INTRODUCTION AND OBJECTIVES: Atrial fibrillation (AF) and heart failure (HF) often coexist. AF catheter ablation improves left ventricular ejection fraction (LVEF), but its impact varies between patients. We aimed to identify predictors of LVEF improvement in HF patients with impaired LVEF undergoing AF ablation. METHODS: We conducted a retrospective single-center study in HF patients with LVEF <50% undergoing AF catheter ablation between May 2016 and May 2022. The primary endpoint was the LVEF recovery rate ('responders'). Secondary endpoints were one-year safety and effectiveness. We also aimed to validate a prediction model for LVEF recovery. RESULTS: The study included 100 patients (79% male, median age 60 years, 70% with probable tachycardia-induced cardiomyopathy [TIC], mean LVEF 37%, 29% with paroxysmal AF). After a median follow-up of 12 months after catheter ablation, LVEF improved significantly (36±10% vs. 53±10%, p<0.001), with an 82% responder rate. A suspected diagnosis of TIC (OR 4.916 [95% CI 1.166-20.732], p=0.030), shorter QRS duration (OR 0.969 [95% CI 0.945-0.994], p=0.015), and smaller left ventricle (OR 0.893 [95% CI 0.799-0.999], p=0.049) were independently associated with LVEF improvement. Freedom from any documented atrial arrhythmia was 86% (64% under antiarrhythmic drugs), and the rate of adverse events was 2%. The prediction model had a good discriminative performance (AUC 0.814 [95% CI 0.681-0.947]). CONCLUSION: In AF patients with HF and impaired LVEF, suspected TIC, shorter QRS duration, and smaller LV diameter were associated with LVEF recovery following AF catheter ablation.

INTRODUCTION AND OBJECTIVES: Echocardiography guidelines suggest normalizing left ventricular (LV) volumes and mass (LVM) to body size. During pregnancy, continuous weight variation impacts on body surface area (BSA) calculation, limiting the longitudinal analysis of cardiac remodeling (CR) and reverse remodeling (RR) variables. Our aim was to identify the most common indexing methodologies in the literature on pregnant populations through a systematic review; and, to compare four scaling methods: (i) none (absolute values); (ii) indexing to the BSA before pregnancy; (iii) allomeric indexing; and (iv) indexing to BSA measured at the same day of cardiac assessment, using an illustrative example. METHODS: We performed a systematic review of CR and RR during pregnancy and post-partum, using two databases. We included studies reporting longitudinal echocardiographic analysis of cardiac chamber volumes in humans. We used a prospective cohort study of healthy pregnant women who underwent four echocardiographic evaluations during pregnancy and postpartum, as an illustrative example. RESULTS: Twenty-seven studies were included, most studies indexed to BSA measured at each evaluation moment (n=21). Within-subjects design was the most reported to analyse longitudinal data (n=17). Indexation to the pre-pregnancy BSA or application of allometric indexes revealed a higher effect than BSA measured at each evaluation and an equal effect to not indexing using within-subjects design. The within-subjects designs also revealed a higher effect size value than the between-subjects design for longitudinal analysis of LVM adaptations during pregnancy and postpartum. CONCLUSION(S): This study concludes that indexation methods do not impact the clinical interpretation of longitudinal echocardiographic assessment but highlights the need to harmonize normalization procedures during pregnancy.

INTRODUCTION: Left ventricular thrombus commonly occurs as a complication of acute anterior myocardial infarction and nonischemic cardiomyopathies with severe left ventricular systolic dysfunction. Its frequency is still high despite medical advances. Current guidelines recommend the use of vitamin k antagonists as first-line therapy, however, the off-label use of direct oral anticoagulants is becoming more frequent and attractive, given the better pharmacological and clinical profile, with the improvement of the patient's quality of life. AIM: To provide an update on the currently existing evidence regarding the outcomes of efficacy and safety of direct oral anticoagulants (DOACs) as first-line therapy in left ventricular thrombus, in comparison to vitamin K antagonists (VKAs). METHODS: A systematic review and meta-analysis of studies on the effects of direct oral anticoagulants versus vitamin K antagonists on left ventricular thrombi and on the results was performed. RESULTS: Fourteen studies were included in the meta-analysis, with a total of 2498 patients (n=631 direct oral anticoagulants and n=1867 for VKAs). No significant differences were found in efficacy and safety outcomes (odds ratio (OR) 0.86; 95% confidence interval (CI), 0.55-1.33; p=0.50; I2=32%) and (OR 1.0; 95% CI, 0.78-1.30; p=0.93; I2=2%) respectively. No difference was noted in all-cause mortality (OR 0.92; 95% CI, 0.58-1.45; p=0.74; I2=0%). Thrombus resolution was observed in 288/416 in direct oral anticoagulants vs. 732/1085 patients treated with VKAs (OR 1.14; 95% CI, 0.77-1.66; p=0.50; I2=33%). CONCLUSIONS: The findings of this meta-analysis suggest the potential utility of DOACs as a first-line strategy in patients with left ventricular thrombus.

INTRODUÇÃO: a insuficiência cardíaca (IC) é uma das três causas mais comuns de doenças cardiovasculares (DCV), grupo de enfermidades que é a principal causa de morbimortalidade no mundo. O eletrocardiograma (ECG) é um dos exames utilizados na avaliação da IC, sendo de baixo custo e amplamente acessível. Quando associado à inteligência artificial, o ECG pode ser uma poderosa ferramenta para triagem de indivíduos com maior probabilidade de IC. O objetivo foi avaliar o desempenho de um algoritmo de IA, aplicado ao ECG, para detecção de DSVE e compará-lo ao das alterações maiores ao ECG (AME), de acordo com o código de Minnesota. MÉTODOS: estudo transversal retrospectivo de acurácia diagnóstica que utilizou a população do Estudo Longitudinal da Saúde do Adulto (ELSA-Brasil). Foram avaliados 2567 indivíduos que possuíam ecocardiograma (ECO) e ECG válidos e valores de predição para disfunção sistólica do ventrículo esquerdo (DSVE) estimadas por um algoritmo de inteligência artificial (IA). A DSVE foi definida como Fração de Ejeção do Ventrículo Esquerdo (FEVE) menor que 40%, calculada utilizando o ECO. A prevalência de DSVE foi de 1,13% na população estudada (29 indivíduos). Foram calculados sensibilidade, especificidade, valor preditivo positivo (VPP), valor preditivo negativo (VPN), razão de verossimilhança positivo (RVP), razão de verossimilhança negativa (RVN), diagnostic odds ratio (DOR) para o algoritmo e para as AME. Calculou-se também a área sob a curva ROC (ASC-ROC) para o algoritmo. RESULTADOS: a população estudada possui mediana de 62 anos, sendo 47,2% do sexo masculino. A ASC-ROC do algoritmo para predição de IC foi de 0,947 (IC 95% 0,913 ­ 0,981). A sensibilidade, especificidade, VPP, VPN, RVP, RVN e DOR para o algoritmo foi de 0,690; 0,976; 0,244; 0,996; 27,6; 0,32 e 88,74, respectivamente. Para as AME foi 0,172; 0,837; 0,012; 0,989; 1,09; 0,990 e 1,07 respectivamente. CONCLUSÕES: A IA aplicada ao ECG é uma fermenta promissora para identificação de pacientes com maior probabilidade de IC e que devem ser priorizados para realização de ECO. Isso poderia aprimorar o diagnóstico de IC em nosso meio e, assim, permitir o início precoce do tratamento, com possível impacto na redução da morbidade e mortalidade.


INTRODUCTION: Heart failure (HF) is one of the three most common causes of cardiovascular diseases (CVD), which are the leading causes of morbidity and mortality worldwide. The electrocardiogram (ECG) is one of the tests used in the evaluation of HF, combining low-cost and wide accessibility. When combined with artificial intelligence, the ECG can be a powerful tool for screening individuals with a higher risk of HF. Our objective was to assess the performance of an AI algorithm applied to the ECG for the detection of left ventricular systolic dysfunction (LVSD) and compare it to the performance of major ECG abnormalities (MEA) according to the Minnesota code. METHODS: This was a retrospective cross-sectional diagnostic accuracy study using data from the Brazilian Longitudinal Study of Adult Health (ELSA-Brazil). A total of 2567 individuals with valid echocardiograms (ECO) and ECGs and probability values for left ventricular systolic dysfunction (LVSD) estimated by an artificial intelligence (AI) algorithm, were evaluated. LVSD was defined as a left ventricular ejection fraction (LVEF) less than 40%, calculated using ECO. The prevalence of LVSD was 1.13% in the studied population (29 individuals). Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (PLR), negative likelihood ratio (NLR), and diagnostic odds ratio (DOR) were calculated for the algorithm and MEA. The area under the ROC curve (AUC-ROC) was also calculated for the algorithm. RESULTS: The study population had a median age of 62 years, with 47.2% being male. The AUC-ROC for the algorithm to predict HF was 0.947 (95% CI 0.913 ­ 0.981). Sensitivity, specificity, PPV, NPV, PLR, NLR, and DOR for the algorithm were 0.690, 0.976, 0.244, 0.996, 27.6, 0.32, and 88.74, respectively. For MEA, it was 0.172, 0.837, 0.012, 0.989, 1.09, 0.990, and 1.07, respectively. CONCLUSIONS: AI applied to the ECG is a promising tool for identifying patients with a higher likelihood of HF who should be prioritized for ECO. This could improve the diagnosis capacity of HF in our setting and thus enable early treatment initiation, with possible impact on reducing morbidity and mortality.

INTRODUCTION AND OBJECTIVES: Among patients with aortic stenosis (AS), interstitial fibrosis has been associated with progression to heart failure and is a marker of poorer prognosis. We aimed to assess the impact of myocardial fibrosis on clinical events after aortic valve replacement (AVR) in low risk, severe AS. METHODS: We prospectively followed 56 severe AS patients with ejection fraction >40%, who underwent AVR with simultaneous myocardial biopsies and collagen volume fraction (CVF) determination. Baseline and follow-up echocardiographic parameters were assessed. Outcomes were all-cause death and the combined endpoint of all-cause death or non-fatal cardiovascular hospitalization. RESULTS: Patients were predominantly women (67.9%) and mean age was 66±12 years. At follow-up, there was a significant decrease in transaortic gradients and wall stress, as well as regression in indexed LV mass. Patients who suffered a fatal event or the combined endpoint had a higher degree of fibrosis (27.1±20.7% vs. 15.4±11.8%, p=0.035; 24.0±18.2% vs. 15.3±12.0%, p=0.038, respectively). Patients with CVF≥15.4% had higher rates of all-cause death (37.5% vs. 97.0%, p=0.001) and lower survival free of the combined endpoint of all-cause death or non-fatal cardiovascular hospitalization (0% vs. 91.2%, p<0.001). CVF was the only independent predictor of all-cause death (hazard ratio (HR) 1.88; 95% confidence interval (CI): 1.08-3.29 for each 10% increase; p=0.026) and all-cause death or cardiovascular hospitalization (HR 1.73; 95% CI: 1.03-2.911 for each 10% increase; p=0.038). CONCLUSIONS: In low risk AS patients, higher levels of fibrosis are independent predictors of all-cause death and the composite of all-cause death or non-fatal cardiovascular hospitalization. Further advances in anti-fibrotic therapies in AS are needed.

Hypertrophic cardiomyopathy (HCM) is one of the most common inherited cardiac diseases, defined as a left ventricular wall thickness of ≥15 mm, in the absence of other causes of abnormal ventricular loading. A major hallmark of this disease is the presence of left ventricular outflow tract obstruction, which develops in up to three quarters of patients, referred to as obstructive hypertrophic cardiomyopathy. Current treatment is offered to symptomatic patients, based on the presence of documented left ventricular obstruction, aimed at reducing symptoms and disease progression. This is achieved through pharmacological empirical therapy, surgery, alcohol ablation and/or pacing. Mavacamten is a first-in-class allosteric inhibitor of cardiac myosin that promises to provide clinicians with targeted therapy for these patients. The aim of this review is to provide a general overview of the modern approach to the diagnosis and management of HCM, as well as to integrate all the current knowledge on mavacamten, in anticipation of a future change in the treatment algorithm of patients with HCM.

Resumo Fundamento: A hipertrofia ventricular esquerda (HVE) é um importante fator de risco cardiovascular, independente da hipertensão arterial. Apesar da evolução dos exames de imagem, o eletrocardiograma (ECG) ainda é o mais utilizado na avaliação inicial, porém, com baixa sensibilidade. Objetivo: Avaliar o desempenho dos principais critérios eletrocardiográficos para HVE em indivíduos hipertensos idosos e muito idosos. Métodos: Em coorte de hipertensos foram realizados ECGs e EcoDopplercardiogramas (ECO), e separados em três grupos etários: <60 anos, Grupo I; 60-79 anos Grupo II; e ≥80 anos, Grupo III. Os critérios eletrocardiográficos mais utilizados foram aplicados para o diagnóstico da HVE: Perúgia; Peguero-Lo Presti; Gubner-Ungerleider; Narita; (Rm+Sm) x duração; Cornell voltagem; Cornell voltagem duração; Sokolow-Lyon voltagem; R de aVL ≥11 mm; RaVL duração. Na avaliação do desempenho desses critérios, além da sensibilidade (Sen) e especificidade (Esp), foram analisadas as "Odds Ratios diagnóstico" (DOR). Consideramos p-valor <0,05 para as análises, com testes bi-caudais. Resultados: Em 2.458 pacientes, a HVE estava presente pelo ECO em 781 (31,7%). Nos Grupos I e II, os melhores desempenhos foram para os critérios de Narita, Perúgia, (Rm+Sm) x duração, sem diferenças estatísticas entre eles. No Grupo III (muito idosos) os critérios de Perúgia e (Rm+Sm) x duração tiveram os melhores desempenhos: Perúgia [44,7/89,3; (Sen/Esp)] e (Rm+Sm) duração [39,4%/91,3%; (Sen/Esp), p<0,05)], com os melhores resultados de DOR:6,8. Isto sugere que nessa população de muito idosos esses critérios têm maior poder discriminatório para separar pacientes com HVE. Conclusão: Nos hipertensos muito idosos os critérios eletrocardiográficos de Perúgia e (Rm+Sm) x duração apresentaram os melhores desempenhos diagnósticos para HVE.


Abstract Background: Left ventricular hypertrophy (LVH) is an important cardiovascular risk factor, regardless of arterial hypertension. Despite the evolution of imaging tests, the electrocardiogram (ECG) is still the most used in the initial evaluation, however, with low sensitivity. Objective: To evaluate the performance of the main electrocardiographic criteria for LVH in elderly and very elderly hypertensive individuals. Methods: In a cohort of hypertensive patients, ECGs and doppler echocardiographies (ECHO) were performed and separated into three age groups: <60 years, Group I; 60-79 years Group II; and ≥80 years, Group III. The most used electrocardiographic criteria were applied for the diagnosis of LVH: Perugia; Pegaro-Lo Presti; Gubner-Ungerleider; Narita; (Rm+Sm) x duration; Cornell voltage; Cornell voltage duration; Sokolow-Lyon voltage; R of aVL ≥11 mm; RaVL duration. In evaluating the performance of these criteria, in addition to sensitivity (Sen) and specificity (Esp), the "Diagnostic Odds Ratios" (DOR) were analyzed. We considered p-value <0.05 for the analyses, with two-tailed tests. Results: In 2,458 patients, LVH was present by ECHO in 781 (31.7%). In Groups I and II, the best performances were for the criteria of Narita, Perugia, (Rm+Sm) x duration, with no statistical differences between them. In Group III (very elderly) the Perugia criteria and (Rm+Sm) x duration had the best performances: Perugia [44,7/89.3; (Sen/Esp)] and (Rm+Sm) duration [39.4%/91.3%; (Sen/Esp), p<0.05)], with the best PAIN results:6.8. This suggests that in this very elderly population, these criteria have greater discriminatory power to separate patients with LVH. Conclusion: In very elderly hypertensive patients, the Perugia electrocardiographic criteria and (Rm+Sm) x duration showed the best diagnostic performance for LVH.

INTRODUCTION AND AIMS: Catheter ablation has been shown to improve left ventricular (LV) ejection fraction (LVEF) in patients with atrial fibrillation (AF) and heart failure (HF). Our aim was to assess the impact of AF ablation on the outcome of patients with HF and LV systolic dysfunction. METHODS: We performed a retrospective observational cohort study of all patients with HF and LVEF <50% and with no apparent cause for systolic dysfunction other than AF who underwent catheter ablation in a tertiary referral center between July 2016 and November 2018. The primary endpoint was a ≥5% improvement in LVEF. Secondary endpoints included improvement in New York Heart Association (NYHA) class and reduction in LV end-diastolic diameter (LVEDD) and left atrial diameter (LAD). RESULTS: Of 153 patients who underwent AF ablation in this period, 22 (77% male, median age 61 [IQR 54-64] years) fulfilled the inclusion criteria. Median follow-up was 11.1 months (IQR 6.1-19.0). After ablation, median LVEF increased from 40% (IQR 33-41) to 58% (IQR 55-62) (p<0.01), mean NYHA class improved from 2.35±0.49 to 1.3±0.47 (p<0.001), and median LAD and LVEDD decreased from 48.0 (IQR 43.5-51.5) mm to 44 (IQR 40-49) mm (p<0.01) and from 61.0 (IQR 54.0-64.8) mm to 55.0 (52.2-58.0) mm (p<0.01), respectively. CONCLUSION: In patients with HF and LV systolic dysfunction, AF ablation is associated not only with improved functional status but also with favorable structural remodeling, including improvement in LVEF and decreases in LAD and LVEDD.

INTRODUCTION AND OBJECTIVES: In severe aortic stenosis (AS), the impact of aortic valve replacement (AVR) on left ventricular (LV) systolic function assessed by strain and measured by echocardiography or cardiac magnetic resonance (CMR) has been controversial. We aimed to investigate LV systolic myocardial function changes six months after AVR using global longitudinal (GLS), circumferential (GCS) and radial (GRS) strain derived from CMR imaging. METHODS: We included 39 severe AS patients (69.3±7.8 years; 61.5% male) with preserved LV ejection fraction (LVEF) who were recruited as part of the EPICHEART study and underwent successful AVR (aortic valvular area: 0.8 cm2 (IQR: 0.2) pre- to 1.8 cm2 (IQR:0.5) post-AVR). Structural and functional parameters were assessed at baseline and six months after AVR, including LV GRS, GCS and GLS analysis by CMR, using cine short-axial and two-, three-, and four-chamber long-axial view. Comparison between baseline and postoperative LV remodeling was performed using Student t-test and Wilcoxon test. RESULTS: At six-month follow-up, LV mass, end-diastolic and end-systolic volumes, stroke volume, cardiac output, lateral E/e', tricuspid annular plane systolic excursion, right ventricular (RV) S wave velocity, GLS [-15.6% (IQR: 4.39) to -13.7% (IQR: 4.62)] and GCS [-17.8±3.58% to -16.1±2.94%] reduced significantly, while LVEF and GRS remained unchanged and lateral e' velocity increased. CONCLUSIONS: Despite favorable reverse LV structural and diastolic functional remodeling six months following AVR, GLS and GCS assessed by CMR reduced compared to baseline, LVEF remained unchanged. The clinical utility and timing of assessment of postoperative strain changes as a marker of systolic function progression needs further research.

O lúpus eritematoso sistêmico (LES) é doença autoimune crônica. Uma das suas importantes complicações é o dano cardiovascular. O objetivo deste trabalho foi estudar a prevalência de disfunção cardíaca em uma população local com LES e verificar se o grau de disfunção está associado com o a atividade da doença medida pelo SLEDAI (Systemic Lupus erythematosus activity index). Foram selecionados 19 pacientes assintomáticos do ponto de vista cardiovascular com diagnóstico de LES e que preenchiam os critérios classificatórios. A atividade de doença foi medida pelo SLEDAI e a função cardíaca por ecocardiografia transtorácica. Em conclusão, não foi possível identificar disfunção cardíaca na amostra estudada. Pontuações mais altas no índice de atividade do lúpus demonstraram correlação com aumento de massa de ventrículo esquerdo e fração de ejeção.


Systemic lupus erythesis (SLE) is a chronic autoimmune disease. One of the important complications of this disease is the cardiovascular damage The objective of this work was to study the prevalence of cardiac dysfunction in a local population with SLE and to verify whether the degree of dysfunction is associated with the disease activity measured by SLEDAI (Systemic Lupus erythematosus activity index). Were selected 19 patients asymptomatic from cardiovascular point of view diagnosed with SLE and who met the classification criteria for SLE. Disease activity was measured by SLEDAI and cardiac function was evaluated by transthoracic echocardiography. In conclusion, cardiac dysfunction could not be identified in the sample studied. Higher scores in the lupus activity index showed a correlation with increased left ventricle mass and ejection fraction.

INTRODUCTION: Acromegaly is a relatively rare chronic hormonal disease resulting in disfigurement. In 90% of cases, acromegaly is caused by a benign pituitary monoclonal human growth hormone-secreting tumor. The aim of the present study was to determine the presence of left ventricular (LV) deformation abnormalities using three-dimensional speckle-tracking echocardiography in a group of acromegalic patients. METHODS: Thirty-eight acromegalic patients were involved in the study. Thirteen patients were excluded due to inadequate image quality. The mean age of the remaining patients was 57.2±13.6 years and seven were male. Their data were compared to an age- and gender-matched control population, which consisted of 34 healthy volunteers (mean age: 52.7±4.9 years, 15 male). RESULTS: Global and mean segmental LV radial strain (RS) (33.2±13.4% vs. 25.2±10.8%, p=0.01 and 36.0±12.1% vs. 28.2±10.0%, p=0.009, respectively) proved to be significantly higher in acromegaly compared to controls. Active acromegalic patients had significantly higher global and mean segmental LV-RS (35.5±14.4% vs. 25.2±10.8%, p=0.03 and 37.9±13.3% vs. 28.2±10.0%, p=0.03, respectively) compared to controls. Between the active and inactive acromegaly groups, only basal LV circumferential strain (-30.2±4.8% vs. -26.7±4.1%, p=0.02) was found to be significantly different. CONCLUSION: The presented clinical, demographic, therapeutic and echocardiographic features demonstrate that active acromegaly is associated with enhanced LV RS as compared to healthy controls and those with inactive acromegaly.

Introducción. En el complejo universo de la falla cardíaca, el pronóstico y el tratamiento de la misma sumaron un aspecto de valor durante la última década del siglo pasado, a partir del reconocimiento de la disincronía mecánica durante la sístole y las consiguientes propuestas de resincronización cardíaca. Desde los inicios la predicción de disincronía mecánica se ha fundamentado en la duración del complejo QRS, las imágenes y más recientemente el análisis del índice de varianza del QRS. Objetivo. Tomando como estándar la dispersión mecánica ventricular izquierda medida por ecocardiografía, valorar la concordancia entre duración promedio del QRS del electrocardiograma (ECG) versus el índice de varianza, en cuanto a sensibilidad, especificidad y valor predictivo de dispersión anormal. Métodos. Población de pacientes consecutivos citados para ecocardiografía strain, sin escara ventricular, para medir dispersión mecánica, duración promedio del QRS del ECG e índice de varianza espacial. Resultados. Se evaluaron 54 pacientes y 57 escenarios distintos. Edad promedio: 66,3±14,2 años; duración promedio del QRS fue de 119,7±31,5 mseg. El índice de varianza promedio fue 0,52±0,35. Un índice de varianza >0,4 demostró sensibilidad y especificidad de 82,6% y 64,7% respectivamente, para diagnóstico de dispersión mecánica anormal (e"56 mseg); valor predictivo positivo: 61,3%, negativo: 84,6%. La duración promedio de QRS e"130 mseg logró sensibilidad y especificidad de 52,6% y 71%, con valores predictivos positivo y negativo de 54,5% y 68,6%, respectivamente. La duración promedio del QRS no guarda dependencia con la disincronía mecánica (p=0,1458); la disincronía mecánica (>56 mseg) y la electrocardiografía de varianza, por otra parte, sí guardan dependencia muy significativa (p=0,0012). Los métodos no son comparables para diagnosticar disincronía (kappa=0,376). Conclusión. El ECG de varianza tiene mejor sensibilidad y valor predictivo negativo para el diagnóstico de dispersión mecánica ventricular izquierda valorada por ecocardiografía.


Introduction. The prediction of mechanical dyssynchrony has generally been based on the duration of the QRS complex, although other resources are available, such as the QRS variance index; these methods have not been compared with each other. Objective. Taking as a standard the left ventricular mechanical dispersion measured by speckle-tracking strain echocardiography, to assess the concordance between the average QRS duration of the standard electrocardiogram versus the spatial variance index in terms of sensitivity, specificity and predictive value of abnormal mechanical dispersion. Methods. Population of consecutive patients cited for strain echocardiography and without ventricular scar tissue, to measure mechanical dispersion, average QRS duration of the standard electrocardiogram and the variance index. Results. Fifty-four patients and 57 different scenarios were evaluated; the average age was 66.3±14.2 years, the QRS average duration on standard electrocardiogram was 119.7±31.5 ms. Average ID was 0.52±0.35. A spatial variation index >0.4 showed sensitivity and specificity of 82.6% and 64.7% respectively, for the diagnosis of abnormal mechanical dispersion (e"56 ms); positive predictive value: 61.3%, negative: 84.6%. The average duration of QRS e"130 ms achieved sensitivity and specificity of 52.6% and 71%, and the positive and negative predictive values were 54.5 and 68.6%, respectively. The average QRS duration are not interdependent with mechanical dyssynchrony (p=0.1458); mechanical dyssynchrony (>56 ms) and spatial variance electrocardiography are, nevertheless, clearly interdependent (p=0.0012). The methods are not comparable between them (kappa=0.376). Conclusion. The QRS variation electrocardiogram has better sensitivity and negative predictive value respect to the average QRS duration for the diagnosis of left ventricular mechanical dispersion prevalence, measured by echocardiography.

No complexo universo da insuficiência cardíaca, seu prognóstico e tratamento agregaram valor durante a última década do século passado, com base no reconhecimento da dissincronia mecânica durante a sístole e nas propostas subsequentes de ressincronização cardíaca. Desde o início, a previsão da dissincronia mecânica tem sido baseada na duração do complexo QRS, nas imagens e, mais recentemente, na análise do índice de variância QRS. Objetivo. Tomando como padrão a dispersão mecânica do ventrículo esquerdo medida pelo ecocardiograma, avalie a concordância entre a duração média do QRS do eletrocardiograma (ECG) versus o índice de variância, em termos de sensibilidade, especificidade e valor preditivo de dispersão anormal. Métodos População de pacientes consecutivos citados para ecocardiograma de deformação, sem escara ventricular, para medir dispersão mecânica, duração média do QRS do ECG e índice de variância espacial. Resultados. Foram avaliados 54 pacientes e 57 cenários diferentes. Idade média: 66,3±14,2 anos; a duração média do QRS foi de 119,7±31,5 mseg. O índice de variância médio foi de 0,52±0,35. Um índice de variância >0,4 mostrou sensibilidade e especificidade de 82,6% e 64,7%, respectivamente, para diagnóstico de dispersão mecânica anormal (e"56 mseg); valor preditivo positivo: 61,3%, negativo: 84,6%. A duração média do QRS e"130 mseg alcançou sensibilidade e especificidade de 52,6% e 71%, com valores preditivos positivos e negativos de 54,5% e 68,6%, respectivamente. A duração média do QRS não depende de dissincronia mecânica (p=0,1458); dissincronia mecânica (>56 mseg) e eletrocardiografia de variância, por outro lado, têm dependência muito significativa (p=0,0012). Os métodos não são comparáveis para diagnosticar dissincronia (kappa=0,376). Conclusão. A variância do ECG apresenta melhor sensibilidade e valor preditivo negativo para o diagnóstico de dispersão mecânica do ventrículo esquerdo avaliada pelo ecocardiograma.

INTRODUCTION: In obstructive hypertrophic cardiomyopathy (HCM), alcohol septal ablation (ASA) can lead to gradient reduction and symptom improvement. We aimed to assess the efficacy and safety of ASA in a long-term outcome study. METHODS: We analyzed patients who underwent ASA over a seven-year period in a tertiary center. The primary echocardiographic endpoint was >50% reduction in left ventricular outflow tract (LVOT) gradient within a year of the procedure. The primary clinical endpoints were improvement in functional capacity and a combined endpoint of cardiac death and rehospitalization for cardiac cause. The follow-up period was 4.17±2.13 years. RESULTS: A total of 80 patients, mean age 63.9±12.3 years, 30.0% male, were analyzed. Baseline LVOT gradient was 96.3±34.6 mmHg and interventricular septal thickness was 21.6±3.1 mm. Minor complications were observed in 6.3% and major complications in 2.5%, and 8.8% received a permanent pacemaker. The primary echocardiographic endpoint was achieved by 85.7%. At three-month follow-up, LVOT gradient was 25.8±26.0 mmHg in the successful procedure group, compared to 69.2±35.6 mmHg in the other patients (p=0.001). At six months, LVOT gradient was 27.1±27.4 vs. 58.2±16.6 mmHg (p=0.024). Among 74 patients in NYHA class III/IV before the procedure, 57 (77%) improved to NHYA class I/II. The combined primary clinical endpoint (cardiac death and rehospitalization for cardiac cause) was observed in 27.5% (n=22). In the unsuccessful group, the combined endpoint was observed in 54.5%, compared to only 22.7% in the successful group. Only two patients died of cardiac causes. CONCLUSION: ASA is a safe procedure with a high success rate. Patients who achieved significant reductions in LVOT gradient suffered less cardiac death and rehospitalization for cardiac cause.

Abstract Background: Myocardial performance index (MPI), demonstrates both systolic and diastolic functions of the left ventricle. Presystolic wave (PSW) is frequently detected on Doppler examination of the left ventricular outflow tract and possible mechanism of PSW is impaired LV compliance and left ventricular stiffness. Objective: To investigate the relationship between PSW and MPI in type 2 diabetic patients. Method: A total of 129 type 2 diabetic patients were included in this study. Patients were divided into two groups according to the presence of PSW on Doppler echocardiography. There were 90 patients (38 male, mean age 57.77 ± 10.91 years) in the PSW-positive group and 39 patients (13 male; mean age: 55.31 ± 11.29 years) in the PSW-negative group. The p values of < 0.05 were considered statistically significant. Results: MPI was higher in PSW- positive group (0.63 ± 0.17vs 0.52 ± 0.13, p < 0.001). In addition, subclinical left ventricle dysfunction (LVD) was higher in the PSW- positive group (p = 0.029). Univariate analysis showed that the presence of PSW associated with abnormal MPI (p = 0.031). Pearson correlation analysis showed that PSW velocity correlated with MPI (r: 0.286, p = 0.006). Conclusion: Presence of the PSW on Doppler examination was associated with subclinical LV dysfunction in patients with DM type 2. This easy-to-perform echocardiographic parameter may be related to subclinical LVD among patients with type 2 DM.


Resumo Fundamento: O índice de performance miocárdica (IPM) avalia as funções sistólica e diastólica do ventrículo esquerdo. A onda pressistólica (OPS) é geralmente detectada no exame Doppler da via de saída do ventrículo esquerdo e seus possíveis mecanismos são complacência prejudicada e rigidez do ventrículo esquerdo. Objetivo: Investigar a relação entre OPS e IPM em pacientes com diabetes tipo 2. Método: 129 pacientes com diabetes tipo 2 foram incluídos no estudo. Os sujeitos foram alocados em dois grupos, com base na presença de OPS no exame ecocardiográfico com Doppler. Foram incluídos 90 pacientes (38 homens, idade média 57,77 ± 10,91 anos) no grupo OPS-positiva e 39 pacientes (13 homens; idade média 55,31 ± 11,29 anos) no grupo OPS-negativa. Valor de p < 0,05 foi considerado para significância estatística. Resultados: O IPM foi mais alto no grupo OPS-positiva (0,63 ± 0,17 vs 0,52 ± 0,13, p < 0,001). Conclusão: Presença de OPS na ecocardiografia com Doppler foi associada à DVE subclínica em pacientes com diabetes tipo 2. Esse exame ecocardiográfico de fácil execução pode ser relacionado à DVE subclínica entre pacientes com diabetes tipo 2.

Abstract Background: Unstable angina (UA) is a common cause of hospital admission; risk stratification helps determine strategies for treatment. Objective: To determine the applicability of two-dimensional longitudinal strain (SL2D) for the identification of myocardial ischemia in patients with UA. Methods: Cross-sectional, descriptive, observational study lasting 60 days. The sample consisted of 78 patients, of which fifteen (19.2%) were eligible for longitudinal strain analysis. The value of p < 0.05 was considered significant. Results: The group of ineligible patients presented: a lower proportion of women, a higher prevalence of diabetes mellitus (DM), use of ASA, statins and beta-blockers and larger cavity diameters. The main causes of non-applicability were: presence of previous infarction (56.4%), previous CTA (22.1%), previous MRI (11.5%) or both (16.7%) and the presence of specific electrocardiographic abnormalities (12.8%). SL2D assessment revealed a lower global strain value in those with stenosis greater than 70% in some epicardial coronary arteries (17.1 [3.1] versus 20.2 [6.7], with p = 0.014). Segmental strain assessment showed an association between severe CX and RD lesions with longitudinal strain reduction of lateral and inferior walls basal segments; (14 [5] versus 21 [10], with p = 0.04) and (12.5 [6] versus 19 [8], respectively). Conclusion: There was very low SL2D applicability to assess ischemia in the studied population. However, the global strain showed a correlation with the presence of significant coronary lesion, which could be included in the UA diagnostic arsenal in the future.


Resumo Fundamento: A angina instável (AI) é uma causa comum de internação hospitalar, a estratificação de risco ajuda a determinar estratégias para o tratamento. Objetivo: Determinar a aplicabilidade do strain longitudinal bidimensional (SL2D) para identificação de isquemia miocárdica, em pacientes com AI. Métodos: Estudo observacional transversal, descritivo, com duração de 60 dias. A amostra foi composta por 78 pacientes, sendo quinze (19,2%) elegíveis para análise do strain longitudinal. O valor de p < 0.05 foi considerado significativo. Resultados: O grupo dos não elegíveis apresentou: menor proporção de mulheres, maior prevalência de diabetes mellitus (DM), do uso de AAS, estatinas e betabloqueadores e maiores diâmetros cavitários. As principais causas da não aplicabilidade foram: presença de infarto prévio (56,4%), ATC prévia (22,1%), RM prévia (11,5%) ou ambos (16,7%) e presença de alterações eletrocardiográficas específicas (12,8%). A avaliação do SL2D revelou um valor de strain global inferior naqueles com estenose maior que 70% em alguma coronária epicárdica (17,1 [3,1] versus 20,2 [6,7], com p = 0,014). A avaliação do strain segmentar demonstrou associação entre lesão grave nas coronárias CX e CD com redução do strain longitudinal dos segmentos basais das paredes lateral e inferior; (14 [5] versus 21 [10], com p = 0,04) e (12,5 [6] versus 19 [8], com p = 0,026), respectivamente. Conclusão: Houve aplicabilidade muito baixa do SL2D para avaliar isquemia na população estudada. Entretanto, o strain global apresentou correlação com presença de lesão coronária significativa, podendo, futuramente, ser incluído no arsenal diagnóstico da AI.

Summary Objective: The idea that different sports and physical training type results in different cardiac adaptations has been widely accepted. However, this remodelling process among different sport modalities is still not fully understood. Thus, the current study aims to investigate the heart morphology variation associated with a set of different modalities characterized by distinct models of preparation and different methods and demands of training and completion. Method: The sample comprises 42 basketball players, 73 roller hockey players, 28 judo athletes and 21 swimmers. Anthropometry was assessed by a single and experienced anthropometrist and the same technician performed the echocardiographic exams. Analysis of variance was used to study age, body size and echocardiograph parameters as well as different sport athlete's comparison. Results: Basketball players are taller (F=23.448; p<0.001; ES-r=0.553), heavier (F=6.702; p<0.001; ES-r=0.334) and have a greater body surface area (F=11.896; p<0.001; ES-r=0.427). Basketball and hockey players have larger left auricle diameters compared with judo athletes (F=3.865; p=0.011; ES-r=0.316). Interventricular end-diastolic septal thickness (F=7.287; p<0.001; ES-r=0.347) and left ventricular posterior wall thickness (F=8.038; p<0.001; ES-r=0.362) of the judokas are smaller compared to the mean values of other sports participants. In addition, relative left parietal ventricular wall thickness is lower among swimmers compared with judokas (F=4.127; p=0.008; ES-r=0.268). Conclusion: The major contributors to changes in heart morphology are for the most part associated with sport-specific training and competition and the specific dynamics and adaptive mechanisms imposed by each sport.


Resumo Objetivo: Os efeitos decorrentes da prática de diferentes modalidades desportivas resultam em padrões divergentes de adaptação cardíaca. A presente pesquisa procura estudar a variação da morfologia do coração associada a um conjunto de modalidades desportivas distintas quanto à natureza do esforço e aos modelos de preparação, incluindo metodologias de treino e sistemas de competição. Método: Foram estudados 42 basquetebolistas, 73 hoquistas, 28 judocas e 21 nadadores. A antropometria foi avaliada por um único e experiente antropometrista e os exames ecocardiográficos foram realizados pelo mesmo operador. Recorreu-se à análise da variância para estudar a variação associada a idade, medidas de tamanho corporal e parâmetros ecocardiográficos, bem como para a comparação entre os atletas de diferentes modalidades desportivas. Resultados: Os basquetebolistas são os atletas mais altos (F=23,448; p<0,001; ES-r=0,553), mais pesados (F=6,702; p<0,001; ES-r=0,334), com maior superfície corporal (F=11,896; p<0,001; ES-r=0,427) e, com os hoquistas, apresentam um diâmetro da aurícula esquerda superior ao dos judocas (F=3,865; p=0,011; ES-r=0,316). A espessura telediastólica do septo interventricular (F=7,287; p<0,001; ES-r=0,347) e da parede posterior do ventrículo esquerdo (F=8,038; p<0,001; ES-r=0,362) dos judocas é inferior à dos outros atletas, mesmo quando controlado para o tamanho corporal. Os nadadores apresentam uma espessura parietal relativa do ventrículo esquerdo superior à dos judocas (F=4,127; p=0,008; ES-r=0,268). Conclusão: As diferentes fontes de variação da morfologia cardíaca prendem-se com as dinâmicas do processo de treino, competição e correspondentemente com os mecanismos adaptativos, sobrepondo-se ao processo de formação desportiva a longo prazo.
