Introduction: Vascular ulcers constitute a serious global public health problem, responsible for causing a significant social and economic impact due to their recurrent, disabling nature and the need for prolonged therapies to cure them. Objective: To evaluate the use and efficacy of the rhEGF in the epithelialization of patients with a diagnosis of CEAP stage 6 venous insufficiency, in the two regimes of the health system in Colombia, the contributive (equivalent to a health system where citizens with payment capacity contribute a percentage of their salary) and the subsidized (equivalent to a health system where the state covers the vulnerable population and low socioeconomic level) versus the other treatments used. Methodology: Observational, descriptive, retrospective, multicenter study, in which 105 medical records with 139 ulcers were reviewed, in 2 centers, one belonging to the subsidized system and the other to the contributive system in Colombia. Results: The association with the epithelialization variable of the different treatment groups for ulcers according to the application of the mixed effect model test, for both regimes was for the Biologicals (EC 34.401/p = 0.000), Bioactive Agents (Hydrogels) (EC 24.735/p = 0.005) groups; for the rest of the treatment groups, the results were neither associated nor statistically significant. Conclusion: Intra- and perilesional therapy with rhEGF expands the therapeutic spectrum in patients with venous ulcers, regardless of the type of health system in which it will be applied, shortening the healing time and reaching a possible therapeutic goal, which according to this study there is an association with epithelialization regardless of the regime applied.

RESUMO

En este trabajo de investigación se realiza un análisis de la eficiencia técnica de las entidades que hacen parte del régimen subsidiado en Colombia durante el año 2010. Se utilizó la metodología no paramétrica Data Envelopment Analysis (DEA), específicamente el modelo CCR-O enfocado a salidas para evaluar la eficiencia del sector; como fuente de información utilizada se tomó la Superintendencia de Salud. De 26 entidades estudiadas, 15 presentaron eficiencia baja, 2 presentaron una eficiencia intermedia, 3 presentaron una eficiencia superior y 6 entidades mostraron ser eficientes, estas fueron: Capresoca EPS, Caja de previsión social de comunicaciones Caprecom, Caja de compensación familiar del Chocó Comfachocó, Caja de compensación familiar de Nariño, Caja de compensación familiar de Córdoba Comfaco y Caja de compensación familiar de Huila Comfamiliar Huila. Con el trabajo de investigación se pudo concluir que el 23,08% del total de DMU's evaluadas presentaron una eficiencia óptima. La eficiencia media de la población estudiada fue de 65,85%


In this research, an analysis of the technical efficiency of the entities that are part of the subsidized regime in Colombia during 2010 was made. The nonparametric Data Envelopment Analysis (DEA) approach was used, specifically the CCR-O model, which focuses on outputs to evaluate the efficiency of the sector. The Superintendence of Health was taken as a source of used information. From the 26 organizations studied, 15 presented low efficiency, 2 presented medium efficiency, 3 presented high efficiency, and 6 entities proved to be fully efficient. These entities were: Capresoca EPS, fund of welfare communications Caprecom, benefit society in Choco Comfachoco, benefit society in Nariño, benefit society in Cordoba Comfaco and benefit society in Huila Comfamiliar Huila. It could be concluded that 23.08% of DMU's evaluated showed optimum efficiency. The average efficiency of the study population was 65.85%.

RESUMO

Se describieron los procesos vividos por 13 de las Empresas Solidarias de Salud ( E.S.S.) de Antioquia durante el período de 1.993 a 1996, con énfasis en: Identificación de la población objeto, fases de conformación (Gestión Sanitaria y Gestión Empresarial), determinación de la demanda, estructura administrativa y organizacional, servicios de salud y situación económica. Todas cumplieron el diseño del Ministerio de Salud para conformarse y vienen prestando servicios a sus beneficiarios. Han mostrado importantes logros en Participación Comunitaria y en la estructuración del Régimen Subsidiado en Salud. Presentan limitaciones administrativas y organizacionales, carencias en sistemas de información y desactualización en sus estados contables. Las E.S.S. son altenativa en Participación Comunitaria y autogestión de los municipios de Colombia en la Administración del Subsidio a la Demanda en Salud para la población más pobre y vulnerable, con particular énfasis en promoción y prevención buscando una nueva cultura de la salud...


The processes lived by 13 of the Solidarity Health Companies (E:S:S:) of Antioquia During the 1993-1996 period were described with emphasis on: identification of the population object, conformation phases (Sanitary Effort, Entrepereunial Effort), determination of demand, administrative and orgazational structure, health services and economic situation. All companies fulfilled the Health Ministry design to be certified and come lending services to their beneficiary. Also have shown important achlevements in Community Participation and in the setup of the Regime Subsididized in Health. These companies present administrative and organizational limitations and lack of information systems. Their countabble states are outdated. The E:S:S companies are alternative in Community Participation and auto effort of the municipallities of Colombia. This is a applied by the Subsidy Administrastion of Health Demand for the poorest and vulnerable population. They have a particular emphasis in promotion and prevention to seek a new culture of health.
