Abstract Background: percutaneous transmitral commissurotomy has become an ideal treatment option for mitral stenosis due to its less adverse events and more favorable outcomes. Patients improve symptomatically after percutaneous transmitral commissurotomy but we have minimal available data about the quality of life after percutaneous transmitral commissurotomy. Objective: to assess the quality of life after percutaneous transmitral commissurotomy through WHOQol scoring covering different aspects of life, to determine its correlation with a net gain in mitral valve area (MVA) and to ascertain its association with gender. Methods: it was a prospective cohort study carried out for a period of 15 months. A total of 100 patients with mitral stenosis who had successful percutaneous transmitral commissurotomy done were enrolled in the study. Among the total included, 4 patients were lost to follow up and data were collected from 96 patients. Quality of life was assessed before the procedure, at 1 month and 3 months follow up after percutaneous transmitral commissurotomy using WHOQol scoring questionnaire. Results: among 96 patients, 64 (67%) were females and 32 (33%) were males. WHOQol scoring improved significantly after percutaneous transmitral commissurotomy from 32.8±8.9 to 54.6±11.2 and 62.8± 9.7, after one month and 3 months respectively. There was a significant association between net gain of MVA and WHOQol scoring with an R value of 0.46 and p value of 0.03. There was no difference in group comparison of all the six domains between male and female patients. Conclusion: successful percutaneous transmitral commissurotomy improves the quality of life in mitral stenosis patients regardless of their gender and has a positive correlation with a net gain in mitral valve area.


Resumen Antecedentes: La comisurotomía mitral percutánea se ha convertido en una opción terapéutica ideal para la estenosis mitral, debido a efectos menos adversos y a resultados más favorables. Los pacientes mejoran sintomáticamente tras la realización de esta técnica, aunque se dispone de escasos datos acerca de la calidad de vida tras su puesta en práctica. Objetivo: Evaluar la calidad de vida tras la comisurotomía mitral percutánea mediante la puntuación WHOQol, que cubre diferentes aspectos de la vida, para determinar su correlación con la ganancia neta del área valvular mitral (AVM), y determinar su asociación con el sexo. Métodos: Estudio prospectivo de cohorte durante un periodo de 15 meses, en el que se incluyeron 100 pacientes con estenosis mitral a quienes se les practicó con éxito comisurotomía mitral percutánea. De entre el total incluido, se perdieron 4 pacientes durante el estudio, por lo que se recolectaron datos de los 96 pacientes restantes. La calidad de vida se evaluó antes del procedimiento, y transcurridos un mes y tres meses de seguimiento, utilizando el cuestionario de puntuación WHOQol. Resultados: De los 96 pacientes, 64 (67%) eran mujeres y 32 (33%) varones. La puntuación WHOQol mejoró considerablemente tras la realización de la comisurotomía mitral percutánea, con valores de 32,8±8,9 a 54,6±11,2 y 62,8± 9,7, transcurridos uno y 3 meses, respectivamente. Se produjo una asociación significativa entre la ganancia neta de AVM y la puntuación WHOQol, con un valor R de 0,46 y un valor p de 0,03. No se produjo diferencia alguna en cuanto a la comparación grupal de los seis dominios entre varones y mujeres. Conclusión: La comisurotomía mitral percutánea exitosa mejora la calidad de vida de los pacientes de estenosis mitral, independientemente del sexo, y tiene una correlación positiva con la ganancia neta del área valvular mitral.

RESUMO

This article discusses measuring quality of life (QOL) loss in litigation. Case examples are provided. The complexity challenge in QOL assessment is more easily addressed since the advent of computer adaptive testing, which is used by physicians and rehabilitationists in the administration of psychometric instruments to determine QOL loss. It is now possible to write algorithms to capture QOL data through text-mining. If QOL domains and their factors could be accessed through text-mining, it would make for an extraordinary opportunity for much needed doctoral level research in QOL issues for injured workers in workers' compensation programs.

BACKGROUND: Parkinson's disease (PD) is characterized by severe motor and non-motor symptoms reducing patients' quality of life (QoL). Instruments have been well established for QoL assessments in PD, including the EuroQol (EQ-5D), the Parkinson's disease questionnaire (PDQ-39), or rather uncommon, like the WHOQOL-100. So far, the impact of variables has been investigated for each of these measures separately in different study populations, limiting the comparability of the results. Thus, this study compared the EQ-5D, PDQ-39, and the WHOQOL-100 (with its short-form WHOQOL-BREF) in the same study population. METHODS: Seventy-five PD outpatients were assessed in a prospective study, including disease severity according to Hoehn and Yahr stage (HY) and Unified Parkinson Disease Rating Scale (UPDRS). The Geriatric Depression Scale (GDS-15) screened for depression. RESULTS: Decreased QoL was found with all three instruments. In multivariate models, sex and treatment complications had an impact on QoL according to all three measures, while duration of PD and HY was not associated with QoL in any of them. Depression was relevant for the WHOQOL-100/WHOQOL-BREF and the PDQ-39, but not for the EQ-5D. The total variances explained by the WHOQOL-100, WHOQOL-BREF, PDQ-39, and the EQ-5D were 0.27, 0.34, 0.70, and 0.50, respectively. CONCLUSIONS: The associations between clinical aspects of PD and QoL vary substantially among all three measures. Importantly, depression as a frequent comorbidity in PD is underestimated by the EQ-5D, but not by the PDQ-39 and the WHOQOL-100/WHOQOL-BREF. In turn, motor impairments are underestimated by the latter and associated strongest with QoL in the EQ-5D.

OBJECTIVE: To assess QoL of infertile Chinese women and determine the specific factors adversely affecting QoL for improving the care and treatment compliance of infertile women. MATERIALS AND METHODS: We conducted a cross-sectional study on a randomized, demographically matched, controlled population of infertile married Chinese women to determine their demographic, menstrual, family stress, and infertility characteristics and then applied the World Health Organization QoL Instrument (WHOQOL-100) to determine which factors would be associated with significant QoL differences between infertile women and their demographically matched fertile controls. RESULTS: Infertile women showed lower QoL scores in the facets of spirituality/religion/personal beliefs, self-esteem, financial resources, and accessibility to and quality of health and social care, as well as increased pain and discomfort, while also experiencing positive QoL adjustments in terms of mobility, daily living activities, work capacity, sexual activity, freedom, physical safety, security, and transport. CONCLUSION: Married infertile Chinese women had significantly lower overall and comprehensive QoL scores, as well as higher anxiety scores, compared with fertile controls.

BACKGROUND: Few studies use modern approaches to develop a quality of life (QOL) questionnaire with acceptable construct validity, especially in Iran. Our main objective was to construct a new validated and uni-dimensional questionnaire, based on WHOQOL-100, using the Rasch analysis. METHODS: In a population-based, cross-sectional study in 2007, 500 Tehran residents aged≥18 were randomly sampled. The Persian version of WHOQOL-100 was used to measure the participants' QOL. After using targeting and person fit analysis, we performed category/threshold ordering, item fit, and differential item functioning analyses, in succession. We used outfit or infit statistics>1.5 and <0.5 for detecting under-fit and overfit items/persons, respectively. We also deleted items with disordered category and/or threshold. Person Separation Index and test reliability were also calculated in the datasets. RESULTS: Male to female ratio was 0.98 and the mean age (SD) of participants was 35.1 (12.8) years. Initial analyses showed significant differences in quality of life between age groups (P=0.002), educational levels (P=0.001), and current health status groups (P<0.001). We eliminated 67 persons for under-fitting, 38 items for category and/or threshold disordering, 6 items for under-fitting, and 26 items for item bias. Test reliability for the final 30-item scale was 0.89. CONCLUSION: We prepared a shortened version of the WHOQOL-100 that is single construct, uni-dimensional and free of item bias or any disordering, according to the Rasch model.

INTRODUCTION: Although there is literature on sexuality in gender dysphoria, few studies have been done prior to genital sex reassignment surgery (SRS). AIMS: To evaluate the perception of sexual QoL in gender-dysphoric patients before genital SRS and the possible factors associated to this perception. METHODS: The final sample consisted of 67 male-to-female and 36 female-to-male gender-dysphoric adults consecutively attended in a gender unit who had not undergone genital SRS; 39.8% was receiving cross-sex hormonal treatment, and 30.1% had undergone breast augmentation or reduction. Sexual QoL was assessed using the sexual activity facet of the World Health Organization Quality of Life (WHOQOL)-100. Sociodemographic (age, gender, partner relationship) and clinical data (being on hormonal treatment and having undergone any breast surgery) were recorded from the clinical records. Depressive symptoms were assessed using the negative feelings facet of the WHOQOL-100. Personality was assessed using the Revised NEO-Five Factor Inventory. MAIN OUTCOME MEASURES: Sexual QoL, negative feelings, hormonal treatment, partner relationship, personality. RESULTS: The mean score of the sexual facet was 10.01 (standard deviation = 4.09). More than 50% of patients rated their sexual life as "poor/dissatisfied" or "very poor/very dissatisfied," around a quarter rated it as "good/satisfied" or "very good/very satisfied," and the rest had a neutral perception. Three variables were significantly associated with a better sexual QoL: less negative feelings (ß = -0.356; P < 0.001), being on hormonal treatment (ß = 0.216; P = 0.018), and having a partner (ß = 0.206; P = 0.022). Age, sex, having undergone some breast surgery, and personality factors were not associated with their perception. CONCLUSION: This study indicates that before genital SRS, about half of gender-dysphoric subjects perceived their sexual life as "poor/dissatisfied" or "very poor/very dissatisfied." Moreover, receiving hormonal treatment, low negative feelings, and having a partner are related to a better subjective perception of sexual QoL.

A obesidade é uma doença crônica que vem acometendo, progressivamente, cada vez mais pessoas no mundo. Por ser uma patologia de difícil controle que favorece a eclosão de outros agravos à saúde, é premente a necessidade de realização de pesquisas que possam contribuir para o aperfeiçoamento dos tratamentos, bem como para a melhoria da qualidade de vida e eficácia adaptativa. Sendo assim, a presente pesquisa visou avaliar a percepção da qualidade de vida (QV), a eficácia adaptativa (EDAO) e o funcionamento global (AGF) de pessoas com obesidade, relacionando os resultados obtidos na avaliação da percepção da QV com os daeficácia adaptativa, bem como aos do funcionamento global (AGF). Para tanto, utilizou-se o questionário WHOQOL-100 versão em português para avaliação da percepção de qualidade de vida, a Entrevista Diagnóstica Preventiva – Escala Diagnóstica Adaptativa Operacionalizada(EDAO) para a eficácia adaptativa e a Escala de Avaliação do Funcionamento Global (AGF) para o funcionamento global. Obesity is a chronic disease that has been progressively affecting a growing number of peoplein the world. Since it is a hard to control pathology that favors the emergence of other health conditions, there is a pressing need to undertake research that might contribute to enhance treatment, as well as improve quality of life and adaptive efficacy. There fore, this study aims at evaluating obese people’s quality of life perception (QL) and adaptive efficacy by relating the results obtained from evaluation of their QL perception with the results from adaptive efficacy, as well as the results from the Global Functioning Evaluation (AGF). For that purpose, the WHOQOL-100 Portuguese-version questionnaire was employed to evaluate their quality of life perception, and a Preventive Diagnosis Interview – Functional Adaptive Diagnostic Scale(FADS) was also undertaken as regards adaptive efficacy, to evaluate thirty obese persons(Body Mass Index – BMI >=30 kg/m2), with an average age of 52.33, who use the services of an ambulatory care center located in the Greater ABC region, in the State of Sao Paulo. ... O objetivo deste estudo foi avaliar a qualidade de vida em adolescentes matriculados no Ensino Médio do Município de São Paulo buscando fornecer subsídios para políticas públicas e privadas visando à melhoria da qualidade de vida desta população. Método. Para avaliar uma amostra representativa de 2434 estudantes do Ensino Médio do município de São Paulo utilizou-se um questionário para avaliação de classe econômica (ABEP, 2007) e o questionário de qualidade de vida WHOQOL-100. Resultados. Observou-se que vários aspectos influenciam na qualidade de vida desta população. Os estudantes do gênero masculino declararam qualidade de vida mais alta do que as do gênero feminino. Da mesma forma, os adolescentes mais novos tenderam a relatar melhor qualidade de vida que os mais velhos. Os representantes das classes sócio-econômicas mais altas também apresentaram melhor qualidade de vida. Conclusões. Tais resultados podem contribuir para o desenvolvimento de políticas públicas e privadas visando à melhoria da qualidade de vida desta população.


Introduction. The objective of this study was to evaluate the quality of life of High School adolescents in the city of São Paulo, seeking assistance for public and private policies and quality of life improvement for this population. Method. In order to evaluate a representative group of 2,434 High School students in the city of São Paulo, a questionnaire for economic evaluation (ABEP, 2007) and the quality of life questionnaire WHOQOL-100 were used. Results. It was noted that a number of aspects influence this population's quality of life. Male students declared a higher quality of life than female students. Similarly, younger adolescents tended to report better quality of life compared to older ones. Representatives of higher socioeconomic classes also presented better quality of life. Conclusions. These results can contribute to the development of public and private policies aiming to improve the quality of life of this population.

Objective. The concept of Health-Related Quality of Life (HRQOL) is important for both the prevention and progress of psychiatric diseases. In the literature, we are aware that there are a few studies on HRQOL among patients with conversion disorder with seizure. Our aim is to measure the HRQOL in Turkey among patients with conversion disorder with seizure and to compare the quality of life of this study group to a healthy control group and to different types of epileptic patient groups. Methods. A total of 330 subjects was included in this study; 102 patients were included in the conversion disorder group, 121 patients were included in the epileptic group and 93 subjects were included in the healthy control group. WHOQOL-100 scale and Trait Anxiety Inventory (TAI) were administered to these groups. Results. HRQOL for the patients with conversion disorder was worse than the control group except for the environment and social relations domains. The generalized epilepsy patients had better physical and psychological health, compared with the patients with conversion disorder. Compared with the partial epilepsy group, HRQOL for the patients with conversion disorder was better in psychological, environmental, social relationship domains. Conclusion. Considering these disorders, patients with conversion disorder need long-term psychiatric treatment. Carrying out this study in our country will be an advantage for the cross-cultural studies which will be undertaken out in the future.

This article reports the development of a pain and discomfort module (PDM) designed to assess the full impact of quality of life (QoL) relating to chronic pain, which could be used with the generic World Health Organisation Quality of Life Assessment (WHOQOL). First, cognitive interviewing was completed with nine participants with chronic pain, for 108 items representing 10 pain-specific facets of QoL. Sixty-eight QoL items and 16 importance questions on pain were relevant, comprehensive, comprehensible and acceptable to users, and were confirmed to assess their purported concepts. Secondly, these items were pilot tested using a cross-sectional survey of 216 people with chronic pain, to investigate the preliminary psychometric properties of the PDM, and reduce its items statistically. All new facets were important to those with chronic pain. Sixteen items within four facets of pain relief, anger and frustration, vulnerability/fear/worry, and uncertainty were retained, and demonstrated acceptable to good internal consistency reliability (α = 0.77-0.85). The PDM is a self-administered, multidimensional subjective assessment of pain-related QoL, with potential to evaluate pain-relieving interventions, identify sufferer's needs, and for survey use.

Objective: To investigate factors related to quality of life among nurses. Methods: 1103 nurses were sampled from 5 hospitals and administered with WHOQOL-100. Results: There were significant differences between nurses' on major variables of quality of life. Levels of depression and anxiety had important influence on the quality of life of nurses. Conclusion: The quality of life of nurses was within normal range. Some variables such as mental health status and family factors might play an important role in nurses' quality of life.

In confronting the advances in the new treatment for incurable illnesses there is an increasing need for doctors to be aware of their patients' cognition and feeling related to their quality of life (QOL). Recognizing this need the authors translated and pilot tested the WHOQOL-100, a genuinely international measure of QOL by the World Health Organization (quality of life group). The WHOQOL-100 Malay version was pilot tested on 50 healthy controls and 250 ill subjects, suffering from hypertension, diabetes mellitus, those suffering from both hypertension and ischaemic heart disease, epilepsy and schizophrenia. The results showed several unique features of the QOL, which were influenced by different types of illnesses. The information obtained is different and probably not observable from clinical consultations. This study will be an impetus for further studies using the WHOQOL-100 assessment tool in the local population.

Objective: To investigate the quality of life of patients with chronic prostatitis. Methods: Two hundred patients were included and tested with WHOQOL-100 . Results: Compared with normal controls, the following aspects had no significant difference, including energy and fatigue, appearance and status, working activity, daily life and environment. There were significant difference to controls in other 6 fields and 20 aspects. The major factors that do harm to QOL include satisfaction on life, income and depression. Conclusion: It should deserve the clinician's attention that the QOL of patients with chronic prostatitis is greatly lower than that of the normal controls.
