Resumo Analisaram-se indicadores de vigilância da qualidade da água para consumo humano no Amazonas, de 2016 a 2020, utilizando 185.528 amostras provenientes de 11 microrregiões. Das amostras analisadas, 93,20% são da área urbana, 66,65% provinham do sistema público (SAA), 31,02% da Solução Alternativa Coletiva (SAC) e 2,33% da Solução de Alternativa Individual (SAI). Observou-se aumento do número de registros pelo SAA, com tendência de queda e oscilações de registros para a SAC e a SAI. Os indicadores de qualidade dos parâmetros químicos e físicos da área urbana foram superiores aos das áreas rurais e de comunidades tradicionais. A maior parte das amostras apresentou valores de pH abaixo do recomendado. Na quantificação dos parâmetros microbiológicos, identificou-se maior presença de coliformes totais e E.coli na área rural e em comunidades tradicionais. Em conclusão, verificaram-se inadequações nos parâmetros químicos, físicos e microbiológicos, assim como problemas relativos ao abastecimento, armazenamento e à vigilância da água distribuída para consumo humano. Tais achados indicam a necessidade de construir uma agenda, pela gestão pública, para o enfrentamento da insegurança hídrica e seus prováveis efeitos sobre a insegurança alimentar existente na região.


Abstract Surveillance indicators of the quality of water for human consumption in the Amazon were analysed from 2016 to 2020 using 185,528 samples from 11 microregions. Of the samples analysed, 93.20% were from urban areas, 66.65% were from the public water supply system (WSS), 31.02% were from the Collective Alternative Solution-CAS, and 2.33% from the Individual Alternative Solution-IAS. There was an increase in the number of records by the WSS, with a downwards trend and fluctuations in records for the CAS and the IAS. The quality indicators of chemical and physical parameters for urban areas were higher than those for rural areas and traditional communities. Most of the samples presented pH values below the recommended level. In the quantification of microbiological parameters, a higher presence of total coliforms and E. coli was identified in samples from rural areas and in traditional communities. In conclusion, there were inadequacies in the chemical, physical and microbiological parameters as well as problems related to the supply, storage and surveillance of water distributed for human consumption. These findings indicate the need to build an agenda for public management to address water insecurity and its likely effects on food insecurity in the region.

Most of the signs and symptoms of heart failure can be explained by fluid overload, which is also related to disease progression. Fluid overload is a complex phenomenon that extends beyond increased intravascular pressures and poses challenges for accurate diagnosis and effective treatment. Current recommendations advise a multiparametric approach, including clinical data (symptoms/signs), imaging tests, and biomarkers. This article proposes a practical therapeutic approach to managing hydrosaline overload in heart failure in both inpatient and outpatient settings. This document is an initiative of the Spanish Society of Internal Medicine (SEMI) in collaboration with the Spanish Society of Cardiology (SEC) and the Spanish Society of Nephrology (S.E.N.).

El espacio de comedores son una forma de garantizar el acceso a alimentos seguros, saludables y medio ambientalmente responsables para los estudiantes universitarios. Objetivo. Este estudio tuvo como objetivo caracterizar los estándares del menú, a partir de la evaluación de la calidad nutricional y del impacto en el medio ambiente por los insumos utilizados para su elaboración por un Comedor Universitario (CU) en Uruguay. Materiales y métodos. Se trató de un estudio de caso retrospectivo, relativo al año 2021. Las variables evaluadas fueron a) cantidad total de alimento comprado en kilogramos (kg); b) cantidad en kilocalorías (kcal) e impactos ambientales por: c) generación de residuos; d) huella hídrica (HH); y e) gases de efecto invernadero (GEI). También se realizaron cálculos para evaluar la ecoeficiencia (EE). Resultados. El CU sirvió 33.740 comidas en 2021, de las cuales el 87,1% fueron almuerzos. De los 78 tipos de alimentos comprados, 41 de ellos representaron el 93,17% del total en kilogramos. En la calidad nutricional, menos del 10% se clasificaron como alimentos procesados o ultraprocesados. En la evaluación de aspectos nutricionales e impactos ambientales, los alimentos de origen animal representaron el 26,52% del total en kg y el 69,78% de los HH. Un alimento de origen vegetal tuvo el mayor impacto de GEI debido a su origen. En cuanto a la EE, la valoración general mostró un resultado de 0,0626 para los alimentos de origen animal y 0,3838 para los de origen vegetal. Conclusiones. Corresponde a los servicios de alimentación para colectividades considerar, en la planificación del menú, además de la calidad nutricional y sanitaria de la oferta alimentaria, los impactos ambientales que estas genera(AU)


The canteens are a way to guarantee access to safe, healthy and environmentally responsible food for university students. Objective. The purpose of this research was to characterize the menu standards, based on the evaluation of the nutritional quality and the environmental impact of the inputs used for its preparation by a university canteen in Uruguay. Materials and methods. This was a retrospective case study, relating to the year 2021. The variables evaluated were a) total amount of food purchased in kilograms (kg); b) quantity in kilocalories (kcal) and environmental impacts due to: c) waste generation; d) water footprint (WF); and e) greenhouse gases (GHG). Calculations were also performed to assess eco-efficiency (EE). Results. The university canteen served 33,740 meals in 2021, of which 87.1% were lunches. Of the 78 types of food purchased, 41 of them represented 93.17% of the total in kilograms. In nutritional quality, less than 10% are classified as processed or ultra-processed foods. In the evaluation of nutritional aspects and environmental impacts, foods of animal origin represented 26.52% of the total in kg and 69.78% of WF. A vegetal based food had the highest GHG impact due to its origin. Regarding the EE calculations, the general assessment showed a result of 0.626 for foods of animal origin and 0.3838 for those of vegetable origin. Conclusions. It corresponds to the food services for communities to consider, in the planning of the menu, in addition to the nutritional and sanitary quality of the food offer, the environmental impacts that it generates(AU)

Objetivo: Avaliar a presença de CI em estudantes das fases iniciais e finais dos cursos de Medicina, Nutrição e Engenharia Civil de uma Universidade no Extremo Sul Catarinense -Criciúma, através da ingestão alimentar e hídrica, dos tipos de fezes, se fazem uso, ou não, de alternativas de evacuação, comparando os hábitos alimentares com influência na constipação nos estudantes das três diferentes áreas. Método:Tal estudo foi realizado através de um questionário adaptado com questões sobre os hábitos de vida do indivíduo, juntamente com os critérios de Roma III, Roma IV e Escala de Bristol. Resultados:Caracterizou-se por 158 estudantes, sendo 71,5% (n=113) representam o sexo feminino e apenas 28,5% (n=45), o sexo masculino. A ingestão de líquidos demonstrou-se ser baixa, sendo 33,5% (n=53) ingerem mais que 1600ml/dia. Através da Escala de Bristol, 15,8% (n=25) revelaram evacuar o Tipo 1 e 2, caracterizando CI. Sobre os laxantes, apenas 3,2% (n=5) confirmaram a utilização. Foi verificada CI em 18,6% (n=21) das mulheres e 8,9% (n=4) dos homens. Conclusão:a alimentação destacou ser pobre em fibras. É notório que os estudantes sofrem com sintomas de CI. Através do auxílio de um profissional de nutrição, é necessário que equilibrem sua alimentação com fibras, consumem mais água diariamente e, consequentemente, auxiliem no bom funcionamento intestinal e na melhora da qualidade de vida.


Objective: To evaluate the presence of IC in students in the initial and final stages of Medicine, Nutrition and Civil Engineering courses at a University in the extreme south of Santa Catarina -Criciúma, through food and water intake, types of feces, whether they use, or no, of evacuation alternatives, comparing eating habits with influence on constipation in students from three different areas. Method: This study was carried out through a questionnaire answered with questions about the individual's life habits, together with the criteria of Rome III, Rome IV and Bristol Scale. Results: Characterized by 158 students, 71.5% (n=113) female and only 28.5% (n=45) male. Liquid intake was low, with 33.5% (n=53) ingesting more than 1600ml/day. Through the Bristol Scale, 15.8% (n=25) revealed to evacuate Type 1 and 2, characterizing CI. Regarding laxatives, only 3.2% (n=5) confirmed their use. CI was found in 18.6% (n=21) of women and 8.9% (n=4) of men. Conclusion:the highlighted diet is low in fiber. It is notorious that students suffer from HF symptoms. Through the help of a nutrition professional, it is necessary that they balance their diet with fiber, consume more water daily and, consequently, help in the good intestinal functioning and in the improvement of the qualityof life.

Método:Este estudio se realizó mediante un cuestionario adaptado con preguntas sobre el estilo de vida del individuo, junto con los criterios de Roma III, Roma IV y la Escala de Bristol. Resultados:Se caracterizó por 158 estudiantes, 71,5% (n=113) mujeres y sólo 28,5% (n=45) hombres. La ingesta de líquidos resultó ser baja, con un 33,5% (n=53) ingiriendo más de 1.600 ml/día. A través de la Escala de Bristol, el 15,8% (n=25) reveló evacuar Tipo 1 y 2, caracterizando CI. Respecto a los laxantes, sólo el 3,2% (n=5)confirmó su uso. La CI se verificó en el 18,6% (n=21) de las mujeres y en el 8,9% (n=4) de los hombres. Conclusión:la dieta era baja en fibra. Es notorio que los estudiantes padecen síntomas de CI. Con la ayuda de un profesional de la nutrición, es necesario que equilibren su dieta con fibra, consuman más agua diariamente y, en consecuencia, ayuden en el buen funcionamiento intestinal y en la mejora de la calidad de vida.

BACKGROUND: Patients undergoing congenital heart surgery with cardiopulmonary bypass frequently require the administration of intravenous fluids and blood products due to hemodynamic instability. Correctly performed fluid resuscitation can revert the state of tissue hypoperfusion in the different organs. However, excessive fluid administration and acute kidney injury may promote fluid overload (FO) and increase the risk of complications, hospital stay, and mortality. METHODS: We conducted a prospective longitudinal study of pediatric patients with congenital heart surgery and cardiopulmonary bypass in the Pediatric Cardiac Intensive Care Unit (PCICU), Instituto Nacional de Pediatría, from July 2018 to December 2019. Fluid overload was quantified every 24 hours during the first 3 days of stay at the PCICU and expressed as a percentage. We recorded PCICU stay, days of mechanical ventilation, and mortality as outcome variables. RESULTS: We included 130 patients. The main factors associated with fluid overload were age < 1 year (p < 0.001), weight < 5 kg (p < 0.001), and longer cardiopulmonary bypass time (p = 0.003). Patients with fluid overload ≥ 5% had higher inotropic score (p < 0.001), higher oxygenation index (p < 0.001), and longer mechanical ventilation time (p < 0.001). Fluid overload ≥ 5% was associated with higher postoperative mortality (odds ratio 89, p = 0.004). CONCLUSIONS: Fluid overload can be used as a prognostic factor in the evolution of pediatric patients undergoing congenital heart surgery since it is associated with increased morbidity and mortality.


INTRODUCCIÓN: Los pacientes con cirugía cardiaca congénita en la que se emplea una bomba de circulación extracorpórea frecuentemente requieren la administración de líquidos intravenosos y hemoderivados por inestabilidad hemodinámica. La resucitación con volumen realizada adecuadamente puede revertir el estado de hipoperfusión tisular en los diferentes órganos. Sin embargo, el ingreso excesivo de líquidos y la falla renal aguda pueden favorecer la sobrecarga hídrica (SH) e incrementar el riesgo de complicaciones, la estancia hospitalaria y la mortalidad. MÉTODOS: Se llevó a cabo un estudio prospectivo longitudinal de pacientes pediátricos con cirugía del corazón y empleo de bomba de circulación extracorpórea en la Unidad de Cuidados Intensivos Cardiovasculares (UCICV), Instituto Nacional de Pediatría, de julio 2018 a diciembre 2019. La SH, registrada como porcentaje, fue cuantificada cada 24 horas durante los primeros 3 días de estancia en UCICV. Como variables de desenlace se registraron la estancia en UCICV, el tiempo de ventilación mecánica y la mortalidad. ­. RESULTADOS: Se incluyeron 130 pacientes. Los principales factores asociados con la SH fueron la edad < 1 año (p < 0.001), peso < 5 kg (p < 0.001) y mayor tiempo de circulación extracorpórea (p = 0.003). Los pacientes con SH ≥ 5% presentaron mayor puntaje inotrópico (p < 0.001), mayor índice de oxigenación (p < 0.001) y mayor tiempo de ventilación mecánica (p < 0.001). La SH ≥ 5% se asoció con una mayor probabilidad de muerte en el periodo posoperatorio (razón de momios: 89, p = 0.004). CONCLUSIONES: La SH puede utilizarse como factor pronóstico en la evolución de los pacientes pediátricos operados de corazón, ya que se asocia con una mayor morbimortalidad.

Introducción: La relación entre los aspectos emocionales y físicos, ha sido motivo de estudio de forma recurrente, en los pacientes en hemodiálisis.Objetivo: Analizar la personalidad de los pacientes en hemodiálisis y ver la relación existente entre esta y las variables sexo e ingesta hídrica.Material y Método: Se estudiaron 54 pacientes en hemodiálisis periódicas. Se ha llevado a cabo un estudio descriptivo correlacional de corte transversal. Se administró el inventario de personalidad NEO-FFI. Se recogieron los datos sobre la ganancia hídrica interdiálisis. Se cuantificaron los valores de frecuencia de las puntuaciones obtenidas en el test y se realizó comparación con la población de referencia no ERC, y correlación con las variables sexo y ganancia hídrica interdiálisis.Resultados: Entre los factores de personalidad existen diferencias estadísticamente significativas con respecto a la población de referencia no ERC; por un lado, en el factor de neuroticismo con tendencia a la superioridad (p=0,000), más en mujeres que en hombres; y por otro, en los factores de extroversión (p=0,000) y apertura a la experiencia (p=0,000) con tendencia a la inferioridad. Por otro lado, no se han encontrado diferencias estadísticamente significativas entre los factores analizados, en relación a la ganancia hídrica interdiálisis.Conclusiones: Los pacientes en hemodiálisis tienden a ser inestables emocionalmente, más las mujeres que los hombres, con tendencia a ser introvertidos, con ideas rígidas e intereses reducidos. El control de la ingesta de líquidos no depende de ninguno de los factores estudiados. (AU)


Introduction: The relationship between emotional and physical aspects has been recurrently studied in haemodialysis patients.Objective: To analyse the personality of haemodialysis patients and to determine the relationship between personality and the variables sex and water intake.Material and Method: Fifty-four patients on periodic haemodialysis were studied. A cross-sectional descriptive correlational study was carried out. The NEO-FFI personality inventory was administered. Data on interdialysis water gain were collected. Frequency values of test scores were quantified. Comparisons were made with the non-CKD reference population, and correlations were made with the variables sex and interdialysis water gain.Results: Among the personality factors, statistically significant differences were found with respect to the non-RCD reference population. Regarding the neuroticism factor, a tendency towards superiority was found (p=0.000), more in women than in men; while in the extroversion (p=0.000) and openness to experience (p=0.000) factors, the tendency was towards inferiority. No statistically significant differences were found between the factors analysed in relation to interdialysis water gain.Conclusions: Haemodialysis patients tend to be emotionally unstable, more women than men, with a tendency to be introverted, with rigid ideas and reduced interests. Control of fluid intake does not depend on any of the factors studied. (AU)

OBJECTIVE: To measure the magnitude and duration of the hypotensive effect of two prostaglandin analogues in glaucoma patients using the water drinking test (WDT). METHODS: Patients received latanoprost or travoprost every 24 h and then every 48 h. Untreated WDT were performed at 7 am and with treatment 12, 36 and 44 h after the last dose; intraocular pressure (IOP) peak, fluctuation and the difference between peak and isolated IOP measurements at consultation times were calculated. RESULTS: Forty-one eyes of 21 patients with primary open-angle glaucoma were included; 22 eyes received latanoprost, and 19 received travoprost. Mean untreated isolated IOP was 17.20 standard deviation (S.D.) 3.73 and 16.95 S.D. 2.61 mmHg and peak pressure 22.45 S.D. 2.91 and 21.58 S.D. 3.79 mmHg, for the latanoprost and travoprost groups, respectively. With treatment, peak pressure was reduced by 22.64% and 20.29% at 12 h, 18.44% and 14.64% at 36 h and 16.17% and 14.46% at 44 h, respectively. The fluctuation without treatment was 4.36 and 5.11 mmHg, and with treatment at 12 h was reduced to 2.77 and 2.89 mmHg, increasing again at 36 and 44 h. CONCLUSIONS: A hypotensive effect was evident up to 44 h after the last dose of latanoprost and travoprost, similar for the two drugs and decreasing over time. IOP fluctuation was only reduced at 12 h.

Objetivo Medir con la prueba de sobrecarga hídrica (PSH) la magnitud y duración del efecto hipotensor de dos análogos de prostaglandinas en pacientes con glaucoma. Métodos Los pacientes recibieron latanoprost o travoprost cada 24 horas y luego cada 48 horas. Se practicaron PSH sin tratamiento a las 7 am y con tratamiento 12, 36 y 44 horas después de la última dosis; se calcularon el pico de presión intraocular (PIO), la fluctuación y la diferencia entre el pico y las medidas aisladas de PIO en horas de consulta. Resultados Se incluyeron 41 ojos de 21 pacientes con glaucoma primario de ángulo abierto, 22 ojos recibieron latanoprost y 19, travoprost. La PIO aislada promedio sin tratamiento fue 17,20 desviación estándar (D.S.) 3,73 y 16,95 D.S. 2,61mmHg y el pico de presión 22,45 D.S. 2,91 y 21,58 D.S. 3,79mmHg, para los grupos de latanoprost y travoprost, respectivamente. Con tratamiento, la presión pico se redujo en 22,64% y 20,29% a las 12 horas, 18,44% y 14,64% a las 36 horas y 16,17% y 14,46% a las 44 horas, respectivamente. La fluctuación sin tratamiento fue 4,36 y 5,11mmHg, y con tratamiento a las 12 horas se redujo a 2,77 y 2,89mmHg, aumentando nuevamente a 36 y 44 horas. Conclusiones Se evidenció un efecto hipotensor hasta 44 horas después de la última dosis de latanoprost y travoprost, similar para los dos medicamentos y decreciente en el tiempo. Objective To measure the magnitude and duration of the hypotensive effect of two prostaglandin analogues in glaucoma patients using the water drinking test (WDT). Methods Patients received latanoprost or travoprost every 24hours and then every 48hours. Untreated WDT were performed at 7 am and with treatment 12, 36 and 44hours after the last dose; intraocular pressure (IOP) peak, fluctuation and the difference between peak and isolated IOP measurements at consultation times were calculated. Results Forty-one eyes of 21 patients with primary open-angle glaucoma were included; 22 eyes received latanoprost, and 19 received travoprost. Mean untreated isolated IOP was 17.20 standard deviation (S.D.) 3.73 and 16.95 S.D. 2.61mmHg and peak pressure 22.45 S.D. 2.91 and 21.58 S.D. 3.79mmHg, for the latanoprost and travoprost groups, respectively. With treatment, peak pressure was reduced by 22.64% and 20.29% at 12hours, 18.44% and 14.64% at 36hours and 16.17% and 14.46% at 44hours, respectively. The fluctuation without treatment was 4.36 and 5.11mmHg, and with treatment at 12hours was reduced to 2.77 and 2.89mmHg, increasing again at 36 and 44hours. Conclusions A hypotensive effect was evident up to 44hours after the last dose of latanoprost and travoprost, similar for the two drugs and decreasing over time. IOP fluctuation was only reduced at 12hours (AU)

RESUMO

The physiological quality of the seeds is an aspect of great relevance for the establishment of a culture. However, plant management conditions during seed production can influence its quality. Given the above, the objective was to evaluate the physiological quality of crambe seeds from plants submitted to different water regimes. The experimental design used was in randomized blocks, composed of five water conditions in the soil, based on field capacity (100, 85, 70, 55 and 40%) that consisted of treatments, with four replications. After harvesting, the seeds were sent to the seed laboratory, processed and then evaluated for water content, germination and vigor. Crambe plants grown in soil water conditions between 59 to 61% of the field capacity produce seeds of better physiological quality, demonstrated by the performance in the germination and vigor tests.


A qualidade fisiológica das sementes é um aspecto de grande relevância para o estabelecimento de uma cultura. No entanto, as condições de manejo das plantas durante a produção de sementes podem influenciar na sua qualidade. Diante do exposto, objetivou-se avaliar a qualidade fisiológica de sementes de crambe oriundas de plantas submetidas a diferentes regimes hídricos. O delineamento experimental utilizado foi em blocos casualizados, composto por cinco condições de água no solo, baseadas na capacidade de campo (100, 85, 70, 55 e 40%) que consistiram nos tratamentos, com quatro repetições. Após a colheita, as sementes foram encaminhadas ao laboratório de sementes, beneficiadas e em seguida, avaliadas quanto ao teor de água, germinação e vigor. Plantas de crambe cultivadas em condições de água no solo entre 59 a 61% da capacidade de campo produzem sementes de melhor qualidade fisiológica, demonstrado pelo desempenho nos testes de germinação e vigor.
