BACKGROUND: Among the spectrum of adipose lesions associated with peripheral nerves, complete circumferential encasement of the nerve by the lesion is a very rare entity. The authors report the first case of a giant lipoma that completely and circumferentially encased the radial nerve. OBSERVATIONS: A 57-year-old woman presented with a large left-arm mass that she had noticed over a year following significant weight loss. She had hypersensitivity and dysesthesias in the left posterior arm but no weakness or other symptoms. Magnetic resonance imaging of the left upper limb demonstrated a large lipoma completely encasing the radial nerve, originating at the level of the spiral grove proximally and extending distally to the lateral epicondyle. The lesion was completely resected at surgery with total preservation of radial nerve function. LESSONS: Lipomas encasing peripheral nerves can be difficult to manage while preserving function. In the present case report, we discuss a case of an uncommon giant lipoma in the left lower abdomen of a 55-year-old female. The case is presented to highlight the need to consider lipoma as one of the possibilities in cases with subcutaneous masses. Despite its abnormal location and considerable magnitude, surgical excision under short general anesthesia resulted in successful outcomes without immediate complications. Finally, by comparing with similar circumstances, it is possible to recognize that individualized management strategies based on patient characteristics can benefit surgical considerations. The significant message is that timely intervention, comprehensive assessment, and teamwork are essential in achieving satisfactory outcomes among patients with rare cases of lipomas, such as this one.

We present a case of a giant Spindle cell lipoma of dimensions 11 cm × 7 cm, involving the middle finger of a 62-year-old female, without distal neurovascular deficits. Spindle cell lipoma is a rare subtype that accounts for 1.5% of all lipomatous tumors. They show a heterogeneous mixture of lipomatous tissue with mature adipocytes interspersed with spindle-shaped cells, without atypia in a sclerotic collagenous stroma. Immunohistochemical (IHC) marker CD34 was positive but negative for S100. The entire tumor was removed with recovery of full range of movements. The case is reported due to the unusual location of a rare variant of giant lipoma involving a finger.

Lipomas, benign tumors originating from the anomalous proliferation of adipocytes, predominantly emerge in regions rich in adipose tissue. However, their presence in the head and neck areas remains rare, constituting approximately 13% of all diagnosed lipoma cases. This study presents a case involving a substantial subcutaneous lipoma located at the posterior neck, measuring about 20 cm × 19 cm × 10 cm. The patient presented with swelling and pain in the back of the neck. And the considerable dimensions of this lipoma significantly impacted the patient's quality of life and aesthetic appearance. Concurrently, the patient exhibited symptoms indicative of degenerative cervical spine disease and cervical disc herniation. After admission, a comprehensive examination, including ultrasound, CT scan, and MRI, was conducted. Given the clinical complexity, the decision for surgical intervention was deemed essential. The surgical strategy entailed a meticulous total excision of the tumor through an incision made in the posterior cross-neck, coupled with the strategic removal of excess skin. To facilitate wound healing, postoperative management included the use of negative pressure drainage. Pathological examination conclusively identified the mass as a lipoma. Postoperative follow-ups indicated successful recovery, as evidenced by the restoration of the neck's aesthetic contour and the complete resolution of the previously observed restrictions in sagittal neck movement.

Pleomorphic lipomas are infrequent soft tissue tumours with pseudosarcomatous behaviour. Their polymorphism provides them certain characteristics that may resemble malignancy, which may mislead the initial diagnosis. The presented case report is a 45-year-old man with a giant growing tumour on the left cervicoscapular region initially categorised as a liposarcoma by magnetic resonance with a final confirmed diagnosis of pleomorphic lipoma after the surgical resection and the examination of the pathologist. The patient presented important functional restriction of the upper left extremity, which decreased after surgical resection, improving the quality of life.

The issue of international migration and health has received increased attention since its rise due to empirical studies on the immigrant paradox. Less examined has been research focused on countries in the Global South with growing immigrant population share and contentious debates on implications. Using data from the longitudinal National Income Dynamics Study (NIDS) (2008-2017) and panel logistic regression this study focused on evaluating whether reported physical health, mental health, and life satisfaction are all associated with immigrant status in South Africa. Accounting for several sociodemographic factors, results emanating from analysis showed no significant evidence of association between overall immigration status and self-reported health or life satisfaction. This however did not apply to depression as immigrants were found to be significantly less likely to report depressive level scores. Interestingly, preference to migrate was also found to be significantly associated with depression, and life satisfaction. Further evaluation amongst racial groups showed that African immigrants are also significantly more likely to report better health than non-immigrant counterparts. This study argues for the need of contextualisation of the immigrant paradox as evidence thereof varies dependent on specific outcomes and communities. The importance of other associated social determinants of wellbeing is also highlighted since gender, race, age, class, education, relationship status, location, and behavioural factors were found to be significantly associated with wellbeing. Policies should thus be aimed at reducing structural inequalities in broad whilst also introducing social programmes that reduce behavioural or lifestyle activities that have negative implications on wellbeing.

INTRODUCTION & IMPORTANCE: Lipomas are slow growing benign soft tissue tumors that arise from mesenchymal preadipocytes. Histologically they are composed of mature adipocytes. They typically have a shawl like distribution in the body, anywhere from the subcutaneous space to bone, but are seen only rarely in the hand. When >5 cm, they are referred to as 'giant lipoma' and can be symptomatic due to neurovascular compression and impaired hand function. CASE PRESENTATION: A 51-year-old forensic analyst presented with a progressively enlarging lump over the thenar eminence and palm of his dominant right hand for 2 years duration. Although initially asymptomatic, he developed progressive numbness over the radial 21/2 fingers and impaired hand function due to its size resulting in occupational impairment. Examination revealed a 5x5cm painless lobulated lump over palm. NCS showed evidence of median nerve compression. MR imaging revealed a giant lipoma involving the thenar and midpalmar space. Enbloc surgical excision was performed and histology was confirmatory. CLINICAL DISCUSSION: Lipomas of the hand could be superficial or deep space. They are slow growing and asymptomatic initially and are brought to attention due to cosmetic concerns, nerve compression or mechanical hand impairment with enlargement. Giant lipomas must be treated with suspicion due to denovo liposarcoma and risk of sarcomatous change. CONCLUSION: Giant multi-compartment lipomas of the hand are rare. Surgical excision is advocated for suspicion of malignancy, nerve compression and functional limitation. Enbloc resection without fragmentation has minimal risk of recurrence and complications.

Lipomas are one of the most common, benign, slow-growing tumours composed of adipose (fat) tissue. These soft, rubbery lumps are usually painless and move easily when touched. Lipomas are generally small, ranging from less than an inch to a few inches in diameter. However, when a lipoma grows to a size larger than 10 cm (about 4 in), it is referred to as a giant lipoma. Only about 1% of all lipomas can be called "giant". Though usually they are benign, in the case of a very large lipoma it is essential to rule out the possibility of malignancy before embarking on its surgical treatment. Lipomas can occur anywhere in the body, but they are most commonly found on the neck, shoulders, back, abdomen, arms, or thighs. Here, we present a case of a 42-year-old woman with a giant lipoma over her left scapula.

Introduction and importance: Lipomas are benign tumours composed of adipocytes surrounded by a thin fibrous capsule. Although they make up 16% of mesenchymal tumours, the occurrence of breast lipomas is uncertain. Giant lipomas, measuring 10 cm or more in diameter or weighing at least 1000 g, commonly occur in the upper back, neck, and thigh, and are rare in the breast. Given its rarity, accurately diagnosing a giant breast lipoma is crucial to prevent potential overtreatment, as it might otherwise be mistaken for a malignant tumour. Case presentation: A 40-year-old woman presented with a painless, gradually enlarging left breast mass. Physical examination and imaging studies revealed a lipoma-compatible mass. Surgery was performed, and the 5 kg mass was enucleated and identified as a lipoma on histopathology. The patient had an uneventful postoperative recovery and was satisfied with the outcome of the surgery after 2 months of follow-up. Clinical discussion: Breast lipomas are more common in the 40-60 years age group, with giant lipomas occurring more frequently in the latter half of this age range. They can mimic various breast conditions, including neoplastic and non-neoplastic conditions, and are often treated with surgical excision to avoid recurrence. The location of the lipoma in the breast can be subcutaneous or intramuscular, and preserving the future pedicle of reduction mammoplasty/mastopexy is essential. Conclusion: Giant breast lipoma is an infrequent condition that can manifests as progressive enlargement of the breast, posing a diagnostic challenge due to its resemblance to various benign or malignant pathologies.

Lipoma is a common type of benign soft tissue tumor that can occur in the shoulders, neck and back, in addition to other body parts. The Retzius space is a small anatomical space between the pubic symphysis and the bladder located extraperitoneally and filled with loose fatty connective tissue. Giant lipomas are rare in the Retzius space. A 61-year-old Chinese male arrived at Beijing Yanhua Hospital (Beijing, China) due to frequent urination, and CT scan images of the lower abdomen observed a large pelvic mass and left inguinal hernia. Preoperative clinical manifestations and auxiliary examination suggested that the tumor originated from the urinary bladder wall. The maximum tumor diameter was ~25 cm and abdominal pressure was increased. Therefore, laparoscopic pelvic tumor resection combined with inguinal hernia repair was attempted. Intraoperatively, the tumor was found to originate from the Retzius space and the postoperative pathological diagnosis was lipoma. The present case report may serve as a reference for minimally invasive treatment of this type of rare disease in future.

The inguinal canal lipoma, known as spermatic cord lipoma in men or round ligament lipoma in women, has a variable incidence (22.5% to 75%) during inguinal hernioplasty procedures. The presence of a true lipoma in this region is considered rare and often underestimated by surgeons. A young female patient was diagnosed with a large true inguinal canal lipoma. Resection was performed using both videolaparoscopic and conventional techniques, based on a careful preoperative evaluation of anatomical parameters. The high incidence of lipomas in the inguinal canal contributes, in part, to the interpretation of fatty masses as "lipomas" during herniorrhaphy procedures. However, many of these are actually extrusions of extraperitoneal adipose tissue, maintaining dimensions within the physiological limits of the region. This confusion in classification highlights the complexity of differentiating between true lipomas and adipose protrusions. Based on a case report enriched with distinct clinical features and images, we sought to exemplify a surgical approach to a large true inguinal canal lipoma. This report not only emphasizes the rarity of the pathology but also underscores the importance of an effective and differentiated surgical approach for true lipomas in this location.

A giant intermuscular lipoma, an exceedingly rare occurrence, constitutes a non-malignant neoplasm originating from the mesodermal germ cell layer, with dimensions surpassing 10 cm. Its differentiation from liposarcoma and other malignant tumours is imperative. We present a case involving a 75-year-old woman who initially raised suspicions of liposarcoma due to pronounced enlargement and fullness in the upper quadrants of the left breast. After comprehensive imaging evaluations, the identification of a sizable BI-RADS 4a lesion positioned between the major and minor pectoral muscles of the left breast was found. The definitive diagnosis of an exceedingly rare giant intramuscular lipoma was validated solely subsequent to the surgical excision of the lipoma, through histological analysis.

OBJECTIVE: Lipomas are very common tumors which usually prefer the upper limbs and, depending on their size, may cause nerve compression, or may be asymptomatic. The current cadaveric report describes a giant lipoma in the distal forearm area. CASE REPORT: A large mass (5.1 × 3.2 × 1.6 cm) was identified on the palmar surface of the distal forearm, during dissection of a 63-year-old male cadaver. The mass caused anteromedial displacement and flattening of the median nerve (MN). Despite the lack of information about the subject's medical history, MN compression was assumed on the basis of the lipoma's size, its vicinity to neural structures, and the MN displacement and flattening. CONCLUSION: The enlarged distal forearm lipoma, located adjacent to the carpal tunnel, displaced and flattened the MN. The cadaveric finding described is clinically relevant for both differential diagnosis and surgical treatment of carpal tunnel syndrome.

Accessory breast is an extremely rare condition that develops in 0.4%-6% of females. It is primarily found in the axilla and is frequently misdiagnosed. It is usually bilateral and manifests during pregnancy or lactation as an asymptomatic tumor. The diagnosis of ectopic breast tissue is crucial because it is capable of undergoing the same pathological changes as normal breast tissue, including mastitis, fibrocystic changes of the breast tissue, and, in extreme cases, even malignancy. We present the case of a 40-year-old female who presented with swelling in the left upper arm for the past eight years, which was associated with pain. Initially, accessory breast was kept as the clinical diagnosis. However, further imaging and histological analysis revealed it to be a giant lipoma of the upper arm.

Lipomas are common benign neoplastic mesenchymal tumours arising from adipose tissue anywhere in the body. Giant intra-thoracic lipoma extending across the anterior mediastinum into bilateral pleural spaces causing compression of the underlying lung resulting in respiratory embarrassment is rarely seen and reported. Giant lipomas of thorax require surgical excision as they continue to grow and cause compressive symptoms.

Intussusception is a common finding among children. Conversely, it is infrequent in adults. Colonic lipomas are generally clinically silent making them a very rare aetiology of intussusception. Presentation of case: The authors present a case of a 48-year-old male who presented to the emergency department suffering from severe abdominal pain. Following examination and investigations, a giant lipoma (GL) in the transverse colon was identified via ultrasound which showed the classical "target sign" Intussusception among adults is unusual as it accounts for only 1% of bowel obstruction cases. Being colo-colonic makes it even more unlikely since it occurs only in 17% of intestinal obstruction cases. GLs exceeding 5 cm in size can present with a variety of symptoms. Intussusception is an uncommon presentation of a GL. Preoperative diagnosis of GL-induced intussusception is highly improbable and surgical resection is the treatment of choice. Conclusion: Despite the dominance of the asymptomatic presentation of lipomas, considering its diagnosis in the case of an intussusception-induced acute abdomen should cross physicians' minds.

INTRODUCTION: Lipomas are benign tumors that occur in the back, shoulder, neck, and extremities. Giant lipomas of the inguinal-perineal area are extremely rare. CASE REPORT: We present a case of a 63-year-old man with a giant lipoma in the inguinal-perineal area. Ultrasound examination showed a heterogeneous hyperechoic mass (14.6 × 8.3 cm) located in the inguinal area, presumably an inguinal hernia. Computed tomography (CT) results showed fat tissue radiographic patterns, and lack of enhancement of the left inguinal area extending to the lateral scrotum. The patient underwent an operation and a radical resection was performed. Histology results revealed a lipoma. At the 1-month follow-up, the patient did not show any evidence of recurrence. CONCLUSION: Giant lipomas occurring in the inguinal-perineal area are extremely rare and are easily confused with other lesions in the groin area. We recommend a detailed preoperative adjunctive examination such as CT. Open surgical complete excision is the ideal treatment.

Lipoma is a benign soft tissue tumour. It is a benign proliferation of mature adipocytes. It is described as giant when its weight exceeds 1 kg or its diameter exceeds 5 cm. Functional and aesthetic impairment may be a major reason for surgical excision. It can be located everywhere, but it mainly occurs in the posterior segment of the chest. We here report a case of giant lipoma of the left posterior-superior segment of the chest.

Introducción: Los lipomas son los tumores más numerosos de tejidos blandos. Se presenta un caso con enfermedad de Hoffman-Zurhelle asociado a lipoma gigante en región toracodorsal y lipomatosis tratado quirúrgicamente de conjunto con las especialidades de cirugía plástica y cirugía general del Hospital Ameijeiras. Objetivo: Mostrar los resultados del tratamiento quirúrgico de exéresis de tumor gigante de espalda. Caso clínico: Paciente masculino de 34 años de edad que se le realizó bajo anestesia general disección del colgajo dermograso en sentido cefálico hasta línea paravertebral y cuello. Se respetó un centímetro de grosor, abordaje a nivel de la interfase del músculo trapecio y dorsal ancho. Se identificó la cápsula del lipoma que se independizó de las fibras musculares y del plano profundo hasta realizar su exéresis y de la piel excedente con nevo lipomatoso cutáneo superficial. Cierre por planos sin tensión. Conclusiones: Se logró la resección de un fibrolipoma gigante en espalda con una planificación cuidadosa que respetó el aporte vascular de los colgajos, la cobertura cutánea sin tensión y conservó la función muscular en ausencia de complicaciones(AU)


Introduction: Lipomas are the most numerous soft tissue tumors. We present a case of Hoffman-Zurhelle disease associated with giant lipoma in the thoracodorsal region and lipomatosis treated surgically in conjunction with the plastic surgery and general surgery specialties of the Ameijeiras Hospital. Objective: To show the results of surgical treatment of excision of a giant tumor of the back. Clinical case: A 34-year-old male patient underwent dissection of the dermographic flap in cephalic direction up to the paravertebral line and neck under general anesthesia. One centimeter thickness was respected, approaching at the level of the trapezius and latissimus dorsi muscle interface. The lipoma capsule was identified and became independent of the muscle fibers and the deep plane until it was excised, as well as the excess skin with superficial cutaneous lipomatous nevus. Closure by planes without tension. Conclusions: Resection of a giant fibrolipoma on the back was achieved with careful planning that respected the vascular supply of the flaps, the skin coverage without tension and preserved muscle function in the absence of complications(AU)
