RESUMO

Objective To study the relationship of the levels of serum interleukin-6 (IL-6) and C-reactive protein (CRP) with insulin resistance in normal glucose tolerant first-degree relatives (NGT) of type 2 diabetics.Methods A total of 218 normal glucose tolerance subjects , including 136 NGT of type 2 diabetes , and 82 normal control ( NC) without type 2 diabetes family history were enrolled.The levels of blood glucose, hemoglobin A1c (HbA1c), lipid, insulin, IL-6, and high-sensitivity C-reactive protein (hs-CRP) were measured.Results ⑴Compared with NC, the levels of homeostasis model assessment of insulin resistance (HOMA-IR), hs-CRP , and IL-6 in NGT were significantly increased [1.03~1.94 vs 0.42~0.95 , P <0.01;0.46~2.78 mg/L vs 0.39~1.56 mg/L, P <0.01;(13.6 ±8.99) pg/ml vs (8.08 ±3.67) pg/ml, P <0.01];⑵In NGT, hs-CRP was positively correlated with HOMA-IR and IL-6 ( r =0.326 , P <0.01;r =0.258 , P <0.05 );IL-6 was positively correlated with HOMA-IR ( r =0.350 , P <0.01).Conclusions IL-6 and CRP may correlate with the degree of insulin resistance in NGT of type 2 diabetics.

We sought to determine whether preoperative statin treatment is more effective in reducing, after cardiac surgery with cardiopulmonary bypass, systemic inflammatory response and myocardial damage markers in patients who have elevated preoperative interleukin-6 levels than in patients who have normal preoperative interleukin-6 levels. The study involved a prospective cohort of 164 patients who underwent coronary and valvular surgery with cardiopulmonary bypass. There were 2 study groups: group A (n = 60), patients with elevated preoperative interleukin-6 levels; and group B (n = 104), patients with normal preoperative interleukin-6 levels. Each group was subdivided according to whether patients were (group 1) or were not (group 2) treated preoperatively with statins. Accordingly, the subdivided study groups were A1 (n = 40), A2 (n = 20), B1 (n = 56), and B2 (n = 48). The plasma levels of proinflammatory interleukin-6 were measured 1, 6, 24, and >72 hours after surgery. The baseline, operative, and postoperative morbidity and mortality characteristics were similar in all groups. Group A1 had significantly lower levels of interleukin-6 and troponin I than did group A2 at all postoperative time points. Group B1 had significantly lower levels of interleukin-6 than did group B2 postoperatively. There were no significant differences in troponin I levels between groups B1 and B2. We conclude that, in patients with preoperative activation of the inflammatory system, preoperative treatment with statins is associated with lower postoperative interleukin-6 and troponin I levels after cardiac surgery with cardiopulmonary bypass.

Objective To explore the levels of IL-1β,IL-6 and tumor necrosis factor α (TNF α) in serum and cerebrospinal fluid of patients with spontaneous subarachnoid hemorrhage and their relationship with systemic inflammatory response syndrome (SIRS) and multiple organ dysfunction syndrome (MODS).Methods The levels of interleukin-1β(IL-1β),interleukin-6(IL-6),and TNF α in serum and cerebrospinal fluid of patients with spontaneous subarachnoid hemorrhage were measured with enzyme-linked immunosorbent assay (ELISA).Results The levels of IL-1β,IL-6,and TNFα in serum and cerebrospinal fluid of patients with spontaneous subarachnoid hemorrhage were significantly higher than those of control group (P < 0.05),but the increased time of these cytokines was different.Three cytokines in serum and the cerebrospinal fluid levels of IL-1β and IL-6 but not TNFα were significantly related to SIRS and MODS.Condusions The increased cytokine levels in serum and cerebrospinal fluid of patients with spontaneous subarachnoid hemorrhage may be related to SIRS and MODS,and the measurement of IL-1β,IL-6,and TNFαin serum,and IL-1β and IL-6 in cerebrospinal fluid of patients with spontaneous subarachnoid hemorrhage can be useful to predict and treat SIRS and MODS.

Objective To investigate serum galectin-3 levels in patients with chronic heart failure and evaluate its clinical significance for heart failure.Methods A total of 108 chronic heart failure patients were selected.The selected patients were divided into three groups according to the classification of New York Heart Association (NYHA,from degree Ⅱ to Ⅳ).In addition,30 healthy persons were chosen as control group.All the patients received cardiac ultrasound examination.Plasma levels of N-terminal probrain natriuretic peptide (NT-proBNP),serum levels of galectin-3 and interleukin-6 (IL-6) were measured and analyzed.Results Galectin-3 levels were significantly higher in chronic heart failure patients compared with control group(P < 0.05).In chronic heart failure group,the increase of serum concentration of galectin-3 was correlated with the degree of NYHA function classification.Correlation analysis showed that blood level of galectin 3 was strongly positive correlation with concentration of NT-proBNP and IL-6.There was no correlation between galectin-3 with the cause of heart failure,left ventricular diameter(LVD),and left ventricular ejection fraction (LVEF).Conclusions The levels of galectin-3 was significantly increased in chronic heart failure,and was correlated with degree of heart failure.Galectin-3 might be a new biomarker of heart failure and could provide additive value to NT-proBNP levels in the diagnosis of heart failure.

ObjectiveTo investigate predictive value of matrix metalloproteinase-9 (MMP-9) and Interleukin-6 (IL-6) combined with cervical length in premature delivery.MethodsPlasma samples of 62 pregnant women were detected in this study.Samples were divided into three groups based on pregnancy outcomes.MMP-9 and IL-6 values were determined by ELISA.Cervical length was measured by transvagihal sonography.ResultsThe level of MMP-9 of the pregnant women in the premature delivery group was significantly higher than that of continuous pregnant group and that of normal pregnant women [(47.24 ±6.41) ng/ml,(523.24 ± 62.35) pg/ml vs (20.39 ±7.23 )ng/ml,(219.29 ± 80.23)pg/ml,(32.18 ±5.16)ng/ml,(406.18 ±71.52) pg/ml,P <0.01].The level of MMP-9 and IL-6 of the pregnant women in the continuous pregnant group was significantly higher than that of normal pregnant women [(32.18 ±5.16)ng/ml,(406.18 ±71.52) pg/ml vs (20.39 ±7.32) ng/ml,(219.29 ± 80.23) pg/ml,P <0.01].Cervical length of the pregnant women in the premature delivery group was significantly shorter than that of normal pregnant women and that of continuous pregnant group [( 15.62 ± 2.31 )mm vs ( 31.05 ± 1.48 )mm,(26.14 ±3.12)mm,P <0.01].ConclusionsDetection of level of MMP-9 and IL-6 combined with measurement of cervical length can be used as predicting index of premature delivery.

Objective To investigate the systemic stress responses of laparoscopic cholecystectomy(LC). Methods Seventy patients with cholecystolithiasis were randomly divided into two groups: LC and open cholecystectomy(OC). Peripheral blood samples were obtained during the preoperative day and 1, 3, 5, 7 day after operation. Serum cytokines levels (TNF-α, IL-1β and IL-6) and C-reactive protein(CRP) were measured. Results Changes of TNF-α and IL-1β before or after surgery in both groups showed no significant difference(P>0.05). Serum concentrations of IL-6 and CRP after surgery in both groups were significantly higher than those before surgery(P<0.05); but there were no significant difference between the two groups(P>0.05). Conclusions The systemic stress responses of LC have no significant difference comparing with OC.

Objective To observe the effect of Tongfu Xiere Recipe (TXP) combined with routine treatment on cytokine level in patients with acute lung injury due to surgery disease of chest. Methods Fifty eight patients with acute lung injury due to surgery disease of chest were randomized into two groups. The two groups received routine treatment subjecting to the Guideline (2006) for Diagnosis and Treatment of Acute Lung Injury/Acute Respiratory Distress Syndrome. The treatment group additionally received TXP (mainly composed of Radix et Rhizoma Rhei, Natrii Sulfas, Fructus Gardeniae, Caulis Lonicerae, Herba Taraxaci, Semen Raphani, Semen Lepidii seu Descurainiae, Rhizoma Polygoni Cuspidati, Radix Paeoniae Rubra, Fructus Aurantii Immaturus, and Cortex Magnoliae Officinalis) for 5 days or till oxygenation index recovered to the normal. The therapeutic effect was evaluated after treatment, and the changes of blood tumor necrosis factor ? (TNF-?), interleukin 6 (IL-6) and interleukin 8 (IL-8) levels were observed before and after treatment. Results The serum levels of TNF-?, IL-6 and IL-8 in the two groups after treatment were decreased (P0.05). Conclusion The therapeutic effect of traditional Chinese medicine integrated with western medicine is superior to the western medicine only for acute lung injury due to surgery disease of chest. TXP can regulate the immune function and relieve the pulmonary inflammation and injury by decreasing the blood levels of TNF-?, IL-6 and IL-8, thus enhance the therapeutic effect of western medicine.

[Objective] To investigate the mechanism of Dachengqi Decoction (DCD) in treating systemic inflammatory response syndrome ( SIRS). [ Methods ] Mouse models induced by zymosan A were administered with DCD orally. Serum endotoxin level was detected by limulus assay and TNF-? and IL-6 levels by radioimmunoassay 6, 12 and 24 hours after medication.
