INTRODUCTION AND OBJECTIVES: Our objective was to measure and compare the intake of macro and micronutrients in a cohort of individuals with Metabolic Syndrome Associated Steatotic Liver Disease (MASLD) compared with matched controls to identify areas of further research in this area; we identified nutrition-associated associations with MASLD in the United States general population. MATERIALS AND METHODS: We used the 2017 - 2018 NHANES dataset. Elastography Controlled Attenuation Parameter (CAP score>280) in the absence of other liver disease was defined as MASLD in adults (>18). Advanced fibrosis was defined by transient elastography >10 kPa. Controls were adults without liver disease. RESULTS: 1648 MASLD cases (11.4 % advanced fibrosis) and 2527 controls were identified. MASLD cases were older (P<0.001), more likely males (P = 0.01), less likely to have a college education (P = 0.04) and more likely married (P = 0.002). MASLD cases were more likely to be of Mexican American or Hispanic ethnicity (P = 0.002), have higher BMI, and have higher prevalence of diabetes, hyperlipidemia and hypertension (P<0.001 for all). MASLD cases had higher hs-CRP (P = 0.02) and ferritin (P = 0.02). MASLD cases had lower total (P = 0.004) and added vitamin E in their diet (P = 0.002), lower vitamin K intake (P = 0.005), and higher selenium intake (P = 0.03). Caloric intake (P = 0.04), carbohydrate intake (P = 0.02), cholesterol intake (P = 0.03) and saturated fatty acid intake (P = 0.05) were higher in MASLD. Individuals with MASLD were more likely to be on a diet (P<0.001), sedentary (P = 0.008) and less likely to participate in moderate or vigorous recreational activities (P<0.001). CONCLUSIONS: The deficiencies of micronutrients and excess of macronutrients point to oxidative stress, pro-inflammatory state, and lipotoxicity as pathways linking the US diet to MASLD. MASLD patients are more often on special diets, which may reflect prior provider counseling on diet changes to improve health.

The aim of this study was to examine the relationship between dietary contribution of ultra-processed foods and urinary phytoestrogen concentrations in the US. Participants from cross-sectional 2009-2010 National Health and Nutrition Examination Survey aged 6+ years, selected to measure urinary phytoestrogens and with one 24-h dietary recall were evaluated (2692 participants). Food items were classified according to NOVA (a name, not an acronym), a four-group food classification based on the extent and purpose of industrial food processing. Ultra-processed foods are formulations manufactured using several ingredients and a series of processes (hence "ultra-processed"). Most of their ingredients are lower-cost industrial sources of dietary energy and nutrients, with additives used for the purpose of imitating sensorial qualities of minimally processed foods or of culinary preparations of these foods. Studied phytoestrogens included lignans (enterolactone and enterodiol) and isoflavones (genistein, daidzein, O-desmethylangolensin and equol). Gaussian regression was used to compare average urinary phytoestrogen concentrations (normalized by creatinine) across quintiles of energy share of ultra-processed foods. Models incorporated survey sample weights and were adjusted for age, sex, race/ethnicity, family income, and education, among other factors. Adjusted enterodiol geometric means decreased monotonically from 60.6 in the lowest quintile to 35.1 µg/g creatinine in the highest, while adjusted enterolactone geometric means dropped from 281.1 to 200.1 across the same quintiles, respectively. No significant linear trend was observed in the association between these quintiles and isoflavone concentrations. This finding reinforces the existing evidence regarding the negative impact of ultra-processed food consumption on the overall quality of the diet and expands it to include non-nutrients such as lignans.

STUDY OBJECTIVES: Decreased sleep duration and quality are associated with poor health. Given that Mexico-born US immigrants (MI) often have favorable health status relative to the general US population (USALL), we tested the hypothesis that MI have better sleep as compared to USALL and to Mexican-Americans (MA) born in the US. RESEARCH DESIGN AND METHODS: Cross-sectional analysis of a stratified multistage probability sample of 18 years and older civilian non-institutionalized USALL enrolled in NHANES during 2005-2006. Age-adjusted population prevalence rates and adjusted odd ratios of short habitual sleep time (SHST) and insomnia were calculated using weighted analyses. SETTING: Computer assisted personal bilingual (English/Spanish) interviews in the participants' home using a sleep questionnaire. PARTICIPANTS: 5160 USALL participants, including 1046 MA, of whom 620 were MI. RESULTS: In logistic regression models unadjusted and adjusted for socioeconomic characteristics, health related behavior risk factors, health status, and depression, MI status was significantly associated with lower odds of SHST (OR = 0.7, 95%CI [0.6-0.9]), insomnia (OR = 0.3, 95%CI [0.2-0.5]), and sleep-associated functional impairments (OR = 0.4, 95%CI [0.2-0.8]). In MA males, MI status was associated with reduced odds of SHST < 7 h/weeknight and insomnia symptoms. Cultural changes measured by increased levels of English compared to Spanish language spoken at home were associated with an increased risk of poor sleep in MA men in univariate models, and may partially explain better sleep in MI men compared to their US-born counterparts. CONCLUSION: Compared to the general US poplulation, Mexican-born US immigrants have more favorable sleep, possibly contributing to their lower reported risk of diabetes, hypertension, CVD, and all-cause mortality (the "Hispanic Paradox").
