RESUMO

El presente documento supone una puesta al día del documento multidisciplinar HEMOMAS, publicado en el año 2016 con el aval de las Sociedades Científicas Españolas de Anestesiología y Reanimación (SEDAR), Medicina Intensiva, Crítica y Unidades Coronarias (SEMICYUC) y de Trombosis y Hemostasia (SETH). El objetivo de este documento fue revisar y actualizar las recomendaciones existentes sobre el manejo de la hemorragia masiva (HM). Se siguió una metodología basada en elementos del método ADAPTE (búsqueda y adaptación de guías publicadas en el ámbito específico de la HM desde 2014, más búsqueda bibliográfica en PubMed y EMBASE desde enero-2014 hasta junio-2021). Tras la revisión de nueve guías y 207 artículos seleccionados, se actualizaron las 47 recomendaciones existentes en el artículo original, manteniendo, suprimiendo o modificando cada una de ellas y sus grados de recomendación y evidencia. Consensuadamente, los autores aprobaron la redacción final del artículo y las 41 recomendaciones resultantes.(AU)

Introducción: actualmente, la indicación principal para la transfusión de FFP es corregir la deficiencia de los factores de coagulación en pacientes con hemorragia activa o sugestión de esta, sin embargo, la práctica clínica ha demostrado que un porcentaje grande de las transfusiones de FFP en neonatología no siguen las recomendaciones de las guías actuales y, en su mayoría, son innecesarias. Objetivo: ampliar el conocimiento que se tiene sobre la transfusión de FFP en neonatología y la implementación de la tromboelastografía para evitar realizar estas intervenciones. Metodología: se realizó una revisión de la literatura en la base de datos PubMed y Elsevier, usando palabras clave como tromboelastografía, transfusión de plasma en neonatos, hemostasia neonatal. Conclusión: se evidenció que los tiempos de coagulación de los neonatos y prematuros sanos son más prolongados en comparación a los adultos, por ello facilita su errada cuando se habla de transfusión de FFP, de ahí que la TEG sea una buena herramienta para evaluar el estado coagulante de los neonatos de manera integral, sin abrir paso a errores de interpretación y facilitando la decisión de tratamientos en los pacientes en UCIN, antes de requerir transfusiones de FFP.


Introduction: Currently the main indication for FFP transfusion is to correct coagulation factor deficiency in patients with active bleeding or its suggestion, however, clinical practice has shown that a large percentage of FFP transfusions in neonatology do not they follow current guideline recommendations and are mostly unnecessary. Objective: To expand the knowledge about these transfusions in neonatology and the implementation of thrombelastography to avoid performing these interventions. Methodology: A review of the literature was carried out in the PubMed and Elsevier databases, using keywords such as "thrombelastography", "plasma transfusion in neonates", "neonatal hemostasis". Conclusion: It was evidenced that the coagulation times of healthy neonates and premature infants are longer compared to adults, thus facilitating their misinterpretation and limiting their use when talking about FFP transfusion, therefore TEG is a good tool. to evaluate the coagulation interpretación y limita su utilización status of neonates in a comprehensive manner, without giving way to interpretation errors and facilitating treatment decisions in patients in the NICU before requiring FFP transfusions.

Resumen La púrpura fulminans (PF) neonatal es un estado de hipercoagulabilidad poco frecuente pero grave. Su presentación clínica es súbita con lesiones purpúricas-necróticas que pueden dejar secuelas permanentes o incluso tener una evolución fatal. Se caracteriza por trombosis en la microcirculación de la piel acompañada de hemorragia perivascular. Los sitios más afectados son las extremidades pélvicas, torácicas o las zonas de presión. La alteración funcional más común es el defecto de la proteína с que fisiológicamente regula la coagulación, el defecto puede ser de causa primaria o secundaria. Caso clínico: Recién nacido varón con 8 días de vida extrauterina que presenta súbitamente rechazo a la vía oral, irritabilidad y fiebre de 39 °С. Dos días después es hospitalizado por deshidratación y rechazo a la vía oral. Al ingreso no se documentó fiebre o foco infeccioso. A las 24 horas presentó lesiones purpúricas-necróticas en el pie derecho. Se realizó un ultrasonido Doppler que confirmó trombosis venosa y arterial. Los dímeros Deran positivos. Se dio tratamiento con plasma fresco congelado (PFC), anticoagulante y antiplaquetario con buena respuesta. Conclusión: La PF es un estado protrombótico grave que requiere un diagnóstico y tratamiento oportunos para mejorar el pronostico.


Abstract Neonatal Purpura Fulminans (PF) is an infrequent hypercoagulable state but very severe. Its clinical manifestation is sudden with purpuric-necrotic injuries. It can leave permanent sequels oreven have a fatal evolution. It is characterized by thrombosis in the skin's microcirculation, accompanied with perivascular hemorrhage. The most affected areas are the pelvic and thoracic limbs, and pressure zones. The most common molecular alteration is the protein с defect, which physiologically regulates coagulation; the defect can be of a primary or secondary cause. Case report: A male newborn with 8 days of extrauterine life suddenly presents oral rejection, irritability and a 39° с fever. Two days later, he was hospitalized for dehydration and oral rejection. He didn't show signs of fever or infection at the time of his admission. Twenty-four hours after his entry, he presented purpuric-necrotic injuries in the right foot, hence, he was diagnosed with purpura fulminans. D-dimer studies and doppler ultrasound were taken. They confirmed venous and arterial thrombosis. The treatment was initiated with fresh frozen plasma, an anticoagulant and an antiplatelet, with a good response. Conclusion: PF is a serious hypercoagulable state that requires an early diagnosis and therapy to improve the outcome.

Resumen Lugar: Fundación Cardioinfantil-instituto de Cardiología (FCI), Bogotá, Colombia. Objetivo: determinar los factores asociados con eventos adversos transfusionales (EAT) con plasma fresco congelado (PFC) en la FCI durante los años 2008-2010. Metodología: estudio de casos y controles, en relación 1:3. Pacientes mayores de 18 años transfundidos con PFC en la FCI durante los años 2008-2010. Los casos fueron identificados a partir del registro de EAT de la FCI. Los controles recibieron transfusión de PFC y no presentaron evento adverso. Resultados: el promedio de edad de los casos fue menor que el de los controles (52.4 vs. 60.5 años, p=0.001). los eventos adversos de tipo alérgico fueron los más frecuentes, aumentaron con la utilización de unidades provenientes de otros bancos de sangre (0.7 vs. 0.0 unidades, p=0.003), cuando el donante fue de sexo femenino (2.5 vs. 1.4 donantes de sexo femenino, p=0.11) y con la paridad aumentada de dichas donantes (4.0 vs. 2.6 partos, p=0.04). La sepsis (or: 0.1, iC95%:0.01-0.87, p=0.015) y la diabetes (or: 0.9, iC95%:0.01-0.54, p=0.003) se comportaron como factores protectores para el desarrollo de los eventos adversos de tipo alérgico. Conclusiones: los EAT con PFC están asociados con donantes mujeres, multíparas y unidades provenientes de bancos de sangre no institucionales. su incidencia podría disminuir al no permitir donantes mujeres con más de cuatro embarazos. Abstract Place: Fundación Cardio-infantil (FCI), Bogotá, Colombia. Objective: to determine factors associated with transfusion-associated adverse events with fresh frozen plasma (FFP) in the FCI during the years 2008-2010. Methodology: case-control study, in ratio 1:3. Patients over 18 years transfused with FFP in the FCI during the years 2008-2010. Cases were identified from the FCI transfusion -associated adverse events registry. Controls received FFP transfusion and had no adverse events. Results: the average age of cases was lower than that of controls (52.4 vs. 60.5 years, p = 0.001). The adverse events were the most frequent ones, and increased with the use of units from other blood banks (0.7 vs. 0.0 units, p = 0.003), when the donor was female (2.5 vs. 1.4 female donors, p = 0.11) and with the increased parity of such donors (4.0 vs. 2.6 deliveries, p = 0.04). Sepsis (or: 0.1, 95% Ci:0,01-0, 87, p = 0.015) and diabetes (or: 0.9, 95% Ci:0,01-0, 54, p = 0.003) behaved as protective factors for the development of adverse allergic type events. Conclusions: the transfusion-associated adverse events with PFC are associated with female multiparous donors and with blood units provided by non-institutional blood banks. Its incidence could diminish by not allowing female donors with more than four pregnancies. (Acta Med Colomb 2013; 38: 127-131).
