RESUMO

Purpose: Acute lumbar sprain (ALS) is a common clinical disease characterized by persistent intolerable low back pain and limitation of movement, and quick pain relief and restoration of mobility in a short time are the main needs of patients when they visit the clinic. This study aims to evaluate the immediate efficacy of contralateral acupuncture (CAT) on SI3 combined with active exercise in treating ALS. Methods and Analysis: This study is a randomized controlled trial which will recruit 118 eligible participants aged 18 to 55 years with ALS at the Second Affiliated Hospital of Yunnan University of Chinese Medicine between March 2024 and December 2026. Participants will be randomly assigned to the acupuncture group or the sham-acupuncture group in a 1:1 ratio. The acupuncture group will receive a 10-minute acupuncture treatment combined with active exercise, while the sham-acupuncture group will receive a 10-minute sham acupuncture treatment combined with active exercise. Randomization will use a computer-generated sequence with allocation concealed in opaque envelopes. The primary outcome will be the pain visual analogue scale (VAS) scores after 10 minutes of treatment. Secondary outcomes will include the pain VAS scores at other time points (2, 4, 6, and 8 minutes post-treatment), the lumbar range of motion (ROM) scores at various time points, blinded assessment, the treatment effect expectancy scale, and the rescue analgesia rate. The analysis will follow the intention-to-treat principle. The primary outcome will be analyzed using ANCOVA, and secondary outcomes with repeated measures ANOVA. The rescue analgesia rate will be assessed using either the χ2 test or Fisher's exact test. Discussion: This study is the first randomized controlled trial to assess the immediate efficacy of CAT in combination with active exercise for ALS. This study will provide a simple, rapid, and effective treatment for the clinical management of ALS.

OBJECTIVE: To observe the clinical efficacy of dynamic qi acupuncture for acute lumbar sprain, and to explore the differences of different needle retention time on the improvement of pain, lumbar mobility and lumbar dysfunction. METHODS: A total of 160 patients with acute lumbar sprain were randomly divided into an observation group A (40 cases, 4 cases dropped off), an observation group B (40 cases, 2 cases dropped off), an observation group C (40 cases, 4 cases dropped off) and a medication group (40 cases, 6 cases dropped off). The patients in the observation group A, the observation group B and the observation group C were treated with acupuncture at "lumbago point" and Sanjian (LI 3) on the left side, and during the needles were kept for 10, 20 and 30 min respectively, the patients were required to take tolerable lumbar active activities, once a day; the patients in the medication group were treated with celecoxib capsules, 0.2 g each time, twice a day. All the patients were treated for 5 d. Before and after treatment, the scores of numerical rating scale-11 (NRS-11), lumbar range of motion (ROM) and modified Oswestry disability index (ODI) were observed, and the clinical efficacy of each group was evaluated. RESULTS: After treatment, the scores of NRS-11, ROM and ODI in each group were decreased compared before treatment (P<0.01). The decreased degree of NRS-11, ROM and ODI in each observation group was greater than that in the medication group (P<0.05), and the decreased degree of ROM and ODI in the observation group B and the observation group C was greater than that in the observation group A (P<0.05). The total effective rates were 94.4% (34/36) in the observation group A, 94.7% (36/38) in the observation group B and 97.2% (35/36) in the observation group C, respectively, which were higher than 79.4% (27/34) in the medication group (P<0.05). CONCLUSION: Dynamic qi acupuncture with needle retention for 10, 20 and 30 min all could effectively improve the pain, lumbar mobility and lumbar dysfunction in patients with acute lumbar sprain. If the lumbar dysfunction is severe, needle retention for 20 min or more is recommended.

OBJECTIVE: To compare the curative effect of balanced acupuncture combined with TongduZhengji manipulation vs acupuncture in the treatment of acute lumbar sprain. METHODS: Clinical data of 71 patients with acute lumbar sprains in our hospital from January 2020 to December 2020 were retrospectively analyzed. Patients were divided into single group (n=35) and combined group (n=36) based on treatment methods. The single group received only acupuncture treatment, while the combined group received balanced acupuncture combined with TongduZhengji manipulation. The treatment efficacy, pain level, lumbar function and motion of the lumbar spine were compared between the two groups. RESULTS: The Visual Analogue Scale (VAS) scores of the combined group were lower than those of the single group after 3, 4, and 5 days of treatment (P<0.05). There was no significant difference in VAS scores between the two groups after 1 and 2 days of treatment (P>0.05). The Roland-Morris Disability Questionnaire (RMDQ) score of the combined group showed no significant difference compared with that of the single group after 1 and 2 days of treatment (P>0.05), and were lower than those of the single group after 3, 4, and 5 days of treatment (P<0.05). The Japanese Orthopedic Association (JOA) score of the combined group after 1, 2, and 3 days of treatment showed no significant difference compared with the single group (P>0.05), and was higher than that of the single group after 4 and 5 days of treatment (P<0.05). The Range of Motion (ROM) score of the combined group showed no significant difference compared with the single group after 1 and 2 days of treatment (P>0.05), and was lower than that of the single group after 3, 4, and 5 days of treatment (P<0.05). The total effective rate of treatment in the combined group was significantly higher than that in the single group (91.67% vs. 71.43%) (P<0.05). CONCLUSION: Compared with acupuncture alone, balanced acupuncture combined with TongduZhengji manipulation can significantly reduce the pain level and improve lumbar spine mobility as well as lumbar spine function, exhibiting better curative effect than acupuncture only.

Objective To evaluate the effect of Xuefu-Zhuyu decoction combined with interactive acupuncture and ibuprofen sustained-release capsule in the treatment of acute lumbar sprain. Methods A total of 102 patients with acute lumbar sprain were randomly divided into two groups, 51 in each group.The control group received ibuprofen sustained-release capsules orally. The study group was treated with Xuefu-Zhuyu decoction combined with interactive acupuncture on the basis of the control group. Both groups were treated for 2 weeks. The degree of low back pain was assessed by VAS and the improvement of lumbar function was assessed by modified Japanese Orthopaedic Association Scores (M-JOA). The Oswestry dysfunction index (ODI) was used to evaluate the degree of lumbar dysfunction. The serum hs-CRP, IL-6, IL-10 and prostaglandin E2 (PGE2) level was detected by ELISA. The clinical efficacy was evaluated. Results The total effective rate was 80.4% (41/51) in the study group and 62.8% (32/51) in the control group. There was significant difference between the two groups (χ2=3.903, P=0.048). After treatment, the VAS (1.85 ± 0.40 vs. 4.06 ± 0.67, t=20.226), M-JOA (4.41 ± 1.35 vs. 11.18 ± 2.25, t=18.426), and ODI (9.66 ± 3.34 vs. 16.37 ± 3.64, t=9.700) in the study group were significantly lower than those in the control group (P<0.01). The serum level of hs-CRP, IL-6, IL-10 and PGE2 in the study group were significantly lower than those in the control group after treatment (t were 6.694, 8.416, 22.110 and 12.137, respectively, all P s<0.01). Conclusions The Xuefu-Zhuyu decoction combined with interactive acupuncture and ibuprofen sustained-release capsule can improve the degree of lumbago and lumbar dysfunction in patients with acute lumbar sprain, reduce the level of serum inflammatory cytokines, and improve the clinical efficacy.

OBJECTIVE: The purpose of this study was to investigate the clinical effects of BU pharmacopuncture therapy consisting of bear's gall(fel ursi) and ox bezoar(bovis calculus) on acute lumbar sprain. METHODS: 12 patients diagnosed as acute lumbar sprain in 6 designated local Korean medicine clinics from October 2017 to February 2018 were treated by BU pharmacopuncture. Several acupoints in abdomen and lumbar region were selected by clinicians at their own discretion. The effectiveness of the therapy was evaluated using VAS and ODI. After that we reviewed the medical records of all these patients to evaluate the effectiveness and safety of the therapy. RESULTS: The VAS and ODI scales were significantly decreased after BU pharmacopuncture therapy. And no major complications and adverse effects were reported. CONCLUSION: BU pharmacopuncture showed rapid pain relief in patients with acute lumbar sprain. It is possible to shorten the treatment period of acute lumbar sprain and prevent progressing to chronic back pain in advance. To establish the effects of BU pharmacopuncture therapy, more succeeding clinical and laboratory studies are needed.

Objective To observe the clinical efficacy of acupuncture of She Medicine in treating acute lumbar sprain.Method Eighty patients with acute lumbar sprain were randomized into a treatment group and a control group,40 cases each.The treatment group was intervened by acupuncture of She medicine,while the control group received conventional medication.After successive 3-day treatment,the changes in Visual Analogue Scale (VAS) and Modified-Modified Schober (MMS) scores were observed,and the clinical efficacies were compared between the two groups.Result After the intervention,the VAS and MMS scores were significant changed in both groups (P<0.01).The VAS and MMS scores of the treatment group were significantly different from those of the control group after the intervention (P<0.01).The total effective rate and recovery rate were respectively 92.5% and 70.0% in the treatment group,versus 77.5% and 40.0% in the control group,and the between-group differences were statistically significant (P<0.01).Conclusion Acupuncture of She medicine is effective in treating acute lumbar sprain,with advantages of swift action in releasing symptoms and high recovery rate.

Objective To investigate the clinical efficacy of exercise cupping plus acupuncture in treating acute lumbar sprain. Method Sixty-four patients with acute lumbar sprain were randomly allocated to treatment and control groups, 32 cases each. The treatment group received exercise cupping plus acupuncture and the control group, acupuncture alone. After one treatment and one course of treatment, the therapeutic effects were evaluated using the VAS for pain, lumbar joint activity and the clinical effect assessment. Result After one course of treatment, the total efficacy rate was 96.9% in the treatment group, which was significantly higher than 84.4% in the control group (P<0.05). After one treatment and one course of treatment, the VAS score was significantly lower in the treatment group than in the control group (P<0.05) and lumbar joint activity improved significantly in the treatment group compared with the control group (P<0.05). Conclusion Exercise cupping plus acupuncture is more effective than acupuncture alone in treating acute lumbar sprain.

Objective To compare the effects between bloodletting at auricular points and acupuncture at specific points [Yintang (GV 29) or Yanglao (SI 6)] for analgesia in acute lumbar sprain.Method Totally 100 patients with acute lumbar sprain were randomized into a treatment group and a control group, 50 cases in each group. The treatment group was intervened by bloodletting at auricular points, while the control group was by acupuncture at specific points [Yintang (GV 29) or Yanglao (SI 6). The Present Pain Intensity (PPI) from the Short-form McGill Pain Questionnaire was taken as the observation index. The changes of PPI were compared respectively 10 min and 30 min after treatment.Result The PPI scores were significantly changed 30 min after treatment in both groups (P<0.05). The PPI scores in the treatment group were significantly different from that in the control group both 10 min and 30 min after intervention (P<0.05).Conclusion Bloodletting at auricular points and acupuncture at specific points are both effective in releasing pain in acute lumbar sprain, while bloodletting at auricular points can produce a more significant real-time analgesic effect.

Objective To observe the clinical efficacy of pricking anti-Ashi points with fire needles in treating acute lumbar sprain.Method Seventy-six patients with acute lumbar sprain were randomized into a treatment group and a control group, 38 cases in each group. The treatment group was intervened by pricking anti-Ashi points with fire needles, while the control group was by warm needling. The Visual Analogue Scale (VAS) and Roland-Morris Dysfunction Questionnaire (RMDQ) were adopted to evaluate lumbar pain and dysfunction after 1-day and 5-day treatment, and the clinical efficacies were compared between the two groups.Result Respectively after the first treatment session and 5 treatment sessions, the VAS and RMDQ scores were significantly changed in both groups (P<0.01). Respectively after the first treatment session and 5 treatment sessions, the VAS and RMDQ scores in the treatment group were significantly different from that in the control group (P<0.05). The total effective rate and recovery rate were respectively 94.7% and 63.2% in the treatment group, versus 86.8% and 42.1% in the control group, and the differences were statistically significant (P<0.01).Conclusion Pricking anti-Ashi points with fire needles is an effective method in treating acute lumbar sprain, and it can produce a significant real-time analgesic effect.

OBJECTIVE:To observe clinical efficacy and safety of flupirtine maleate for pain caused by acute lumbar sprain. METHODS:60 patients with acute lumbar sprain were selected and divided into trial group and control group according to even and odd-numbered admission order. Trial group received flupirtine maleate capsule,1 piece/time,3 times/d;control group was giv-en codeine sustained-release tablet,2 tablets/time,2 times/d. The VAS score,clinical efficacy and ADR were compared between 2 groups. RESULTS:The VAS score of treatment group after treatment was significantly lower than that of control group,with statis-tical significance(t=2.375,P=0.013). The clinical efficacy of trial group was significantly higher than that of control group,with statistical significance (u=9.431,P=0.024). The ADR of trial group was mild,and there was no significant difference between two groups(χ2=0.131,P=0.717). CONCLUSIONS:Flupirtine maleate has a good clinical efficacy and safety in the treatment of pain caused by acute lumbar sprain.

ObjectiveTo investigate the clinical efficacy of Yaotongxue(Ex-UE7)finger manipulation plus ultrashort waves in treating acute lumbar sprain.MethodForty-two patients with acute lumbar sprain were enrolled and randomized into groups. The control group received lumbar ultrashort waves treatment and the observation group, Yaotongxue finger manipulation in addition. Pain relief and activities of daily living were assessed using the Visual Analogue Scale (VAS) and the Barthel Index before and after one and five sessions of treatment.ResultThere was no statistically significant pre-treatment difference in the VAS score between the two groups (P>0.05). After one and five sessions of treatment, the VAS score was significantly lower in the observation group than in the control group and there was a statistically significant difference (P0.05). No adverse events occurred during the study.ConclusionYaotongxue finger manipulation plus ultrashort waves is safe and highly effective in treating acute lumbar sprain.

[Objective] To observe the therapeutic effects of acupuncture and manipulation on acute lumbar sprain.[Methods] 144 patients were randomly divided into 3 groups: the first treatment group treated with acupuncture at YAOTON point,the second group with manipulation and the third group with two methods above.The effects were assessed after 1 w and 1 m.[Results]After 1 w,there was significant difference in 3 groups: the therapeutic effects in the band group was better than other 2 groups,but no difference in other 2 groups.[Conclusions] Both acupuncture at YAOTONG point and manipulation are good methods for acute lumbar sprain.

Fifty cases of acute lumbar sprain were treated by the combination of acupuncture, cupping and kinesitherapy. Forty-three cases were cured and 7 cases improved, the total effective rate being 100%. This therapy has an exact effect with a short course of treatment.
