RESUMO

A hospital-based health technology assessment (HB-HTA) can provide the evidence needed to inform clinical decisions at the administrative level. With the implementation of a new round of medical and health care system reforms in China, such as the abolition of medical mark-ups, adoption of modern hospital management systems, reform of diagnosis related groups (DRGs) payment, and performance evaluations for public hospitals, medical institutions increasingly need HB-HTA. The development of HB-HTA in China can be divided into three phases: An initiation phase (2005-2014), a preliminary exploratory phase (2015-2017), and a rapid development phase (2018-present). HB-HTA has been used to manage medical consumables, medical devices, and medicines, but there are still problems and challenges in terms of concept recognition, the mode of development, and limited professionals and data. To promote and use HB-HTA in developing countries, we have identifies the development paths and recommendations for implementation based on a case study in China, which can be summarized as follows: enhancing the top-level design of HB-HTA, formulating HB-HTA guidelines, further promoting the main ideas of HB-HTA, concentrating on the training of evaluation personnel, establishing an HB-HTA network and paying attention to the flexibility of HB-HTA in the application process, and multi- stakeholder participation.

RESUMO

BACKGROUND: The modern era of human society has seen the rise of a different variety of diseases. The mortality rate, therefore, increases without adequate care which consequently causes wealth loss. It has become a priority of humans to take care of health and wealth in a genuine way. METHODS: In this article, the authors endeavored to design a hospital management system with secured data processing. The proposed approach consists of three different phases. In the first phase, a smart healthcare system is proposed for providing an effective health service, especially to patients with a brain tumor. An application is developed that is compatible with Android and Microsoft-based operating systems. Through this application, a patient can enter the system either in person or from a remote place. As a result, the patient data are secured with the hospital and the patient only. It consists of patient registration, diagnosis, pathology, admission, and an insurance service module. Secondly, deep-learning-based tumor detection from brain MRI and EEG signals is proposed. Lastly, a modified SHA-256 encryption algorithm is proposed for secured medical insurance data processing which will help detect the fraud happening in healthcare insurance services. Standard SHA-256 is an algorithm which is secured for short data. In this case, the security issue is enhanced with a long data encryption scheme. The algorithm is modified for the generation of a long key and its combination. This can be applicable for insurance data, and medical data for secured financial and disease-related data. RESULTS: The deep-learning models provide highly accurate results that help in deciding whether the patient will be admitted or not. The details of the patient entered at the designed portal are encrypted in the form of a 256-bit hash value for secured data management.

RESUMO

PURPOSE: To examine the nature of the interruptions during medication work. DESIGN: An exploratory descriptive design was used with 28 nurses during 15 medication rounds in the medical, surgical, and gynecology wards at a 1000-bed tertiary teaching hospital in Saudi Arabia. METHODS: Non-participant prospective observational and follow-up interview methods were used to collect data. A standard paper-based tool and digital stopwatch to count the interruptions were used. Nurses' sources, the secondary tasks undertaken, and the impacts of the interruptions were considered. The data were analyzed via descriptive analysis. RESULTS: A total of 87 medication-related events occurred. In these, 182 interruptions accounted for 90.0% of the total. Interruptions were more frequent during medication administration and often occurred in the corridors and patients' rooms. Nurses, medical officers, and impediments were frequent sources of interruptions. The secondary tasks attended by the nurses were mostly related with impediment resolutions and patients' needs. System failures appeared to be associated with clinical and procedural errors. CONCLUSION: The results of this study provide information regarding the course of interruptions during medication work in a Saudi Arabian hospital. Nurses were the main source of interruptions related with the provision of patient care and resolution of impediments. Interruptions tended to be associated with clinical errors and procedural failures due to system failure. Promoting a culture of respect and workplace etiquette in global healthcare settings demands minimizing non-life-threatening interruptions during nursing works. CLINICAL RELEVANCE: Strategies used in hospital management systems that affect nursing, pharmacies, human resources, and ancillary service department collaborations should be monitored for effectiveness and to ensure patient safety. Various approaches toward medication management systems, such as the utilization of an Omnicell automatic dispensing machine and cabinets within each patient's room, as well as others, such as wearing a "no-interruption zone" sign, could be tailored and matched to the context of the practice to reduce interruptions.

RESUMO

Beijing Union Medical College Hospital applied the standardized management theory, focused on establishing a scientific and standardized system management mechanism, and carried out the exploration of modern hospital management standardization system construction through systematic practice. With the application of the principles of simplification, unification, coordination, optimization and competition of standardization into the construction of modern hospital management system, the basic method for the construction of standardization system was proposed based on standard system management theory. It included the standardization of system architecture, system construction process and system management mechanism. Its purpose was to effectively address the problems of the lack of systematicness, practicality and sustainability in the construction of hospital system. Importantly, the value and vitality of a system depended on its sustainable development. According to the standard system management principle of system effect, structural optimization, orderly development and circular control, the optimization of hospital system could be promoted, which is worth further practice and exploration.

RESUMO

The " 13th five-year Plan" period is a critical period for the modern hospital management system in China. The basic framework of the combination of the macro-external management system and the micro-internal management system has been preliminarily constructed, and new progress has been made in the new hospital operation mechanism that is guided by the needs of patients, high-quality development and guaranteed sustainable. In 2021, China has entered the " 14th five-year Plan" period, and the modern hospital management system will enter the comprehensive implementation stage. In view of the problems existing in the modern hospital management system at the macro, medium and micro levels, the authors put forward relevant approaches from three aspects: reforming the management system to promote the modernization of hospital governance system, creating a good policy environment to promote the scientific and standardized hospital management, and deepening the reform of internal management system to realize the modernization of governance ability. For example, according to the constitution of the hospital, the internal governance system should be established, the reform and innovation of personnel establishment should be promoted, the emergency management capacity of the hospital should be improved, the ability evaluation system of clinicians should be rebuilt, the discipline construction and talent construction should be strengthened, etc., which can provide reference for the implementation of the modern hospital management system during the " 14th five-year Plan" period

RESUMO

In 2015, the Emergency Medicine Department at Muhimbili National Hospital (MNH) installed and implemented the first Electronic Medical Record (EMR) tailored to the emergency centre (EC). The EMR deployed was designed for emergency centre use only (Emergency Department Information System (EDIS)) and linked with the existing EMR that focused on registration and billing. This very collaborative experience can be used as a reference to share the many lessons learnt by all, including hospital management, EC staff, private funders and EMR vendors. The IT Project Plan was developed to make sure steps were followed for EDIS implementation. This included the IT plan documents, specific user requirements, development of a Memorandum of Understanding and user manuals. Super key users were identified among the staff during the training and they helped to empower staff, consolidate knowledge and share the workload. Several challenges have been overcome, including when the power was not regulated so an automatic generator and uninterruptible power supply (UPS) devices installed to protect all computers. Providers were primarily a very novice group of computer users and many had little to no computer experience so were taught both basic computing skills and EDIS specific tasks. Trained staff were moved around the hospital and a lot of time was taken up training new staff, so discussion with hospital management led to retention of staff in the EC. Specific templates have been introduced to ensure adequate minimum documentation. However, even with these, clinical notes are often very brief and we are searching for further mechanisms to improve this. Hospitals in low-resource settings considering the implementation of an EMR should ensure that a comprehensive plan is in place that involves significant staff training, improvement of existing, or installation of new information technology systems, ongoing ICT support and funds for unforeseen issues and ongoing maintenance.

RESUMO

Objective To investigate the present professionalism of clinicians at public hospitals in China and its relationship with practicing environment and hospital management system. Methods From April 2015 to March 2016, 659 clinicians of 11 public hospitals, from Guandong, Hubei and Guangxi provinces, were recruited into the study based on multi-stage sampling. The questionnaire included professional attitude(20 items)and professional behavior(10 items).Descriptive statistics and spearman rank correlation analysis were applied for analysis. Results For professional attitude, 15 items were agreed by over 80% of respondents. All the positive professional behaviours were agreed by over 80% of respondents and all the negative professional behaviours were denied by less than 30% of respondents. Professional attitude was positively correlated with professional behaviour(r=0.157, P<0.01), practicing environment (r= 0.269, P < 0.01 ) and management system ( r = 0.199, P < 0.01). Professional behaviour was uncorrelated with practicing environment or management system ( P >0.05 ). Conclusions Clinicians at public hospitals in China present positive professional attitude which is correlated with professional behaviour, but they are not in high accordance with each other. Clinicians have low satisfaction with the practicing environment and management system, which are important factors influencing professional attitude of clinicians.

RESUMO

The most appropriate organizational software is always a real challenge for managers, especially, the IT directors. The illustration of the term "enterprise software selection", is to purchase, create, or order a software that; first, is best adapted to require of the organization; and second, has suitable price and technical support. Specifying selection criteria and ranking them, is the primary prerequisite for this action. This article provides a method to evaluate, rank, and compare the available enterprise software for choosing the apt one. The prior mentioned method is constituted of three-stage processes. First, the method identifies the organizational requires and assesses them. Second, it selects the best method throughout three possibilities; indoor-production, buying software, and ordering special software for the native use. Third, the method evaluates, compares and ranks the alternative software. The third process uses different methods of multi attribute decision making (MADM), and compares the consequent results. Based on different characteristics of the problem; several methods had been tested, namely, Analytic Hierarchy Process (AHP), Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS), Elimination and Choice Expressing Reality (ELECTURE), and easy weight method. After all, we propose the most practical method for same problems.

RESUMO

The modernization of contemporary hospital management system embodies such five aspects as clear-cut responsibilities, scientific management, reasonable governance, efficient operation and powerful supervision. The first calls for clarifying the powers and responsibilities within and beyond the hospital and those of its external " regulation and administration" . The second calls for check and balance in internal supervision, and the external supervision to extend from results supervision to that of medical service performance. The third calls for modernization of hospital governance system and governance capabilities. The fourth calls for explorations of both the rules of action to achieve hospital objectives in practice, and hospital management patterns in theory. The fifth calls for inner dynamism of efficient operations resulting from performance appraisal and remuneration system reform. Fundamentally, enhanced construction of the Party paves the way for enhancing modernization of such management system.

RESUMO

Data analysis based on breast cancer risk factors such as age, race, breastfeeding, hormone replacement therapy, family history, and obesity was conducted on breast cancer patients using a new enhanced computerized database management system. My Structural Query Language (MySQL) is selected as the application for database management system to store the patient data collected from hospitals in Malaysia. An automatic calculation tool is embedded in this system to assist the data analysis. The results are plotted automatically and a user-friendly graphical user interface is developed that can control the MySQL database. Case studies show breast cancer incidence rate is highest among Malay women, followed by Chinese and Indian. The peak age for breast cancer incidence is from 50 to 59 years old. Results suggest that the chance of developing breast cancer is increased in older women, and reduced with breastfeeding practice. The weight status might affect the breast cancer risk differently. Additional studies are needed to confirm these findings.
