BACKGROUND: Tuberculosis (TB) is the second most common cause of death due to a single infectious agent worldwide after COVID-19. Up to 15% of the cases are extrapulmonary, and if it is located in the central nervous system (CNS-TB), it presents high morbidity and mortality. Still, the global epidemiology of CNS-TB remains unknown. AIM: To estimate the global prevalence and incidence of CNS-TB based on the available literature. METHODS: We systematically searched in MEDLINE, Cochrane Central, Scopus, and LILACS databases (April 2020) and included observational studies evaluating the epidemiology of CNS-TB. Two independent researchers selected and assessed the quality of the studies and extracted relevant data. We performed random-effects model meta-analysis of proportions to estimate the pooled prevalence. The protocol of this study was registered in PROSPERO (CRD 42018103946). RESULTS: We included 53 studies from 28 countries, representing 12,621 patients with CNS-TB. The prevalence of CNS-TB was 2 per 100,000 inhabitants. According to the clinical setting, the prevalence of CNS-TB represented the 13.91% of all cases of meningitis and 4.55% of all cases of TB. The mortality was calculated by tuberculous meningitis due to the lack of data of other presentation, and it rose up to 42.12% in hospitalized patients. The burden of countries' TB, Human Development Index (HDI), and the prevalence of HIV were the most important prevalence moderators, especially in patients with TB. No data on incidence were found. CONCLUSION: The prevalence and mortality of CNS-TB remain high, and TB meningitis is the most frequent presentation. The highest prevalence was reported in developing countries, and its main moderators were the countries' HDI and HIV infection. Our study was limited by high heterogeneity, risk of bias, and potential data under registration from developing countries. The integration of CNS-TB early detection and management into national TB programs and population-based studies from developing countries are needed for better global estimation and response.

RESUMEN Objetivo Determinar la prevalencia de Chlamydia trachomatis (CT), virus herpes simple-2 (VHS-2) y N. gonorrhoeae (NG) y los factores de riesgo relacionados con la infección, en un grupo de estudiantes universitarios de la ciudad de Medellín. Metodología Se realizó un estudio descriptivo en un grupo de 323 estudiantes universitarios. A través de un formulario en línea, se realizó el reporte de los factores de riesgo y síntomas relacionados con infecciones de transmisión sexual (ITS) y luego se determinó la presencia de anticuerpos IgG y IgM para CT y VHS-2 y se realizó una prueba de PCR-RT para detectar NG y CT. Resultados La frecuencia de IgG para CT fue del 13% y la positividad para IgM fue del 11,9%. La frecuencia de IgG para VHS-2 fue del 11,8% y la frecuencia de CT y NG por la prueba PCR-RT fue del 1,5% y del 0%, respectivamente. Los factores de riesgo más frecuentes fueron: vida sexual activa en el 96,9%, uso algunas veces o nunca del condón en un 75,2%. Reportaron que tenían secreción genital el 13,6% de los estudiantes; úlceras, el 2,8%; verrugas, el 5,3%; ardor al orinar, el 15,5%; ampollas, el 4,6%, y diagnóstico previo de una ITS, el 18,9% de los estudiantes. Conclusión Se encontró CT y VHS-2 entre los jóvenes estudiados y además una alta frecuencia de factores de riesgo para la adquisición de ITS. Se recomienda mejorar las campañas de prevención y diagnóstico de las ITS en los jóvenes universitarios.


ABSTRACT Objective The aim of this study was to determine the prevalence of Chlamydia trachomatis (CT), Herpes Simple-2 virus (HSV-2) and N. gonorrhoeae (NG), and the risk factors related to the infection in a group of college students in the city of Medellín, Colombia. Methods A descriptive study was carried out in a group of 323 university students. Through an online form, the risk factors and symptoms related to sexually transmitted infection (STI) were reported, and then the presence of IgG and IgM antibodies for CT and HSV-2 was determined, and RT-PCR testing was performed to detect NG and CT. Results 13% was the IgG frequency for CT while 11.9% was for IgM. The frequency for IgG for VHS-2 was 11.8 while the frequencies determined by RT-PCR for CT and NG were 1.5% and 0.0% respectively. The risk factors most commonly associated were active sex life, 96.9%, and random use of condom, 75.2%. It was also found that 13.6% of the students had genital secretions, 2.8% had ulcers, 5.3% had genital warts, 4.6% had blisters, 15.5% had dysuria, and 18.9% had a previous STI diagnosis. Conclusions CT and HVS-2 were found among college students, besides a series of risk factors associated with STI. It is recomended to improve prevention and diagnosis campaigns among young college students.

RESUMEN Objetivo Determinar los diagnósticos más frecuentes de los pacientes atendidos en el servicio de consulta externa de medicina general de la Universidad Santo Tomás (USTA), seccional Bucaramanga, entre 2015 y 2017. Método Estudio descriptivo retrospectivo de los registros diarios de pacientes atendidos en el servicio médico con un N=13 892 pacientes -estudiantes, administrativos, docentes, egresados y División de Educación Abierta y a Distancia (DUAD)-. Se excluyeron 5 907 registros para un n=7 985 datos de exploración, correspondientes a pacientes que hicieron uso exclusivamente del servicio de consulta médica. Resultados 5 808 (73%) de las consultas fueron realizadas por mujeres y 2 177 (27%) hombres; jóvenes entre 19 y 26 años (60,9%), estudiantes de pregrado (71,8%), de odontología (29,6%) y derecho (26,5%), atendidos por enfermedades del aparato respiratorio (16,4%), con afectaciones más frecuentes a adolescentes (15,4%) y jóvenes (17,2%). La rinofaringitis aguda fue el principal diagnóstico CIE-10. Los jóvenes consultaron por diarrea y gastroenteritis de presunto origen infeccioso (6,7%); los adultos (5,7%) y docentes (6,1%), por cervicalgia, y el personal administrativo, por infección viral no especificada (5,7%). Conclusiones La población atendida en consulta médica de la USTA entre 2015 y 2017 fueron principalmente mujeres y jóvenes entre 19 a 26 años. Las enfermedades transmisibles como las enfermedades del aparato respiratorio fueron más frecuentes que las enfermedades crónicas no transmisibles, principalmente la rinofaringitis aguda, enfermedad de alta transmisibilidad en comunidad. Las mujeres, adolescentes y estudiantes de pregrado, fueron atendidos en consulta por consejo y asesoramiento sobre la anticoncepción, lo que coincide con la Encuesta Nacional De Demografía y Salud.

Method A retrospective descriptive study of the daily records of patients treated in primary medical care services with an N=13 892 patients -students, administrative workers, professors, graduates, and Open and at Distance Education Division (DUAD, by its initials in Spanish)-. 5 907 records were excluded for a n=7 985 scan data corresponding to patients who used the medical consultation service exclusively. Results 5 808 (73%) of the consultations were conducted by women and 2 177 (27%) by men; young people between 19 and 26 years (60.9%), undergraduate students (71.8%), in dentistry (29.6%) and law (26.5%), people treated for respiratory diseases (16.4%), affected adolescents (15.4%) and young people (17.2%) more frequently. Acute rhinopharyngitis was the main diagnosis ICD-10. Young people consulted for diarrhea and gastroenteritis of suspected infectious origin (6.7%); adults (5.7%) and professors (6.1%), for neck pain, and administrative workers, for unspecified viral infection (5.7%). Conclusions The population who attended medical consultation of the Santo Tomás University in Bucaramanga throughout 2015-2017 were mainly women and young people between 19 and 26 years. Communicable diseases such as respiratory diseases were more frequent than chronic noncommunicable diseases, mainly acute rhinopharyngitis, a disease of high transmissibility in the community. Women, adolescents, and undergraduate students were treated in consultation for contraceptive counseling, which coincides with the National Demography and Health Survey.
