RESUMO

Screening studies on dental morbidity of elderly people in the urban and rural population of the Republic of Kazakhstan were conducted. Three age groups were investigated in the districts of Almaty and Almaty oblast: middle-aged (45-59 years old), elderly (60-79 years old) and the oldest (80 years and above). A high loss of teeth was revealed, which was 79.3% in the urban and 90.6% in the rural population of the Republic of Kazakhstan, respectively. Complete absence of teeth on the upper jaw was most common in rural and city dwellers in the oldest and elderly age groups. A high frequency of complete absence of teeth was revealed in elderly rural residents (44.9%). The production of removable prostheses with a double layer base improves the quality thereof. Indications for these prostheses have been determined. In conclusion, the need for orthopedic dental care was the highest in rural areas. Complete absence of teeth on the upper jaw was most common in the oldest and elderly age groups among rural and urban dwellers. A high frequency of complete absence of teeth was revealed among elderly rural residents (44.9%). Orthopedic treatment of persons with complete absence of teeth requires improvement of this assistance. The use of removable prostheses with a two-layer base improved the quality of manufactured prostheses, providing comfort and functional efficiency for patients. Indications for the manufacture of soft-lined prostheses have been determined and clarified.

Restoration of the masticatory function is one of the key elements in the comprehensive rehabilitation for patients with acute cerebrovascular disorders (ACVD) of various degrees, as their consequences may include an impaired coordinated action of the neuromuscular system, and the dentoalveolar system in particular. In this context, the clinical presentation of stroke is characterized by motor and sensory deficiencies, which can eventually lead to dysfunction of the masticatory muscles, tongue, lips, soft palate and pharynx. The bilateral difference between such disorders, manifested by the discrepancy in the muscle mass and kinematic characteristics, is the peculiarity of this phenomenon. AIM: The aim of the research was to examine the features of articulatory activity in the mandible during chewing at the stages of orthopedic rehabilitation in patients with complicated acute cerebrovascular disorder with the neurological deficit by hemitype during the prosthetics with removable orthopedic appliances. MATERIALS AND METHODS: The study sample consisted of 45 subjects aged from 40 to 65 years, including 24 women (53%) and 21 men (47%). The study group included 25 patients with complicated ACVD with the neurological deficit by hemitype and 20 subjects for control who did not have general somatic disorders. All patients underwent prosthetics with partial removable laminar dentures with acrylic base and clasp fixation system. CONCLUSIONS: Non-parametric comparison in the groups showed that the most significant differences in videokinesiography between the representatives of the second group (ACVD) and the control group on the 30th day of observation were: a relatively smaller amplitude of vertical movements (p =.0001) and smaller amplitude of horizontal movements (p =.0000). Thus, the amplitude of vertical and horizontal movements of the mandible, in our opinion, can be considered as a reliable marker of the functional activity of the dentoalveolar system in patients with a complicated course.

PURPOSE: The purpose of this retrospective, cross-sectional study is to evaluate if there is a difference in number of visits (including fabrication and postoperative) and remake rate when comparing conventionally fabricated and digitally fabricated complete dentures by dental students in a predoctoral student dental clinic. MATERIALS AND METHODS: This two-year retrospective cross-sectional study consisted of a chart review for patients receiving maxillary and/or mandibular complete dentures between 2017 and 2019 (n = 314) at the UNC Adams School of Dentistry predoctoral student clinic. No control group was determined for this study. Data were extracted for 242 conventional dentures and 39 digital dentures. Objective treatment outcomes were obtained for each included denture: the number of patient appointments from preliminary impressions to denture placement, the number of postoperative visits, any complications noted, and any need for remakes. Fisher's Exact Test and Cochran-Mantel-Haenszel analysis were completed with statistical significance set at p < 0.05. RESULTS: For the number of visits from preliminary impression to placement, 50% of conventionally fabricated dentures had 6 or more visits, while only 5% of digitally fabricated dentures had 6 or more visits. This difference for the number of patient visits was statistically significant (p < 0.05). Additionally, conventionally fabricated dentures required an average of 2-3 postoperative visits, whereas digitally fabricated dentures required 1-2 postoperative visits. This difference was also statistically significant (p < 0.05). For the number of dentures requiring remake, there was no statistical difference (p = 0.1904). CONCLUSIONS: When comparing conventionally fabricated and digitally fabricated dentures in the predoctoral clinic, the digitally fabricated dentures required fewer patient appointments from start to finish, and fewer postoperative appointments than conventionally fabricated dentures. Fewer visits may be an important consideration for patients, especially those with limited access to care.

BACKGROUND AND OVERVIEW: In this report, the authors describe a combined digital and conventional work flow that enabled placement of immediate dentures in 2 patient visits. CASE DESCRIPTION: Digital intraoral impressions and interarch relationship were obtained with an intraoral scanner during the first visit. The residual teeth from the digital intraoral impressions were removed, and a series of modules were used to design trial denture base and occlusal rim assemblies. The assemblies of both arches were fabricated with a 3-dimensional printer and resin material. Dental stone was poured into the intaglio of each assembly to fabricate dental casts. The assemblies and their corresponding dental casts were mounted on the articulator. The portion of the resin occlusal rim was removed, and the artificial teeth were arranged in an alternating manner; the immediate dentures were processed via conventional protocols. During the second visit, these immediate dentures were placed after extraction and relined with a denture relining material. CONCLUSIONS AND PRACTICAL IMPLICATIONS: Advances in digital technologies have enabled the establishment of treatment plans that would not have been considered previously. The combination of digital and conventional work flow in fabricating immediate dentures has eliminated the risk of experiencing unintentional tooth extraction during impression-making procedures, reduced the number of patient visits for the placement of immediate dentures, and resulted in satisfactory esthetic and functional outcomes.

Introducción: La belleza tiene una "proporción ideal" que se presenta en ciertas formas geométricas de la naturaleza con las características de ser armónicas, simétricas y equilibradas. Según Ricketts "es necesario utilizar proporciones áuricas ya que son atractivas y recordadas en el sistema límbico como bellas, armónicas y equilibradas". Objetivo: comparar proporciones faciales en el paciente desdentado sin prótesis y después de la rehabilitación con prótesis totales. Métodos: se obtuvieron los datos de 41 pacientes rehabilitados en la Clínica Odontológica de Prótesis Totales del Departamento de Prótesis, de la Universidad de Chile. Ellos fueron fotografiados mediante el equipo de Cone beam computed volumetric tomography con escaneo facial integrado y luego se midieron y compararon tres proporciones faciales antes y después de la rehabilitación con prótesis removible, y se observó cómo estas proporciones se acercaban o alejaban de la proporción áurica. El análisis estadístico de los datos obtenidos se realizó utilizando el software STATA 11. Se aplicaron las pruebas de Shapiro-Wilk y t de Student. Resultados: en la proporción 1 antes de la rehabilitación, se observó que 51,2 por ciento de los pacientes presentó proporción áurica y después de la rehabilitación se obtuvo que 63,4 por ciento presentó proporción áurica. En la proporción 2 antes de la rehabilitación, se observó que 22 por ciento de los pacientes presentó proporción áurica y después de la rehabilitación se obtuvo que 17,1 por ciento presentó proporción áurica. En la proporción 3 antes de la rehabilitación, se observó que ningún paciente presentó una proporción áurica y después de la rehabilitación se obtuvo que 7,3 por ciento presentó proporción áurica. Conclusiones: en esta muestra se presenta un cambio significativo en las proporciones faciales encontradas antes y después de la rehabilitación con prótesis en el paciente adulto mayor. La proporción ángulo externo del ojo a Stomion, en proporción con Stomion a Menton es la que muestra mayor cercanía a la proporción áurica antes y después de la rehabilitación con prótesis totales(AU)


Introduction: beauty has an "ideal proportion" that occurs in certain shapes of nature with the characteristics of being harmonious, symmetrical and balanced. According to Ricketts, you need to use auric proportions as they are attractive and remembered in the limbic as beautiful, harmonious and balanced system. Objective: to compare facial auricle proportions in the edentulous patient without dentures and after rehabilitation with dentures. Methods: we obtained data of 41 patients rehabilitated with total dental prostheses at the Clinic Department of Prosthodontics, University of Chile. They were photographed by the team of Cone beam computed volumetric tomography with facial scanning integrated and then measured and compared three facial proportions before and after rehabilitation with removable prosthesis, watching these ratios approaching or receding from the auric proportion. Statistical analysis of the data was performed using software STATA version 11. The tests Shapiro-Wilk and Student-T test were applied. Results: in the facial proportion 1 before rehabilitation, was observed that 51.2 percent of patients have auric proportion and after rehabilitation was obtained that 63.4 percent had auric proportion. In the facial proportion 2 before rehabilitation, was observed that 22 percent of patients had auric proportion and after rehabilitation was obtained that 17.1 percent had auric proportion. In the facial proportion 3 before rehabilitation, was observed that no patient had auric proportion and after rehabilitation we found that 7.3 percent had auric proportion. Conclusions: in this sample shows a significant change in facial proportions found before and after prosthetic rehabilitation in the elderly patients. Proportion external angle of the eye Stomion in proportion to Stomion Menton is showing closer to the auric ratio before and after rehabilitation with dentures(AU)

RESUMO

The present clinical report describes the prosthodontic management for a patient with uncontrolled bleeding and diabetes mellitus treated with a maxillary complete denture and a mandibular partial fixed dental prosthesis designed to interface with a removable cast framework partial denture retained by 2 ERA attachments. This approach was undertaken to improve both retention and stability of the distal extension Kennedy Class I removable partial denture. The rehabilitation provided better anterior esthetics than if treated with a conventional clasp retained removable partial denture, by employing a simple, practical design and offering a significant biomechanical advantages, restoring both oral health and function. Thus, this treatment modality, involving an ERA system and transfixation in fixed crowns, is an effective treatment and can be indicated as a clinical alternative for edentulous and partially edentulous patients with systemic disorders or for patients in economic situations that might preclude implant-based rehabilitation.
