RESUMO

BACKGROUND: Spinal masses can be diagnosed by clinical and radiographic examinations. Infrequently, pseudotumors may be due to retained masses after surgical interventions. In fact, these spinal or paraspinal expansions are caused by iatrogenic foreign bodies. Pseudotumors are mentioned as textilomas. CASE DESCRIPTION: We present a case of a patient with a history of lumbar diskectomy in the L2-L3 segments performed in 2017. A 53-year-old woman was admitted with the complaint of persistent mechanical lower back and leg pain for 2 months. CONCLUSIONS: There are no specific clinical and paraclinical manifestations for retained surgical foreign bodies. The number of cases of textilomas associated with spinal surgery are few in comparison with abdominal or thoracic interventions. It is better to integrate textiloma in the differential diagnosis of soft-tissue masses in the paraspinal region with surgical history. Although the definitive treatment of textilomas is surgical elimination of the foreign body, exact evaluation of surgical site before its closure is essential to prevent these cases.

RESUMO

Materials used in neurosurgery to achieve hemostasis may be of resorbable or nonresorbable substance and may cause foreign body granuloma if left at the operative site. Foreign body granuloma depending on clinical history may be indistinguishable from an abscess, resolving infarction, and hematoma. Here we present two cases, who had decompressive craniectomy following road traffic accident. Follow-up computerized tomography (CT) scan revealed hyperdense lobulated lesion with peripheral rim enhancement. On magnetic resonance imaging (MRI), lesions were predominantly hypointense on T1-weighted images, and hyperintense on T2-weighted images and showed a lack of restricted diffusion. In view of recent craniectomy and imaging findings diagnosis of foreign body granuloma was made. Both patients underwent surgery, intraoperatively gauze pieces were retrieved from lesions which confirmed preoperative diagnosis. The combination of CT and MRI can diagnose foreign body granuloma, especially in trauma settings. Thus, we can help the surgeon by providing the probable diagnosis for proper management.

The authors report three cases of intrathoracic foreign body that is defined as a cotton matrix mass, mostly retained surgical sponge, a rare complication of a thoracic surgery. The patients wereevaluated by chest radiography and computed tomography with the imaging findings confirmed after thoracotomy and anatomopathological study. The mainly imaging findings consisted of intrathoracic masses in patients with previous thoracic surgery that return to hospital with lower respiratory tract symptoms in different period after surgery procedure. The three cases were related with a brief review of the literature.
