RESUMO

Un paciente de 18 anos de edad se presentó para una rehabilitación mandibular parcialmente edéntula con implantes luego de una herida por arma de fuego. El examen radiográfico mostró avulsión severa en el área entre el primer molar inferior izquierdo y el primer premolar inferior derecho. El paciente deseaba una rehabilitación fija con el restablecimiento del plano oclusal y su apariencia facial. Desde el punto de vista estético, era necesario proveer una restauración con coronas y eliminar la situación biomecánica desfavorable. Se realizó una distracción vertical mandibular por medio de dos distractores tipo Martin. Al terminar el periodo de distracción y consolidación los dos distractores fueron retirados y se injerto el nuevo reborde alveolar. Luego de 4 meses se colocaron 10 implantes los cuales fueron cargados 4 meses después. Seguimiento a largo plazo de 7 anos mostrando un resultado satisfactorio.


An 18-year old male patient presented for rehabilitation of a partial edentulous mandible with endosseous implants after shot injury. Radiologic examination showed evidence of severe atrophy in the interforaminal area and pronounced bone destruction. The patient desired a fixed rehabilitation with re-establishment of anterior oclussal plane and his facial appearance. From an esthetic standpoint, it was necessary to provide a restoration with adequate crowns while avoiding an unfavorable biomechanical situation. Vertical distraction of the complete mandible was performed using two Martin distractors at the end of the period of activation and consolidation; two distractors were removed and autogenous veneer grafting was performed. After four months 10 implants were placed in predetermined sites. They were loaded with restorations four months later. The absence of any pathologic symptoms or negative radiologic findings 3 years after the surgery suggests a satisfactory result in the long-term.
