RESUMO

Background: Academic libraries play a significant role in the student learning process. However, student needs and preferences as well as new paradigms of learning are driving libraries to transition from quiet book repositories to places of collaboration and open information. This descriptive, mixed methods case presentation explores the transition of one library, the United States Air Force School of Aerospace Medicine Franzello Aeromedical Library, in three key areas: collection, capability, and facility. Due to the niche subject matter and audience the library serves, this case also describes how the Franzello Aeromedical Library's distinct collection and capability remained intact throughout modernization. Case Presentation: The Franzello Aeromedical Library's modernization project aimed to augment the library as a cutting-edge resource supporting USAFSAM's education, consultation, and research mission to equip Aerospace Medicine Airmen with the skills and knowledge for healthcare delivery in austere environments. This project was approached using five phases: 1) best practices baseline, 2) baseline evaluation of library visitor needs, 3) collection weeding, 4) capability, and 5) space design and construction. Conclusion: As a result of this complex two-year project, several recommendations were gleaned. Use the effort as an opportunity to market library services to new audiences. Ensure all stakeholders are at the table from day one and in perpetuity to save time, and consider using purposeful decision-making models, such as Courses of Action, to make tough calls. Be prepared for delays by padding your timeline and compromise where necessary to keep the project alive. Finally, the authors recommend using in-project discovery and findings to plan for future need justification.

RESUMO

The article presents review summarizing contemporary National and foreign experience of implementing digital technologies under provision of services in health care. The systematic analysis of data from Scopus, eLibrary, PubMed and others electronic databases permitted to select 30 sources in Russian and English for 2016-2023. Modern digitization trends affect collaborations of companies where vectors of modernization become development of unified digital framework based on common state information system of health care; common educational platform for medical knowledge enriching by AI capabilities, through development of organizational activities and workflows based on digital technologies and services utilizing digital transformation. The deficiency of tools evaluating efficiency of reading software products and scalability of health digitization processes become problematic issues. As of today, diagnostic technologies using AI systems, technologies of lung cancer screening, examination methods of patients with post-traumatic deformations of cheekbone-orbital complex, monitoring of readings of devices with digital registration, further office consulting of various levels and application of AI in neurosurgery were described already. The article also considers issues of telemedicine consultations and development of modernized care models. The authors expect that digital ecosystem in development addressing issues of legal concept of management, financing and system of patient legal protection will do everything necessary to mitigate cyber incidents.

OBJECTIVE: This article presents the National Healthcare Safety Network (NHSN)'s approach to automation for public health surveillance using digital quality measures (dQMs) via an open-source tool (NHSNLink) and piloting of this approach using real-world data in a newly established collaborative program (NHSNCoLab). The approach leverages Health Level Seven Fast Healthcare Interoperability Resources (FHIR) application programming interfaces to improve data collection and reporting for public health and patient safety beginning with common, clinically significant, and preventable patient harms, such as medication-related hypoglycemia, healthcare facility-onset Clostridioides difficile infection, and healthcare-associated venous thromboembolism. CONCLUSIONS: The NHSN's FHIR dQMs hold the promise of minimizing the burden of reporting, improving accuracy, quality, and validity of data collected by NHSN, and increasing speed and efficiency of public health surveillance.

RESUMO

The progression from gastric mucosal inflammation to cancer signifies a pivotal event in the trajectory of gastric cancer (GC) development. Chinese medicine (CM) exhibits unique advantages and holds significant promise in inhibiting carcinogenesis of the gastric mucosa. This review intricately examines the critical pathological events during the transition from gastric mucosal inflammation-cancer transformation (GMICT), with a particular focus on pathological evolution mechanisms of spasmolytic polypeptide-expressing metaplasia (SPEM). Moreover, it investigates the pioneering applications and advancements of CM in intervening within the medical research domain of precancerous transformations leading to GC. Furthermore, the analysis extends to major shortcomings and challenges confronted by current research in gastric precancerous lesions, and innovative studies related to CM are presented. We offer a highly succinct yet optimistic outlook on future developmental trends. This paper endeavors to foster a profound understanding of forefront dynamics in GMICT research and scientific implications of modernizing CM. It also introduces a novel perspective for establishing a collaborative secondary prevention system for GC that integrates both Western and Chinese medicines.

OBJECTIVE: To study the actual state of the organization of medical care for patients with the profile of «maxillofacial surgery¼ in the Southern Federal District of the Russian Federation in order to develop proposals for its optimization within the framework of a three-level system of medical care. MATERIAL AND METHODS: We analyzed accounting and reporting materials characterizing the organization of medical care in the field of «Oral and maxillofacial surgery¼ to the population in the Southern Federal District of the Russian Federation. RESULTS: The population of the Southern Federal District of the Russian Federation is currently provided with accessible round-the-clock medical care in the field of maxillofacial surgery. The forces and means at the disposal of the district government allow this to be implemented. However, the article describes a number of obstacles to the provision of high-quality medical care in the field of maxillofacial surgery and suggests ways to eliminate them. CONCLUSION: Preparation of an Order defining the functioning of a three-level system of medical care for adults and children with maxillofacial pathology in the Southern Federal District of the Russian Federation, as well as more effective use of telemedicine consultations will significantly improve the availability and quality of medical care.

Acupuncture has demonstrated positive efficacy in the treatment of brain disorders. However, significant challenges lie in integrating acupuncture with modern technologies, promoting its clinical application in treating brain disorders, elucidating the mechanisms underlying acupuncture's preventive and therapeutic effects on brain disorders, and accelerating the pace of translational development in acupuncture medicine. This paper briefly outlines the current research status, challenges, and potential future directions in acupuncture treatment for brain disorders, aiming to provide essential insights for the modernization and development of acupuncture in the treatment of brain disorders.

RESUMO

Health Equity Action Plans (HEAPs) are a recent strategy employed across health and human services to promote health equity. To inform the development of future HEAPs, as well as to build upon previous initiatives, we evaluated 52 health equity plans and resources from Oregon counties using five criteria: creation date, process orientation, racial equity lens, metrics, and community engagement. When developing future HEAPs, we recommend explicit commitments to collaborate with marginalized communities, to establish measurable goals and defined metrics for assessing progress, to include voices and perspectives of those affected by health inequities, and to detail community strengths, assets, and resources.

Since the 18th National Congress of the Communist Party of China (CPC), a continuous stream of scientific and technological innovations has unfolded in the realm of traditional Chinese medicine (TCM). With the aim of implementing the spirit of the 20th National Congress of the CPC, and the Opinions on Promoting the Inheritance, Innovation and Development of TCM, and to underscore the exemplary role of significant scientific and technological achievements, the China Association of Chinese Medicine, in alignment with relevant requirements and under the guidance of authoritative experts, has organized a comprehensive review of the important scientific and technological achievements in the field of TCM since the 18th National Congress of the CPC. Through rigorous procedures, including collecting and reviewing achievements, writing achievement reports, organizing expert reviews, and seeking public opinions, remarkable research achievements in TCM during 2012—2022 were compiled.

Objective Based on the theory of"lung governs the skin and hair"in"Yellow Emperor's Inner Classic",this paper analyzes the medication rules of whitening and freckle-removing.The aim of this study is to provide reference for the clinical practice of traditional Chinese medicine(TCM)theory and the medication in TCM cosmetics.Methods"Chinese Medical Classics"was used to search the records of whitening and freckle-related drugs.The frequency,nature,flavor,meridian tropism and compatibility laws of TCM for whitening and freckle-removal were analyzed by statistics and association rules.The network pharmacology research was used to analyze the whitening and freckle-removing effects and mechanisms of high-frequency drugs.Then,the potential active ingredients were analyzed.The whitening and anti-freckle effect was verified through cytotoxicity experiments and melanin content detection.Results A total of 171 external prescriptions were selected in eligible articles,including 261 Chinese medicinals,most of which were pungent and belong to the lung meridian.The most frequently used Chinese medicinals was"Erbai Yixin"(EBYX,Angelicae Dahuricae Radix,Typhonii Rhizoma,Asari Radix et Rhizoma).Network pharmacological analysis showed that the core targets of EBYX for whitening and removing freckles are TP53,EGFR,ALB,etc.,which are mainly involved in oxygen perception and response,skin immune regulation,skin cell growth,differentiation,stress,inflammatory response,and other biological processes.Based on the results of molecular docking,biological analysis proved that the active ingredients of EBYX are chrysophanol,gallic acid and caffeic acid,which have inhibitory effects on the proliferation of melanoma cells and melanin production.Conclusion Most of the ancient prescriptions for whitening and removing freckles are pungent and belong to the lung meridian,which embodies the theory of"lung governs the skin and hair".The high-frequency drug EBYX may play a role by regulating skin redox,immunity and inflammation.The active ingredients of EBYX have an inhibitory effect on melanin formation.This study enriches the scientific connotation of TCM whitening and freckle-removing prescriptions based on the theory of"lung governs the skin and hair",realizes interdisciplinary integration and provides support for the modernization of TCM.

Due to the complexity of traditional Chinese medicine (TCM) interventions and the diversity of herbal components, single-omics technologies such as genomics, transcriptomics, proteomics, and metabolomics often cannot comprehensively elucidate the scientific connotations of TCM. Multi-omics technologies driven by system biology can analyze the theoretical connotations and application mechanisms of TCM from different levels such as genes, gene expression, proteins, and metabolites, in line with the holistic view of TCM, which helps to promote the modernization of TCM. By reviewing the literature on the application of omics technologies in the field of TCM, it is found that multi-omics technologies have been widely used in TCM for syndrome differentiation, evaluation of herbal quality, elucidation of pharmacological mechanisms, and drug toxicity assessment, providing comprehensive explanations of the mechanisms of action of TCM and overcoming the limitations of single-omics technologies, and having obtained significant achievements. However, multi-omics technologies also face challenges such as high cost, difficulties in data analysis due to large data volumes, and insufficient translation of research results. In the future, it is expected that through strengthening interdisciplinary cooperation, conducting long-term and dynamic clinical research, standardizing and normalizing data analysis processes, adopting appropriate and reasonable multi-omics integration patterns, establishing multi-omics databases for TCM, revealing the individualized characteristics, therapeutic mechanisms, and disease regulatory networks of TCM, the modernization of TCM will be promoted.

The modernization of traditional Chinese medicine (TCM) theories is not only a prerequisite for the modernization of TCM but also a way to deal with the external environment and the difficulties of its development. The modernization of TCM is based on the premise of maintaining its own characteristics and advantages, and along with the progress of modern science and technology, the entire TCM has undergone new changes and improvements from theory to practice, and has become a scientific system with modern scientific level that adapts to the needs of the times. In the context of the unprecedented profound changes in contemporary TCM academics, this paper puts forward the definition of modernization of TCM theory and argues that the modernized theoretical system of TCM should maintain originality to build the foundation of TCM inheritance, clarify the frontier of the discipline and establish the conceptual system, form an open network to be compatible with multi-disciplinary interaction, incorporate the falsification and establish self-renewal system, creatively transform and innovatively develop, and modernize the interpretation and update the discourse system.

The long 19th century was a period of many developments and technical innovations in agriculture and animal biology, during which actors sought to incorporate new practices in light of new information. By the middle of the century, however, while heredity steadily became the dominant concept in animal husbandry, some policies related to livestock improvement in Brazil seemed to have been tailored following a climate-deterministic concept established in the mid-18th century by the French naturalist Georges-Louis Leclerc, the Comte de Buffon. His theory of animal degeneration posited, among other things, the necessity of recurrent crossbreeding to preserve animal species living in nonnative environments from climate-induced degeneration. Although largely discredited by the early 19th century, the teachings of the French naturalist seem to have found supporters in a Brazilian program to modernize national agriculture through the application of the natural sciences. Herein I examine the revival of Buffon's theories in that government-sponsored program to improve animal husbandry and breeding techniques, including actual applications of this theory in the real world. Ultimately, I argue that Buffon's theory of degeneration was used to tailor public policies and funding for the improvement of domesticated animals in Brazil between 1856 and 1860.

Public health data modernization efforts frequently overlook the far-reaching effects of structural racism across the data life cycle. Modernizing data requires creating data ecosystems grounded in six principles: dismantling structural racism and building community power explicitly; centering justice in all stages of data collection and analysis; ensuring communities can govern their data; driving positive population-level change; engaging nonprofit organizations; and obtaining commitments from governments to make changes in policy and practice. As government agencies spearhead and finance data modernization initiatives, it is imperative that they address structural racism head-on and integrate these principles into all aspects of their work.

Context: Public health survey systems are tools for informing public health programming and policy at the national, state, and local levels. Among the challenges states face with these kinds of surveys include concerns about the representativeness of communities of color and lack of community engagement in survey design, analysis, and interpretation of results or dissemination, which raises questions about their integrity and relevance. Approach: Using a data equity framework (rooted in antiracism and intersectionality), the purpose of this project was to describe a formative participatory assessment approach to address challenges in Oregon Behavioral Risk Factor Surveillance System (BRFSS) and Student Health Survey (SHS) data system by centering community partnership and leadership in (1) understanding and interpreting data; (2) identifying strengths, gaps, and limitations of data and methodologies; (3) facilitating community-led data collection on community-identified gaps in the data; and (4) developing recommendations. Results: Project team members' concerns, observations, and critiques are organized into six themes. Throughout this engagement process, community partners, including members of the project teams, shared a common concern: that these surveys reproduced the assumptions, norms, and methodologies of the dominant (White, individual centered) scientific approach and, in so doing, created further harm by excluding community knowledges and misrepresenting communities of color. Conclusions: Meaningful community leadership is needed for public health survey systems to provide more actionable pathways toward improving population health outcomes. A data equity approach means centering communities of color throughout survey cycles, which can strengthen the scientific integrity and relevance of these data to inform community health efforts.

The COVID-19 pandemic has exposed shortcomings in the US public health data system infrastructure, including incomplete or disparate processes related to data collection, management, sharing, and analysis. Public health data modernization is critical to ensure health equity is at the core of preparedness and response efforts and policies that prioritize equitable responses to health emergencies. To address the inequitable uptake and distribution of COVID-19 vaccinations in communities most disproportionately impacted by the pandemic, the CDC Foundation's Response Crisis and Preparedness Unit began partnering with community-based organizations in March 2021 to provide education and outreach and facilitate access to vaccines. These organizations engaged with partners and communities to address vaccine-related concerns, develop innovative and culturally appropriate communication strategies, and promote timely vaccination. Two grantees, Out Boulder County in Colorado and the Coalition of Asian American Leaders in Minnesota, experienced issues related to public health data collection standards and practices for COVID-19. Data collection tools often lack the appropriate or necessary demographic variables or level of disaggregation needed to be able to assess prioritization and disparities within racial and ethnic groups and across sexual orientation and gender identity categories. In this case study, both grantee organizations document their experiences, challenges, and strategies to overcome barriers to implementing their projects resulting from a lack of meaningful data. These examples identify inequities and systems-level changes related to data collection and surveillance, and they provide recommendations and lessons learned to improve data surveillance for more equitable public health responses.

RESUMO

A new psychiatric institution emerged in the late nineteenth and early twentieth centuries: the psychopathic hospital. This institution represented a significant development in the history of psychiatry, as it marked the profession's reorientation from asylum-based to hospital-based care, and in this way presaged the deinstitutionalization movement that would begin half a century later. Psychopathic hospitals were also an important marker of psychiatry's efforts to redefine its professional boundaries and respond to its vociferous critics. This entailed both a rapprochement with general medicine in an effort to assert its scientific bona fides and a redefinition of its scope of practice to absorb non-certifiable 'borderland' cases in order both to emphasize non-coercive treatment and to enlarge the profession's boundaries.

As part of its data modernization initiative (DMI), the Centers for Disease Control and Prevention, Division of Cancer Prevention and Control is testing and implementing innovative solutions to improve cancer surveillance data quality and timeliness. We describe a consensus-based effort to create a framework to guide the evaluation of cancer surveillance modernization efforts by addressing specific context, processes, and costs related to cancer registration. We drew on prior theories, consulted with experts, and sought feedback from cancer registry staff. We developed the cancer surveillance systems, context, outcomes, and process evaluation (CS-SCOPE) framework to explain the ways in which cancer registry data quality, timeliness, and efficiency are impacted by external and internal contextual factors and interrelated process and content factors. The framework includes implementation measures to understand acceptability of process changes along with outcome measures to assess DMI initiation and ongoing sustainability. The framework's components and structures can be tailored for use in other DMI evaluations.

Modernization of wastewater treatment plants is usually caused by their significant wear and changes in the flow rate and concentration of pollutants. If there is no initial data on the flow or pollution, their determination by calculation is required, which may lead to an increase in concentration. Within the study, the modernization of treatment facilities was estimated under conditions of reduced flow and increased pollution concentration. Calculations were carried out both manually and using the CapdetWorks software package. The focus was on secondary treatment facilities as the main element of the municipal wastewater treatment plant within their upgrade from only organic pollutants removal (plug-flow reactor) to removal of both organic pollutants and nutrients (technology of the University of Cape Town). The calculations of tank volumes have shown that the concentration of pollutants has a much greater impact on them than the change in flow, especially when improvement in the treatment quality is required. The study revealed that membrane sludge separation allows tanks to be reduced in volume by 1.5-2.5 times (depending on the value of mixed liquor suspended solids) in comparison with gravity separation, which means smaller capital costs. However, membrane application requires significant energy costs for membrane aeration. For the initial data of the study, the specific energy costs for aeration before the upgrade, after the upgrade (gravity separation), and after the upgrade (membrane separation) were 0.12 kWh/m3, 0.235 kWh/m3, and 0.3 kWh/m3, respectively. If the membrane lifetime is 10 years, membrane costs were determined to be 10-15% of the energy costs for aeration.

A new model of relations emerged in schools after the establishment of new educational centers in Iran in the mid-Qajar era. The cultural authorities of the time were particularly interested in school health, which was adopted as a principle from the European, particularly French, school systems. During the period 1925 to 1941, with special attention to the new Western models of education, school health changed more and more. This study provides a descriptive-analytical report on state health policies in girl schools in Tehran, Iran, from 1927 to 1934, based on health records available at the National Archives of Iran. The findings reveal that since the mid-first Pahlavi era, officials from the Ministry of Science became increasingly involved in the issue of health, which resulted in institutionalization of health and medical examination of students, establishment of the School Health Office, publication of theoretical health discussions in magazines, and teaching of health principles to students, thereby improving the level of health in Tehran schools during the study period. The study aims to provide Iranian physicians and health policymakers with a review of this historical experience.
