RESUMO

Objective: This study aims to investigate the occurrence of maternal postpartum depression (PPD) during menstruation and analyze the influencing factors and risk prediction modeling of maternal PPD in Chinese puerperal women of sitting the month. Methods: A total of 286 mothers were selected using convenience sampling, who came for a routine postpartum follow-up visit were surveyed, including face-to-face, telephone, and online. They completed questionnaires including the basic profile questionnaire, Postpartum Partner Support Scale (PPSS), Edinburgh PPD Scale (EPDS), Parenting Self-Efficacy Scale (SICS), and Simple Coping Style Questionnaire (SCSQ), who were advised to complete the survey alone, in private, reducing the impact of husband's presence on the quality of the questionnaire. Variables showing statistical significance in the one-way analysis were further analyzed using logistic regression analysis. The predictive value of the logistic regression model was analyzed using the Receiver Operating Characteristic Curve (ROC), and the predictive reliability was expressed as the area under the ROC [Area Under the Curve (AUC)]. Results: The total score of PPD was 7.78 ± 4.57, and 22 people (7.69%) experienced depression during the postpartum period. PPD was found to be correlated with postpartum partner support, positive coping, negative coping, and parenting self-efficacy, with correlation coefficient values of -0.63, 0.62, 0.56, and - 0.70, respectively (all p < 0.05). Logistic regression analysis revealed that postpartum partner support and parenting self-efficacy were independent factors influencing PPD, with odds ratios (95% confidence intervals) of 0.76 (0.61 ~ 0.94) and 0.83 (0.75 ~ 0.93), respectively both p < 0.05.The area under the curve, sensitivity, and specificity for postpartum partner support and parenting self-efficacy were 1.00 (95% confidence intervals 0.99 ~ 1.00), 99.24, and 90.91%. Conclusion: Postpartum partner support and parenting self-efficacy independently predict the occurrence of PPD. Healthcare professionals and maternal families should prioritize timely attention to maternal partner support and parenting issues to reduce the occurrence of PPD.

Introduction: SARS-CoV-2 causes severe pneumonias. Pregnant women experience immunological and physiological changes, which may make them more susceptible to viral respiratory infections, including COVID-19. Objective: To present the characteristics of a case series of maternal deaths confirmed with COVID-19. Methods: A serial study of partial autopsies of postpartum women confirmed with SARS-CoV-2, reviewed by the special working group of pathological anatomy for COVID-19, in the year 2021, was carried out. The variables age, obstetric history and causes of death were analyzed at the Central Military Hospital "Dr. Luis Díaz Soto". Results: In 2021, 425 pregnant women with confirmed SARS-CoV-2 were attended, 16 of them died (3.8%). All of them underwent cesarean section for maternal-fetal benefit and were admitted to the intensive care unit, with comorbidities among which obesity and diabetes were more frequent. The mean date of symptom onset was 5.18 days, all contact positive cases; in the causes of death systemic hypoxia affected one third of the deceased; permeability pulmonary edema was present in 100 % of the puerperal women and in all maternal deaths there was multiple organ damage. Conclusions: Permeability pulmonary edema affects all cases, with important impact as a cause of death, as well as in the expression of hypoxia and systemic inflammatory response. COVID-19 is the basic cause of death in all cases.

Breastfeeding, according to the World Health Organization, should be the only food for babies up to 6 months of age and the main food for children up to 24 months of age. Among its many advantages, there are lower rates of infant morbidity due to diarrhea, respiratory infections, otitis media, protection against overweight and diabetes. Objective: The aim of the study was to analyze the prevalence and conditioning factors for breastfeeding among the investigated pregnant and lactating women, noting the standards for implementation in the maternal routine and their difficulties with this practice. Methods: This is a descriptive, quantitative study with a cross-sectional design. Data collection was carried out through a semi-structured questionnaire for maternal knowledge about breastfeeding, carried out with pregnant and postpartum women registered at a Health Unit located in the city of Ubá, Minas Gerais, featuring a non-probabilistic sample of 118 women. Results: Considering the knowledge of the term Exclusive Breastfeeding, 56.7% of the women interviewed reported not knowing, at the time of the interview, its meaning; 23.7% reported thinking that there are situations in which the baby should not be breastfed or that the practice should be interrupted; 19.4% of the women reported that they thought there was "weak milk", while 100% believed that it was a suitable food for the baby. Conclusion: In this study, it was verified that the dissemination of information about breastfeeding, despite having already reached many women, the number of mothers who breastfeed correctly, even having received information about it and those who breastfeed until the appropriate age, is still low

Antenatal care is essential to ensure a good health for pregnant women and for expected offspring. The objectives of this study were to analyze factors that influence the utilization of antenatal services, defined as at least four antenatal visits. A cross-sectional descriptive study, using both quantitative and qualitative methods, was carried out at 1o de Maio Health Center in Maputo City. The quantitative component was based on a questionnaire administered to postpartum women in the maternity ward and in the postpartum consultation. The qualitative component was based on participatory observation and in-depth interviews with providers of maternal and child health services. A total of 271 postpartum women, with ages ranging from 16 and 42 years, were responded to a questionnaire. Among them, 233 (86%) had complete more than four antenatal visits. Initiation of anti-retroviral treatment at antenatal care was the only factor associated with lower use of antenatal services (p≤ 0.05). The qualitative data show that overall, the health Center was ready to provide antenatal services, with the exception of lack of drinking water for the direct observation treatment, and an insufficient stock of iron and folic acid supplementation. Although there are some deficiencies in the provision of some services at the health Center level, the use of antenatal services is, in general, high. This high utilization of services may be explained by the location - an urban area, where access to information and health services is easier.

RESUMO

BACKGROUND: Postpartum depression (PPD) is a common complication of cesarean section. S-ketamine given intravenously during surgery can help prevent PPD. However, whether S-ketamine in patient-controlled intravenous analgesia (PCIA) can reduce the incidence of PPD is unknown. This study assessed the effect of S-ketamine as an adjuvant in PCIA for preventing PPD in women undergoing cesarean delivery. METHODS: A total of 375 parturients scheduled to undergo cesarean section and then receive PCIA were recruited from a single center and were randomly assigned to control (C) group (sufentanil 2 µg/kg + tropisetron 10 mg) or S-ketamine (S) group (S-ketamine 0.5 mg/kg + sufentanil 2 µg/kg + tropisetron 10 mg). The primary outcome was the incidence of PPD measured by the Edinburgh postnatal depression scale (EPDS) after surgery. The secondary outcomes were EPDS scores, visual analog scale (VAS) scores, Ramsay sedation scale (RSS) scores, and the rate of adverse events, including headache, nausea, dizziness, drowsiness, and vomit. RESULTS: A total of 275 puerperal women were included in the study. The rate of depression in parturient on postoperative days 3, 14, 28 in the C group and S group were 17.6 and 8.2% (p < 0.05), 24.2 and 9.8% (p < 0.05), and 19.0 and 17.2% (p = 0.76) respectively. EPDS scores in the C group and S group on postoperative days 3,14, and 28 were 7.65 ± 3.14 and 6.00 ± 2.47 (p < 0.05), 7.62 ± 3.14 and 6.38 ± 2.67 (p < 0.05), and 7.35 ± 3.17 and 6.90 ± 2.78 (p = 0.15), respectively. The rate of adverse events in the C group and S group were headache 3.3 and 4.1% (p = 0.755), nausea 5.9 and 8.2% (p = 0.481), dizziness 9.2 and 12.3% (p = 0.434), drowsiness 6.5 and 10.7%(p = 0.274), and vomit 5.9 and 5.7% (p = 0.585). CONCLUSIONS: S-ketamine (0.01 mg/kg/h) as an adjuvant in PCIA significantly reduces the incidence of PPD within 14 days and relieves pain within 48 h after cesarean delivery, without increasing the rate of adverse reactions. TRIAL REGISTRATION: Registered in the Chinese Clinical Trial Registry ( ChiCTR2100050263 ) on August 24, 2021.

RESUMO

PURPOSE: Few studies have yet examined sleep quality among puerperal women during the coronavirus disease 2019 (COVID-19) pandemic. This cross-sectional study aimed to compare the sleep quality of puerperal women before and during the COVID-19 pandemic in Lanzhou, China. The study exploreed the factors that affect sleep quality among puerperal women. METHODS: The study population comprised puerperal women who went to the obstetric department of the Gansu Provincial Maternity and Childcare Hospital on the 42nd day after childbirth. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI). Data were collected by doctors in the obstetric department before the COVID-19 pandemic (Oct.-Dec. 2019) and during the COVID-19 pandemic (Feb.-Apr. 2020) in China. Multiple linear regressions were used to examine the association between the sleep quality of puerperal women and COVID-19 and to identify factors that affect the total PSQI scores. RESULTS: In 2019, 546 puerperal women were assessed compared with 655 who were assessed in 2020. Compared with before COVID-19, the total PSQI scores of puerperal women improved during the COVID-19 pandemic. In addition, the influencing factors of sleep quality among puerperal women were the pandemic itself, age, conception method, and postpartum depression (PPD). CONCLUSIONS: In contrast with other studies, there was no evidence for worsening sleep quality of puerperal women during the COVID-19 pandemic in Lanzhou, China. Puerperal women who underwent assisted pregnancy or developed PPD were more likely to experience poor sleep quality.

RESUMO

Introduction  This study aimed to clarify social networking and help seeking behaviors in relation to the health of pregnant and puerperal Brazilian women in Japan, using unanalyzed data collected in the article titled “Socio-cultural factors affecting the health of pregnant and puerperal Brazilian women in Japan” published in this journal. Methods  We conducted semi-structured interviews of pregnant and puerperal Brazilian women aged ≥20 years in their homes in Prefectures A and B during 2013-2014. We used the Japanese version of the interview guide based on the cultural determinants of help seeking developed by Arnault. The analysis was guided by analytic ethnography, and core themes were derived. Results  The interviews were conducted with 18 women. Their mean age was 32.4 years (5 women in their 20s and 13 women in their 30s). The average length of stay was 12.6 years (6 women: <10 years and 12 women: ≥10 years). Regarding residential status, 12 women were permanent residents. Two categories for social networking were derived: (1) limited social interaction and (2) reliance on family and trusted others. Two categories for help-seeking behaviors were derived: (1) reliance on informal resources close to them and (2) self-reliant problem solving. Conclusions  The social networking and help-seeking behaviors of pregnant and puerperal Brazilian women in Japan were related to their way of cultivating relationships in their transnational life as migrant workers. Their behaviors were also related to their perception on the family, their religion, and their self-reliant problem-solving behaviors.

RESUMO

BACKGROUND: To evaluate the potential factors associated with the nutritional composition of human milk of puerperal women. METHODS: cross-sectional study, conducted between March 2016 and August 2017, with 107 women, selected in a Tertiary Health Care Tertiary Health Facility of the Unified Health System (SUS) in the Municipality of Rio de Janeiro. Data were collected two months after delivery. The dependent variable of the study was the nutritional composition of human milk. We divided the independent variables into hierarchical levels: distal (age, schooling, parity and pregestational nutritional status), intermediate (number of prenatal visits and gestational weight gain) and proximal (alcohol consumption, smoking, diabetes mellitus and hypertension). For data analysis, we applied the multiple linear regression, centered on the hierarchical model. Only the variables associated with the nutritional composition of breast milk remained in the final model at a 5% level of significance. RESULTS: The nutritional composition of human milk yielded by women with pregestational overweight, smokers and hypertensive had higher amounts of lipids and energy. Conversely, women with gestational weight gain below the recommended had lower amounts of these components. CONCLUSION: The evaluation of factors associated with the nutritional composition of human milk is extremely important to assist post-partum care practices. In this study, we observed that lipid and energy contents were associated to pregestational nutritional status, gestational weight gain, smoking and hypertension.

RESUMO

Dietary intake may affect maternal health, but it remains unclear about puerperal dietary intake and its association with maternal health. This study investigated the dietary patterns and their related factors and association with postpartum weight retention (PPWR) in Chinese puerperal women. Participants were from the mother-infant cohort study, in which both mothers and infants were followed up from childbirth to the next 2 years, in seven cities around China. Maternal puerperal dietary patterns were derived by a food frequency questionnaire and principal component analysis (PCA) within 1 month postpartum. PPWR was assessed by the difference of weight at 42 days and 6 months postpartum minus the pre-pregnancy weight. Of 503 postpartum women, four dietary patterns were identified, including 'plant food' pattern (rice and vegetables as dominant foods), 'diverse' pattern (starchy roots, fruit, livestock meat and aquatic products), 'traditional northern' pattern (poultry, eggs and soup) and 'marine-flour' pattern (flour, coarse food grains and marine fish). The diverse pattern was associated with professional puerperal family care and counselling service (p < .05). PPWRs at 42 days and 6 months postpartum were 6.37 and 4.70 kg averagely. The plant food dietary pattern tended to be associated with higher 42-day PPWR (ß = .105, p < .05), and diverse pattern was associated with lower 6-months PPWR (ß = -.137, p < .05). Conclusively, this study presented four dominant dietary patterns in Chinese postpartum women and showed a lower PPWR in adherence to diverse dietary pattern. The results would provide evidence to furtherly guide dietary practice and improve maternal health.

RESUMO

INTRODUCTION AND OBJECTIVE: Fibroblast growth factor (FGF-23) and matrix extracellular phosphoglycoprotein (MEPE) are bone-related factors and their role in physiologic conditions and in different life stages are unknown. We aimed to evaluate age- and pregnancy-related changes in MEPE and FGF-23 levels and their correlations with calcium (Ca)-phosphate (PO4) metabolism. METHODS: The study population included 96 healthy children (50 females) and 31 women (11 healthy, 10 pregnant, and 10 lactating). Intact FGF-23 (iFGF-23), MEPE, ferritin, parathyroid hormone (PTH), 25-OH vitamin D, alkaline phosphatase (ALP), IGF-I, IGFBP-3 and, Ca, PO4 and creatine (Cre) in serum (S) and urine (U) samples were determined. The renal phosphate threshold (TmPO4/GFR) and z-scores for the parameters that show age-related changes were calculated. RESULTS: Serum iFGF-23 concentrations showed nonsignificant changes with age; however, MEPE decreased with age, reaching the lowest levels after 7 years. Additionally, higher serum MEPE concentrations were observed during pregnancy. Other than ALP, all other examined parameters demonstrated age-related changes. ALP, BUN, S-Cre, and U-Ca/Cre showed puerperal and pregnancy related changes together with MEPE. iFGF-23 was positively correlated with S-PO4 and TmPO4/GFR. MEPE was positively correlated with S-Ca, S-PO4 and TmPO4/GFR and negatively correlated with PTH, IGF-1, and IGFBP-3. CONCLUSION: Not iFGF-23 but MEPE showed age-dependent changes and was affected by pregnancy. Although, MEPE and iFGF-23 did not correlate with each other, they seem to affect serum and urinary phosphate in the same direction. Additionally, we found evidence that ferritin and growth factors might have a role in serum calcium and phosphate regulation.

The aim of this study was to determine the level of oral health knowledge in 200 postpartum women attending an integral service for women of a private clinic in the northern sector of Santiago de Chile during the year 2011. We used a self-administered questionnaire based on 24 questions related to the level of knowledge of oral diseases, prevention, pregnancy, dental care and dental development. The cutoff score for being considered a good level of knowledge was higher than or equal to 14 points and regular cooperation between 7 and 13 points. The average age of the sample was 29.11 years (+ 5.71) and half reported having one child. 57% refers university education, 70.5% reported to be working and only a third received dental care during pregnancy. Thirty-five percent of puerperal women had a good level of knowledge and 64% had a regular level. The highest score identified was the knowledge on prevention in oral health and the lowest was health growth and development of dental. The mean age was higher in the group with higher level of knowledge. No statistically significant difference was observed between the level of knowledge and having received dental care during pregnancy (p = 0.635). The relation between level of oral health knowledge and parity (p = 0.367), educational level (p = 0.345) and occupation (p = 0.388) was not statistically significant. Using linear regression we analyzed the relation between level of knowledge score and other variables. The level of education of the mother (95% CI = 0.3:1.8, P = 0.007) and parity (95% CI = -0.1: 1.5, p = 0.05) were significant, but the confidence interval for parity does not reject the null hypothesis. Mothers with higher or technical level of education...

RESUMO

PURPOSE: The purpose of this study was to determine the effects of music therapy on postpartum blues and maternal attachment of puerperal women. METHODS: The research design was a nonequivalent control group non-synchronized design. The participants were puerperal women who agreed to participate in this study and through a convenience sampling, 60 puerperal women were recruited (30 in the experimental group, 30 in the control group). After measuring postpartum blues and maternal attachment, music therapy was provided to the experimental group over 40 min, once a day, and for 8 days. Then, postpartum blues and maternal attachment for the experimental and control group were measured again on the 8th day. The data were analyzed using the SPSS WIN 12.0 Program. RESULTS: The first hypothesis that "the degree of postpartum blues for the experimental group who participated in music therapy would be lower than that of the control group" was accepted (t=4.350, p<.001). The second hypothesis that "the degree of maternal attachment of the experimental group who participated in music therapy would be higher than that of the control group" was accepted (t=4.828, p<.001). CONCLUSION: These findings indicate that music therapy has positive influences on decreasing postpartum blues and increasing maternal attachment of puerperal women.
