RESUMO

The majority of people with dementia live in low or middle-income countries (LMICs) where resources that play a crucial role in brain health, such as quality education, are still not widely available. In Brazil, illiteracy remains a prevalent issue, especially in communities with lower socioeconomic status (SES). The PROAME study set out to explore basic education in illiterate adults as a means to improve cognitive reserve. Objective: This manuscript aims to explore the relationship between SES and learning, as well as cognitive outcomes, in an older illiterate population. Methods: This six-month clinical trial (NCT04473235) involved 108 participants, of which 77 concluded all assessments, enrolled in late-life basic education. SES assessments included Quality of Urban Living Index, Municipal Human Development Index and Household SES calculated for each participant. Cognitive assessments encompassed the Free and Cued Selective Reminding Test (FCSRT), a word list to assess reading, and the Beta III matrix. Results: The sample consisted primarily of women, with a mean age of 58.5. Participants improved their reading (p=0.01) and their FCSRT (p=0.003). Regarding episodic memory, women outperformed men (p=0.007) and younger participants improved more than their older counterparts (p=0.001). There was no association observed between SES and cognitive outcomes. Conclusion: Irrespective of SES, participants demonstrated positive outcomes after attending basic education. These findings highlight that late life education could be an important non-pharmacologic preventative measure, especially in LMICs.


A maioria das pessoas com demência vive em países de baixa/média renda, onde recursos essenciais para a saúde cerebral, como educação de qualidade, ainda não são amplamente acessíveis. No Brasil, o analfabetismo ainda é frequente, especialmente em comunidades de baixo nível socioeconômico. O estudo PROAME teve como objetivo explorar a educação básica tardia em pessoas analfabetas como ferramenta para o aumento da reserva cognitiva. Objetivo: Investigar a relação entre nível socioeconômico com aprendizado e com desempenho em testes cognitivos, em adultos analfabetos. Métodos: Este estudo clínico de seis meses (NCT04473235) contou com 108 participantes inscritos no projeto Educação para Jovens e Adultos (EJA), dos quais 77 completaram os testes. O nível socioeconômico de cada participante foi medido usando-se: o Índice de Qualidade de Vida Urbana, o Índice de Desenvolvimento Humano Municipal e o nível socioeconômico doméstico. Avaliações cognitivas incluíram: o Teste de Recordação Seletiva Livre e Guiada (TRSLG), uma lista de palavras para avaliar leitura e a matriz Beta III. Resultados: A amostra era predominantemente feminina, com idade média de 58,5. Os participantes melhoraram a leitura (p=0,01) e o TRSLG (p=0,003). Com relação à memoria episódica, as mulheres tiveram resultados superiores aos dos homens (p=0,007) e participantes mais jovens melhoraram mais que seus colegas mais velhos (p=0,001). Não foi observada nenhuma relação entre o nível socioeconômico e o desempenho cognitivo. Conclusão: Independentemente do nível socioeconômico, participantes obtiveram resultados positivos após frequentar a educação básica. Isso sugere que a educação tardia pode ser uma medida preventiva não farmacológica importante, especialmente em países de baixa/média renda.

Abstract Emerging studies indicate the persistence of symptoms beyond the acute phase of COVID-19. Cognitive impairment has been observed in certain individuals for months following infection. Currently, there is limited knowledge about the specific cognitive domains that undergo alterations during the post-acute COVID-19 syndrome and the potential impact of disease severity on cognition. The aim of this review is to examine studies that have reported cognitive impairment in post-acute COVID-19, categorizing them into subacute and chronic phases. The methodology proposed by JBI was followed in this study. The included studies were published between December 2019 and December 2022. The search was conducted in PubMed, PubMed PMC, BVS - BIREME, Embase, SCOPUS, Cochrane, Web of Science, Proquest, PsycInfo, and EBSCOHost. Data extraction included specific details about the population, concepts, context, and key findings or recommendations relevant to the review objectives. A total of 7,540 records were identified and examined, and 47 articles were included. The cognitive domains most frequently reported as altered 4 to 12 weeks after acute COVID-19 were language, episodic memory, and executive function, and after 12 weeks, the domains most affected were attention, episodic memory, and executive function. The results of this scoping review highlight that adults with post-acute COVID-19 syndrome may have impairment in specific cognitive domains.


Resumo Estudos emergentes indicam a persistência dos sintomas além da fase aguda da COVID-19. O comprometimento cognitivo foi observado em alguns indivíduos durante meses após a infecção. Atualmente, há pouco conhecimento sobre os domínios cognitivos específicos que sofrem alterações durante a síndrome pós-aguda da COVID-19 e o possível impacto da gravidade da doença na cognição. O objetivo desta revisão é examinar estudos que relataram comprometimento cognitivo na COVID-19 pós-aguda, categorizando-os em fases subaguda e crônica. A metodologia proposta pela Joanna Briggs Institute foi seguida neste estudo. Os estudos incluídos foram publicados entre dezembro de 2019 e dezembro de 2022. A busca foi realizada no PubMed, PubMed PMC, BVS - BIREME, Embase, SCOPUS, Cochrane, Web of Science, Proquest, PsycInfo e EBSCOHost. A extração de dados incluiu detalhes específicos sobre a população, os conceitos, o contexto e as principais descobertas ou recomendações relevantes para os objetivos da revisão. Um total de 7.540 registros foi identificado e examinado, e 47 artigos foram incluídos. Os domínios cognitivos mais frequentemente relatados como alterados de 4 a 12 semanas após a COVID-19 aguda foram linguagem, memória episódica e função executiva e, após 12 semanas, os domínios mais afetados foram atenção, memória episódica e função executiva. Os resultados dessa revisão de escopo destacam que adultos com síndrome pós-aguda da COVID-19 podem apresentar comprometimento em domínios cognitivos específicos.

Background and Purpose: Episodic memory is a system that receives and stores information about temporally dated episodes and their interrelations. Our study aimed to investigate the relevance of episodic memory to time perception, with a specific focus on simultaneity/order judgment. Methods: Experiment 1 employed the simultaneity judgment task to discern differences in time perception between patients with mild cognitive impairment or dementia, and age-matched normals. A mathematical analysis capable of estimating subjects' time processing was utilized to identify the sensory and decisional components of temporal order and simultaneity judgment. Experiment 2 examined how differences in temporal perception relate to performance in temporal order memory, in which time delays play a critical role. Results: The temporal decision windows for both temporal order and simultaneity judgments exhibited marginal differences between patients with episodic memory impairment, and their healthy counterparts (p = 0.15, t(22) = 1.34). These temporal decision windows may be linked to the temporal separation of events in episodic memory (Pearson's ρ = -0.53, p = 0.05). Conclusions: Based on our findings, the frequency of visual events accumulated and encoded in the working memory system in the patients' and normal group appears to be approximately (5.7 and 11.2) Hz, respectively. According to the internal clock model, a lower frequency of event pulses tends to result in underestimation of event duration, which phenomenon might be linked to the observed time distortions in patients with dementia.

Studies investigating amnesic patients have shown the involvement of the medial temporal lobe during working memory (WM) tasks, especially when multiple items or features have to be associated. However, so far, no study has examined the relationship between episodic memory and WM components in patients with amnesia for comprehensive neuropsychological evaluation. Objective: The objective of this study was to investigate whether the null retention relates to deficits in the episodic buffer (EB) or the central executive (CE) components of WM. Methods: This study included 15 amnesic patients with mixed etiologies and 13 matched healthy controls. These 15 amnesic patients with mixed etiologies were divided into two subgroups: NUL subgroup (n=7) patients whose raw score was 0 (zero) on the Logical Memory delayed recall test and MOR subgroup (n=8) patients who recalled at least 1 item. The EB was assessed by complex span tasks, and the CE was assessed by random number generation (RNG) test. Results: EB tasks were impaired in both subgroups compared with controls. RNG was impaired in NUL (p=0.03), but not in MOR (p=0.99), subgroup. Conclusions: CE impairment hampers the retrieval mode action, preventing it from initiating the mental reconstruction of the context in which the to-be-remembered information was presented minutes ago.


Estudos que investigaram pacientes amnésicos demonstraram envolvimento do lobo temporal medial durante tarefas de memória de trabalho, especialmente quando vários itens ou características devem ser associados. No entanto, até o momento, não há estudos que tenham examinado a relação entre memória episódica e os subcomponentes da memória de trabalho em pacientes com amnésia por meio de avaliação neuropsicológica ampla. Objetivo: Investigar se a retenção nula está relacionada a déficits no buffer episódico ou nos componentes do executivo central da memória operacional. Métodos: Quinze pacientes amnésicos com etiologias mistas foram divididos em dois subgrupos: subgrupo NUL (n=7), de pacientes cuja pontuação bruta foi 0 (zero) na memória lógica tardia, e subgrupo MOR (n=8), de pacientes que recordaram pelo menos um item; além de 13 controles saudáveis pareados. O retentor episódico foi avaliado por tarefas de spam complexo e o executivo central com geração aleatória de números. Resultados: As tarefas do retentor episódico estavam prejudicadas em ambos os subgrupos em comparação com os controles. O teste de geração aleatória de números foi prejudicado em NUL (p=0,03), mas não no subgrupo MOR (p=0,99). Conclusões: O comprometimento do executivo central dificulta a ação do modo de recuperação, impedindo-o de iniciar a reconstrução mental do contexto em que a informação a ser lembrada foi apresentada, minutos antes.

RESUMO O objetivo deste estudo foi verificar associações entre medidas de memória episódica e depressão em idosos saudáveis submetidos a intervenções cognitivas, físicas e psicopedagógicas. A amostra foi composta por 31 idosos, com idade média de 69,70 anos (DP± 6,38), sendo 25 mulheres e 6 homens. O procedimento consistiu em diferentes combinações de intervenções e suas avaliações em testes diagnósticos e de memória em follow-up de um ano. A análise de dados consistiu-se no teste não paramétrico de Friedman, com o nível de significância de p=0,05. Os resultados indicaram que a combinação que melhor apresentou desempenho cognitivo frente às variáveis relacionadas à aprendizagem (p=0,04) e interferência proativa (p=0,01) foi a "Stimulus + Psicopedagógica". O nível de depressão diminuiu ao longo das avaliações em todas as combinações (p=0,01). Esses achados indicam que intervenções combinadas podem ser promissoras em ganhos cognitivos e de humor


ABSTRACT The objective of this study was to verify associations between measures of episodic memory and depression in healthy elderly people submitted to cognitive, physical and psychopedagogical interventions. The sample consisted of 31 elderly people, with an average age of 69.70 years (SD ± 6.38), 25 women and 6 men. The procedure consisted of different combinations of interventions and their evaluations in diagnostic and memory tests in a one-year follow-up. Data analysis consisted of Friedman's non-parametric test, with a significance level of p=0.05. The results indicated that the combination that best presented cognitive performance in relation to the variables related to learning (p=0.04) and proactive interference (p=0.01) was the "Stimullus + Psicopedagógica". The level of depression decreased throughout the evaluations in all combinations (p=0.01). These findings indicate that combined interventions can be promising in cognitive and mood gains

RESUMEN INTRODUCCIÓN: La cirugía resectiva del lóbulo temporal anterior con amigdalo-hipocampectomía es un tratamiento efectivo para la epilepsia farmacorresistente del lóbulo temporal con esclerosis hipocampal. Sin embargo, este procedimiento conlleva riesgo de deterioro de la memoria episódica verbal y no verbal postoperatoria, dependiendo de la dominancia hemisférica para la memoria y el lenguaje. OBJETIVO: Explorar el desenlace de la memoria episódica posterior a la cirugía resectiva mediante lobectomía temporal anterior con amigdalo-hipocampectomía. MÉTODOS: Se analizó retrospectivamente la memoria episódica verbal y no verbal mediante pruebas neurop-sicológicas de 51 pacientes consecutivos sometidos a lobectomía temporal anterior con amigdalo-hipocampectomía del lado izquierdo y derecho. Todos los pacientes fueron sometidos a resonancia magnética cerebral preoperatoria, video-electroencefalografía y evaluaciones neuropsicológicas. A 12 pacientes (24 %) no se les realizó el test de Wada. RESULTADOS: Hubo disminución en la memoria episódica verbal postoperatoria con diferencias respecto a la preoperatoria, en la subprueba de textos II recuerdo de la escala de memoria de Wechsler III (p = 0,035). El resultado en la memoria episódica visual se mantuvo igual, no hubo diferencias en el grupo de lobectomía temporal estándar. CONCLUSIÓN: La lobectomía temporal anterior más amigdalo-hipocampectomía izquierda afecta levemente el desempeño de la memoria episódica postoperatoria, que clínicamente no es significativo en pacientes con epilepsia del lóbulo temporal mesial farmacorresistente.


SUMMARY INTRODUCTION: Resective surgery of the anterior temporal lobe with amygdalohippocampectomy is an effective treatment for drug-resistant epilepsy of the temporal lobe with hippocampal sclerosis. However, this procedure carries a risk of post-operative episodic verbal and nonverbal memory impairment depending on the hemispheric dominance for memory and language. OBJECTIVE: To explore the outcome of episodic memory after resective surgery by means of anterior temporal lobectomy with amygdalohippocampectomy. METHODS: Verbal and non-verbal episodic memory was retrospectively analyzed by neuropsychological tests of 51 consecutive patients undergoing anterior temporal lobectomy with amygadalohyppocampectomy on the left and right sides. All patients underwent preoperative brain MRI, video electroencephalography, and neuropsychological evaluations. 12 patients (24%) did not undergo the Wada test. RESULTS: There was a decrease in postoperative verbal episodic memory with differences compared to preoperative, in the text II subtest recall of the Wechsler III memory scale (p = 0.035). The result in visual episodic memory remained the same, there were no differences in the standard temporal lobectomy group. CONCLUSION: Anterior temporal lobectomy plus left amygadalohyppocampectomy slightly affects the performance of postoperative episodic memory, which is clinically not significant in patients with drug-resistant mesial temporal lobe epilepsy.

BACKGROUND: Rumination and overgeneral autobiographical memory are dysfunctional cognitions commonly found in older adults with depression. The theoretical underpinnings of mindfulness-based cognitive therapy (MBCT) address the ruminative tendencies and the non-specific retrieval of autobiographical memories. This study aims to examine the efficacy and cognitive mechanisms of MBCT in older adults with active depressive symptoms. METHODS: 57 older adults (mean age, 70 years) with normal cognition and mild to moderate depressive symptoms were randomly allocated to either the MBCT group or the active control group for 8 weeks. The MBCT group consisted of eight 2-hour weekly sessions and a 7-hour full-day retreat, with different themes for each class, guided mindfulness exercises, feedback and discussion, homework review, and psychoeducation. The active control group comprised a 1-hour physical exercise and a standardised health education of the specific theme with group discussion (eg fall prevention, chronic pain). Participants were assessed before and after the 8-week intervention for four outcome measures: the Hamilton Depression Rating Scale (HAMD), the Ruminative Response Scale (RRS), the Autobiographical Memory Test (AMT), and the Mindful Attention Awareness Scale (MAAS). RESULTS: There was a significant reduction in severity of depressive symptoms (HAMD score) in both the MBCT group (F(1, 27) = 35.9, p < 0.001, η2 = 0.57) and the active control group (F(1, 28) = 9.29, p < 0.01, η2 = 0.24), but only the MBCT group showed substantial improvements in autobiographical memory specificity (AMT score), rumination (RRS score), and mindfulness (MAAS score). CONCLUSION: Although both MBCT and active control programme decrease the severity of depressive symptoms in older adults, only MBCT improves AMS, rumination, and mindfulness. Our findings provide empirical support for the theoretical underpinnings of MBCT. Older adults with more severe depression and more severe dysfunctional cognition may benefit more from the specific therapeutic effects of MBCT.

Cognitive reserve has been described as offering protection against Alzheimer's disease (AD) and other neurodegenerative conditions, but also against age-associated brain changes. Using data from the Alzheimer's Disease Neuroimaging Initiative, we defined cognitive reserve using the residual reserve index: episodic memory performance residualized for 3T MRI-derived brain volumes and demographics. We examined whether cognitive reserve predicted executive function (EF) decline equally across 2 groups of older adults-AD biomarker-positive (n = 468) and -negative (n = 402)-defined by the tau-to-amyloid ratio in cerebrospinal fluid. A significant interaction between the residual reserve index and biomarker group revealed that the effect of cognitive reserve on EF decline was dependent on pathology status. In the biomarker-positive group, higher cognitive reserve predicted EF decline over five years. However, cognitive reserve did not predict EF decline in the biomarker-negative group. These results suggest a certain level of AD pathology may be needed before cognitive reserve exerts its protective effects on future cognition; however, further research that tracks cognitive reserve longitudinally is needed.

OBJECTIVE: Persons with multiple sclerosis (MS) often report prospective memory (PM) failures that directly impact their everyday life. However, it is not known whether PM deficits confer an increased risk of poorer everyday functioning. The aims of this study were to: (1) compare time- (Time-PM) and event-based PM (Event-PM) performance between persons with MS and healthy controls (HCs), (2) examine the neuropsychological correlates of PM in MS, and (3) examine the relationship between PM and everyday functioning in MS. METHOD: A between-subjects design was used to examine 30 adults with MS and 30 community-dwelling HC. Participants were administered the Memory for Intentions Screening Test (MIST) to assess PM skills, the Actual Reality™ (AR) to assess everyday functioning, and a battery of cognitive tests. RESULTS: The MS group performed significantly worse on Time-PM compared to HC but not on Event-PM tasks. While both Time-PM and Event-PM subscales were correlated with retrospective learning and memory, the MIST Time-PM subscale was correlated with executive functions. Significant correlations were observed between AR and the MIST Time-PM, but not Event-PM, subscales. CONCLUSIONS: The results highlight the role of executive functions on Time-PM. Furthermore, significant relationships with AR extend the ecological validity of the MIST to MS populations.

BACKGROUND: ε4 allele possession is associated with an increased risk of Alzheimer's disease. Its effects earlier in life are less well understood. Previous studies have reported both detrimental effects and a lack of effect on cognition outside dementia. We used genotype based recall from the ALSPAC study to investigate whether APOE genotype influences cognition in earlier adult life. METHODS: We invited all individuals with the rarer ε22 or ε44 genotypes and equal numbers of those with ε32, ε33 or ε34 APOE genotypes (total n invited = 1936, ages 23-67). Participants were screened for dementia using the Addenbrooke's Cognitive Examination Revised (ACE-R). Participants were asked to complete a 3 h battery of neuropsychological tests covering a range of cognitive domains. The primary outcome was performance on the Rey Auditory Verbal Learning Test (RAVLT). Transformation of variables was used where required to permit parametric testing. As genotypes are unlikely to be confounded unadjusted analyses were performed. RESULTS: 114 participants were recruited to the study (39 ε33, 27 ε34, 15 ε44, 26 ε32 & 7 ε22). ε4+ participants had higher scores on the cognitive failures questionnaire (10 point increase, p = 0.006) but no deficits on objective cognitive testing. ε2 carriers had slightly better episodic memory performance (p = 0.016), slightly improved n-back accuracy and better executive functioning (trails A&B, p = 0.005). CONCLUSIONS: It is intriguing that the ε2+ group performed better as this group have a lower risk of Alzheimer's disease. Most previous studies have analysed as ε4/non ε4 so may have missed this effect.

As opposed to conscious, personally relevant (explicit) memories that we can recall at will, implicit (unconscious) memories are prototypical of 'hidden' memory; memories that exist, but that we do not know we possess. Nevertheless, our behaviour can be affected by these memories; in fact, these memories allow us to function in an ever-changing world. It is still unclear from behavioural studies whether similar memories can be formed during anaesthesia. Thus, a relevant question is whether implicit memory formation is a realistic possibility during anaesthesia, considering the underlying neurophysiology. A different conceptualization of memory taxonomy is presented, the serial parallel independent model of Tulving, which focuses on dynamic information processing with interactions among different memory systems rather than static classification of different types of memories. The neurophysiological basis for subliminal information processing is considered in the context of brain function as embodied in network interactions. Function of sensory cortices and thalamic activity during anaesthesia are reviewed. The role of sensory and perisensory cortices, in particular the auditory cortex, in support of memory function is discussed. Although improbable, with the current knowledge of neurophysiology one cannot rule out the possibility of memory formation during anaesthesia.

OBJETIVO: O objetivo deste estudo foi descrever a relação entre competência funcional da memória episódica e fatores associados à independência de idosos saudáveis do município de Juiz de Fora, Minas Gerais, Brasil. MÉTODOS: Trata-se de estudo transversal, descritivo e exploratório em que 60 idosos saudáveis compuseram a amostra final. Para avaliar a capacidade funcional, foram utilizadas as escalas de Katz e de Lawton & Brody. A memória episódica foi avaliada utilizando-se teste de West & Thorn, validado e adaptado para a população brasileira por Yassuda, Lasca e Neri em 2005. A análise estatística foi realizada por meio do software estatístico Sphinx Léxica e Eureka, versão 5.0. RESULTADOS: Foi encontrada associação estatisticamente significante entre escolaridade e desempenho em tarefa de memória episódica e entre esta e três itens da escala de avaliação das atividades instrumentais de vida diária, embora inconclusiva. CONCLUSÕES: Acredita-se que esses resultados se devam ao fato de existir muitas diferenças entre os estudos com relação à metodologia empregada e a escolha dos instrumentos que avaliam tanto a memória episódica, quanto a capacidade funcional de idosos. Além disso, a hipótese de que no envelhecimento saudável existe uma capacidade de, frente às perdas graduais na memória episódica, compensá-las utilizando outras estratégias e recursos, deve receber atenção de futuras pesquisas. .


OBJECTIVE: This study aimed to describe the relationship between the functional competence of episodic memory and its associations with functional independence of healthy elderly in the city of Juiz de Fora, Minas Gerais state, Brazil. METHODS: It is a cross-sectional, descriptive and exploratory research in which 60 healthy elderly comprised the final sample. To evaluate the functional capacity, the Index of Independence of Daily Living and the Lawton Instrumental Activities of Daily Living Scale were used. The episodic memory was assessed by a test of West & Thorn, adapted and validated for the Brazilian population by Yassuda, Lasca e Neri in 2005. Statistical analysis were performed using the 5.0 version of the statistical software Sphinx Léxica and Eureka. RESULTS: Statistically significant association between education level and performance in episodic memory task was found, as well as between the latter and three items of assessment scale of instrumental activities of daily living, although inconclusive. CONCLUSIONS: It is believed that these results are due to the fact that there are so many differences between researches with respect to methodology and the choice of instruments that evaluates both episodic memory and the erderly's functional capacity. Furthermore, the hypothesis that in healthy aging there is a capacity to face gradual losses in episodic memory and compensate them using other strategies and resources, should receive attention in further research. .
