RESUMO

Background: A combination of standard biomedical treatment and traditional Chinese medicine (TCM) has been suggested as a therapeutic approach for rosacea that may significantly lower the recurrence rate and clinical symptom scores. We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) to evaluate the impact of this combination treatment on clinical symptom and TCM syndrome scores, as well as on the scores of the Dermatology Life Quality Index (DLQI), erythema index (EI), and interleukin 37 (IL-37) levels in patients with rosacea. Methods: The PROSPERO registration number for the study is CRD42023472737. We systematically searched the PubMed, Embase, Web of Science, China National Knowledge Infrastructure Wanfang Database, China Biomedical Medicine database (CBM), and the VIP information resource integration service platform (cqvip) databases for RCTs (published from the beginning to September 2023, regardless of the language used) that compared the traditional Chinese medicine and standard biomedical treatment combination treatment to conventional anti-rosacea treatments. Our primary outcomes comprised the clinical symptom and TCM syndrome scores, and the scores of Dermatology Life Quality Index, erythema index, and IL-37 levels. We used a random-effects model to evaluate the pooled data. Results: We identified 260 studies. Of these, 13 eligible studies were employed for analysis (N = 1,348 participants). Compared with other anti-rosacea treatments, the TCM and standard biomedical treatment combination treatment yielded an improved mean reduction in the clinical symptom score -2.24% [95% CI (-3.02 to -1.46), p < 0.00001], TCM syndrome score -4.42 [95% CI (-5.33 to -3.50), p < 0.00001], and the score of DLQI of -2.55 [95% CI (-3.73 to -1.36), p < 0.00001], EI of -151.97 [95% CI (-276.59 to -27.36), p < 0.00001], and IL-37 level -4.23 [95% CI (-4.95 to -3.51), p = 0.854], as well as in the overall effective rate risk ratio (RR) = 1.25 [95%CI (1.18, 1.32), p = 0.994] and the recurrence rate = 0.27 [95%CI (0.15, 0.46), p = 0.297]. Conclusion: The TCM and standard biomedical treatment combination treatment can provide a better outcome, including a reduction in the TCM syndrome and clinical symptom scores, and in the scores of DLQI, EI, and IL-37. Hence, this combination is a viable and more effective therapeutic approach for rosacea. However, these results should be considered cautiously because of uncertain evidence and the low quality of the study reports considered in this meta-analysis. At present,there is a lack of consensus on the diagnosis, treatment and management of erectile dysfunction(ED)/premature ejaculation(PE) comorbidity in China. To develop a clinical diagnosis and treatment strategy for ED/PE comorbidity, according to the guidelines for ED/PE comorbidity published in China and abroad in recent years and combined with the clinical practice of many urology and andrology research units in China,the clinical experts organized by the Branch of Sexology of Traditional Chinese Medicine, China Sexology Association formed this consensus through consensus drafting, academic review, expert correspondence interview and full discussion at the review meeting based on the existing evidence-based medical evidence. Based on the clinical experience of doctors,research evidence,patients' wishes and living environment factors,this consensus comprehensively evaluates the diagnosis and treatment norms of ED/PE comorbidity with integrated traditional Chinese and Western medicine,aiming at the diagnosis and treatment orientation and value of integrated traditional Chinese and Western medicine,so as to benefit the whole process management of more patients. Traditional Chinese medicine diagnosis pays attention to the collection of four diagnostic information of inspection, auscultation, inquiry and palpation, and carries out pattern differentiation of six meridians, pattern differentiation of Wei-defence, Qi, Ying nutrients and blood, and pattern differentiation of zang-fu organs as described in the Shanghanlun (《伤寒论》). Based on the synergy of traditional Chinese and Western medicine, the advantages of the overall regulation of traditional Chinese medicine are brought into play, and the individualized treatment of integrated traditional Chinese and Western medicine is implemented. Internal treatment of traditional Chinese medicine is based on the pattern differentiation of yin and yang described in Section Ⅱ, Chapter Ⅳ Syndrome Differentiation of Surgical Diseases in Surgery of Traditional Chinese Medicine. As stated in Leijing: Yinyanglei (《类经·阴阳类》), "Human diseases ... must have their roots, either in Yin or in Yang. Although there are many pathological changes, their roots are one." Huangdineijing:Suwen: Yinyangyingxiangdalun(《黄帝内经·素问·阴阳应象大论》) states that "Those who are good at diagnosis should look at the color and pulse, and differentiate Yin and Yang syndromes first." This consensus describes the yang and yin syndromes according to the principle that "Yin and Yang syndromes differentiation should be the priority in the pattern differentiation by the eight principles." In addition, for the syndromes of ED/PE comorbidity, based on the same treatment of different diseases, Chaihu Guizhitang, Wenjingtang and Buzhong Yiqitang are used, which can be referred to.

Since December 2019, Wuhan city of Hubei Province has been the center of a number of patients infected with coronavirus disease 2019 (COVID-19). People are susceptible to be infected. The infection has spread to newborn and small infants, and severe cases of children have now occurred. Cases of infection in children have drawn great attention from the society. The pathogenesis of COVID-19 is unclear, and there is no specific drug for treatment, which brings great difficulties to epidemic prevention and clinical treatment. In this paper, we try to understand COVID-19 in children from different perspectives of TCM and western medicine. We also try to analyze the etiology, epidemic characteristics, disease development characteristics and treatment measures of COVID-19 in children. At the same time, we try to explore the etiology, pathogenesis, syndrome differentiation and treatment of COVID-19 in children with TCM. It is hoped that the combination of TCM and western medicine can better understand the new coronavirus pneumonia in children and its pathological basis, analyze the current trend of the epidemic situation, so as to find out safe and effective prevention and control measures and provide reference for clinical prevention and treatment of COVID-19 in children.

Inspired by the traditional Chinese herbal pair of Polygala tenuifolia-Acori Tatarinowii for treating epilepsy, 33 novel substituted cinnamic α-asaronol esters and analogues were designed by Combination of Traditional Chinese Medicine Molecular Chemistry (CTCMMC) strategy, synthesized and tested systematically not only for anticonvulsant activity in three mouse models but also for LDH inhibitory activity. Thereinto, 68-70 and 75 displayed excellent and broad spectra of anticonvulsant activities with modest ability in preventing neuropathic pain, as well as low neurotoxicity. The protective indices of these four compounds compared favorably with stiripentol, lacosamide, carbamazepine and valproic acid. 68-70 exhibited good LDH1 and LDH5 inhibitory activities with noncompetitive inhibition type, and were more potent than stiripentol. Notably, 70, as a representative agent, was also shown as a moderately positive allosteric modulator at human α1ß2γ2 GABAA receptors (EC50 46.3 ±â€¯7.3 µM). Thus, 68-70 were promising candidates for developing into anti-epileptic drugs, especially for treatment of refractory epilepsies such as Dravet syndrome.

Under the guidance of combination of traditional Chinese medicine chemistry (CTCMC), this study describes the preparation of a phenolic acid/dipeptide/borneol hybrid consisting of phenolic acid and a bornyl moiety connected to the dipeptide N-terminal and C-terminal respectively. It also evaluates their angiotensin converting enzyme (ACE) inhibitory and synergistic antihypertensive activities. Briefly, a series of novel 2-hydroxypyrrolobenzodiazepine-5,11-dione analogues were prepared and investigated for their ability to inhibit ACE. The influence of the phenolic acid and bornyl moiety on subsite selectivity is also demonstrated. Among all the new compounds, two compounds-7a and 7g-reveal good inhibition potency in in vitro ACE-inhibitory tests. Interestingly, favorable binding results in molecular docking studies also supported the in vitro results. Additionally, the bioassay showed that oral administration of the two compounds displayed high and long-lasting antihypertensive activity both in acute antihypertensive tests and in therapeutic antihypertensive tests by non-invasive blood pressure measurements in spontaneously hypertensive rats.

Objective To investigate the clinical effect of combination of traditional Chinese and western medicine on the treatment of senile multiple organ failure. Methods The control group of elderly patients with multiple organ failure was given routine western medicine treatment, and the study group was given the combination of Chinese and Western medicine treatment. Changes of BUN, PaO2, PaCO2, Cr, LVEF, CO and so on were recorded before and after treatment in two groups. Results There was no significant difference in comparison of related indexes of heart and kidney function value between two groups before treatment; after treatment in two groups, there were significantly decreased in BUN, PaCO2 and Cr index values compared with before, while PaO2, CO and LVEF significantly increased than before, index of the study group were improved better than those of the control group (P<0.05). Conclusion the application of conventional western medicine treatment of multiple organ failure in the elderly based on the use of Chinese medicine can significantly improve the clinical efficacy, to ensure the quality of life, life has important value in safety.

This study was aimed to investigate the effect of acupuncture combined with traditional Chinese medicine (TCM) on the incidence of lipid metabolism and the incidence of hyperandrogenism in patients with polycystic ovary syndrome (PCOS).The clinical data of 85 PCOS cases in our hospital from March 2015 to August 2016 were retrospectively analyzed.There were TCM treatment group and acupuncture combined with TCM group according to the treatment method.The difference of blood lipid level,hormone level,incidence rate of hyperandrogenism and clinical symptom score before and after treatment in both groups were observed.The results showed that the incidence rate of hyperandrogenism was 2.22% in the acupuncture combined with TCM group,which was significantly lower than TCM group (x2=7.070,P =0.008).There was no difference on blood lipid level before treatment in both groups.After treatment,levels of TC,LDL-C and TG of the acupuncture combined with TCM group were significantly lower than those of the TCM group;however,level of HDL-C was higher than that of the TCM group (t =1.817,-3.627,2.554,2.226,P <0.05).There was no difference on hormone levels before treatment.After treatment,hormone levels of the acupuncture combined with TCM group were significantly lower than the TCM group.Before treatment,there was no difference on TCM symptom score between both groups.After treatment,the TCM symptom score of the acupuncture combined with TCM group was significantly lower than that of the TCM group.It was concluded that acupuncture combined with TCM had a better therapeutic effect on PCOS.It can improve the blood lipid level of patients and reduce the incidence of hyperandrogenism.It had good clinical value.

This study was aimed to explore clinical effect on treatment of rheumatoid arthritis (RA) of cold-dampness syndrome by the combination of Wen-Jing San-Han Tong-Luo (WJSHTL) decoction and acupuncture. A total of 88 RA patients were selected and randomly divided into the control group and the treatment group. All 44 cases in the control group were orally administrated with diclofenac sodium, sulphasalazine (SASP) and methotrexate (MTX). All 44 cases in the treatment group were treated with the combination of WJSHTL decoction and acupuncture on the ba-sis of western medication the same as the control group. Each group was treated for three months. Improvements of clinical symptoms, body signs and laboratory indexes were observed. The results showed that clinical symptoms, body signs and laboratory indexes were greatly improved in the comparison before and after treatment in both groups with statistical significance (P < 0.05). Compared to the control group, the treatment group had no statistical significance in the improvement of rest pain, swelling numbers and index, tenderness numbers and index, morning stiffness time, liver and kidney function. Compared with the control group, the treatment group had more obvious advantages in the improvement of joint function, hand grip strength, erythrocyte sedimentation rate (ESR) decreasing, C-reactive protein (CRP) and rheumatoid factor (P< 0.05). It was concluded that compared with single western medication, the combi-nation of WJSHTL decoction and acupuncture with western medication had certain advantages in the improvement of joint function of RA and management of patient's disease condition.

Objective]To discuss and analyze Professor Cheng Jing's clinical experience for treating decline in ovarian reserve(DOR). [Method]Through learning from the tutor in the outpatient service and case studies ,we analyzed and summarized Pro. Cheng's recognition on the etiology and pathogenesis of DOR,together with his dialectical way of thinking, combined therapy of Chinese and western medicine ,and empirical prescriptions. [Result] Pro. Cheng believes that this disease lies in the kidneys as wel as in the thoroughfare and conception meridian ,the pathogenesis is kidney deficiency and the imbalance of the thoroughfare and conception meridian,or combined with the stagnation of liver qi,spleen deficiency,and blood stasis and phlegm fire (heat); the therapy is mainly nourishing and warming kidneys as wel as conditioning the thoroughfare and conception meridian ,combined with Chinese medicine's periodical therapy,such as adjusting the circulation of qi and blood in livers,spleens,hearts and other organs. [Conclusions] Pro. Cheng's combined therapy of Chinese and western medicine for curing DOR ,of which his empirical prescriptions the Bushenyichongkangshuaitang is the main part,is of remarkable curative effect and significant clinical value.

Objective To evaluate clinical effects of integrated traditional Chinese and western medicine therapy for epilepsy after stroke. Methods Literatures about the treatment of epilepsy after stroke with the combination of traditional Chinese medicine and western medicine which come from the database such as CNKI, VIP, WanFang and Pubmed, were studied and all the randomized controlled trials on comparing therapeutic effects of treating epilepsy between combined treatment of TCM and western medicine and western medicine were enrolled. The Cochrane Collaboration's software RevMan 5 was used for meta-analysis. Results 7 articles were included in the study, there is homogeneity among these articles(P>0.05), The fixed effect model was used for the meta analysis. the combined results were OR=5.17, 95%CI(3.02, 8.82), the overall effect of test Z=6.01, P<0.01, the difference between the treatment group and the control groups was significant, that means the overall effect of the combination of traditional Chinese medicine and Western medicine treatment was better than western medicine treatment; Funnel plot graphic displayed asymmetrically, which indicated the literatures publication bias. Conclusion The current data indicate that the curative effect of TCM and west medicine combined treatment is better than pure western medicine therapy for epilepsy after stroke, but as the literature publication bias, clinical studies need more high quality large samples to prove this conclusion.

Objective To explore the effects of combination of traditional Chinese medicine (TCM)and western medicine on a variety of allergen induced severe perennial seasonal allergic rhinitis patients who were treated invalidly with antihistamines and intranasal flixonase for more than two years.Methods 16 patients with moderate to severe allergic rhinitis who were uncontrolled with antihistamines and intranasal flixonase for one week were treated by combination with TCM of Xin-Yi-Qing-fei San or Gyokubeifu-san mixed formula for two weeks.Nasal symptoms,its effects on the quality of daily life,and telephone follow-up half a year later were evaluated.Mini Rhinoconjunctivitis Quality of Life Questionnaire (Mini-RQLQ) and Nocturnal Rhinoconjunctivitis Quality of Life Questionnaire (NRQLQ) were used to evaluate the quality of life.Results After 2 weeks combination-treatment,the nasal symptoms of congestion,rhinorrhea,sneezing,nasal itching,and nasal obstruction were significantly improved compared with combination-before [(52.0± 11.1)%,(69.1±5.3)%,(68.3±12.3)%,(55.5±10.0)%to (10.9±2.4)%,(8.9±3.2)%,(12.5±4.3)%,(13.2±5.3)%,respectively].The before and after treatment of nasal function of nasal minimum cross-sectional area (MCA),nasal cavity volume (V5) and nasal airway resistance (R75 and R150) respectively were (0.58±0.12)cm2,(0.38 ± 0.23) Pa/cm3 · s-1,(0.44 ± 0.32) Pa/cm3 · s-1,(3.50 ± 2.33) cm2 to (0.48 ± 0.23) cm2,(0.31 ±0.33) Pa/cm3 · s-1,(0.31 ±0.37)Pa/cm3 · s-1,(2.24± 1.03) cm2 (P<0.01).In addition,findings of patients with nasal symptoms by half year follow-up were significantly better than those of before.Conclusion The interventionof TCM can effectively control the perennial and seasonal severe symptoms of patients with AR,and improve the quality of life of patients.

Objective To explore the influence of integrated traditional Chinese medicine and western medicine on neurological function in patients with ischemic stroke.Methods One hundred patients with ischemic stroke were randomly divided into a treatment group and a control group.Both groups were given basic treatment.At this basis,sixty patients in the treatment group were given Chinese decoction,Kudiezi injection,traditional Chinese medicine soaking,and comprehensive rehabilitation; forty patients in the control group were given citicoline,modem rehabilitation techniques.Both groups were treated for 21 days.The scores of NIHSS,ADL,and MMSE were assessed at 7th day,14th day,and 21st day after admission of the trial,respectively.Results After 14 days,the scores of NIHSS (22.33± 1.06)in treatment group were decreased significantly (P<0.05).After 21 days,the scores of NIHSS,ADL,MMSE in treatment group were(18.73±1.23),(20.19±2.17),(38.68±2.85),and(21.65±l.41),(25.37±2.34),(32.12±3.11)in control group,the improvement of scores in treatment group were superior to the control group (P<0.05).The total effective rate was 91.67% in treatment group,and 70% in control group,it was significantly higher in treatment group than that in the control group (x2=7.98,P<0.05).Conclusion The therapy of integrated traditional Chinese medicine and western medicine can significantly improve ischemic stroke,enhance the therapeutic efficiency of patients with ischemic stroke.
