RESUMO

Bacterial extracellular vesicles (BEVs) have emerged as critical factors involved in gut health regulation, transcending their traditional roles as byproducts of bacterial metabolism. These vesicles function as cargo carriers and contribute to various aspects of intestinal homeostasis, including microbial balance, antimicrobial peptide secretion, physical barrier integrity, and immune system activation. Therefore, any imbalance in BEV production can cause several gut-related issues including intestinal infection, inflammatory bowel disease, metabolic dysregulation, and even cancer. BEVs derived from beneficial or commensal bacteria can act as potent immune regulators and have been implicated in maintaining gut health. They also show promise for future clinical applications in vaccine development and tumor immunotherapy. This review examines the multifaceted role of BEVs in gut health and disease, and also delves into future research directions and potential applications.

RESUMO

Atherosclerosis is a chronic inflammatory and degenerative process that mainly occurs in large- and medium-sized arteries and is morphologically characterized by asymmetric focal thickenings of the innermost layer of the artery, the intima. This process is the basis of cardiovascular diseases (CVDs), the most common cause of death worldwide. Some studies suggest a bidirectional link between atherosclerosis and the consequent CVD with COVID-19. The aims of this narrative review are (1) to provide an overview of the most recent studies that point out a bidirectional relation between COVID-19 and atherosclerosis and (2) to summarize the impact of cardiovascular drugs on COVID-19 outcomes. A growing body of evidence shows that COVID-19 prognosis in individuals with CVD is worse compared with those without. Moreover, various studies have reported the emergence of newly diagnosed patients with CVD after COVID-19. The most common treatments for CVD may influence COVID-19 outcomes. Thus, their implication in the infection process is briefly discussed in this review. A better understanding of the link among atherosclerosis, CVD, and COVID-19 could proactively identify risk factors and, as a result, develop strategies to improve the prognosis for these patients.

With the flourishing development of material simulation methods (quantum chemistry methods, molecular dynamics, Monte Carlo, phase field, etc.), extensive adoption of computing technologies (high-throughput, artificial intelligence, machine learning, etc.), and the invention of high-performance computing equipment, computational simulation tools have sparked the fundamental mechanism-level explorations to predict the diverse physicochemical properties and biological effects of biomaterials and investigate their enormous application potential for disease prevention, diagnostics, and therapeutics. Herein, the term "computational biomaterials" is proposed and the computational methods currently used to explore the inherent properties of biomaterials, such as optical, magnetic, electronic, and acoustic properties, and the elucidation of corresponding biological behaviors/effects in the biomedical field are summarized/discussed. The theoretical calculation of the physiochemical properties/biological performance of biomaterials applied in disease diagnosis, drug delivery, disease therapeutics, and specific paradigms such as biomimetic biomaterials is discussed. Additionally, the biosafety evaluation applications of theoretical simulations of biomaterials are presented. Finally, the challenges and future prospects of such computational simulations for biomaterials development are clarified. It is anticipated that these simulations would offer various methodologies for facilitating the development and future clinical translations/utilization of versatile biomaterials.

RESUMO

Over the past several decades, liposomes have been extensively developed and used for various clinical applications such as in pharmaceutical, cosmetic, and dietetic fields, due to its versatility, biocompatibility, and biodegradability, as well as the ability to enhance the therapeutic index of free drugs. However, some challenges remain unsolved, including liposome premature leakage, manufacturing irreproducibility, and limited translation success. This article reviews various aspects of liposomes, including its advantages, major compositions, and common preparation techniques, and discusses present U.S. FDA-approved, clinical, and preclinical liposomal nanotherapeutics for treating and preventing a variety of human diseases. In addition, we summarize the significance of and challenges in liposome-enabled nanotherapeutic development and hope it provides the fundamental knowledge and concepts about liposomes and their applications and contributions in contemporary pharmaceutical advancement.

Due to a combination of a relatively low energy intake and a high demand of energy required for milk production, dairy cows experience a negative energy balance (EB) at the start of lactation. This energy deficit causes body weight reduction and an increased risk for metabolic diseases. Severity and length of negative EB can differ among cows. Peripartum time profiles of EB for dairy cows are not described yet in the literature. Creating EB-derived time profiles with corresponding metabolic status and disease treatments could improve understanding the relationship between EB and metabolic status, as well as enhance identification of cows at risk for compromised metabolic status. In this research we propose a novel method to cluster EB time series and examine associated metabolic status and disease treatments of dairy cows in the peripartum period. In this study, data of 3 earlier experiments were merged and examined. Four dairy cow clusters for time profiles of EB from wk -3 until +7 relative to calving were generated by the global alignment kernel algorithm. For each cluster, mean of body weight prepartum was distinguishable, indicating this might be a possible on-farm biomarker for the peripartum EB profile. Moreover, cows with severe EB drop postpartum were more treated for milk fever and had high plasma nonesterified fatty acids and ß-hydroxybutyrate concentration, and low IGF-1, insulin, and glucose concentration in the first 7 wk of lactation. Overall, this study demonstrated that cows can be clustered based on EB time profiles and that characteristics such as prepartum body weight, and postpartum nonesterified fatty acids and glucose concentration are promising biomarkers to identify the time profile of EB and potentially the risk for metabolic diseases.

RESUMO

The 11th World Pharma Pricing and Market Access Congress 2019, organized by Terrapinn and held in Amsterdam, the Netherlands, brought together speakers and delegates from across the Globe, and enabled discussions surrounding market access, pricing, value and evidence generation. There was a focus on developments in rare diseases and advanced therapies, with emphasis on the growing importance of digital health and big data to generate results and manage real-world evidence (RWE). The conference was divided into both keynote plenaries and six parallel-running tracks, with deliberations concerning customer-driven outcomes to enhance value, the importance of utilizing digital health in RWE analysis, modifying policy to better facilitate pricing discussions for novel advanced therapies, and adapting combination and indication on the basis of pricing to prepare for the many multi-indication approved drugs to come.

RATIONALE & OBJECTIVE: Understanding whether family members' experiences with patients' treatment for end-stage kidney disease (ESKD) were expected could guide the development of family-centered interventions that enhance the preparedness of patients and their care partners for kidney replacement therapies. We explored unexpected negative experiences with ESKD treatments among family members of dialysis and posttransplantation patients to identify meaningful directions for family-centered research and clinical care. STUDY DESIGN: Qualitative study. SETTING & PARTICIPANTS: 8 focus groups comprising 49 family members of dialysis patients and living donor kidney transplant recipients undergoing medical care in Baltimore, MD. ANALYTICAL APPROACH: Focus groups were stratified by patients' treatment (in-center hemodialysis, home hemodialysis, peritoneal dialysis, or living donor kidney transplantation) and family members' self-reported race (African American vs non-African American), resulting in 2 groups per treatment experience. Inductive thematic analysis was used to identify themes in focus group transcripts. Themes shared across different treatment groups were highlighted to provide insight into common experiences. RESULTS: We identified 4 themes that described family members' unexpected negative treatment experiences: becoming a care partner (unanticipated responsibilities and sleep disruptions), adverse psychological treatment responses in patients (eg, depression) and family members (eg, anxiety), treatment delivery and logistics (insufficient information, medication regimen, and logistical inconveniences), and patient morbidity (dialysis-related health problems and fatigue). All themes were relevant to discussions in the in-center hemodialysis, peritoneal dialysis, and transplantation groups, whereas psychological responses and morbidity themes did not reflect discussions in home hemodialysis groups. LIMITATIONS: Data collection occurred from 2008 to 2009; family members were recruited through patients undergoing care in 1 geographic area; 1 family member participant per patient. CONCLUSIONS: Family members described a broad range of unexpected negative experiences with ESKD treatments. Efforts to prepare families for ESKD treatments through more family-centered care, early and tailored education, and interventions targeting care partner preparedness, health provider-family member communication, and relationship dynamics in family member-patient dyads are needed.
