RESUMO

The intestinal microbiota exhibits a strong correlation with the function of the central nervous system, exerting influence on the host brain through neural pathways, immune pathways, and microbial metabolites along the gut-brain axis. Disorders in the composition of the intestinal microbial are closely associated with the onset and progression of neurological disorders, such as depression, Alzheimer's disease, and Parkinson's disease. It has been proven that fecal microbiota transplantation can improve symptoms in animal models of neurological diseases and clinical patients. This paper provides a comprehensive review of the composition and function of the human intestinal microbiota, as well as the intricate the relationship between the human intestinal microbiota and nervous system diseases through the gut-brain axis. Additionally, it delves into the research advancements and underlying mechanism of fecal microbiota transplantation in the treatment of nervous system diseases. These findings offer novel insights and potential avenues for clinical interventions targeting nervous system diseases.

Osteosarcoma (OS) is a highly prevalent bone malignancy among adolescents, accounting for 40% of all primary malignant bone tumors. Neoadjuvant chemotherapy combined with limb-preserving surgery has effectively reduced patient disability and mortality, but pulmonary metastases and OS cells' resistance to chemotherapeutic agents are pressing challenges in the clinical management of OS. There has been an urgent need to identify new biomarkers for OS to develop specific targeted therapies. Recently, the continued advancements in genomic analysis have contributed to the identification of clinically significant molecular biomarkers for diagnosing OS, acting as therapeutic targets, and predicting prognosis. Additionally, the contemporary molecular classifications have revealed that the signaling pathways, including Wnt/ß-catenin, PI3K/AKT/mTOR, JAK/STAT3, Hippo, Notch, PD-1/PD-L1, MAPK, and NF-κB, have an integral role in OS onset, progression, metastasis, and treatment response. These molecular classifications and biological markers have created new avenues for more accurate OS diagnosis and relevant treatment. We herein present a review of the recent findings for the modulatory role of signaling pathways as possible biological markers and treatment targets for OS. This review also discusses current OS therapeutic approaches, including signaling pathway-based therapies developed over the past decade. Additionally, the review covers the signaling targets involved in the curative effects of traditional Chinese medicines in the context of expression regulation of relevant genes and proteins through the signaling pathways to inhibit OS cell growth. These findings are expected to provide directions for integrating genomic, molecular, and clinical profiles to enhance OS diagnosis and treatment.

Introduction. Les luxations temporo-mandiblaires sont peu fréquentes au CHU de Bouaké, mais elles sont invalidantes. Les objectifs étaient de : Distinguer les aspects épidémiologiques, et d'identifi er les formes cliniques et les aspects thérapeutiques. Méthodes. Il s'agissait d'une étude rétrospective et descriptive qui s'est déroulée du 1er janvier 2018 au 30 décembre 2019 dans le service d'odontostomatologie et de chirurgie maxillo-faciale du Centre Hospitalier et Universitaire de Bouaké. Résultats. Quarante et un (41) cas ont été colligés soit une fréquence hospitalière de 5%. L'âge moyen était de 40,18 ans avec des extrêmes de 11 ans et 77 ans. Le sex-ratio était de 0,78. Le délai moyen de consultation était de 3 jours. Le bâillement représentait 48,78% des causes et avait pour facteur favorisant l'édentation partielle postérieures dans 90% des cas. Les luxations isolées de l'articulation temporo mandibulaire étaient observées dans 85,37% des cas, et étaient toutes antérieures. Parmi ces luxations isolées, 88,57% étaient bilatérales et 11,43% unilatérales. La luxation supérieure a été retrouvée dans 4,88% de cas, et elle était toujours associée à une fracture de la symphyse mandibulaire. Dans 80,49% des cas, le traitement a été orthopédique. L'évolution a été favorable dans 82,93% des cas. Conclusion. Les luxations temporo-mandibulaires sont des affections très invalidantes. Les absences dentaires constituent le principal facteur prédisposant d'où la nécessité d'une bonne réhabilitation dentaire prothétique afi n d'éviter les récidives.

Excessive crying and sleep problems affect up to 30% of infants and often coexist. Although usually benign and self-limiting, persistent crying, and sleep problems exceeding 6 months of age need attention as they may impair the mental health of the infant and its family. The source and the impact of these persistent regulatory problems is often not restricted to the infant, but extends to the parents and the parent-infant relationship. Clinical practice needs interdisciplinary and multi-method interventions focusing beyond regulatory problems of the infant but also on parental self-regulation and parent's co-regulatory responses toward the infant. Treating clinicians may encounter limitations of home-visits, outpatient, and pediatric residential settings when working with families in distress. We describe an infant mental health day-clinic treatment, drawing attention to this viable future direction. It offers a therapeutic climate based on forming a triangle of co-regulation between clinician, parent and infant to first help the parent and the infant settle down. This stress reduction restores parent-infant connectedness and parental learning and reflecting capacity. Clinicians then use established therapeutic modalities to support parental self- and co-regulatory skills which is important for the development of self-regulation in the infant. Experience with this treatment program suggests that a day-clinic setting facilitates interdisciplinary and integrative multi-method intervention, infant and parental stress reduction and integration of parental self- and co-regulatory skills in daily family life, improving overall outcomes. This perspective warrants further investigation.

Background: The effectiveness of the psychotherapeutic treatment of posttraumatic stress disorder is evidence-based and generally considered proven. However, the effectiveness of multimodal, group-based day clinic treatment programs has rarely been investigated. Moreover, there is no consensus in the literature concerning the question whether psychotherapeutic approaches for trauma-related disorders are also applicable for patients with complex PTSD (cPTSD). The aim of the study was to evaluate our multimodal group-based treatment program regarding a change of psychiatric burden, a change of protective factors, and possible differences in therapy outcome for patients with or without cPTSD. Methods: The group-based treatment for patients with trauma-related disorders was examined in 66 patients who filled out the following questionnaires in the first and in the last week of treatment: Essen Trauma Inventory (ETI), Screening for complex PTSD (SkPTBS), Patient Health Questionnaire-somatization module (PHQ-15), Beck Depression Inventory-Revised (BDI-II), Posttraumatic Growth Inventory (PTGI), and Questionnaire on social support (F-SozU). Results: The treatment was shown to significantly reduce depressive symptoms (p < 0.001, d = -0.536) and increase posttraumatic growth (New Possibilities: p = 0.004, d = 0.405; Personal Strength: p = 0.005, d = 0.414). For patients with cPTSD, depressive (p = 0.010, d = -0.63) as well as cPTSD symptoms (p = 0.020, d = -0.796) were significantly reduced; perceived social support was increased after day clinic treatment (p = 0.003, d = 0.61). Contrary to our expectations, somatoform symptoms were increased after therapy. Conclusions: The present work expands previous research by demonstrating that multimodal group-based, day clinic treatment is effective in the treatment of trauma-related disorders, also in their complex form.

The breast cancer is a usual and serious malignant tumor which threatens the women′s health.Molecular subtyping bases on the molecular level, and provides a new classification method for the breast cancer pathology classification, and plays an important guidance significance for the clinical treatment.At present, the breast cancer molecular subtyping is mainly divided into the following subtypes: the Luminal A type and Luminal B type, HER-2 overexpression and the triple negative breast cancer.Different molecular subtyping has different characteristics in treatment reaction, prognosis and the clinical application situation.

Transference Focused Psychotherapy for Borderline-Adolescents in a Day Clinic Treatment Program This paper focuses on the concept of transference focused psychotherapy (TFP) modified for juvenile borderline patients. Adolescents with borderline developmental personality disorder (bpd) have an essential deficit in their personality structure that leads to oscillations in their self-esteem and in a "split" perception of the world. They suffer from a variety of symptoms and severe impairments on their own and their families' quality of life. Their fragmented perception of themselves and others make relationships almost unbearable for them. Relationships are mostly marked by severe anxiety of resentment and rejection. For these patients this causes intolerable trouble at school where every day conflicts take place. Self-mutilation and suicidal thoughts often seem the only way out. By now, there is an agreement that an early specialized assessment and treatment is necessary in order to stop the typical consequences of their self-mutilative and dysfunctional behavior. Still, in contrast to adult age, empirical evidence is missing which proves the effectiveness of treating adolescent borderline patients. In this paper we present a research project on the effectiveness of transference focused psychotherapy with adolescent borderline patients (TFP-A) in a day clinic setting, combining TFP with group skills training as known from dialectic behavior therapy (DBT). Furthermore, we give first results on analyzing the effectiveness of our day clinic treatment program based on TFP-A, focusing on improving core symptoms such as affective problems, aggressive behavior against self and others and interpersonal problems.

Internalized stigma is a complicating feature in the treatment of schizophrenia spectrum disorders and considerably hinders the recovery process. The empowerment and recovery-oriented program of our day clinic might contribute to a reduction in internalized stigma. The aim of the study was to explore the influence of this day clinic program on internalized stigma and other subjectively important outcome measures such as quality of life and psychopathology. Data from two groups of patients had been collected twice, at baseline and after 5 weeks. The experimental group attended the day clinic treatment (N=40) and the control group waited for the day clinic treatment (N=40). The following significant differences between the two groups were found: Patients in day clinic treatment showed a reduction in internalized stigma while the control group showed a minimal increase (Cohen's d=0.446). The experimental group as compared with the control group also showed a greater improvement in the quality of life domain psychological health (Cohen's d=0.6) and in overall psychopathology (Cohen's d=0.452). Interestingly, changes in internalized stigma and psychological quality of life were not associated with changes in psychopathology. Results are encouraging but have to be confirmed in a randomized design.

OBJECTIVE To investigate the clinical characteristics and the present condition of infective diabetic foot. METHODS A total of 100 cases with infective diabetic foot were classified into the wet gangrene and composite gangrene, and treated by the systemic and local modality in combination. RESULTS The cure rate of the wet gangrene and composite gangrene was 29.5% and 18.2%, respectively and the effective rate was 83.3% and 68.2%, respectively. CONCLUSIONS The combined therapy is effective for the treatment of diabetic foot.

Objective To evaluate the value of CT in the diagnoses and treatment of the liquid in the renal sub capsule.Methods The spiral CT images and clinic material of the liquid in the renal sub capsule were concluded in 25 cases.Results The total accuracy of CT showed 100% for the liquid in the renal sub capsule and reliable for the injury of liver and spleen,fracture of lumbar vertebra and abdominal accumulation of water.Conclusion CT diagnoses is the evidance to choose treatment for the liquid in the renal sub capsule with conservation,intervention,or operation.
