RESUMO

Chlamydia trachomatis is the most prevalent sexually transmitted bacterium worldwide and the causative agent of trachoma. Its strains are classified according to their ompA genotypes, which are strongly linked to differential tissue tropism and disease outcomes [ocular disease, urogenital disease and lymphogranuloma venereum (LGV)]. While the genome-based species phylogenetic tree presents four main clades correlating with tropism/prevalence, namely ocular, LGV, urogenital T1 (more prevalent genotypes) and urogenital T2 (less prevalent genotypes), inter-clade exchange of ompA is considered a rare phenomenon probably mediating marked tropism alterations. An LGV epidemic, associated with the clonal expansion of the L2b genotype, has emerged in the last few decades, raising concerns particularly due to its atypical clinical presentation (ulcerative proctitis) and circulation among men who have sex with men (MSM). Here, we report an LGV outbreak, mostly affecting human immunodeficiency virus-positive MSM engaging in high-risk sexual practices, caused by an L2b strain with a rather unique non-LGV ompA signature that precluded the laboratory notification of this outbreak as LGV. C. trachomatis whole-genome capture and sequencing directly from clinical samples was applied to deeply characterize the genomic backbone of this novel LGV outbreak-causing clone. It revealed a chimeric genome structure due to the genetic transfer of ompA and four neighbouring genes from a serovar D/Da strain, likely possessing the genomic backbone associated with the more prevalent urogenital genotypes (T1 clade), to an LGV (L2b) strain. The hybrid L2b/D-Da strain presents the adhesin and immunodominant antigen MOMP (major outer membrane protein) (encoded by ompA) with an epitope repertoire typical of non-invasive genital strains, while keeping the genome-dispersed virulence fingerprint of a classical LGV strain. As previously reported for inter-clade ompA exchange among non-LGV clades, this novel C. trachomatis genomic mosaic involving a contemporary epidemiologically and clinically relevant LGV strain may have implications on its transmission, tissue tropism and pathogenic capabilities. The emergence of variants with epidemic and pathogenic potential highlights the need for more focused surveillance strategies to capture C. trachomatis evolution in action.

We report the occurrence of two severe illnesses experienced by one patient over a 19 year period of time. Both illnesses were characterized by severe inflammation and tissue destruction. Signs and symptoms of the first illness were characteristic of lymphogranuloma venereum (LGV). The second illness mimicked scrofula. During the second illness the patient was discovered to have a rare immunodeficiency due to auto-antibodies to Interleukin (IL)-12 and infection by Burkholderia gladioli, a plant pathogen usually harmless in humans. We were able to retrieve biopsies from the first illness to establish that B. gladioli was already present during the original presentation. That first illness lasted 5 year s, but she survived without the correct pathogen ever being identified, and without a diagnosis of immunodeficiency. After a remission of 10 year s, she experienced her second illness. The responses to treatment before and after the correct diagnoses were established provide us with an excellent opportunity to consider and discuss how disease expression reflects complex relationships between host defenses and microbial characteristics.

In eastern Europe, few countries have so far reported laboratory-confirmed cases of lymphogranuloma venereum (LGV). Here we describe 22 LGV cases in men who have sex with men (MSM) detected in Hungary from November 2012 to July 2016. Sequence analyses show that 16 of these 22 cases were affected by the L2c genovariant, with from 2012 to 2014, one LGV L2c case detected per year, followed by seven cases in 2015 and six up to July 2016. Of the 16 total L2c LGV cases, 10 had severe haemorrhagic proctitis. These findings are concerning as cases with this new genovariant among MSM have not been frequently reported in Europe to date. More research is needed to assess the spread of the L2c genovariant and its potential association with virulence and severe clinical manifestation.

Las infecciones por Chlamydia han alcanzado alta incidencia en el mundo. Constituyen importantes patógenos para los animales y el hombre. En la especie humana pueden ocasionar múltiples infecciones en diferentes órganos y sistemas de órganos. Objetivo: resumir los aspectos más novedosos relacionados con las Chlamydias en cuanto a sus características y formas de evitarla.Métodos: se realizó una revisión bibliográfica sobre la importancia clínica de las Chlamydias en la especie humana y su elevada incidencia. Se consultó la base de datos Medline disponible en INFOMED, de donde se extrajeron referencias bibliográficas entre los años 1995 y 2011, acerca de algunos aspectos etiológicos, clínicos, diagnósticos y terapéuticos de acuerdo a los últimos apuntes más relevantes en la literatura médica sobre Chlamydias y su relación con el hombre. Resultados: la Chlamydia Trachomatis en particular responsable del tracoma, primera causa de ceguera prevenible, provoca un impacto deletéreo en la salud reproductiva, produciendo entidades como abortos espontáneos, rotura prematura de membranas, restricción del crecimiento intrauterino, endometritis, conjuntivitis y neumonía del recién nacido. Es responsable además de Linfogranuloma Venéreo, Uretritis no Gonocóccica, Síndrome de Reiter, Enfermedad Inflamatoria Pélvica, Síndrome de Fitz Hugh Curtis, Embarazo Ectópico Crónico, Dolor Pélvico Crónico y más recientemente Carcinoma Cervical, entre otras. Conclusiones: partiendo de la base del impacto social y económico que tiene la infección por Chlamydias en el mundo y en nuestro país, pudimos resumir los aspectos más novedosos en cuanto a los tipos, infecciones que causan y sus características, diagnóstico, terapéutica y medidas generales para evitarlas o reducir su impacto negativo en la sociedad...


Chlamydia Infections have reached high incidence in the world. They are important pathogens for both animals and man. In humans, they can cause multiple infections in different organs and organ systems. Objective: to summarize the most novel aspects related to Chlamydias with regard to their characteristics and ways to avoid them.Methods: a bibliographic review on the clinical importance of Chlamydias and their high incidence in humans was conducted. The MEDLINE database available in INFOMED was consulted; from which bibliographic references were taken between the years 1995 and 2011 about some etiologic, clinical, diagnostic and therapeutic aspects in relation to the latest relevant notes appearing in the medical literature about Chlamydias and their relation with the man. Results: Chlamydia Trachomatis, in particular, the responsible for trachoma and the first cause of preventable blindness, causes a deleterious impact on reproductive health, leading to entities such as spontaneous abortions, premature rupture of membranes, intrauterine growth restriction, endometritis, conjunctivitis, and pneumonia in the newborn. It is also responsible for Lymphogranuloma Venereum (LGV), Nongonococcal Urethritis (NGU), Reiter's syndrome, Pelvic Inflammatory Disease, Fitz-Hugh-Curtis Syndrome, Chronic Ectopic Pregnancy, Chronic Pelvic Pain, and most recently, Cervical Carcinoma, among others.Conclusions: on the basis of social and economic impact that the infections by Chlamydias have in the world and in our country, we could summarize the most recent aspects in relation to types, infections that they cause and their characteristics, diagnosis, therapy and general measures to avoid them or reduce their negative impact on society...
