RESUMO

OBJECTIVE: To analyze the use of packed red blood cells (PRBCs) for patients with pelvic fracture and evaluate factors associated with PRBC transfusion for patients with pelvic fracture. METHODS: This retrospective cohort study collected 551 patients with pelvic fractures from six hospitals between September 1, 2012, and June 31, 2019. The age span of patients varied from 10 to 95 years old, and they were classified into two groups based on high-energy pelvic fractures (HE-PFs) or low-energy pelvic fractures (LE-PFs). The study's outcome was the use of PRBCs, fresh frozen plasma (FFP), and albumin. Demographic data, characteristics, laboratory tests, clinical treatment details, and clinical outcomes were compared between the two groups. Factors that were statistically associated with perioperative PRBCs in univariate analyses were included to conduct an optimal scale regression to determine the independent factors for perioperative PRBCs. RESULTS: A total of 551 patients were screened from six hospitals, and after inclusion and exclusion, 319 were finally included and finished the follow-up from admission to discharge, while four patients died during hospitalization. Three hundred and nineteen patients were classified into two groups by their injury mechanisms. A total of 230/319 (72.1%) patients were classified into the HE-PF group, and 89/319 (27.8%) patients were classified into the LE-PF group. Patients in the HE-PF group were transfused with 4.5 (3-8) units of PRBCs, 300 (0-600) ml of FFP, and 0 (0-30) g of albumin, while patients in the LE-PF group were transfused with 3.5 (2-4.5) units of PRBCs, 0 (0-295) ml of FFP, and 0 (0-0) g of albumin (all P < 0.001). There were higher proportions of male patients and patients under 65 in the HE-PF group (all P < 0.001). HE-PF group patients were more severely injured and likely to take external fixation. The optimal scale regression revealed four significant factors associated with perioperative transfused PRBCs, which were patients on admission with hemorrhagic shock (importance = 0.283, P = 0.004), followed by fracture types identified by Tile classification (importance = 0.156, P < 0.001), hemoglobin levels below 70 g/L on admission (importance = 0.283, P = 0.004), followed by fracture types identified by Tile classification (importance = 0.156, P < 0.001), hemoglobin levels below 70 g/L on admission (importance = 0.148, P = 0.039), and methods of pelvic fixation (importance = 0.008, P = 0.026), ranked by the importance. CONCLUSION: Patients with HE-PFs had increased transfusions of PRBCs, FFP, and albumin, and hemorrhagic shock on admission, Tile classification, Hb levels, and stabilization methods were found to be associated with perioperative PRBCs.

OBJECTIVE: This study evaluated the pelvic ring fractures and injuries in patients admitted to and treated at this ward between August, 2012 and January, 2014. METHODS: 66 patients were submitted to treatment protocols according to their age, gender, skin color, injury mechanism, location of the trauma, classification of their injuries, emergency intervention, associated injuries, injured side of the body, treatment, and mortality. The most relevant data were classified according to statistic procedures, such as Goodman's association test. Measures were compared with Student's t-test and analysis of variance associated with Tukey's multiple comparison test. RESULTS: The mean age was 47 years; white race and male gender were most common. Car or truck accident was the most common cause of injuries, which occurred mainly in urban sites. Type A injuries were the most frequent. 16.6% of the cases were submitted to emergency surgery. 42.4% displayed associated injuries. The right side of the body was the most commonly affected side. Non-invasive treatment was most commonly used. Death was the outcome in 3% of the cases, associated to high-energy trauma. CONCLUSIONS: Pelvic ring fractures and injuries are more often verified among males. In general and among younger individuals, traffic accidents are the most common cause of the injury, while among the elderly, ordinary falls are the most commonly verified cause. The majority of those injuries are suffered in urban areas. Type A fractures are more frequent. The majority of cases do not require emergency intervention nor do they feature associated injuries. Non-invasive treatment is most common and death outcomes are associated to high-energy traumas with severe injuries.


OBJETIVO: Estudo das fraturas/lesões do anel pélvico atendidas e tratadas neste serviço de agosto de 2012 a janeiro de 2014. MÉTODOS: Elaborou-se um protocolo para os 66 pacientes, consideraram-se os dados: idade, sexo, cor, mecanismo da lesão, local do trauma, classificação das lesões, intervenção de urgência, lesões associadas, lado acometido, tratamento e óbito. Para os dados de maior interesse foram usados os procedimentos estatísticos que envolveram o teste de associação de Goodman e as técnicas de comparações de medidas por meio do teste t de Student e da análise de variância complementada com as comparações múltiplas de Tukey. RESULTADOS: A idade média foi de 47 anos; pacientes do sexo masculino e brancos foram mais frequentes. A causa mais comum das lesões foi acidente carro/caminhão e a zona urbana foi o local onde elas mais ocorreram. Fraturas tipo A foram as mais frequentes. Em 16,6% dos pacientes, foi necessária cirurgia de urgência e 42,4% apresentaram lesão associada. O lado direito foi mais acometido. O tratamento incruento foi o mais usado e o óbito ocorreu em 3%, em casos de trauma de alta energia. CONCLUSÕES: As fraturas/lesões do anel pélvico são mais frequentes no sexo masculino. De modo geral e em jovens, o acidente de trânsito é o mecanismo mais frequente, já em idosos é queda banal. A maioria das lesões ocorre na zona urbana. Fraturas do tipo A são as mais frequentes. A maioria não necessita de intervenção de urgência e não apresenta lesões associadas. O tratamento incruento é o mais usado e os óbitos estão associados a trauma de alta energia com graves lesões associadas.

RESUMO

Objective To review a treatment of Tile B and Tile C Morel-Lavallée lesions with a texas scottish rite hospital (TSRH) and external fixation.Methods From September 2008 to September 2013,21 patients were operated with a treatment with TSRH system and external fixation.Results Those 21 cases were followed up from 12 to 24 months with an average of 15.8 months.All fractures were healed without osteomyelitis and patients showed no sing of limp.The rate of good recovery was reached 80.9%(17/21).Conclusions Internal fixation with TSRH system and external fixation for treating Tile B and Tile C Morel-Lavallée lesions can result in a high success rate.
