RESUMO

This study addressed the challenge of accurately detecting mycotoxins in herbs and spices, which have gained popularity as alternative medicines but pose health risks due to potential contamination. We used a competitive direct ELISA kit (Art No. 8610), Veratox for Ochratoxin, to quantify Ochratoxin A in the herb and spice samples. The samples were first prepared using solid-liquid extraction with 70% methanol. The resulting filtrate was then subjected to ELISA analysis. The results of the analysis were then further analyzed using principal component analysis (PCA). In this study, PCA was used to classify the concentration levels of Ochratoxin A based on various factors, such as the packaging type, country of origin, shelf life, and sample weight. The limits of detection (LOD) and quantification (LOQ) values indicate the lowest amount of Ochratoxin A that can be detected and quantified, respectively, with high accuracy and precision. The range of the LOD and LOQ values (0.43-0.58 µg/kg and 1.45-1.95 µg/kg, respectively) suggests that the method used was capable of detecting and quantifying Ochratoxin A in the herb and spice samples at different concentrations with a high degree of accuracy and precision. These results suggest that while most of the samples (73.33%) were below the maximum residue limit (MRL) for Ochratoxin A, a significant number of samples (26.67%) had concentrations of Ochratoxin A that were higher than the MRL. This highlights the importance of monitoring Ochratoxin A in herb and spice samples and ensuring the products are safe for consumption.

The availability of efficient diagnostic methods is crucial to monitor the incidence of crop diseases and implement effective management strategies. One of the most important elements in diagnostics, especially in large acreage crops, is the sampling strategy as hundreds of thousands of individual plants can grow in a single farm, making it difficult to assess disease incidence in field surveys. This problem is compounded when there are no external disease symptoms, as in the case for the ratoon stunting disease (RSD) in sugarcane. We have developed an alternative approach of disease surveillance by using the crude cane juice expressed at the sugar factory (mill). For this purpose, we optimized DNA extraction and amplification conditions for the bacterium Leifsonia xyli subsp xyli, the causal agent of RSD. The use of nucleic acid dipsticks and LAMP isothermal amplification allows to perform the assays at the mills, even in the absence of molecular biology laboratories. Our method has been validated using the qPCR industry standard and shows higher sensitivity. This approach circumvents sampling limitations, providing RSD incidence evaluation on commercial crops and facilitating disease mapping across growing regions. There is also potential is to extend the technology to other sugarcane diseases as well as other processed crops.

RESUMO

This is a narrative review of intravenous ketamine infusions for the treatment of complex regional pain syndrome (CRPS). It briefly covers the definition of CRPS, its epidemiology, and other treatments before introducing ketamine as the article's focus. A summary of ketamine's evidence base and its mechanisms of action is provided. The authors then review ketamine dosages reported in peer-reviewed literature for the treatment of CRPS, and their associated duration of pain relief. The observed response rates to ketamine and predictors of treatment response are also discussed.

RESUMO

Restless legs syndrome (RLS) affects 2% of children presenting with symptoms of insomnia, restless sleep, decreased quality of life, and effects on cognition and behavior. The International RLS Study Group and the American Academy of Sleep Medicine have published guidelines for the diagnosis and treatment of RLS in children. Restless sleep disorder has been recently identified in children and presents with frequent movements during sleep and daytime symptoms with polysomnography findings of at least 5 large muscle movements at night. Treatment options for both disorders include iron supplementation, either oral or intravenous with improvement in nighttime and daytime symptoms.

Bacteria exposed to stress survive by regulating the expression of several genes at the transcriptional and translational levels. For instance, in Escherichia coli, when growth is arrested in response to stress, such as nutrient starvation, the anti-sigma factor Rsd is expressed to inactivate the global regulator RpoD and activate the sigma factor RpoS. However, ribosome modulation factor (RMF) expressed in response to growth arrest binds to 70S ribosomes to form inactive 100S ribosomes and inhibit translational activity. Moreover, stress due to fluctuations in the concentration of metal ions essential for various intracellular pathways is regulated by a homeostatic mechanism involving metal-responsive transcription factors (TFs). Therefore, in this study, we examined the binding of a few metal-responsive TFs to the promoter regions of rsd and rmf through promoter-specific TF screening and studied the effects of these TFs on the expression of rsd and rmf in each TF gene-deficient E. coli strain through quantitative PCR, Western blot imaging, and 100S ribosome formation analysis. Our results suggest that several metal-responsive TFs (CueR, Fur, KdpE, MntR, NhaR, PhoP, ZntR, and ZraR) and metal ions (Cu2+, Fe2+, K+, Mn2+, Na+, Mg2+, and Zn2+) influence rsd and rmf gene expression while regulating transcriptional and translational activities.

Detection of residual formaldehyde (FA) in dairy products could be explained by direct addition of this preservative to extend the shelf life of raw material or final product at room temperature. FA is not authorized as a preservative by international standards and its addition to dairy products is prohibited due to its potentially harmful effects on consumers. Although the carcinogenicity of FA by oral exposure has not been proven, it is also known it cause histopathological and cytogenic changes in tissues at first contact, so its toxicity by ingestion should not be underestimated. This research determined both residual FA levels in locally produced fresh white cheese and its variation according to the seasons of the year and its association with ambient temperature. None of the FA levels quantified in cheese exceeded the maximum tolerable concentration (2.6 mg/kg) and although average FA contents did not vary significantly with seasonal changes (0.093-0.181 mg/kg), the number of positive cases did, since the highest prevalence occurred in the dry (60.9 %) and transitional dry-rainy (79.7 %) seasons of 2021, which are characterized by having the highest average ambient temperatures (27.5 °C and 28.3 °C, respectively). It was also shown that 79.6 % of the variability of FA-positive samples is explained by changes in the average temperature according to the year´s season. The association between these variables and quantified levels of aldehyde in raw milk sampled at the plant could indicate that FA was used to prevent milk and/or the final product from decomposing due to the effect of high ambient temperature. In addition, residual FA contents decreased in both milk and cheese, depending on added preservative levels, and the time elapsed prior to analysis.

Purpose: To evaluate the performance of a deep learning (DL) algorithm for retinopathy of prematurity (ROP) screening in Nepal and Mongolia. Design: Retrospective analysis of prospectively collected clinical data. Participants: Clinical information and fundus images were obtained from infants in 2 ROP screening programs in Nepal and Mongolia. Methods: Fundus images were obtained using the Forus 3nethra neo (Forus Health) in Nepal and the RetCam Portable (Natus Medical, Inc.) in Mongolia. The overall severity of ROP was determined from the medical record using the International Classification of ROP (ICROP). The presence of plus disease was determined independently in each image using a reference standard diagnosis. The Imaging and Informatics for ROP (i-ROP) DL algorithm was trained on images from the RetCam to classify plus disease and to assign a vascular severity score (VSS) from 1 through 9. Main Outcome Measures: Area under the receiver operating characteristic curve and area under the precision-recall curve for the presence of plus disease or type 1 ROP and association between VSS and ICROP disease category. Results: The prevalence of type 1 ROP was found to be higher in Mongolia (14.0%) than in Nepal (2.2%; P < 0.001) in these data sets. In Mongolia (RetCam images), the area under the receiver operating characteristic curve for examination-level plus disease detection was 0.968, and the area under the precision-recall curve was 0.823. In Nepal (Forus images), these values were 0.999 and 0.993, respectively. The ROP VSS was associated with ICROP classification in both datasets (P < 0.001). At the population level, the median VSS was found to be higher in Mongolia (2.7; interquartile range [IQR], 1.3-5.4]) as compared with Nepal (1.9; IQR, 1.2-3.4; P < 0.001). Conclusions: These data provide preliminary evidence of the effectiveness of the i-ROP DL algorithm for ROP screening in neonatal populations in Nepal and Mongolia using multiple camera systems and are useful for consideration in future clinical implementation of artificial intelligence-based ROP screening in low- and middle-income countries.

The investigation of the adsorption process putatively involved in the olfactory perception of apocynin, guaiacylacetone, homovanillyl alcohol, 4-ethylguaiacol and homoguaiacol molecules on the mouse eugenol olfactory receptor mOR-EG was a very useful tool for comprising olfaction process at a molecular level. Indeed, the experimental data were correlated by using an advanced monolayer adsorption model with identical and independent binding sites. Thanks to the grand canonical ensemble in statistical physics formalism, the physico-chemical interpretations of modeling results indicated that the five odorants were adsorbed via a multi-molecular mechanism. Hence, the calculation of adsorption energies, that described the interaction between the odorant molecules and the olfactory receptor binding cavities, indicated that weak bonds were made between apocynin, guaiacylacetone, homovanillyl alcohol, 4-ethylguaiacol and homoguaiacol molecules and mOR-EG binding pockets amino acid residues. In addition, theoretical stereographic and energetic characterizations of mOR-EG were made via the determination of the olfactory receptor site size distributions (RSDs) and the adsorption energy distributions (AEDs) relative to apocynin, guaiacylacetone, homovanillyl alcohol, 4-ethylguaiacol, homoguaiacol molecules. The RSD provided the size of different binding cavities of mOR-EG. Indeed, the five RSDs spectrums situated between 0.5 and 10 nm were spread out around an average size each one. The mean values obtained from the peaks of the distributions were 2.14 nm, 2.20 nm, 2 nm, 2.10 nm and 1.83 nm for apocynin, guaiacylacetone, homovanillyl alcohol, 4-ethylguaiacol and homoguaiacol molecules, respectively. The AED gave a whole spectrum of adsorption energies that was activated by the odorant molecule. Thus, the apocynin, guaiacylacetone, homovanillyl alcohol, 4-ethylguaiacol and homoguaiacol AEDs were spread out from 5 to 27.5 kJ/mol, from 5 to 30 kJ/mol, from 5 to 35 kJ/mol, from 0 to 22.5 kJ/mol, 5 to 25 kJ/mol, respectively. The thermodynamic study, via the establishment of the adsorption entropy, indicated that the peak of the disorder was obtained when half of the binding sites were occupied. In addition, the Gibbs free enthalpy and the internal energy were determined and their negative values indicated that the adsorption phenomenon involved in the olfactory perception was spontaneous and exothermic physisorption phenomenon.

Gallic acid (GA) is a natural polyphenolic compound with many health benefits. To assess the potential risk of long-term consumption of GA to gut health, healthy dogs were fed a basal diet supplemented with GA (0%, 0.02%, 0.04%, and 0.08%) for 45 d, and fecal microbiota and metabolomics were evaluated. This study demonstrated that GA supplementation regulated serum lipid metabolism by reducing serum triglyceride, fat digestibility, and Bacteroidetes/Firmicutes ratio. In addition, the relative abundance of Parasutterella was significantly lower, and the SCFAs-producing bacteria were increased along with fecal acetate and total SCFAs contents accumulation in the 0.08% GA group. Metabolomics data further elucidated that 0.08% GA significantly affected carbohydrate metabolism by downregulating succinic acid in fece, thereby alleviating inflammation and oxidative stress. Overall, this study confirmed the beneficial effects of long-term consumption of GA on lipid metabolism and gut health, and the optimal level of GA supplementation was 0.08%.

Surface-enhanced Raman spectroscopy (SERS) is commonly used for super-selective analysis through nanostructured silver layers in the environment, food quality, biomedicine, and materials science. To fabricate a high-sensitivity but a more accessible device of SERS, DC magnetron sputtering technology was used to realize high sensitivity, low cost, a stable deposition rate, and rapid mass production. This study investigated various thicknesses of a silver film ranging from 3.0 to 12.1 nm by field emission scanning electron microscope, X-ray diffraction, and X-ray photoelectron spectroscopy. In the rhodamine 6G (R6G) testing irradiated by a He-Ne laser beam, the analytical enhancement factor (AEF) of 9.35 × 108, the limit of detection (LOD) of 10-8 M, and the relative standard deviation (RSD) of 1.61% were better than the other SERS substrates fabricated by the same DC sputtering process because the results showed that the 6 nm thickness silver layer had the highest sensitivity, stability, and lifetime. The paraquat and acetylcholine analytes were further investigated and high sensitivity was also achievable. The proposed SERS samples were evaluated and stored in a low humidity environment for up to forty weeks, and no spectrum attenuation could be detected. Soon, the proposed technology to fabricate high sensitivity, repeatability, and robust SERS substrate will be an optimized process technology in multiple applications.

Atopic dermatitis (AD) is a chronic, itchy skin condition that affects 15-20% of children but may occur at any age. It is estimated that 16.5 million US adults (7.3%) have AD that initially began at age >2 years, with nearly 40% affected by moderate or severe disease. Therefore, a quantitative measurement that tracks the evolution of AD severity could be extremely useful in assessing patient evolution and therapeutic efficacy. Currently, SCOring Atopic Dermatitis (SCORAD) is the most frequently used measurement tool in clinical practice. However, SCORAD has the following disadvantages: (i) time consuming-calculating SCORAD usually takes about 7-10 minutes per patient, which poses a heavy burden on dermatologists and (ii) inconsistency-owing to the complexity of SCORAD calculation, even well-trained dermatologists could give different scores for the same case. In this study, we introduce the Automatic SCORAD, an automatic version of the SCORAD that deploys state-of-the-art convolutional neural networks that measure AD severity by analyzing skin lesion images. Overall, we have shown that Automatic SCORAD may prove to be a rapid and objective alternative method for the automatic assessment of AD, achieving results comparable with those of human expert assessment while reducing interobserver variability.

There have been some modest recent advancements in the research of Complex Regional Pain Syndrome, yet the amount and quality of the work in this complicated multifactorial disease remains low (with some notable exceptions; e.g., the recent work on the dorsal root ganglion stimulation). The semi-systematic (though in some cases narrative) approach to review is necessary so that we might treat our patients while waiting for "better research." This semi-systematic review was conducted by experts in the field, (deliberately) some of whom are promising young researchers supplemented by the experience of "elder statesman" researchers, who all mention the system they have used to examine the literature. What we found is generally low- to medium-quality research with small numbers of subjects; however, there are some recent exceptions to this. The primary reason for this paucity of research is the fact that this is a rare disease, and it is very difficult to acquire a sufficient sample size for statistical significance using traditional statistical approaches. Several larger trials have failed, probably due to using the broad general diagnostic criteria (the "Budapest" criteria) in a multifactorial/multi-mechanism disease. Responsive subsets can often be identified in these larger trials, but not sufficient to achieve statistically significant results in the general diagnostic grouping. This being the case the authors have necessarily included data from less compelling protocols, including trials such as case series and even in some instances case reports/empirical information. In the humanitarian spirit of treating our often desperate patients with this rare syndrome, without great evidence, we must take what data we can find (as in this work) and tailor a treatment regime for each patient.

Alternaria mycotoxins are ubiquitous mycotoxins that contaminate food and animal feed. Here, an UPLC-MS/MS was developed and used for the detection of seven Alternaria mycotoxins in 19 different edible and medicinal herbs. Extensive optimization resulted in a simple and convenient sample preparation procedure with satisfactory extraction and a lower matrix effect. LOQs ranged from 0.01 to 2.0 ng/mL. Recoveries varied between 71.44% and 112.65%, with RSD less than 12%. The method was successfully applied for use in the mycotoxin analysis of 260 samples. A high percentage (28.46%) of samples were contaminated by 1-5 mycotoxins. Alternariol mono methylether was the predominant mycotoxin with high percentage of positive samples (37.5%), followed by alternariol (22.5%), alternariol (17.5%), tentoxin (10.83%), altertoxin Ⅰ (7.5%), and altenusin (4.17%). Collectively, the natural incidence data obtained from this study will help with better, validated risk assessments and efforts towards more comprehensive, future regulation.

BACKGROUND: Cardiac surgery-associated acute kidney injury (AKI) can increase the mortality and morbidity, and the incidence of chronic kidney disease, in critically ill survivors. The purpose of this research was to investigate possible links between urinary metabolic changes and cardiac surgery-associated AKI. METHODS: Using ultra-high-performance liquid chromatography coupled with Q-Exactive Orbitrap mass spectrometry, non-targeted metabolomics was performed on urinary samples collected from groups of patients with cardiac surgery-associated AKI at different time points, including Before_AKI (uninjured kidney), AKI_Day1 (injured kidney) and AKI_Day14 (recovered kidney) groups. The data among the three groups were analyzed by combining multivariate and univariate statistical methods, and urine metabolites related to AKI in patients after cardiac surgery were screened. Altered metabolic pathways associated with cardiac surgery-induced AKI were identified by examining the Kyoto Encyclopedia of Genes and Genomes database. RESULTS: The secreted urinary metabolome of the injured kidney can be well separated from the urine metabolomes of uninjured or recovered patients using multivariate and univariate statistical analyses. However, urine samples from the AKI_Day14 and Before_AKI groups cannot be distinguished using either of the two statistical analyses. Nearly 4000 urinary metabolites were identified through bioinformatics methods at Annotation Levels 1-4. Several of these differential metabolites may also perform essential biological functions. Differential analysis of the urinary metabolome among groups was also performed to provide potential prognostic indicators and changes in signalling pathways. Compared with the uninjured kidney group, the patients with cardiac surgery-associated AKI displayed dramatic changes in renal metabolism, including sulphur metabolism and amino acid metabolism. CONCLUSIONS: Urinary metabolite disorder was observed in patients with cardiac surgery-associated AKI due to ischaemia and medical treatment, and the recovered patients' kidneys were able to return to normal. This work provides data on urine metabolite markers and essential resources for further research on AKI.

Skin, largest organ of human, is directly exposed to environment and hence is prone to high rates of injuries and microbial infections. Over the passage of time these microbes have developed resistance to antibiotics making them ineffective especially in lower doses and hence, higher dosages or new drugs are required. The current study deals with designing of nano-emulsion (NE) formulations composed of garlic and ginger oils (0.1 %) with neomycin sulphate used in different ratios (0.001, 0.01 and 0.1 %) and combinations. The resulting NEs were characterized for droplet size (145-304 nm), zetapotential (-3.0-0.9 mV), refractive index (1.331-1.344), viscosity (1.10-1.23cP), transmittance (96-99 %), FT-IR and HPLC and found stable over a period of three months. All NEs were also found effective against both gram positive and negative bacterial strains i.e., B. spizizenii, S. aureus, E. coli and S. enterica as compared to pure neomycin sulphate (NS) used as control with highest activity recorded for NE-2 and NE-4 against all strains showing zone of inhibition in range of 22-30 mm and 21-19 mm, respectively. NEs were also tested using rabbit skin excision wound model which potentiates that all the NEs resulted in early recovery with 86-100 % wound healing achieved in 9 days as compared to NS ointment (71 %). The studies confirmed that essential oils when used in combination with traditional drug can lead to much higher efficacies as compared to pure drugs.

We present the acute worsening of clinical presentation in complex regional pain syndrome (CRPS), following mRNA-based COVID-19 vaccination. We report the case series of three patients diagnosed with CRPS type I who presented with acute worsening of symptoms after mRNA-based COVID-19 vaccination and their medical management. The acute clinical worsening after mRNA-based COVID-19 vaccination was transient and effectively managed with adjustment of individualized therapy in all three patients. The mortality and morbidity of COVID-19 infection are serious, and vaccination is recommended in the general population, including patients with a diagnosis of CRPS. However, clinicians should be aware of the possibility that there may be a clinical worsening CRPS after mRNA-based COVID-19 vaccination.


This case series presents the worsening of symptoms in complex regional pain syndrome (CRPS), following mRNA-based COVID-19 vaccination. We report on three patients diagnosed with CRPS who presented with worsening of symptoms after mRNA-based COVID-19 vaccination and their medical management. The worsening of symptoms was temporary and managed with adjustment of individualized therapy in all three patients. The dangers of COVID-19 infection are serious, and vaccination is highly recommended in the general population, including patients with a diagnosis of CRPS. However, healthcare providers should be aware of the possibility of clinical worsening of CRPS after mRNA-based COVID-19 vaccination.

BACKGROUND: In the wake of the controversy surrounding the SYMPLICITY HTN-3 trial and data from subsequent trials, this review aims to perform an updated and more comprehensive review of the impact of renal sympathetic denervation on cardiac arrhythmias. METHODS AND RESULTS: A systematic search was performed using the Medline, Scopus and Embase databases using the terms "Renal Denervation" AND "Arrhythmias or Atrial or Ventricular", limited to Human and English language studies within the last 10 years. This search yielded 19 relevant studies (n = 6 randomised controlled trials, n = 13 non-randomised cohort studies) which comprised 783 patients. The studies show RSD is a safe procedure, not associated with increases in complications or mortality post-procedure. Importantly, there is no evidence RSD is associated with a deterioration in renal function, even in patients with chronic kidney disease. RSD with or without adjunctive pulmonary vein isolation (PVI) is associated with improvements in freedom from atrial fibrillation (AF), premature atrial complexes (PACs), ventricular arrhythmias and other echocardiographic parameters. Significant reductions in ambulatory and office blood pressure were also observed in the majority of studies. CONCLUSION: This review provides evidence based on original research that 'second generation' RSD is safe and is associated with reductions in short-term blood pressure and AF burden. However, the authors cannot draw firm conclusions with regards to less prominent arrhythmia subtypes due to the paucity of evidence available. Large multi-centre RCTs investigating the role of RSD are necessary to comprehensively assess the efficacy of the procedure treating various arrhythmias.

Portable Emission Measurement Systems (PEMS) are commonly used to measure absolute (mass per unit distance) emissions of a range of pollutants from road vehicles under real driving conditions. Because measuring large numbers of vehicles with PEMS is impractical, this paper investigates how vehicle emission remote sensing device (RSD) can supplement the use of PEMS. We simulate whether remote sensing measurements can accurately predict a vehicle's real-world distance-specific nitrogen oxides (NOX) emissions using RSD without measuring its exhaust flow rate. The approach uses readily available type-approval carbon dioxide (CO2) emission data together with average real-world divergences from studies based on user-reported fuel economy data. We find that at least 30 RS measurements from a given vehicle's journey are needed to reach a mean absolute error of 30% compared to a large reference data set of individual PEMS measurements. With that condition met, it is concluded that estimates agree well with actual NOX emissions from cars and the applied method does not introduce a systematic bias. It is also found that the accuracy of estimates for distance-specific NOX emissions does not significantly improve when more than 300 remote-sensing samples are available, with a mean absolute error converging to 23%. We conclude that this method could be used to screen large car fleets and identify vehicles or group of vehicles that are likely grossly exceeding air pollution standards.

Everolimus is an immunosuppressant agent used in organ transplantation and, more recently, in cancer therapy. It has demonstrated beneficial effects in breast cancer, renal cancer, and neuroendocrine tumours. However, the treatment is not without side effects, some of which are still little known. We report the case of a 56 year-old man with a diagnosis of neuroendocrine tumour who developed a complex regional pain syndrome (CRPS) secondary to treatment with everolimus. CRPS has been linked to treatments with everolimus in renal and breast cancer patients as well as in renal transplant patients. To our knowledge, this is the first case of CRPS in a neuroendocrine tumour patient on everolimus treatment.

Angelica dahurica is a famous functional food and herb. To guarantee quality of A. dahurica, a strategy "Q-markers targeted screening" was successfully developed by sufficient extraction of compounds and the targeted screening of qualitative and quantitative markers calculated through chemometric methods based fingerprints. Accelerated solvent extraction was selected due to its prominent advantages exhibiting the maximum extraction yields and varieties of compounds and especially excellent reproducibility (RSD < 1). After extraction, the fingerprints of A. dahuricae samples were established. For the preliminary herb authenticity, the targeted screening of 23 quantitative markers were performed by similarity analysis and hierarchical cluster analysis based on the fingerprints, which were identified by liquid chromatography tandem mass spectrometry (LC-MS). Subsequently, for further quality control, the targeted screening of nine quantitative markers were done by similarity analysis & linear discriminant analysis, which were determined by LC. Lastly, the strategy was successfully applied to quality assessment of A. dahurica samples.
