RESUMO

Isokinetics is a proven method to train and objectively assess the capability of muscle groups, particularly at the knee. The current re-injury rates and less than optimal return to sport percentages seen following anterior cruciate ligament surgery highlights the need for greater focus on what tests and methods are used to make these critical decisions. Isokinetics remains the best single method to objectively determine dynamic muscle strength, power, rate of force development and endurance. These factors make it well-suited to play a crucial role in influencing the appropriate patient progression through a rehabilitation program and assisting in determining return to play readiness following injury or surgery. In this article we will discuss why we believe isokinetics is a useful and necessary testing method, and elucidate testing parameters and goals used during knee extension/flexion assessment.

The purpose of this study was to examine the effect of pelvic tilt angle on maximum hip and knee muscles' strength and antagonist/agonist strength ratios. Twenty-one young males and females performed maximum isokinetic concentric knee extension-flexion and hip extension-flexion efforts at 60°·s-1, 120°·s-1, and 180°·s-1 from three positions: anterior, neutral, and posterior pelvic tilt. Peak torques and knee flexor-to-extensor and hip flexor-to-extensor torque ratios were analyzed. An analysis of variance showed that peak hip extensor torque was significantly greater in the anterior pelvic tilt condition compared to either neutral or posterior pelvic tilt angles (p > 0.05). No effects of changing pelvic tilt angle on hip flexor, knee flexor, or knee extension values were found (p > 0.05). The hip flexor-to-extensor torque ratio decreased (p < 0.05) in the anterior pelvic tilt position relative to the other positions, while no difference in the knee flexor-to-extensor ratio between pelvic positions was observed (p > 0.05). This study shows that an increased anterior pelvic tilt affects the maximum isokinetic strength of the hip extensors, supporting previous suggestions regarding the link between pelvic position and hip and knee muscle function. Isokinetic testing from an anterior pelvic tilt position may alter the evaluation of hip flexion/extension strength.

This cross-sectional observational study aims to determine isokinetic normality data at different speeds, and isometric data of ankle and knee joints, in healthy basketball players aged 15-16 years old. The participants were recruited through non-probabilistic convenience sampling. Sociodemographic, anthropometric, and biomechanical variables were collected. The study involved 42 participants. Right-leg dominance was higher in women (85.7%) than in men (78.6%). Men had a higher weight, height, and body mass index compared to women. Statistically significant differences were observed between sex and height (p < 0.001). Significant differences were found between sexes in knee flexor and extensor strength at different isokinetic speeds (30°, 120°, and 180°/s), except for the maximum peak strength knee flexion at 180°/s in the right leg. In the ankle, the variables inversion, eversion, and work strength values at different isokinetic speeds and full RoM, by sex, were not significantly different, except for the right (p = 0.004) and the left (p = 0.035) ankle full RoM. The study found lower knee extensor strength in women, indicating the need to improve knee flexor/extensor strength in women to match that of men, as seen in other joints. The results can guide the development of preventive and therapeutic interventions for lower limb injuries in basketball players.

The tennis serve is closely related to the quality of the tennis match. However, the isokinetic parameters associated with the tennis serve are still unclear. The aim of this study was to investigate the relationships between ball velocity and isokinetic shoulder strength in tennis serve and to determine isokinetic strength parameters that can predict tennis serve velocity. A total of 13 elite male athletes (16.8 ± 1.5 years) voluntarily participated in the study. The athletes' shoulder internal-external rotation, extension-flexion and abduction-adduction strengths were measured with 5-5-15 repetitions at 60°/s, 180°/s, 240°/s angular velocities. Later, the athletes' 1st, 2nd, and average serve ball speeds were determined using a handheld radar gun. Significant correlations were found between ball speed and isokinetic tennis serve strength (r = 0.556-0.819; p < 0.05). The correlations between ball speed and isokinetic strength performance were higher at 180°/s and 240°/s angular velocities. Extension (240°/s; r = 0.819), flexion (180°/s; r = 0.755), abduction (240°/s; r = 0.733), adduction (240°/s; r = 0.684) and internal rotation (180°/s; r = 0.803) were highly correlated with ball velocity. These findings suggest that strength training to increase the ball speed of the tennis serve should be performed fast at high angular velocities and planning should focus on the strength of extension, flexion, abduction, adduction and internal rotation.

Background: Pitchers are prone to upper extremity injury due to repetitive high joint loads. Clinical measures of shoulder strength and range of motion (ROM) have shown links to injury risk in pitchers, however, these factors have rarely been studied in relation to throwing joint loads. The purpose of this study was to identify which clinical ROM and isokinetic strength variables were related to peak shoulder and elbow joint torques in collegiate pitchers. Methods: Thirty-three healthy collegiate pitchers participated in this study. Fastball velocity, shoulder concentric and eccentric strength, and passive shoulder ROM variables were analyzed using a Lasso regression to determine what factors influenced shoulder internal rotation torque and elbow varus torque. Results: Fastball velocity was selected by the Lasso as indicator of increased shoulder and elbow torque. Passive shoulder external rotation ROM was also selected as an important factor in joint loading with increased shoulder external rotation ROM being related to lower joint loads. The bilateral ratio of shoulder internal rotator concentric strength was related to peak shoulder and elbow torques with an increase in the bilateral ratio of shoulder strength leading to reduced joint torques. Increases in the eccentric external rotator to concentric internal rotator strength (functional ratio) of the dominant arm and increases in dominant arm eccentric internal rotator strength were both related to increases in each joint torque. Conclusion: Results from the study indicate that pitch speed, passive shoulder external rotation ROM, and the isokinetic shoulder strength profile including internal rotator strength and functional strength ratio of pitchers are related to joint loading during the pitch and may be important to monitor in relation to injury risk and/or during rehabilitation. These results provide insight into the role that both shoulder ROM and rotator cuff strength play in the dynamic stabilization of the elbow and shoulder during pitching.

Ankle flexibility and isokinetic knee torque/power generating capacity were previously suggested to contribute or to be correlated to the vertical countermovement jump (CMJ) performance. The aim of this study was to investigate the effect of the passive ankle joint dorsi flexion (θPDF) and the knee muscle's isokinetic torque and power on the CMJ in adolescent female volleyball players. The θPDF at a knee extension angle of 140 degrees were measured for 37 female post-pubertal volleyball players. Then, the players were assigned to either the flexible (n = 10) or inflexible (n = 14) groups according to earlier recommended criteria. Testing included the CMJ with and without an arm swing, and maximal knee extensions and flexions in 3 angular velocities on an isokinetic dynamometer. CMJ height performed with or without an arm swing (r(22) = 0.563, p = 0.040 and r(22) = 0.518, p = 0.009, respectively) and relative power (r(22) = 0.517, p = 0.010 and r(22) = 0.446, p = 0.030, respectively) were positively correlated with the extensors' torque at 180°/s and were negatively correlated with the flexibility level of the dominant side ankle (r(22) = -0.529, p = 0.008 and r(22) = -0.576, p = 0.030, respectively). A moderate positive correlation was also revealed between the CMJ height with and without an arm swing and the power of the non-dominant knee extensors (r(22) = 0.458, p = 0.024 and r(22) = 0.402, p = 0.049, respectively) and flexors (r(22) = 0.484, p = 0.016 and r(22) = 0.477, p = 0.018, respectively). Results of the 2 × 2 repeated ANOVA measurements revealed that flexible players jumped significantly (p < 0.05) higher during the CMJs, whilst there was a group effect only on the isokinetic knee extensor muscles' torque. In conclusion, a more flexible ankle joint and a higher isokinetic knee extensor's torque generating capacity resulted in higher CMJ performance. Therefore, ankle flexibility should be emphasized in training and is suggested to be included in preseason screening tests of youth female volleyball players.

BACKGROUND: The aim of the study was to conduct a comprehensive functional and radiological follow-up assessment in patients at least 10 years after adductor magnus MPFL reconstruction, and to assess the presence of early degenerative changes. METHODS: The mean age at the time of surgery was 16 years (range: 8 to 18 years, SD 2.5). The follow-up examination was performed at least 10 years following adductor magnus MPFL reconstruction (mean 11 years). Twenty-one patients (26 operated knees) attended the follow-up. The mean age at follow-up was 25.1 years (range 20-29 years). RESULTS: The significant improvement observed at 3 years, indicated by the Kujala and Lysholm scores, was maintained after 10 years of follow-up (p < 0.001). A single recurrence of dislocation was noted in three patients. A significant improvement in radiological parameters was noted. No significant difference in the incidence of chondromalacia, of any degree, was observed compared to controls. Significantly higher quadriceps peak torque was noted for both angular velocities (60 and 180°/sec) compared to the preoperative readings (p < 0.001). Knee flexors were found to be significantly stronger at both 60 and 180°/sec at 10 years follow-up examination (p = 0.008 and p < 0.001 respectively). CONCLUSION: The use of MPFL reconstruction according to Avikainen yields improvements in clinical and radiological results which are maintained throughout the observation period. No significantly greater articular cartilage degeneration was noted in patients after surgical treatment for recurrent patellar dislocation compared to healthy peers. OBJECTIVE: Occlusal disturbances affect human posture and sports performance. This study aimed to monitor biomechanical adaptations to personalized occlusal splints. METHODS: Splints were customized based on stabilometry, thermography, sEMG, and kinesiography, and administered to three triathlon athletes. They were evaluated during a 4-month period, using isokinetic indexes, running kinematics and anaerobic outputs. RESULTS: Individuality emerged as a key factor driving type, quantity, quality, and time trajectories of adaptations. The use of instrumental and clinical tests allowed the detection of static balance and biting function improvements, but not necessarily parallel to sports performance improvements. CONCLUSION: The authors argue that strength and kinematic imbalances are joint and task-specific and support the use of multi-stage monitoring of the biomechanical effect of mouthpieces. Kinematics of cycling and running may be widely assessed with ecological and inexpensive methods. Strength imbalances need to be continuously monitored due to the high informative value to injury prevention.

INTRODUCTION: Propulsive wheelchair capacity may be influenced by arm muscle performance.Objectives: To determine whether children and adolescent wheelchair-users with spina bifida show reduced arm muscle strength compared to their typically developing peers and to compare muscle strength data obtained by isokinetic and handheld dynamometry (HHD). METHODS: Participants (mean age 12.5 ± 2.5 years) were assigned to spina bifida (SB; n = 11) or control (n = 22) groups. Isokinetic dynamometry was used to measure peak torque (PT) during dynamic (60°s-1 and 120°s-1) and isometric strength testing (MVIC) for shoulder and elbow flexors and extensors. HHD was used to measure MVIC of the same muscle groups. RESULTS: SB showed reduced isokinetic PT for shoulder extensors at 60°s-1 and 120°s-1, shoulder flexor MVIC, and elbow flexors at 60°s-1 and MVIC; higher PT for shoulder flexors and elbow flexors at 120°s-1; and decreased MVIC for elbow flexors but not extensors compared to controls when assessed by HHD. The SB and control groups showed strong positive correlations between MVIC data obtained by the two devices for all muscle groups (r ≥ 0.81; p < .01), except for shoulder flexor MVIC in controls (r = 0.68; p < .01). CONCLUSION: SB presented reduced levels of dynamic slow-speed and isometric shoulder and elbow muscle strength and greater dynamic high-speed shoulder and elbow flexor strength than controls possibly due to the exposure to self-sustained wheelchair ambulation. Exercise-based intervention protocols to increase slow-speed arm muscle strength should be considered in youth with SB. Strong positive correlations observed between muscle strength assessed by isokinetic dynamometry and HHD support the use of HHD in this clinical population.

The correct torque ratio between the knee joint extensor and flexor muscle groups can effectively prevent injuries to the anterior cruciate ligament and hamstring strain. However, it is unclear whether a high torque ratio of the knee joint flexor muscles to the extensor muscles is beneficial for sport performance. Therefore, the aim of the study was to investigate the relationship between the hamstring-to-quadriceps (H/Q) ratio and sprint times (10- and 30-m) and jump heights (CMJ-countermovement jump and SJ-squat jump) in soccer players. The study examined 26 young elite soccer players (age: 18.1 ± 0.7 years; body height: 1.77 ± 0.05 m; body mass: 72.7 ± 5.7 kg). Knee joint flexor and extensor peak torques were assessed using the Cybex dynamometer (at 60°/s, 120°/s and 180°/s). Additionally, each participant performed the CMJ, SJ, and 30 m sprint. A significant relationship was obtained between the H/Q ratio (60°/s) and 30 m sprint time (r = 0.47). The positive direction of this relationship may indicate an important role of knee joint extensors in sprinting performance. Moreover, the H/Q ratio was not significantly associated with the CMJ, SJ or 10 m sprint performance. The H/Q ratio should be considered together with the peak torque values in terms of the assessment of sprinting and jumping performance.

Objective: The purpose of this study was to determine the prevalence of muscle imbalance among young adolescent acrobats (n=15) and if there was a potential link to injury. Methods: Isokinetic strength of the lower extremity, isometric strength of the trunk, and flexibility of both the trunk and lower extremity were assessed. Pearson correlation (r) and χ2 correlation tests were performed on all explanatory variables. Results: Significant correlations were found between isokinetic peak torque of the quadriceps and hamstrings (p=0.000) and the plantar flexors and dorsiflexors (p=0.000) on both sides, along with plantar flexor dominance (p=0.000). Non-significant findings were seen when identifying dominance between the quadriceps and hamstrings (p=0.933) as well as when correlating peak torque and flexibility of the lower extremity (right hamstrings: p=0.668, left hamstrings: p=0.338, right quadriceps: p=0.171, left quadriceps: p=0.707, right plantar flexors: p=0.282, left plantar flexors: p=0.382, right dorsiflexors: p=0.297 and left dorsiflexors: p=0.393). Conclusion: Acrobats demonstrated noticeably high ranges of flexibility, and the most common injury site was found to be the ankle. However, these mentioned injuries were not all due to acrobatic participation. The limited sample size warrants extensive research with a larger sample size to further verify or dispute the results found in this study. Muscle imbalances found within this population could increase the risk of injury.

CONTEXT: Overhead athletes place tremendous demands on the shoulder complex, which result in adaptive kinematics but potential susceptibility to injury. This study aimed to compare shoulder glenohumeral range of motion (ROM) and rotator muscles strength in bilateral and unilateral overhead sports. DESIGN: Cross-sectional study; secondary care. METHODS: Forty-two elite athletes (17.7 [4.5] y; 42.9% female), including 18 swimmers and 24 badminton players, were included. Preseason glenohumeral rotation ROM and isokinetic shoulder internal and external rotator muscles strength was tested (60°·s-1). Bilateral (nondominant:dominant) and agonist:antagonist (external rotator:internal rotator) conventional and functional deceleration ratios (eccentric to concentric) were calculated. The impact of sport and number of competitive years on shoulder ROM and strength was tested. RESULTS: Badminton players had greater glenohumeral internal rotation deficit, total ROM deficit, and lower bilateral strength ratio than swimmers (P < .050). Rotational strength was positively associated with the competitive years, but greater in swimmers (P < .050) and on the internal rotator (P < .001). The functional deceleration ratio was negatively associated with the competitive years on the dominant side in swimmers and for both sides in badminton. CONCLUSIONS: Unilateral overhead athletes had greater ROM and rotational strength asymmetries than bilateral athletes. Interestingly, the functional deceleration ratio was lower over time on the dominant shoulder for both sports, but, surprisingly, also on the nondominant shoulder in badminton, potentially creating a greater risk for shoulder injuries.

Eccentric exercise is discussed as a treatment option for clinical populations, but specific responses in terms of muscle damage and systemic inflammation after repeated loading of large muscle groups have not been conclusively characterized. Therefore, this study tested the feasibility of an isokinetic protocol for repeated maximum eccentric loading of the trunk muscles. Nine asymptomatic participants (5 f/4 m; 34±6 yrs; 175±13 cm; 76±17 kg) performed three isokinetic 2-minute all-out trunk strength tests (1x concentric (CON), 2x eccentric (ECC1, ECC2), 2 weeks apart; flexion/extension, 60°/s, ROM 55°). Outcomes were peak torque, torque decline, total work, and indicators of muscle damage and inflammation (over 168 h). Statistics were done using the Friedman test (Dunn's post-test). For ECC1 and ECC2, peak torque and total work were increased and torque decline reduced compared to CON. Repeated ECC bouts yielded unaltered torque and work outcomes. Muscle damage markers were highest after ECC1 (soreness 48 h, creatine kinase 72 h; p<0.05). Their overall responses (area under the curve) were abolished post-ECC2 compared to post-ECC1 (p<0.05). Interleukin-6 was higher post-ECC1 than CON, and attenuated post-ECC2 (p>0.05). Interleukin-10 and tumor necrosis factor-α were not detectable. All markers showed high inter-individual variability. The protocol was feasible to induce muscle damage indicators after exercising a large muscle group, but the pilot results indicated only weak systemic inflammatory responses in asymptomatic adults.

In sweep rowers, the lower extremities muscle strength translates into the driving force of the boat. Therefore, isokinetic assessment of muscle torque is used by coaches as an indicator of the level of athletes' preparation for competitions. A total of 22 elite sweep rowers performed an isokinetic test of knee joint flexors and extensors, and a test on an asymmetric ergometer with the electromyography measures. Significantly higher quadriceps peak torque was shown during the beginning than the end of the preparatory period (p < 0.05). However, there was no significant knee peak torque difference between the lower extremities (p = 0.398). In the case of the electromyography test of quadriceps, hamstrings and lumbar erector spinae, analysis demonstrated significant bioelectrical activity differences between the sides (p < 0.05). Lower values of quadriceps peak torque at the end of the preparatory period may result from a decrease in the number of power training units during the entire preparatory period. A similar muscle strength between both lower extremities seems to be the correct result in the context of injury prevention. In turn, significant bioelectrical activity differences between the sides possibly are associated with the asymmetric movement pattern in sweep rowers.

PURPOSE: The main objective of this study was to evaluate the level of muscle strength by using isokinetic and isometric measurements-more specifically, the force ratio between the knee flexors and extensors (values of the torques). METHODS: An experimental group of elite volleyball players (n = 14) were compared to a control group (n = 14) of healthy non-athletes of comparable ages. Torque measurements were obtained under three concentric conditions (angular velocities of 60 °/s, 180 °/s, and 300 °/s) and one static condition by utilizing the Biodex System 3. RESULTS: In all trials, the volleyball players achieved significantly higher peak torque (PT) values for both the extensors and flexors (p < 0.05) than those of the control group. However, the strength ratio of the flexors and extensors (H/Q) in the experimental group was only 83% of the standard reported in the literature. The most developed and dominating muscles in the knee joints of the volleyball players were the extensors, which accounted for the low strength ratio and dynamic instability of this joint. CONCLUSION: Based on a proper assessment of the strength ratio of the knee flexors and extensors, properly selected and implemented resistance training can improve the maximum strength and power production and reduce the incidence of injuries in volleyball.

BACKGROUND: We assessed the association between: the severity of hip chondral or labral pathology with dynamic hip muscle strength or quality of life in patients with femoroacetabular impingement syndrome scheduled for hip arthroscopy. We also assessed the association between dynamic hip muscle strength with quality of life. METHODS: Eighty-three participants with femoroacetabular impingement syndrome scheduled for hip arthroscopy were included. We measured dynamic hip abduction and adduction muscle strength with an isokinetic dynamometer (Nm/kg), and quality of life with the iHoT-33 questionnaire. The severity of hip chondrolabral pathologies was scored using the modified Beck classification. Linear regression analyses were conducted to assess the association between severity of hip chondral or labral pathology with dynamic hip muscle strength and quality of life. FINDINGS: The regression analyses showed no association between the (i) severity of hip chondral (adjusted r2: 0.14) or labral (adjusted r2: 0.08) pathology and quality of life (P > 0.05), (ii) between the severity of hip chondral or labral pathology and dynamic hip abduction and adduction muscle strength (P > 0.05). Significant correlation was observed for quality of life and hip abduction (adjusted r2:0.29; P < 0.001) or adduction (adjusted r2: 0.32; P < 0.001) muscle strength. INTERPRETATION: The severity of hip chondral or labral pathologies were not associated with quality of life or dynamic hip muscle strength in participants with femoroacetabular impingement syndrome. Greater dynamic hip abduction and adduction muscle strength were associated with better quality of life in participants with femoroacetabular impingement syndrome scheduled for hip arthroscopy.

Characterization of scapular kinematics under demanding load conditions might aid to distinguish between physiological and clinically relevant alterations. Previous investigations focused only on submaximal external load situations. How scapular movement changes with maximal load remains unclear. Therefore, the present study aimed to evaluate 3D scapular kinematics during unloaded and maximal loaded shoulder flexion and extension. Twelve asymptomatic individuals performed shoulder flexion and extension movements under unloaded and maximal concentric and eccentric loaded isokinetic conditions. 3D scapular kinematics assessed with a motion capture system was analyzed for 20° intervals of humeral positions from 20° to 120° flexion. Repeated measures ANOVAs were used to evaluate kinematic differences between load conditions for scapular position angles, scapulohumeral rhythm and scapular motion extent. Increased scapular upward rotation was seen during shoulder flexion and extension as well as decreased posterior tilt and external rotation during eccentric and concentric arm descents of maximal loaded compared to unloaded conditions. Load effects were further seen for the scapulohumeral rhythm with greater scapular involvement at lower humeral positions and increased scapular motion extent under maximal loaded shoulder movements. With maximal load applied to the arm physiological scapular movement pattern are induced that may imply both impingement sparing and causing mechanisms.

Strength capabilities of the leg-extending muscles play an important role in athletic throwing events, where a high momentum must be transferred to the throwing device. The objective of this study was to quantify the force-velocity profiles of the leg-extending muscles considering gender, age and event. The leg-extending forces of 143 elite-athletes (59 females, 84 males) of the athletic throwing events were measured during isokinetic leg press tasks at four speeds. The maxima of force, power and contraction velocity of a knee-extending model muscle could be determined by means of a scalable geometric model and linear fitting. This method makes different groups of athletes comparable. Furthermore, the individually achieved release speeds were measured in a field test. Female and male throwers showed a different development of force-velocity profiles during their maturation process. For the men the development is more speed-oriented. We identified shot putters and javelin throwers to be the athletes with the highest leg-extending power. The importance of leg-extending muscle strength for the complex field performance for the throwing events was confirmed. The presented approach allows to give support in controlling and guiding of leg-extension strength training for both different stages of maturing and varying throwing events.

The primary aim of our study was to evaluate the strength of ankle evertor muscles in patients who have undergone a lateral ankle ligamentoplasty (which combined tensioning of the primary ligament group and reinforcing it with a pediculated extensor retinaculum flap), using 2 measurement systems (isokinetic and the functional weightbearing test [MyoLux). Our hypothesis was the strength of evertor muscles on the treated side was comparable to that of the contralateral healthy side. This prospective study included 23 patients who had chronic ankle instability and underwent an inferior extensor retinaculum flap ligamentoplasty. Clinical and functional results were assessed using the American Orthopaedic Foot & Ankle Society and Karlsson scores. The evertor muscle strength was analyzed, in both treated and healthy contralateral ankles, using isokinetic testing (gold standard) - an open kinetic chain test and a functional closed kinetic chain test (MyoLux). Data were interpreted using the Stata 14.0 software. The American Orthopaedic Foot & Ankle Society score was 88.1 ± 4.5, and the Karlsson score was 89.6 ± 4.0. Isokinetic tests did not show any significant difference between the treated ankles and the healthy one. Functional tests measuring inversion control at the ankle did not demonstrate any functional differences between the 2 ankles. As confirmed by good functional scores and the lack of difference in evertor muscle strength, this study reports that the inferior extensor retinaculum flap ligamentoplasty is a satisfactory treatment of chronic ankle instability. In addition, the MyoLux is a reliable and effective test to properly assess proprioception at the ankle.

BACKGROUND/AIM: Different authors have tried to correlate the peak isokinetic torque values with the incidence of soccer match injuries. However, due to the wide variety of assessment testing protocols, such an inference becomes difficult. This study aimed to verify the capacity of an isokinetic test to establish injury risk reference values for hamstring strain injuries. METHODS: A retrospective cohort study based on isokinetic data and clinical records from the last 10 years was conducted in 582 Brazilian elite-professional soccer players, who were subjected to the same isokinetic test protocol, machine, and tester. A Multivariate Logistic Regression Analysis for Complex Data Sampling was used to generate injury risk statistical indexes. RESULTS: Multivariate regression analysis of both legs provided important data to identify the cut-off values of Concentric Peak Torque (181.82 Newton/*metres), Concentric Work (236.23 watts) and Concentric Power (130.11 joules). CONCLUSIONS: The injury risk indexes indicate that an increase of just one Newton unit in CPT (Concentric Peak Torque) and CJ (Concentric Power) above those cut-off values, can reduce the risk of future injuries by 2% and 2.7%, respectively.
