RESUMO

gossypiboma is used to describe a retained surgical swab in the body after an operation. It remains an unwanted complication of surgical practice that increase morbidity and mortality of the patient and profound medico legal problems. Intra-abdominal gossypiboma can migrate in to the ileum, stomach, colon or bladder without any apparent opening in the wall of these luminal organs. Vigilant sponge counting during procedures and thorough exploration prior to closure of the abdomen, are essential practices to avoid such occurrences. Herein we present a case of gossypiboma in a 26-year old woman that was in the lumen of small bowel and caused acute intestinal obstruction.

RESUMO

A 10-year-old spayed female Japanese Shiba Inu had an intraperitoneal mass that was excised surgically. The central area of the mass was composed of osteoblast-like neoplastic cells, osteoid, macrophages, and numerous fibers. The neoplastic cells showed nuclear atypia and many mitotic figures. Therefore, the central area of the mass was diagnosed as an extraskeletal osteosarcoma. The peripheral area of the mass was granuloma tissue with numerous fibers. The neoplastic and granuloma area included two types of fibers, which were identified as rayon and polyester by their morphological and staining characteristics. These fibers were consistent with those of commercial surgical swab, suggesting that the fibers may have been derived from retained surgical swabs at the time of ovariohysterectomy. Therefore, this lesion was considered an extraskeletal osteosarcoma associated with a retained surgical swab.

INTRODUCTION: Ultra-thin (diameter: 3 mm) surgical swabs have not been in practical use as it is difficult to manufacture these using the pre-existing methods; therefore, a new technology has been adopted. This study aimed to evaluate the safety and efficiency of ultra-thin surgical swabs in bench-top and preclinical settings. MATERIAL AND METHODS: We performed liquid particle counter (LPC) test, cotton strength test, shaft bending comparison and surgical vision test for checking the durability of swabs as bench-top investigation, and laparoscopic surgery in a porcine model as preclinical investigation. All tests were compared with conventional 5-mm swabs. RESULTS: The ultra-thin swabs had fewer eluted particles in the LPC test, endured a 5 kg pulling force in the cotton strength test, their shaft did not break at 100 mm bending in the shaft bend comparison test, and interfered less in the surgical vision test. They were used for manipulating organs with no damage in preclinical investigation. CONCLUSIONS: Three millimeters ultra-thin surgical swabs manufactured with the new manufacturing technology are effective and safe.

RESUMO

Gossypiboma is a rare condition caused by retained postoperative foreign bodies. The condition is under reported because of diagnostic difficulties and medicolegal implications associated with it. It may mimic a benign or malignant soft-tissue tumour in the abdomen and pelvis. A 22-year-old woman presented with non specific symptoms and was referred to us with radiological diagnosis of dermoid cyst. On laparotomy the mass was proved to be a gossypiboma resulted from gauze which was retained in caesarean section done two years back. Although gossypiboma is rarely seen in daily clinical practice, it should be considered in the differential diagnosis of postoperative patients presenting with non specific complains and lump abdomen. Despite thorough history, physical examination, laboratory, and radiographical findings, usually gossypibomas are not suspected and remain an accidental finding. Employment of all preventive measures during surgical procedures and high index of suspicion in post operative patients are the key stones in its management.

RESUMO

Os autores relatam três casos de corpo estranho intratorácico, que é definido como uma massa de matriz de algodão, e geralmente se refere a compressa cirúrgica retida, uma rara complicação das cirurgias torácicas. Os pacientes foram examinados por radiografia de tórax e tomografia computadorizada,sendo os achados de imagem confirmados por toracotomia e estudo anatomopatológico. Os principais achados de imagem são de massas intratorácicas em pacientes com história de cirurgia prévia apresentando sintomas relacionados ao trato respiratório inferior em tempo variávelapós a intervenção cirúrgica. Foram analisados os principais achados de imagem dos três casos de corpo estranho intratorácico e correlacionados com revisão da literatura.


The authors report three cases of intrathoracic foreign body that is defined as a cotton matrix mass, mostly retained surgical sponge, a rare complication of a thoracic surgery. The patients wereevaluated by chest radiography and computed tomography with the imaging findings confirmed after thoracotomy and anatomopathological study. The mainly imaging findings consisted of intrathoracic masses in patients with previous thoracic surgery that return to hospital with lower respiratory tract symptoms in different period after surgery procedure. The three cases were related with a brief review of the literature.
