OBJECTIVE: To verify breathiness in the cisgender and transgender men and women's voices, compare values of acoustic and perceptual indicators of breathiness and fundamental frequency (f0) between groups, and compare them between the voices attributed as female and male. STUDY DESIGN: Cross sectional retrospective study. METHODS: The study was approved by the Research Ethics Committee (4,937,140). Sustained vowel /a/ and continuous speech recordings of 21 cisgender men (CISM), 31 transgender men (TM), 32 cisgender women (CISW), and 31 transgender women (TW) were analyzed. Three judges conducted a perceptive-auditory analysis regarding the degree breathiness, using a visual analog scale, and attributed gender (female or male). The ABI (Acoustic Breathiness Index) was extracted using the PRAAT software (6.1.16). The f0, Harmonic-Noise Ratio (HNR), Voice Turbulence Index (VTI), and Soft Phonation Index (SPI) were analyzed using the Multi-Dimensional Voice Program (KayPentax). RESULTS: The ABI value for CISM was lower than for TM and CISW. CISW had a higher f0 than; TM had a higher f0 than CISM; and TW had a higher f0 than CISM. The groups did not differ for HNR and VTI. Regarding the SPI, CISM had higher values than CISW. Regarding the auditory perception, TM presented more intense breathiness than CISM in the vowel. Regarding gender attribution by voice, the voices CISM and CISW were 100% identified as male and female. On the other hand, in the vowel analysis, 45.2% of the TM voices were perceived as female, and 59.4% of TW voices as male. CONCLUSION: Breathiness occurs differently between groups and the voices perceived as male and female. Even when TM is submitted to the use of testosterone and undergoes vocal changes, the transglottal airflow remains, which is a female characteristic of phonation.

ABSTRACT Purpose To assess the influence of the listener experience, measurement scales and the type of speech task on the auditory-perceptual evaluation of the overall severity (OS) of voice deviation and the predominant type of voice (rough, breathy or strain). Methods 22 listeners, divided into four groups participated in the study: speech-language pathologist specialized in voice (SLP-V), SLP non specialized in voice (SLP-NV), graduate students with auditory-perceptual analysis training (GS-T), and graduate students without auditory-perceptual analysis training (GS-U). The subjects rated the OS of voice deviation and the predominant type of voice of 44 voices by visual analog scale (VAS) and the numerical scale (score "G" from GRBAS), corresponding to six speech tasks such as sustained vowel /a/ and /ɛ/, sentences, number counting, running speech, and all five previous tasks together. Results Sentences obtained the best interrater reliability in each group, using both VAS and GRBAS. SLP-NV group demonstrated the best interrater reliability in OS judgment in different speech tasks using VAS or GRBAS. Sustained vowel (/a/ and /ɛ/) and running speech obtained the best interrater reliability among the groups of listeners in judging the predominant vocal quality. GS-T group got the best result of interrater reliability in judging the predominant vocal quality. Conclusion The time of experience in the auditory-perceptual judgment of the voice, the type of training to which they were submitted, and the type of speech task influence the reliability of the auditory-perceptual evaluation of vocal quality.

RESUMO Objetivo Comparar as medidas acústicas de Cepstral Peak Prominence Smoothed (CPPS) e Acoustic Voice Quality Index (AVQI) de crianças com vozes normais e alteradas, relacionar com o julgamento perceptivo-auditivo (JPA) da voz e estabelecer pontos de corte. Método Gravações vocais das tarefas de vogal sustentada e contagem de números de 185 crianças foram selecionadas em um banco de dados e submetidas a análise acústica com extração das medidas de CPPS e AVQI, e ao JPA. O JPA foi realizado individualmente para cada tarefa e as amostras foram classificadas posteriormente como normal ou alterada, e para as tarefas em conjunto definindo-se se a criança passaria ou falharia em uma situação de triagem vocal. Resultados Crianças com JPA alterado e que falharam na triagem apresentaram valores menores de CPPS e maiores de AVQI, do que as com JPA normal e que passaram na triagem. O JPA da tarefa de vogal sustentada se relacionou ao CPPS e AVQI, e da tarefa de contagem de números relacionou-se apenas ao AVQI e CPPS números. Os pontos de corte que diferenciam crianças com e sem desvio vocal são 14,07 para o CPPS vogal, 7,62 para o CPPS números e 2,01 para o AVQI. Conclusão Crianças com JPA alterado apresentaram maiores valores de AVQI e menores valores de CPPs. O JPA da tarefa de vogal previu todas as medidas acústicas, porém, de contagem previu apenas as medidas extraídas dela. As três medidas foram semelhantes na identificação de vozes sem desvio e vozes disfônicas.

Conclusion Children with altered voices, have higher AVQI values and lower CPPS values, when detected in children with voices within the normal range. The acoustic measurements were related to the auditory perceptual judgment of vocal quality in the sustained vowel task, however, the number counting task was related only to the AVQI and CPPS. The cut-off points that differentiate children with and without vocal deviation are 14.07 for the CPPS vowel, 7.62 for the CPPS numbers and 2.01 for the AVQI. The three measures were similar in identifying voices without deviation and dysphonic voices.

Objective: To verify the relationship between smoking, age, schooling, and the vocal self-perception of trans women. Methods: Cross-sectional observational quantitative study conducted with 24 trans women over 18 years old, living in their affirmed gender for a minimum of 6 months. Data collection involved selected questions from the translated and authorized Portuguese version of the Trans Woman Voice Questionnaire (TWVQ) and information regarding age, education, occupation, and smoking status. All variables were analyzed descriptively, and the association with smoking was assessed using Pearson's Chi-square, Fisher's Exact, Student's T, and Mann-Whitney tests, with a significance level of 5%. Results: The mean age of trans women was 28.2 +/- 6.5 years (range: 21 - 48). Most participants (41.6%) had completed their high school education and pursued diverse careers. Regarding smoking habits, 58.3% of women were either current smokers or had smoked at least 100 cigarettes in their lifetime. A statistically significant association was found between smoking and age (p = 0.001), with smokers having a lower average age (24.9 years) compared to nonsmokers (32.9 years). However, no statistical significance was observed between smoking, education, and self-perception of vocal femininity. Only 9 (37.5%) trans women perceived their voices as feminine, while 17 (70.7%) desired a more feminine-sounding voice. Limitation: The limited sample size in this study may have constrained the ability to detect significant differences between the analyzed groups using statistical tests. Conclusion: The smoking prevalence was notably high among younger trans women. However, there was no statistically significant difference in vocal self-perception between those who smoked and those who did not. The majority of trans women expressed a desire for their voices to sound more feminine, particularly among smokers.
Objetivo: Verificar la relación entre el tabaquismo, edad, escolaridad y la autopercepción vocal de mujeres trans. Métodos: Estudio observacional transversal cuantitativo realizado con 24 mujeres trans mayores de 18 años, que viven en su género afirmado durante 6 meses. Para la recolección de datos se utilizaron preguntas seleccionadas de la versión portuguesa traducida y autorizada del Trans Woman Voice Questionnaire (TWVQ) y datos sobre edad, educación, ocupación y tabaquismo. Todas las variables se analizaron descriptivamente y la asociación con el tabaquismo se realizó mediante las pruebas Chi-cuadrado de Pearson, Exacta de Fisher, T de Student y Mann Whitney, con un nivel de significación del 5%. Resultados: La edad promedio de las mujeres trans fue de 28,2 +/- 6,5 (rango 21 - 48). La mayoría de las participantes (41,6%) había terminado la escuela secundaria con carreras muy diversas. Respecto al tabaquismo, el 58,3% de las mujeres fuman actualmente o han fumado al menos 100 cigarrillos en su vida. Hubo una asociación estadísticamente significativa entre el tabaquismo y la edad (p = 0,001), en la que la edad promedio entre los fumadores (24,9 años) fue menor que la de los no fumadores (32,9 años). No hubo significación estadística entre el tabaquismo, la educación y la autopercepción vocal. Solo 9 (37,5%) mujeres trans actualmente consideran su voz femenina y 17 (70,7%) dijeron que la voz ideal podría sonar más femenina. Limitación: La pequeña casuística puede haber limitado la identificación de diferencias entre los grupos analizados a través de pruebas estadísticas. Conclusión: El tabaquismo fue alto entre las mujeres trans, especialmente las más jóvenes. La autopercepción vocal no fue estadísticamente diferente entre los grupos de fumadores y no fumadores. La mayoría de las mujeres trans dijeron que sus voces podrían sonar más femeninas, especialmente las fumadoras.

OBJECTIVE: To propose a Dysphonia Risk Screening Protocol for Actors (DRSP-A), test its usability in conjunction with the General Dysphonia Risk Screening Protocol (G-DRSP), determine the cut-off point for a high risk of dysphonia in actors, and compare the risk of dysphonia between actors with and without voice disorders. METHOD: Observational cross-sectional study with 77 professional actors or students. The questionnaires were applied individually and the total scores were summed to calculate the final score of the Dysphonia Risk Screening (DRS-Final). The validity of the questionnaire was verified from the area under the Receiver Operating Characteristic (ROC) curve, and cut-offs were obtained based on diagnostic criteria for screening procedures. Voice recordings were collected for auditory-perceptual analysis and subsequent division into groups with and without vocal alteration. RESULTS: The sample showed a high risk of dysphonia. Higher scores in the G-DRSP and in the DRS-Final were found in the group that presented vocal alteration. The cut-off points established for the DRSP-A and DRS-Final were 0.623 and 0.789, respectively, with higher degrees of sensitivity than specificity. Thus, above these values, the risk of dysphonia is greater. CONCLUSION: A cut-off value was calculated for the DRSP-A. This instrument was proven to be viable and applicable. The group with vocal alteration had a higher score in the G-DRSP and DRS-Final, but there was no difference in the DRSP-A.

RESUMO Objetivo descrever quais são as habilidades auditivas do processamento auditivo central mais frequentes, relatadas por um grupo de especialistas para a realização do exame de Comparação de Locutor, tradicionalmente realizado por peritos forenses. Métodos estudo prospectivo, descritivo, com análise quantitativa e qualitativa. Os dados foram obtidos por meio de um consenso de especialistas. Participaram da reunião cinco fonoaudiólogos, sendo dois especialistas em audiologia, dois especialistas em voz e uma fonoaudióloga perita. A reunião foi realizada de forma virtual e síncrona, com duração de uma hora e 30 minutos. As tarefas realizadas durante o exame de Comparação de Locutor foram consideradas a partir de um protocolo disponível na literatura. As especialistas em fonoaudiologia receberam explicações a respeito de cada uma das tarefas e foram solicitadas a discutir sobre quais as habilidades do processamento auditivo central estariam envolvidas na execução de cada uma delas. Resultados sete habilidades foram consideradas na reunião dos especialistas como imprescindíveis para as tarefas realizadas no exame de Comparação de Locutor. A ordenação temporal foi a habilidade mais citada, podendo estar presente em seis tarefas, e a tarefa de transcrição do material de fala foi mencionada como sendo a que necessita de mais habilidades do processamento auditivo central. ABSTRACT Purpose to describe which abilities of central auditory processing are more frequently related for the group of specialists to the performance of the speaker comparison test (CL), traditionally performed by forensic experts. Methods a prospective, descriptive study with quantitative and qualitative analysis and data were obtained through a consensus of experts. Five speech therapists participated in the meeting, two specialists in audiology (EA), two specialists in voice (VS), and an expert speech therapist (FP). The meeting was held virtually and synchronously, lasting 1 hour and 30 minutes. The tasks performed during the Speaker Comparison (LC) exam were considered from a protocol available in the literature. The AEs received explanations about each of the tasks and were asked to discuss which auditory processing skills (ACP) would be involved in the performance of each of them. Results seven PAC skills were considered in the experts' meeting as essential for the tasks performed in the CL exam. Temporal ordering was the most cited skill, being present in six tasks, and the speech material transcription task is the one that requires more skills from the PAC. Conclusion Seven PAC skills were considered in the experts' meeting as essential for the tasks performed in the CL exam. RESUMO Objetivo identificar um conjunto de requisitos para o desenvolvimento de um simulador de treinamento perceptivo-auditivo (TPA) a partir da experiência de docentes que realizam o TPA. Método Trata-se de um estudo transversal, descritivo, com abordagem quantitativa. Vinte e dois docentes responderam um questionário online contendo 31 itens relacionados ao TPA, envolvendo itens sobre o perfil profissional, condições para o TPA nos cursos de graduação e pós-graduação em Fonoaudiologia, estrutura do TPA, avaliação do efeito do TPA. Resultado observou-se que existe variação nos procedimentos de TPA realizados no Brasil. Os principais requisitos indicados pelos respondentes para o TPA envolvem o uso de vozes sintetizadas nos momentos iniciais, seguindo para vozes humanas posteriormente; a utilização de tarefas de fala com vogais sustentadas e fala encadeada; a inserção de informações complementares tais como o gênero, idade, profissão do falante e a espectrografia do sinal vocal; treinamento com tempo mínimo de seis horas; a avaliação do efeito do treinamento pela comparação da concordância intra e inter-juizes pré e pós treinamento; a adição dos parâmetros de grau geral de desvio vocal, rugosidade, soprosidade e tensão; a utilização de escalas contínuas e numéricas validadas; e ser realizado a partir do segundo ano de graduação. Conclusão embora haja uma variabilidade da resposta dos especialistas, foi identificado um conjunto mínimo de requisitos indicados para a realização de TPA com novos juízes.
ABSTRACT Purpose to identify a set of requirements for the development of an auditory-perceptual training simulator (APT) based on the experience of professors who provide APT. Methods This is a cross-sectional, descriptive study with a quantitative approach. Twenty-two professors answered an online questionnaire containing 31 items related to APT, involving items about the professional profile, conditions for APT in undergraduate and postgraduate courses in Speech Therapy, APT structure, and evaluation of the APT effect. Result it was observed that there is a variation in APT procedures performed in Brazil. The main requirements indicated by the respondents for the APT involve the use of synthesized voices in the initial moments, followed by human voices later; the use of speech tasks with sustained vowels and connected speech; the insertion of complementary information such as gender, age, the profession of the speaker and the spectrography of the vocal signal; training with a minimum time of six hours; the evaluation of the training effect by comparing intra- and inter-judge agreement before and after training; the addition of the parameters of general degree of vocal deviation, roughness, breathiness, and strain; the use of validated continuous and numerical scales; and offering it from the second year of the undergraduate program. Conclusion although there is variability in the response of experts, a minimum set of requirements indicated for performing APT with new judges was identified.

RESUMO Objetivo comparar as tarefas de emissão de vogal e de contagem de números na diferenciação perceptivo-auditiva de crianças com e sem lesão laríngea. Método Estudo observacional, analítico, transversal. Utilizou-se banco de dados de uma pesquisa de doutorado, com resultados de avaliações laringológicas e gravação de amostras vocais de 44 crianças que se dividiram em: Grupo sem lesão laríngea (GSLL), com 33 crianças; e grupo com lesão laríngea (GCLL), com 11 crianças. Para a avaliação perceptivo-auditiva, as amostras vocais foram separadas de acordo com o tipo de tarefa e analisadas separadamente por uma juíza, que analisou o grau geral do desvio vocal e informou se, diante de uma situação de triagem, a criança passaria ou falharia. Resultados Houve diferença entre os grupos GSLL e GCLL quanto ao grau geral do desvio vocal para tarefa de emissão de números, com predomínio de desvios discretos no GSLL e moderados no GCLL. Quanto à triagem, houve diferença entre os grupos para a tarefa de contagem, com mais falhas no GCLL. Os grupos foram semelhantes na tarefa de vogal, tanto no que se refere à intensidade do desvio quanto ao resultado da triagem. A maior parte das crianças do GCLL falhou em ambas as tarefas na situação de triagem vocal, com diferença em relação às crianças do GSLL que, em geral, falharam em apenas uma tarefa. Conclusão A tarefa de contagem de números contribui para a diferenciação auditiva de crianças com e sem lesão laríngea, por identificar desvios de maior intensidade em crianças com lesão.
ABSTRACT Purpose To compare the vowel emission and number counting tasks in perceptual-auditory differentiation among children with and without laryngeal lesions. Methods Observational, analytical, and cross-sectional methods were used. Medical records of 44 children were selected from a database of an otorhinolaryngology service at a University Hospital and they were divided into groups: without laryngeal lesion (WOLL), and with laryngeal lesion (WLL), with 33 and 11 children. For the auditory-perceptual evaluation, the vocal samples were separated according to the type of task. They were analyzed separately by a judge who analyzed the general degree of vocal deviation and assessed whether the child would pass or fail in the face of a screening situation. Results There was a difference between the WOLL and WLL groups in terms of the overall degree of vocal deviation for the task of number counting, with a predominance of mild deviations in WOLL and moderate in WLL. In the screening, there was a difference between the groups during the number counting task, with more failures in the WLL. The groups were similar in the sustained vowel task, both in terms of the overall degree of vocal deviation and the vocal screening. Most children in the WLL failed in both tasks during vocal screening compared to the children in the WOLL who, in general, failed in only one task. Conclusion The task of number counting contributes to the auditory differentiation in children with and without laryngeal lesion, by identifying deviations of greater intensity in children with laryngeal lesion.

OBJECTIVES: To verify whether there are differences in the judgments of listeners' attitudes as a function of the presence, severity, and type of vocal deviation, and to identify the predictive factors of listeners' attitudes toward dysphonic voices in Brazilian Portuguese speakers. METHODS: A sample of CAPE-V sentences was used from 44 subjects of both genders. The samples presented different severities of vocal deviation (general grade - GG) (healthy, mild, moderate, and severe) and different degrees of roughness (GR), breathiness (GB), and strain (GS), characterized by a perceptual-auditory judgment performed by speech therapists. The samples were presented to 152 listeners of both genders who performed the judgment of 12 attitudes inserted in a semantic differential scale previously validated for this study. Here, we used a Logistic Regression Model. RESULTS: There was no association between listener's gender and the judgment of attitudes. Subjects with vocal quality deviation were judged more negatively in relation to vocally healthy individuals. We observed that higher the GG, higher the negative judgment of listeners in relation to the speakers. Listeners judged breathy and strained voices as more negative. In the general model for female and male voices, the increase in GB and GG reduced the chance of a voice being evaluated positively by 16 and 20%, respectively. For female voices, GB and GS reduced the chance of a female voice being evaluated positively by 36 and 19%, respectively. GG reduces the chance of a male voice being evaluated positively by 46%, while GR increases the chance of a male voice being judged positively by 62 %. CONCLUSIONS: There is an association between the presence, severity, and type of vocal deviation and listeners' attitudes. We identified a predictive model of listeners' attitudes related to dysphonic voices among Brazilian Portuguese speakers. In general, GG and GB were predictors of negative attitudes toward listeners. GB and GS were predictors of negative attitudes toward female voices. The GG is a predictor of negative attitudes toward male voices, while the GR increases the chance of positive judgments.

OBJECTIVE: To investigate the auditory perception of roughness and breathiness by dysphonic women. METHODS: Twenty-two dysphonic native Brazilian Portuguese women participated in this research. All participants underwent audiological evaluation and laryngeal examination to confirm the diagnosis. During the tests, they recorded the sustained vowel /Æ/. A speech-language pathologist performed the auditory-perceptual judgment of voice quality for these vocal samples, categorizing the general degree of vocal deviation (mild, moderate, and severe degree) and the predominant type of deviation (roughness or breathiness). Thirty-two (32) stimuli were selected from a voice database, including twenty-four (24) dysphonic voice samples and eight (8) voice samples from vocally healthy women. The authors conducted five perception experiments, being three categorization tasks (normal vs. deviated, breathy vs. nonbreathy, rough vs. nonrough) and two tasks for discriminating the degree of deviation (roughness degree and breathiness degree). RESULTS: The experiments showed a difference between the answers for presence/absence of deviation, presence/absence of breathiness, and presence/absence of roughness in the stimuli, and a difference in the proportion of similar answers of dysphonic women (P < 0.001) regarding the identification of the deviation. Participants classified a large part of the deviated (57.9%), breathy (63.13%), and rough (65.31%) voices as normal. The degree of vocal deviation (P = 0.008) and the degree of roughness in the stimuli correlated positively with the proportion of similar answers of the participants. As for the discrimination of breathiness degrees, less deviated (normal and mild) voices were less discriminated, and more deviated (moderate and severe) voices were better discriminated. Regarding the discrimination of roughness degrees, only the voices with severe deviations showed good discrimination. CONCLUSION: Dysphonic women had a high rate of not similar answers in the identification of normal and deviated voices. They identified more than half of the deviated voices as normal. Samples with more severe deviations were proportionally more identified as deviated by the participants. The greater the vocal deviation of the participants' voices, the smallest the number of similar answers. Participants had a high rate of not similar answers in the identification of normal and breathy voices. Dysphonic women show less ability to perceive mildly and moderately breathy voices in the breathy category. Participants had a high rate of similar answers in the identification of normal and rough voices. Dysphonic women show less ability to perceive mildly and moderately breathy voices in the breathy category. Participants show less ability to perceive only mildly roughness voices with similar responses. Dysphonic women could discriminate between voices with adjacent degrees of roughness but had a low percentage of similar answers for discrimination between voices with adjacent degrees of breathiness.

INTRODUCTION: The Consensus Auditory Perceptual Evaluation of Voice (CAPE-V) scale is a modern, clinical-scientific approach to voice analysis. It has been translated and culturally adapted to Brazilian Portuguese, but it still lacks validation. OBJECTIVE: To validate the Brazilian Portuguese version of the CAPE-V scale using the previously translated and culturally adapted version. METHOD: Forty voice samples were selected (30 dysphonic, 10 nondysphonic), and the degree of vocal deviation was evaluated by a committee of three voice specialists. Nine voice specialists judged the 40 voice samples plus 20% repetition (total of 48 samples) using the CAPE-V. To ensure construct validity of the CAPE-V, its analysis was compared to the Grade-Roughness-Breathiness-Asthenia-Strain (GRBAS) scale that was performed 48-72 hours later. Finally, the intra- and inter-rater reliability values were verified and the correlation between the nine judges and the previously defined evaluation was analyzed. RESULTS: The Brazilian CAPE-V presented significant intra (0.860-0.997) and inter-rater reliability values (0.707-0.964) for the overall degree and strong correlation with GRBAS (above 0.828). Deviant voice quality had greater consensus among raters than normal voices. A strong correlation was observed between the analysis of the nine raters and that of the committee. CONCLUSION: CAPE-V is an important diagnostic instrument that contributes to the standardization of vocal quality evaluation in several languages, including Brazilian Portuguese. Thus, its usefulness is neither related to a single language nor to a single set of raters.

OBJECTIVES: The objective of the study was to establish a cut-off point for high dysphonia risk in children using the Child Dysphonia Risk Screening Protocol (DRSP-C). METHODS: Through a preliminary study, voice recordings of 59 children (4-12 years of age) were collected during an auditory-perceptual analysis using the Consensus Auditory-Perceptual Evaluation of Voice protocol. Thirty of the patients had voice disorders (patient group), and 29 did not (control group). A risk score for dysphonia was then calculated, and data were compared between groups. The relationship between overall degrees of deviation and questionnaire scores was analysed. The questionnaire's validity was verified from the area under the Receiver Operating Characteristic (ROC) curve, and cut-off points were obtained based on diagnostic criteria for screening procedures. RESULTS: The DRSP-C score was found to be higher for the patient group, as was the partial score for vocal behaviour. No correlation was found between overall degrees of dysphonia and questionnaire scores. The area under the ROC curve was measured as 0.678, denoting limited diagnostic capacity. The cut-off point was set at 16.50. Thus, above this value, dysphonia risk is higher. CONCLUSION: A cut-off point for high dysphonia risk was calculated. The DRSP-C proved to be a promising tool for children's clinical vocal and health promotion and should be used in conjunction with General Dysphonia Risk Screening.

PURPOSE: Investigate the vocal and laryngopharyngeal symptoms, vocal quality, Voice Activity and Participation Profile (VAPP), and vocal acoustic and auditory-perceptual characteristics from evangelical pastors comparing with the same aspects of nonprofessional voice men. METHOD: Sixty professionally active male subjects were divided into two groups: 30 pastors and 30 nonprofessional voice men, with ages between 24 and 59 years old. Participants answered the Screening Index for Voice Disorder (SIVD) questionnaire and the VAPP, as well as an auditory-perceptual and acoustic evaluation of the voice. RESULTS: The evangelical pastors reported problems with throat clearing, cough with secretion, secretion/phlegm in the throat, and strained speech. Although there were no differences in vocal characteristics between pastors and nonprofessional voice men, the score for effects on the job and the total score of the VAPP were higher for the evangelical pastors. CONCLUSION: Regarding the vocal characteristics, evangelical pastors do not present distinct vocal alterations in the auditory-perceptual and acoustic parameters analyzed when compared with nonprofessional voice men. However, they show laryngopharyngeal symptoms when speaking and perceive that some vocal alterations limit their professional performance but do not prevent the practice of this profession.

INTRODUCTION: The supracricoid partial laryngectomy allows the preservation of laryngeal functions with good local cancer control. OBJECTIVE: To assess laryngeal configuration and voice analysis data following the performance of a combination of two vocal exercises: the prolonged /b/vocal exercise combined with the vowel /e/ using chest and arm pushing with different durations among individuals who have undergone supracricoid laryngectomy. METHODS: Eleven patients undergoing partial laryngectomy supracricoid with cricohyoidoepiglottopexy (CHEP) were evaluated using voice recording. Four judges performed separately a perceptive-vocal analysis of hearing voices, with random samples. For the analysis of intrajudge reliability, repetitions of 70% of the voices were done. Intraclass correlation coefficient was used to analyze the reliability of the judges. For an analysis of each judge to the comparison between zero time (time point 0), after the first series of exercises (time point 1), after the second series (time point 2), after the third series (time point 3), after the fourth series (time point 4), and after the fifth and final series (time point 5), the Friedman test was used with a significance level of 5%. The data relative to the configuration of the larynx were subjected to a descriptive analysis. RESULTS: In the evaluation, were considered the judge results 1 which have greater reliability. There was an improvement in the general level of vocal, roughness, and breathiness deviations from time point 4 [T4]. CONCLUSION: The prolonged /b/vocal exercise, combined with the vowel /e/ using chest- and arm-pushing exercises, was associated with an improvement in the overall grade of vocal deviation, roughness, and breathiness starting at minute 4 among patients who had undergone supracricoid laryngectomy with CHEP reconstruction.

RESUMO Objetivo Investigar a acurácia das medidas acústicas, isoladas e combinadas, na discriminação da intensidade do desvio vocal (GG) e da qualidade vocal predominante (QVP) em pacientes disfônicos. Método Participaram 302 pacientes do gênero feminino, com queixa vocal. A partir da vogal /ɛ/ sustentada, foram extraídas as medidas acústicas de média e desvio padrão (DP) da frequência fundamental (F0), o jitter, o shimmer e o Glottal to noise excitation (GNE) e a média dos três primeiros formantes (F1, F2, F3). A avaliação perceptivo-auditiva do GG e QVP foi realizada por três fonoaudiólogos especialistas em voz. Resultados Isoladamente, apenas o GNE obteve desempenho satisfatório na discriminação do GG e da QVP. Houve uma melhora na classificação do GG e QVP com a combinação das medidas acústicas. A média de F0, F2 e GNE (saudável × desvio leve a moderado), DP de F0, F1 e F3 (leve a moderado × desvio moderado), Jitter e GNE (moderado × desvio intenso) foram as melhores combinações para discriminar o GG. As melhores combinações para discriminação da QVP foram média de F0, Shimmer e GNE (saudável × rugosa), F3 e GNE (saudável × soprosa), média de F0, F3 e GNE (rugosa × tensa), média de F0, F1 e GNE (soprosa × tensa). Conclusão De forma isolada, o GNE mostrou-se o único parâmetro acústico capaz de discriminar o GG e a QVP. Houve um ganho no desempenho da classificação com a combinação das medidas acústicas tradicionais e formânticas, tanto para a discriminação do GG como da QVP.
ABSTRACT Purpose Investigate the accuracy of isolated and combined acoustic measurements in the discrimination of voice deviation intensity (GD) and predominant voice quality (PVQ) in patients with dysphonia. Methods A total of 302 female patients with voice complaints participated in the study. The sustained /ɛ/ vowel was used to extract the following acoustic measures: mean and standard deviation (SD) of fundamental frequency (F0), jitter, shimmer, glottal to noise excitation (GNE) ratio and the mean of the first three formants (F1, F2, and F3). Auditory-perceptual evaluation of GD and PVQ was conducted by three speech-language pathologists who were voice specialists. Results In isolation, only GNE provided satisfactory performance when discriminating between GD and PVQ. Improvement in the classification of GD and PVQ was observed when the acoustic measures were combined. Mean F0, F2, and GNE (healthy × mild-to-moderate deviation), the SDs of F0, F1, and F3 (mild-to-moderate × moderate deviation), and mean jitter and GNE (moderate × intense deviation) were the best combinations for discriminating GD. The best combinations for discriminating PVQ were mean F0, shimmer, and GNE (healthy × rough), F3 and GNE (healthy × breathy), mean F 0, F3, and GNE (rough × tense), and mean F0 , F1, and GNE (breathy × tense). Conclusion In isolation, GNE proved to be the only acoustic parameter capable of discriminating between GG and PVQ. There was a gain in classification performance for discrimination of both GD and PVQ when traditional and formant acoustic measurements were combined.

BACKGROUND: Paracoccidioidomycosis (PCM) is a systemic mycosis of acute and chronic evolution, caused by species belonging to the genus Paracoccidioides. It is considered the most prevalent systemic endemic mycosis in Latin America, with cases in the tropical and subtropical regions. Residual PCM refers to the fibrotic scar sequelae resulting from the disease treatment which, when associated with collagen accumulation, leads to functional and anatomic alterations in the organs. AIMS: The aim of this study was to evaluate the vocal function of patients with residual PCM in upper airways and digestive tract. METHODS: We performed a cross-sectional study in 2010 in a cohort of 21 patients with residual PCM in upper airways and digestive tract. RESULTS: The average age was 49.48±9.1 years, and only two (9.5%) patients were female. The study was performed in the 1-113 month-period (median 27) after the end of drug treatment. Five (23.8%) patients had alterations in the larynx as a sequela of the disease. However, all patients had vocal changes in vocal auditory perceptual analysis by GRBASI scale. The computerized acoustic analysis using the software Vox Metria, showed that 11 patients (52.4%) presented alterations in jitter, 15 (71.4%) in shimmer, 8 (38.1%) in F0, 4 (19%) in glottal to noise excitation (GNE), 7 (33.3%) in the presence of noise and 12 (57.1%) in the presence of vibratory irregularity. CONCLUSIONS: The great frequency of alterations in residual PCM suggests that the patients in such phase could benefit from a multidisciplinary treatment, offering them integral monitoring of the disease, including speech rehabilitation after the PCM is healed.

OBJECTIVE: The study aimed to compare and correlate perceptual-auditory analysis of vocal parameters and self-perception in individuals with adductor spasmodic dysphonia before and after the application of botulinum toxin. STUDY DESIGN: This is a prospective cohort study. METHODS: Sixteen individuals with a diagnosis of adductor spasmodic dysphonia were submitted to the application of botulinum toxin in the thyroarytenoid muscle, to the recording of a voice signal, and to the Voice Handicap Index (VHI) questionnaire before the application and at two time points after application. Two judges performed a perceptual-auditory analysis of eight vocal parameters with the aid of the Praat software for the visualization of narrow band spectrography, pitch, and intensity contour. RESULTS: Comparison of the vocal parameters before toxin application and on the first return revealed a reduction of oscillation intensity (P = 0.002), voice breaks (P = 0.002), and vocal tremor (P = 0.002). The same parameters increased on the second return. The degree of severity, strained-strangled voice, roughness, breathiness, and asthenia was unchanged. The total score and the emotional domain score of the VHI were reduced on the first return. There was a moderate correlation between the degree of voice severity and the total VHI score before application and on the second return, and a weak correlation on the first return. CONCLUSIONS: Perceptual-auditory analysis and self-perception proved to be efficient in the recognition of vocal changes and of the vocal impact on individuals with adductor spasmodic dysphonia under treatment with botulinum toxin, permitting the quantitation of changes along time.

OBJECTIVE: To evaluate the effectiveness of the Comprehensive Voice Rehabilitation Program (CVRP) compared with Vocal Function Exercises (VFEs) to treat functional dysphonia. STUDY DESIGN: This is a randomized blinded clinical trial. METHODS: Eighty voice professionals presented with voice complaints for more than 6 months with a functional dysphonia diagnosis. Subjects were randomized into two voice treatment groups: CVRP and VFE. The rehabilitation program consisted of six voice treatment sessions and three assessment sessions performed before, immediately after, and 1 month after treatment. The outcome measures were self-assessment protocols (Voice-Related Quality of Life [V-RQOL] and Voice Handicap Index [VHI]), perceptual evaluation of vocal quality, and a visual examination of the larynx, both blinded. RESULTS: The randomization process produced comparable groups in terms of age, gender, signs, and symptoms. Both groups had positive outcome measures. The CVRP effect size was 1.09 for the V-RQOL, 1.17 for the VHI, 0.79 for vocal perceptual evaluation, and 1.01 for larynx visual examination. The VFE effect size was 0.86 for the V-RQOL, 0.62 for the VHI, 0.48 for the vocal perceptual evaluation, and 0.51 for larynx visual examination. Only 10% of the patients were lost over the study. CONCLUSIONS: Both treatment programs were effective. The probability of a patient improving because of the CVRP treatment was similar to that of the VFE treatment.

This study seeks to provide evidence of the dynamics associated with the configurations of discourse-voice regulatory strategies in patient-therapist interactions in relevant episodes within psychotherapeutic sessions. Its central assumption is that discourses manifest themselves differently in terms of their prosodic characteristics according to their regulatory functions in a system of interactions. The association between discourse and vocal quality in patients and therapists was analyzed in a sample of 153 relevant episodes taken from 164 sessions of five psychotherapies using the state space grid (SSG) method, a graphical tool based on the dynamic systems theory (DST). The results showed eight recurrent and stable discourse-voice regulatory strategies of the patients and three of the therapists. Also, four specific groups of these discourse-voice strategies were identified. The latter were interpreted as regulatory configurations, that is to say, as emergent self-organized groups of discourse-voice regulatory strategies constituting specific interactional systems. Both regulatory strategies and their configurations differed between two types of relevant episodes: Change Episodes and Rupture Episodes. As a whole, these results support the assumption that speaking and listening, as dimensions of the interaction that takes place during therapeutic conversation, occur at different levels. The study not only shows that these dimensions are dependent on each other, but also that they function as a complex and dynamic whole in therapeutic dialog, generating relational offers which allow the patient and the therapist to regulate each other and shape the psychotherapeutic process that characterizes each type of relevant episode.

INTRODUÇÃO: O envelhecimento da voz caracteriza-se por alterações decorrentes do envelhecimento do corpo, interferindo na entonação vocal reveladora de estados da alma e do corpo. Investigar a qualidade vocal de mulheres na pós-menopausa por meio dos parâmetros objetivos torna-se importante para fomentar as pesquisas nessa área, bem como auxiliar na melhoria da qualidade de vida desse grupo. OBJETIVO: Verificar os valores dos parâmetros acústicos das vozes de mulheres no período de pós-menopausa. Método: Realizou-se estudo de caráter quantitativo e corte transversal com 23 mulheres em período de pós-menopausa, utilizando o software Praat 5.2.0. Os parâmetros considerados foram frequência fundamental, jitter, shimmer, proporção harmônico ruído e análise espectrográfica. A amostra foi composta por mulheres matriculadas numa Universidade Aberta da Terceira Idade em Salvador-BA. A análise dos dados foi descritiva, por porcentagem simples, e usou-se o teste t para verificar a relação entre aumento de idade e piora vocal. RESULTADOS: Apresentaram valores da frequência fundamental abaixo de 150Hz 13% da amostra, e 34% entre 150 e 217Hz, demonstrando agravamento da voz. Observou-se significância estatística para piora desse parâmetro relacionado ao aumento da idade (p=0,02471). Houve diferença quanto aos parâmetros jitter e shimmer de acordo com o aumento da idade, porém sem significância estatística. Esses parâmetros acústicos estão relacionados a alterações no padrão vibratório das pregas vocais (ciclo vibratório). INTRODUCTION: Voice aging is characterized by changes caused by body aging, interfering with the vocal intonations, revealing states of soul and body. To investigate the vocal quality of postmenopausal women using objective parameters is important to foster research in this area, as well as assisting in improving the quality of life of this group. OBJECTIVE: To determine the acoustic parameters of the voice of women in post-menopause. METHOD: Quantitative cross-sectional study was conducted with 23 women in the postmenopausal period, using the Praat software 5.2.0. The parameters considered were fundamental frequency, jitter, shimmer, harmonic noise and spectrographic analysis. The sample consisted of women enrolled in the Open University of Third Age in Salvador-BA. Data analysis was descriptive, using percentages, and t test to check the relationship between increasing age and vocal worsening. RESULTS: In this sample, 13% had values below the fundamental frequency of 150Hz and 34% between 150 and 217Hz, demonstrating voice deepening. There was a statistically significant worsening of this related to increasing age parameter (p=0.02471). There was difference between the jitter and shimmer parameters according to increasing age, but without statistical significance. These acoustic parameters are related to changes in the vibratory pattern of the vocal folds (vibration cycle). A harmonic noise ratio was above 15.000 in all women, but also had lower values with increasing age, with no statistical significance. CONCLUSION: The observed changes in acoustic parameters show that postmenopausal women had changes in vocal patterns, which can lead to decreased quality of life. Therefore, it requires a closer look at the need of speech therapy in this group. .
