RESUMO

Objectives: This article reports on four rare cases involving multiple trauma-induced adjacent missing anterior teeth in the maxillary or mandibular region. These cases were successfully treated using a 4-axial implant-based alternative insert and an immediate loading protocol. Material and methods: This series of cases was summarized by retrospective study that 4 patients who received a total of 20 immediately loaded implants. These patients had suffered from trauma-induced loss of 8-9 adjacent anterior teeth. The 4-axial-implants were inserted with the assistance of digital pioneer drill guides. The surgical procedure involved alveolar bone trimming or ultrasonic osteotomy, eliminating the need for traditional large-area bone augmentation. Pre- and post-operative CBCT was matched using DTX Studio Implant software, the deviation of implant between actual position and preoperative design was measured and compared using SPSS software package. Results: The average follow-up duration 48 months after implant prostheses, the cumulative retention rate of the implants was 100%, the marginal bone loss averaged 0.53 mm (SD 0.15 mm), and buccal plate bone loss averaged 0.62 mm (SD 0.41 mm). Conclusions: This retrospective clinical report demonstrates the successful treatment of several patients with multiple adjacent maxillary or mandibular anterior teeth using four implant-supported screws to fix the frame and employing immediate loading. The approach resulted in long-term stable clinical outcomes. Moreover, the method not only shortens the period of edentulism but also facilitates easy disassembly, maintenance, and cleaning. Consequently, it emerges as a highly favorable clinical option for patients suffering from extensive tooth loss.

A cirurgia guiada estática se caracteriza por utilizar de um guia cirúrgico que tem como objetivo reproduzir a posição virtual ideal do implante, por meio de sítios criados para a inserção de uma série de brocas de acordo com a trajetória pré estabelecida pelo planejamento digital. Realizar uma revisão integrativa da literatura sobre cirurgia guiada estática em implantodontia, demonstrando sua importância, vantagens, desvantagens, limitações e complicações em comparação com técnica convencional. Tratou-se de uma revisão integrativa da literatura de natureza exploratória, realizada durante o período de agosto de 2021 e maio de 2022, a partir das produções científicas indexadas na base eletrônica da PubMed. Um total de 13 estudos foram incluídos após a seleção e suas evidências foram reunidas para esclarecer os objetos propostos. Os estudos demostraram que uma das principais vantagens da técnica guiada estática é a precisão em relação a técnica convencional e melhor pós operatório de dor relatado pelos pacientes. Não se pode confiar totalmente no processo de produção dos guias, devido o fator experiencia está associado, além que o custo e tempo dessa produção são elevados. Como complicação e limitação os estudos citam possibilidades de ocorrer mínimo acesso ao sítio cirúrgico e fratura do guia. A cirurgia guiada é uma excelente alternativa, as vantagens mais citadas são em relação a precisão, redução da dor, no entanto há fatores negativos como custo, a impossibilidade de mudança no transoperatório e possíveis erros de fabricação. Existem ainda limitações como fratura da guia e baixo acesso ao sítio cirúrgico.


Static guided surgery is characterized by using a surgical guide that aims to reproduce the ideal virtual position of the implant, through sites created for the insertion of a series of drills according to the trajectory pre-established by digital planning. Perform an integrative review of the literature on static guided surgery in implantodontia, demonstrating its importance, advantages, disadvantages, limitations and complications compared to conventional technique. This was an integrative review of the exploratory literature, carried out during the period of August 2021 and May 2022, from the scientific productions indexed in pubmed's electronic database. A total of 13 studies were included after selection and their evidence was gathered to clarify the proposed objects. Studies have shown that one of the main advantages of the static guided technique is accuracy in relation to the conventional technique and better postoperative pain reported by patients. One cannot fully trust the production process of the guides, due to the experience factor is associated, and the cost and time of this production are high. As a complication and limitation, studies mention possibilities of minimal access to the surgical site and fracture of the guide. Guided surgery is an excellent alternative, the most cited advantages are in relation to accuracy, pain reduction, however there are negative factors such as cost, the impossibility of change in the transoperative period and possible manufacturing errors. There are also limitations such as fracture of the guide and low access to the surgical site.

En la implantología actual, la confección de prótesis de carga inmediata se ha convertido en un procedi-miento de rutina. Contar con elementos pre-formados con un correcto ajuste al implante o transepitelial so-bre el que se trabaja, minimiza el tiempo de trabajo sin renunciar a la eficiencia. En el presente trabajo se muestran elementos preformados articulados para la realización de prótesis de carga inmediata y su forma de uso, así como un análisis biomecánico de las estructuras para conocer su repercusión en las distintas fuerzas recibidas durante la masticación. Resultados: Al aplicar la carga en la zona central de la barra (paralela a los implantes), la tensión máxima recibida en la zona correspondiente al extremo de la barra sufre variaciones importantes, desde 128 Mpa en la longitud de 13 mm hasta un máximo de 391 Mpa (megapascales) en la longitud de 5 mm, siendo la ten-sión máxima, media para todas las medidas, de 242 Mpa (+/-96,76). En el ensayo de las diferentes medi-das de la barra se observa también una tensión cre-ciente para longitudes de barra a partir de 7 mm, al aplicar la tensión en la zona media de la estructura, por lo que longitudes entre 5 y 7 mm pueden consi-derarse prácticamente con la misma distribución de tensiones hacia los extremos y en la zona de unión. En conclusión, las barras articuladas son un elemento de confección de prótesis provisionales de carga in-mediata de gran utilidad, que pueden confeccionarse de forma rápida y generan un comportamiento bio-mecánico predecible (AU)


In current implantology, the fabrication of immediately loaded prostheses has become a routine procedure. Being able to have pre-formed elements with a correct fit to the implant or transepithelial on which we are working minimizes working time without sacrificing efficiency. Material and methods: We show articulated preformed elements for immediate loading prostheses and how they are used, as well as a biomechanical analysis of the structures to determine their repercussion on the different forces received during mastication. Results: When the load is applied in the central area of the bar (parallel to the implants) the maximum stress received in the area corresponding to the end of the bar undergoes significant variations, from 128 Mpa in the 13 mm length to a maximum of 391 Mpa in the 5 mm length, the average maximum stress for all the measurements being 242 Mpa (+/-96.76). In the test of the different bar sizes we can also observe an increasing stress for bar lengths from 7 mm onwards when applying the stress in the middle zone of the structure, so that lengths between 5 and 7 mm can be considered to have practically the same stress distribution towards the ends and in the joint zone. Conclusions: Hinged bars are a very useful fabrication element for immediately loaded provisional prostheses, which can be fabricated quickly and generate a predictable biomechanical behavior (AU)

BACKGROUND: The appropriate loading protocol to improve masticatory performance (MP) is still unclear in elderly patients and two-implant overdentures (2-IODs) wearers. This study aimed to compare the long-term MP and maximum occlusal force of immediate loading (IL) and conventional loading (CL) of 2-IODs retained by magnetic attachments. Nineteen edentulous patients were randomly assigned to either an IL (n=10) or CL group (n = 9). In the IL group, the implant was loaded on the same day as insertion, whereas it was loaded 3 months after insertion in the CL group. Magnetic attachments were used to retain all overdentures to the implants. MP, measured by a piece of color-changeable chewing gum and a gummy jelly test, and maximum occlusal force, measured using an occlusal force measuring device, were assessed in both groups at baseline and at 3-, 4-, and 5-year follow-ups. RESULTS: No significant differences were observed in the MP and the maximum occlusal force between the IL and CL groups at any time point. However, a significantly higher MP was observed at the 3-year time point in the IL group (P = 0.036). The maximum occlusal force revealed a significant correlation with MP, both with the color-changeable chewing gum and gummy jelly at 5 years. CONCLUSION: After long-term observation, no significant differences in MP and maximum occlusal force were observed between the IL and CL groups. However, compared to pre-implant insertion of the complete denture, the MP in the IL group significantly improved at 3 years. Furthermore, the maximum occlusal force was significantly correlated with MP at 5 years. TRIAL REGISTRATION: UMIN, UMIN000009889 . Registered on 28 January 2013.

BACKGROUND: To evaluate the prevalence of peri-implant disease after immediate implant placement and loading. MATERIAL AND METHODS: This cross-sectional analysis included a total of 47 patients with 64 implants exhibiting a mean loading time of 2 to 10 years (4.23 ± 1.7 years). The surgical and prosthetic procedures were standardized in all patients. Peri-implant health and disease was assessed based on the established case definitions. RESULTS: The prevalence of peri-implant health, peri-implant mucositis, and peri-implantitis amounted to 38.3%, 57.5%, and 4.2% of the patients, respectively. Mucosal recession of 1 mm was present at 4 (6%) implants. No suppuration, pain, or implant failures were reported. Ordinal logistic regression revealed that reduced keratinized mucosa height was significantly associated with the diagnosis of peri-implant mucositis and peri-implantitis (OR = 0.514, P = 0.0125). CONCLUSION: Immediate implant placement and loading was associated with high success rates at 2 to 10 years.

Tooth loss is a problem that affects both old and young people. It may be caused by several conditions, such as poor oral hygiene, lifestyle choices or even diseases like periodontal disease, tooth grinding or diabetes. Nowadays, replacing a missing tooth by an implant is a very common process. However, many limitations regarding the actual strategies can be enumerated. Conventional screwed implants tend to induce high levels of stress in the peri-implant bone area, leading to bone loss, bacterial bio-film formation, and subsequent implant failure. In this sense, root-analogue dental implants are becoming promising solutions for immediate implantation due to their minimally invasive nature, improved bone stress distribution and because they do not require bone drilling, sinus lift, bone augmentation nor other traumatic procedures. The aim of this study was to analyse and compare, by means of FEA, the stress fields of peri-implant bone around root-analogue and screwed conventional zirconia implants. For that purpose, one root-analogue implant, one root-analogue implant with flaps, two conventional implants (with different threads) and a replica of a natural tooth were modelled. COMSOL was used to perform the analysis and implants were subjected to two simultaneous loads: 100 N axially and 100 N oblique (45°). RESULTS: revealed that root-analogue implants, namely with flaps, should be considered as promising alternatives for dental implant solutions since they promote a better stress distribution in the cortical bone when compared with conventional implants.

PURPOSE: To evaluate the accuracy of computer-aided dental implant positions obtained with mucosal-supported templates as compared to Three-Dimensional (3D) planning. MATERIALS AND METHODS: One-hundred implants were inserted into 14 edentulous patients using the All-on-4/6 protocol after surgical virtual planning with RealGUIDE, 3DIEMME, and Geomagic software. After 6 months, three-dimensional neck (V) and apex (S) spatial coordinates of implants and angle inclination displacements as compared to virtual plans were evaluated. RESULTS: The S maxilla coordinates revealed a significant discrepancy between clinical and virtual implant positions (p-value = 0.091). The V coordinates showed no significant differences (p-value = 0.71). The S (p-value = 0.017) and V (p-value = 0.038) mandible coordinates showed significant discrepancies between the clinical and virtual positions of the screws. Implant evaluation showed a 1-mm of the horizontal deviation in the V point and a 1.6-mm deviation in the S point. A mean 5° angular global deviation was detected. The multivariate permutation test of the S (p-value = 0.02) confirmed the difference. Greater errors in the mandible were detected as compared to the maxilla, and a higher S discrepancy was found in the posterior jaw compared to the anterior section of both the mandible and maxilla. CONCLUSIONS: Computer-aided surgery with mucosal-supported templates is a predictable procedure for implant placement. Data showed a discrepancy between the actual dental implant position as compared to the virtual plan, but this was not statistically significant. However, the horizontal and angle deviations detected indicated that flap surgery should be used to prevent implant positioning errors due to poor sensitivity and accuracy in cases of severe jaw atrophy.

ABSTRACT The demand for orthodontic treatment in adults has been increasing. However, these patients often require a multidisciplinary approach, due to the lack of posterior teeth, requiring additional anchorage. The skeletal anchorage by endosseous implants is an option, since they may be used later for prosthetic rehabilitation. The application of immediate load on these appliances for orthodontic movement may reduce the costs and total treatment time. This paper discusses the utilization of endosseous dental implants with immediate load for absolute anchorage for orthodontic movement, with later utilization for prosthetic rehabilitation.


RESUMO A demanda por tratamento ortodôntico em pacientes adultos tem sido cada vez maior. Entretanto, esses pacientes frequentemente exigem um tratamento multidisciplinar, devido às ausências de dentes posteriores, necessitando de ancoragem adicional. A ancoragem esquelética usando implantes osseointegrados apresenta-se como alternativa, uma vez que eles serão posteriormente utilizados para reabilitação protética. A aplicação de carga imediata sobre esses dispositivos, para movimentação ortodôntica, pode reduzir custos e o tempo total de tratamento. O presente artigo tem como objetivo discutir o emprego de implante dentário osseointegrado, com carga imediata, como ancoragem absoluta para movimentação ortodôntica e, posteriormente, utilizado para reabilitação protética.

Introducción: Dentro del marco de su actividad profesional el estomatólogo se enfrenta a diversas afecciones como la reabsorción radicular interna, cuyos casos avanzados pueden complicar el pronóstico del tratamiento endodóntico, debido al debilitamiento de la estructura dental remanente y las posibles afectaciones periodontales. Debido a que la boca está localizada en uno de los puntos focales de la cara, cualquier alteración en la apariencia estética puede provocar implicaciones psicológicas que pueden ir desde una simple forma de esconder el defecto hasta la más grande introversión. Objetivo: Presentar un caso clínico de reabsorción dentaria interna resuelto con implante postextractivo de carga inmediata. Presentación de caso: Paciente femenina de 31 años con reabsorción dental interna de incisivo central superior. Se llevó a cabo un adecuado diagnóstico y plan de tratamiento del caso, colocándose un implante transalveolar cargado de forma inmediata y la rehabilitación definitiva metalcerámica una vez concluida la fase de osteointegración. Conclusiones: Se consiguió una rehabilitación estética, funcional y biomecánicamente estable a largo plazo, que coincide con las expectativas del paciente, lo cual demuestra que cuando se cumplan los requisitos adecuados y la planificación sea exhaustiva, la implantología ofrece el tratamiento más avanzado en casos en que es inminente la extracción dental(AU)


Introduction: In their daily professional practice dentists are faced with a variety of conditions. An example is internal root resorption, the advanced stage of which may complicate the prognosis of dental treatment, due to the weakening of the remnant dental structure and the potential appearance of periodontal disease. Because the mouth is located in one of the focal points of the face, any alteration of its esthetic appearance may result in psychological disorders ranging from a simple attempt to mask the defect to the greatest introversion. Objective: Present a clinical case of internal tooth resorption solved with a post-extraction immediate load implant. Case presentation: A female 31-year-old patient presents with internal tooth resorption of the maxillary central incisor. Appropriate diagnosis was performed and a treatment plan devised, consisting in immediate placement of a transalveolar implant and definitive metal-ceramic rehabilitation upon completion of the osseointegration stage. Conclusions: Stable long-term esthetic, functional and biomechanical rehabilitation was achieved which met the patient's expectations. This result shows that when the established requirements are complied with and exhaustive planning is performed, implantology offers the most advanced treatment when dental extraction is imminent(AU)

Este artigo consiste na reabilitação protética através do protocolo I de Brånemark em mandíbula submetido a carga imediata. A instalação de implantes imediatos pós-exodontia tem como principais indicações a preservação da arquitetura dos tecidos peri-implantares e a redução de tempo total de tratamento. Após a cirurgia, foram escolhidos o uso de pilares protéticos multi base e, para reabilitação superior, prótese total, visando a melhoria e conforto do paciente portador de próteses totais removíveis convencionais, onde as mesmas prejudicam a função, a estética, saúde psicológica e as relações sociais de seus usuários.


This article consists of prosthetic rehabilitation through the Branemark protocol I, in mandible submitted to immediate loading. The installation of immediate post-exodontic implants has as main indications the preservation of the architecture of the peri-implant tissues and the reduction of total time of treatment, where after the surgery, the use of multi-prosthetic abutments and for superior rehabilitation, prosthesis total. aiming at the improvement and comfort of the patient with conventional removable total dentures, where they impair the function, aesthetics, psychological health and social relations of its users.

Se realizó un estudio descriptivo, longitudinal y prospectivo para describir la evolución de los 86 pacientes tratados con 99 implantes mediatos unitarios de carga inmediata temprana (marca Leader Implus), colocados sobre hueso cicatrizado en la Clínica Estomatológica Provincial Docente Mártires del Moncada de Santiago de Cuba, durante el período comprendido desde febrero de 2014 hasta septiembre de 2015. En la casuística predominaron las féminas de 40-49 y 30-39 años de edad. La región anatómica más implantada fue el maxilar en la zona anterior, en tanto los criterios más desfavorables resultaron ser: la higiene bucal inadecuada, la dimensión seleccionada del implante y el incumplimiento de las indicaciones estomatológicas. La evolución de los integrantes de la serie se evaluó como satisfactoria en 86,1 por ciento del total


A descriptive, longitudinal and prospective study to describe the progress of the 86 patients treated with 99 single nonimmediate implants of early immediate load (Leader Implus), placed on healed bone in Mártires del Moncada Teaching Provincial Stomatological Clinic was carried out in Santiago de Cuba, during February, 2014 to September, 2015. In the case material there was a prevalence of the females aged 40-49 and 30-39. The most implanted anatomical region was the anterior maxillar, as long as the most unfavorable approaches were: the inadequate oral hygiene, the selected dimension of the implant and the nonfulfillment of the stomatological indications. The clinical course of the members of the series was evaluated as satisfactory in 86.1 percent of the total

Objetivo: Este estudo avaliou o impacto da reabilitação bucal na qualidade de vida através da instalação de prótese implanto-suportada sob carga imediata em mandíbulas edêntulas. Material e Métodos: Foi realizado um estudo longitudinal de base ambulatorial reunindo 14 pacientes com idade igual ou superior a 18 anos, atendidos nos cursos de especialização em Implantodontia da Associação Brasileira de Odontologia (Juiz de Fora) e da Faculdade de Odontologia da UFJF. Os participantes foram entrevistados sobre dados de identificação, auto percepção de saúde geral e saúde bucal e dados socioeconômicos. Foi utilizado o Oral Health Impact Profile (OHIP-14). Após o teste de normalidade Kolmogorov-Smirnov os valores de tendência central do OHIP-14 foram comparados pelo teste de Wilcoxon pareado com nível de significância de 5%. Resultados: A pontuação média do questionário de qualidade de vida foi 30,14 (antes) e 48,93 (depois). Houve redução significativa do impacto em todos os domínios no período pós cirúrgico, exceto na limitação funcional que se manteve inalterada. Não foi identificado impacto das dimensões dor física e deficiência pós a instalação de implantes. Conclusão: A reabilitação protética e autopercepção são impactantes na saúde bucal, por isso vale ressaltar a necessidade do cuidado, da observação criteriosa de todos os fatores que possuem influência sob as enfermidades, e não somente aos seus sinais e sintomas. Objective: This study assessed the impact of oral rehabilitation with immediate-loading fixed prostheses on the quality of life of patients with mandibular edentulism. Material and Methods: A longitudinal study was conducted on an outpatient basis. The sample included 14 patients over 18 years of age, treated in the Implantology Specialization programs of the Brazilian Dental Association (in Juiz de Fora, Brazil) and the School of Dentistry, Federal University of Juiz de Fora. Participants were interviewed about identification data, self-perceived general health and oral health, and socioeconomic data. The Oral Health Impact Profile (OHIP-14) was used. After the Kolmogorov-Smirnov normality test, the OHIP-14 central tendency values were compared using the Wilcoxon paired test with a 5% significance level. Results: The overall mean found for the quality of life questionnaire was 30.14 (before) and 48.93 (after). There was a significant reduction of impact in all areas in the post-surgical period, except for functional limitation. No impact was identified for the dimensions of physical pain and disability after installing implants. Conclusion: Prosthetic rehabilitation and self-perception have high impact on oral health, thus emphasizing the need for care, careful observation of all the factors that may have an influence on infirmities, and not only their signs and symptoms.(AU)

Computer-assisted implant planning and subsequent production of a surgical template based on this plan has gained attention because it provides restoratively driven esthetics, patient comfort, satisfaction, and the option of flapless surgery and immediate restoration. However, it adds expense and requires more time. Another significant but not so apparent advantage may be improved survival and success over freehand techniques in types III and IV bone. This retrospective analysis was undertaken to examine that possibility. It reports 1-year outcome for 80 implants in 27 consecutively presenting patients treated over a 7-year period using computer-assisted techniques across all bone qualities in commonly encountered treatment indications in private practice. Implants were placed to support single teeth, small bridges, and complete arch restorations in exposed or immediately restored applications, based on primary stability as determined by insertion torque, resonance frequency analysis, and Periotest. For the 80 implants supporting 35 restorations, the median observation period is 2.66 years; 73 implants supporting prostheses in 22 patients had readable radiographs at 1 year. There was a 1-year overall implant survival and a success rate of 100%. Radiographic analysis demonstrated the change in bone level from the platform at 1-year is less than 2 mm. Intra-operative median measurements of primary stability were insertion torque, 40 Ncm; resonance frequency, 76 ISQ; and Periotest, -3. All intra-operative measurements were consistent for acceptable primary stability regardless of bone density. Restoratively driven diagnosis and precision planning and initial fit were possible with computer-assisted techniques resulting in the achievement of high primary stability, even in areas of less dense bone. The ability to plan implant position, drill sequence, and implant design on the basis of predetermined bone density gives the practitioner enhanced pretreatment information which can lead to improved outcome.

The aim of this study is to compare the success rates of immediately placed and loaded NobelActive implants with the success rate of immediately placed implants that were allowed to osseointigrate prior to loading. The charts of all patients in a private oral surgery office receiving single-unit dental implants in the maxillary anterior region in fresh extraction sites from 2008-2011 were evaluated. All patients receiving NobelActive implants and immediate restorations were included in the study group, while those receiving implants with delayed restorations were included in the control group. Patient records were evaluated for variables such as age, gender, torque values at time of implant placement, smoking habits, use of bisphosphonates, and other significant diseases such as diabetes. The success rate of the study group was 92.9%, whereas the success rate of the control group was 97.6%. This was not statistically significant. Torque values of the failed implants of the study group were similar to those of successful implants in the study group. All implants placed in patients scheduled for immediate loading achieved high torque values and were able to be restored immediately. NobelActive implants were able to obtain high torque values for predictable immediate restoration in fresh extraction sites. Acceptable success rates with excellent soft tissue healing were achieved.

PURPOSE: The first aim of this study is to compare the outcomes in rehabilitating the atrophic maxilla using zygomatic implants (ZIs) and regular implants (RIs) using the classical zygomatic technique (CZT) versus the zygomatic anatomy-guided approach (ZAGA). The second goal of this paper is to propose a standardized system to report rhinosinusitis diagnosis. MATERIALS AND METHODS: Twenty-two consecutive zygomatic patients operated on from 1998 to 2002 and 80 consecutive zygomatic patients operated on from 2004 to October 2009 were selected. All included patients were in a maintenance program. Survival rates (SRs) of ZI and RI were recorded. Implants were individually tested using Periotest® (Periotest value [PTv], Siemens AG, Bensheim, UK). Sinus health was radiographically and clinically assessed according to Lund-Mackay system and Lanza and Kennedy survey recommended by Task Force on Rhinosinusitis for research outcomes. A satisfaction questionnaire (Oral Health Impact Profile for assessing health-related quality of life in Edentulous adults) and different anatomical measurements were also performed. RESULTS: No significant differences (p = .602) were observed with respect to SR between the two groups (95.12% vs 96.79%). Significant differences (p = .000) were found comparing measurements of ZI head distance to the alveolar crest (5.12 ± 2.38 mm vs 2.92 ± 2.30 mm). With the CZT, more palatal emergence of ZI was observed. PTv gave significantly greater stability for the CZT compared with the ZAGA group in both measurements (-4.38 ± 1.75 vs -2.49 ± 4.31, p = .000; -4.94 ± 1.46 vs -3.11 ± 5.06, p = .000). Lund-Mackay score was significantly lower for the ZAGA group (2.38 ± 3.86 vs 0.56 ± 1.26, p = .042). Statistically significant difference (p = .047) regarding the percentage of patients with no signs or symptoms of rhinosinusitis (Lanza and Kennedy test negative and Lund-Mackay score zero) was observed between groups (54.55% vs 76.25%, p = .047). CONCLUSIONS: Both procedures had similar clinical outcomes with respect to implant survival. The ZAGA concept is able to immediately rehabilitate the severely atrophic maxillae, minimizing the risk of maxillary sinus-associated pathology. Moreover, less bulky, more comfortable, and easy to clean prostheses are achieved.

Se realizó una investigación descriptiva, longitudinal y prospectiva en la Clínica Estomatológica Provincial Docente "Mártires del Moncada" de Santiago de Cuba en el período comprendido desde septiembre de 2010 hasta julio de 2012, a fin de evaluar la evolución de 17 pacientes tratados con implantes roscados marca MIS, con abutmen o atache de bola, cargados inmediatamente, para lo cual se convirtió la misma prótesis que portaban en una sobredentadura mandibular soportada en la mucosa y retenida por el implante, pues su aparato protésico convencional carecía de la funcionalidad deseada. La rehabilitación con ese tipo de sobredentadura fue efectiva en aquellos pacientes con problemas en los principios biomecánicos de retención y estabilidad, así como también en los que presentaban una buena disponibilidad de hueso y en los que se colocaron implantes de mayor longitud, todos los cuales respondieron mejor al tratamiento; por consiguiente, se elevó su calidad de vida al proporcionarles confort y seguridad con el uso de su aparato protésico.


A descriptive longitudinal and prospective study was conducted in "Martires del Moncada" Provincial School of Dentistry of Santiago de Cuba from September 2010 to July 2012 in order to evaluate the progress of 17 patients treated with threaded implants MIS, abutmen or ball attachment, immediately loaded, for which the same prosthesis became a mandibular overdenture supported on the mucosa and retained by the implant, because their conventional prosthetic device lacked the desired functionality. The rehabilitation with this type of overdenture was effective in those patients with compromised biomechanical principles of retention and stability, as well as in those having a good bone availability and in those with greater length implants, thus responding better to treatment. Therefore, their quality of life increased by providing comfort and safety with the use of this prosthetic device.

Una de las opciones de tratamiento para los pacientes que requieren el reemplazo de uno o varios dientes, es el uso de prótesis implantosoportadas como una alternativa de tratamiento. Una mayor demanda tanto estética como funcional por parte de los pacientes hace que se intente reducir el tiempo de carga del implante. Por lo que se han realizado varias modificaciones al protocolo quirúrgico y protésico convencional reduciendo el tiempo de carga al implante. El objetivo de este artículo es el de presentar la colocación de implantes de carga inmediata en la zona estética como una alternativa en el plan de tratamiento, y la importancia de la interdisciplina quirúrgico-protésica para lograr un mejor éxito del tratamiento.


One of the treatment options offered to patients requiring replacement of one or more teeth is the use of prostheses supported by implants. Patients nowadays demand greater aesthetic and functional restorations; therefore, the clinician tries to reduce implant load time. All this leads to the implementation of several modifications to the conventional surgical and prosthetic protocol leading to a reduction in the load time of the implant. The objective of this article is to present placement of immediate load implants in an aesthetic zone as an alternative for the treatment plan, as well as highlighting the importance of observing surgical-prosthetic inter-discipline to achieve greater success in treatment.

PURPOSE: An integrated fixture mount/impression coping/ temporary abutment can provide many advantages for immediate loading of dental implants, such as simpler procedure, less chair time, cost reduction, and comfort for the patients. MATERIALS AND METHODS: A newly designed dental implant fixture mount (DIFMA) can be used as an impression coping for taking an immediate impression. An immediate load provisional prosthesis can then be fabricated shortly after implant placement to immediately load the implants. This fixture mount can also serve as a temporary abutment for immediate chair-side fabrication of provisional prosthesis. Two clinical cases are presented. RESULTS: A clinical case utilizing the fixture mount abutment (DIFMA)/implant assembly is presented. The precision of fitting between the impression copings and implants is secured with this system. The chair time for taking an immediate impression is greatly reduced. Less cost for the restoration is provided and patient comfort is delivered. CONCLUSIONS: More patient satisfaction can be conferred by employing the fixture mount in the process of immediate impression taking and as an immediate provisional abutment.
