OBJECTIVE: The aim of this study was to assess whether the micronutrients zinc and copper, provided by human milk additives, are sufficient for very low birth weight preterm infants. METHOD: A phase 1 randomized double-blind controlled trial was conducted with very low birth weight preterm infants. This is a secondary analysis of copper and zinc. Sixty-six newborns were part of the initial sample, with forty participating and reaching the final stage of the study. Inclusion criteria were: gestational age less than 37 weeks, birth weight greater than or equal to 750 g and less than or equal to 1500 g, small or appropriate for gestational age, exclusively receiving human milk at a volume greater than or equal to 100 mL per kilogram per day, and hemodynamically stable. Participants were randomly assigned to two groups: intervention, Lioneo (received human milk with additive based on lyophilized human milk), n = 20, and control, HMCA (received human milk with commercial additive based on cow's milk protein), n = 20, and their serum levels of zinc and copper were measured on the first and twenty-first days. RESULTS: There was a reduction in intragroup zinc serum levels from the first to the twenty-first day of the study (p < 0.01). There was no intergroup difference. No difference was found in serum copper levels. CONCLUSION: Human milk additives were not sufficient to maintain adequate zinc serum levels in very low birth weight newborns. It was not possible to affirm whether human milk additives were sufficient to maintain adequate serum copper levels in the studied sample. UTN: U1111-1220-0550.

Assessing growth quality in preterm infants present challenges, particularly with the use of the standard fat-free mass measurement. We report here a moderate correlation between indirect skeletal muscle mass measurements using the D3-creatine dilution method and fat-free mass measured with air-displacement plethysmography. Skeletal muscle mass could serve as an indicator of growth quality.

OBJECTIVE: To describe recent trend in procedural closure of the patent ductus arteriosus (PDA) among premature infants and compare the clinical characteristics of infants receiving surgical vs transcatheter closure. STUDY DESIGN: We conducted a descriptive, retrospective cohort study of preterm infants born between 220/7 and 296/7 weeks' gestation from 2014 through 2021. Infants were identified from the Pediatrix Clinical Data Warehouse. We excluded infants with any major congenital anomaly. We identified all preterm infants with a PDA and all those who underwent procedural closure (surgical ligation or transcatheter occlusion) and compared changes over time using ANOVA for continuous variables and the Cochran-Armitage trend test to evaluate time-related changes in proportions. RESULTS: The study cohort included 64â580 infants, of whom 24â028 (37.2%) were diagnosed with a PDA. The number of infants receiving any procedural closure of the PDA decreased from 371 (4.4%) in 2014 to 144 (1.9%) in 2021. During the same period, number of surgical ligations decreased from 369 (4.36%) to 64 (0.84%), and the number of transcatheter occlusions increased from 2 (0.02%) to 80 (1.05% p for all < 0.001). The median age at time of surgical ligation increased from 25 days (10th and 90th percentile, 10, 61) to 31 days (10th and 90th percentile, 16, 66), and the median age of transcatheter occlusion decreased from 103 days (10th and 90th percentile, 32, 150) to 43 days (10th and 90th percentile, 22, 91). CONCLUSIONS: There was a decrease in surgical closure and an increase in transcatheter occlusion of the PDA in infants born at 22-30 weeks' gestation from 2014 to 2021. Despite the decline in overall procedural closure, the rate of transcatheter occlusion surpassed surgical ligation by 2021. Narrowing differences in the median age and weight at closure suggest increasing overlap in the types of infants who received each type of procedural closure.

Abstract Objectives To evaluate the therapeutic effect of Saccharomyces boulardii supplementation on jaundice in premature infants undergoing phototherapy. Methods In this article, the authors reviewed 100 hospitalized jaundiced premature infants under 35 weeks of gestational age. All infants were assigned to a control group (n= 45) and a treatment group (n= 55) randomly. The infants in the treatment group received S. boulardii supplementation by undergoing phototherapy and the infants in the control group were only treated by phototherapy. The total serum bilirubin levels were detected before and at the end of phototherapy, and transcutaneous bilirubin levels were measured on the 1st, 4th, 8th and 15th day of treatment. The duration of jaundice resolution and phototherapy, stool frequency, and characteristics were compared after phototherapy. Results The duration of jaundice resolution and phototherapy were shortened. Total serum bilirubin level was lower than the control group at the end of phototherapy (p < 0.05). Transcutaneous bilirubin levels decreased more significantly on the 8th and 15th day of treatment (p < 0.05), while there were no significant differences on the post-treatment 1st and 4th day (p > 0.05). In addition, bowel movements including stool frequency and Bristol Stool Form Scale ratings of stools also improved after treatment. Conclusions S. boulardii in combination with phototherapy is effective and safe in reducing bilirubin levels and duration of phototherapy, accelerating jaundice resolution in premature infants with jaundice. The procedure also provided an ideal therapeutic effect of diarrhea induced by phototherapy to promote compliance and maternal-infant bonding.

OBJECTIVE: To examine associations between early exposure to mother's own milk (MOM) and neurodevelopmental outcomes among preterm infants, and to compare these associations between singletons and twins. STUDY DESIGN: Retrospective cohort study that included low-risk infants born at <32 weeks gestational age. Nutrition was documented over a 3-day period at mean ages of 14 and 28 days of life; an average of the 3 days was calculated. The Griffiths Mental Development Scales (GMDS) were administered at 12 months corrected age. RESULTS: Preterm infants (n = 131) with median gestational age of 30.6 weeks were included; 56 (42.7%) were singletons. On days 14 and 28 of life, 80.9% and 77.1% were exposed to MOM, respectively. Exposure rate was comparable, but MOM intake (mL/kg/day) was higher among singletons than among twins (P < .05). At both time points, MOM-exposed infants scored higher on personal-social, hearing-language, and total GMDS assessments than nonexposed infants. These differences were significant for the entire cohort and for twins (P < .05). MOM intake correlated with total GMDS score for both singletons and twins. Any exposure to MOM was associated with additional 6-7 points on total GMDS score or 2-3 additional points for every 50 mL/kg/day of MOM. CONCLUSIONS: The study supports the positive association between early MOM exposure among low-risk preterm infants and neurodevelopmental outcomes at 12 months corrected age. The differential effects of MOM exposure on singletons vs twins need further exploration.

RESUMEN Objetivo Caracterizar, desde las experiencias propias, las dinámicas de la interacción entre los profesionales vinculados a la Unidad de Cuidados Intensivos Neonatales de Hospitales Públicos de Colombia y la estrategia de Bancos de Leche Humana en torno a la alimentación de los prematuros. Método Estudio cualitativo con enfoque fenomenológico interpretativo, en el que participaron 43 profesionales de la salud de 8 Instituciones Prestadoras de Salud que cuentan con Unidad de Cuidado Intensivo Neonatal y Banco de Leche Humana en Colombia. Resultados Las dinámicas de interacción entre los profesionales de la salud de las Unidades de Cuidados Intensivos Neonatales y los Bancos de Leche Humana relacionadas con la alimentación de los prematuros están mediadas por prácticas que vinculan competencias, materialidades y sentidos atravesados por la utilización o no de la leche humana en el tratamiento de los mismos. Discusión La disponibilidad de Bancos de Leche Humana en instituciones prestadoras de salud que brindan atención a recién nacidos prematuros posibilita la existencia y el despliegue de unas indicaciones nutricionales para esta población que van en línea con los protocolos diseñados para su tratamiento. Sin embargo, las decisiones tomadas en esta interacción están matizadas por debates que van surgiendo en el proceso, los cuales, lejos de ser una barrera para el uso de los Bancos de Leche Humana, se convierten en herramientas de vital importancia en la construcción de conocimiento en lo que respecta a la práctica de alimentación de los recién nacidos prematuros y la valoración de la estrategia.
ABSTRACT Objective To characterize from their own experiences the dynamics of the interaction between the professionals linked to the Neonatal Intensive Care Unit of Public Hospitals in Colombia and the Human Milk Bank strategy regarding the feeding of premature infants. Methods Qualitative study with an interpretative phenomenological approach, in which 43 health professionals from 8 health care institutions with Neonatal Intensive Care Unit and Human Milk Bank in Colombia participated. Results The dynamics of interaction between health professionals in Neonatal Intensive Care Units and Human Milk Banks related to the feeding of premature infants are mediated by practices that link competencies, materialities and meanings which are crossed by the use or not of human milk in the treatment of premature infants. Discussion The availability of Human Milk Banks in health care institutions that pro-vide care to premature newborns makes possible the existence and development of nutritional indications for this population that are in line with the protocols designed for their treatment. However, the decisions taken in this interaction are tinged by some debates that arise in the process, which far from being a barrier to the use of Human Milk Banks, become tools of vital importance in the construction of knowledge regarding the feeding practice of preterm newborns and the assessment of the strategy.

BACKGROUND: As general spontaneous movements at the writhing and fidgety ages have been important for the early identification of neurodevelopmental impairment of both full-term and preterm infants, the knowledge of the spontaneous orofacial movements at these ages also seems to be important for the diagnosis of oral function, particularly in preterm infants. Therefore, we decided to first classify preterm and full-term infants according to general movements ages, and then to record, describe, compare, and discuss their spontaneous orofacial movements. METHODS: This cross-sectional study included 51 preterm infants (born between 28 and 36 weeks) and 43 full-term infants who were classified at the writhing and fidgety ages of Prechtl's method of general movements assessment. Their spontaneous orofacial movements were recorded on video, and The Observer XT software (Noldus) was used to record the quantitative values of the movements. RESULTS: Poor repertoires of writhing movements were more frequent in the preterm infants (90.9%) compared to full-term ones (57.9%). Positive fidgety movements were observed in 100% of both preterm and full-term infants. Oral movements were similar for both preterm and full-term infants, regardless of their movement stage. CONCLUSION: All spontaneous orofacial movements were present both in preterm and full-term infants, albeit with higher frequency, intensity, and variability at fidgety age.

PURPOSE: To determine the refractive outcome after treatment with laser photocoagulation, intravitreal antiangiogenic injection or both, for the treatment of type 1 ROP and/or aggressive posterior ROP using defocus equivalent terms. These results will be compared with the spherical equivalent (SE) notation in an initial refraction (during the first 3 years after treatment) and in a final evaluation (three years after treatment). METHODS: Clinical charts review of preterm patients who developed type 1 ROP and/or aggressive posterior ROP who were treated with laser photocoagulation, intravitreal antiangiogenic injection, or both, between January 2007 and December 2014 at Fundación Oftalmológica de Santander were reviewed. Refractive error was evaluated at two different time points in three different treatment groups. RESULTS: Seventy-seven eyes (56 patients) with ROP diagnosis treated with laser, antiangiogenics or in a combined scheme were included. Median gestational age was 29 weeks (IQR = 3), median birth weight was 1100 (IQR = 335) and mean corrected age at the time of treatment was 37.3 weeks (SD 2.2). Refractive outcomes in SE terms for the first refraction had a median of -0.50 diopters and in defocus terms, 4.00 diopters (D). For the second refraction, -3.00 diopters and 4.00 diopters (D), respectively. Pearson correlation test result for the first measurement was 93% (p = 0.000) and for the second evaluation was 99% (p = 0.000). CONCLUSIONS: Low birth weight had a statistically significant association to the increase of the refractive outcome. The high correlation between SE and DE for refractive errors allows us to confirm that DE is an appropriate metric to report refractive outcomes in this group of patients due to the presence of combined refractive ametropias.

AIM: The protective effects of Kangaroo mother care (KMC) on the neurodevelopment of preterm infants are well established, but we do not know whether the benefits persist beyond infancy. Our aim was to determine whether providing KMC in infancy affected brain volumes in young adulthood. METHOD: Standardised cognitive, memory and motor skills tests were used to determine the brain volumes of 20-year-old adults who had formed part of a randomised controlled trial of KMC versus incubator care. Multivariate analysis of brain volumes was conducted according to KMC exposure. RESULTS: The study comprised 178 adults born preterm: 97 had received KMC and 81 were incubator care controls. Bivariate analysis showed larger volumes of total grey matter, basal nuclei and cerebellum in those who had received KMC, and the white matter was better organised. This means that the volumes of the main brain structures associated with intelligence, attention, memory and coordination were larger in the KMC group. Multivariate lineal regression analysis demonstrated the direct relationship between brain volumes and duration of KMC, after controlling for potential confounders. CONCLUSION: Our findings suggest that the neuroprotective effects of KMC for preterm infants persisted beyond childhood and improved their lifetime functionality and quality of life.

To assess stress experienced during Neonatal Intensive Care Unit (NICU) stay, we analyzed fingernail Cortisol (CORT) and Dehydroepiandrosterone (DHEA) levels and ratios in mothers and preterm infants (PI); compared hormones levels/ratio (CORT and DHEA) in kangaroo care (KC) versus standard care (SC) groups and examined relationships between PI hormone levels total days spent in the NICU. Mothers and their infants were recruited in the NICU, included levels I-IV and kangaroo care unit, within one week of infant birth in hospitals in Brazil. At 3 months after birth, mothers provided 3-month growth clippings from all ten digits of their own and their infants' fingernails. CORT and DHEA were measured using enzyme immunoassays (mothers) and high-performance-liquid-chromatography-with-mass-spectrometry (infants). Sample: n = 59 mothers (KC = 30/SC = 29) and 63 infants (KC = 32/SC = 31). Data were analyzed using non-parametric/parametric comparative statistics. NICU stay ranged from 3-103 days. For mothers in Kangaroo and Standard Care the CORT, DHEA levels and DHEA:CORT ratio (DC) ratio did not differ. Infants in KC had higher DHEA (p = 0.003) and a higher DC ratio (p = 0.011) than SC infants. Even though KC infants stayed in the NICU for a greater number of days than infants in SC, they had higher mean level of DHEA, and DC ratio, suggesting that KC played a role in promoting their stress regulatory capacities and may mitigate toxic effects of chronic hypercortisolemia. However, for mothers, KC did not reduce chronic stress compared to that in women in the SC condition. Further research warranted.

OBJECTIVE: To investigate recent trends in bronchopulmonary dysplasia (BPD) and its risk factors among extremely preterm infants. STUDY DESIGN: Demographic and clinical data were reviewed for 19 370 infants born at 22-27 weeks of gestation registered in the affiliated hospitals of the Neonatal Research Network of Japan between 2003 and 2016. We investigated the overall survival and prevalence of bronchopulmonary dysplasia (BPD) at 36 weeks' postmenstrual age and risk factors for developing BPD among the survivors. RESULTS: Among 19 370 infants, 2244 (11.6%) died by 36 weeks' postmenstrual age. The mortality rate decreased from 19.0% (99% CI, 15.7%-22.8%) in 2003 to 8.0% (99% CI, 6.2%-10.3%) in 2016. Among 17 126 survivors, BPD developed in 7792 (45.5%) infants, and its proportion significantly increased from 41.4% (99% CI, 36.5%-46.4%) in 2003 to 52.0% (99% CI, 48.2%-55.9%) in 2016. A multivariable analysis of the survivors showed a positive association of BPD with ≥4 weeks' supplemental oxygen or invasive ventilation, birth weight <750 g, small for gestational age, ≥4 weeks' noninvasive positive pressure ventilation, chorioamnionitis, <26 weeks' gestational age, <20 cases per year of center patient volume, or treated patent ductus arteriosus. Although the median duration of invasive ventilation was shortened, the proportions of factors associated adversely with BPD generally showed increasing trends over time. CONCLUSIONS: The mortality rate of extremely preterm infants has decreased, but the rate of BPD has increased in survivors between 2003 and 2016. Despite the decreasing duration of invasive ventilation over time, increasing rates of BPD suggest that differences in the patient population or other management strategies influence the development of BPD.

We measured percent body fat by air-displacement plethysmography in 86 infants born at <32 weeks of gestation randomized to receive either high-volume (180-200 mL/kg/day) or usual volume feeding (140-160 mL/kg/day). RESUMEN Objetivos Evaluar el impacto nutricional de un minimercado mensual de alto valor nutricional entregado, mediante charlas educativas, a familias de recién nacidos prematuros y/o de bajo peso al nacer (BPN), en el marco de los Programas Madre Canguro (PMC), en Bogotá y Cundinamarca; asimismo, identificar los factores de riesgo que predisponen la aparición de la desnutrición (<-2DE). Métodos Estudio observacional, descriptivo y prospectivo de una cohorte de 392 niños inscritos en 10 PMC que presentan en su seguimiento un riesgo alimenticio o una desnutrición confirmada provenientes de familias de ingresos < a 2 salarios mínimos y que reciben mensualmente un minimercado de alto valor nutricional con charla educativa después de los 3 meses de edad corregida (EC). Resultados Según el Sistema de Aseguramiento en Salud, no hubo diferencias en los resultados nutricionales. La entrega de los minimercados no tuvo impacto en la nutrición sino en la adherencia a los PMC. A las 40 semanas, el 19,2% tenía un peso <-2DE, de los cuales 20% tenía Retardo de Crecimiento Intrauterino (RCIU) al nacer. A los 3 y 12 meses de EC, el 24,5% y 36,5% tenía un peso <-2DE. Los demás estaban en riesgo alimenticio. Dos factores de riesgo fueron significativos: la edad de la madre y el grado de desnutrición a la entrada. Conclusión Se requiere un seguimiento prolongado con intervención rápida en los PMC para poder recuperar y vigilar estos niños en riesgo alimenticio o con desnutrición confirmada, conociendo el impacto que tiene en el desarrollo neurológico y cognitivo futuro.
ABSTRACT Objectives To both evaluate the nutritional impact of a high-nutritional- valued, monthly-delivered groceries, combined with educational talks addressed to families of premature and/or low birth weight infant (LBW), followed by Kangaroo Mother Care Program (KMCP) in Bogota and Cundinamarca; and identify the risk factors that predispose the onset of malnutrition. Methods Observational, descriptive, and prospective study of a cohort of 392 children enrolled in 10 KMCP who present in their follow-up a nutritional risk or a malnutrition (Weight <-2 SD [Standard Deviation]); from families with incomes <2 current legal minimum wages and who receive a high-nutritional-valued, monthly-delivered groceries with educational talks after 3 months of corrected age. Results According to the health insurance system, there were no differences in nutritional outcomes. The monthly groceries delivery had no impact on nutrition but on adherence to KMCP. At 40 weeks, 19,2% had a weight of <-2SD, of which 20% had Intrauterine Growth Retardation. At 3 and 12 months of corrected age, 24,5% and 36,5% had a weight of <-2SD, the remaining were at malnutrition risk. Two risk factors were significant: the age of the mother and the level of malnutrition at admission. Conclusion A longer follow-up with faster intervention in the KMCP is necessary, to recover and monitor these children at dietary risk or with confirmed malnutrition, aware of the future impact it may have on neurological and cognitive development.

Background: Retinopathy of prematurity (ROP) is the principal cause of blindness during childhood. The objective of this study was to analyze the frequency of ROP and risk factors associated with ROP in a cohort of very low birth weight infants. Methods: A cases (ROP) and controls (no ROP) study of infants less than 1500 g was conducted. Perinatal and neonatal variables were analyzed. For the statistical analysis, χ2 test, Student's t-test and Mann-Whitney's U-test were used. Results: For the study, 282 neonates were included: 152 (53.9%) with ROP and 130 (46.1%) without ROP. The most frequent stages observed were stage 1 and 2, with 139 (91.4%) patients, and stages 3 to 5, with only 13 patients (8.5%). In those neonates with ROP compared with neonates without ROP, the birth weight was less (902.7 vs. 1037.9 g) and the difference was significant (p < 0.0001). Also, the difference with gestational age (28.2 vs. 29.6; p < 0.0001), total ventilation days (32.8 vs. 16.1; p < 0.00001) and total oxygen days (87.7 vs. 62.6; p < 0.0001) was significant in neonates with ROP and neonates without the disease. Bronchopulmonary dysplasia, intraventricular hemorrhage and late onset sepsis were significant with patients with ROP. Conclusions: The frequency of ROP reported here is higher than the reported in Mexican population, with less cases of severe ROP. The neonatal surveillance in babies with less birth weight and gestational age is important to decrease the incidence of ROP.
Introducción: La retinopatía del prematuro (ROP) es una de las principales causas de ceguera infantil. La inmadurez y la exposición a oxígeno son algunos factores de riesgo. El objetivo de este artículo fue analizar la frecuencia y los factores de riesgo de ROP en una cohorte de recién nacidos menores de 1,500 g. Métodos: Se llevó a cabo un estudio de casos (con ROP) y controles (sin ROP) de recién nacidos menores de 1,500 g. Se analizaron variables prenatales y neonatales, y para su comparación se utilizaron las pruebas estadísticas t de Student, χ2 y U de Mann-Whitney. Resultados: Se analizaron 282 recién nacidos: 152 (53.9%) con ROP y 130 (46.1%) sin ROP. La mayor frecuencia se encontró en los estadios 1 y 2, con 139 pacientes (91.4%), seguidos de los estadios 3 a 5, con 13 pacientes (8.5%). En los pacientes con ROP, el peso al nacer fue menor (902.7 vs. 1037.9 g; p < 0.0001), así como la edad gestacional (28.2 vs. 29.6 semanas de gestación; p < 0.0001). Los días de ventilación (32.8 vs. 16.1; p < 0.00001) y los días de oxígeno requerido durante la estancia hospitalaria (87.7 vs. 62.6; p < 0.0001) fueron mayores en los pacientes con ROP. La displasia broncopulmonar, la hemorragia intraventricular y la sepsis tardía fueron comorbilidades significativas para el desarrollo de ROP. Conclusiones: En este estudio, la frecuencia de ROP fue mayor que la reportada en la población mexicana, con una baja proporción de formas graves. La vigilancia estrecha del manejo de los neonatos con menor peso y menos edad gestacional es fundamental para lograr disminuir esta enfermedad.

Resumen Introducción: La retinopatía del prematuro (ROP) es una de las principales causas de ceguera infantil. La inmadurez y la exposición a oxígeno son algunos factores de riesgo. El objetivo de este artículo fue analizar la frecuencia y los factores de riesgo de ROP en una cohorte de recién nacidos menores de 1,500 g. Métodos: Se llevó a cabo un estudio de casos (con ROP) y controles (sin ROP) de recién nacidos menores de 1,500 g. Se analizaron variables prenatales y neonatales, y para su comparación se utilizaron las pruebas estadísticas t de Student, χ2 y U de Mann-Whitney. Abstract Background: Retinopathy of prematurity (ROP) is the principal cause of blindness during childhood. The objective of this study was to analyze the frequency of ROP and risk factors associated with ROP in a cohort of very low birth weight infants. Methods: A cases (ROP) and controls (no ROP) study of infants less than 1500 g was conducted. Perinatal and neonatal variables were analyzed. For the statistical analysis, χ2 test, Students t-test and Mann-Whitneys U-test were used. Results: For the study, 282 neonates were included: 152 (53.9%) with ROP and 130 (46.1%) without ROP. The most frequent stages observed were stage 1 and 2, with 139 (91.4%) patients, and stages 3 to 5, with only 13 patients (8.5%). In those neonates with ROP compared with neonates without ROP, the birth weight was less (902.7 vs. 1037.9 g) and the difference was significant (p < 0.0001). Also, the difference with gestational age (28.2 vs. 29.6; p < 0.0001), total ventilation days (32.8 vs. 16.1; p < 0.00001) and total oxygen days (87.7 vs. 62.6; p < 0.0001) was significant in neonates with ROP and neonates without the disease. Bronchopulmonary dysplasia, intraventricular hemorrhage and late onset sepsis were significant with patients with ROP. Conclusions: The frequency of ROP reported here is higher than the reported in Mexican population, with less cases of severe ROP. The neonatal surveillance in babies with less birth weight and gestational age is important to decrease the incidence of ROP.

OBJECTIVES: To examine the association between mortality or neurodevelopmental impairment at 18-24 months of corrected age and the Transport Risk Index of Physiologic Stability (TRIPS) score on admission to the neonatal intensive care unit (NICU) in extremely premature infants. STUDY DESIGN: Retrospective cohort study of extremely premature infants (inborn and outborn) born at 22-28 weeks of gestational age and admitted to NICUs in the Canadian Neonatal Network between April 2009 and September 2011. TRIPS scores and clinical data were collected from the Canadian Neonatal Network database. Follow-up data at 18-24 months of corrected age were retrieved from the Canadian Neonatal Follow-Up Network database. Neurodevelopment was assessed using the Bayley Scales of Infant and Toddler Development, Edition III. The primary outcome was death or significant neurodevelopmental impairment at 18-24 months of corrected age. The secondary outcomes were individual components of the Bayley Scales of Infant and Toddler Development, Edition III assessment. RESULTS: A total of 1686 eligible infants were included. A TRIPS score of ≥20 on admission to the NICU was significantly associated with mortality (aOR 2.71 [95% CI, 2.02-3.62]) and mortality or significant neurodevelopmental impairment (aOR 1.91 [95% CI, 1.52-2.41]) at 18-24 months of corrected age across all gestational age groups of extremely premature infants. CONCLUSION: The TRIPS score on admission to the NICU can be used as an adjunctive, objective tool for counselling the parents of extremely premature infants early after their admission to the NICU.

OBJECTIVE: To determine associations between home oxygen use and 1-year readmissions for preterm infants with bronchopulmonary dysplasia (BPD) discharged from regional neonatal intensive care units. STUDY DESIGN: We performed a secondary analysis of the Children's Hospitals Neonatal Database, with readmission data via the Pediatric Hospital Information System and demographics using ZIP-code-linked census data. We included infants born <32 weeks of gestation with BPD, excluding those with anomalies and tracheostomies. Our primary outcome was readmission by 1 year corrected age; secondary outcomes included readmission duration, mortality, and readmission diagnosis-related group codes. A staged multivariable logistic regression was adjusted for center, clinical, and social risk factors; at each stage we included variables associated at P < .1 in bivariable analysis with home oxygen use or readmission. RESULTS: Home oxygen was used in 1906 of 3574 infants (53%) in 22 neonatal intensive care units. Readmission occurred in 34%. Earlier gestational age, male sex, gastrostomy tube, surgical necrotizing enterocolitis, lower median income, nonprivate insurance, and shorter hospital-to-home distance were associated with readmission. Home oxygen was not associated with odds of readmission (OR, 1.2; 95% CI, 0.98-1.56), readmission duration, or mortality. Readmissions for infants with home oxygen were more often coded as BPD (16% vs 4%); readmissions for infants on room air were more often gastrointestinal (29% vs 22%; P < .001). Clinical risk factors explained 72% of center variance in readmission. CONCLUSIONS: Home oxygen use is not associated with readmission for infants with BPD in regional neonatal intensive care units. Center variation in home oxygen use does not impact readmission risk. Nonrespiratory problems are important contributors to readmission risk for infants with BPD.

AIM: Building strategies for the country-level dissemination of Kangaroo mother care (KMC) to reduce the mortality rate in preterm and low birth weight babies and improve quality of life. KMC is an evidence-based healthcare method for these infants. However, KMC implementation at the global level remains low. METHODS: The international network in Kangaroo mother brought 172 KMC professionals from 33 countries together for a 2-day workshop held in conjunction with the XIIth International KMC Conference in Bogota, Colombia, in November 2018. Participants worked in clusters to formulate strategies for country-level dissemination and scale-up according to seven pre-established objectives. RESULTS: The minimum set of indicators for KMC scale-up proposed by the internationally diverse groups is presented. The strategies for KMC integration and implementation at the country level, as well as the approaches for convincing healthcare providers of the safety of KMC transportation, are also described. Finally, the main aspects concerning KMC follow-up and KMC for term infants are presented. CONCLUSION: In this collaborative meeting, participants from low-, middle- and high-income countries combined their knowledge and experience to identify the best strategies to implement KMC at a countrywide scale.
