RESUMO

Se presenta el caso clínico de persona viviendo con VIH, con mala adherencia a tratamiento, sin vacunación previa para mpox, que evolucionó con un cuadro clínico probable de síndrome de reconstitución inmune posterior a reinicio de TAR, debido a la progresión de las lesiones cutáneas. Recibió tratamiento con tecovirimat por siete días, con evolución clínica favorable. Corresponde al primer caso reportado que recibió terapia con tecovirimat en Chile.


We report a clinical case of a person living with HIV with poor adherence to treatment, no previous mpox vaccination, who had a probable mpox syndrome immune reconstitution after restarting ART, due to worsening of skin lesions. He received treatment with tecovirimat for 7 days, clinically improved and was discharged in good condition. We reported this first clinical case that received tecovirimat in Chile.

Resumen Si bien la buena adherencia al tratamiento antirretroviral en la infección por el virus de la inmunodeficiencia humana (HIV por Human Immunodeficiency virus en inglés o VIH, por su sigla en español) demostró una sustancial mejoría clínica en las personas que viven con él, lograr el acceso al tratamiento continúa siendo un desafío en los tiempos que corren. Los estudios de laboratorio para diagnóstico y seguimiento de la infección por HIV se centran en estudios virológicos y recuento de linfocitos CD4+ y CD8+. Sin embargo, no se evalúa de forma detallada el perfil inmunológico de las personas que viven con HIV, ni la reconstitución inmune luego de recibido el tratamiento. Es por ello que el objetivo de esta revisión fue describir, por un lado, los diversos estudios realizados desde el laboratorio para el diagnóstico y seguimiento de la infección por HIV. Por otro lado, describir los aportes del laboratorio bioquímico en el estudio del perfil inmunológico de las personas que viven con HIV, el cual incluye desde determinaciones actualmente en uso, hasta nuevos biomarcadores inflamatorios solubles o de membrana celular, como así también las subpoblaciones linfocitarias. Dichos biomarcadores podrían ser herramientas valiosas como descriptores del estado inmunológico de las personas e, incluso, predictores de patologías asociadas a la infección por HIV.


Abstract Although a correct adherence rate to antiretroviral treatment in human immunodeficiency virus (HIV) infection reveals a substantial clinical improvement in people living with HIV, achieving access to treatment is still challenging. Laboratory studies for diagnosis and follow-up are mainly focused on virological status, CD4+ and CD8+ T cells counts. However, the immunological profile of people living with HIV is not evaluated in detail, neither is the immune reconstitution after treatment. Consequently, the aim of this review was is to describe, on the one hand, the studies carried out in the laboratory for the diagnosis and monitoring of HIV infection, and on the other hand, is to describe the contributions of the biochemical laboratory in the study of the immunological profile of people living with HIV. This study includes determinations currently in use, and the determination of new soluble or cell membrane inflammatory biomarkers, as well as T cell subsets. These biomarkers could be valuable tools as descriptors of the impervious state of people, and even predictors of pathologies associated with HIV infection.


Resumo Embora a boa adesão ao tratamento antirretroviral na infecção pelo vírus da imunodeficiência humana (HIV) tenha demonstrado melhora clínica substancial em pessoas que vivem com HIV, conseguir o acesso ao tratamento continua sendo um desafio nos momentos em que correm. Os estudos laboratoriais para diagnóstico e monitoramento da infecção pelo HIV concentram-se em estudos virológicos e contagem de linfócitos CD4+ e CD8+. No entanto, o perfil imunológico das pessoas que vivem com HIV não é avaliado detalhadamente, nem a reconstituição imunológica após o tratamento. É por isso que o objetivo desta revisão foi descrever, por um lado, os vários estudos realizados em laboratório para o diagnóstico e monitoramento da infecção pelo HIV. Por outro lado, descrever as contribuições do laboratório bioquímico no estudo do perfil imunológico de pessoas que vivem com HIV, que inclui desde determinações atualmente em uso, até novos biomarcadores inflamatórios solúveis ou de membrana celular, bem como subpopulações de linfócitos. Esses biomarcadores podem ser ferramentas valiosas como descritores do estado imunológico das pessoas e até mesmo como preditores de patologias associadas à infecção pelo HIV.

Strongyloides stercoralis es un nemátodo que se caracteriza por causar infección intestinal, usualmente asintomática, en pacientes inmunocompetentes. Sin embargo, en aquellos que viven con VIH, y de acuerdo con su estado inmune, puede generar un síndrome de hiperinfección con complicaciones diversas por diseminación a diferentes órganos. Se presenta el caso de un paciente de 30 años con diagnóstico de novo de infección por el virus de la inmunodeficiencia humana, con documentación de enfermedad linfoproliferativa. En los estudios complementarios se documenta la presencia de Strongyloides stercoralis de manera inusual en la médula ósea. A pesar de contar con un recuento de linfocitos T CD4+ mayor a 400 células/ µL y de haberse iniciado el manejo para esta condición con ivermectina, el paciente fallece por un estado séptico asociado al síndrome de hiperinfección, por lo cual se considera que este es un caso inusitado que obliga al clínico a tener en cuenta la presencia del nemátodo en pacientes que viven con VIH.


Summary Strongyloides stercoralis is a nematode that is characterized by causing a usually asymptomatic intestinal infection in immunocompetent individuals. However, in patients living with HIV and depending on their immune status, it can generate a hyperinfection syndrome with various complications due to dissemination to different organs. We present the case of a 30-year-old patient with a de novo diagnosis of human immunodeficiency virus infection and lymphoproliferative disease. Within the laboratory workup, the presence of Strongyloides stercoralis was documented in the bone marrow. Despite having a CD4+ T lymphocyte count greater than 400 cells/microliter and having started treatment for this condition with ivermectin, the patient died due to a septic state associated with hyperinfection syndrome, which is why this is considered an unusual case that alerts the clinician to take into account the presence of the nematode in patients living with HIV.

Introducción: La histoplasmosis es una micosis profunda o sistémica causada por un hongo dimórfico que se puede diseminar principalmente en pacientes con inmunosupresión, como los que tienen diagnóstico de virus de la inmunodeficiencia humana. El síndrome de reconstitución inmune consiste en un empeoramiento paradójico de una condición conocida o de nueva aparición después del inicio de la terapia antirretroviral. Objetivo: Describir un caso de histoplasmosis diseminada asociada a síndrome de reconstitución inmune en un paciente con infección por virus de la inmunodeficiencia humana. Caso clínico: Paciente masculino de 32 años con diagnóstico de infección por virus de la inmunodeficiencia humana, con cuadro clínico de tres semanas de evolución. Este cuadro inició posterior al comienzo de la terapia antirretroviral, que consistió en pápulo-nódulos umbilicados diseminados, con compromiso pulmonar; además, tenía histopatología y cultivo positivos para Histoplasma capsulatum sl. y prueba de antigenuria para histoplasma también positiva. Se consideró un diagnóstico de histoplasmosis diseminada con presentación cutánea, fue la expresión de un síndrome de reconstitución inmune por desenmascaramiento. Se inició manejo con anfotericina B liposomal y se mantuvo la terapia antirretroviral; posteriormente se continuó el tratamiento con itraconazol durante 12 meses con mejoría de las lesiones. Conclusiones: El diagnóstico clínico, histopatológico y microbiológico fue oportuno; el paciente presentó una adecuada respuesta al tratamiento. Esta es una micosis curable e incluso prevenible, si se diagnostica a tiempo, se inicia tratamiento precoz y se mantiene la terapia retroviral(AU)


Introduction: Histoplasmosis is a deep or systemic mycosis caused by a dimorphic fungus which may disseminate mainly in immunocompromised patients, such as those diagnosed with human immunodeficiency virus. Immune reconstitution syndrome is a paradoxical worsening of a known condition or a condition appearing after the start of antiretroviral therapy. Objective: Describe a case of disseminated histoplasmosis associated to immune reconstitution syndrome in a patient with human immunodeficiency virus infection. Case report: A case is presented of a male 32-year-old patient diagnosed with human immunodeficiency virus with a clinical status of three weeks' evolution. The current status developed after the start of antiretroviral therapy. It consisted in disseminated umbilicated papular nodules with pulmonary involvement, as well as positive Histoplasma capsulatum sl. histopathology and culture, and a positive histoplasma antigen test. A diagnosis of disseminated histoplasmosis with a cutaneous presentation was considered. It was the expression of immune reconstitution syndrome by unmasking. Treatment was started with liposomal amphotericin B, maintaining the antiretroviral therapy. Management was then continued with itraconazole for 12 months with improvement of the lesions. Conclusions: Timely clinical, histopathological and microbiological diagnosis was performed. The patient displayed an adequate response to treatment. This mycosis is curable and even preventable when a diagnosis is made in time, treatment is started early and the retroviral therapy is maintained(AU)

La tuberculosis es una enfermedad muy frecuente en nuestro medio. A pesar de que la detección precoz y el tratamiento adecuado logran la curación en la mayoría de los pacientes, la dificultad en el diagnóstico, el abandono del tratamiento y la aparición de resistencia a los fármacos tradicionales generan que, en la actualidad, continúe siendo un importante problema de salud pública. En la Argentina, la tasa de morbilidad es de 25/100 000 habitantes, con un leve aumento en la mortalidad.Se presenta el caso de una paciente pediátrica con tuberculosis, que tuvo múltiples complicaciones asociadas a la enfermedad y a su tratamiento, entre las cuales se incluye el síndrome in-flamatorio de reconstitución inmunológica, también conocido como reacción paradojal al tratamiento antituberculoso. Este representa una consecuencia clínica adversa al restablecimien-to de la inmunidad en el paciente que padece una infección sistémica grave, como la tuberculosis miliar.


Tuberculosis is a very frequent disease in our environment. Although early detection and adequate treatment achieve cure in most patients, the difficulty in diagnosis, the abandonment of treatment and the appearance of resistance to traditional drugs generate that at present it continues to represent an im-portant public health problem. In Argentina, the morbidity rate is 25/100,000 inhabitants, with a slight increase in mortality.We present the case of a pediatric patient with tuberculosis and multiple complications associated with the disease and its treatment. One of these complications was the immune re-constitution inflammatory syndrome or paradoxical reaction to antituberculosis treatment. It represents an adverse clinical con-sequence of the restoration of immunity in the patient suffering from a serious systemic infection such as miliary tuberculosis.

RESUMEN El síndrome de reconstitución inmune se produce debido a un aumento de la inmunocompetencia en pacientes previamente inmunocomprometidos. La situación es frecuente tras iniciar un tratamiento antirretroviral de alta eficacia, en pacientes con infección por el virus de inmunodeficiencia humana. En determinados casos, puede conllevar un empeoramiento paradójico de una infección previa. El citomegalovirus, es un germen oportunista que, en el seno de un síndrome de reconstitución inmune, puede dar lugar a perforación intestinal multifocal y peritonitis secundaria de difícil tratamiento. Es más frecuente en pacientes con recuento de linfocitos cooperadores inferior a 50 células/mm3 al iniciar el tratamiento antirretroviral. El objetivo es comunicar dicha situación a través, de un caso clínico para facilitar su sospecha lo más pronto posible, y realizar un tratamiento adecuado. Presentamos el caso de un paciente con virus de inmunideficiencia humana de reciente diagnóstico, en tratamiento con terapia antirretroviral de alta eficacia, que acude a urgencias con abdomen agudo secundario a perforación por citomegalovirus. La infección conlleva importante morbimortalidad, siendo imprescindible un diagnóstico temprano e iniciar precozmente el tratamiento antiviral intravenoso, asociado generalmente a tratamiento quirúrgico(AU)


ABSTRACT Immune reconstitution syndrome occurs due to increased immunocompetence in previously immunocompetent patients. The condition is frequent in patients with human immunodeficiency virus infection who have started a highly active antiretroviral therapy. In certain cases, the syndrome can lead to a paradoxical worsening of a previous infection. Cytomegalovirus is an opportunistic germ that, during an immune reconstitution syndrome, can lead to multifocal intestinal perforation and secondary peritonitis, in cases that are difficult to treat. The syndrome is more frequent in patients with CD4 lymphocyte count below 50/mm3 at the time of starting antiretroviral treatment. The objective is to communicate this situation through a clinical case presentation in order to facilitate suspicion as soon as possible, and to carry out appropriate treatment. We present the case of a patient with a recently diagnosed human immunodeficiency virus, under treatment with highly active antiretroviral therapy, who attended the emergency department with an acute abdomen secondary to perforation due to cytomegalovirus. Infection carries significant morbidity and mortality, and early diagnosis is essential and intravenous antiviral treatment should be started early, generally associated with surgical treatment(AU)

INTRODUCTION: The incidence of immune reconstitution inflammatory syndrome (IRIS) in HIV-infected patients after an episode of Pneumocystis jirovecii pneumonia (PJP) seems to be lower than with other opportunistic infections. We conducted an observational study in order to determine the incidence, clinical characteristics and outcome of patients diagnosed with PJP-related IRIS. METHODS: We conducted an observational study of HIV patients diagnosed with PJP-related IRIS from January 2000 to November 2015. We analyzed epidemiological and clinical characteristics as well as laboratory findings. We also carried out a systematic review of published cases. RESULTS: Six cases of IRIS out of 123 (4.9%) HIV-infected patients with PJP who started ART were diagnosed. All six cases were men with a median age of 34 (IQR: 8) years. The six patients developed paradoxical IRIS. Subjects younger than 40 years old (p=0.084) and with an HIV-RNA viral load >100000 copies/ml (p=0.081) at diagnosis showed a tendency to develop IRIS. Thirty-seven published cases of PJP-related IRIS were identified. Although 51% of cases involved respiratory failure, no deaths were reported. CONCLUSIONS: PJP-related IRIS is rare condition compared to other opportunistic infections. It can lead to a severe respiratory failure in a significant proportion of cases, although no deaths have been reported.

La tuberculosis es un factor de riesgo en los pacientes con sida, ya que una vez iniciado el tratamiento antirretroviral pueden de desarrollar un síndrome de reconstitución inmune, lo que favorecería el deterioro del su estado clínico. Se presenta el caso de un paciente masculino, de 24 años de edad, diagnosticado de sida hace 4 años, y tratamiento irregular con antirretrovirales. Acudió al Hospital Universitario Clínico Quirúrgico "Comandante Faustino Pérez Hernández" con fiebre elevada, acompañado de cuadro general, manifestaciones respiratorias y dolor inguinal derecho. En el examen físico se constató un cuadro adénico generalizado, fue hospitalizado para estudio y tratamiento. Se diagnosticó un síndrome de reconstitución inmune en un paciente de sida con una tuberculosis diseminada, el cual fallece a pesar de la terapéutica impuesta. Este síndrome se caracteriza por una restauración gradual de la inmunidad patógeno-específica, donde el sistema inmune es capaz de reconocer patógenos presentes pero clínicamente ocultos. Se asocia a otros factores de riesgo y puede ser letal; de ahí que el reconocimiento oportuno de los pacientes con alto riesgo de contraerlo, así como un adecuado manejo sobre cuándo iniciar la terapia antirretroviral en cada caso específico, es quizá la única forma de prevenir su desarrollo (AU).

The case of a male patient, aged 24 years is presented. He was diagnosed with AIDS four years ago, and was irregularly treated with antiretroviral. The patient assisted the Clinic-surgical University Hospital "Comandante Faustino Pérez Hernández" with high fever accompanied by general characteristics, respiratory manifestations and right inguinal pain. At the physical examination, generalized adenic characteristics were found. A syndrome of immune reconstitution was diagnosed in an AIDS patient with disseminated tuberculosis; the patient died in spite of the imposed therapy. This syndrome is characterized by the gradual restoration of the pathogen-specific immunity, where the immune system is able of recognizing the pathogens that are present but clinically hidden. It is associated to other risk facts and may be lethal; therefore the timely recognition of the patients at high risk of suffering it, and also an adequate management about when to begin the anti-retroviral therapy in each specific case, is the unique way of preventing its development (AU).

ResumenEl síndrome inflamatorio de reconstitución inmune se presenta en pacientes con infección por VIH o infección avanzada por el virus, días, semanas o meses después del inicio de la terapia antirretroviral. Se caracteriza por una restauración gradual de la inmunidad patógeno-específica donde el sistema inmune es capaz de reconocer atógenos presentes pero clínicamente ocultos. Característicamente se presenta después de iniciar la TARV cuando el sistema inmunitario comienza a recuperarse. Puede ser leve o potencialmente mortal.


AbstractThe immune reconstitution inflammatory syndrome occurs in patients with advanced HIV infection or HIV infection, days, weeks or months after initiation of antiretroviral therapy. It is characterized by a gradual restoration of pathogen specific immunity where the immune system is able to recognize pathogens presents but clinically occult.Characteristically it occurs after starting antiretroviral therapy when the immune system starts to recover. It can be mild or life threatening.

Introducción: Antes del año 1996, la infección por el virus de la inmunodeficiencia humana (VIH) era considerada una condición fatal; sin embargo, con la introducción de la terapia antirretroviral combinada, se transformó en una enfermedad crónica con gran incremento en su expectativa de vida. A pesar de esta terapia, existen una serie de complicaciones a las que los individuos con VIH están expuestos. El pulmón es el órgano más comúnmente afectado. Las infecciones oportunistas como la neumonía por Pneumocystis jirovecii , el compromiso pulmonar asociado a histoplasmosis, tuberculosis y micobacterias no tuberculosas, entre otras, aún son condiciones que amenazan a esta población. Objetivo: Esta revisión pretende abordar los cambios en el diagnóstico, pronóstico y epidemiología de los pacientes con VIH e infecciones pulmonares, luego de la introducción de la terapia antirretroviral combinada. Metodología: Se revisaron las bases de datos electrónicas MEDLINE (pubmed), EMBASE, SciELOy LILACS. Se incluyeron revisiones sistemáticas, ensayos clínicos aleatorizados, estudios controlados, series de tiempo, « antes y después ¼ , estudios observacionales desde el año 1995 hastadiciembre de 2014, así como datos epidemiológicos de la OMS. Conclusiones: Después de casi 20 años de su introducción, la terapia antirretroviral combinadaha modificado la historia natural de la infección por VIH, con disminución en la frecuencia depresentación y mortalidad relacionada con la mayoría de los patógenos que comprometen eltracto respiratorio.


Background: Prior to 1996, HIV was considered practically fatal. However, after highly active antiretroviral therapy was introduced, HIV became a chronic disease with a great increase in life expectancy. Despite this therapy, there are many complications to which an HIV infected person is exposed, with the lung the most commonly affected organ. Opportunistic infections such as pneumocystis pneumonia, histoplasmosis, tuberculosis, nontuberculous mycobacteria, among others, are still important threats to the HIV population. Objective: This review aims to address changes in the diagnosis, prognosis and epidemiology of HIV-infected patients with lung infections since the introduction of highly active antiretroviral therapy was introduced. Methodology: The electronic databases MEDLINE (PubMed), EMBASE, LILACS and SciELO were reviewed. Systematic reviews, randomised, controlled studies, case series, « before and after ¼ ,observational studies from 1995 until December 2014 were included in addition to epidemiological data from the WHO. Conclusions: After nearly 20 years since its introduction, combined antiretroviral therapy has changed the natural history of HIV infection, with a decrease in the frequency of occurrence and mortality related to most pathogens involving the respiratory tract.

BACKGROUND: Fungal infections have been commonly diagnosed in individuals with advanced HIV disease. Cryptococcosis, pneumocystosis, and histoplasmosis are the most frequent systemic mycoses in people suffering from HIV/AIDS. CASE REPORT: We report a case of multiple fungal infections in an advanced AIDS-patient. A 33-year-old HIV-positive man from Brazil was hospitalized due to diarrhea, dyspnea, emaciation, hypoxemia, extensive oral thrush, and a CD4+ T lymphocyte count of 20cells/mm(3). Honeycombed-structures consistent with Pneumocystis jirovecii were observed by direct immunofluorescence in induced sputum. Cryptococcus neoformans was recovered from respiratory secretion and cerebrospinal fluid cultures. Histopathology of the bone marrow also revealed the presence of Histoplasma capsulatum. Molecular assays were performed in a sputum sample. Nested-PCR confirmed the presence of P. jirovecii and H. capsulatum; qPCR multiplex was positive for C. neoformans and H. capsulatum. With the treatment of antifungal drugs the patient progressed satisfactorily. CONCLUSIONS: The diagnosis of several systemic mycoses demonstrates the vulnerability of advanced AIDS-patients. Thus, the detection of AIDS cases in the early stages of infection is necessary for a prompt and adequate introduction of HAART therapy, and the use of prophylaxis to control opportunistic infections.

Patients receiving treatment for tuberculosis may experience an unexpected deterioration of their disease; this is known as a paradoxical reaction. We present the case of a 59-year-old man with lupus vulgaris who experienced a paradoxical deterioration of cutaneous lesions after starting antituberculosis therapy. The reaction was self-limiting; the lesions gradually improved, and the final outcome was very good. Paradoxical reactions are well-known in patients with human immunodeficiency virus (HIV) infection who start antiretroviral therapy, but they can also occur in non-HIV-infected patients with tuberculosis who start antituberculosis therapy. In the literature reviewed, paradoxical reactions involving skin lesions were described in patients with miliary tuberculosis. The case we report is the first of a paradoxical reaction in lupus vulgaris. The increasing frequency of tuberculosis in Spain could lead to a rise in the number of paradoxical reactions.

Hombre de 58 años con diagnóstico de leucemia linfoide aguda llevado a trasplante de progenitores de sangre periférica de donante intrafamiliar. Requirió manejo inmunosupresor por presencia de enfermedad injerto contra huésped aguda y crónica. Consultó por dolor lumbar severo y en los exámenes se encontraron lesiones compatibles con espondilodiscitis L4-L5 y colecciones paravertebrales. Con sospecha de tuberculosis se inició tratamiento empírico, la evolución clínica inicial fue tórpida por persistencia de síntomas y compromiso neurológico progresivo, por lo cual se sospechó presencia de micobacterias atípicas vs resistentes. Se logró aislamiento de Mycobacterium tuberculosis susceptible a la terapia instaurada. Se continuó esquema de tratamiento TBC con evolución clínica a la mejoría. Se interpretó como reacción paradójica a la terapia, pero con presencia de lesión renal en la que se confirmó recaída de la enfermedad hematológica...


We report the case of a 58 year-old male patient diagnosed with acute lymphoid leukemia who received a peripheral blood stem cell transplant from a family member. He required immunosuppressive management for developing acute and chronic graft-versus-host disease. He presented with severe back pain. Imaging showed spondylodiscitis L4/L5 and paravertebral fluid collections. Empirical anti-TB therapy was instituted for suspicion of tuberculosis. The initial phase was torpid due to persistent symptoms and progressive neurologic compromise, thus, the presence of atypical vs resistant mycobacteria was suspected. Initiated-therapy-susceptible Mycobacterium tuberculosis was isolated. The anti-TB therapy regimen was continued with improvement in his clinical state. It was interpreted as a paradoxical response to therapy, associated with the presence of a renal lesion in which a relapse of the hematologic condition was confirmed...

This syndrome occurs in human immunodeficiency virus infected patients after starting antiretroviral treatment. The most frequent associated infections are those produced by mycobacteria, herpes, cryptococcosis, hepatitis B, cytomegalovirus, Pneumocystis jirovecii and worsening of progressive multifocal leukoencephalopathy secondary to JC virus. We present the case of a patient with human immunodeficiency virus who developed the immune reconstitution syndrome secondary to Pneumocystis jirovecii.

En el contexto de una severa inmunodepresión existe una marcada dificultad para hacer diagnóstico definitivo de las infecciones oportunistas que son causa principal de morbilidad y mortalidad. La presencia de infecciones del SNC durante la evolución del paciente con VIH, es frecuente, siendo menos habitual la presencia múltiple y concurrente de éstas, pues tal situación somete a los pacientes a peor pronóstico. Por este motivo se muestra el estudio de un paciente que presentó múltiples infecciones oportunistas en el SNC, quien tras iniciar y continuar el TARGA mostró buena evolución. Caso clínico: varón de 51 años con antecedentes de alcoholismo que acude al hospital con s¡ndrome de hipertensión endocraneana. Es diagnosticado de meningitis criptocócica e infección por VIH estadio C sin TARGA. Durante el tratamiento se evidenci ademas infecciones oportunistas concurrentes, como meningitis tuberculosa, leucoencefalopatia multifocal progresiva y toxoplasmosis cerebral; conjuntamente presentó síndrome inflamatorio de reconstitución inmunológica (IRIS). El paciente al iniciar y mantener el TARGA se recuperó ¡ntegramente y manifestó buena evolución, actualmente es tratado ambulatoriamente y no se ha complicado. Interpretación: los episodios múltiples y concurrentes de infecciones oportunistas del SNC descritos as¡ como el IRIS, generalmente inducen a los pacientes a mal pronóstico, sin embargo, son pocos los casos que se recuperan copiosamente y muestran buen pronóstico. Este es un caso particular de recuperación después de seguir el TARGA correctamente. Se describe este reporte por su importancia para valorar una apropiada adherencia al TARGA.


In the context of severe immunosuppression, there is a marked difficulty in making a definitive diagnosis of opportunistic infections, which are leading causes of morbidity and mortality. The presence of CNS infections during the evolution of HIV patients is common, less common being multiple and concurrent presence of these. This situation undergoes patients with poor prognosis. For this reason we report the case of a patient who developed multiple opportunistic infections in the CNS, who after HAART initiation and continuation showed good performance. Clinical case: 51 year old male with a history of alcoholism, was admitted to hospital with intracranial hypertension syndrome. He was diagnosed with cryptococcal meningitis and HIV infection stage C without HAART. During treatment also showed concurrent opportunistic infections such as TB meningitis, progressive multifocal leukoencephalopathy and cerebral toxoplasmosis, jointly presented immune reconstitution inflammatory syndrome (IRIS). The patient, initiating and maintaining HAART, fully recovered and showed good performance. Currently he is treated as an outpatient and has no complications. Interpretation: The multiple concurrent episodes of opportunistic infections of the CNS described, as well as the IRIS, generally induce poor prognosis in patients. However, there are few cases that improve and show good prognosis. This is a particular case of recovery after HAART follow correctly. We describe this report because of its importance in assessing adherence to HAART.

OBJECTIVES: To investigate whether an unrecognised diagnosis of tuberculosis (TB) at the start of antiretroviral therapy (ART) influences subsequent CD4+ T cell (CD4) count recovery in an urban HIV clinic in Uganda. METHODS: In a retrospective cohort study, a multivariable polynomial mixed effects model was used to estimate CD4 recovery in the first 96 weeks of ART in two groups of patients: prevalent TB (started ART while on TB treatment), unrecognised TB (developed TB within 6 months after start ART). RESULTS: Included were 511 patients with a median baseline CD4 count of 57 cells/mm(3) (interquartile range: 22-130), of whom 368 (72%) had prevalent TB and 143 (28%) had unrecognised TB. Compared with prevalent TB, unrecognised TB was associated with lower CD4 count recovery at 96 weeks: -22.3 cells/mm(3) (95% confidence interval -43.2 to -1.5, P = 0.036). These estimates were adjusted for gender, age, baseline CD4 count and the use of zidovudine-based regimen. CONCLUSIONS: Unrecognised TB at the time of ART initiation resulted in impaired CD4 recovery compared with TB treated before ART initiation. More vigilant screening with more sensitive and rapid TB diagnostics prior to ART initiation is needed to decrease the risk of ART-associated TB and sub-optimal immune reconstitution.

La criptococosis es una enfermedad oportunista definitoria de sida con bajo incidencia en población pediátrica y de elevada mórbida-mortalidad. Presentamos el caso de un adolescente con diagnóstico reciente de sida con criptococosis diseminada como parte de un síndrome de reconstitución a la séptima semana de inicio de terapia antirretroviral, quien recibió manejo de forma exitosa con anfotericina B en monoterapia y sin complicaciones a un año de seguimiento.


Cryptococcosis is an AIDS-defining opportunistic disease with low incidence in the pediatric population associated with high morbidity and mortality. We report the case of a teenager newly diagnosed with AIDS with disseminated cryptococcosis as part of an immune reconstitution syndrome in the seventh week of initiation of antiretroviral therapy, who received mono therapy with amphotericin B without complications to a year follow up

RESUMO

La pandemia global del VIH ha representado un inmenso desafío al diagnóstico, tratamiento, y prevención de la tuberculosis- la principal causa de morbilidad y mortalidad entre personas que viven con el VIH/SIDA. Los elevados índices de mortalidad temprana documentados en personas con ambos padecimientos subrayan la necesidad de pruebas de diagnóstico rápidas y de tratamiento oportuno por la atipicidad de su presentación , la presencia en algunos pacientes del síndrome inflamatorio de reconstitución inmune al comienzo de la terapia antirretroviral y el desarrollo de diferentes grados de resistencia a las drogas tuberculostáticos.


HIV global pandemic has represented an immense challenge when diagnosing, treating and preventing tuberculosis- the main cause of morbidity and mortality among persons living with HIV/AIDS. The high indexes or early mortality documented in persons with both maladies states the necessity of fast diagnostic tests and opportune treatment because of the atypicity of their presentation, the presence of the immune reconstitution inflammatory syndrome at the beginning of the antirretroviral therapy, and the development of different resistance levels to antituberculosis drugs.
