RESUMO

We employed a multiple probe design to test the effects of an accelerated auditory matching protocol with five toddlers receiving early intervention services (four males, one female) for developmental delays. All participants emitted poorly articulated vocal mands and tacts. The dependent variables were the number of full echoics, partial echoics, and incorrect responses within a set of 20 two-syllable words. The independent variable was an accelerated Auditory Matching Protocol that targets auditory discrimination using an iPad App. Each phase of the protocol targets different sound discriminations with growing complexity. Four out of five participants emitted more full or partial echoic responses upon mastery of the Auditory Matching Protocol. Moreover, compared to those who received the full dosage of the standard Auditory Matching Protocol in previous studies, the participants in this study required fewer learn units to master all phases and to demonstrate improvement. This study aimed to compare the effects of echoic and listener responding in the emergence of complex intraverbal behavior in four children with autism spectrum disorder (ASD). Each participant was taught to provide an echoic response or a listener response for different discriminative stimuli for each condition. We used a nonconcurrent multiple probe design across participants and adapted an alternating treatment design to compare the effects between the two conditions. Pre- and posttests were used to evaluate the effects of the two different prompt types in the emergence of complex intraverbals. The results indicate that the echoic response was more effective than the listener response at increasing the emergence of complex intraverbal responses in three out of four participants.

Aims: Among primary thyroid lymphomas (PTLs), diffuse large B-cell lymphoma (DLBCL) has a poorer prognosis than other indolent lymphomas such as mucosa-associated lymphoid tissue (MALT) or follicular lymphoma (FL). However, the clinical differences between DLBCL and indolent lymphoma remain unclear. Therefore, this retrospective study on PTL was aimed at investigating the clinical differences between DLBCL and indolent lymphomas and identifying the factors differentiating DLBCL from indolent lymphomas. Materials and Methods: Medical records of 28 patients diagnosed with PTL and treated at our institution between 2005 and 2022 were retrospectively analyzed. Data on the following clinical variables were extracted: sex, age, symptoms (pain and dysphagia), ultrasonographic appearance patterns, the presence of airway stenosis on computed tomography and laryngeal endoscopy, blood test results, disease stage, and pathological diagnosis. Results: In all, 13 patients were histologically diagnosed with DLBCL, 12 with MALT lymphoma, and 3 with FL. Significant differences in disease-specific survival rates were evident between the DLBCL and indolent lymphoma groups (68.2 vs 100%, P = .043). High lactate dehydrogenase levels (>230 U/mL) and airway stenosis were observed only in patients with DLBCL. Multivariate analysis identified that the presence of a linear echoic strand pattern and the absence of an echoic nodular pattern on ultrasound were independently associated with DLBCL (P = .0497 and .012, respectively). Conclusion: DLBCL can cause airway stenosis. The linear echogenic strand pattern and the absence of a nodular pattern should be recognized as predictive factors of DLBCL.

RESUMO: Um dos fatores de maior preocupação dos pais de filhos com autismo é o déficit na comunicação social. O ensino dos pais de como desenvolver tal habilidade em seu filho, além de ser uma forma de ampliar o tempo de intervenção é um tratamento com resultados satisfatórios. Destaca-se a importância do ensino do ecoico no desenvolvimento da fala da criança por ser um prérequisito para a instalação de comportamentos mais complexos. No que se refere ao cenário pandêmico de isolamento social, faz-se necessária uma intervenção comportamental a distância, e o pacote de Behavior Skills Training (BST) tem sido útil para ensinar pais a aplicarem procedimentos comportamentais em seus filhos. O presente estudo teve como objetivo verificar os efeitos do pacote de treinamento ministrado de modo remoto aos pais sobre o ensino da aplicação do protocolo teste do repertório de ecoico a crianças com autismo. Por meio de um delineamento de linha de base múltipla, foram observados os resultados de três mães participantes, das quais uma apresentou 24% de acertos em linha de base, atingindo 100% após a introdução do BST, o que indicou a eficácia do procedimento de ensino. Após o aprendizado, as mães não voltaram a aplicar o procedimento nos filhos.


ABSTRACT: One of the factors of greatest concern for parents of children with autism is the deficit in social communication. Teaching parents how to develop this ability in their child, in addition to being a way to extend the intervention time, is a treatment with satisfactory results. The importance of teaching the echoic in the development of the child's speech is highlighted, as it is a prerequisite for the installation of more complex behaviors. With regard to the pandemic and social isolation scenario, a remote behavioral intervention is necessary, and the Behavior Skills Training (BST) package has been useful to teach parents to apply behavioral procedures to their children. This study aimed to verify the effects of the training package given remotely to parents on teaching the application of the echoic repertoire test protocol to children with autism. Through a multiple baseline design, the results of three participating mothers were observed, one of which had 24% of correct answers at baseline, reaching 100% after the introduction of BST, indicating the effectiveness of the teaching procedure. After learning, the mothers did not apply the procedure to their children again.

In studying how neural populations in sensory cortex code dynamically varying stimuli to guide behavior, the role of spiking after stimuli have ended has been underappreciated. This is despite growing evidence that such activity can be tuned, experience-and context-dependent and necessary for sensory decisions that play out on a slower timescale. Here we review recent studies, focusing on the auditory modality, demonstrating that this so-called OFF activity can have a more complex temporal structure than the purely phasic firing that has often been interpreted as just marking the end of stimuli. While diverse and still incompletely understood mechanisms are likely involved in generating phasic and tonic OFF firing, more studies point to the continuing post-stimulus activity serving a short-term, stimulus-specific mnemonic function that is enhanced when the stimuli are particularly salient. We summarize these results with a conceptual model highlighting how more neurons within the auditory cortical population fire for longer duration after a sound's termination during an active behavior and can continue to do so even while passively listening to behaviorally salient stimuli. Overall, these studies increasingly suggest that tonic auditory cortical OFF activity holds an echoic memory of specific, salient sounds to guide behavioral decisions.

Background: Microorganisms of tuberculosis (TB) are frequently difficult to identify from the airway specimen; therefore, lung biopsy for further histologic and microbiologic study is required. Endobronchial ultrasound-guided transbronchial biopsy (EBUS-TBB) is used for the diagnosis of pulmonary malignancy, but is rarely in the TB population. The purpose of this study was to verify the effectiveness and safety of EBUS-TBB with histologic study and tissue culture in the diagnosis of sputum smear-negative pulmonary TB. Methods: Patients who underwent EBUS-TBB with histologic study and TB tissue culture for clinically suspected, but sputum smear-negative pulmonary TB from January 2016 to December 2018, were included. The accuracy of each diagnostic modality was calculated, respectively. Factors that might influence the positive rate of TB culture (washing fluid and tissue specimen) were also evaluated. Results: One hundred sixty-one patients who underwent EBUS-TBB for clinically suspected, but sputum smear-negative pulmonary TB, were enrolled, and 43 of them were finally diagnosed as having pulmonary TB. The sensitivity of washing fluid (a combination of smear, culture, and polymerase chain reaction for TB) and tissue specimen (a combination of pathology and tissue culture) via EBUS-TBB for TB diagnosis were 48.8 and 55.8%, respectively. The sensitivity for TB diagnosis would be elevated to 67.4% when both washing fluid and tissue specimens are used. The positive TB culture rate would not statistically increase with a combination of tissue specimens and washing fluid. Univariate analysis revealed that TB microorganisms would be more easily cultivated when lesions had an abscess or cavity on the computed tomography (CT) image (presence vs. absence; 62.5 vs. 26.3%, p = 0.022), heterogeneous echogenicity on the EBUS finding (heterogeneous vs. homogeneous; 93.3 vs. 21.4%, p = 0.001), or a necrotic pattern via histologic study (presence vs. absence; 70.6 vs. 30.8%, p = 0.013). Heterogeneous echogenicity in the EBUS finding was the independent predictor according to the results of multivariate analysis. None of our patients encountered major adverse events or received further intensive care after EBUS-TBB. Conclusion: Endobronchial ultrasound-guided transbronchial biopsy is safe and effective for use in diagnosing sputum smear-negative pulmonary TB. EBUS echoic feature is also a predictor of the positive TB culture rate in pulmonary TB. However, tissue culture via EBUS-TBB has little effect in improving the positive TB culture rate.

The purposes of this study were to evaluate the effects of an auditory-distractor stimulus and vocal-blocking task on performance on a math task and measures of overt verbal operants. College students served as participants, and they were instructed to solve an arithmetic problem while continuously emitting overt verbal behavior. The overt verbal behavior consisted of either talking aloud while solving the problem or reciting the alphabet. A third condition consisted of playing an auditory-distractor file containing the alphabet during the response interval while participants talked aloud. Data were collected on response accuracy, latency to respond, and frequency of echoic and self-echoic responses emitted during the response interval. The vocal-blocking task significantly affected performance on the math task when visual stimuli were absent. The decrease in performance coincided with significant decreases in participants' mean frequency of echoic and self-echoic responses. In sum, correctly responding to an arithmetic problem was significantly affected by the vocal-blocking task, indicating support for the necessity of verbal mediation during problem solving.

Restenosis after stent implantation in femoropopliteal lesions is still big issue. However, restenosis has been reduced by the recent new drug-eluting stent "Eluvia" (Boston Scientific, Marlborough, MA, USA). However, it was reported that "low echoic area (LEA)" finding around stent by ultrasound that they called "aneurysmal degeneration," but no blood flow was identified outside the stent was confirmed after Eluvia implantation. In this report, we describe the similar findings that were observed after other types of stents (S.M.A.R.T. bare-nitinol stent (Cordis Corporation, Hialeah, FL, USA), Zilver PTX drug-coated stent (Cook Medical, Bloomington, IN, USA), and Viabahn stent graft (W. L. Gore & Associates, Newark, Delaware, USA)) for superficial femoral artery disease. These findings did not change to "aneurysmal change" during the follow-up.

In contrast to classical views of working memory (WM) maintenance, recent research investigating activity-silent neural states has demonstrated that persistent neural activity in sensory cortices is not necessary for active maintenance of information in WM. Previous studies in humans have measured putative memory representations indirectly, by decoding memory contents from neural activity evoked by a neutral impulse stimulus. However, it is unclear whether memory contents can also be decoded in different species and attentional conditions. Here, we employ a cross-species approach to test whether auditory memory contents can be decoded from electrophysiological signals recorded in different species. Awake human volunteers (N = 21) were exposed to auditory pure tone and noise burst stimuli during an auditory sensory memory task using electroencephalography. In a closely matching paradigm, anesthetized female rats (N = 5) were exposed to comparable stimuli while neural activity was recorded using electrocorticography from the auditory cortex. In both species, the acoustic frequency could be decoded from neural activity evoked by pure tones as well as neutral frozen noise burst stimuli. This finding demonstrates that memory contents can be decoded in different species and different states using homologous methods, suggesting that the mechanisms of sensory memory encoding are evolutionarily conserved across species.

BACKGROUND: Chronic vessel enlargement (CVE) of the coronary artery is observed in patients who undergo percutaneous coronary intervention (PCI) for severely stenotic or occluded lesions. Recently, the presence of a peri-medial high-echoic band (PHB) identified by intravascular ultrasound (IVUS) has been reported to correlate with this phenomenon. We sought to assess the incidence, predictive factors of CVE, and association between CVE and PHB. METHODS: From January 2017 to December 2018, 97 patients, with 101 severely stenotic and occluded lesions, who underwent IVUS-guided PCI and a 9-month follow-up angiography were enrolled. CVE was defined as more than a 10% increase of distal lumen gain at follow-up angiography. All lesions were stratified into 2 groups: CVE group and non-CVE group. Clinical outcomes were compared and predictive factors for CVE were assessed. Relationships between PHB angle and CVE were also assessed. RESULTS: At follow-up angiography, CVE was observed in 27 lesions (26.7%, 27/101). PHBs were frequently observed in the CVE group (88.9%), which was significantly higher than that in the non-CVE group (40.5%). The multivariate logistic regression analysis revealed that the presence of PHB was the only predictive factor for CVE (odds ratio, 11.3; 95% confidence interval, 2.95-43.0; p < 0.001). In addition, a linear relationship was observed between the incidence of CVE and PHB angle. The number of patients with CVE significantly increased in cases with a PHB angle more than 180 degrees. CONCLUSIONS: The presence of PHB strongly predicts CVE after PCI for severely stenotic or occluded lesions.

An important communication skill for children with autism is answering multiple questions about visual stimuli (e.g., "What is it?" "What color is it?"). We targeted answering "What number?" and "What shape?" in the presence of numbers inside shapes, and "What is it?" and "What color?" in the presence of colored objects (e.g., a yellow cat) with 3 preschoolers with autism. In addition to a progressive time delay, we required the participants to answer the questions by echoing a keyword from the questions. For example, we taught them to answer, "What color?" with "color blue." In the context of a multiple-probe design across behaviors within a multiple-probe design across participants, the procedure was effective in increasing trained responses and producing within- and across-category generalization. The echoic may have facilitated the responses by increasing the salience of the auditory stimuli and strengthening intraverbals within autoclitic frames.

Tinnitus is the perception of sound without the occurrence of an acoustic event. The deficit in auditory sensory or echoic memory may be the cause of the perception of tinnitus. This study considered the mismatch negativity (MMN) to investigate the potential difference between and within groups of persons with normal hearing (NH) and tinnitus. Using an auditory multi-feature paradigm to elicit the MMN, this study considered the MMN peak amplitude at two central frequencies for two MMN subcomponents. These central frequencies were 1 and 5 kHz, which the latter was closer to the perceived tinnitus frequency in the group with tinnitus. The deviants were higher frequency, lower frequency, higher intensity, lower intensity, duration, location (left), location (right), and gap. The pure tone audiometry (PTA) test and distortion product otoacoustic emissions (DPOAE) test showed no meaningful difference between the two groups. For the frontal subcomponent, the mean amplitudes of the MMN peak for the two groups illustrated less negative meaningful MMN peak amplitudes in the group of persons with tinnitus. For the supratemporal component at 5 kHz central frequency, amplitudes were lower for the group of persons with tinnitus, whereas for the central frequency of 1 kHz, most deviants exhibited meaningful differences. Additionally, within-group comparisons indicated that mean amplitudes for both groups were more negative at the central frequency of 1 kHz for the frontal MMN subcomponent. In comparison, the supratemporal component illustrated a lower peak amplitude at 5 kHz central frequency in the group of persons with tinnitus and no difference in the NH group, which is a unique observation of this study. Results of the between-groups are in accordance with previous studies and within-group comparisons consider the probability of decreasing the change detection capability of the brain. The results of this study indicate that increasing the frequency of the stimuli close to the tinnitus perceived frequencies decreases the prediction error, including the prediction error of the silence. Such a decrease may cause the prediction error of the spontaneous neural activity in the auditory pathway to exceed the silence prediction error, and as a result, increases the probability of occurrence of tinnitus in higher frequencies according to the predictive coding model.

Congenital high airway obstruction syndrome (CHAOS) is a rare congenital malformation, which results from deficient recanalization of the upper airways. Laryngeal atresia is the most common cause, other etiologies being trachea atresia, laryngeal or tracheal webs, subglottic stenosis, obstructing laryngeal cysts, and laryngeal or tracheal agenesis. There is decreased clearance of the fluid produced by fetal lungs due to obstruction leading to increased intratracheal pressure and thereby secondary proliferative lung growth. The heart becomes compressed in the midline due to hyperexpansion of the lungs causing elevated intrathoracic pressure, decreased venous return, and fetal cardiac failure. This sequence causes ascites, placento-megaly, and eventually hydrops fetalis. We present a case of antenatal diagnosis of a fetus with CHAOS corroborated by fetal autopsy.

Children with autism are at a higher risk of being affected by speech disorders and often require remedial intervention. Eikeseth and Nesset (Journal of Applied Behavior Analysis, 36(3), 325-337, 2003) used sufficient-response exemplar training of vocal imitation in conjunction with prompting, chaining, and shaping procedures to successfully teach 2 typically developing children to articulate several Norwegian words with blends. The present study extends and adapts these procedures to children with autism. Participants were TA, an 11-year-old boy, and KS, a 15-year-old girl, both with autism and speech sound disorders. For each participant, 3 sets of 10 words with specific blends in the initial position were targeted for training. Vocal imitation training with within-stimulus prompts was used for both participants. For KS, lip-tongue-teeth position prompts and chaining were added during the training of certain words. A multiple-baseline across-behaviors (word sets with target blends) design demonstrated improvement in the articulation of trained words and generalization of correct articulation to untrained words with both participants. The findings suggest that speech sound disorders in children with autism can be addressed with behavioral interventions.

The current experiment is a systematic replication of previous studies that evaluated the efficiency of echoic and tact prompts on the acquisition of intraverbals (i.e., French-to-English translations) following exposure to each prompt type. We extended these studies by (a) evaluating participants' language skills on standardized assessments, (b) incorporating descriptive praise for correct responding, (c) presenting trials via voice recording, and (d) evaluating teacher preference for each prompt type as a social validity measure. All participants learned at least one set of intraverbals faster with the procedure that was most recently used during teaching. These findings suggest that results from previous prompt comparison studies might be a function of previous exposure to prompt types and that it might be possible to manipulate learning histories such that a particular prompt type becomes more efficient.

Resumo O ensino de intraverbais para indivíduos com autismo tem sido realizado, principalmente, através de procedimentos que envolvem reforçamento diferencial. Recentemente, o procedimento de observação de pareamento de estímulos (SPOP) surgiu enquanto alternativa para verificar a emergência de repertórios verbais. O SPOP implica o pareamento de estímulos sem exigir outra resposta que a observação do pareamento. O ecoico tem sido apontado como facilitador na emergência de operantes verbais. O presente estudo comparou, com um delineamento de sondas múltiplas concorrentes com tratamento alternado adaptado, a eficácia do SPOP com um procedimento de apresentação contígua de estímulos com requisição de resposta ecoica na emergência de intraverbais em três crianças com autismo. Observou-se a eficácia do SPOP na indução de intraverbais e verificou-se o papel facilitador do ecoico. Discutem-se possíveis efeitos de interferência dos tratamentos, da extinção de respostas no contexto experimental e da utilização de reforçadores condicionados como parte do SPOP.


Abstract Intraverbals have been taught to individuals with autism primarily through procedures involving differential reinforcement. Recently, Stimulus Pairing Observation Procedure (SPOP) emerged as an alternative to verify the emergence of verbal repertoires. The SPOP consists in the simultaneous presentation of two stimuli and requires only the observation response by the participant. The echoic behavior had been pointed out as a mediator in the emergence of verbal operants. This study compared the efficacy of the SPOP and of a contiguous stimulus presentation procedure with an echoic response requirement in emergence of intraverbals in three children with autism by using a concurrent multiple probe design with an adapted alternating treatment. The results showed the SPOP's efficacy for intraverbals induction and suggested a facilitating role of the echoic. A possible carry-over effect between treatments, extinction in the experimental context and the use of conditioned reinforcers in SPOP are discussed.


Résumé L'enseignement de l'intraverbal chez des individus autistes a été principalement réalisé avec des procédures impliquant le renforcement différentiel. Plus récemment, la Procédure d'Observation de Paires des Stimuli (SPOP en anglais) s'est imposée comme alternative pour vérifier l'émergence de répertoires verbaux. Le SPOP implique l'appariement des stimuli sans exiger d'autre réponse que l'observation des paires. Par ailleurs, l'échoïque a été signalé comme facilitateur de l'émergence d'opérants verbaux. Nôtre étude a comparé, avec un plan expérimental de sondes multiples concurrents et traitements alternés adapté, l'efficacité du SPOP en comparaison à une procédure de présentation du stimulus contigu avec réponse échoïque dans l'émergence du comportement intraverbal chez trois enfants autistes. Nous avons pu observer l'efficacité du SPOP dans l'induction de l'intraverbal ainsi qu'un rôle facilitateur de l'échoïque. Nous discutons les effets possibles de l'interférence entre traitements, de l'extinction de réponses dans le contexte expérimental, et de l'emploi de renforçateurs conditionnés dans le cadre du SPOP.


Resumen La enseñanza de intraverbales a autistas se ha realizado principalmente a través de procedimientos con refuerzo diferencial. Más recientemente, el procedimiento de observación de emparejamiento de estímulo (POEE) surgió como alternativa para verificar la emergencia de repertorios verbales. El POEE implica el emparejamiento de estímulos, sin exigir otra respuesta que la observación del emparejamiento. El ecoico se considera facilitador en la emergencia de operantes verbales. Este estudio comparó, desde un delineamiento de sondas múltiples con tratamiento alternado adaptado, la eficacia del POEE con un procedimiento de presentación de estímulo contiguo solicitante de respuesta ecoica en la emergencia de intraverbales en tres niños autistas. El POEE fue eficaz en la inducción de intraverbales, y el ecoico tuvo un papel facilitador. RESUMO: A literatura tem demonstrado uma grande lacuna em estudos com o objetivo de estabelecer ecoico em pessoas com Transtorno do Espectro Autista (TEA). Assim sendo, este trabalho teve por objetivo identificar o acervo de pesquisas direcionadas para o ensino de ecoico em indivíduos com TEA. A revisão foi pautada no modelo Preferred Reporting Items for Systematics Review and Meta-Analysis (PRISMA). A busca, de todas as ocorrências, sem limite de tempo, até o ano de 2018, foi realizada nas bases de dados Web of Science, Pubmed, Scopus, ERIC e PsicINFO. Adotaram-se as palavras-chave e marcadores boleanos autism [or] autist [or] autistic [and] echoic [and] verbal behavior. Os critérios de inclusão foram artigos em português, inglês e espanhol que estabeleceram ecoico como variável dependente em intervenções com pessoas com TEA, apresentar delineamento experimental de sujeito único e demonstrar os dados da aquisição do ecoico no decorrer das sessões de ensino. Como resultados, foram encontrados 338 artigos e, com a aplicação dos critérios de inclusão, foram selecionados três. Os resultados corroboraram os dados da literatura que apontam o déficit de estudos com objetivo de estabelecer ecoico como alvo principal de ensino em pessoas com TEA. A maior parte dos estudos adota o ecoico como prompt para o estabelecimento de outro comportamento verbal. Ainda que escassos, os artigos apontam os efeitos de procedimentos distintos sobre a aquisição do comportamento ecoico em pessoas com TEA e com repertórios iniciais diversificados.


ABSTRACT: Literature has shown a large gap in studies aiming to establish an echoic in people with Autism Spectrum Disorder (ASD). Thus, the objective of this study was to identify the collection of research aimed at teaching echoic in individuals with ASD. The review was based on the Preferred Reporting Items for Systematics Review and Meta-Analysis (PRISMA) model. The search, of all occurrences, without time limit, until the year 2018, was performed in the Web of Science, Pubmed, Scopus, ERIC and PsichINFO database. The keywords and boolean operators autism [or] autist [or] autistic [and] echoic [and] verbal behavior were adopted. The inclusion criteria were articles in Portuguese, English and Spanish that established echoic as a dependent variable in interventions with people with ASD, present a single subject experimental design and demonstrate the data of the echoic acquisition during the teaching sessions. As a result, 338 articles were found and, with the application of the inclusion criteria, three were selected. The results corroborate the data in the literature that point out the deficit of studies to establish echoic as the main target of teaching in people with ASD. Most studies adopt echoic as a prompt for establishing other verbal behavior. Even if scarce, the articles indicate the effects of different procedures under the acquisition of the echoic behavior in people with ASD and with diversified initial repertories.

Comparisons of the relative efficiency of different prompt topographies (visual or auditory), when teaching intraverbal behavior to children with disabilities, have yielded idiosyncratic results. Recent research has shown that previous exposure to a specific prompt type may affect its efficiency when teaching intraverbal behavior to preschool children. The current study was an attempt to replicate these results with 3 children diagnosed with autism spectrum disorder. For all participants, increased exposure to one prompt topography was sufficient to make it relatively more efficient as measured by number of trials to criterion. These results suggest that previous history with a prompt type may predict its efficiency.
