RESUMO

With the continuous advancement of health informatization and the wide application of medical big data, electronic health records came into being and spread rapidly. However, because electronic health records contain a large amount of private information, privacy protection is the primary consideration for the sustainable development of electronic health records. By analyzing the shortcomings of privacy protection of electronic health records in law, technology, management and protection consciousness, this paper put forward some countermeasures, such as perfecting the relevant laws and regulations of privacy protection of electronic health records, improving the technical level, improving the management defects of electronic health records, and cultivating the privacy protection consciousness of professionals and the public, so as to improve the overall privacy protection level of China’s health records information management system and provide effective protection for the privacy information of Chinese residents’ electronic health records.

RESUMO

The surveillance of occupational disease has entered a new stage ofdevelopment, with the implementation of the national health informatization project. To improve the efficiency and quality of occupational disease monitoring information reporting in this paper, the system architecture and related management regulations, as long as the major changes and achievement of "surveillance system of occupational disease and health hazards information" under the framework of National Health Insurance Informatization Project were elaborated. The deficiencies existing in the system were analyzed, and expectation for the construction of the occupational disease surveillance system was addressed.

Objective:To analyze the health development plans of the provinces in China during the " 14th Five-Year Plan" , and explore the key tasks, similarities and differences of health informatization construction in each province.Methods:Using the website of local people′s government and the official website of the provincial Health Commission, 27 copies of health development plans of various provinces during the " 14th Five-Year Plan" period were retrieved and collected from February 16 to June 5, 2022. The relevant statements of health information in the plan were extracted, content analysis was used to reveal the structural characteristics of the policy in the form of word frequency statistics, and discourse analysis was used to study the policy content.Results:The health information policies of 27 provinces during the " 14th Five-Year Plan" period could be summarized as 10 major themes, such as accelerating the construction of hierarchical diagnosis and treatment system, promoting the high-quality development of public hospitals, and deepening the reform of medical security system. The health information policy in the eastern, central and western regions was relatively clear, and there were certain differences in the construction points according to their own characteristics.Conclusions:During the " 14th Five-Year Plan" period, the policies of each province around the field of health information are well defined, and the core structure and content are similar. The distribution of key points in the eastern region is relatively balanced; the construction of health information in the central region is more prioritized and prominent; the construction of health information in the western region is focused on complementing the weak links and weaknesses.

The common coding of main diagnosis (so-called "manual coding") is accompanied by quite many errors associated with inobservance of classification algorithm and subjective causes that in statistical morbidity and mortality data corruption. The article presents the results of implementing automated system of diagnosis coding by the ICD-10 in the departments of the Medicosanitary Unit of MVD of Russia in City of Moscow. The functional and time losses and the error rate were studied under manual coding and automated coding system. In case of common coding of complete clinical diagnosis of one discharged patient per day, it takes from 5 to 15 minutes, for coding diagnoses of all discharged patients - about 1 hour. The number of errors in common coding made up to 14.1% (n=2472) and in case of automated coding it made up to 1.5% (n= 63, p>0.05). The application of automated coding system in medical organizations ensures exact correspondence of clinical and statistical diagnosis, unifies wording of clinical diagnosis, minimizes subjective deviations from algorithm of the ICD-10.

By applying advanced health information technology to the health care field, health informatization helps optimize health resource allocation, improve health care services, and realize universal health coverage. COVID-19 has tested the status quo of China's health informatization, revealing challenges to the health care system. This viewpoint evaluates the development, status quo, and practice of China's health informatization, especially during COVID-19, and makes recommendations to address the health informatization challenges. We collected, assessed, and evaluated data on the development of China's health informatization from five perspectives-health information infrastructure, information technology (IT) applications, financial and intellectual investment, health resource allocation, and standard system-and discussed the status quo of the internet plus health care service pattern during COVID-19. The main data sources included China's policy documents and national plans on health informatization, commercial and public welfare sources and websites, public reports, institutional reports, and academic papers. In particular, we extracted data from the 2019 National Health Informatization Survey released by the National Health Commission in China. We found that China developed its health information infrastructure and IT applications, made significant financial and intellectual informatization investments, and improved health resource allocations. Tested during COVID-19, China's current health informatization system, especially the internet plus health care system, has played a crucial role in monitoring and controlling the pandemic and allocating medical resources. However, an uneven distribution of health resources and insufficient financial and intellectual investment continue to challenge China's health informatization. China's rapid development of health informatization played a crucial role during COVID-19, providing a reference point for global pandemic prevention and control. To further promote health informatization, China's health informatization needs to strengthen top-level design, increase investment and training, upgrade the health infrastructure and IT applications, and improve internet plus health care services.

BACKGROUND: The National Health and Family Planning Commission (NHFPC) in China organized a nationwide survey to investigate the informatization in hospitals and regional Health and Family Planning Commissions (HFPCs) in 2017. The survey obtained valid results from 79.69% (2021/2536) of major hospitals and 81% (26/32) of provincial and 73.1% (307/420) of municipal HFPCs. The investigated topics covered hardware infrastructure, information resources, applications, systems, and organizations in health informatics. OBJECTIVE: This study aimed to provide evidence collected from the survey regarding China's health informatization and assist policy making regarding health informatics in the 13th Five-Year Plan of China. METHODS: Based on the survey, the paper presented the status quo of China's health informatization and analyzed the progress and potential problems in terms of the country's health information development policies. RESULTS: Related policies have helped to construct 4-level information platforms and start converging the regional data to the 3 centralized databases. The principle of informatics has been transiting from finance-centered to people-centered. Alternatively, the quality, usability, and interoperability of the data still need to be improved. CONCLUSIONS: The nationwide survey shows that China's health informatization is rapidly developing. Current information platforms and databases technically support data exchanging between all provinces and cities. As China is continuing to improve the infrastructure, more advanced applications are being developed upon it.

Following an overview of the present big medical data sharing abroad, the paper identified the problems of the regional health information platform in data sharing and utilization as follows. Namely, poor data integration, low data availability, poor data security and privacy, unclear data sharing model, and poor data management accountability. On such basis, the authors made thoughtful studies in data quality management, information security and privacy protection and data sharing model. These efforts provide useful references for big health data integration sharing and open access.

Objective:To analyze and evaluate population health informatization development of China. Meth-ods:Through this investigation, 8 578 copies of a questionnaire designed for all levels of health administration and parts of primary hospitals were collected. These collected data, were analyzed to evaluate the population health infor-matization development of China through the evaluation index system established for population health informatization development and comprehensive evaluation model. Results:The results of this empirical study indicate that the popu-lation &Health Informatization overall has rapid development , but still has regional differences, and total score of the top 10 provinces are Zhejiang, Shanghai, Jiangsu, Liaoning, Beijing, Chongqing, Tianjin, Hunan, Henan and Gan-su. A statistical difference (p<0. 05) exists in regional development on population & health informatization of Chi-na, therefore serval provinces have obvious weakness or advantage in the construction of informatization. Discussion& suggestions:Government of China should pay attention to evaluation during the construction of informatization;pro-mote a balanced and sustainable development by strengthening financial support and optimizing the input pattern;up-grade regional population health informatization overall according to the information resources as the core;and develop the key priorities of population health informatization depending on practical situation and developing trend.

Objective:The present study aims at setting up an evaluation index system aiming at providing a set of measurement tools for population & health informatization developing level for administrative departments. Meth-ods: Delphi method, analytic hierarchy process, Synthetic Index Method and some mathematical or statistical meth-ods were used respectively to select the primary indexes, calculate the weight and combined weight of each index, build up the synthetic evaluation mode,and test the evaluation system. Results:According to the results of this work, the evaluation index system has been established which is composed of 6 level 1 indexes, 16 level 2 indexes, and 33 level 3 indexes, and it covers 6 aspects, such as information resources construction, information infrastructure con-struction, information technology application, informatization talents and the information policy, planning, and stand-ards. The weights of these indexes were also calculated. The synthetic evaluation model has been built. Finally, the reliability, the validity and the discrimination of the evaluation system were tested. Conclusions:The evaluation index system has been established, which is able to be used in the evaluation, and to some extent guide the work of popula-tion & health informatization construction.

A competency dictionary of advanced health informatization application persons was compiled by inter-viewing 20 health information persons with behavior events interview , their competency characteristics were coded By Nvivo, the coding frequency and characteristics score in good performance group and ordinary group were ana-lyzed by variance analysis, which showed 8 characteristics of key competency and 12 characteristics of basic compe-tency in advanced health information application persons.
