RESUMO

There is a lack of reports onr stent fracture with pseudoaneurysm formation in the femoropopliteal artery, which can cause restenosis or occlusion of the treated arterial segment. We present a case of a large pseudoaneurysm of the popliteal artery that was observed 18 months after popliteal stenting using a self-expandable nitinol stent. We describe an endovascular approach to overcome this severe complication. Stent fractures are an often overlooked complication of femoropopliteal stenting and can be associated with serious diseases. The popliteal artery was successfully treated using self-expandable Viabahn endoprosthesis.

Fractures in stents are usually detected by visual analysis, which may be affected by the presence of noise and image deformations. The lack of research into automating stent fracture detection has motivated this work, in which techniques are developed to facilitate diagnosis by observation (Image Delineation Algorithm) and, when possible, to point out areas of possible fractures (Fracture Detection Algorithm). The use of classical elements and the development of additional computational techniques contributed to the process of image analysis, providing a possible way to aid medical diagnosis. The developed algorithms are applied to image samples from femoropopliteal arteries, and the results are compared to those of medical diagnosis. As a result, aside from the improvement of image display, a kappa concordance index of 0.878 for the detection of fractures confirms the method as satisfactory, with very good agreement with medical diagnosis.

Os stents farmacológicos representam um importante avanço na terapêutica da doença aterosclerótica coronariana. Recentemente tem sido descrita a fratura de stents farmacológicos, associada à reestenose e trombose. Relataremos aqui um caso de fratura de um stent farmacológico 18 meses após o seu implante, associada com reestenose.


Drug-eluting stents represent a significant evolution in the therapy of coronary artery disease. Recently, restenosis and thrombosis related to drug-eluting stent fractures have been described. This work reports a case of fracture of a drug-eluting stent 18 months after implantation, associated with restenosis.
