RESUMO

After endodontic therapy, restoring severely broken or damaged crown structure is a difficult task in conservative dentistry. Regular post and core followed by crown repair cannot restore a crown with steep incisal guidance, very little overjet, and highly damaged crown structure. Richmond crown is better recommended in these situations since Richmond crown is a crown having post. It is prepared as a single piece having a ceramic facing. We frequently encounter teeth having very less or no clinical crown portion that are structurally damaged. Support and retention of the restoration are challenging to achieve in such situations. The rehabilitation of anterior teeth that has been endodontically treated and structurally impaired is reviewed in two cases.

Objective@#To explore the application effect of a four-dimensional smile design as the leading and facial streamline as the reference system in the aesthetic restoration of anterior teeth.@*Methods@# A case of scattered space in anterior teeth was treated with a four-dimensional smile design and digital aesthetic restoration with ultrathin porcelain veneer. The digital information of the patients was obtained through oral and facial scanning, and a four-dimensional smile design and prediction were carried out. After the treatment plan was jointly determined by doctors and patients, minimally invasive abutment preparation was carried out, and ultrathin porcelain veneer was made and bonded in place. After the operation, the esthetic degree and marginal fit were observed and reexamined one year after the repair.@* Results @#The edges of the ultrathin ceramic veneers were naturally tight, and the color was coordinated, with satisfying pink and white esthetics. The face was more harmonious and natural when smiling. One year after the restoration, the facial aesthetics were wonderful, the abutment teeth and periodontal tissues were healthy, and the patients were satisfied. The results of the literature review showed that the fitting of temporal facial scanning and intraoral scanning data can accurately predict four-dimensional smile aesthetics, simulate the real state of the dynamic smile and pronunciation process, and combine it with facial streamlines to design a natural and coordinated personalized smile for patients with asymmetric faces. However, for patients with occlusal changes, unstable occlusion or temporomandibular joint disorder, it is necessary to add data fitting, such as electronic facial arch and cone beam CT, to more accurately simulate postoperative mandibular movement.@*Conclusion@# With a four-dimensional smile design as the leading and facial streamline as the reference system, the whole process digital design assisted the restoration of asymmetric anterior teeth with ultrathin porcelain veneer and had a good effect. The postoperative smiling of patients is harmonious and beautiful, which is in line with the expected effect. Patient participation and satisfaction are high; thus, this method is worthy of clinical promotion.

OBJECTIVES: To evaluate the clinical effect of integrated angulated screw channel (ASC) abutment crown in implant-supported rehabilitation of the aesthetic area. METHODS: Sixteen patients who received single implant-supported rehabilitation using integrated ASC abutment crown in the aesthetic area were included in the study. After one-year follow-up, the cumulative survival rate, aesthetic effectiveness, bone resorption around implants, and patient satisfaction were analyzed through periapical film, clinical examination, and the visual analog scale (VAS). RESULTS: The implant cumulative survival rate of the integrated ASC abutment crown in the implant-supported rehabilitation of the aesthetic area was 100%. The average pink esthetic index scores reached 9.5, and the white esthetic index scores reached 9.4. The mean marginal bone loss of implants was (0.439±0.123) mm at the mesial side and (0.341±0.118) mm at the distal side. The average VAS satisfaction score was 8.9, which showed that all patients were satisfied with the final restorative effect. CONCLUSIONS: Using integrated ASC abutment crown for implant restoration is an ideal implant restoration design in the aesthetic zone and can be applied clinically.

OBJECTIVES: A study was conducted to investigate the clinical effects of oral digital design on the aesthetic restoration of anterior teeth of cleft lip/palate patients. METHODS: Nine adult cleft lip/palate patients who need aesthetic restoration of anterior teeth were recruited. Digital information of patients' dental arches, the surrounding soft tissue and face were captured by digital camera and scanner. The aesthetic analysis and design were conducted using keynote and 3shape software and were demonstrated to the patients. The optimized treatment plan was ensured by communicating with the patients. Digital wax-up models were exported and printed into resin diagnostic models, which were then utilized in the treatment process to guide the doctors and the technicians in tooth preparation and in making the final restorations, respectively. The adhesive procedure was completed after satisfactory try-in. Aesthetics assessment was conducted in accordance with the anterior esthetic evaluation form. The scores of patient's satisfaction were recorded on a questionnaire containing six items of aesthetic index and doctor-patient communication. Patients were interviewed and examined after 1, 3, 6, and 12 months, respectively, and the clinical effects of restorations were evaluated. RESULTS: All nine patients had satisfactory clinical results. The aesthetic defects of the patients were effectively addressed. All treatments met the requirements of the preoperative digital designs. The patients' scores were all above 90 on the satisfaction scale. At 12 months after the operation, the clinical effects of restorations of all cases achieved A class in each evaluation indicator. CONCLUSIONS: For cleft lip/palate patients with esthetic defect in the anterior teeth, the digital design plays an important role in optimizing the treatment plan and guides the whole treatment process. This design can help clinicians achieve predictable satisfactory aesthetic results.

Dentin dysplasia is a rare hereditary disorder, transmitted by autosomal dominant mode, affecting both dentin and pulp. In Type I crown morphology is normal, but root dentin organization loss leads to shorter roots. Mutations in the SSUH2, VPS4B and SMOC2 genes have been reported as responsible for this condition. Orthodontic treatment was conducted on an 11-year-old female patient presenting the disorder along with bilaterally impacted permanent maxillary canines, in close proximity to the roots of the lateral and central incisors. Treatment plan included lateral incisors extraction, surgical exposure and traction of the impacted canines. Light forces were applied from a custom-made trans-palatal arch. Comprehensive orthodontic treatment was performed using edgewise appliances. After 3 years and 2 months, group function occlusion was achieved. The canines underwent composite resin restorations. At one year post-retention, the dentition remained stable. Family-based genetic analysis did not reveal any mutations in the aforementioned genes pointing to further genetic heterogeneity of this disorder. As dental medicine becomes more sophisticated and personalized, the association between mutation type/function and orthodontic treatment response may provide useful therapeutic insights. The positive treatment response of the presented case could be attributed to a more "benign" mutation awaiting to be identified.

Purpose This study aimed to evaluate the effects of degradation on the strength of computer-aided design/computer-aided manufacturing (CAD/CAM) resin composite blocks (RCBs) by subjecting them to accelerated degradation in water and conducting biaxial flexural strength tests.Methods Six commercial RCBs were tested. The RCBs were cut into disks, after which the disks were immersed in purified water. For the aging experiment, the samples were subjected to heat treatment at 37, 60, 70, and 80 °C, in a constant temperature oven and stored statically for 30 d. After the aging experiment, the elements released from the RCB fillers were measured by inductively coupled plasma atomic emission spectroscopy. In addition, the biaxial flexural strength of the RCB fillers was measured after accelerated degradation at 70 °C.Results Si (the main component of the filler) was detected in all the RCB solutions after the aging experiment; however, the type and amount of other elements differed considerably among the RCBs. The flexural strength of some of the RCBs decreased by approximately 20-40% after the accelerated degradation. For most materials, the Weibull coefficient decreased or remained unchanged after the test, whereas it increased in some materials.Conclusions The strength of all the RCBs decreased after the accelerated degradation tests; however, this behavior differed among the materials. In addition, the release of elements from the filler of some of the materials into the water correlated with the decrease in the strength of these materials. These findings indicate that the evaluation of the degradation behavior of RCBs in water is essential for their long-term usage.

Existen determinadas situaciones donde los implantes, considerados estándar, no se pueden colocar o suponen un riesgo para el mantenimiento de la cresta marginal. Los implantes de diámetro estrecho se han empleado para numerosas indicaciones, estableciéndose protocolos de tratamiento, científicamente fundamentados, con excelentes resultados a largo plazo. En el presente trabajo presentamos una revisión histórica de los implantes estrechos, destacando sus indicaciones y limitaciones y presentamos un caso clínico en el que se han mostrado útiles Caso clínico: Se presenta el caso de una mujer de 59 años de edad, con antecedentes de cáncer de mama en el 2008, que precisa la extracción de los cuatro incisivos supriores. El caso se resuelve con dos implantes estrechos postextracción y prótesis inmediata para preservar el aspecto estético. Conclusiones: En el caso que presentamos se evidencia que los implantes estrechos son una alternativa cuando rehabilitamos espacios edéntulos con poca disponibilidad ósea en el sentido horizontal o espacios mesio-distales reducidos en zonas estéticas. There are certainly situations where the standard implants can not be placed or suppose a risk for the crestal bone maintenance. The narrow implants have been used for a wide variety of indications, stablishing scientifically-based treatment protocols with excellent long-term results.In the present study we present a narrative review of narrow implants highlighting their indications and limitations, and a case report where narrow implants have been shown useful. Case Report:A 59-year-old woman case report is presented, with a breast cancer history in 2008, who required the extraction of the four upper incisors. The case is solved with two post-extraction narrow implants and an immediate prosthesis to preserve aesthetics. Conclusions: In this case report, it is evident that narrow implants are an alternative when rehabilitate edentulous spaces with low available bone in the horizontal direction or reduced mesio-distal spaces in aesthetic areas. Objective@#To explore the clinical effect of bulk-fill composite resin combined with transparent preformed crowns for aesthetic restoration of deciduous incisor of children.@*Methods@#A predesigned clinical prospective randomized controlled research method was used to select 90 patients (123 teeth). The random number table method was divided into three groups: A, B, and C. Group A was treated with a bulk-fill composite resin of SF (SonicFill) combined with a transparent preformed crown (41 teeth in 30 cases), and group B was treated with a large block of Tetric N-Ceram Bulk Fill Composite resin combined with transparent preformed crowns (39 teeth in 29 cases). Group C was treated with 3M Z350 XT universal nano resin combined with transparent preformed crowns (43 teeth in 31 cases). The visual analog scale (VAS) and the modified USPHS standard were used to evaluate the completeness, marginal steps, marginal discoloration, surface condition, secondary caries and satisfaction of the parents with prostheses after 12 months.@*Results @#Twelve months after the operation, the evaluation indexes of group A were better than those of group B and group C, and the differences were statistically significant, including edge integrity (χ2=10.847, P=0.028), edge step (χ2=7.799, P=0.020), edge discoloration (χ2=10.391, P=0.034), surface state (χ2=11.476, P=0.021), and secondary caries (χ2=10.447, P=0.034). The satisfaction of parents in group A on the overall contour (χ2=10.238, P=0.037), shape and texture (χ2=11.521, P=0.021) were better than those in group B and group C, and the differences were statistically significant. There was no significant difference in the evaluation of color satisfaction among the three groups (χ2=0.990, P=0.610).@* Conclusion@#SonicFill bulk-fill composite resin combined with transparent preformed crown is good for short-term aesthetic restoration of deciduous incisor, and parental satisfaction is high.

Aesthetic alternatives to stainless-steel crowns for restoring primary molars attain growing interest. We studied the mechanical properties of prefabricated zirconia crowns and conventional crowns. Three brands of prefabricated zirconia crowns were compared with computer-aided design/computer-aided manufacturing (CAD/CAM) zirconia crowns, preveneered stainless-steel crowns, and conventional stainless-steel crowns regarding: (i) fracture load under each of three conditions [no pretreatment, artificial aging in saliva for 12 wk, and after chewing simulation/thermocycling (1.68 × 106 cycles/5-55°C)]; and (ii) survival rate during chewing simulation, considering decementation, fracture, chipping, fatigue cracks, and occlusal holes. Without pretreatment, the prefabricated zirconia crowns showed mean fracture load values between 893 N and 1,582 N, while the corresponding values for CAD/CAM zirconia crowns and preveneered stainless-steel crowns were 2,444 N and 6,251 N. Preveneered stainless-steel crowns showed significantly lower fracture loads after artificial aging (5,348 N after saliva aging; 3,778 N after chewing simulation) than without artificial aging, whereas the fracture load of zirconia crowns was not influenced negatively. The survival rate of the different groups of zirconia crowns and preveneered stainless-steel crowns during chewing simulation was 100%, but only 41.7% for the stainless-steel crowns. These in-vitro data suggest that prefabricated zirconia crowns are aesthetically and durable alternatives to stainless-steel crowns for primary molars.

Objective: To analyze the postoperative satisfaction and repair effect of prosthesis of the patients who received digital smile design(DSD) combined with porcelain veneers in the anterior teeth, and to illustrate the their application in anterior teeth aesthetic restoration. Methods: A total of 91 anterior teeth of 32 patients whose clinical diagnosis conformed to the indications of porcelain veneers and aesthetic restoration because of tetracycline pigmentation teeth, dental fluorosis, color stained teeth, enamel defects and anterior teeth space were selected. DSD software was used to design the preoperative aesthetic analysis of the teeth morphology and to show the virtual effect of restoration. The preparation of tooth under the guidance of the design results was finished. The IPS e-max porcelain veneers were prepared after the conventional impression preparation. At last, the permanent restorations were manufactured and bonded to teeth. After the adhesive program was completed, the satisfaction of the patients in the shape of restorations, the degree of coordination with the adjacent teeth, color, pronunciation, smile effect and doctor-patient communication effect were evaluated by the questionnaires. The patients were required to regularly return after one month, three months, six months and twelve months, and observe the clinical effects of restoration was observed referring to the modified American Public Health Service (USPHS) standard. Results: The patients' satisfaction rates of each evaluation content were all over 90%. Among them, the satisfaction rates of the smile effect and the doctor patient communication were as high as 100%. The clinical performances of the rehabilitations in each period were as follows: One month after treatment, there were four teeth occurred mild gingivitis and three teeth appeared the postoperative sensitivity symptoms; three months after treatment, there were one tooth showed a minor fracture in the restoration without affecting the appearance and function and three teeth had mildly congestive gums reaction, at the same time one tooth showed slight discoloration of the restoration; six months after treatment, one tooth showed ceramics fracture of restoration and four teeth developed mild gingivitis; twelve months after treatment, two teeth showed ceramics fracture, the edge of three restorations had slightly crack and one tooth appeared gingivitis. The periodontal conditions of the patients with gingival inflammation after treatment were improved significantly after receiving the correct oral hygiene guidance. The above defects didn't affect the aesthetics effect and the functions of restorations, and the restorations could be used normally after intraoral adjustments. Both the doctors and the patients were satisfied with the repair effects of the restorations. Conclusion: The combination of DSD and porcelain veneer in repairing anterior aesthetic teeth is good treatment plan that can achieve high patient satisfaction and ideal clinical effect.

@#With the development of digitalization, information and internet technology, digital dental aesthetic restoration technology is changing the traditional mode of treatment. The development of digital aesthetic restoration therapy is similar to the digital development track in other fields of medicine, in which the use of digital technology for intervention in particular treatment steps is evolving to digitalization throughout the treatment process. Digitalization improves the precision and quality of aesthetic restoration, increases the efficiency of clinical work and is favorable for patients. Digitalization could replace traditional methods, propelling aesthetic restoration into a fully digitalized era. Function, aesthetics and minimal invasiveness are the three major concepts of contemporary prosthodontics, and digital aesthetic restoration technology represents a perfect interpretation of these three concepts. This paper describes recently developed digital technologies for aesthetic restoration and cites the advantages, limitations, and developmental direction of digital restoration dentistry.

The color of dental restorative material should be maintained throughout its functional lifetime in an oral environment. However, the frequent use of mouthwash may affect the color stability of these composite restorations. The aim of this study is to assess the effects of using various mouthwashes on the color stability of various dental restorative composite materials. For this purpose, four mouthwashes/gels (Flocare gel (0.4% stannous fluoride), Pascal gel (topical APF fluoride), Pro-Relief mouthwash (sodium fluoride), and Plax Soin mouthwash (sodium fluoride)), and distilled water as a control, were selected. These were divided into five groups: Group 1: Flocare gel; Group 2: Pascal gel; Group 3: Pro-Relief mouthwash; Group 4: Plax Soin mouthwash; and Group 5: distilled water (control). Prepared restorative materials samples were immersed in the groups of mouthwashes/gels and the distilled water (control) for 24, 48, and 72 h. The discoloration that all materials exhibited with all immersion groups was significantly different at each of the three time periods for all groups (p < 0.05). Results from immersion in Flocare gel, Pascal gel, Pro-Relief mouthwash, and Plax Soin mouthwash were statistically significant (p < 0.05). The color change chroma was not significant for Pro-Relief and Plax Soin mouthwash (p > 0.05). Mouthwashes/gels affect color shifting for all composite resin materials, and changes are exaggerated over time. However, discoloration effects are not perceptible to the human eye.

@#In patients with anterior teeth loss, especially in patients with soft tissue defects, the aesthetic problem of soft tissue is still a major problem in the delayed planting restoration. In the restoration of anterior teeth, the health of the soft tissue around implants play a key role in guaranteeing the long-term success of dental implants. Therefore, consideration is worth being taken that how to deal with the soft tissue during the process of the delayed implant restoration as well as how to evaluate the effects after treatment. According to the process of the delayed implant operation sequence therapy-preoperative, intraoperative and postoperative treatment, this article will have a literature review and summary through these aspects as followings: the red aesthetic evaluation index, treatment of soft tissue around delayed implant, clinical curative effect evaluation.

The purpose of this study was to investigate the color samples (A2, A3 and B1) of three different brands of resin composites using dentine masses. 135 discs were prepared (5 plates for each thickness, color and brand of composite material). A colorimetric evaluation, using white and black background, was performed just after preparation. The color was measured corresponding to "Vita" scale and ΔL, Δa, Δb and ΔE values were calculated using a spectrophotometer. The results showed that Value, Chroma and Hue often differ even if the same commercial color and same thickness is used. In conclusion, this study showed that the perfect aesthetics restoration is possible combining individual abilities, experience and correct techniques.

INTRODUCTION: Not many investigations have evaluated the relationship between the height of the interdental papillae, gingival zenith and maxillary anterior teeth. The assessment of these parameters can aid in fabricating definitive aesthetic restoration. AIM: The aim of this study was to evaluate the existence of proportional ratio between the incisal edge to gingival zenith (HGZ) and the tip of the interdental papilla (HIP) in maxillary anterior teeth for Indian population. MATERIALS AND METHODS: One hundred healthy volunteers with an average age of 25-30 years comprised the evaluation group. Impression was made using irreversible hydrocolloid impression material and type IV Dental stone cast was made. A calibrated digital caliper measured the distance between HGZ and HIP. The values for all six maxillary teeth were recorded and the proportional ratio was evaluated and statistically analysed. RESULTS: The mean ratio between HGZ and HIP of all maxillary anteriors in 1(st) and 2(nd) quadrant was 1.80, 1.71, and 2.03 in central incisor, lateral incisor and canine respectively with p-value > 0.9. CONCLUSION: The mean proportional measurements for maxillary anterior teeth were determined and no definitive proportion existed between HGZ and HIP of maxillary anterior teeth.

OBJECTIVES: Large, full thickness upper eyelid defects are still difficult to deal with in oculoplastic surgery. High specificity in the anatomy and function of the upper eyelid shows how difficult reconstruction can be. We aimed to assess the results provided by the Cutler-Beard flap technique. METHODS: We conducted a retrospective study of all patients who had undergone Cutler-Beard flap reconstruction in the Oculoplastic service of La Fondation Rothschild in Paris, from January 2008 to June 2013. Sixteen patients were included. RESULTS: Fifteen flaps remained viable during the follow-up examinations. Opening, occlusion and palpebral position were very satisfactory in 11, 12 and 11 cases respectively. The ocular surface was preserved in 8 cases and exhibited only mild alterations in 5 cases. The average aesthetic result was noted 2/3 for 11 patients with postoperative photographs. We did not observe any relapses with oncologic indications. CONCLUSIONS: Upper eyelid reconstruction by the Cutler-Beard flap technique appears to provide good functional and aesthetic restoration in the majority of cases. In the others, a simple additional surgery under local anaesthetic and/or sedation provides adequate correction in most instances.

RESUMO

96 anterior teeth with different degree of caries in 80 patients were randomly divided into 2 groups(n =48 teeth,40 patients). In the experimental group the teeth were repair with nanometer resin filling,in the control group with ceramic crown.In 0.5 to 1 year follow-up,the satisfaction rate and the clinical success rate were similar in the 2 groups(P >0.05).Application of nano resin aesthetic restoration can lower tooth injury,postoperative complications and the cost.

A Ortodontia Lingual já alcançou um lugar de destaque entre as técnicas utilizadas atualmente. São muitos os pacientes que têm optado por uma opção de tratamento mais estético. Como na Ortodontia convencional, alguns problemas são muito similares aos encontra dos quando se opta pelo tratamento com a Ortodontia Invisível. Deve-se sempre estar atento aos problemas de origem periodontal e de discrepâncias de tamanho dentário. O caso apresentado mostra um tratamento ortodôntico lingual, onde após a correção da Classe II, foi necessário uma reanatomização dos dentes anteriores para alcançar uma estética maior para o sorriso da paciente.


Lingual Orthodontics has reached a place of prominence among the techniques currently used. There are many patients who have opted for a more aesthetic treatment. As in conventional Orthodontics, some problems are very similar to those found when opting for Invisible Orthodontics treatment. One should always be mindful of periodontal problems and tooth size discrepancies. This case shows a lingual orthodontic treatment, in which after correction of Class II, a reanatomization of anterior teeth was needed in order to achieve better aesthetic for the patients’ smile.
