RESUMO

OBJECTIVE: To derive and validate internally a novel risk assessment tool to identify young children at risk for all-cause mortality ≤60 days of discharge from hospitals in sub-Saharan Africa. STUDY DESIGN: We performed a prospective observational cohort study of children aged 1-59 months discharged from Muhimbili National Hospital in Dar es Salaam, Tanzania and John F. Kennedy Medical Center in Monrovia, Liberia (2019-2022). Caregivers received telephone calls up to 60 days after discharge to ascertain participant vital status. We collected socioeconomic, demographic, clinical, and anthropometric data during hospitalization. Candidate variables with P < .20 in bivariate analyses were included in a multivariable logistic regression model with best subset selection to identify risk factors for the outcome. We internally validated our tool using bootstrapping with 500 repetitions. RESULTS: There were 1933 young children enrolled in the study. The median (IQR) age was 11 (4, 23) months and 58.7% were males. In total, 67 (3.5%) died during follow-up. Ten variables contributed to our tool (total possible score 82). Cancer (aOR 10.6, 95% CI 2.58, 34.6), pedal edema (aOR 6.94, 95% CI 1.69, 22.6), and leaving against medical advice (aOR 6.46, 95% CI 2.46, 15.3) were most predictive of post-discharge mortality. Our risk assessment tool demonstrated good discriminatory value (optimism corrected area under the receiver operating characteristic curve 0.77), high precision, and sufficient calibration. CONCLUSIONS: After validation, this tool may be used to identify young children at risk for post-discharge mortality to direct resources for follow-up of high-risk children.

RESUMO

Digital adherence technologies are increasingly used to support tuberculosis (TB) treatment adherence. Using microcosting, we estimated healthcare system costs (in 2022 US dollars) of 2 digital adherence technologies, 99DOTS medication sleeves and video-observed therapy (VOT), implemented in demonstration projects during 2018-2021. We also obtained cost estimates for standard directly observed therapy (DOT). Estimated per-person costs of 99DOTS for drug-sensitive TB were $98 in Bangladesh (n = 719), $119 in the Philippines (n = 396), and $174 in Tanzania (n = 976). Estimated per-person costs of VOT were $1,154 in Haiti (87 drug-sensitive), $304 in Moldova (173 drug-sensitive), $452 in Moldova (135 drug-resistant), and $661 in the Philippines (110 drug-resistant). 99DOTS costs may be similar to or less expensive than standard DOT. VOT is more expensive, although in some settings, labor cost offsets or economies of scale may yield savings. 99DOTS and VOT may yield savings to local programs if donors cover infrastructure costs.

RESUMO

Newcastle disease (ND) is among the most important poultry diseases worldwide. It is the major threat to poultry production in Africa and causes major economic losses for both local and commercial chickens. To date, half of ND class II genotypes have been reported in Africa (I, IV, V, VI, VII, XI, XIII, XIV, XVII, XVIII, and XXI). The information on the circulating NDV genotypes is still scarce despite the endemic nature of ND in most countries on the African continent.A total of 659 oro-cloacal swabs were collected from local chickens in Mawenzi live bird market located in Morogoro, Tanzania, between June 2020 and May 2021. Newcastle disease virus was detected by using reverse transcription real-time polymerase chain reaction (RT-qPCR) and conventional PCR followed by sequencing of PCR products. The prevalence of NDV in the surveilled live bird markets was 23.5%. Sequencing and phylogenetic analysis revealed the presence of sub-genotype VII.2. The detected sub-genotype VII.2 has phylogenetic links to Zambian NDV strains implying a Southeast dissemination of the virus, considering that it was first detected in Mozambique. This study underscores the need of active NDV surveillance to determine the distribution of this NDV genotype in the country and monitor its spread and contribution to the emergence of new ND viruses.

RESUMO

The aim of this work was to identify efficient vigor tests for differentiating the seed lots, forecasting seedling emergence in the field and assessing the physiological quality of Panicum maximum seeds. 12 seed lots from the cultivar Tanzania and 11 seed lots from the cultivar Massai were evaluated for water content, germination, first count and germination speed index, emergence and first emergence count of seedlings in sand, root length and shoot length, analysis of SVIS® images (seedling length, vigor and uniformity index) and seedling emergence in the field. The work was conducted in a completely randomized design for tests performed in the laboratory and in randomized blocks for tests in the field. The data were subjected to analysis of variance and the means compared by Scott Knott's test at 5% probability and statistical multivariate clustering analysis and principal components analysis. The shoot and root length tests are efficient for the evaluation of the physiological potential of P. maximum cv. Massai, while the seedling length, vigor index and growth uniformity index tests using image analysis, seedling emergence in sand and first seedling emergence count in sand are efficient in assessing the physiological potential of seeds of P. maximum cv. Tanzania, and providing information similar to that of seedling emergence in the field.

RESUMO

Rapid oral HIV self-tests (HIVST) have potential to increase the proportion of people who know their HIV status, especially among stigmatised populations. This study was embedded in two cohorts of female sex workers (FSW) in the Dominican Republic (DR) and Tanzania. Qualitative interviews with 40 FSW were conducted to explore perceived acceptability of HIVST. Interviews were analysed using inductive and deductive thematic coding. Emergent themes were organised by socio-ecological framework levels. FSW in both settings responded positively to the ease of use of HIVST but questioned test accuracy due to the use of saliva rather than blood. FSW in the DR had a more cautious response, while women in Tanzania had favourable perceptions expressing eagerness to use it. At the individual level, themes shaping participants' interest included autonomy, HIV risk perception, and emotional well-being for those with reactive test results, and self-efficacy. At the interpersonal level, privacy, confidentiality, sex work and HIV stigma and social support were salient. Structural level themes focused on health systems including linkages to HIV treatment, provider roles, and access (cost, travel, distribution). Understanding FSW's perceptions and acceptability of HIVST is essential to its integration into health systems and programmes using a community-driven approach.

RESUMO

In sub-Saharan Africa (SSA),cognitive screening is complicated by both cultural and educational factors, and the existing normative values may not be applicable. The Identification of Dementia in Elderly Africans (IDEA) cognitive screen is a low-literacy measure with good diagnostic accuracy for dementia. OBJECTIVE: The aim of this study is to report normative values for IDEA and other simple measures [i.e., categorical verbal fluency, the Consortium to Establish a Registry for Alzheimer's Disease (CERAD) 10-word list] in representative community-dwelling older adults in SSA. METHODS: Individuals aged ≥60 years resident in 12 representative villages in Kilimanjaro, Tanzania and individuals aged ≥65 years resident within three communities in Akinyele Local Government Area, Oyo State, Nigeria underwent cognitive screening. The normative data were generated by the categories of age, sex, and education. RESULTS: A total of 3,011 people in Tanzania (i.e., 57.3% females and 26.4% uneducated) and 1,117 in Nigeria (i.e., 60.3% females and 64.5% uneducated) were screened. Individuals with higher age, lower education, and female gender obtained lower scores. The 50th decile values for IDEA were 13 (60-64 years) vs. 8/9 (above 85 years), 10-11 uneducated vs. 13 primary educated, and 11/12 in females vs. 13 in males. The normative values for 10-word list delayed recall and categorical verbal fluency varied with education [i.e., delayed recall mean 2.8 [standard deviation (SD) 1.7] uneducated vs. 4.2 (SD 1.2) secondary educated; verbal fluency mean 9.2 (SD 4.8) uneducated vs. 12.2 (SD 4.3) secondary educated], substantially lower than published high-income country values. CONCLUSIONS: The cut-off values for commonly used cognitive screening items should be adjusted to suit local normative values, particularly where there are lower levels of education.

The effect of pasture management on CH4 emissions was investigated from goats in a tropical climate. Two experiments were conducted in a "Tanzania Guinea grass" (Panicum maximum Jacq.) pasture to assess enteric CH4 production in a completely randomized design. Emissions from light, moderate, and heavy grazing intensities were analyzed in the first experiment, and variations between grazing days were explored in the second experiment. Grazing intensity was defined as 2.4, 1.6, and 0.8 post-grazing leaf area index. Pasture management employed intermittent grazing with variable stocking rate using Anglo Nubian female adult goats. SF6 tracer gas technique was used to measure CH4 production. Grazing intensity was not found to affect CH4 emissions per animal, dry matter forage intake (DMI), and gross energy (GE) intake. However, the second experiment showed that CH4 production was influenced by the grazing day. CH4 emissions were 18.1 g day-1, and the variables were 0.88 g kg-1 of metabolic weight, 17.45 g kg-1 of DMI, and 5.5% of GE. CH4 production increased linearly with the grazing day, possibly reflecting a reduction in forage quality. These findings suggest that the day of occupation in intermittent grazing has a greater effect on CH4 emissions than that by grazing intensity and that a single day grazing of Tanzania Guinea grass could mitigate CH4 emissions.

RESUMO

Herbicides have been used to control Brachiaria grass in pastures established or in formation given their practicality, however their efficiency is questionable due to the lack of specific graminicides for different forage species. Therefore, the goal of this study was to evaluate the efficiency of glyphosate and fluazifop-p-butyl in the control of Brachiaria decumbens (signalgrass) and the intoxication levels of Panicum maximum cv. Tanzania (Tanzania quinea grass) in pasture establishment. The experiments were designed in randomized blocks, in a 2 × 5 × 4 factorial arrangement of two herbicides (fluazifop-p-butyl and glyphosate), five doses equivalent to the commercial dosage of each herbicide (0.25; 0.50; 1.00; 1.50; 200), and four evaluation times after herbicide application (15, 21, 30 and 45 days). There was interaction between doses and evaluation times. The dose 1.5 L ha-1 fluazifop-p-butyl provides efficient control of signalgrass, however, leads to high intoxication in Tanzania guinea grass. Glyphosate is efficient in the control of signalgrass even at the lowest dose (90 g ha-1 ), however, it causes high intoxication in Tanzania guinea grass, preventing its use in developing pastures. It can be concluded that fluazifop-p-butyl and glyphosate herbicides are not recommended for the control of B. decumbens, cv. Basilisk in developing pastures of Tanzania guinea grass.(AU)

RESUMO

Rice farming is characterized by various factors including environmental and non-environmental factors. The current paper analyses the influence of households characteristics, and perceptions of weather variability on rice yield. Authors used primary data collected from small-scale rice farmers in the Mbeya region of Tanzania. Garret technique and Stata software were used for data analyses. Results confirmed that farmers education, marital status, gender, and land ownership have a positive influence on rice yield. Thus, for every 1% increase in each variable increases rice yield by 14%, 98%, 26%, and 21% respectively. Owing to empirical results on farmers perceptions, it is confirmed that if the drought period increased by 1%, would on average reduce rice yield by 20%. The majority of farmers (70%) have perceived that drought period reduced their rice yield. In addition, farmers used averagely less quantity of rice seeds which have impaired their rice yields. If farmers increased the seed quantity by 1%, rice yields would increase by 17%. Our findings also revealed that the previous market price has positive association with the next seasons rice yield. Therefore, our study concluded that among households variables and their perceptions have positive implications on rice yield; however, with the empirical analyses other variables were insignificant due to multicollinearity effects. Authors recommended to the government and other responsible organizations to capitalize on irrigation infrastructures, agricultural extension and technological services to restraint the enduring food insecurity coercions in Tanzania.(AU)

The use of herbicide underdoses allows minimizing the competition of grasses on annual crops, enabling simultaneous cultivation. In this context, the objective of this study was to investigate glyphosate underdoses on the suppression of the initial growth of three Panicum maximum cultivars aiming at the integrated cultivation, in addition to the effects of forage species on the incidence and development of weeds. Three field experiments were conducted. The experimental design was a randomized block design with four replications and eight treatments consisting of increasing glyphosate doses (0, 54, 108, 270, 378, 540, 756, and 1,080 g a.e. ha−1). An atrazine dose of 1,200 g a.i. ha−1 was added to each treatment. Plant phytotoxicity assessments were performed at 7, 14, 21, and 28 days after application. At 80 and 125 days after sowing, the assessments of total dry matter production, leaf dry matter, stem dry matter, and leaf to stem ratio were carried out, in addition to density and dry matter production of weed community. Glyphosate underdoses below 215, 65, and 90 g a.e. ha-1 have a potential to be investigated aiming at the management of P. maximum cv. Atlas, P. maximum cv. Mombasa, and P. maximum cv. Tanzania under intercropping. The three forage species are effective in suppressing weeds.(AU)

The aim of this study was to evaluate the effect of three different grazing systems: isolated, alternate and simultaneous, on feed intake, performance and carcass characteristics of sheep. About 5.2 ha area of Tanzania grass (Panicum maximun Jacq cultivate Tanzania) was divided into 13 paddocks. This area was used as a stocking rate of two animal units (AU) per ha for 7 days occupation and 21 days rest. A total number of 58 animals were used consisting of 12 heifers and 30 Santa Ines lambs with the addition of 16 adult ewes that were used to stabilize grazing pressure in the isolated system. The sheep were fed on 200 g per head per day of concentrate and cattle 2 kg per head per day. The parameters determined were the following: weekly weight (WW), total live weight gain (LWG) and mean daily weight gain (MDW). Also dry matter intake was estimated 84 days after the start of the experiment using external indicators (Purified and Enriched Lignin) in addition to carcass traits and composition which were also estimated. The result obtained for carcass composition revealed that the muscle:bone ratio and bone percentage were better in the alternate system. Moreover, the simultaneous and isolated systems showed higher lamb performance than the alternate system, while there was no effect on dry matter intake. Furthermore, there was no difference of the different systems on carcass traits and feed intake of sheep. Nevertheless, the simultaneous grazing system showed better sheep performance than the alternate grazing system.(AU)

RESUMO

The upper Triassic deposits of the Selous Basin in south Tanzania have not been prospected for fossil tetrapods since the middle of last century, when Gordon M. Stockley collected two rhynchosaur bone fragments from the so called "Tunduru beds". Here we present the results of a field trip conducted in July 2015 to the vicinities of Tunduru and Msamara, Ruvuma Region, Tanzania, in search for similar remains. Even if unsuccessful in terms of fossil discoveries, the geological mapping conducted during the trip improved our knowledge of the deposition systems of the southern margin of the Selous Basin during the Triassic, allowing tentative correlations to its central part and to neighbouring basins. Moreover, we reviewed the fossil material previously collected by Gordon M. Stockley, confirming that the remains correspond to a valid species, Supradapedon stockleyi, which was incorporated into a comprehensive phylogeny of rhynchosaurs and found to represent an Hyperodapedontinae with a set of mostly plesiomorphic traits for the group. Data gathered form the revision and phylogenetic placement of Su. stockleyi helps understanding the acquisition of the typical dental traits of Late Triassic rhynchosaurs, corroborating the potential of hyperodapedontines as index fossils of the Carnian-earliest Norian.

RESUMO

OBJECTIVE: To assess the relationship between breastfeeding initiation time and postneonatal mortality, morbidity, and growth through 24 months in a cohort of Tanzanian infants. STUDY DESIGN: We included 4203 infants from 2 trials of micronutrient supplementation. We used Cox proportional hazards models or general estimating equations to estimate relative risks. RESULTS: A total of 13% of infants initiated breastfeeding >1 hour after birth (n = 536). There was no association between breastfeeding initiation time and risk of all-cause or cause-specific mortality, nor infant growth failure, from 6 weeks to 2 years of age. However, delayed breastfeeding was associated with an increased risk of several common infectious morbidities in early infancy, including upper respiratory infection symptoms and vomiting. Compared with those who initiated breastfeeding within the first hour of birth, delayed breastfeeding initiation was associated with an 11% increased risk of cough (relative risk 1.11, 95% CI 1.02-1.21) and a 48% increased risk of difficulty breathing (relative risk 1.48, 95% CI 1.09-2.01) during the first 6 months. Delayed initiation was associated with a greater risk of difficulty breathing from 6 to 12 months of age, but it was not associated with risk of any other morbidity during this time, nor any morbidity between 12 and 24 months. CONCLUSION: Delayed breastfeeding initiation is associated with an increased risk of infant morbidity during the first 6 months of life. Early breastfeeding initiation, along with exclusive and prolonged breastfeeding, should be prioritized and promoted in efforts to improve child health.

RESUMO

Genetic breeding of forage plants has increasingly contributed to the release of more productive plants. In this regard, evaluating the genotypic value is essential when aiming to rank genotypes based on the mean free of environmental factors. Therefore, this study aimed to predict the genotypic value of agronomic and nutritive value characters of three progenies of Panicum maximum. Hybrids were evaluated in a clonal test in an incomplete-randomized design with three treatments (progenies 1, 2, and 3) and two replications (clones). Six harvests were performed at 25cm from the ground level throughout one year. Progeny 2 provided better results for total and leaf dry mass yield, regrowth, and height, and lower incidence of leaf spot. Progenies 1 and 3 had a better response for qualitative characters such as higher crude protein and digestibility and lower lignin and fiber content. Hybrid progenies of P. maximum have forage characters of interest for breeding, and when using Mombaça grass as parents, the progeny stands out for leaf production and resistance to leaf spot and for Tanzania grass as parent has resulted in better forage quality.(AU)
