Introdução: O implante de prótese mamárias é uma das cirurgias mais realizadas no mundo. Ao longo do tempo, diversos materiais foram utilizados com objetivo de reconstituir o volume mamário. Apesar das melhorias técnicas, cirúrgicas e da segurança dos implantes atuais, os pacientes são confrontados com potenciais complicações "não usuais": rupturas intracapsulares e extracapsulares, hematomas tardios e deformidade de contorno, silicone intralinfonodal ou herniação da cápsula fibrosa. A ressonância magnética (RMN) é a modalidade de imagem mais útil para investigação dessas complicações. Método: Trata-se de uma série de casos em que foram levantadas alterações ditas "não usuais", pela baixa frequência ou ausência na citação da literatura, após cirurgias de inclusão de prótese de silicone. Os dados foram coletados da experiência pessoal da clínica privada de um dos autores, na cidade de Brasília-DF, entre abril de 2015 e março de 2023. Resultados: Foram um total de 211 pacientes avaliados, e foram encontradas alterações menos frequentes nas RMN de 12 pacientes (5,68%), das quais: 5 com volumosa quantidade de líquido pericapsular, 3 com granuloma capsular, 1 seroma tardio com conteúdo hemorrágico,1 rotura intra e extracapsular, 1 nódulo junto à cápsula fibrosa do implante, 2 linfonodopatia axilar ipsilateral, 1 silicone intralinfonodal, 1 edema do músculo peitoral, 2 tumor desmoide e 1 herniação da cápsula fibrosa. Conclusão: Estima-se que existam 50 milhões de mulheres com próteses de mama no mundo. Com base nesse dado, o número de complicações ditas "não usuais" passa a ser um desafio diagnóstico para o cirurgião plástico e o radiologista.


Introduction: Breast prosthesis implantation is one of the most performed surgeries in the world. Over time, different materials were used to reconstitute breast volume. Despite technical and surgical improvements and the safety of current implants, patients are faced with potential "unusual" complications: intracapsular and extracapsular ruptures, late hematomas and contour deformity, intra-nodal silicone, or herniation of the fibrous capsule. Magnetic resonance imaging (MRI) is the most useful imaging modality for investigating these complications. Method: This is a series of cases in which so-called "unusual" changes were reported, due to their low frequency or lack of mention in the literature, after surgeries to include a silicone prosthesis. The data were collected from the personal experience of one of the authors in his private clinic, in the city of Brasília-DF, between April 2015 and March 2023. Results: A total of 211 patients were evaluated, and less frequent changes were found in the MRI of 12 patients (5.68%), of which: 5 with a large amount of pericapsular fluid, 3 with capsular granuloma, 1 late seroma with hemorrhagic content, 1 intra and extracapsular rupture, 1 nodule close to the implant's fibrous capsule, 2 axillary lymph node disease ipsilateral, 1 intra-nodal silicone, 1 pectoral muscle edema, 2 desmoid tumor and 1 herniation of the fibrous capsule. Conclusion: It is estimated that there are 50 million women with breast implants in the world. Based on this data, the number of so-called "unusual" complications becomes a diagnostic challenge for the plastic surgeon and radiologist.

While functioning in the oral cavity, denture soft linings (SL) are exposed to contact with the microbiota. Dentures can offer perfect conditions for the multiplication of pathogenic yeast-like fungi, resulting in rapid colonisation of the surface of the materials used. In vitro experiments have also shown that yeast may penetrate SL. This may lead to changes in their initially beneficial functional properties. The aim of this work was to investigate the effect of three months of exposure to a Candida albicans suspension on the mechanical properties of SL material and its bond strength to the denture base polymer, and to additionally verify previous reports of penetration using a different methodology. Specimens of the SL material used were incubated for 30, 60 and 90 days in a suspension of Candida albicans strain (ATCC 10231). Their shore A hardness, tensile strength, and bond strength to acrylic resin were tested. The colonization of the surface and penetration on fractured specimens were analysed with scanning electron and inverted fluorescence microscopes. Exposure to yeast did not affect the mechanical properties. The surfaces of the samples were colonised, especially in crystallized structures of the medium; however, the penetration of hyphae and blastospores into the material was not observed.

Maintenance of the quality and hygiene of maxillofacial prosthesis allows to maintain the health of the residual tissues. Sampling of the maxillofacial prostheses has relieved presence of microbial colonization on silicone surfaces. Cleaning procedures of maxillofacial silicones are done using mechanical means or using adjunctive with chemical means. Cleaning with a 2-4% chlorhexidine gluconate spray or dipping in solution for a minute and then washing under running water can sufficiently condition to reduce the amount of bacterial contamination. Due to rising microorganism resistance and fewer adverse effects, phytoextracts appear to be a viable option. Additionally, the use of excipients derived from plants is provides new opportunities for the pharmaceutical industry into the creation of innovative pharmaceutical products that are sustainable. Aim: To evaluate and compare the leaf extracts of Mangifera indica (M.indica), Anacardium occidentale(A.occidentale) and 0.2% chlorhexidine gluconate (CHX) on disinfection of maxillofacial silicone material surface contaminated with Staphylococcus aureus (S.aureus) and Candida albicans (C.albicans). Methods: Of the 150 maxillofacial silicone elastomer silicone samples, 75 samples were contaminated with S. aureus and 75 with C.albicans. The contaminated disc was rolled on blood agar and pre-disinfection Colony Forming Units (CFU) were evaluated followed by subjecting the discs to disinfection protocols. The contaminated discs with S. aureus and C.albicans were disinfected using M.indica leaf extracts, A.occidentale leaf extracts and 0.2% CHX for 10 min. Post-disinfection CFUs were evaluated by rolling the disc on blood agar. The results were tabulated and analysed using dependent t-test, one-way ANOVA and Tukeys multiple posthoc procedure. Results: Pair-wise comparison of pre-and post-disinfection log CFU counts of S.aureus gave a statistical significance between 0.2% CHX and and M.indica leaf extract. No statistically significant results were found between 0.2% CHX and A.occidentale. Pair wise comparison of the log CFU from pre-disinfection to post-disinfection of C.albicans gave a statistical significance between all the three groups. Conclusions: In the present study A.occidentale leaf extract and M.indica leaf extract have shown significant reduction in CFU of both the organisms. 0.2% CHX showed the most CFU reduction post disinfection of maxillofacial silicone material surface contaminated S.aureus and C.albicans followed by A.occidentale leaf extracts and M.indica leaf extracts. Given the limitations of the current research, A.occidentale leaf extract and M.indica leaf extract can be used as an alternative for disinfection of maxillofacial silicone prosthesis.

PURPOSE: This study investigated the color stability of different commercially available silicone materials for facial prostheses upon exposure to everyday beverages. It was hypothesized that the beverages would not alter the color of the silicone under conditions simulating daily exposure. METHODS: Sixty specimens were fabricated using two materials - VST-50 and Silfy - and exposed to commercially available cold mineral water, tea, or coffee. Specimen color was evaluated on days 1 and 15. The color was measured with a spectrophotometer based on CIELab system and color differences were calculated as ΔE. Statistical analysis was performed using the Kruskal-Wallis test and Mann-Whitney U test. RESULTS: The mean ΔE values after 15 days of exposure to mineral water, cold green tea, and coffee were 1.016, 3.480, and 3.636 for VST-50 and 0.440, 0.798, and 1.425 for Silfy, respectively. Both materials showed significant differences in color, and VST-50 showed a greater color change than Silfy, especially for coffee. CONCLUSION: Pigmented silicone elastomers have low color stability, leading to an overall color change in silicone prostheses when exposed to pigmented beverages. Color changes in such prostheses can be mitigated by selecting materials with better color stability to extend their longevity.

Introdução: A recente preocupação sobre a segurança dos implantes de silicone tem levado muitas mulheres a buscarem a retirada de seus implantes, mesmo sem aparente complicação nas mamas. Por outro lado, muitos cirurgiões não se sentem confortáveis em realizar o explante por receio de que a paciente não gostará do resultado estético após a cirurgia. A ressonância nuclear magnética (RNM) é um recurso valioso para avaliação diagnóstica das mamas e pode ser usada no planejamento do explante. O objetivo é demonstrar como a análise sistematizada das imagens da ressonância magnética das mamas pode auxiliar no planejamento do explante de silicone. Método: Uma análise detalhada dos cortes axial e sagital da RNM foi feita para avaliar a quantidade de tecido em cada mama. Essas imagens foram apresentadas às pacientes durante a consulta pré-operatória para que elas pudessem perceber, com clareza, o quanto o implante influencia no tamanho de suas mamas. No mesmo momento, foram apresentadas fotos de pós-operatório de pacientes com características semelhantes para que a paciente pudesse analisar, de forma mais objetiva, se ficaria satisfeita ou não com a estética das mamas após o explante. Resultados: As pacientes demonstraram alto grau de compreensão das imagens apresentadas e se mostraram satisfeitas com esta análise detalhada de expectativa de resultado. Conclusão: A comparação das imagens da RNM das mamas e das imagens de resultados de pós-operatório confere maior objetividade ao diálogo pré-operatório, favorecendo a compreensão do resultado esperado e trazendo maior clareza à decisão pelo explante.


Introduction: Recent concerns about the safety of silicone implants have led many women to seek the removal of their implants, even without apparent breast complications. On the other hand, many surgeons do not feel comfortable performing the explant for fear that the patient will not like the aesthetic result after surgery. Magnetic resonance imaging (MRI) is a valuable resource for diagnostic evaluation of the breast and can be used in explant planning. The objective is to demonstrate how the systematic analysis of breast MRI images can assist in planning silicone explantation. Method: A detailed analysis of the axial and sagittal MRI sections was performed to assess the amount of tissue in each breast. These images were presented to patients during the preoperative consultation so that they could clearly understand how much the implant influences the size of their breasts. At the same time, post-operative photos of patients with similar characteristics were presented so that the patient could analyze, more objectively, whether or not she would be satisfied with the aesthetics of her breasts after explantation. Results: The patients demonstrated a high degree of understanding of the images presented and were satisfied with this detailed analysis of expected results. Conclusion: The comparison of breast MRI images and postoperative results images provides greater objectivity to the preoperative dialogue, favoring the understanding of the expected result and bringing greater clarity to the decision for explantation.

Organosilicon polymers (silicones) are of enduring interest both as an established branch of polymer chemistry and as a segment of commercial products. Their unique properties were exploited in a wide range of everyday applications. However, current silicone trends in chemistry and materials engineering are focused on new smart applications, including stretchable electronics, wearable stress sensors, protective coatings, and soft robotics. Such applications require a fresh approach to methods for increasing the durability and mechanical strength of polysiloxanes, including crosslinked systems. The introduction of self-healing options to silicones has been recognized as a promising alternative in this field, but only carefully designed multifunctional systems operating with several different self-healing mechanisms can truly address the demands placed on such valuable materials. In this review, we summarized the progress of research efforts dedicated to the synthesis and applications of self-healing hybrid materials through multi-component systems that enable the design of functional silicon-based polymers for smart applications.

During the fabrication of a complete denture, functional impression is taken. Literature studies show that polydimethylsiloxane (condensation silicone) has not been reported by United States dental schools to perform border molding. Thus, the purpose of this article is to review the functional impression technique when border molding is performed with a laboratory condensation silicone putty.

Background: Parry-Romberg syndrome (PRS) is an enigmatic and acquired degenerative condition. It is characterized by a gradual and progressive facial hemiatrophy, after which facial deformities appear unexpectedly in a stable state. The degree of disfigurement depends on tissue involvement, illness onset, and duration. Nowadays, non-invasive treatments with lower morbidity, such as structured fat grafting (SFG) are preferred over traditional operations such as free flaps and bone grafts. However, for severe cases of PRS, multiple sessions of SFG are required. Case study: Despite the therapeutic failure that a 20-year-old lady had suffered after five previous conventional surgical procedures, the outcome was successful after only one surgical therapy employing a particular combination of SFG and malar silicone elastomers implant (SI). Conclusion: The author highlights the interest in a combination of non-invasive procedures using SFG and malar augmentation with SI to repair large volume loss while improving symmetry and skin texture. Otherwise, the role of adipose-derived stem cells in fat survival is crucial and should be the focus of future studies and the key to technological advancement.

Silicone elastomers play a crucial role in the field of maxillofacial prosthodontics. To maintain optimal hygiene, various disinfectants have been reported to clean silicone prostheses. Nevertheless, when selecting a disinfectant, it is important to consider not only its antimicrobial efficacy, but also its compatibility with the materials, to minimize any potential impact on the physical properties of the material surfaces. The coloring effect of such disinfectants on different types of silicone is of interest. A total of 144 silicone specimens (72 pure silicones, 72 nano-TiO2-incorporated silicones, from A-2000 and A-2006 silicones) were fabricated in this study. The spectrophotometric analysis was carried out, and the initial CIE L*a*b* color values were measured prior to disinfection. Specimens in each silicone group (with or without nano-TiO2) were subjected to a 30-h disinfection period simulating 1 year of disinfection with the following disinfectants: Control (tap water), 0.2% chlorhexidine gluconate, 4% chlorhexidine gluconate, 1% NaOCl, neutral soap, and effervescent. After the second color values were recorded, the color change (∆E*) was calculated. Significant differences were observed among the disinfectants for both the A-2000 and A-2006 silicone groups. Nano-TiO2 did not show a color protection effect in A-2000 silicone. In contrast, nano-TiO2 incorporation provided color protection against CHG 0.2%, CHG 4%, and NaOCl in A-2006 silicone. Most of the disinfectants did not show acceptable color stability over time. In pure A-2000 silicone, except for 0.2% chlorhexidine, all disinfectant groups demonstrated a color change within the acceptability threshold of 50:50% (∆E* = 3.0). On the other hand, in nano-TiO2-incorporated A-2006 silicone, only 0.2% and 4% chlorhexidine demonstrated an acceptable color change. Overall, chlorhexidine could be used as a suitable disinfectant in maxillofacial silicone prostheses.

The development of high-performance thermally conductive interface materials is the key to unlocking the serious bottleneck of modern microelectronic technology through enhanced heat dispersion. Existing methods that utilize silicone composites rely either on loading large doses of randomly distributed thermal conductive fillers or on filling prealigned thermal conductive scaffolds with liquid silicone precursors. Both approaches suffer from several limitations in terms of physical traits and processability. We describe an alternative approach in which malleable silicone matrices, based on the dynamic cyclic disulfide nature cross-linker (α-lipoic acid), are readily prepared using ring-opening polymerization. The mechanical properties of the resultant dynamic silicone matrix are readily tunable. Stress-dependent depolymerization of the disulfide network demonstrates the ability to reprocess the silicone elastomer matrix, which allows for the fabrication of highly efficient thermal conductive composites with a 3D interconnecting, thermally conductive network (3D-graphite/MxBy composites) via in situ methods. Applications of the composites as thermal dispersion interface materials are demonstrated by LEDs and CPUs, suggesting great potential in advanced electronics.

There are often bonding problems between acrylic resins and silicone. PEEK (polyetheretherketone), which is a high-performance polymer, has great potential for the implant, and fixed or removable prosthodontics. The aim of this study was to evaluate the effect of different surface treatments on PEEK to be bonded to maxillofacial silicone elastomers. A total of 48 specimens were fabricated from either PEEK or PMMA (Polymethylmethacrylate) (n = 8). PMMA specimens acted as a positive control group. PEEK specimens were divided into five study groups as surface treatments as control PEEK, silica-coating, plasma etching, grinding, or nano-second fiber laser. Surface topographies were evaluated by scanning electron microscopy (SEM). A platinum-primer was used on top of all specimens including control groups prior to silicone polymerization. The peel bond strength of the specimens to a platinum-type silicone elastomer was tested at a cross-head speed of 5 mm/min. The data were statistically analyzed (α = 0.05). The control PEEK group showed the highest bond strength (p < 0.05) among the groups. No statistical difference was found between control PEEK, grinding, or plasma etching groups (p > 0.05). The lowest bond strength was seen in the laser group, which was not statistically different from silica-coating (p > 0.05), and statistically different from control PEEK, grinding, or plasma groups (p < 0.05). Positive control PMMA specimens had statistically lower bond strength than either control PEEK or plasma etching groups (p < 0.05). All specimens exhibited adhesive failure after a peel test. The study results indicate that PEEK could serve as a potential alternative substructure for implant-retained silicone prostheses.

The loss of a finger in any capacity as a result of trauma has a significant influence on the patient's everyday life, as well as their psychological and physical health. Multiple conventional techniques have been reported in the literature, mostly offering psychological and cosmetic benefits to such individuals. However, there is a paucity of literature for functional finger prosthesis. This case report describes rehabilitation of an amputated index finger using an innovative digital workflow, thereby making it impression-free, cast-free, accurate, less time-consuming, and above all functionally viable. Digital technology was used for designing, and fabrication of this prosthesis was done using three-dimensional (3-D) printing. When compared to traditional prostheses, this 3-D-printed prosthesis was functional, allowing the patient to conduct everyday activities and providing the patient's confidence a psychological boost.

Introducción y objetivo: El linfoma anaplásico de células grandes asociado a implantes mamarios (LACG-AIM) es una enfermedad poco frecuente y parcialmente comprendida que ha despertado gran interés por el impacto social y económico que genera. Poco se conoce sobre la posible aparición de la enfermedad asociada al uso de implantes en otras localizaciones corporales. El objetivo de este artículo es caracterizar y exponer los casos de pacientes descritos en la literatura con linfoma anaplásico de células grandes asociado a implantes de silicona fuera de la mama. Material y método: Revisión de la literatura en bases de datos Pubmed, Cochrane, ScienceDirect y SciELO buscando reportes de linfoma anaplásico de células grandes asociado a implantes (LACG-AI) en sitios diferentes a la mama. Resultados: Encontramos dos reportes de caso de LACG en glúteos en pacientes con previa gluteoplastia de aumento con implantes realizada con fines estéticos. Llamó la atención el hallazgo de un número mayor de otro tipo de linfomas asociados a la colocación de diferentes dispositivos de uso médico y a la aplicación de biopolimeros glúteos en un caso adicional. Conclusiones: Es posible la existencia de una nueva enfermedad asociada a implantes de silicona en otros sitios corporales. La literatura es escasa y no permite afirmar aun su existencia. Debe tenerse un alto índice de sospecha ante cualquier caso sugerente de la entidad para su estudio, clasificación adecuada y manejo oportuno. Proponemos un algoritmo diagnóstico de utilidad en pacientes portadores de implantes glúteos que presenten alguna sintomatología asociada. (AU)


Background and objective: Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) is a rare and partially understood disease that has aroused great interest due to the social and economic impact it generates. Little is known about the possible occurrence of the disease associated with the use of implants in other body locations. The objective of this article is to characterize and present the cases of anaplastic large cell lymphoma associated with implants located outside the breast described in the literature. Methods: A review of the literature was performed in PubMed, Cochrane, ScienceDirect and SciELO databases looking for reports of implant-associated anaplastic large cell lymphoma (IA-ALCL) in sites other than the breast. Results: Two case reports of IA-ALCL in the buttocks were found in patients with previous augmentation gluteoplasty with implants performed for aesthetic purposes. Additionally, the finding of a greater number of other types of lymphomas associated with the placement of different devices for medical use and the application of gluteal biopolymers in an additional case. Conclusions: The existence of a new disease associated with silicone implants in other body sites is possible. The literature is scant and does not allow us to confirm its existence. There must be a high index of suspicion in any case suggestive of the entity for its study, proper classification and timely management. We propose a diagnostic algorithm that can be very useful in patients with gluteal implants who present some associated symptoms. (AU)

ABSTRACT Objective: To evaluate the dimensional change of edentulous models made of high density laboratory silicones at different storagetimes. Methods: A metallic master model simulating an edentulous maxilla was used. The model had projections of 1.8 cm located in the regions of canines (A and B) and molars (C and D) allowing the linear measurement of distances AB, BC, CD, DA, AC and BD. A laboratory silicone mold was made for duplication of master model in the following materials (n = 10): G1: Zetalabor 85, G2: Titanium 95, G3: Platinum 95 and G4: Gypsum type III (control). The samples were photographed and measured using a software (Image J) at times T1 (baseline) and T2 (after one week of models fabrication). The data were statistically analyzed using Kruskal- Wallis and Dun non-parametric tests (α = 0.05). Results: The storage time influenced the behavior of the materials evaluated. All groups presented dimensional changes in both times and the group Tit95 presented the best results. Only Zet85 group showed a different behavior related to the region of the model. Conclusions: The high density laboratory silicones showed satisfactory results when compared to the gypsum group. Tit95 was the most stable material and it can be recommended as an edentulous model for total protheses.


RESUMO Objetivo: Avaliar a alteração dimensional de modelos edêntulos fabricados em silicone laboratorial de alta densidade em diferentes tempos de armazenagem. Métodos: Um modelo mestre metálico simulando uma maxila edêntula foi utilizado. O modelo possuía projeções oclusais de 1,8 cm localizadas nas regiões de caninos (A e B) e molares (C e D), permitindo a mensuração das distâncias lineares AB, BC, CD, DA, AC e BD. Um molde foi fabricado em silicone laboratorial para duplicação do modelo mestre com os seguintes materiais (n = 10): G1: Zetalabor 85, G2: Titanium 95, G3: Platinum 95 e G4: Gesso tipo III (controle). As amostras foram fotografadas e mensuradas com auxílio de um software (Image J) nos tempos T1 (baseline) e T2 (após uma semana de confecção). Os dados obtidos foram avaliados estatisticamente utilizando os testes não paramétricos de Kruskal- Wallis e Dunn (α = 0.05). Resultados: O tempo de armazenagem influenciou o comportamento dos materiais avaliados. Todos os grupos apresentaram alterações dimensionais em ambos os temos de avaliação e o G2 apresentou os melhores resultados. Apenas o grupo G1 mostrou alteração dimensional dependente da região mensurada. Conclusão: Silicones laboratoriais de alta densidade mostraram resultados satisfatórias de alteração dimensional quando comparados ao gesso tipo III. O material Titanium 95foi o mais estável e pode ser recomendado como alternativa para a confecção de modelos edêntulos.

Introdução: A descrição do linfoma anaplásico de células T e o recente aumento das cirurgias de explante resultou na elevação do número de exames anatomopatológicos nas cirurgias de retirada de implantes mamários de silicone. O objetivo desta pesquisa é analisar a qualidade e quantidade de dados contidos na requisição do exame histopatológico. Métodos: Foram estudados 251 casos. Os seguintes dados foram analisados: sexo, idade, localização anatômica e espacial, lateralidade, história clínica, sinais e sintomas, quimioterapia e radioterapia prévia, hipótese diagnóstica, cirurgias prévias, tipo e marca do implante, exames de imagem prévios e número e características dos espécimes enviados. Resultados: A idade média foi de 43 anos. A lateralidade não foi mencionada em 16 (0,84%). A localização anatômica foi citada em 15 casos. O tipo de cirurgia foi mencionado por 40 (15,94%). O número de contêineres variou de 1 a 5, com mediana de 2. A cápsula foi enviada em 242 casos, em 161 de forma isolada, tecido mamário em conjunto com cápsula em 27, tecido mamário e cápsula em contêineres diferentes em 54 casos. A história clínica foi incluída em 19,12%, sinais e sintomas em 13,94%, em que a contratura foi o único termo inserido em 64. Em 27 requisições foi citado linfoma. Em 15 pacientes a presença de seroma foi referida e destes nove foram enviados. O tipo e marca do implante não foi citado. Conclusão: Os dados são escassos. Recomenda-se a criação de protocolos na retirada da cápsula e tecido adjacente, incluindo a orientação anatômica e espacial.


Introduction: The description of the Anaplastic Large Cell Lymphoma and the explantation surgery resulted in an increase of histopathological exams in breast implant removing surgery. Methods: 251 pathology requests were studied. The following data from the medical requests were analyzed: gender, age, type of surgery, number of specimens containers sent, laterality, anatomical and spatial location, clinical history, signs and symptoms, previous radiotherapy, previous chemotherapy, diagnostic hypothesis, previous surgeries, and reference to previous breast exams. Results: The mean age was 43 years old. Laterality was not mentioned in 16 requests. The surgery performed was mentioned in 15.94% requests. The number of containers varies from 1 to 5, with a median of 2. The containers include capsules in 242 cases, 161 as isolated capsule, 27 mammary tissue, and capsule in the same specimen, 54 mammary tissues sent in a separate container, anatomical and spatial location was mentioned in 6.33% cases. The detailed clinical data was included in 19.12%, signs and symptoms 13.94%, contracture as the only item mention in 64 of them. In 27 requests, lymphoma evaluation was requested. 15 included seroma and from nine of those, liquid was sent with a request for immunohistochemical and cytology analysis. None of the requests had any data on implant type or brand. Conclusion: The amount of information contained in the medical request forms is minimal. The authors recommend the need for a protocol to standardize the surgical removal of the capsule and the adjacent mammary tissue. Surgical specimens should be spatially oriented.

A síndrome autoimune induzida por adjuvantes (ASIA) e seus critérios diagnósticos foram descritos por Shoenfeld em 2011, relacionando sintomas de autoimunidade a adjuvantes, como o silicone, presente em próteses mamárias. Essa revisão sistemática objetivou reunir dados da literatura sobre a sintomatologia, a incidência e os tratamentos propostos para ASIA causada por implantes mamários de silicone (IMS). Foram realizadas pesquisas nas bases de dados PubMed, LILACS, Embase e Cochrane, utilizando os descritores "Autoimmune Syndrome Induced by Adjuvants", "Breast implant" e "Silicone Implant Incompatibility Syndrome". A estratégia de busca gerou 95 artigos, dos quais 20 foram incluídos na revisão. São as três as principais teorias sugeridas pelos autores para explicar o desenvolvimento da síndrome: predisposição genética, silicone bleeding e a formação de uma cápsula periprótese. As manifestações clínicas mais frequentemente descritas incluem fadiga crônica, artralgia, mialgia, distúrbios cognitivos e do sono. Não há consenso sobre os achados laboratoriais e os fatores de risco associados, além disso, estudos recentes propõem a ampliação dos critérios diagnósticos inicialmente descritos. O tratamento adequado permanece controverso, envolvendo desde o uso de medicações até o explante da prótese. Apesar dos artigos revisados sugerirem a existência da ASIA relacionada aos IMS, sua fisiopatologia precisa é desconhecida, os sintomas relatados são inespecíficos e o tempo entre a exposição e o surgimento das manifestações é incerto. Por meio dessa revisão sistemática, conclui-se que, até o presente momento, não existem evidências científicas suficientes para estabelecer a causalidade do desenvolvimento da síndrome autoimune induzida por adjuvantes decorrente de implantes mamários de silicone.


Adjuvant-induced autoimmune syndrome (ASIA) and its diagnostic criteria were described by Shoenfeld in 2011, relating symptoms of autoimmunity to adjuvants, such as silicone, present in breast implants. This systematic review aimed to gather data from the literature on symptomatology, incidence and proposed treatments for ASIA caused by silicone breast implants (SBI). Searches were carried out in PubMed, LILACS, Embase and Cochrane databases, using the descriptors "Autoimmune Syndrome Induced by Adjuvants," "Breast implant," and "Silicone Implant Incompatibility Syndrome." The search strategy generated 95 articles, of which 20 were included in the review. The authors suggest three main theories to explain the development of the syndrome: genetic predisposition, silicone bleeding and the formation of a periprosthetic capsule. The most frequently described clinical manifestations include chronic fatigue, arthralgia, myalgia, and cognitive and sleep disorders. There is no consensus on laboratory findings and associated risk factors; recent studies propose expanding the diagnostic criteria initially described. Adequate treatment remains controversial, ranging from medications to prosthesis explantation. Although the reviewed articles suggest the existence of ASIA related to SBI, its precise pathophysiology is unknown, the symptoms reported are nonspecific, and the time between exposure and the onset of manifestations is uncertain. This systematic review concludes that, to date, there is not enough scientific evidence to establish the causality of the development of adjuvant-induced autoimmune syndrome resulting from silicone breast implants.

Aim: This study aimed to evaluate the effect of frozen storage on the physical properties of a silicone-based test food material, highly used to evaluate the masticatory performance in research settings. Methods: A total of 1,666 silicone cubes of Optosil Comfort® with 5.6-mm edges were shaped and stored at -18°C. The cubes were subsequently tested for flexural strength (maximum force, displacement, stress, and strain) before breaking (n = 136), changes in weight and size (n = 170), and masticatory performance (n = 1360) at eight timepoints: immediately after cube preparation (baseline, no freezing), and 1, 2, 3 and 4 weeks, and 2, 4 and 6 months after frozen storage. The cubes were thawed 8 h before each assessment. Results: The maximum force, stress, maximum displacement, and deformation values for the cubes were not affected by freezing (P > 0.05). At all of the time points, the cubes exhibited similar weight (P = 0.366) and size (identical values). The masticatory performance for the cubes also showed no differences from baseline through 6 months (P = 0.061). Conclusion: Freezing Optosil Comfort® silicone cubes did not alter the physical and mechanical properties of the material, being suitable to optimize the assessment of masticatory parameters for research purposes

General information concerning different kinds of chemical additives used in the textile industry has been described in this paper. The properties and applications of organofunctional silanes and polysiloxanes (silicones) for chemical and physical modifications of textile materials have been reviewed, with a focus on silicone softeners, silane, and silicones-based superhydrophobic finishes and coatings on textiles composed of silicone elastomers and rubbers. The properties of textile materials modified with silanes and silicones and their practical and potential applications, mainly in the textile industry, have been discussed.

BACKGROUND: This study aimed to compare the dimensional accuracy, hydrophilicity and detail reproduction of the hybrid vinylsiloxnether with polyether and polyvinylsiloxane parent elastomers using modified digital techniques and software. This was done in an attempt to aid in solving the conflict between the different studies published by competitive manufacturers using different common manual approaches. METHODS: A polyether, polyvinylsiloxanes and vinyl polyether silicone hybrid elastomeric impression materials were used in the study. Dimensional accuracy was evaluated through taking impressions of a metallic mold with four posts representing a partially edentulous maxillary arch, that were then poured with stone. Accuracy was calculated from the mean of measurements taken between fixed points on the casts using digital single-lens reflex camera to produce high-resolution digital pictures for all the casts with magnification up to 35×. Hydrophilicity was assessed by contact angle measurements using AutoCAD software. The detail reproduction was measured under dry conditions according to ANSI/ADA Standard No. 19 and under wet conditions as per ISO 4823. A metallic mold was used with three V shaped grooves of 20, 50, and 75 µm width. Specimens were prepared and examination was made immediately after setting using digital images at a magnification of 16×. RESULTS: The hybrid impression (0.035 mm) material showed significantly higher dimensional accuracy compared to the polyether (0.051 mm) but was not as accurate as the polyvinyl siloxane impression material (0.024 mm). The contact angles of the hybrid material before and after setting was significantly lower than the parent materials. With regard to the detail reproduction, the three tested materials were able precisely to reproduce the three grooves of the mold under dry conditions. Whereas, under wet conditions, the hybrid material showed higher prevalence of well-defined reproduction of details same as polyether but higher than polyvinylsiloxane that showed prevalence of details with loss of sharpness and continuity. CONCLUSIONS: The digital technique used could be a more reliable and an easier method for assessment of impression materials properties. The hybridization of polyvinyl siloxane and polyether yielded a promising material that combines the good merits of both materials and overcomes some of their drawbacks.

Introdução: Após a colocação de implantes mamários de silicone, algumas pacientes apresentam sintomas descritos como doença do implante mamário e buscam a cirurgia de explante. O objetivo deste estudo é analisar o histórico de sintomas e verificar as impressões dos pacientes submetidos ao explante mamário em três momentos distintos: antes de colocar os implantes mamários, enquanto estavam com os implantes e após a cirurgia de explante. Métodos: Essa pesquisa foi delineada como um estudo observacional longitudinal multicêntrico utilizando um questionário on-line de participação voluntária enviado por e-mail. Resultados: Foram analisados 156 pacientes, 84% apresentavam três ou mais sintomas e 66,1% destes obtiveram melhora de sua sintomatologia após o explante (p<0,001). Antes da colocação de silicone, a mediana de autossatisfação corporal era de 7, enquanto estavam com os implantes a mediana tornou-se 9 e após a cirurgia de explante a mediana se manteve em 9 (p<0,001). Grupos de apoio em redes sociais auxiliaram na decisão de fazer o explante em 87,2% das pacientes. Conclusão: Pacientes que têm sintomas após colocarem silicone apresentam melhora com a retirada dos implantes mamários. A autossatisfação corporal aumenta com a colocação de implantes mamários e permanece elevada após a retirada destes. Pacientes que fazem a cirurgia do explante costumam estar arrependidas de terem colocado silicone, muito satisfeitas com a decisão de removêlos e igualmente satisfeitas com o resultado da cirurgia de explante mamário. Grupos de apoio em redes sociais foram importantes na tomada de decisão destas pacientes.


Introduction: Following silicone breast implant placement, some patients present symptoms described as breast implant illness and seek explant surgery. This study aims to analyze the historical symptoms and ascertain breast explant patients' impressions at three different times: before breast implant placement while having the implants, and after the explant surgery. Methods: This survey was designed as a multicenter longitudinal observational study using an online voluntary participation questionnaire sent by e-mail. Results: 156 patients were analyzed, 84% had three or more symptoms, and 66.1% improved their symptoms after the explant (p<0.001). Before the placement of silicone, the median self-body satisfaction was 7, while with the implants, the median became 9, and after the explant surgery, the median remained up to 9 (p<0.001). Support groups on social networks helped in the decision to explant in 87.2% of the patients. Conclusion: Patients presenting symptoms after silicone placement show improvement with breast implant removal. Body self-satisfaction increases with the placement of breast implants and remains increased after their removal. Patients who undergo the explant surgery usually regret having implanted silicone; they are very satisfied with the decision to remove them and equally satisfied with the result of the breast explant surgery. Support groups on social networks were important in the decision-making of these patients.
