RESUMO

This study explored the effectiveness of mobile-assisted vocabulary learning through digital flashcards on receptive and productive aspects of academic vocabulary knowledge among Iranian EFL university students. In a quasi-experimental design, 86 participants were divided into three groups: a digital flashcard group (DFs), a traditional paper flashcard (PFs) group, and a control group using word lists, to assess the impact of these methods on vocabulary acquisition over five weeks. The findings revealed that students utilizing DFs exhibited significant improvements in both receptive and productive vocabulary knowledge compared to those using PFs and the control method. Notably, the increase in receptive vocabulary was more substantial than in productive vocabulary, highlighting the differential effects of DFs on various aspects of vocabulary learning. This finding underscores the need for targeted strategies to enhance productive aspects of academic vocabulary specifically. The study supports the integration of DFs into English for Academic Purposes (EAP) programs to leverage their potential in boosting vocabulary acquisition effectively. However, the lesser gains in productive vocabulary suggest the necessity for complementary instructional methods, which focus on more active vocabulary learning tasks. Based on these findings, the study argues that mobile-assisted vocabulary learning should be considered a practical strategy for supporting academic vocabulary development among university students.

RESUMO

BACKGROUND: The phase 3 BEACON CRC study demonstrated the survival benefits of encorafenib and cetuximab, with or without binimetinib (the BEACON triplet or doublet regimen), for BRAFV600E-mutant metastatic colorectal cancer (mCRC). This expanded access program (EAP) and subsequent follow-up study assessed the efficacy and safety of the BEACON triplet regimen in Japanese patients with BRAFV600E-mutant mCRC. MATERIALS AND METHODS: The EAP was an open-label, single-arm study including Japanese patients with BRAFV600E-mutant mCRC whose disease progressed after 1 to 2 prior regimens. The patients received the BEACON triplet regimen with 28-day cycles. The subsequent follow-up study assessed the survival outcomes following EAP completion. Safety was assessed only during the EAP. RESULTS: Among the 86 enrolled patients, 81 received the BEACON triplet regimen. The objective response rate and median progression-free survival were 27.6% (95% confidence interval [CI], 18.0%-39.1%) and 5.26 (95% CI, 4.14-5.52) months, respectively. Grade 3 to 4 adverse events and treatment-related adverse events occurred in 43.2% and 28.4% of patients, respectively. No new safety signals were observed during the EAP. Among 58 patients with confirmed survival at EAP completion, 57 were included in the follow-up study. With a median observation period of 9.17 months through the EAP and follow-up study, the median overall survival was 10.38 (95% CI, 9.00-16.16) months. CONCLUSION: The efficacy and safety of the BEACON triplet regimen in Japanese patients with BRAFV600E-mutant mCRC were consistent with those reported in the BEACON CRC trial, supporting its use as a standard treatment for pretreated patients with BRAFV600E-mutant mCRC.

RESUMO

A chemotherapy-based mobilization regimen in patients who mobilize poorly, based on etoposide, cytarabine and pegfilgrastim (EAP), has recently been introduced. The aim of this prospective study was to investigate the efficacy and safety of the EAP regimen in patients with poorly mobilizing multiple myeloma (MM) or lymphoma. This single-arm clinical trial was performed at eight public hospitals in China and was registered as a clinical trial (NCT05510089). The inclusion criteria were; (1) diagnosis of MM or lymphoma, (2) defined as a 'poor mobilizer' and (3) aged 18-75 years. The EAP regimen consisted of etoposide 75 mg/m2/day on days 1-2, cytarabine 300 mg/m2 every 12 h on days 1-2 and pegfilgrastim 6 mg on day 6. The primary endpoint of the study was the ratio of patients achieving adequate mobilization (≥2.0 × 106 CD34+ cells/kg). From 1 September 2022 to 15 August 2023, a total of 58 patients were enrolled, 53 (91.4%) achieved adequate mobilization, while 41 (70.7%) achieved optimal mobilization with a median number of cumulative collected CD34+ cells was 9.2 (range 2.1-92.7) × 106/kg and the median number of apheresis per patient of 1.2. The median time from administration of the EAP regimen to the first apheresis was 12 days. Approximately 8.6% of patients required plerixa for rescue, which was successful. Twelve (20.7%) of the 58 patients suffered grade 2-3 infections, while 25 (43.1%) required platelet transfusions. The duration of neutrophil and platelet engraftment was 11 days. In conclusion, these results suggest that the EAP mobilization regimen might be a promising option for poorly mobilizing patients with MM or lymphoma.

RESUMO

Advancements in multichannel recordings of single-unit activity (SUA) in vivo present an opportunity to discover novel features of spatially-varying extracellularly-recorded action potentials (EAPs) that are useful for identifying neuron-types. Traditional approaches to classifying neuron-types often rely on computing EAP waveform features based on conventions of single-channel recordings and thus inherit their limitations. However, spatiotemporal EAP waveforms are the product of signals from underlying current sources being mixed within the extracellular space. We introduce a machine learning approach to demix the underlying sources of spatiotemporal EAP waveforms. Using biophysically realistic computational models, we simulate EAP waveforms and characterize them by the relative prevalence of these sources, which we use as features for identifying the neuron-types corresponding to recorded single units. These EAP sources have distinct spatial and multi-resolution temporal patterns that are robust to various sampling biases. EAP sources also are shared across many neuron-types, are predictive of gross morphological features, and expose underlying morphological domains. We then organize known neuron-types into a hierarchy of latent morpho-electrophysiological types based on differences in the source prevalences, which provides a multi-level classification scheme. We validate the robustness, accuracy, and interpretations of our demixing approach by analyzing simulated EAPs from morphologically detailed models with classification and clustering methods. This simulation-based approach provides a machine learning strategy for neuron-type identification.

RESUMO

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a common inflammatory immune disease of the urogenital system. High glucose intake is considered to be a potential promoter of autoimmune diseases. However, the influence of high glucose intake on CP/CPPS is unknown. This research aimed to explore the influences of high glucose intake on experimental autoimmune prostatitis (EAP), a valid animal model of CP/CPPS, and the underlying mechanism. NOD mice received 20% glucose water or normal water treatment during EAP induction. EAP severity and Th17 cell responses were evaluated. Then, we explored the effects of an IL-17A neutralizing antibody, an inhibitor of TGF-ß, the reactive oxygen species (ROS) inhibitor NAC, and the mitochondrial ROS (mtROS) antioxidant MitoQ on glucose-fed EAP mice. The results demonstrated that high glucose intake aggravated EAP severity and promoted Th17 cell generation, which could be ameliorated by the neutralization of IL-17A. In vitro experiments showed that high dextrose concentrations promoted Th17 cell differentiation through mtROS-dependent TGF-ß activation. Treatment with TGF-ß blockade, NAC, or MitoQ suppressed Th17 cell generation both in vivo and in vitro, resulting in the amelioration of EAP manifestations caused by high glucose intake. This study revealed that high glucose intake exacerbates EAP through mtROS-dependent TGF-ß activation-mediated Th17 differentiation. Our results may provide insights into the molecular mechanisms underlying the detrimental effects of an environmental factor, such as high glucose intake, on CP/CPPS.

RESUMO

Large-scale tests often contain mixed-format items, such as when multiple-choice (MC) items and constructed-response (CR) items are both contained in the same test. Although previous research has analyzed both types of items simultaneously, this may not always provide the best estimate of ability. In this paper, a two-step sequential Bayesian (SB) analytic method under the concept of empirical Bayes is explored for mixed item response models. This method integrates ability estimates from different item formats. Unlike the empirical Bayes method, the SB method estimates examinees' posterior ability parameters with individual-level sample-dependent prior distributions estimated from the MC items. Simulations were used to evaluate the accuracy of recovery of ability and item parameters over four factors: the type of the ability distribution, sample size, test length (number of items for each item type), and person/item parameter estimation method. The SB method was compared with a traditional concurrent Bayesian (CB) calibration method, EAPsum, that uses scaled scores for summed scores to estimate parameters from the MC and CR items simultaneously in one estimation step. From the simulation results, the SB method showed more accurate and reliable ability estimation than the CB method, especially when the sample size was small (150 and 500). Both methods presented similar recovery results for MC item parameters, but the CB method yielded a bit better recovery of the CR item parameters. The empirical example suggested that posterior ability estimated by the proposed SB method had higher reliability than the CB method.

RESUMO

For rubber-like materials, there are three popular methods of equibiaxial tension available: inflation tension, equibiaxial planar tension, and radial tension. However, no studies have addressed the accuracy and comparability of these tests. In this work, we model the tension tests for a hyperelastic electroactive polymer (EAP) membrane material using finite element method (FEM) and investigate their experimental accuracy. This study also analyzes the impact of apparatus structure parameters and specimen dimensions on experimental performances. Additionally, a tensile efficiency is proposed to assess non-uniform deformation in equibiaxial planar tension and radial tension tests. The sample points for calculating deformation in inflation tensions should be taken near the top of the inflated balloon to obtain a more accurate characteristic curve; the deformation simulation range will be constrained by the material model and its parameters within a specific limit (λ ≈ 1.9); if the inflation hole size is halved, the required air pressure must be doubled to maintain equivalent stress and strain values, resulting in a reduction in half in inflation height and decreased accuracy. The equibiaxial planar tension test can enhance uniform deformation and reduce stress errors to as low as 2.1% (at λ = 4) with single-corner-point tension. For circular diaphragm specimens in radial tension tests, increasing the number of cuts and using larger punched holes results in more uniform deformation and less stress error, with a minimum value of 3.83% achieved for a specimen with 24 cuts and a 5 mm punched hole. In terms of tensile efficiency, increasing the number of tensile points in the equibiaxial planar tension test can improve it; under radial tension, increasing the number of cuts and decreasing the diameter of the punched hole on the specimen has a hedging effect. The findings of this study are valuable for accurately evaluating various equibiaxial tension methods and analyzing their precision, as well as providing sound guidance for the effective design of testing apparatus and test plans.

RESUMO

Background: Due to the SARS-CoV-2 pandemic, healthcare workers (HCWs) are experiencing tremendous levels of emotional and physical stress. Hospitals are trying to help personnel cope with work-related pressure. The aim of this study was to assess HCWs' awareness and utilization of counseling and support services during the pandemic, HCWs' unmet counseling and support needs, and the type and content of these services. Methods: A cross-sectional online survey was conducted from January to June 2021 through the German national research organization Network University Medicine (NUM). All participating hospitals (6 in total) were asked to inform their employees about the study. Results: A total of 1,495 HCWs were included in the analysis. Of these, 42.8% (n = 637) were frontline HCWs (who had contact with COVID-19 patients), 23.1% (n = 344) were second-line HCWs (who only had contact with non-COVID-19 patients) and 34.1% (n = 508) had no contact with any patients. Participating hospitals offer various counseling and support services for their staff. The percentage of respondents who were unaware of available counseling and support services ranged from 5.0 to 42.0%. Depending on the type of counseling and support services, 23.0-53.6% of the respondents indicated that counseling and support services were provided but not used, while 1.7-11.6% indicated that, despite the need for them, such services were not available. HCWs' overall satisfaction with the provided counseling and support services and their unmet support needs differed by patient contact: Frontline HCWs reported more unmet needs for counseling and support than second-line HCWs, while second-line HCWs reported more unmet needs than HCWs without patient contact. Conclusion: The results indicate that hospitals should make more efforts to inform HCWs about available counseling and support services. Hospitals could also create networks where HCWs could share information about the type and content of services and their experiences with various counseling and support services. These steps would enable hospitals to respond more quickly and effectively to the problems facing HCWs during pandemics.

RESUMO

The field of English for Specific Purposes (ESP) has gained considerable attention, mainly due to the growing importance of English teaching and the need for preparing international professionals. The primary academic emphasis of this field is largely centered on the study of English for Academic Purposes (EAP). It has become increasingly important for China to provide EAP education to its vast population of college students, who represent a significant proportion of global tertiary-level English learners. By doing so, they can improve their academic literacy and participate more effectively in international academic communication. In 2016, China's College English Test Examination Board issued the National College English Test Syllabus which emphasized the importance of academic English literacy in the test design. This syllabus was released during a contentious debate among Chinese academics about the role of EAP in college English teaching and testing. Despite this heated discussion, there are currently few studies that have analyzed the linguistic nature of academic English tests in a quantitative manner. This paper calculates the academic word coverage of the reading passages in two crucial English proficiency tests designed for college students in China, the College English Test (CET) and the Test of English for Academic Purposes (TEAP). It is found that the academic word coverage of CET is increasing from the year 2013 to 2021 and that the academic word coverage of TEAP is slightly higher than that of CET. In this sense, these English tests can meet the requirements for measuring Chinese college students' academic literacy. It is also found that there is a positive correlation between academic word coverage and language difficulty as is indicated by Flesch-Kincaid Grade Level. These findings provide an empirical reference for the study of academic English tests in China, and other parts of the world, and contribute to EAP teaching and testing reform for the development of students' academic literacy.

RESUMO

BACKGROUND: Oral case presentations - structured verbal reports of clinical cases - are fundamental to patient care and learner education. Despite their continued importance in a modernized medical landscape, their structure has remained largely unchanged since the 1960s, based on the traditional Subjective, Objective, Assessment, Plan (SOAP) format developed for medical records. We developed a problem-based alternative known as Events, Assessment, Plan (EAP) to understand the perceived efficacy of EAP compared to SOAP among learners. METHODS: We surveyed (Qualtrics, via email) all third- and fourth-year medical students and internal medicine residents at a large, academic, tertiary care hospital and associated Veterans Affairs medical center. The primary outcome was trainee preference in oral case presentation format. The secondary outcome was comparing EAP and SOAP on 10 functionality domains assessed via a 5-point Likert scale. We used descriptive statistics (proportion and mean) to describe the results. RESULTS: The response rate was 21% (118/563). Of the 59 respondents with exposure to both the EAP and SOAP formats, 69% (n = 41) preferred the EAP format as compared to 19% (n = 11) who preferred SOAP (p < 0.001). EAP outperformed SOAP in 8 out of 10 of the domains assessed, including advancing patient care, learning from patients, and time efficiency. CONCLUSIONS: Our findings suggest that trainees prefer the EAP format over SOAP and that EAP may facilitate clearer and more efficient communication on rounds, which in turn may enhance patient care and learner education. A broader, multi-center study of the EAP oral case presentation will help to better understand preferences, outcomes, and barriers to implementation.

RESUMO

In recent years, plasticized poly (vinyl chloride) (PVC) gel has attracted increasing attention in soft robotics. However, there is scarce research on the deformation mechanism and modeling of PVC gel actuators. In this study, to investigate the deformation mechanism of fiber-constrained planar PVC gel actuators, we propose a complex nonlinear model based on traditional thermodynamic electroactive polymer (EAP) multi-field coupling theory. The proposed model can reveal the dielectric breakdown strength of PVC gels and predict the deformation of planar PVC gel actuators with varying levels of pre-stretching. The theoretical results were in good agreement with the experimental results, indicating the feasibility of the proposed model.

RESUMO

Mobile-assisted language learning (MALL) technology and the link it creates with language education domains made researchers interested in the plausible effects of MALL tools on English for academic purposes (EAP) students' language development. Likewise, this study investigated the effect of Instagram as a MALL tool on technical vocabulary knowledge of Iranian EAP university students (physical education major) in an electronic course conducted via Adobe Connect as well as their perceptions and attitudes toward Instagram. In a pretest-posttest quasi-experimental research design, 60 EAP university students at the intermediate level were randomly assigned to a control group and an experimental group who practiced technical vocabulary in physical education via different tasks on Instagram. Both groups participated in an online pretest and posttest of technical vocabulary, yet only the experimental group completed a questionnaire about their perceptions and attitudes toward Instagram. The repeated-measures two-way ANOVA on the test scores indicated that the experimental group outperformed the control group in learning technical vocabulary. Further, analysis of the responses to the questionnaire revealed positive perceptions and attitudes toward Instagram as a supplementary MALL tool for learning technical vocabulary. The findings encourage EAP instructors to use Instagram as an accessible MALL application to share their teaching materials with learners.

RESUMO

With the global rise in international journals over the past decades, successful communication in science largely hinges upon developing competency in using English as the academic lingua franca. Accordingly, one aspect of developing academic literacy entails helping university students learn a group of medium-frequency and cross-disciplinary words (i.e., core academic vocabulary) employed extensively to describe abstract processes and organize rhetorical aspects of academic discourse. The current study aimed to investigate the contribution of mobile-assisted vocabulary learning with digital flashcards in scaffolding academic vocabulary learning and self-regulatory capacity development among university students. The participants were 54 Iranian university students selected based on their availability in the study context. The participants were assigned to an experimental group (N = 33) and a control learning condition (N = 21). Those in the experimental group used digital flashcards (i.e., Quizlet) to learn academic words in a recently developed core academic wordlist (i.e., NAWL), and the control group used traditional materials (wordlists) to learn the same vocabulary items. The participants' vocabulary knowledge and self-regulatory capacity for vocabulary learning were tested before and after the treatments. The findings indicated that although both groups improved their vocabulary knowledge and self-regulatory capacity after 4 months, the experimental group outperformed the control group in both measures, and the effect sizes of the observed differences were very large. Consequently, the study provided empirical evidence for the effectiveness of mobile-assisted vocabulary learning over traditional materials in developing academic literacy. The findings also indicated that using digital flashcards for vocabulary learning improves university students' capacity for undertaking self-regulated vocabulary learning. The implications of these findings for EAP programs are highlighted.

RESUMO

This study examined the test-retest reliability and predictive validity of the East Asia-Pacific Early Child Development Scales (EAP-ECDS) Short Form. In China, preschools typically provide children with educational activities in age-segregated classrooms - Kindergarten Level 1 (K1) (3 to 4 years), Kindergarten Level 2 (K2) (4 to 5 years), and Kindergarten Level 3 (K3) (5 to 6 years). A total of 709 children in K2 (Mage = 57.85 months, SD = 4.77) were randomly selected from 29 kindergartens in Shanghai municipality and Guizhou province of China. Children were assessed using the EAP-ECDS in K2 and K3. School readiness was assessed in K3, and literacy and mathematics achievement were assessed in Grade 2. Pearson's correlation coefficient and intraclass correlation coefficient (ICC = 0.73) indicated that the tool had good test-retest reliability across K2 and K3. Regarding predictive validity, K2 EAP-ECDS predicted K3 school readiness (ß = 0.26), Grade 2 language and literacy (ß = 0.18) and mathematics (ß = 0.22) after adjusting for age, gender, socioeconomic status, and region. Findings support using the tool to measure the holistic development of preschool-aged children in China and the region.

RESUMO

OBJECTIVES: To apply item response theory as a framework for studying measurement error in superiority trials which use patient-reported outcome measures (PROMs). METHODS: We reanalyzed data from the The Total or Partial Knee Arthroplasty Trial, which compared the Oxford Knee Score (OKS) responses of patients undergoing partial or total knee replacement, using traditional sum-scoring, after accounting for OKS item characteristics with expected a posteriori (EAP) scoring, and after accounting for individual-level measurement error with plausible value imputation (PVI). We compared the marginalized mean scores of each group at baseline, 2 months, and yearly for 5 years. We used registry data to estimate the minimal important difference (MID) of OKS scores with sum-scoring and EAP scoring. RESULTS: With sum-scoring, we found statistically significant differences in mean OKS score at 2 months (P = 0.030) and 1 year (P = 0.030). EAP scores produced slightly different results, with statistically significant differences at 1 year (P = 0.041) and 3 years (P = 0.043). With PVI, there were no statistically significant differences. CONCLUSION: Psychometric sensitivity analyses can be readily performed for superiority trials using PROMs and may aid the interpretation of results.

RESUMO

This study revisits the tourism-led growth discourse and differs from the existing literature to examine if information and communications technology (ICT) moderates the relationship between tourism and economic growth in East Asia and the Pacific. Using data on 33 selected countries, the study deploys the Driscoll and Kraay (1998) [1] panel spatial correlation consistent (PSCC) approach, Machado and Santos Silva (2019) [2] method of moments quantile regression (MMQR) and Arellano and Bond (1991) [3] generalized method of moments (GMM) technique. Using a composite ICT index on four indicators ((mobile phones, fixed telephones, fixed broadband, and secured internet servers) derived from the Principal Component Analysis (PCA), the results which are mostly consistent across the three estimation methods reveal, among others, that (1) ICT moderates the tourism-growth path and the effect is positive and statistically significant; (2) the moderation effect is consistently positive across all quantiles of Q0.25, Q0.50 and Q0.75; (3) the results are sustained when omitted variables (growth enablers) - institutions, R&D, and human capital - are accounted for. Policy recommendations are discussed.

RESUMO

BACKGROUND: TP53 mutations are frequent in non-small cell lung cancer (NSCLC). Different categories of TP53 mutations may be associated with survival in advanced NSCLC, but their effect on prognosis is diverse. To date, a comprehensive comparison of the relationship between different classes of TP53 alterations and survival in advanced NSCLC has rarely been performed. Moreover, the prognostic significance of a novel approach called the evolutionary action of TP53 (EAp53) in advanced NSCLC is unclear. METHODS: A total of 210 patients with NSCLC harboring TP53 mutation data were enrolled. Genomic and clinical data for the Memorial Sloan Kettering Cancer Center (MSKCC) cohort with advanced NSCLC were obtained from cBioPortal. Relationship between clinical characteristics and TP53 mutations was performed by Fisher's exact test or χ2 test. Overall survival (OS) analysis was evaluated using Kaplan-Meier method and Cox proportional hazards regression model. RESULTS: TP53 mutations were identified in 51.4% of NSCLC patients and were mainly located in exons 5, 7, and 8. The distribution patterns of missense and truncating mutations of TP53 were remarkably different. Among patients with advanced NSCLC who never received immune checkpoint inhibitor treatments, EAp53 high-risk mutations were significantly associated with poor OS in both our cohort and the MSKCC cohort. Moreover, marked differences were observed in the mutational landscape between patients with EAp53 high-risk mutations (HR group) and other patients (OT group). The HR group displayed higher mutation frequencies in the RTK, cell cycle, and DNA damage repair (DDR) pathways than the OT group. In addition, the tumor mutation burden in the HR group was significantly higher than that in the OT group. CONCLUSIONS: This study provided important insights into the molecular-clinical profile of TP53-mutated NSCLC patients. Moreover, the data revealed that EAp53 high-risk mutations were an independent prognostic factor for worse OS in advanced NSCLC.

RESUMO

PURPOSE: We seek to reformulate the so-called Propagator Anisotropy (PA) and Non-Gaussianity (NG), originally conceived for the Mean Apparent Propagator diffusion MRI (MAP-MRI), to the Micro-Structure adaptive convolution kernels and dual Fourier Integral Transforms (MiSFIT). These measures describe relevant normalized features of the Ensemble Average Propagator (EAP). THEORY AND METHODS: First, the indices, which are defined as the EAP's dissimilarity from an isotropic (PA) or a Gaussian (NG) one, are analytically reformulated within the MiSFIT framework. Then a comparison between the resulting maps is drawn by means of a visual analysis, a quantitative assessment via numerical simulations, a test-retest study across the MICRA dataset (6 subjects scanned five times) and, finally, a computational time evaluation. RESULTS: Findings illustrate the visual similarity between the indices computed with either technique. Evaluation against synthetic ground truth data, however, demonstrates MiSFIT's improved accuracy. In addition, the test-retest study reveals MiSFIT's higher degree of reliability in most of white matter regions. Finally, the computational time evaluation shows MiSFIT's time reduction up to two orders of magnitude. CONCLUSIONS: Despite being a direct development on the MAP-MRI representation, the PA and the NG can be reliably and efficiently computed within MiSFIT's framework. This, together with the previous findings in the original MiSFIT's article, could mean the difference that definitely qualifies diffusion MRI to be incorporated into regular clinical settings.

RESUMO

The recently discovered neurological disorder NEDAMSS is caused by heterozygous truncations in the transcriptional regulator IRF2BPL. Here, we reprogram patient skin fibroblasts to astrocytes and neurons to study mechanisms of this newly described disease. While full-length IRF2BPL primarily localizes to the nucleus, truncated patient variants sequester the wild-type protein to the cytoplasm and cause aggregation. Moreover, patient astrocytes fail to support neuronal survival in coculture and exhibit aberrant mitochondria and respiratory dysfunction. Treatment with the small molecule copper ATSM (CuATSM) rescues neuronal survival and restores mitochondrial function. Importantly, the in vitro findings are recapitulated in vivo, where co-expression of full-length and truncated IRF2BPL in Drosophila results in cytoplasmic accumulation of full-length IRF2BPL. Moreover, flies harboring heterozygous truncations of the IRF2BPL ortholog (Pits) display progressive motor defects that are ameliorated by CuATSM treatment. Our findings provide insights into mechanisms involved in NEDAMSS and reveal a promising treatment for this severe disorder.
