RESUMO

Sludge generation from wastewater treatment plants in Uruguay has increased in recent years. Agricultural soils may be a final destination. A greenhouse experiment was conducted to quantify the effect of this sludge on 1) plant biomass production and nutrient concentration of sorghum (Sorghum bicolor var. vulgare); 2) the chemical properties of amended soils; and 3) assess whether heavy metal concentrations in sludge are appropriate according to environmental regulations. Two soils (S1 and S2) were amended with pure sludge (PS) and limed sludge (LS), with low dose (LD) of 16.0 and 17.3 Mg ha-1 and high dose (HD) of 32.0 and 34.6 Mg ha-1, respectively. Sludge treatments increased plants' nutrient absorption and dry matter production. The LS treatments incremented plant biomass production, depending on soil pH and nutrient availability. The effect of sludge treatments on elemental concentration in aboveground biomass depended on the element, treatments, and soil type. Mineralized nitrogen (N) and plant available phosphorus (P-Bray 1) values increased with sludge addition without exceeding Uruguay's critical soil level of P-Bray 1 for the sorghum crop. The PS did not increase metal concentration in soils. The LS slightly decreased soil Pb and slightly increased Cr and Zn soil concentration; levels were according to Uruguayan environmental guidelines. Therefore, agriculture soils are a viable final destination for PS and LS. Land applied sludge has acceptable levels of metals and promotes crop development.

RESUMO

The sanitary problem of Aedes aegypti mosquito acquires relevance around the world because it is the vector of dengue, zika, chikungunya and yellow fever. The vector is adapting to southern regions faster, and the propagation of these diseases in urban areas is a complex problem for society. We aimed to contribute to the risk prevention of disease transmission in the Metropolitan Area of Buenos Aires, through monitoring Aedes aegypti population levels and developing education campaigns with government agencies and society participation. Monitoring activities aimed to diagnose the presence of the vector and its ecology behaviour, and to generate education and prevention politics to avoid its propagation. The results show that (1) the mosquito is in the territory and it is spreading, (2) prevention activities of the municipalities are insufficient to generate an effective sanitary response and (3) it is necessary to improve the education programmes to the population about the life cycle of the vector. The integration of university, government and society improved the work of the team because it combined knowledge about vector ecology, diseases and territory characteristics.

RESUMO

Introduction of alien species is a well-known threat to biodiversity. Where newly introduced, alien species may pose a risk for the local ecological community by competing for resources or by introducing pathogens. Sylvilagus floridanus is an American lagomorph introduced into Europe in the second half of 20th century, for hunting. This study evaluated the structure and epidemiological characteristics of the gastrointestinal parasite community in an introduced population of S. floridanus in the Province of Alessandria (Piedmont Region-Italy). Three alien parasites were reported out of 271 animals: Obeliscoides cuniculi in the stomach, Trichostrongylus calcaratus in the small intestine, and Passarulus nonnanulatus in the large intestine. All these nematodes are commonly reported in S. floridanus in its natural range, but they represent alien species in Europe. The report of these alien parasites is an example of the unexpected consequences caused by the introduction of non-native vertebrates. The documented introduction of new pathogens may alter the parasite community of the native lagomorphs, with possible long-term effects on local ecological dynamics.

RESUMO

The sanitary risk prevention associated with the body overtakes nearly all other considerations. The organisation of funerals isn't actually managed by the authorities, and left to the will of funeral officers. How acceptable is this during a pandemic with elevated risks and a hightened awareness of death - not only of people, but also of a society at fault? An ethical approach is the only way to raise to the challenge of organising funerals more responsibly.

RESUMO

This study was conducted to address the dearth in works that simultaneously compare the growth and inactivation behaviors of selected pathogens in different milk products. In worst-case scenarios where hygienic practices are absent and heavy microbiological contaminations occur, Salmonella enterica, Escherichia coli O157:H7, Listeria monocytogenes, Pseudomonas aeruginosa, and Staphylococcus aureus multiplied in all samples at room temperature (27 °C). Most organisms readily proliferated with growth lag (tlag) values ranging from 0.00 to 5.95 h. Growth rates (KG) ranged from 0.16 to 0.67 log CFU/h. Sanitary risk times (SRTs) for a 1-log population increase ranged from 1.85 to 6.27 h, while 3.69-12.55 h were the SRTs determined for 2-log population increase. Final populations (Popfin) ranged from 7.11 to 9.36 log CFU/mL. Inactivation in heavily contaminated milk during Holder pasteurization revealed biphasic inactivation behavior with total log reduction (TLR) after exposure to 62.5 °C for 30 min ranging from 1.91 (90.8%) to 6.00 (99.9999%). These results emphasize the importance food safety systems in the handling of milk and milk products during manufacture and preparation.

Abstract The study aimed to develop a Quantitative Health Inspection Instrument (IQIS) large-sized Brazilian food and nutrition services. The inspection technology based on the Potential Risk Assessment Model (MARP) and the Brazilian Health Legislation was used. Twelve dimensions, 41 modules, and 57 risk control (critical/non-critical) indicators were structured on a scale of 0-5, totalling 1,512 indices with closed-ended response coding. The IQIS was validated with the Kappa Coefficient, with excellent agreement for the attributes of clarity and relevance (k = 0.82 and k = 0.92) and good agreement for applicability (k = 0.78). The Kruskal-Wallis test showed no statistically significant difference between the assessments (p = 0.423), the Intraclass Correlation Coefficient was satisfactory (ICC = 0.53), and Cronbach's Alpha (α = 0.71) was acceptable. The final result made it possible to classify the service as having an unacceptable health risk. IQIS is considered to have validated content, be reliable and reproducible to assess the hygienic-sanitary conditions, being a technological innovation for food and nutrition services and sanitary, allowing a detailed and rigorous inspection.

RESUMO

Sanitary inspection is used in low-, medium- and high-income settings to assess the risk of microbial contamination at water sources. However, the relationship between sanitary inspection and water quality is not well understood. We conducted a critical literature review and synthesized the findings of 25 studies comparing the results of sanitary inspection and microbial water quality analysis. Most studies used sub-standard sanitary inspection and water quality analysis methods, and applied simplistic comparisons that do not characterize the complexity of the relationship. Sanitary risk score was used to represent sanitary inspection results in 21 (84%) studies; of which 12 (57%) found a significant association between score and microbial water quality and nine (43%) did not. Participatory sanitary inspection (12%) and reporting results back to communities (24%) were uncommon. Most studies relied on laboratory-based water quality analysis as an independently sufficient measure of safety, but reported inadequate quality control (52%) and/or sub-standard sample processing methods (66%). We found that sanitary inspections could contribute to improving water safety through four mechanisms: guiding remedial action at individual water sources, allowing operators and external support programs to prioritize repairs, identifying programmatic issues, and contributing to research. The purpose of the sanitary inspection should be considered when planning sanitary inspection execution, data analysis, and reporting to ensure appropriate methods are employed and results are fit for purpose. Further exploration should recognize that sanitary risk factors represent sources of contamination, pathways for contaminants to enter water supplies, and breakdowns in barriers to contamination. These different sanitary risk factor types have different and inter-dependent effects on water quality.

RESUMO

A large amount of contaminated sites is shown around the world which may induce a health risk due to the presence of contaminants such as metal (loïd)s bearing phases. Health risk assessment is based on contaminant bioaccessibility. However, it is needed to understand every contaminant behavior in physiological matrix to be a realistic way to assess and interpret these sanitary risks. Due to the complexity of contaminated soil matrix, the use of synthetic minerals seems to be the better tool to understand their behavior in physiological matrix. Then, this study aims to highlight the environmental stability and the behavior during bioaccessibility ingestion (UBM) of selected synthetic lead-bearing phases. For this purpose, three Pb phases (galena, beudantite, and anglesite) commonly found in contaminated environments (particularly mining sites) were synthesized and characterized (structurally and morphologically). The sequential BCR extractions have shown that most of the lead is in a stable and non-mobilizable form (up to 93%). The lead present in these phases represents very few risks of migrating into the environment during physicochemical condition changes. The results of the bioaccessibility revealed a relatively high stability of the pure bearing phases in the physiological matrix. Lead is stable for 97.0% to 99.2% during the gastric phase and 97.0% to 99.9% during the gastro-intestinal phase. Moreover, the synthetic mixtures of galena/beudantite and anglesite/beudantite have been realized considering the proportions commonly found in the mining contexts. This has shown a similar behavior compared to pure phases except in the case of the anglesite mixture inducing a clear cocktail effect (drastic increase of Pb amount from gastro-intestinal phases). At last, this study is a first and interesting step to assess the behavior of these bearing phases in heterogeneous and complex medium such as soil.

Viral infections are frequently associated with reproductive problems in dairy and beef cattle worldwide. The aim of this study was to verify managerial practices that may constitute risk factors associated with infection by BoHV-1 and/or BVDV in dairy herds with a history of reproductive disease in extensively reared dairy cows without a previous history of vaccination against IBR and BVD. Neutralizing antibodies anti-BoHV-1, anti-BVDV or both were detected in 62.5% (165/264), 45.1% (119/264), and 31.4% (83/264), respectively, in the samples analyzed. The risk factors associated with infection by BoHV-1 were herds with more than 100 cows, the presence of mechanical milking, the non-utilization of artificial insemination, and the infrequent acquisition of animals. Risk factors associated with BVDV were dual-purpose herds (milk and beef), these include the utilization of mechanical milking, absence of quarantine for newly acquired animals, the presence of picket calving, and the absence of artificial insemination. For simultaneous infections by both viruses (BoHV-1 and BVDV) the use of mechanical milking increased the chance of infection 3.36-fold while the use of artificial insemination reduced the risk of infection by 56% in these herds.(AU)

RESUMO

Sanitary risk inspection, the systematic observation of contamination hazards, is often used to manage safety of water sources such as wells and boreholes. However, the replicability of sanitary risk inspections undertaken by different observers has not been studied. This study aimed to assess the replicability of sanitary risk inspections by two different observers in urban and peri-urban neighbourhoods of Greater Accra, Ghana. Two observers independently used a standard protocol to record contamination hazards around 62 groundwater sources, additionally recording urban-specific hazards such as damaged sewage pipes via a modified protocol. We calculated risk scores as the proportion of hazards observed at each source, separately for each observer. Linn's concordance correlation coefficient indicated very high agreement between the two observers' risk scores (n = 62; c = 0.949, 95% confidence limits 0.917-0.968). However, risk scores from urban-specific observations were uncorrelated with those from the standard protocol (r = 0.11, p = 0.41 for observer 1; r = 0.16, p = 0.22 for observer 2). Ours is the first study of replicability of sanitary risk observations and suggests high inter-observer agreement. However, urban contamination hazards were not captured using the standard protocol. In the future, assessment of inter-observer agreement and observations of urban-specific hazards could be incorporated into nationwide or regional sanitary risk surveys.

Groundwater nitrate contamination from agriculture is of paramount environmental interest. A continuous consumption of polluted water as drinking water or for culinary purposes is by no means a minor hazard for people's health that must be studied. This research presents a new methodology for the spatial analysis of health risk rate from intake of nitrate-polluted groundwater. The method is illustrated through its application to a water quality sampling campaign performed in the south of Spain in 2003. The probability risk model used by the US Environmental Protection Agency has been applied, considering a residential intake framework and three representative population age groups (10, 40 and 65 years).The method was based upon coupling Monte Carlo simulations and geostatistics, which allowed mapping of the health risk coefficient (RC). The maps obtained were interpreted in the framework of water resources management and user's health protection (municipalities). The results showed waterborne health risk caused by nitrate-polluted water is moderately low for the region. The observed risk was larger for the elderly and children, although no significant differences were found among the three age groups (RC average values of 95th percentile for age of 0.37, 0.33 and 0.37, respectively). Significant risk values of RC > 1 were obtained for 10 % of the surface in the NW site of the study area, where the municipalities with the highest contamination thresholds are located (agricultural activity). Nitrate concentration and intake rate stood out as the main explanatory variables of the RC.

RESUMO

The mechanisms responsible for negative biological effects due to airborne particulate matter (PM) exposure are still being studied, however the interactions between metals and biologic systems seem to be of primary importance. The aim of the study was to estimate a healthy risk linked to exposure to airborne PM10 metals by means of an environmental-sanitary risk assessment. Metals PM10 size distribution analysis was carried out in a central site of Torino city - Italy, then the Target Hazard Quotient (THQ) and the Cancer Risk (CR) were applied, according to standard EPA methods. All sampled metals were present on the different PM10 fractions, however some metals were distributed in some specific fractions: ANOVA test shows Cr, Cu, Mo and Pb as differently distributed among the eight fractions, rising the hypothesis of potential effects in specific tracts of respiratory system. Regarding the risk assessment, in general the CR was higher for an adult than for a child, conversely the THQ resulted higher for a child. While the concentrations of all the sampled metals appeared to be under control, their presence in the different PM10 fractions and their THQ and CR provided indications related to the body districts potentially in contact with these substances.

Health surveillance (HS) is defined as a group of actions capable of eliminating, reducing or preventing health risks and of intervening in sanitary problems resulting from the environment, the production and circulations of goods and the provision of health-care related services. This definition introduced the concept of risk, highlighting the role of HS: to control the risk in the consumption of products and technologies, in the use of health services, and in the exposition to environmental conditions. When intervening in sanitary risk, the HS professional needs to not only interact with others, but also integrate technical knowledge and legal framework in order to control an object which has multiple dimensions and properties and requires various forms of investigation.. However, there is a gap that is precariously explored within the HS regarding the comprehension of risk, especially when the magnitude of this objection to its actions is considered and the meanings of risk assigned by its professionals have not been studied. To understand the meanings attributed to risk in HS actions, a qualitative study was conducted using Symbolic Interactionism as a theoretical background and Grounded Theory as a method. Based on interviews with managers and professionals from local, state and national health surveillance agencies, a theoretical structure that contains the meanings of risk and its related categories was created.Understanding the meanings of risk was not restricted to its expression approaching the contexts, conditions, actions and consequences for understanding how professionals interpret the risk and assume its meanings in places in which they operate. The risk passes through the technical description, in the objective sense that refers to both formal aspects related to administrative, political and legal issues, as to the subjective aspects, linked to representations and value systems, surpassing the merely quantitative sense. These meanings...

RESUMO

Expired or unused medication at people's homes is normally disposed of in normal garbage, sewage system or, in certain cases, returned to the public health system. There is still no specific legislation regarding this leftover medication to regulate and orient the handling and correct disposal of medication waste. However, there is defined regulation regarding health services' solid waste. This article has the objective of discussing management models for the disposal of medication waste and the recommendations made by pertinent national and international legislation. By means of literature reviews, the management structure for medication waste of international legislation and the regulations regarding the environment, as well as the national legislation for the solid waste from health services was analyzed. Through the analysis it was possible to present better clarifications as to the possible impacts to the environment, to the public's health and alternatives in order to obtain the efficient disposal of medication, reducing and/or avoiding sanitary risk, guaranteeing the quality and safety of public health.

The term sanitary safety (SS) appeared in the international debate mainly due to the emerging sanitary crisis, although its meaning has remained obscure. This paper aims to analyze the concept of SS brought into the Brazilian sanitary surveillance upon the creation of the National Health Surveillance Agency. An exploratory case study was undertaken with technical data analysis and semi-structured interviews with informants who had taken part in the process of formulating the body's institutional design. The following categories were analyzed: incorporation of the SS term into the institutional mission, the SS concept and SS mechanisms. The SS concept was analyzed in both institutional and technical discursive dimensions. The former elicits the sense of strategy, a reliable relationship and legitimacy whereas the latter shows the sense of an acceptable risk-benefit relationship from the perspective of individual and collective health protection and promotion. The SS concept was found to encompass health-related products, technologies and services, especially those designed for medical diagnosis and treatment, but environmental issues received little mention. The scope of the SS concept was shown to be widening to include the surveillance of hospital infection, drugs and blood.

The world’s population is growing at such rate that not only the demands for water supply have raised considerably but also its contamination levels. Residues from domestic and industrial origin have generated environmental problems and public health issues. Water contamination coming from domestic origin, shows a higher risk because of the quantity of organic matter and the presence of pathogenic microorganisms. Due to determinations of these pathogens requiressophisticated laboratories and the techniques used for their detection are very costly and time consuming, it is proposed an alternative technique using bacteria, virus and parasites as faecal indicators, since they exhibit similar behaviour, and their determination techniques are a lot simpler, faster and low cost. The objective of the present study is to evaluate the occurrence, concentration and behaviour of indicators from bacterial origin as faecal coliforms, from viral origin as somatic coliphages and F-specific RNA bacteriophages, and from parasite origin such as helminth eggs, Giardia cysts and Cryptosporidium oocysts. A series of samples were taken at surface, groundwater, drinking water and wastewater treatment system in the surrounding area of Bogotá (Colombia). The results show the presence of all microbial indicators in all water samples, in similar concentrations to those found in different countries with different environments.
