public health emergency. Dentists are the health care providers most directly exposed to this condition, since their area of activity is the mouth, a location recognized as the site through which the disease is transmitted. Objective: Clarify some notions about COVID-19 and the measures to be taken to prevent transmission to dental care providers. Main remarks: A single method has not been discovered which meets all the requirements concerning infections associated to medical care. In the struggle against COVID-19 an important role is played by standard precautions, i.e. measures applied to all patients, irrespective of their diagnosis or whether they are known to have an infection or be colonized by a given agent. SARS-CoV-2 is an enveloped virus, a characteristic that makes it more sensitive to the action of disinfectants. General considerations: COVID-19 presents non-specific symptoms, the most common of which are fever, anorexia and dry coughing. Transmission probably occurs by the spread of the virus in the air and by direct contact. Implementation of standard precautions, among which are hand hygiene, the use of personal protection equipment and care for the environment, will lead to a reduction in the chances of contagion and will contribute to control the spread of the disease. It is important for medical care providers, among them dental care professionals, and the public at large to know about this new virus, so that coordinated, timely and effective actions may be taken which will help prevent the occurrence of new cases or bad results in the struggle against this pandemic(AU)
