RESUMO

The ectopic nail (EN) is an additional nail located in an abnormal site. It belongs to the onycho-heterotopia, a rare condition whose pathogenesis is indeterminate. This article illustrates the clinical-morphological and dermoscopic points of view, the diagnostic criteria, the possible pathogenesis, and surgical treatment of this pediatric onycho- heterotopia.

Double little toenails, or accessory nails of the fifth toe, are a rare and particular deformity distinct from other nail dysplasias. We describe 4 cases of female patients, including a 6-year-old girl, experiencing this bilateral condition after birth. The nail anomaly shows an autosomal-dominant type of transmission that affects only female individuals. The subjects examined were from a family without marriages between blood relatives and without neuroectodermal hereditary diseases. The 4 patients had no bone alterations of the terminal phalanges of the fifth toe or alterations of the joints of the toes or hair. One patient had nail deformity associated with epilepsy; another patient, associated with an infection of an accessory nail.

RESUMO

Onychoheterotopia (ectopic nail) is a rare condition characterized by the development of nail tissue, distinct from the normal nail unit. It is usually acquired following traumatic inoculation of nail matrix; the congenital variety being less common. The exact pathogenesis of the disease is not clear. It affects the dorsal aspect of fingers and toes mostly. Herein, we report a case of a 35-year-old man with post-traumatic onychoheterotopia of left middle finger, who was treated with surgical avulsion of the ectopic nail along with chemical matricectomy of the well-formed ectopic matrix. The patient had a satisfactory cosmetic outcome with normal growth of the nail unit and no recurrence. The report serves to highlight clinical presentation of acquired onychoheterotopia along with its surgical management.

RESUMO

Ectopic nail is characterized by growth of nail-like tissue in a location other than the nail bed. It is an extremely rare disorder that can be either congenital or acquired from incidents such as trauma. An 8-year old female presented with a complaint of a nail-like lesion on the dorsal surface around the center of the right fifth toe. It had been present for 3 years, while the patient continuously clipped it upon growth. The fifth toe was initially injured by a chair that had fallen on that toe 4 years prior to her visit. There was no associated bony deformity or other physical changes in the nail. A clinical diagnosis of ectopic nail was made and it was surgically excised under local anesthesia. The histopathological examination revealed a normal nail unit and there was no recurrence during the 6 month follow-up period.

RESUMO

A congenital ectopic nail is a rare anomaly and usually manifests itself as a small abnormal nail appearing alongside the normal nail. In most of the cases, abnormal nails have been found on the palmar surface of the fifth finger. The pathogenesis of the ectopic nail remains unclear. It has been postulated to be a kind of polydactyly or generation from stray germ cell or inoculation of the nail matrix after trauma. We observed a case of a 5-month-old boy with congenital ectopic toenail on the right second toe. The ectopic toenail was removed by surgical excision and the histopathological finding was consistent with ectopic toenail. We report a case of congenital ectopic toenail on the right second toe of a 5-month-old boy.
