RESUMO

In Japan, public health improvements have lessened parasitic infections, but increased global travel has introduced new disease risks. We report the case a Japanese man who had previously resided in Vietnam, who experienced appendicitis and the presence of segments in his stool. Gastrografin led to the expulsion of Taenia saginata, as confirmed by scolex identification. Remarkably, the appendicitis symptoms subsided after deworming. This case underscores the importance of considering cestode infections in the differential diagnosis of appendicitis, particularly for patients with relevant travel histories, and highlights the evolving challenges in global health and the significance of acquiring detailed patient histories.

RESUMO

A Southeast Asian man in his 30s residing in Japan developed severe disseminated strongyloidiasis during long-term steroid therapy. Initially misdiagnosed as gastroenteritis, his condition was later identified as strongyloidiasis by fecal smears and endoscopy. An endoscopic examination revealed salmon roe-like nodular lesions, indicating an advanced stage. Despite treatment, the patient died of the illness. This case underscores the importance of considering the patient's geographical background and immunosuppressive history in medical assessments. It further highlights the need for early and proactive diagnostic approaches, such as stool testing and gastrointestinal endoscopy, to effectively detect and treat imported infectious diseases.

BACKGROUND: Little is known about the trends of imported infectious diseases among travelers to non-endemic countries during the novel coronavirus disease 2019 (COVID-19) pandemic. This article aimed to describe those among travelers to Japan. METHODS: This is a descriptive study based on national surveillance data. Imported infectious disease cases were defined as those with a reported overseas source of infection among 15 diseases pre-selected based on the probability and impact of importation. The number of notified cases from April 2016 to March 2021 were described by disease and time of diagnosis. The relative ratio and absolute difference in case counts-both by number and per arrival-were calculated by disease comparing those from the pandemic period (April 2020-March 2021) to the pre-pandemic period (April 2016-March 2020). RESULTS: A total of 3,524 imported infectious disease cases were diagnosed during the study period, including 3,439 cases before and 85 cases during the pandemic. The proportionate distribution of diseases changed but notification counts of all 15 diseases decreased during the pandemic. Accounting for arrivals, however, seven diseases showed a two-fold or greater increase, with a notable absolute increase per million arrivals for amebiasis (60.1; 95% confidence interval [CI], 41.5-78.7), malaria (21.7; 95% CI, 10.5-33.0), and typhoid fever (9.3; 95% CI, 1.9-16.8). CONCLUSION: The epidemiology of imported infectious diseases changed during the pandemic. While the number of imported infectious disease cases decreased, the number of cases per arrivals increased considerably both in relative and absolute terms for several diseases of public health and clinical importance.

RESUMO

Shanghai is a super-large metropolis with a highly developed globalization. Since the 21st century, Shanghai has experienced several threats of emerging and imported infectious diseases. Infectious disease surveillance has been established and developed from single-disease surveillances to a comprehensive surveillance network. Integration of clinical and preventive medicine has been gradually extended, which facilitates the improvement in the monitoring and early warning system. In 2020, when the COVID-19 epidemic spread, Shanghai quickly established a prevention and control expert team and a clinical medical expert team to effectively and shortly control local COVID-19 epidemic. In order to improve Shanghai’s capacity to respond to major epidemics of infectious diseases and public health emergencies, the metropolis will build and improve a three-level diagnosis and treatment system for emerging, imported, rare, and unknown infectious diseases in the next five years. Based on the big data monitoring platform of medical institutions, Shanghai will achieve the intelligence-supported diagnosis and treatment of infectious diseases, and consolidate the cooperation to implement the integration of clinical and preventive medicine.

Imported infectious diseases are becoming a serious public health threat in China. However, limited information concerning the epidemiologic characteristics of imported infectious diseases is available. In this study, we collected data related to imported infectious diseases in mainland China from the National Information Reporting System of Infectious Diseases and analyzed demographic, temporal, and spatial distributions. The number of types of imported infectious diseases reported increased from 2 in 2005 to 11 in 2016. A total of 31,740 cases of infectious disease were imported to mainland China during 2005-2016; most of them were found in Yunnan Province. The cases were imported mainly from Africa and Asia. As a key and effective measure, pretravel education should be strengthened for all migrant workers and tourists in China, and border screening, cross-border international cooperation, and early warning should be further improved.

RESUMO

We report a case of imported dengue hemorrhagic fever (DHF) in a 10-year-old Philippine boy. The patient was admitted to the hospital with a 4-day history of high fever, headache, and malaise, and a 2-day history of epistaxis and hematemesis. Symptoms deteriorated after admission, and the patient was subsequently diagnosed with DHF. DHF occurs more frequently among cases of reinfection than among cases of primary infection. Therefore, physicians should recognize the difference in the risk of developing DHF between patients in endemic and nonendemic areas.

RESUMO

Melioidosis is an endemic disease in Southeast Asia and northern Australia. An increasing number of cases are being reported in nonendemic countries, making the diagnosis less obvious. We discuss the identification of Burkholderia pseudomallei using matrix-assisted desorption ionization-time of flight mass spectrometry on the occasion of recent cases of imported melioidosis in French travellers.

The health of human, animal and environment is under serious threat from the increasing emerging infectious diseases (EID).Through strengthening the three-level disease prevention network, expanding infectious disease surveillance system and multi-sectoral joint cooperation mechanism, the quick, effective and strong prevention and control system of emerging and imported infectious diseases has been established in Shanghai.Since 2013, through effective control of EIDs such as severe acute respiratory syndrome ( SARS), influenza A H1N1 and H7N9 avian influenza, as well as imported infectious diseases (IID) such as Ebola virus disease, middle east respirators syndrome(MERS), Zika virus disease and yellow fever, the surveillance and response capacity has been improved significantly, and the prevention and control system has been improved gradually.As an international megalopolis under globalization, Shanghai is faced with the challenges as follows: growing pressure to infectious diseases prevention and control, increasingly serious situation of EIDs and IIDs;prevention and control skills need to be improved and the current personnel cannot meet with the demands.In order to meet the challenges, infectious disease monitoring and early warning technology should be strengthened; the sensitivity of infectious disease surveillance and early-warning capacity should be improved; EID symptom complex monitoring system should be established; personnel training should be strengthened, domestic and international cooperation and exchange should be carried out;so as to safeguard public health security and public health in Shanghai.

A 25-year-old Japanese man was admitted with general malaise and fever, which had developed 12 days after coming back to Japan from Indonesia. Blood examination revealed elevated transaminase levels and positivity for the IgM anti-HAV antibody; therefore, he was diagnosed with acute hepatitis A. HAV-RNA was detected in his serum and phylogenetically classified as subgenotype IA. The partial genome in the VP1/P2A region was consistent with the strain recently isolated from Surabaya, which indicated that he had been infected during his stay in Indonesia. Thus, HAV vaccination is recommended before visiting HAV-endemic countries for a long period of time.

Fever in ill travelers returning home from developing nations is common. Most travelers present with undifferentiated febrile syndromes. Regional proportionate morbidity rates and patients' travel histories are essential in narrowing the differential diagnosis. Most patients in whom a diagnosis is confirmed have malaria, dengue fever, enteric fever, or rickettsial disease. Empiric treatment based on the clinical presentation is required in many cases, because acquisition of confirmatory laboratory data is often delayed. The focus of this article is travel-related illness that falls within the spectrum of the acute febrile syndrome.
