RESUMO

At the actual stage of becoming of Russian stomatology, the problem of sustainable development of rural stomatological care, as system of local formations of complex medical social nature belongs to national priorities and is considered as one of most important directions of public social policy. The stomatological health of rural population is to be considered as indicator of national stomatological health. This is confirmed by spatial structure: rural territories (inhabited territories outside cities) cover 2/3 of territory of the Russian Federation with population of 37.3 million or 1/4 of total population. The spatial structure of the Belgorod Oblast is reliably similar to the all-Russian one. The number of national and foreign studies proved that accessibility, quality and timeliness of insurance (state) stomatology is lower for rural residents that can be considered as one of signs of social inequality. Depending on the social economic status of region, signs of social inequality in the stomatology field are conditioned by broad spectrum of factors. Some of them are discussed in the article.

RESUMO

The article considers the problem of access to stomatological care, in particular to prosthetic dentistry, for people with limited health capacities in conditions of the COVID-19 pandemic. The range of factors impeding to get quality dental services in state stomatological polyclinics and at home was established by empirical study organized in two Subjects of the Russian Federation located in the Central Federal Okrug.

RESUMEN Como consecuencia del alargamiento de la vida, aumenta también el número de adultos con discapacidades y enfermedades crónicas degenerativas, que pueden causarlas. El objetivo de esta revisión es conducir un análisis de las evidencias disponibles sobre la importancia de la atención estomatológica al paciente geriátrico con dismovilidad. Se tuvo en cuenta las características del adulto mayor dependiente, la atención domiciliaria como actividad asistencial y la atención odontológica domiciliaria que arrojó como conclusiones que para garantizar una buena salud bucodental de los adultos mayores con dismovilidad, se requiere de una correcta valoración geriátrica y un trabajo en equipo donde los estomatólogos juegan un papel importante ya que la atención estomatológica demanda de una asistencia odontológica domiciliaria provista de los elementos necesarios para su funcionamiento.


ABSTRACT As a result of the lengthening of life, the number of adults with disabilities and chronic degenerative diseases, which can cause them, also increases. The objective of this review is to conduct an analysis of available evidence on the importance of stomatological care to the geriatric patient with dysmobility. The characteristics of the dependent elderly were taken into account, the home care as a health care activity and the home dental care that was given as conclusions to ensure good oral health in the elderly with dismobility, proper geriatric assessment and teamwork are required where stomatologists play an important role as stomatological care requires home dental care provided with the necessary elements for its operation.

The issue of preventing various diseases in children with disabilities is relevant around the world. The purpose of the study: to investigate the oral health of disabled children with hearing impairments and with visual impairments and to determine problematic issues of their dental care support. The individual face-to-face questionnaire survey was carried out covering parents (legal representatives) of students of special general education school for children with hearing and visual impairments. The analysis of results of preventive examination of students was implemented by stomatologist. Two categories of students were covered by the study: children with hearing impairment (first group) and children with visual impairment (second group), their families included. The first group consisted of 61 families with disabled children (37 boys (60.7%) and 24 girls (39.3%). The second group included 63 families with disabled children (35 boys (55.6%) and 28 girls (44.4%). The comparable structure of compared groups by age was established. The children aged 10-14 years dominated in both groups: 39.3% in the first one and 57.1% in the second one. The comparison of dental status of disabled children with various disorders of the senses was implemented according number of stomatological indices: caries activity, hygiene level, need for consultation by orthodontist. The patients with poor hygiene were significantly more common among children with visual impairment (38.1%) as compared with children with hearing impairment (3.3%). Also. 77.0% of patients with hearing impairments and 79.4% of patients with visual impairments needed advises of orthodontist. The families of 28.8% and 38.1% of patients in corresponding compared groups resorted to fee-for-service dental services. The first group characterized by statistically significantly higher percentage of dissatisfaction with quality of treatment and prevention (59.0% vs. 38.1%). The second most important problem was dissatisfaction with attitude of stomatologist to patients and their parents, which was noted in 16.4% and 28.6% of cases correspondingly. The stomatological health problems are very relevant for disabled children as they often suffer from oral diseases and have difficulties in accessing quality dental care. The prevalence of caries, especially sub- and de-compensated, in disabled children with visual and hearing impairments is significantly higher than in children population as a whole.

RESUMO

The epidemiological stomatological examination is the basis of assessing stomatological status of population and its needs for dental care. The first national epidemiological survey in the Russian Federation was carried out in 1996-1998. The second survey was conducted nine years later and covered 47 subjects of the Russian Federation. At that, the Kirov Oblast was not included in both national epidemiological surveys. The purpose of the study was to assess the state of stomatological health of residents of certain districts of Kirov Oblast. The assessment was based on epidemiological stomatological examination of adult population. It was established that the retired, agricultural workers and residents of rural settlements require increased epidemiological attention. The adult population from the mentioned groups in the Kirov Oblast is provided with ineffective stomatological care. The strategy of organization of stomatological care in the Kirov Oblast is be targeted to development of complex medical social programs of timely detection and treatment of caries, increasing of attention of medical specialists to prevention and treatment of periodontal diseases in population of problematic groups.

OBJECTIVE: The aim: To study and evaluate the condition of the dental health of children of school age and the characteristics of their food. PATIENTS AND METHODS: Materials and methods: Statistics on the dental health of children in Transcarpathian region for the years 2009-2018 were analyzed with the application of statistical, sociological and clinical methods and the data on dental health and way of nutrition of 163 children in Uzhgorod were determined and analyzed. RESULTS: Results and conclusions: Reduction of 28.9% of coverage of children with preventive dental examinations was determined with the need for mouth cavity sanation of 45.93%. Redevelopment was held for 91.7% of those who needed help. From 10% to 50% of the examined children in different age groups have caries. Up to 10% of children between 11 and 15 years old have teeth removed. It was determined that nutrition of children with caries diagnosed is not rational.

Doynikov is the founder of the Chair of Propaedeutics of Orthopedic Stomatology of the Moscow Medical Stomatological Institute, a well-known stomatologist, inventor, one of the prominent national organizers of stomatological care. The article presents dynamics of volumes and costs of primary stomatological care of population considering structure of stomatological morbidity in 2012-2016. The establishment of trends of development of stomatological morbidity, dynamics of population appealability for primary stomatological care is needed for supporting accessibility of medical care and rational distribution of resources including financial ones within the framework of territorial program of mandatory medical insurance.

Se realizó un estudio descriptivo y transversal durante 2013 para evaluar el grado de satisfacción de 5 proveedores de salud bucal (2 especialistas en estomatología general integral y 3 licenciados en atención estomatológica) con la atención que brindan y de 195 usuarios (148 escolares y 47 preescolares) con el servicio recibido, pertenecientes al Policlínico Universitario "Josué País García" de Santiago de Cuba, mediante criterios, indicadores y estándares. En los primeros, solo uno de los 13 criterios resultó adecuado; en los segundos, el total de la muestra manifestó estar satisfecho con la atención recibida.


A descriptive and cross sectional study was carried out during 2013 to evaluate the satisfaction degree of 5 oral health suppliers (2 specialists in comprehensive general stomatology and 3 graduates in stomatological care) with the care they provide and from 195 users (148 scholars and 47 preschool) with the received service, belonging to "Josué País García" University Polyclinic in Santiago de Cuba, by means of criteria, indicators and standards. In the first ones, just one of the 13 criteria was appropriate; in the second ones, the total of the sample manifested to be satisfied with the received care.
