Background In 2018, the World Health Organisation (WHO) released interim guidelines, advising a change of regimens to dolutegravir-based first- and second-line antiretroviral therapy (ART), based on which, in 2021, the National Aids Control Organisation (NACO) updated its guidelines to include the tenofovir + lamivudine + dolutegravir (TLD) regimen as a first line of therapy for all people living with HIV (PLHIV) and second- and third-line regimens to dolutegravir-based regimens. Considering this change of regimen, the adverse drug reaction (ADR) profiling and longitudinal prescription pattern of antiretroviral and concomitant medications in adult patients at the ART centre of a tertiary care hospital were assessed in this study. Methods Ninety-seven PLHIV out of all the patients who attended the ART centre from September 2021 to July 2022 were enrolled and followed up for six months. The ADRs that occurred during this period were collected along with details of prescription patterns and analyzed by descriptive statistics. Causality assessment for ADR was done using the World Health Organisation-Uppsala Monitoring Centre (WHO-UMC) scale. Results Seventy-eight percent (n=76 out of 97) of patients experienced at least one ADR, and 128 ADRs were seen in 97 patients. The most common ADRs were increased alkaline phosphatase (39.0%, n=128), dyslipidaemia (12.5%, n=128), and nephrotoxicity (10.1%, n=128). The drug most suspected of causing ADRs was dolutegravir (27.5%, n=342). The most common therapeutic regimen was TLD (71.2%, n=97). The most prescribed drug was lamivudine (30.6%, n=1183). The most prescribed concomitant medication was cotrimoxazole (15%, n=312). Conclusions Dolutegravir-based regimens have been implemented for PLHIV in a phased-out manner from previous non-dolutegravir-based ART regimens, which is in line with the recent NACO guidelines. However, it has also led to an increase in dolutegravir-associated ADRs like increased alkaline phosphatase, dyslipidaemia, and nephrotoxicity. Continuous monitoring of prescriptions and ADRs can add to our knowledge regarding their use and ADRs.

RESUMO

Tourette's syndrome (TS) is a neuropsychiatric disease featuring tics and vocal tics, with a prevalence of approximately 1%, including 75% of the total number of male patients. TS seriously disturbs the patients' career, education, and life and brings a serious and unbearable psychological burden to the patients themselves and their families. At present, there are no specific clinical medications recommended for treating TS. Therefore, it is necessary to select the appropriate medication for symptomatic treatment based on the doctor's personal experience and the patient's symptoms, with the main goal of relieving symptoms, thus improving the patient's social skills and psychological problems. Here we conducted a comprehensive search on PubMed to review and organize the history and current status of the development of drug therapy for TS through a timeline format. We also systematically evaluated the effects of each drug for TS treatment to summarize the current problems and new research directions and to provide some ideas for clinical treatment.

Objective: to assess the adherence of people with type 1 diabetes to self-care activities. Method: quantitative study with 60 diabetics, from Porto, Portugal. The Self-Care Scale for Diabetes in its version translated and adapted to Portuguese was applied in January to February 2019. Results: respondents are mostly young (36.7%), employed (63.4%), female (73.3%), married (36.7%), high educated (60.0%) and diagnosed with recent diabetes (50.0%). Most have good mean adherence to self-care behaviors related to food (5.0), blood glucose monitoring (6.8), foot care (5.6) and medication (6.4). Regarding physical activity (2.6) and smoking, 73.3% of respondents say they smoked a cigarette in the last seven days lower adherence was identified. Conclusions: there is a need to develop educational programs to ensure a higher adherence to healthy lifestyles.(AU)


Objetivo: avaliar a adesão de pessoas com diabetes tipo 1 às atividades de autocuidado. Método: estudo com 60 diabéticos, do Porto, Portugal. Foi aplicada a Escala de Autocuidado para Diabetes traduzida e adaptada para o português em janeiro a fevereiro de 2019. Resultados: os inquiridos são maioritariamente jovens (36,7%), empregados (63,4%), mulheres (36,7%), casados (36,7%), com ensino superior (60,0%) e com diagnóstico recente de diabetes (50,0%). A maioria tem boa adesão aos comportamentos de autocuidado relacionados à alimentação (5,0), monitoramento da glicemia (6,8), cuidados com os pés (5,6) e medicamentos (6,4). Em relação à atividade física (2,6) e ao tabagismo, 73,3% dos entrevistados afirmam que fumaram um cigarro nos últimos sete dias foi identificada menor adesão. Conclusões: há necessidade de desenvolvimento de programas educativos que garantam uma maior adesão aos estilos de vida saudáveis.(AU)

Se aplicó la Escala de Autocuidado de la Diabetes y se tradujo al portugués en enero a febrero de 2019. Resultados: los encuestados son principalmente jóvenes (36,7%), empleados (63,4%), mujeres (36,7%), casados (36,7%), con estudios superiores (60,0%) y con diagnóstico reciente de diabetes (50,0%). La mayoría tiene una buena adherencia al autocuidado relacionado con la alimentación (5,0), control de la glucosa en sangre (6,8), cuidado de los pies (5,6) y medicación (6,4). En cuanto a actividad física (2,6) y tabaquismo, 73,3% de los encuestados dice haber fumado un cigarrillo en los últimos siete días, se identificó menor adherencia. RESUMEN Introducción: La extravasación endovenosa de citostáticos es una grave complicación del tratamiento en el paciente con cáncer, que puede provocar graves daños tisulares. Objetivo: Sistematizar las medidas de prevención y tratamiento a tomar por los profesionales de enfermería en la extravasación de quimioterapia intravenosa. Métodos: Revisión bibliográfica sistemática con análisis de contenido de documentos originales y de revisión publicados en español e inglés desde 2009 a 2016, con las siguientes palabras clave: "extravasación", "quimioterapia", "medidas de prevención", "tratamiento de extravasación" en SciELO. Se identificaron y revisaron artículos que fueron útiles para el desarrollo de la revisión, así como monografías de varias revistas y tesis doctorales actualizadas que permitieron el análisis histórico lógico de los criterios que sustentan medidas de prevención y tratamiento en la extravasación de quimioterapia intravenosa. Conclusiones: La prevención de esta complicación ha demostrado ser la mejor medida para evitar el daño a los tejidos, sin embrago existen tratamientos alternativos que el personal de enfermería debe conocer y aplicar con rapidez y eficiencia(AU)


ABSTRACT Introduction: Intravenous cytostatic extravasation is a serious complication of treatment in cancer patients, which can cause serious tissue damage. Objective: To systematize the prevention and treatment measures to be taken by nursing professionals in case of extravasation of intravenous chemotherapy. Methods: Systematic bibliographic review with content analysis of original and review documents published in Spanish and English in SciELO from 2009 to 2016, with the following keywords: extravasación [extravasation], quimioterapia [chemotherapy], medidas de prevención[prevention measures], tratamiento de extravasación[treatment of extravasation]. Articles that were useful for the development of the review were identified and consulted, as well as monographs of several updated journals and doctoral theses, which allowed the logical-historical analysis of the criteria that support prevention and treatment measures in the extravasation of intravenous chemotherapy. Conclusions: The prevention of this complication has proven to be the best measure to avoid tissue damage; however, there are alternative treatments that nurses should know and apply quickly and efficiently(AU)

Medications for obesity management can be divided into 4 groups: antidepressants (naltrexone/bupropion), stimulants (phentermine, phendimetrazine, diethylpropion, phentermine/topiramate), fat blockers (orlistat), and diabetes medications (liraglutide). Each group has specific therapeutic effects, adverse effects, and costs. Two medications are indicated for long-term use in obesity: lorcaserin and orlistat. Other available medications are for short-term use. High cost makes many of these medications inaccessible for underserved and poor patients. Because of misuse potential, many obesity medications are also classified as controlled substances. There are no medications currently approved for use in pregnant or lactating women.

Dyslipidemia often affects overweight and obese adolescents and can be present along with hypertension, insulin resistance, type 2 diabetes, and polycystic ovarian syndrome. This article is the third of six discussing the comorbidities of childhood obesity and will focus on the individual parts of the lipid profile and the impact of dyslipidemia on the heart and other body systems. Since few pharmacologic therapies are approved to treat dyslipidemia in children and adolescents younger than 18, treatment consists of lifestyle changes that can be supported and modeled by the school nurse. The school nurse can also be an advocate for a healthy lifestyle in the school district and community. More success in the treatment of dyslipidemia will be realized with less attention to changing the individual and more attention to changing the wider populations, including schools and the community.
