RESUMO

OBJECTIVE: In China, grade 2 to 3 immune-related rash will probably lead to the interruption of immunotherapy. Corticosteroid (CS) is the main treatment, but not always effective. The external application of clearing heat and removing dampness, which is represented by Qing-Re-Li-Shi Formula (QRLSF), has been used in our hospital to treat immune-related cutaneous adverse events (ircAEs) for the last 5 years. The purpose of this study was to discuss its efficacy and safety in the treatment of grade 2 to 3 rash. METHODS: A retrospective study of patients with grade 2 to 3 immune-related rash in our hospital from December 2019 to December 2022 was conducted. These patients received QRLSF treatment. Clinical characteristics, treatment outcome, and health-related quality of life (HrQoL) were analyzed. RESULTS: Thirty patients with grade 2 to 3 rash (median onset time: 64.5 days) were included. The skin lesions of 24 cases (80%) returned to grade 1 with a median time of 8 days. The accompanying symptoms were also improved with median time of 3 to 4 days. The addition of antihistamine (AH) drug didn't increase the efficacy of QRLSF (AH + QRLSF: 75.00% vs QRLSF: 83.33%, P = .66). No significant difference was observed in the efficacy of QRLSF treatment regardless of whether patients had previously received CS therapy (untreated population: 88.24% vs treated population: 69.23%, P = .36). During 1-month follow-up, 2 cases (8.33%) underwent relapses. In terms of HrQoL, QRLSF treatment could significantly reduce the median scores of all domains of Skindex-16, including symptoms (39.58 vs 8.33, P < .0001), emotions (58.33 vs 15.48, P < .0001), functioning (46.67 vs 13.33, P < .0001) and composite (52.60 vs 14.06, P < .0001). CONCLUSION: External application of clearing heat and removing dampness was proven to be an effective and safe treatment for such patients. In the future, high-quality trials are required to determine its clinical application in the field of ircAEs.

RESUMO

Diabetic kidney disease is one of the complications of diabetes mellitus, which can eventually progress to end-stage kidney disease. The increasing prevalence of diabetic kidney disease has brought huge economic burden to society and seriously jeopardized public health. Ferroptosis is an iron-dependent, non-apoptosis-regulated form of cell death. The regulation of ferroptosis involves different molecular mechanisms and multiple cellular metabolic pathways. In recent years, ferroptosis has been proved to be closely related to the occurrence and development of diabetic kidney disease, and can interact with pathological changes such as fibrosis, inflammation, oxidative stress, and disorders of glucose and lipid metabolism, destroying the structure, form and function of the inherent cells of the kidney, and promoting the progression of the disease. Traditional Chinese medicine has a long history of treating diabetic kidney disease with remarkable curative effect. Current scholars have shown that the oral administration of traditional Chinese medicine and the external treatment of Chinese medicine can regulate GPX4, Nrf2, ACSL4, PTGS2, TFR1 and other key signaling molecules, curb ferroptosis, and prevent the progressive deterioration of diabetic kidney disease. In this paper, the mechanism of ferroptosis and diabetic kidney disease and the prevention and treatment of traditional Chinese medicine are analyzed and summarized, in order to provide new ideas and new plans for the treatment of diabetic kidney disease.

RESUMO

Currently, the gut-organ axis has become a hot research topic. As increasing attention has been paid to the role of gut microbiota in the health of organs, the complex and integrated dialogue mechanism between the gastrointestinal tract and the associated microbiota has been demonstrated in more and more studies. Skin as the largest organ in the human body serves as the primary barrier protecting the human body from damage. The proposal of the gut-skin axis has established a bidirectional link between the gut and the skin. The disturbance of gut microbiota can lead to the occurrence of skin diseases, the mechanism of which is complex and may involve multiple pathways in immunity, metabolism, and internal secretion. According to the theory of traditional Chinese medicine(TCM), the connection between the intestine and the skin can be established through the lung, and the interior disorders will definitely cause symptoms on the exterior. This paper reviews the research progress in the gut-skin axis and its correlation with TCM theory and provides ideas and a basis for cli-nical treatment and drug development of skin and intestinal diseases.

RESUMO

OBJECTIVE: To observe the clinical effect of bird-induced anus-lifting surgery (BIAL) combined with oral Relinqing Granules (RG) in the treatment of lower urinary tract symptoms / chronic prostatitis (LUTS/CP) with damp-heat syndrome using infrared thermal imaging (ITI). METHODS: This study included 165 cases of LUTS/CP with damp-heat syndrome treated in the Departments of Andrology and Urology of our hospital, which were divided into three groups of an equal number and treated by medication of oral tamsulosin hydrochloride at 0.2 mg, qd plus levofloxacin at 0.2 g bid (control), anus-lifting exercise (15 lifts once a day) combined with oral RG at 8 g tid (routine treatment) and BIAL (15 lifts once a day) combined with oral RG at 8 g tid (BIAL+RG), respectively, all for two weeks. We recorded the ITI indicators, NIH-CPSI and Self-Rating Anxiety Scale (SAS) scores of patients before and after treatment, observed adverse reactions during the treatment, and compared them among the three groups of patients. RESULTS: Compared with the baseline, NIH-CPSI and SAS scores were significantly reduced in all the three groups after treatment (P < 0.05), even more significantly in the BIAL+RG (10.49 ± 2.97 and 49.73 ± 2.79) than in the control (21.15 ± 3.56 and 52.62 ± 5.38) and routine treatment groups (15.55 ± 2.77 and 52.35 ± 3.87) (P < 0.05). The ITI temperature at the related parts of the body was remarkably decreased in the three groups after treatment (P < 0.05), even lower in the BIAL+RG group (30.47 ± 1.61, 32.14 ± 1.25, 31.16 ± 1.58, 31.97 ± 1.19) than in the control (33.39 ± 0.6, 33.64 ± 1.07, 32.78 ± 0.58, 33.18 ± 0.78) and routine treatment groups (31.93 ± 1.14, 33.06 ± 0.8, 33.08 ± 0.82, 33.17 ± 1.05) (P < 0.05), with statistically significant difference from the baseline in the BIAL+RG group after 12 hours and 24 hours of treatment (ï¼»34.26 ± 0.06ï¼½ vs ï¼»32.65 ± 0.05ï¼½ and ï¼»32.72 ± 0.09ï¼½, P < 0.05), but not between the two time points (P > 0.05). The incidence rate of adverse reactions was markedly lower in the BIAL+RG and routine treatment groups than in the control (P < 0.05) but not significantly different between the BIAL+RG and routine treatment groups (P > 0.05). CONCLUSION: Bird-induced anus-lifting surgery combined with oral Relinqing Granules can safely and significantly improve infrared thermal imaging indicators and NIH-CPSI scores in LUTS/CP patients with damp-heat syndrome.

RESUMO

Objectives: Musculoskeletal trauma is a common type of injury that can result from damage to the muscular or skeletal system and has been recognized as a leading cause of death and disability worldwide. This study aims to analyze the efficacy of Pyritum external treatment for musculoskeletal trauma. Methods: Randomized controlled trials evaluating the external treatment effect of Pyritum on various types of musculoskeletal traumatic injuries will be considered and identified in the searches of eight databases from their inception to Feb 2023. There will be no restrictions with respect to the publication status, language, or country. The experimental intervention group will be treated with external application of Pyritum alone or in combination with other therapies, and the comparator intervention group will include all types of control interventions. The primary outcome will be measured as treatment efficacy rate, and secondary outcomes will include pain reduction, pain disappearance time, swelling, joint function, and recovery period. Assessment of the methodological quality of this study will be concluded using the risk of bias assessment recommended by the Cochrane Collaboration. If there are sufficient numbers of studies per group in terms of specific rating scales to compare the treatment effects of Pyrium alone and combined external treatment groups, we will consider subgroup analysis. Results: This systematic review will be conducted in compliance with the PRISMA-P statement. Conclusion: We will conduct an extensive search on the proposed topic within the available literature and provide systematic evidence for the efficacy and safety of external application of Pyritum for all types of musculoskeletal trauma. The evidence generated will help design interventions for the external use of Pyritum for this patient group.

RESUMO

Traditional Chinese Medicine (TCM) external therapy for sleep disorder of chronic fatigue syndrome (CFS) has good anti-fatigue effect and can improve sleep quality of patients. The treatment for sleep disorders of CFS with TCM external treatment mainly adopts acupuncture, moxibustion, massage, TCM bath, transcutaneous acupoint electrical stimulation and auricular point sticking, etc., or alone, or comprehensive application, or combined with oral Chinese materia medica. The appropriate treatment method can be selected according to the patients' condition and compliance, which reflects the unique advantages of TCM syndrome differentiation and treatment and the treatment according to people and time. The existing research still needs to further form a standardized and recognized diagnosis and treatment system, so as to better guide clinical popularization and application.

RESUMO

ObjectiveTo explore the mechanisms of internal treatment (Renshen Baidusan), external treatment (Yurui Enema), and combination of the two methods in treating intestinal mucosal injury in the rat model of ulcerative colitis (UC) from the changes of phosphatidylinositol-3 kinase (PI3K)/protein kinase B (Akt)/nuclear factor-κB (NF-κB) pathway. MethodFifty SPF-grade SD rats were randomized into blank, model, Renshen Baidusan (15.6 g·kg-1), Yurui Enema (25 g·kg-1), and combined treatment (15.6 g·kg-1 Renshen Baidusan + 25 g·kg-1 Yurui Enema) groups (n=10). The rat model of UC was established in other groups except the blank group by 2,4, 6-trinitrosulfonic acid (TNBS)/ethanol. The rats were administered with corresponding drugs once a day for 14 consecutive days since the 8th day after modeling. The histopathological changes of colon were observed by hematoxylin-eosin (HE) staining. Enzyme-linked immunosorbent assay (ELISA) was employed to measure the levels of tumor necrosis factor (TNF)-α, interferon (IFN)-γ, interleukin (IL)-4, and IL-10 in the colon tissue. The apoptosis of colon epithelial cells was detected by terminal deoxynucleotidyl transferase-mediated nick end labeling (TUNEL). The location and expression of B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X protein (Bax), TNF-α, and IL-6 in the colon tissue were examined by immunohistochemistry. Real-time quantitative fluorescence polymerase chain reaction (Real-time PCR) and Western blot were employed to determine the mRNA and protein levels, respectively, of the proteins in the PI3K/Akt/NF-κB pathway in the colon tissue. ResultIn the model group, HE staining showed a large number of inflammatory cell infiltration in the mucosa and submucosa. Compared with the blank group, the model group showed elevated levels of TNF-α and IFN-γ and lowered levels of IL-4 and IL-10 in the colon tissue, increased apoptosis rate of colon epithelial cells, increased positive expression of Bax, TNF-α, and IL-6, and decreased positive expression of Bcl-2 (P<0.05). Moreover, the model group showed up-regulated mRNA levels of PI3K, Akt, and NF-κB and protein levels of PI3K, p-PI3K, Akt, p-Akt, p65, p-p65, Bax, and cleaved Caspase-3, increased Bax/Bcl-2 and cleaved Caspase-3/Caspase-3 ratios, and down-regulated protein levels of NF-κB suppressor protein α(IκBα), Bcl-2, and Caspase-3 in the colon tissue (P<0.05). Compared with the model group, the internal treatment, the external treatment, and the combination (referred to as the three groups) alleviated the colonic mucosal injury, lowered the levels of TNF-α and IFN-γ and elevated the levels of IL-4 and IL-10 in the colon tissue, decreased the apoptosis rate of colon cells, inhibited the positive expression of Bax, TNF-α, and IL-6, and promoted the positive expression of Bcl-2 (P<0.05). Furthermore, the combination group down-regulated the mRNA level of PI3K (P<0.05). The three groups down-regulated the mRNA levels of Akt and NF-κB and the protein levels of p-PI3K, Akt, p-Akt, p65, p-p65, Bax, and cleaved Caspase-3 in the colon tissue, decreased the Bax/Bcl-2 and cleaved Caspase-3/Caspase-3 ratios, and up-regulated the protein levels of IκBα, Bcl-2, and Caspase-3 (P<0.05). ConclusionRenshen Baidusan, Yurui Enema, and their combination may inhibit the activation of PI3K/Akt/NF-κB signaling pathway and regulate the expression of genes and proteins related to this pathway to achieve anti-inflammatory and anti-apoptotic effects, thus restoring the intestinal mucosal barrier function of UC rats.

RESUMO

ObjectiveTo observe the clinical efficacy and safety of traditional Chinese medicine (TCM) external therapeutic protocol of enriching pus for tissue growth (EPTG) in the treatment of Wagner 2-3 diabetic foot ulcer. MethodThe randomized controlled trial (RCT) design was adopted. Patients receiving basic treatment were divided into the EPTG group and the control group (debridement and change of nano-silver medical antibacterial dressing). Ulcer healing rate, ulcer area, ulcer depth, TCM symptom score, visual analogue scale(VAS), transcutaneous partial pressure of oxygen, wound blood flow, inflammatory factors [C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α)], growth factors [vascular endothelial growth factor (VEGF), transforming growth factor-β1 (TGF-β1), and fibroblast growth factor-β (FGF-β)], adverse events, and outcome events of the two groups before and after treatment were observed. ResultCompared with the conditions before treatment, the ulcer healing rate, ulcer area, ulcer depth, TCM symptom score, VAS score, transcutaneous partial pressure of oxygen, wound blood flow, CRP, IL-6, TNF-α, VEGF, TGF-β1, and FGF-β were significantly improved (P<0.01). In terms of the improvement in the ulcer healing rate, ulcer area, ulcer depth, VAS score, transcutaneous partial pressure of oxygen, wound blood flow, CRP, IL-6, TNF-α, VEGF, TGF-β1, and FGF-β, the EPTG group was superior to the control group (P<0.01). There were no statistically significant differences in adverse events and outcome events between the two groups. ConclusionThe TCM external therapeutic protocol of EPTG is safe and effective in the treatment of Wagner 2-3 diabetic foot ulcer. It can greatly reduce the area and depth of diabetic foot ulcer, improve the ulcer healing rate and TCM symptom score, relieve the pain of patients, and improve the microcirculatory blood supply in the local ulcer. Its mechanism of action may be related to the reduction of the local inflammatory response of the wound and the improvement of the proliferation of fibroblasts and vascular endothelial cells.

RESUMO

Diabetes and depression are both incurable chronic diseases, which are complementary and integrated. They are clinically called diabetic depression. A large number of epidemiological and clinical studies have confirmed that diabetes interacts with depression, leading to complex conditions of diabetic depression, difficult treatment, and poor prognosis. Diabetes and depression are considered two independent diseases in the majority of the existing clinical methods for the treatment of diabetic depression, with many defects such as low efficiency and severe side effects. Traditional Chinese medicine (TCM) treatment of diabetic depression, characterized by multiple targets, multiple pathways, and overall coordination, can adjust the synergy between multiple systems of the body and is advantageous in the treatment of diabetic depression. However, the evaluation of the efficacy of TCM in the treatment of diabetic depression is mostly limited to the improvement of clinical symptoms, the control of blood sugar level, and the score of depression scale, and there is a lack of in-depth research on the mechanism of action. Therefore, many researchers are devoted to exploring the underlying mechanism of TCM in the treatment of diabetes combined with depression. Clarifying the comorbidity mechanism is the key in treating diabetic depression. In this study, we made an introduction according to the mechanism based on the relevant research reports on the treatment of diabetic depression with TCM and summarized its specific mechanisms, including insulin resistance, oxidative stress and inflammation, and nervous system disorder. The interaction between the various mechanisms has further aggravated the complexity of diabetic depression. We have drawn a diagram of the mechanism pathways of diabetic depression in order to provide new ideas for clinical diagnosis and treatment.

RESUMO

Objective:To investigate the effects of external application of modified Xiangsha Liujunzi Tang in improving gastrointestinal reaction, immune function, and quality of life in patients with gastrointestinal tumors after moderately/highly emetogenic chemotherapy. Method:A total of 140 inpatients (from January 2018 to May 2021) with gastrointestinal malignancies diagnosed by the Oncology Department of Integrated Traditional Chinese and Western Medicine of China-Japan Friendship Hospital and treated with moderately/highly emetogenic chemotherapy were randomly divided into an experimental group (n=70) and a control group (n=70) according to the Good Clinical Practice (GCP). Participants were given routine antiemetic treatment once 20 minutes before chemotherapy (tropisetron 5 mg + dexamethasone 5 mg or methylprednisolone 40 mg). On this basis, the patients in the experimental group were treated with external application of modified Xiangsha Liujunzi Tang for 1 week on the second day after chemotherapy (the selected points were Zhongwan, Neiguan and Zusanli, 6 hours a day, and the application was changed every day), and the patients in the control group were applied with comfort patch at the same time. The gastrointestinal reaction grade, subset concentration of lymphocytes, and Karnofsky score in two groups before and one week after chemotherapy were recorded, and all data were analyzed by SPSS 19.0 statistical software. Result:The grades of chemotherapy-related gastrointestinal reaction in two groups after treatment decreased as compared with that before treatment (P<0.05). The experimental group was superior to the control group after treatment in terms of grade decrease (P<0.05). The total response rate of the experimental group was 81.43% (57/70), higher than 62.86% (44/70) in the control group (χ2=9.73, P<0.05). Stratified analysis was performed on the experimental group. Compared with the conditions before treatment, the grade of gastrointestinal reaction in patients with esophageal cancer and colorectal cancer of the experimental group decreased after treatment (P<0.05), and that in patients of different genders decreased after treatment (P<0.05). The results of immune function showed that there was no significant difference in the concentrations of CD3+, CD4+, CD8+, and NK cell subsets of the control group before and after treatment, while the concentrations of CD3+, CD4+, and NK cell subsets of the experimental group were higher than those before treatment and even superior to those in the control group (P<0.05). The Karnofsky score of quality of life in the experimental group was higher than that in the control group, with quality of life improved (P<0.05). RESUMO

BACKGROUND: The objective of this study was to evaluate the clinical efficacy of the external application of internal expulsion pus-expelling decoction (IEPED) combined with surgery in the treatment of granulomatous lobular mastitis (GLM). METHODS: A total of 110 patients in our hospital with sepsis GLM were randomly divided into two groups: treatment group (n=60, the wound was treated with IEPED) and control group (n=50, the wound was not treated with IEPED). We assessed the recurrence, contra lateral breast form, and aesthetic evaluation of the patients in the two groups. RESULTS: The total effective rates in the patients in the treatment group and the control group were 90% and 68%, respectively, after the preoperative pretreatment and before radical surgery (P<0.05). After 10 days of receiving the debridement treatment, the two groups were compared in term of physical signs scores and the difference was statistically significant (P<0.05). Within one year of the regular follow-up after treatment, 0 case recurred in the treatment group and 1 case recurred in the control group (P>0.05). In the treatment group, 30 cases showed excellent results in the aesthetic evaluation of breast appearance, 18 cases were good, and the overall excellent and good rate was up to 80%. In the control group, 12 cases showed excellent results and 16 cases showed good results, with the overall excellent and good rate reaching 56% (P<0.05). CONCLUSIONS: In patients with abscess debridement of GLM, the external application of IEPED can significantly reduce the primary lesion of patients with abscess GLM, reduce the surgical resection area, and maximize the preservation of the patients' breast appearance.

RESUMO

It is well believed that children's spleen is often insufficient, as the structure and qi of spleen-stomach are incomplete. The deficiency of spleen-stomach plays an important role in the occurrence, development and pathogenesis of pediatric diseases. Professor LIU Xiao-ying has inherited the academic thought of NI Zhu-ying, a famous physician of TCM, and proposed the external treatment method of "supporting the middle". This method emphasizes the constant care of the spleen-stomach of children in the aspect of acupoint selection, and adoptes the external treatment of children's tuina, supporting-yang cupping therapy (including warming tuina, warming moxibustion and warm scrapping), acupuncture (including abdominal acupuncture, intradermal needling) to treat children's diseases of spleen, lung, kidney and others, which has achieved satisfactory therapeutic effects.
