Studies indicate that ultra-processed food (UP) consumption correlates negatively with essential vitamin and mineral intake and positively with sodium and lipid intake. The objective of this study was to explore the relationship between UP consumption and deviations from nutritional guidelines. An observational, cross-sectional analytical study was conducted on a probability sample of manufacturing workers in the state of Rio Grande do Norte, Brazil. Food consumption was assessed with a 24 h recall survey, and nutrient intake inadequacies were calculated as the difference between individuals' intake of energy, macronutrients, minerals and vitamins, and the dietary reference intakes for individuals of the same sex and age group, and then analyzed for trends across the percentage contribution of UP to total energy intake with nonparametric multiple regression adjusted for covariates. The study included 921 workers from 33 industries, 55.9% male, with a mean age of 32 years. Overall, the study population exhibited deficits in energy, all macronutrients, and in some micronutrients. With increasing UP contribution to total energy intake, there is a trend towards a greater intake of energy (p < 0.001), total, saturated, monounsaturated, and trans fats (p < 0.001), n6-polyunsaturated fatty acids (p = 0.03), carbohydrates (p < 0.001), calcium (p = 0.008), and manganese (p < 0.001), thiamin (p < 0.001), and vitamin B6 (p = 0.01); however, this comes with a negative consequence in terms of reducing the protein consumption (p = 0.037), fiber (p = 0.035), copper (p = 0.033), and vitamin E (p = 0.002) intake. The results show that correcting energy and micronutrient deficiencies by increasing UP consumption can also lead to a decrease in diet quality.

Background: Diet is a significant contributor to health and wellbeing of individuals. Aim: In this study we investigated patterns of dietary intakes, levels of nutrient inadequacies and associated sociodemographic, anthropometric and lifestyle factors among adults in Trinidad and Tobago. Method: The study was cross-sectional in nature. A convenience sample of 11783 persons from districts throughout Trinidad and Tobago completed a self-administered questionnaire comprising socio-demographic and lifestyle items. Anthropometry was self-reported with 15% of participants having measurements done according to recommended procedures. Dietary patterns were determined by principal component analysis (PCA) while nutrient intakes and adequacy were assessed using the NutriGenie 7.0 software and nutrient adequacy ratio (MAR) respectively. Foods were categorised as unprocessed/minimally processed and processed/ultra-processed. The University of The West Indies Ethics Committee approved the study. Results: Approximately 72.5% of participants met the Goldberg criteria for plausible reporting. The three predominant dietary patterns 'Typical', 'Fruits and Vegetables', and 'High Fat' explained 45% of the total variance in foods consumed. Processed/ultra-processed foods accounted for most of the energy (80%) and nutrients consumed. Nutrient inadequacies were observed for potassium, vitamins B12, D, E, K, fibre, magnesium; and iron among females. The mean adequacy ratio (MAR) for participants was 67%. MAR was positively associated with predominant dietary patterns independent of socioe demographic and lifestyle factors (p < 0.001). Conclusion: Irrespective of their nature, the predominant dietary pattern was associated with nutrient adequacy among participants. Reducing the risk of inadequate nutrient intakes may be addressed by increasing availability, access and consumption of appropriate sources of these micronutrients.

Background: The Young Lives longitudinal study switched to remote data collection methods including the adaptation of dietary intake assessment to online modes due to the physical contact restrictions imposed by the COVID-19 pandemic. This study aimed to describe the adaptation process and validation of an online quantitative food frequency questionnaire (FFQ) for Peruvian young adults. Methods: A previously validated face-to-face FFQ for the adult Peruvian population was adapted to be administered through an online self-administered questionnaire using a multi-stage process. Questionnaire development was informed by experts' opinions and pilot surveys. FFQ validity was assessed by estimating misreporting of energy intake (EI) using the McCrory method, and the FFQ reliability with Cronbach alpha. Logistic regressions were used to examine associations of misreporting with sociodemographic, body mass index (BMI), and physical activity covariates. Results: The FFQ was completed by 426 Peruvian young adults from urban and rural areas, among whom 31% were classified as misreporters, with most of them (16.2%) overreporting daily EI. Men had a lower risk of under-reporting and a higher risk of over-reporting (OR = 0.28 and 1.89). Participants without a higher education degree had a lower risk of under-reporting and a higher risk of over-reporting (OR = 2.18 and 0.36, respectively). No major difference in misreporting was found across age groups, areas, studying as the main activity, being physically active or sedentary, or BMI. Results showed good internal reliability for the overall FFQ (Cronbach alpha = 0.82). Conclusion: Misreporting of EI was mostly explained by education level and sex across participants. Other sociodemographic characteristics, physical activity, sedentary behavior, and BMI did not explain the differences in EI misreporting. The adapted online FFQ proved to be reliable and valid for assessing dietary intakes among Peruvian young adults during the COVID pandemic. Further studies should aim at using and validating innovative dietary intake data collection methods, such as those described, for informing public health policies targeting malnutrition in different contexts after the COVID-19 pandemic.

OBJECTIVE: Understanding the socio-economic inequalities in dietary intake is crucial when addressing the socio-economic gradient in obesity rates and non-communicable diseases. We aimed to systematically assess the association between socio-economic position (SEP) and dietary intake in Chile. DESIGN: We searched for peer-reviewed and grey literature from inception until 31 December 2019 in PubMed, Scopus, PsycINFO, Web of Sciences and LILACS databases. Observational studies published in English and Spanish, reporting the comparison of at least one dietary factor between at least two groups of different SEP in the general Chilean population, were selected. Two researchers independently conducted data searches, screening and extraction and assessed study quality using an adaptation of the Newcastle Ottawa Quality Assessment Scale. RESULTS: Twenty-one articles (from eighteen studies) were included. Study quality was considered low, medium and high for 24, 52 and 24 % of articles, respectively. Moderate-to-large associations indicated lower intake of fruit and vegetables, dairy products and fish/seafood and higher pulses consumption among adults of lower SEP. Variable evidence of association was found for energy intake and macronutrients, in both children and adults. CONCLUSIONS: Our findings highlight some socio-economic inequalities in diets in Chile, evidencing an overall less healthy food consumption among the lower SEP groups. New policies to reduce these inequalities should tackle the unequal distribution of factors affecting healthy eating among the lower SEP groups. These findings also provide important insights for developing strategies to reduce dietary inequalities in Chile and other countries that have undergone similar nutritional transitions.

BACKGROUND: Little is known regarding the associations between maternal factors and B-vitamin and choline concentrations in early milk and the trajectories of these vitamins during lactation. OBJECTIVES: In this hypothesis-generating study, we modeled the association between maternal and offspring factors and longitudinal changes in milk B-vitamin and choline concentrations throughout lactation. METHODS: A hundred women were studied in a prospective birth cohort and milk samples from 52 women were collected at 2-8 d, 76 women at 28-50 d, and 42 women at 88-119 d postpartum. Maternal dietary intake during pregnancy and lactation was assessed by an FFQ. Linear mixed-effects models with interaction terms were used to evaluate changes in milk B-vitamin and choline concentrations over time based on maternal factors and the early postpartum concentrations of these micronutrients. RESULTS: The women with higher early postpartum milk concentrations of niacin (ßinteraction = -0.02; SE = 0.00; P < 0.001), pantothenic acid (ßinteraction = -0.10; SE = 2.56; P < 0.001), vitamin B-12 (ßinteraction= -0.10; SE = 0.03; P < 0.001), and choline (ßinteraction= -0.90; SE = 0.18; P < 0.001) exhibited a decrease in their concentrations throughout lactation. The participants with overweight and obesity prepregnancy experienced an increase in milk vitamin B-12 concentrations over time (ßinteraction = 0.04; SE = 0.02; P = 0.06). In contrast, a decrease in vitamin B-12 concentration was observed among women with vitamin B-12 intake below the RDA during pregnancy (ßinteraction= -0.08; SE = 0.05; P = 0.07). The women with niacin intake below the RDA during lactation experienced an increase in milk concentrations over time (ßinteraction = 0.01; SE = 0.01; P = 0.03). A gestational age at birth >40 wk was associated with an increase in milk choline concentration throughout lactation (ßinteraction = 0.54; SE = 0.16; P< 0.01). CONCLUSIONS: Changes in B-vitamin and choline concentrations in human milk over time may be associated with the early concentrations of these micronutrients in milk, maternal prepregnancy BMI, dietary intake, and gestational age at delivery.

Urinary metabolites of organophosphate (OP) and pyrethroid (PYR) pesticides and biomarkers of effects were studied in a population (n=40) residing in an important agricultural area of the province of Córdoba (Argentina). Detection frequencies (DF) higher than 85% were observed for the metabolites of pirimiphos (2-diethylamino-6-methylpyrimidin-4-ol -DEAMPY-, median 7.5 µg/g creatinine, DF: 100%), parathion (p-nitrophenol, 0.99 µg/g creatinine, 100%), and chlorpyrifos (3,5,6-trichloro-2-pyridinol, 0.25 µg/g creatinine, 85%). The DEAMPY concentrations doubled the levels found in other studies and were negatively associated with Er-AChE activity, suggesting the appearance of health effects already in environmental exposure levels below established acceptable daily intakes (ADIs). 3-Phenoxybenzoic acid, the metabolite of several PYR pesticides, was also found in all samples. This metabolite was also significantly negatively correlated with Er-AChE, indicating effects of pyrethroid pesticides on the acetylcholine system even at concentrations below the ADI.

OBJECTIVE: To calculate the amount of bioavailable iron consumed among 6- to 12- month-old infants considering differences in iron bioavailability among dietary iron sources and to compare this with current recommended intakes. STUDY DESIGN: We used the Feeding Infants and Toddlers Study database of dietary intakes from 2016 and the estimated bioavailability of dietary iron sources to evaluate the proportion of infants whose calculated total daily absorbed iron fell below physiologic requirements, that is, the recommended amount needed to fully support growth and erythropoiesis. RESULTS: The calculated daily iron absorption was below the recommended amount in 54.3% of infants evaluated ranging from 19.5% of 448 exclusively formula-fed infants, to 95.8% of 296 exclusively breastfed infants and 72.2% of 102 mixed fed infants. The calculated mean iron absorption of 6- to 9- month-old breastfed infants was 0.27 mg/day, far less than the estimated physiologic requirement of 0.69 mg/day. The most highly bioavailable iron, heme iron, was <12% of the contributor to total iron absorbed in breastfed infants. CONCLUSIONS: These data indicate a need for further education and public health policies to support increased iron intake in 6- to 12- month-old infants, emphasizing those receiving any breast milk. Exclusively formula-fed infants are at lower risk, but rates of low absorbed iron indicate that all infants may need monitoring for clinical evidence of low iron status. Consideration should be given to increasing the proportion of heme iron obtained from animal products in the diet where feasible.

"Chiniy-tref" (CT) is a traditional preparation used in folk medicine in Martinique Island (French West Indies) that is nowadays mainly taken orally to prevent or act against any "manifestation of evil". CT is easily prepared at home by macerating larvae of the endemic swallowtail Battus polydamas (ssp.) cebriones (Dalman, 1823), sometimes accompanied by a leaf of its host-plant Aristolochia trilobata L., in commercial rum. We have previously reported the detection of nephrotoxic and carcinogenic aristolochic acids (AAs) I and II in CT, leading the Regional Health Agency (ARS) of Martinique to issue an alert regarding the potential risks associated with its consumption in 2015. In order to complete the toxicity risk assessment for oral consumption of CT, a full qualitative analysis of AAs and their analogues (AAAs) was performed, as well as a quantitative determination of the major AAs, namely AAs I and II. The phytochemical profiling of AAAs present in CT, that also corresponds to that of B. polydamas cebriones larvae feeding on A. trilobata, has been established for the first time by ultra-high performance liquid chromatography/electrospray ionization quadrupole time of-flight tandem mass spectrometry. AAs I and II were quantified in a small panel of tinctures by using a validated UHPLC/UV method, allowing us to estimate the probable daily intakes of these toxins by CT consumers. The results proved the existence of a real risk of renal toxicity and carcinogenicity associated with the chronic oral consumption of CT in Martinique, and more generally of similar "snake bottles" throughout the Caribbean.

OBJECTIVE: To estimate usual diets among Brazilian children regarding the consumption of school meals and social vulnerability risks. DESIGN: A cross-sectional study. School meal consumers were considered those children who reported consuming school meals ≥3 times/week. Social vulnerability risk was classified by an index. Dietary intake was evaluated by one 24 h dietary recall for the whole sample; a second 24 h dietary recall was administered in a sub-sample (38·6 %). The National Cancer Institute's method was used to estimate children's usual intake of nutrients and food groups. SETTING: Municipal public schools from Belo Horizonte, Brazil. PARTICIPANTS: Children (n 1357) aged 8-12 years. RESULTS: Half of the sample lived in low/medium social vulnerability risk areas and 27·9 % were school meal non-consumers. School meal consumers more frequently lived in high/very high social vulnerability risk areas (76·2 v. 68·7 %). Children with low/medium social vulnerability risk had a higher mean intake of thiamin (1·13 v. 1·04 mg) and a lower mean intake of candy (1·35 v. 1·42 g). Consumption of school meals among children under high/very high social vulnerability risk was associated with higher mean consumption of vitamin C (31·9 v. 24·1 mg), unprocessed/minimally processed foods (956·3 v. 851·9 g), fruits (128·5 v. 90·9 g) and vegetables (58·2 v. 47·1 g). Ultra-processed food product consumption was lower among school meal consumers (136·2 v. 187·7 g), especially ultra-processed beverages (252·5 v. 305·7 g). CONCLUSIONS: Consuming school meals was associated with a better usual diet quality, particularly among those with higher social vulnerability risk.

Chronic exposure to inorganic arsenic (As) is associated with numerous adverse effects. Argentina is one of the countries affected by arsenicism; however, there are few studies that evaluate inorganic As exposure and its effects on child population. The aim of this study is to evaluate exposure to As through water and food in child populations living in the provinces of Santiago del Estero and Chaco (n = 101), and to determine the impact of this exposure analysing biomarkers of exposure (urine and hair As contents) and effect [8-oxo-7,8-dihydro-2'-deoxyguanosine (8-OHdG)]. The populations selected live in three areas with different levels of As in the drinking water (Santa Teresa de Carballo, 0.925 mg/L; Taco Pozo, 0.210 mg/L; Jumi Pozo, 0.016 mg/L). The As intakes through water and food are especially high in the areas with the greatest As exposure (Santa Teresa de Carballo, 1575 ±â€¯8 µg/day; Taco Pozo, 386 ±â€¯8 µg/day; Jumi Pozo, 39 ±â€¯1 µg/day). The total As contents in most of the samples of hair (0.11-13.11 mg/kg) and urine (31-4258 µg/g creatinine) are higher than the reference values (hair: 1 mg/kg; urine: 50 µg/g creatinine). The increase in the level of As exposure alters the profile of metabolites in urine, with a decrease of dimethylarsinic acid (10%) and an increase in the percentages of monomethylarsonic acid (4%) and inorganic As (6%). The results also show high values of 8-OHdG (3.7-37.8 µg/g creatinine), a oxidative DNA damage marker, in the two areas with greater As exposure.

This study assessed dietary intakes, nutritional composition, and identified commonly eaten foods among Jamaicans in Florida. Dietary intake was assessed among 44 study participants to determine commonly eaten foods and nutrient composition. Weighed recipes were collected and analyzed to determine nutrient composition for traditional foods. Top foods that contributed to macronutrient and micronutrient intake were identified and adherence to dietary recommendations was evaluated. Mean daily energy intake was 2879 (SD 1179) kcal and 2242 (SD 1236) kcal for men and women respectively. Mean macronutrient intakes were above dietary recommendations for men and women. Top foods contributing to energy included rice and peas, sweetened juices, chicken, red peas soup, and hot chocolate drink. Results showed sodium intake was more than double the adequate intake estimate (1300-1500 mg). Findings highlight the need to include commonly eaten traditional foods in dietary questionnaires to accurately assess diet-related chronic disease risk. Findings have implications for risk factor intervention and prevention efforts among Jamaicans.

Several descriptive studies on the intake of polyphenols, mostly flavonoids, have been published, especially in Europe and the USA, but insufficient data are still available in Latin-American countries, where different types of foods are consumed and different dietary habits are observed. The goal of this cross-sectional study was to estimate dietary intakes of polyphenols, including grand total, total per classes and subclasses and individual compounds, and to identify their main food sources in Mexican women. The Mexican Teachers' Cohort includes 115 315 female teachers, 25 years and older, from twelve states of Mexico, including urban and rural areas. Dietary data were collected in the period 2008-2011 using a validated FFQ, and individual polyphenol intake was estimated using food composition data from the Phenol-Explorer database. Median total polyphenol intake was the highest in Baja California (750 mg/d) and the lowest in Yucatan (536 mg/d). The main polyphenols consumed were phenolic acids (56·3-68·5 % total polyphenols), followed by flavonoids (28·8-40·9 %). Intake of other polyphenol subclasses (stilbenes, lignans and others) was insignificant. Coffee and fruits were the most important food sources of phenolic acids and flavonoids, respectively. Intake of a total of 287 different individual polyphenols could be estimated, of which forty-two were consumed in an amount ≥1 mg/d. The most largely consumed polyphenols were several caffeoylquinic acids (ranging from 20 and 460 mg/d), ferulic acid, hesperidin and proanthocyanidins. This study shows a large heterogeneity in intakes of individual polyphenols among Mexican women, but a moderate heterogeneity across Mexican states. Main food sources were also similar in the different states.

Resumen Desde la infancia, las prácticas alimentarias parentales (PAP) influyen en el aprendizaje del comer saludablemente. Sin embargo, la terminología inconsistente y la falta de descripciones claras de las PAP obstaculizan la comprensión de su influencia en dicho aprendizaje. A partir de una revisión selectiva de artículos científicos que incluyeron descripciones de las PAP y su relación con algún aspecto de la conducta alimentaria infantil (CAI), se buscó analizar las descripciones conductuales planteadas en la literatura durante la última década (2006-2016), con el propósito de clarificar las relaciones documentadas entre éstas y la CAI. Los resultados fueron categorizados a partir de las relaciones planteadas entre las PAP y algunos de los retos principales que el tópico de la CAI, como son: la aceptación de alimentos nuevos, el desarrollo de preferencias alimentarias y la autorregulación alimentaria. Los resultados reflejan algunas PAP asociadas tanto a conductas alimentarias deseables como indeseables para la salud de los niños. La confusión en la terminología y la falta de consistencia en las descripciones conductuales de las PAP, así como las explicaciones sobre los mecanismos por los cuales tales prácticas influyen en la CAI son todavía desconocidas. Se proponen algunas consideraciones a retomar en investigaciones futuras.


Abstract Parental feeding practices influence the learning of healthy eating since childhood. But the lack of clear descriptions and inconsistent terminology of such practices hampers the understanding of their influence on such learning. From a selected review of scientific articles that included descriptions of parental feeding practices and relationships with any aspect of children's eating behavior, we analyzed the behavioral descriptions stated in literature during the last decade (2006-2016) with the purpose to clarify relations among these and the children's eating behavior. The results were categorized based on the relationships between feeding practices and the challenges in infant feeding behavior such as acceptance of new foods, development of food preferences, and food self-regulation. The results reflect some practices associated with both desirable and undesirable children's eating behavior. Confusion in terminology and lack of consistency in behavioral descriptions of parental feeding practices and explanations of the mechanisms of such practices are still unknown. Some considerations are proposed for future research.

RESUMEN Cuantificar los niveles de selenio en alimentos cocinados y consumidos en una zona endémica de arsénico y calcular las ingestas diarias para estudiar si contribuyen a disminuir la acción tóxica del arsénico. Se muestrearon n= 38 alimentos consumidos en comedores escolares y en una casa de familia de Taco Pozo. La cuantifícación se realizó con Flow Injection Hydride Generation Atomic Absorption Spectrometry (FI-HG-AAS). Para el cálculo de ingesta se consideró los pesos de las porciones de alimentos consumidos y el peso corporal de referencia para los individuos (adultos y menores) según la edad. Las concentraciones promedios de selenio en los desayunos/meriendas oscilan entre 32,2 a 69,1 ng/g peso húmedo (ph), los almuerzos/cenas entre 14,0 a 67,6 ng/g ph. La ingesta diaria total se encuentra entre 13,0 a 67,4 μg/día. Los desayunos presentaron mayores concentraciones que los almuerzos/cenas. Las ingestas diarias resultaron inferiores a la dosis diaria recomendada a excepción de uno de los puntos muestreados. Se estima que estas dietas no mitigarían los efectos tóxicos del arsénico, aumentando la posibilidad de apariciones de patologías asociadas a éste.


ABSTRACT Quantify selenium levels in cooked and consumed foods in an arsenic endemic area and calculate the daily intakes to study if they contribute to diminish the toxic action of arsenic. We sampled n= 38 foods consumed in school canteens and in a family home in Taco Pozo. Quantification was performed with Flow Injection Hydride Generation Atomic Absorption Spectrometry (FI-HG-AAS). The weights of the portions of food consumed and the reference body weight for individuals (adults and minors) according to age were considered for the calculation of intake. We quantified selenium levels in foods cooked and consumed in an arsenic-endemic area and calculated daily intake to study if consumption contributed to a decrease in the toxic activity of arsenic. Thirty-eight foods consumed in school cafeterias and in a family home in Taco Pozo were sampled. Quantification was performed using Flow Injection Hydride Generation Atomic Absorption Spectrometry (FI-HG-AAS). For consumption calculation, we considered portion weight of each food and the body weight of the individuals studied. The average selenium concentration in breakfast / snacks ranged from 32.2 to 69.1 ng/g wet weight (ww) and between 14.0 and 67.6 ng/g ww for lunches/dinners. The total daily intake was between 13.0 and 67.4 μg/day1. The daily intakes were lower than the recommended daily dose except for one of the points sampled. These diets do not help to mitigate the toxic effects of arsenic, thus increasing the possibility of occurrences of associated pathologies.

This research assessed the proportion of whole-wheat flour in whole-wheat bread relative to the level mandated by law, as well as the content of lipids (Lip) and sodium (Na) in French bread, and of Lip, Na and total dietary fiber (TDF) in whole-wheat bread made in bakeries in Montevideo and Canelones, Uruguay. The percentage ofNa and TDF using both Adequate Intakes (Als) and Tolerable Upper Intake levels (Uls) were estimated in reference to recommended daily quantities. Na in French bread consists of 64% of AIs for individuals aged nine and older, and over 100% of AIs for adults aged 51 and older. Whole-wheat bread values are slightly lower. TDF consists of 38% of AIs in 9 year-old children and 53% in adults older than 50 years of age. This work helps to establish a baseline for the development of formulations with minimum levels of Na and optimum levels of TDF in artisan bread.


La presente investigación evaluó la proporción de harina integral en pan integral con el nivel legislado; el contenido de lípidos y sodio (Na) en pan francés y lípidos, Na y fibra dietética total (FDT) en pan integral elaborado en panaderías de Montevideo y Canelones. Se estimó según cantidades diarias recomendadas el porcentaje de Na y FDT en relación a las Ingestas Adecuadas (AIs) y los Límites Máximos Tolerables (Uls). El Na en pan francés cubre desde los 9 años un 64% de las IAs hasta cerca del 100% en adultos desde los 51 años; el pan integral alcanza valores algo inferiores. La FDT cubre en niños de 9 años el 38% de las IAs y 53% en adultos mayores de 50 años. Este trabajo contribuye a disponer de una línea de base para el desarrollo de formulaciones con mínimos niveles de Na y óptimos de FDT en panes artesanales.

The number of days of data required to accurately estimate usual nutrient intake of children is not well established. This study aims to calculate the variability and the number of days required to estimate usual nutrient intake in children aged 13-32 months. This cross-sectional study, which is part of the BRISA Project in São Luís, Maranhão, Brazil, involved 231 children from April 2011 to January 2013. Socio-economic and demographic data were collected using a questionnaire, and 3 non-consecutive days of food consumption were collected using a 24-h dietary recall (24HDR) survey. Intrapersonal and interpersonal variability and variance ratio (VR) were obtained for each nutrient using the Multiple Source Method® program (version 1.0.1). The number of days (d) needed was calculated using the formula proposed by Black et al. for different correlation coefficients (r) (i.e. 0·7, 0·8 or 0·9). For the vast majority of nutrients, intrapersonal and interpersonal variability values of <1 were observed, with even smaller intrapersonal variabilities, resulting in low VR (<1). More days were needed to estimate intakes of soluble fibre (12), insoluble fibre (11), total fibre (10), vitamin C (9) and PUFA (7), while fewer days were needed for energy, carbohydrate, SFA, Ca, Fe, P and Zn (all had 2 d for r 0·9). However, most nutrients required one, two or three 24HDR for r 0·7, 0·8 or 0·9.

BACKGROUND: Breast milk is the recommended source of nutrients for infant growth, but its adequacy to meet infants' mineral and trace element needs is unknown. OBJECTIVES: We used breast-milk mineral and trace element concentrations of Guatemalan mothers at 3 lactation stages to estimate total daily intakes and to determine whether intakes were associated with early infant growth. METHODS: In this cross-sectional study, breast-milk samples were collected from Mam-Mayan mothers during transitional (5-17 d, n = 56), early (18-46 d, n = 75), and established (4-6 mo, n = 103) lactation; z scores for weight (WAZ), length (LAZ), and head circumference (HCAZ) were measured. Concentrations of 11 minerals (calcium, potassium, magnesium, sodium, copper, iron, manganese, rubidium, selenium, strontium, and zinc) were analyzed by inductively coupled plasma-mass spectrometry (ICP-MS). WHO equations were used to calculate the estimated energy requirement, which was divided by the energy density of breast milk to estimate daily milk volume, and this number was multiplied by breast-milk mineral concentrations to estimate intakes. Principal component analyses identified clusters of minerals; principal components (PCs) were used in regression analyses for anthropometric outcomes. RESULTS: Estimated breast-milk intakes during established lactation were insufficient to compensate for the lower milk sodium, copper, manganese, and zinc concentrations in male infants and the lower sodium, iron and manganese concentrations in female infants. Estimated intakes of calcium, magnesium, potassium, sodium, and selenium were below the Institute of Medicine Adequate Intake for both sexes at all 3 stages of lactation. In early lactation, multiple linear regressions showed that PC1 (calcium, magnesium, potassium, rubidium, and strontium intakes) was positively associated with WAZ, LAZ, and HCAZ. In established lactation, the same PC with sodium added was positively associated with all 3 anthropometric outcomes; a second PC (PC2: zinc, copper, and selenium intakes) was associated with WAZ and LAZ but not HCAZ. CONCLUSIONS: Breast milk may be inadequate in selected minerals and trace elements where higher estimated intakes were associated with greater infant growth.

There are statistical methods that remove the within-person random error and estimate the usual intake when there is a second 24-h recall (24HR) for at least a subsample of the study population. We aimed to compare the distribution of usual food intake estimated by statistical models with the distribution of observed usual intake. A total of 302 individuals from Rio de Janeiro (Brazil) answered twenty, non-consecutive 24HR; the average length of follow-up was 3 months. The usual food intake was considered as the average of the 20 collection days of food intake. Using data sets with a pair of 2 collection days, usual percentiles of intake of the selected foods using two methods were estimated (National Cancer Institute (NCI) method and Multiple Source Method (MSM)). These estimates were compared with the percentiles of the observed usual intake. Selected foods comprised a range of parameter distributions: skewness, percentage of zero intakes and within- and between-person intakes. Both methods performed well but failed in some situations. In most cases, NCI and MSM produced similar percentiles between each other and values very close to the true intake, and they better represented the usual intake compared with 2-d mean. The smallest precision was observed in the upper tail of the distribution. In spite of the underestimation and overestimation of percentiles of intake, from a public health standpoint, these biases appear not to be of major concern.

BACKGROUND: A National Health and Nutrition Survey (ENSANUT) conducted in Mexico in 1999 identified a high prevalence of inadequate mineral intakes in the population by using 24-h recall questionnaires. However, the 1999 survey did not adjust for within-person variance. The 2012 ENSANUT implemented a more up-to-date 24-h recall methodology to estimate usual intake distributions and prevalence of inadequate intakes. OBJECTIVE: We examined the distribution of usual intakes and prevalences of inadequate intakes of calcium, iron, magnesium, and zinc in the Mexican population in groups defined according to sex, rural or urban area, geographic region of residence, and socioeconomic status (SES). METHODS: We used dietary intake data obtained through the 24-h recall automated multiple-pass method for 10,886 subjects as part of ENSANUT 2012. A second measurement on a nonconsecutive day was obtained for 9% of the sample. Distributions of usual intakes of the 4 minerals were obtained by using the Iowa State University method, and the prevalence of inadequacy was estimated by using the Institute of Medicine's Estimated Average Requirement cutoff. RESULTS: Calcium inadequacy was 25.6% in children aged 1-4 y and 54.5-88.1% in subjects >5 y old. More than 45% of subjects >5 y old had an inadequate intake of iron. Less than 5% of children aged <12 y and 25-35% of subjects aged >12 y had inadequate intakes of magnesium, whereas zinc inadequacy ranged from <10% in children aged <12 y to 21.6% in men aged ≥20 y. Few differences were found between rural and urban areas, regions, and tertiles of SES. CONCLUSIONS: Intakes of calcium, iron, magnesium, and zinc are inadequate in the Mexican population, especially among adolescents and adults. These results suggest a public health concern that must be addressed.
