With the aim of expanding the potential application scope of mulberries, eleven pH-switchable deep eutectic solvents were screened for the ultrasonic-assisted extraction of mulberry polysaccharides, and a salt/salt aqueous two-phase system was constructed for the efficient separation of mulberry polysaccharides by regulating the system pH. DES-9 (tetraethylammonium chloride: octanoic acid molar ratio = 1: 2) with a critical response pH value of approximately 6.1 was concluded to be the best extraction solvent for extracting mulberry polysaccharides. A maximum polysaccharide extraction yield of 270.71 mg/g was obtained under the optimal conditions. The maximum polysaccharide extraction efficiency was 78.09 % for the pH-driven tetraethylammonium chloride/K2HPO4 aqueous two-phase system. An acidic ß-pyran mulberry polysaccharide with a low-molecular weight of 9.26 kDa and a confirmed monosaccharide composition were obtained. This efficient and environmentally friendly polysaccharide separation method offers a new approach for the efficient extraction and utilization of other plant polysaccharides.

Enquanto no Norte Global se discute uma crise na Atenção Primária à Saúde, a maioria dos países nunca chegou a constituir sistemas de saúde baseados propriamente numa atenção primária robusta. Nesse cenário, o Brasil apresenta uma tendência mais favorável, com conquistas importantes para a atenção primária e a medicina de família e comunidade nos últimos dez anos. Restam desafios a serem superados para que o Sistema Único de Saúde alcance níveis satisfatórios de acesso a seus serviços, com profissionais adequadamente formados e valorizados pela população.


While the Global North is discussing a crisis in primary health care, the majority of countries have never managed to establish health systems based on robust primary care. Brazil presents a more favorable trend, with important achievements for primary care and family practice over the last ten years. There are still challenges to be overcome so that the Unified Health System achieves satisfactory levels of access to its services, with professionals who are properly trained and valued by the public.


Mientras que en el Norte Global se habla de una crisis de la atención primaria, la mayoría de los países nunca han creado realmente sistemas sanitarios basados en una atención primaria robusta. Brasil, muestra una tendencia más favorable, con importantes logros para la atención primaria y la medicina familiar y comunitaria en los últimos diez años. Aún quedan retos por superar para que el Sistema Único de Salud alcance niveles satisfactorios de acceso a sus servicios, con profesionales debidamente formados y valorados por la población.

OBJECTIVES: Given the success of galanthamine in treating Alzheimer's disease, this study aims to establish an effective method to find drugs from Amaryllidaceae alkaloids and to clarify its mechanism in treating Alzheimer's disease. METHODS: The pharmacodynamic basis and mechanism of action between Amaryllidaceae alkaloids and Alzheimer's disease were explored by constructing a compound-target-disease network, targets protein-protein interaction, gene ontology, Kyoto Encyclopedia of Genes and Genomes pathway enrichment, and molecular docking verification. KEY FINDINGS: In total, a chemical library of 357 potential alkaloids was constructed. A total of 100 active alkaloid components were identified. Thirty-nine associated targets were yielded based on network construction, and the key targets were defined as HSP90AA1, ESR1, NOS3, PTGS2, and PPARG using protein-protein interaction network. Gene ontology items (490) and 68 Kyoto Encyclopedia of Genes and Genomes pathways were selected through the enrichment of target functions, including neuroactive ligand-receptor interaction, calcium signaling pathway, cAMP signaling pathway, Alzheimer disease, and serotonergic synapse that were related to Alzheimer's disease. Lastly, molecular docking demonstrated good stability in combining selected alkaloids with targets. CONCLUSIONS: This study explained the mechanisms of Amaryllidaceae alkaloids in preventing and treating Alzheimer's disease and established a novel strategy to discover new drugs from biological chemical sources.

Written to serve as a guideline for future research in this field, this roadmap provides some perspectives on the main developments and remaining challenges in the field of marine animal acclimatisation, adaptive potential and resilience to climate change. There has been extensive research conducted on the impact of climate change stress on marine animals, with studies recognising the potential for cross- and multi- generational impacts. Parents can potentially pass on resilience to offspring. The response of marine animals to climate change stressors is complex where utilising marginal and extreme systems as natural laboratories can help to address key research gaps and provide an understanding of the plastic and adaptive changes necessary for survival under stress.

Background: Mixed venous saturation (SvO2) is considered the gold standard to assess the adequacy of tissue oxygen delivery (DO2) in shock states. However, SvO2 monitoring is challenging as it requires an invasive catheter and frequent blood sampling. Non-invasive methods, including near-infrared spectroscopy, have demonstrated low sensitivity to tissue dysoxia. Methods: We fabricated a new device that uses resonance Raman spectroscopy (RRS) to quantify oxyhemoglobin saturation (ShbO2) in the esophagus (eShbO2), tongue (tShbO2), and liver (hShbO2). In two rat models of hemorrhagic shock, we quantified (1) The correlation of RRS-measured ShbO2 to SvO2 during progressive hemorrhage (n=20) and (2) The value of these metrics to predict near-term mortality in fixed, severe hemorrhage (mean blood pressure =25 mm Hg; n=18). Results: In model 1, eShbO2 (r=0.705, p<0.0001) and tShbO2 (r=0.724, p<0.0001) correlated well with SvO2 and with serum lactic acid (eShbO2-lactate r=0.708, p<0.0001; tShbO2-lactate r=0.830, p<0.0001). hShbO2 correlated poorly with both SvO2 and lactic acid. Using time-matched ShbO2-SvO2 pairs, the performance of ShbO2 to detect severe tissue hypoxia (SvO2<20%) was excellent (AUC 0.843 for eShbO2, 0.879 for tShbO2). In model 2, eShbO2 showed a maximized threshold of 40% with 83% of animals dying within 45 minutes of this cut-off, demonstrating accuracy as a monitoring device. This was similar for tShbO2, with a threshold of 50%, predicting death within 45 minutes in 76% of animals. ShbO2 showed superior sensitivity to invasive monitoring parameters, including MABP<30 mm Hg (sensitivity 59%), pulse pressure<15 mm Hg (sensitivity 50%), and heart rate>220 bpm (sensitivity 39%, p=0.004). Conclusions: eShbO2 represents a new paradigm to assess the adequacy of DO2 to a tissue. It constitutes a promising monitoring method to evaluate tissue oxygen saturation in real time and non-invasively, correlating with SvO2 and time to death. Cancer is the leading cause of death globally, and conventional treatments have limited efficacy with severe side effects. The use of nanotechnology has the potential to reduce the side effects of drugs by creating efficient and controlled anticancer drug delivery systems. Nanoparticles (NPs) used as drug carriers offer several advantages, including enhanced drug protection, biodistribution, selectivity and, pharmacokinetics. Therefore, this review is devoted to various organic (lipid, polymeric) as well as inorganic nanoparticles based on different building units and providing a wide range of potent anticancer drug delivery systems. Within these nanoparticulate systems, chitosan (CS)-based NPs are discussed with particular emphasis due to the unique properties of CS and its derivatives including non-toxicity, biodegradability, mucoadhesivity, and tunable physico-chemical as well as biological properties allowing their alteration to specifically target cancer cells. In the context of streamlining the nanoparticulate drug delivery systems (DDS), innovative nanoplatform-based cancer therapy pathways involving passive and active targeting as well as stimuli-responsive DDS enhancing overall orthogonality of developed NP-DDS towards the target are included. The most up-to-date information on delivering anti-cancer drugs using modern dosage forms based on various nanoparticulate systems and, specifically, CSNPs, are summarised and evaluated concerning their benefits, limitations, and advanced applications.

This paper defines a revolution as an orthogonal change in direction, a 90-degree perpendicular turn from the status quo ways of thinking, being and doing, so as to create a complete break, an abolitionist rupture with current and past ways of producing knowledge. David Bowie was a relatable example of a revolutionary and orthogonal innovator who completely and courageously broke with the past and the present and opened up new vistas in music and performing arts. The late anthropologist and public intellectual David Graeber also argued that a revolution fundamentally changes the assumptions in a given field of inquiry. Changing the entrenched assumptions that are long ossified, outdated or uncritically internalized by a knowledge community and profession can have multiplying revolutionary effects on downstream knowledge production. Thinking orthogonally to change the prevailing assumptions is indeed a revolutionary act. Orthogonal innovation as described in this paper is not a repackaging of an innovation in a different field. An orthogonal innovation is proposed as coalescence of ideas drawn from orthogonal domains, e.g., epistemologically speaking as in medicine and political theory, with an eye to pave the way for unprecedented social change and innovation. Grounding systems medicine in political determinants of planetary health, to link two fields of inquiry that have remained isolated and orthogonal since the 17th century, is nothing short of a revolution and orthogonal innovation in the making. For systems medicine to be a truly revolutionary field, it ought to acknowledge that there is no single-issue health nor single-issue politics.

The co-occurrence of nutrients and pharmaceuticals and personal care products (PPCPs) in sewage effluent can degrade water quality of the receiving watersheds. This study investigated the simultaneous removal of excess nutrients and PPCP contaminants by developing a novel woodchip bioreactor and biochar (B2) treatment system. The result revealed that woodchip bioreactors could effectively remove nitrate via a denitrification process and adsorb some PPCPs. Biochar as a secondary treatment system significantly reduced the concentrations of PPCPs and dissolved reactive phosphorus (DRP) (p < 0.05), compared to the woodchip bioreactor. The removal efficiencies of all targeted contaminants by the B2 system were evaluated using various hydraulic retention times (HRTs) and biochar types (pelletized versus granular biochar). Longer HRTs and smaller biochar particles (granular biochar) could enhance the removal efficiencies of targeted contaminants. Average contaminant removals were 77.25 % for nitrate-N, 99.03 % for DRP, 69.51 % for ibuprofen, 73.65 % for naproxen, 91.09 % for sitagliptin, and 96.96 % for estrone, with woodchip bioreactor HRTs of 12 ± 1.4 h and granular biochar HRTs of 2.1 ± 0.1 h. Notably, the second-stage biochar systems effectively mitigated by-products leaching from woodchip bioreactors. The presence of PPCPs in the woodchip bioreactors enriched certain species, such as Methylophilus (69.6 %), while inhibiting other microorganisms and reducing microbial community diversity. Furthermore, a scaled-up B2 system was analyzed and assessed, indicating that the proposed engineering treatment system could provide decades of service in real-world applications. Overall, this study suggests that the B2 system has promising applications for addressing emerging and conventional contaminants.

Thrips [Megalurothrips usitatus (Bagnall)] (Thysanoptera: Thripidae) is a pest that poses a serious challenge to global crop production and food supply, especially to the cowpea industry. Nano-delivery systems have broad application prospects in the prevention and control of pests in agriculture. Herein, three types of amino acid (AA) modified polysuccinimide nano-delivery carriers (PSI-GABA, PSI-ASP and PSI-GLU) were constructed with a diameter of approximately 150 nm to load thiamethoxam (THX), which enhanced THX effective distribution and use with cowpea plants. Significantly, the PSI-GLU nanocarrier effectively delivered THX to cowpea plant tissues following 6 h of soil application. Compared with commercial THX suspension (SC), the THX content in the leaves of cowpea plants was increased by 2.3 times. Confocal laser scanning microscopy revealed that the FITC-labeled PSI-GLU nanocarrier reached the leaves through the vascular system after being absorbed by the roots of cowpea plants. The PSI-GLU nanocarrier decreased the LC50 of THX from 11.45 to 7.79 mg/L and significantly enhanced the insecticidal effect. The PSI-GLU nanocarrier also improved the safety of THX to worker bees at 48 h, and moreover showed a growth-promoting effect on cowpea seedlings. These results demonstrated that the PSI-GLU nano-delivery carrier has promising uses on improving the effective utilization of THX for the sustainable control of thrips and reducing the risk to non-target pollutions.

A composite terminal sliding mode controller (CTSMC) for a kind of uncertain nonlinear system (UNS) is developed in this study. The primary aim of the design is to enhance the control performance of the CTSMC by learning its unknown parameters using a newly fuzzy neural network (FNN). Firstly, the stability and convergence of CTSMC for UNS with known parameters are demonstrated. Secondly, since some parameters of actual UNS are unmeasurable, a self-organizing feature selection fuzzy neural network (SOFSFNN) is intended to approach these unknown parts. Finally, the CTSMC using SOFSFNN is applied to UNS. The outcomes demonstrate that it has minimal tracking error, good robustness, and the ability to dynamically modify the network structure.

The development of photocatalytic systems that enable the simultaneous production of H2O2 and value-added organic chemicals presents a dual advantage: generating valuable products while maximizing the utilization of solar energy. Despite the potential, there are relatively few reports on photocatalysts capable of such dual functions. In this study, we synthesized a series of donor-acceptor covalent organic frameworks (COFs), designated as JUC-675 to JUC-677, to explore their photocatalytic efficiency in the co-production of H2O2 and N-benzylbenzaldimine (BBAD). Among them, JUC-675 exhibited exceptional performance, achieving a H2O2 production rate of 22.8 mmol g-1 h-1 with an apparent quantum yield of 15.7%, and its solar-to-chemical conversion efficiency was calculated to be 1.09%, marking it as the most effective COF-based photocatalyst reported to date. Additionally, JUC-675 demonstrated a high selectivity (99.9%) and yield (96%) for BBAD in the oxidative coupling of benzylamine. The underlying reaction mechanism was thoroughly investigated through validation experiments and density functional theory (DFT) calculations. This work represents a significant advancement in the design of COF-based photocatalysts and the development of efficient dual-function photocatalytic platforms, offering new insights and methodologies for enhanced solar energy utilization and the synthesis of value-added products.

This paper analyses the structure of and variability in taxation and prescription drug distribution policies and quantifies the impact of such policies on the cost of prescription drugs to health systems in 35 countries. Taxes on prescription drugs remain highly prevalent (83% of the sample) although 63% of the sample countries implement a lower than standard VAT rate. Three remuneration types of the wholesale and retail distribution chain have been identified. Wholesale and retail distributors are remunerated on a regressive mark-up basis, which is price-dependent, although fixed fees and fixed percentages, which are non-price dependent, are also highly prevalent. Price component analysis for three groups of products classed as high-, medium- and low-priced suggests that mark-ups plus taxes varied significantly across countries and products, and ranged from 5% to 187% of ex-factory prices. Average margins also vary significantly by countries and products ranging 5-65% of retail prices. The cost of distribution and taxation contributes significantly to prescription drug costs for health systems. Although distribution chain remuneration raises efficiency and overall affordability questions, these need to be considered together with the regulatory framework shaping market structure of the distribution chain, as well as any prevailing horizontal and vertical integration policies. The overall cost of prescription drugs could be reduced immediately by eliminating taxation; this could go some way to alleviate fiscal pressures on health budgets, whilst avoiding resource re-allocation from health to other sectors.

Wheat stripe rust (yellow rust), caused by Puccinia striiformis f. sp. tritici (Pst), is an important airborne disease worldwide. Pst inoculum strength in southern Henan in winter or early spring is important for spring epidemic in the main autumn-sown wheat-growing regions of China. However, there is limited knowledge about the source and time of initial infection on winter wheat in southern Henan. The first occurrence of wheat stripe rust in southern Henan was recorded annually from 2011-2022, from which we used the backward trajectory approach to infer the likely source of Pst inoculum responsible for the initial disease occurrence. The results suggested that the Pst inoculum responsible for initial rust established in the winter in southern Henan originated from the Gansu Pst oversummering area in China, whereas it originated from the adjacent winter Pst sporulation regions in southern Shaanxi and northwestern Hubei if Pst symptoms were first observed in early spring in southern Henan. Another possible Pst source is southern Hubei where Pst can also sporulate in the winter. Thus, early Pst development in winter in the main wheat production in China (Henan) is likely to be caused by Pst inoculum spread from the oversummering inocula or Pst epidemics in autumn seedlings in Gansu.

Modern drug discovery is driven by high demand in the pharmaceutical industry to test growing libraries of compounds against potential targets. High-throughput screening (HTS) is characterized by fully automated experimentation that leverages robotic liquid handling systems, analytical techniques, and advanced computing and statistics, including the recent integration of artificial intelligence. To align with this trend, it is crucial to develop and implement new HTS platforms that offer improved predictivity and physiological relevance. In recent years, microphysiological systems, commonly known as organ-on-chip (OoC) systems, have progressed from a theoretical concept to a powerful alternative to conventional in vitro and animal models. High-throughput OoC (HT-OoC) systems could represent the disruptive technology sought by pharmaceutical companies to address their enormous research and development (R&D) expenses. In this study, we provide a brief overview of commercial products utilizing modern HT-OoC systems in drug discovery and development. Additionally, we discuss recent trends in R&D aimed at industrialization.

Thiamine (vitamin B1), under the proper conditions, is able to reversibly open the thiazole ring, forming a thiol-bearing molecule that can be further oxidized to the corresponding disulfide. To improve the bioavailability of the vitamin, several derivatives of thiamine in the thioester or disulfide form were developed and extensively studied over time, as apparent from the literature. We have examined three thiamine-derived disulfides: thiamine disulfide, sulbutiamine, and fursultiamine with reference to their intervention in modulating the thiol redox state. First, we observed that both glutathione and thioredoxin (Trx) systems were able to reduce the three disulfides. In particular, thioredoxin reductase (TrxR) reduced these disulfides either directly or in the presence of Trx. In Caco-2 cells, the thiamine disulfide derivatives did not modify the total thiol content, which, however, was significantly decreased by the concomitant inhibition of TrxR. When oxidative stress was induced by tert-butyl hydroperoxide, the thiamine disulfides exerted a protective effect, indicating that the thiol form deriving from the reduction of the disulfides might be the active species. Further, the thiamine disulfides examined were shown to increase the nuclear levels of the transcription factor nuclear factor erythroid 2 related factor 2 and to stimulate both expression and activity of NAD(P)H quinone dehydrogenase 1 and TrxR. However, other enzymes of the glutathione and Trx systems were scarcely affected. As the thiol redox balance plays a critical role in oxidative stress and inflammation, the information presented can be of interest for further research, considering the potential favorable effect exerted in the cell by many sulfur compounds, including the thiamine-derived disulfides.

AIM: The objective of this study was to explore how selected sub-national (provincial) primary healthcare units in Ethiopia responded to coronavirus disease 2019 (COVID-19) and what impact these measures had on essential health services. BACKGROUND: National-level responses against the spread of COVID-19 and its consequences are well studied. However, data on capacities and challenges of sub-national health systems in mitigating the impact of COVID-19 on essential health services are limited. In countries with decentralized health systems like Ethiopia, a study of COVID-19 impacts on essential health services could inform government bodies, partners, and providers to strengthen the response against the pandemic and document lessons learned. METHODS: We conducted a qualitative study, using a descriptive phenomenology research design. A total of 59 health leaders across Ethiopia's 10 regions and 2 administrative cities were purposively selected to participate in key informant interviews. Data were collected using a semi-structured interview guide translated into a local language. Interviews were conducted in person or by phone. Coding of transcripts led to the development of categories and themes, which were finalized upon agreement between two investigators. Data were analysed using thematic analysis. FINDINGS: Essential health services declined in the first months of the pandemic, affecting maternal and child health including deliveries, immunization, family planning services, and chronic disease services. Services declined due to patients' and providers' fear of contracting COVID-19, increased cost of transport, and reallocation of financial and human resources to the various activities of the response. Authorities of local governments and the health system responded to the pandemic immediately, capitalizing on multisectoral support and redirecting resources; however, the intensity of the response declined as time progressed. Future investments in health system hardware - health workers, supplies, equipment, and infrastructure as well as carefully designed interventions and coordination are needed to shore up the COVID-19 response.

While thousands of health systems have begun to implement the Age-Friendly Health System's 4Ms Framework to improve care for older patients, an important phase of work is achieving consistent adherence to 4Ms care processes. Identifying mechanisms that may lead to higher versus lower adherence serves to guide efforts to achieve consistent, equitable adherence. Drawing from prior literature, we identified three mechanisms that may influence 4Ms adherence. We then conducted a 3-year retrospective, observational study of inpatient encounters (n = 28 833) at UCSF Health System with patients aged 65+. We used least squares regression models to assess associations between hospital encounter-level measures of 4Ms adherence and proxy measures of patient and encounter characteristics for each hypothesized mechanism along with control variables. Encounter-level adherence to the 4Ms was 65.5% (SD = 14.3%). We found support for all three mechanisms. Negative implicit biases were associated with lower adherence for patients who were obese [0.79 percentage points (PP) lower; P < .001] or on Medicaid (0.64 PP lower; P = .002). Positive implicit biases were associated with higher adherence for the oldest old (aged 85+; 2.85 PP higher; P < .001) or with reduced mobility (2.01 PP higher; P < .001). Patients with comprehensive geriatrics care contact (ACE unit and a geriatrics consult) had 5.33 PP higher adherence (P < .001). While most effects were modest in magnitude, our results suggest that both positive and negative implicit biases, as perceived by the provider, may influence the level of 4Ms adherence. Contact with comprehensive geriatrics care appeared most influential. These insights can be leveraged to develop strategies to achieve equitable delivery of 4Ms care.

Aging populations face significant cognitive challenges, particularly in working memory (WM). Transcranial alternating current stimulation (tACS) offer promising avenues for cognitive enhancement, especially when inspired by brain physiology. This study (NCT04986787) explores the effect of multifocal tACS on WM performance in healthy older adults, focusing on fronto-parietal network modulation. Individualized physiology-inspired tACS applied to the fronto-parietal network was investigated in two blinded cross-over experiments. The first experiment involved monofocal/bifocal theta-tACS to the fronto-parietal network, while in the second experiment cross-frequency theta-gamma interactions between these regions were explored. Participants have done online WM tasks under the stimulation conditions. Network connectivity was assessed via rs-fMRI and multichannel electroencephalography. Prefrontal monofocal theta tACS modestly improved WM accuracy over sham (d = 0.30). Fronto-parietal stimulation enhanced WM task processing speed, with the strongest effects for bifocal in-phase theta tACS (d = 0.41). Cross-frequency stimulations modestly boosted processing speed with or without impairing task accuracy depending on the stimulation protocol. This research adds to the understanding of physiology-inspired brain stimulation for cognitive enhancement in older subjects.

Background: Newly industrialized countries like Thailand have been influenced by globalization, westernization, and urbanization over the last decades, leading to changes in dietary habits as well as food production. Consequences of these changes include rising non-communicable diseases (NCDs) and environmental degradation, which are defined as the leading global challenges today. The objectives of this study are to identify Thailand's dietary changes, considering health and sustainability aspects, and to determine correlations between these changes and NCD cases as well as environmental impacts (GHG emissions, land-, nitrogen-, phosphorus-use). In this way, diet-related adjustments can be identified to promote planetary and human health. Methods: In this longitudinal ecological study, relative differences between the average food consumption in Thailand and the reference values of a healthy and sustainable diet, the Planetary Health Diet (PHD), were calculated. Furthermore, a bivariate correlation analysis was conducted, using data, based on Food and Agriculture Organization's (FAO's) data, results from the Global Burden of Disease Study (GBD), and PHD's reference values. Findings: The consumption quantities of meat, eggs, saturated oils, and sugar increased significantly since 1961. The food groups, that have exceeded PHD's upper reference values, include sugar (+452%), red meat (+220%), grains (+143%), saturated oils (+20%) and eggs (+19%), while vegetables (-63%), and unsaturated oils (-61%) have fallen below PHD's lower limits. Concerning the bivariate correlation analyses, all investigated variables show significant correlations. The most significant correlations were found in NCD cases (r = 0.903, 95% CI 0.804-0.953), nitrogen use (r = 0.872, 95% CI 0.794-0.922), and land use (r = 0.870, 95% CI 0.791-0.921), followed by phosphorus use (r = 0.832, 95% CI 0.733-0.897), and green-house gas (GHG) emissions (r = 0.479, 95% CI 0.15-0.712). Interpretation: The results show, that the determined differences of unhealthy or unsustainable food groups have increased concurrently with NCD cases and environmental impacts over the last decades in Thailand. A shift towards a reduced intake of sugar, red meat, grains, saturated oils and eggs along with an increase in vegetables and unsaturated oils, might support environmental and human health.
