OBJETIVE: In Europe, units with Dual-trained Neurovascular Surgeons (DTNS) skilled in both open neurosurgery (ON) and endovascular neurosurgery (EN) are scarce. For instance, in Spain, our unit is unique within the public health system, where all neurovascular procedures are carried out by DTNS. Our study aims to evaluate the evolution in treating ruptured intracranial aneurysms (rICAs) and assess the impact of this evolution on clinical outcomes. METHODS: A retrospective cohort study was performed on rICAs treated in our unit from October 2012 to June 2023. We reviewed clinical and radiological data to analyze the evolution of ON and EN over time, as well as their impact on patient outcomes. Univariate, multivariate, and mixed-effects models were utilized to discern temporal changes. RESULTS: The modified Fisher Scale (mFS) and the modified World Federation of Neurological Surgeons scale (mWFNS) showed strong correlation with the outcome at 6 months outcomes, both with p < 0.00001. However, the surgical intervention method, ON versus EN, did not significantly affect outcomes (p > 0.85). In adjusted multivariate logistic regression, mFS (-1.579, p: 0.011) and mWFNS (-0.872, p < 0.001) maintained their significance. rICAs location was significant when comparing ON to EN p = 0.0001. A significant temporal trend favored the selection of EN p = 0.0058). Mixed-effects time series modeling indicated that while patient characteristics and rICA specifics did not predict treatment choice, the year of treatment was significantly correlated (0.161, p = 0.002). Logistic regression with interaction terms for time and treatment type did not produce significant results. CONCLUSION: Our findings suggest that despite an increased adoption of EN techniques, there has been no change in patient outcomes. Even with the rise of EN, our unit continues to perform ON for a higher proportion of rICAs than most national hospitals. We propose that a "dual approach" offers advantages in a patient individualized treatment decision protocol in the European context.

The clinical effect, degree of OMNP, size of the aneurysm, type of treatment, subarachnoid hemorrhage (SAH), and time from onset to treatment were analyzed on the resolution of OMNP. Results: Ninety-six patients with 99 Pcom aneurysms were enrolled and treated endovascularly, with the success rate of 100%. Immediately after endovascular treatment, 75 aneurysms (75.75%) got complete occlusion, and 24 (24.24%) nearly complete occlusion. Followed up for 3–18 (mean 8.52 ± 0.56) months, complete resolution of the OMNP was achieved in 63 patients (65.63%), partial resolution in 21 (21.88%), and non-recovery in the other 12 (12.50%). The degree of OMNP at onset, SAH, and time from onset to treatment were significantly (P < 0.05) correlated with the resolution of OMNP. Univariate analysis revealed that younger age of the patient, degree of OMNP at onset, presence of subarachnoid hemorrhage, and time from disease onset to treatment were significantly (P < 0.05) associated with the recovery of OMNP. Multivariate analysis revealed that the younger age, degree of OMNP at onset, and time from disease onset to treatment were significantly (P < 0.05) associated with the recovery of OMNP. Conclusion: Endovascular embolization of Pcom aneurysms concomitant with OMNP can effectively improve the OMNP symptoms, especially for patients with moderate and a shorter history of OMNP. Younger age, degree of oculomotor nerve palsy at onset, and time from onset to treatment may significantly affect recovery of oculomotor nerve palsy.(AU)


Objetivo: Investigar la eficacia de la embolización intravascular del aneurisma de comunicación posterior (Pcom) en pacientes con parálisis oculomotora (OMNP) y los factores que influyen en la eficacia. Materiales y métodos: Se analizaron retrospectivamente los datos clínicos de la terapia intravascular en pacientes con aneurismas Pcom con OMNP. Todos los pacientes recibieron tratamiento intravascular. Se analizaron los efectos de la eficacia clínica, el grado de OMNP, el tamaño del aneurisma, el método de tratamiento, la hemorragia subaracnoidea y el tiempo desde el inicio hasta el tratamiento en la regresión de OMNP.Resultados: Un total de 96 pacientes con 99 aneurismas Pcom fueron tratados con éxito. Inmediatamente después del tratamiento intravascular, 75 casos (75,75%) de aneurismas fueron completamente ocluidos y 24 casos (24,24%) casi completamente ocluidos. Durante el seguimiento de 3 a 18 meses (promedio: 8,52 ± 0,56 meses), se logró la resolución completa en 63 casos (65,63%), la resolución parcial en 21 (21,88%) y la no recuperación en los otros 12 (12,50%). El grado de OMNP al inicio, la hemorragia subaracnoidea y el tiempo de inicio a tratamiento se correlacionaron significativamente con la resolución de la OMNP (p < 0,05). El análisis univariado mostró que la menor edad del paciente, el grado de OMNP, la presencia de hemorragia subaracnoidea y el tiempo transcurrido desde el inicio de la enfermedad hasta el tratamiento se correlacionaron significativamente con la recuperación de OMNP (p < 0,05). Conclusión: La embolización intravascular del aneurisma Pcom combinada con OMNP puede mejorar eficazmente los síntomas de OMNP, especialmente en pacientes con OMNP a corto y mediano plazo. La edad temprana, el grado de parálisis del nervio oculomotor al inicio y el tiempo desde el inicio hasta el tratamiento tuvieron un efecto significativo en la recuperación de la parálisis del nervio oculomotor.(AU)

BACKGROUND: Ascending aortic aneurysms are rare pathologies in childhood, especially in the absence of previous diseases such as Marfan syndrome. OBJECTIVE: Present the possibility of successful endovascular management of large vessel aneurysms, using stents and microcatheters with embolization of the aneurysm sac. METHOD: We present the case of a previously healthy ten-year-old patient, in whom a pseudoaneurysm was documented between the origin of the left common carotid artery and left subclavian artery, successfully managed endovascularly, initially with a stent covering the neck of the aneurysm to remodel it and later with embolization of the aneurysm sac using a microcatheter. RESULTS: Aneurysms of large vessels, such common carotid artery and subclavian artery, are at risk of rupture with devastating complications; endovascular management is considered a minimally invasive management option, with favorable results. CONCLUSION: The endovascular management of large vessel aneurysms using stents and microcatheters with embolization of the aneurysmal sac is a novel management option that achieves successful results.

MÉTODO: Presentamos el caso de una paciente de 10 años y 2 meses, previamente sana, en quien se documentó un pseudoaneurisma entre el origen de la arteria carótida común izquierda y la arteria subclavia izquierda, que logró manejarse de forma endovascular, inicialmente con un stent cubriendo el cuello del aneurisma con el fin de remodelarlo y posteriormente por medio de microcatéter se realizó embolización del saco del aneurisma con coils, con resultado exitoso. RESULTADOS: Los aneurismas de los grandes vasos, como la arteria carótida común y la arteria subclavia, tienen riesgo de ruptura con complicaciones devastadoras; el manejo endovascular se plantea como una opción poco invasiva de manejo, con resultados favorables. CONCLUSIÓN: El manejo de aneurismas de grandes vasos, por vía endovascular usando stent y microcatéter con embolización del saco aneurismático, es una opción novedosa de manejo que logra resultados exitosos.

Objetivo: Dar a conocer la posibilidad del manejo endovascular exitoso de los aneurismas de grandes vasos, usando stent y micro catéter con embolización del saco aneurismático. INTRODUCTION: Renal trauma is the most common of urological trauma and accounts for up to 5% of all. The AAST scale is the most widely used to assess renal trauma. This study focuses on high-grade trauma, whose treatment has evolved towards a conservative approach, with techniques such as angioembolization. The aim is to describe the evolution in the management of high-grade renal trauma in all patients treated at La Paz University Hospital from 2001 to 2022. METHODS: A descriptive retrospective study was conducted on patients treated at the hospital. The study was divided into two periods (2001-2010 and 2011-2022). A total of 285 patients with renal trauma were collected, of which 54 were high grade. The main variable is the type of management, conservative (embolization) or interventional through nephrectomy. RESULTS: In the completed series, there was a decrease in radical nephrectomy in high-grade renal trauma from 50% to 13.8% over time, with an increase in embolization from 23,1% to 44,8%. In patients with isolated renal trauma, those treated with embolization increased from 28.6% to 69.2%, while those undergoing radical/partial nephrectomy decreased from 42.8% to 7.69%. CONCLUSION: The management of renal trauma has evolved over the years in our center. The number of patients treated by embolization has increased, while the number of complications and nephrectomies has decreased.

Presentamos el caso de un niño de 12 años que consultó por hemoptisis, sin otros sín- tomas asociados. Se realizó radiografía de tórax (patológica), laboratorio con aumen- to moderado de reactantes de fase aguda, PPD (negativa), esputos x 3 con bacilosco- pias negativas y tomografía de tórax con contraste i.v. que mostró imágenes de árbol en brote en todos los lóbulos y una imagen de dilatación vascular de una rama de la ar- teria pulmonar en lóbulo superior izquierdo. Se plantearon diagnósticos diferenciales: malformación vascular primaria o lesión secundaria a infección. La angiografía digital permitió confirmar el pseudoaneurisma y embolizarlo. Luego de 17 días, 2/3 cultivos de esputo fueron positivos para Mycobacterium tuberculosis. El niño realizó tratamiento antituberculoso con drogas de primera línea con evolución clínica favorable. Este caso resalta la importancia de considerar el pseudoaneurisma de Rasmussen en- tre las posibles complicaciones de un paciente con tuberculosis y hemoptisis recurren- te o masiva.


We present the case of a 12-year-old boy admitted to the hospital due to hemoptysis without other symptoms. We performed a Thorax X-Ray (pathological), laboratory with elevated acute phase reactants, TST (negative), sputum x 3 with negative smear and computed tomography angiography showing a tree-in-bud pattern in all lobes, and di-latation of a brunch of the pulmonary artery in the upper left lobe. We considered pri-mary vascular anomaly or lesion due to infection as a differential diagnosis. The patient underwent digital angiography and therapeutic embolization of this pseudoaneurysm. After seventeen days, 2/3 of the sputum cultures were positive for Mycobacterium tu-berculosis. The patient received standard anti-TB therapy with favorable evolution. This case highlights the importance of considering complications such as Rasmussen's pseudoaneurysm in patients with pulmonary tuberculosis and recurrent or massive hemoptysis.

La hemoptisis se define como la expectoración de sangre del árbol traqueobronquial, por lo general se origina en las arterias bronquiales. Una vez confirmada la presencia y el sitio de sangrado se debe elegir entre los diferentes métodos de manejo de la hemoptisis, cada uno con sus beneficios y limitaciones. La embolización de arterias bronquiales es una técnica endovascular mínimamente invasiva. Se ha convertido en el método de elección para tratar hemoptisis masiva y recurrente. Tiene una tasa de éxito en el primer episodio superior al 80%. La tasa de recurrencia posterior al procedimiento va de un 10% a un 55%, en el cual la cirugía llega a tener un papel de importancia. Objetivos: Describir las características demográficas, clínicas, diagnóstico etiológico y tratamiento de pacientes con hemoptisis en un hospital de tercer nivel de la Ciudad de México. Material y métodos: Estudio retrospectivo de pacientes con diagnóstico de hemoptisis en el periodo comprendido entre enero de 2014 a diciembre de 2016. Los datos fueron obtenidos del expediente clínico. Resultados: Se estudiaron 34 pacientes media de edad 52 años, con predominio en hombres (52,9%). La etiología de la hemoptisis fue tuberculosis (45,5%), neoplasias (20,6%), bronquiectasias (15,2%), malformación arteriovenosa (6,1%). El sitio de embolización más frecuente fue la arteria bronquial superior derecha (56,6%), seguido de la arteria bronquial inferior izquierda (23,3%) y un grupo de 6 pacientes (18,7%) requirieron un segundo evento de embolización por recurrencia del sangrado. Conclusión: El manejo de la hemoptisis debe de ser integral. El objetivo principal es mantener una vía aérea permeable y evaluar cada paciente para un manejo óptimo de acuerdo al tipo y etiología de la hemoptisis.


Hemoptysis is defined as the expectoration of blood from the tracheobronchial tree, typically originating from bronchial arteries. Once the presence and bleeding site are confirmed, one must choose among different methods for managing hemoptysis, each with its own benefits and limitations. Bronchial artery embolization is a minimally inva sive endovascular technique. It has become the method of choice for treating massive and recurrent hemoptysis. Its success rate in the first episode is over 80%. The recur rence rate after the procedure ranges from 10% to 55%, in which surgery may play an important role. Objectives: to describe the demographic and clinical characteristics, the etiological diagnosis and treatment of patients with hemoptysis at a tertiary care level hospital in the City of Mexico. Materials and methods: retrospective study of patients diagnosed with hemoptysis during the period from January 2014 to December 2016. The data were obtained from the clinical records. Results: a total of 34 patients with a mean age of 52 years were studied, with a pre dominance of males (52.9%). The etiology of hemoptysis was tuberculosis (45.5%), neoplasms (20.6%), bronchiectases (15.2%), and arteriovenous malformation (6.1%). The most frequent embolization site was the right upper bronchial artery (56.6%), followed by the left lower bronchial artery (23.3%); and a group of 6 patients (18.7%) required a second embolization procedure due to recurrence of bleeding. Conclusion: the management of hemoptysis should be comprehensive. The main objective is to maintain airway permeability and evaluate each patient for optimal man agement based on the type and etiology of the hemoptysis.

RESUMEN Se presenta el caso de un paciente varón de 55 años con antecedente de colecistitis aguda, que durante la hospitalización presentó anemia por hemobilia secundaria a un pseudoaneurima de la arteria cistica. Se realizó, como tratamiento de primera elección, embolización con microparticulas de alcohol polivinílico (PVA) de la arteria cistica, logrando detención del sangrado.


SUMMARY We present the case of a 55-year-old male patient with a history of acute cholecystitis who presented during his hospitalization haemobilia due to a pseudoaneurysm of the cystic artery. Embolectomy of the cystic artery with polyvinyl alcohol microparticles was performed stopping the bleeding.

El mioma uterino representa el tumor más frecuente en la mujer de edad reproductiva, aunque solo un 25% de mujeres precisarán tratamiento debido a la presencia de síntomas. Disponemos de pocos estudios aleatorizados que hayan comparado las distintas opciones de tratamiento para el mioma uterino entre ellas; por ahora, la cirugía sigue siendo la opción que ha ofrecido mejores tasas de resolución sintomática, mejoría en calidad de vida y menor tasa de recurrencia. A pesar de la amplia difusión de la cirugía, sí que parece razonable pensar que, disminuyendo los tratamientos quirúrgicos, ya sea con terapias menos invasivas o con nuevos tratamientos médicos, se podría disminuir por un lado la morbilidad asociada a la cirugía y, por otro lado, los costes asociados. En este sentido, las técnicas ablativas para el tratamiento del mioma, la EAU, el HIFU y la radiofrecuencia, han demostrado resultados en cuanto a control sintomático y calidad de vida equiparables a la cirugía, con una baja tasa de complicaciones graves. El mayor problema sería las tasas de recurrencia y el impacto que pueden tener sobre la fertilidad futura de las pacientes. Será necesario disponer de mayor evidencia y estudios comparativos a largo plazo respecto a nuevas técnicas ablativas alternativas a los abordajes quirúrgicos.(AU)


Uterine fibroids are the most common tumours in women of reproductive age, but only 25% of women will require treatment due to the presence of symptoms. There are few randomised studies that have compared the different treatment options for uterine myoma; for the moment, surgery remains the option that offers the best rates of symptomatic resolution, improved quality of life, and a lower recurrence rate. Despite the widespread use of myoma surgery, it seems reasonable to believe that by reducing surgical treatments, either with less invasive therapies or with new medical treatments, both associated morbidity and costs would be reduced. In this sense, ablative techniques for myoma treatment, UAE, HIFU, and radiofrequency, have demonstrated results in terms of symptomatic control and quality of life comparable to surgery, with a low rate of major complications. Recurrence rates and the impact on patients’ future fertility would be the main concern. Future long-term comparative studies of ablative techniques and surgical approaches are needed.(AU)

Introducción: la endofuga de tipo II (EFT2) es la más frecuente tras la reparación endovascular de aneurismas de aorta abdominal (EVAR). Objetivos: analizar la presencia de endofugas de tipo II durante el seguimiento, la regresión del saco aneurismático, la tasa de reintervención debido a EFT2, el análisis de los resultados en nuestra serie de casos tratados con embolización de la AMI antes del implante de la endoprótesis aórtica como método útil para disminuir las EFT2 durante el seguimiento. Material y métodos: análisis retrospectivo de los pacientes tratados en nuestro centro con embolización de la AMI previa al EVAR en el periodo 2019-2021. Los criterios utilizados para la embolización de la AMI fueron: diámetro > 3 mm y AL con diámetro > 2 mm o aneurismas aortoilíacos. Se incluyeron 7 pacientes varones (edad media: 72,1 años). El 42 % presentaba aneurismas aortoilíacos. En dos casos se llevó a cabo la embolización de la AMI en un primer tiempo y posteriormente el EVAR; en los restantes se realizó en el mismo procedimiento. El diámetro medio de la AMI fue 5,02 ± 0,9 mm. Todos los pacientes presentaban, al menos, dos AL enfrentadas al origen de la AMI con un diámetro > 2 mm. Resultados: el éxito técnico fue del 100 %. La mediana de seguimiento, 20,7 meses. En los angio TAC al mes y a los 12 meses se objetivó una correcta embolización de la AMI. No hubo fugas de tipo II durante el seguimiento. En todos los casos se visualizó una disminución en el diámetro del saco aneurismático (mediana de regresión: 5,08 mm). No hubo reintervenciones relacionadas con la patología aórtica. Conclusiones: la embolización de la AMI previa al EVAR en pacientes con un diámetro > 3 mm y al menos dos AL con diámetro > 2 mm o aneurismas aortoilíacos parece proteger frente al desarrollo de EFT2 a los 12 meses, a la espera de poder confirmar los resultados a medio y largo plazo. Alto éxito técnico y aceptable regresión del saco aneurismático.(AU)


Introduction: type II endoleak (T2EL), through the inferior mesenteric artery (IMA) or lumbar arteries (LA), is themost common endoleak after endovascular abdominal aortic aneurysm repair (EVAR). Objectives: the primary endpoint was the presence of type II endoleak at follow-up. Secondary endpoints includedaneurysm sac regression and reoperation rate due to T2EL, as well as the analysis of the results in our series of casestreated with IMA embolization prior to the endovascular procedure as a useful method to reduce T2EL at follow-up. Material and methods: this was a retrospective analysis of patients treated at our unit with IMA embolizationprior to EVAR from 2019 through 2021. The criteria used for IMA embolization were IMA diameter > 3 mm, presenceof LA with a diameter > 2 mm, or aortoiliac aneurysms. A total of 7 male patients were included with a mean age of72.1 years. A total of 42 % had aortoiliac aneurysms. In 2 of the cases, IMA embolization was performed initiallyfollowed by EVAR while in the remaining cases it was performed within the same procedure. The mean diameterof IMA was 5.02 mm ± 0.9 mm. All patients had at least 2 LAs facing the origin of the IMA with a diameter > 2 mm.Results: technical success was 100 %. The median follow-up was 20.7 months. In the CCTA performed 1 monthand 12 months postoperatively, correct IMA embolization was observed. There were no type II leaks at follow-up.In all cases, a decrease in the diameter of the aneurysmal sac was observed with a mean regression of 5.08 mm. There were no subsequent reinterventions associated with aortic valve disease. Conclusions: IMA embolization prior to EVAR in patients with a diameters > 3 mm and the presence of at least2 ALs with diameters > 2 mm and/or aortoiliac aneurysms seems to protect against the development of T2EL at12 months, waiting to be able to confirm the results in the mid- and long-term...(AU)

Introduction: Small intestine diverticula are a rare condition with an incidence of 0.6% to 2%. Their location at the level of the jejunum is a rare alteration, and their diagnosis is often delayed due to low clinical suspicion. The clinical manifestation of this pathology is related to the development of complications -15% to 30% of patients, with approximately 10% requiring surgical intervention. Clinical case: We present a case of a middle-aged adult patient who experienced a complication due to a bleeding jejunal diverticulum. The patient underwent surgical management, which resulted in a satisfactory outcome. Objective: This article aims to describe jejunal diverticulosis, a rare condition that can have a significant impact on affected individuals. Emphasizing its clinical suspicion as a differential diagnosis for gastrointestinal bleeding is crucial. Additionally, we discuss diagnostic methods and highlight various therapeutic options, including surgical management.


Introducción: los divertículos del intestino delgado presentan una incidencia del 0,6% al 2%, su localización a nivel del yeyuno es una alteración poco frecuente y, dada su baja sospecha clínica, se retrasa el diagnóstico oportuno. La manifestación clínica de esta patología se encuentra relacionada con el desarrollo de complicaciones, las cuales ocurren entre el 15% y el 30% de los pacientes, y el 10% de estos pacientes requiere manejo quirúrgico. Caso clínico: un paciente adulto medio cursó con una complicación secundaria a la presencia de un divertículo del yeyuno sangrante, al cual se le dio un manejo quirúrgico con un resultado satisfactorio. Objetivo: este artículo tiene como objetivo describir la diverticulosis yeyunal que, aunque poco frecuente, puede generar un compromiso importante en quien la padece, lo que prioriza su sospecha clínica como diagnóstico diferencial causante de hemorragia gastrointestinal, así como dilucidar métodos diagnósticos y estar al tanto de las diferentes opciones terapéuticas que existen, incluido el manejo quirúrgico.

Los miomas uterinos son el tumor más frecuente en la edad reproductiva y suponen un impacto negativo en la calidad de vida de las pacientes. El tratamiento de esta entidad debe hacerse de forma individualizada. La embolización de la arteria uterina es un tratamiento mínimamente invasivo, seguro, eficaz y con una baja tasa de complicaciones cuando se usa como tratamiento de miomas sintomáticos. El objetivo de este artículo es realizar una revisión bibliográfica exhaustiva sobre el estado actual de la embolización de las arterias uterinas en el tratamiento de los miomas que incluya consideraciones previas sobre enfoques técnicos y selección de casos, resultados clínicos en comparación con otras opciones de tratamiento y resultados en términos de calidad de vida y fertilidad.(AU)


Uterine fibroids are the most common tumour in reproductive age and have a negative impact on the quality of life of patients. Cases must be treated on an individual basis. Uterine artery embolization is a minimally invasive, safe, effective treatment with a low rate of complications when used as a treatment for symptomatic fibroids. The objective of this article is to conduct a comprehensive literature review on the current status of uterine artery embolization in the treatment of fibroids, including preliminary considerations on technical approaches and case selection clinical outcomes compared with other treatment options, and outcomes in terms of quality of life and fertility.(AU)

OBJECTIVES: To evaluate the safety and efficacy of the embolization of bronchial arteries and nonbronchial systemic arteries with n-butyl-cyanoacrylate (NBCA) in patients with hemoptysis. METHODS: We analyzed a total of 55 consecutive patients with hemoptysis (14 mild, 31 moderate, and 10 massive) treated with the embolization of bronchial arteries and nonbronchial systemic arteries with n-butyl-cyanoacrylate between November 2013 and January 2020. The main variables analyzed were the rates of technical success, of clinical success, of recurrence, and of complications. Statistics included a descriptive analysis and Kaplan-Meier survival curves. RESULTS: Embolization was a technical success in 55 (100%) and a clinical success in 54 (98.2%). During follow-up (mean, 23.8 months; interquartile range, 9.7-38.2 months), hemoptysis recurred in 5 (9.3%) patients. The nonrecurrence rate was 91.9% one year after the initial procedure and 88.7% two years and four years after the initial procedure. Minor complications related with the procedure occurred in 6 (10.9%); no major complications occurred. CONCLUSIONS: The embolization of bronchial arteries and nonbronchial systemic arteries with n-butyl-cyanoacrylate is safe and efficacious for controlling hemoptysis, resulting in low recurrence rates.

Objetivo: Determinar los resultados clínicos y angiográficos en pacientes con aneurismas intracraneales múltiples tratados endovascularmente en una única sesión. Materiales y Métodos: Se incluyó a todos los pacientes mayores de 18 años con aneurismas múltiples (≥2), rotos o no rotos, tratados con terapia endovascular en una única sesión entre 2019 y 2021. Se recolectaron los datos clínicos y angiográficos. Se determinó la tasa de oclusión inmediata y del seguimiento. La escala de Rankin modificado se usó para valorar el resultado clínico. Resultados: Se trataron 25 pacientes, de los cuales 14 se presentaron con hemorragia subaracnoidea. Se diagnosticaron un total de 78 aneurismas, de los cuales 59 aneurismas fueron tratados. La localización más frecuente fue el segmento oftálmico. La altura máxima promedio fue de 5.2mm, lo cual tuvo diferencia estadística significativa con el estado de ruptura (p ≤ 0.02). El principal tipo de tratamiento endovascular fue la técnica de remodeling en el 39 % de casos. El Raymond Roy inmediato fue I en el 60 % y IIIa en el 35 % de casos. La tasa de complicaciones fue del 24 % y de mortalidad fue del 8 %. Conclusiones: El tratamiento endovascular en una única sesión es una opción efectiva y segura en casos de aneurismas intracraneales múltiples en nuestra institución con tasa de oclusión y complicaciones aceptable.


Objective: To determine clinical and angiographical outcomes in patients with multiple intracranial aneurysms who underwent endovascular therapy in a single session. Materials and Methods: Patients older than 18 years with multiple (≥2) ruptured or non-ruptured aneurysms were included, and all of them underwent endovascular therapy in a single session between 2019 and 2021. Clinical and angiographic data was collected. Immediate occlusion and follow-up data were collected. Rankin modified scale was used for assessing clinical outcomes. Results: Twenty-five patients were treated, and fourteen had subarachnoid hemorrhage. Seventy-eight aneurysms were diagnosed, and 59 of them were treated. The most frequent location was at the ophthalmic segment. Maximum average height was 5.2- mm, which showed significant statistical difference with a ruptured condition (p≤0.02). The main modality for endovascular therapy was the remodeling technique, which was used in 39% of all cases. Immediate Raymond Roy staging was I in 60% of all cases, and IIIa in 35% of all cases. Complication rate was 24%, and mortality rate was 8%. Conclusions: Single session endovascular therapy is an effective and safe option for cases of multiple intracranial aneurysms in our institution. Occlusion and complication rates were acceptable.

Objetivos Evaluar la seguridad y la eficacia de la embolización de arterias bronquiales y arterias sistémicas no bronquiales con n-butil-cianoacrilato en pacientes con hemoptisis. Métodos Se han analizado un total de 55 pacientes consecutivos con hemoptisis (14 leves, 31 moderadas y 10 masivas) tratados mediante embolización de arterias bronquiales y arterias sistémicas no bronquiales con n-butil- cianoacrilato entre noviembre de 2013 y enero de 2020. Las variables principales estudiadas son tasa de éxito técnico, tasa de éxito clínico, tasas de recurrencia y complicaciones. Se ha realizado un análisis estadístico descriptivo y un análisis de supervivencia mediante el método de Kaplan-Meier. Resultados En 55 (100%) pacientes se ha realizado la embolización con éxito técnico y en 54 (98,2%), con éxito clínico. Durante el seguimiento (media, 23,8 meses; rango intercuartílico, 9,7-38,2) ha recurrido en 5 de los 54 (9,3%) pacientes. La tasa de no recurrencia al año ha sido del 91,9%, y a los 2 y 4 años, del 88,7% después del procedimiento inicial. Ha habido 6 (10,9%) complicaciones menores relacionadas con el procedimiento y ninguna mayor. Conclusiones La embolización de arterias bronquiales y arterias sistémicas no bronquiales con n-butil-cianoacrilato es segura y eficaz para controlar la hemoptisis con tasas de recurrencia bajas (AU)

Endovascular middle meningeal artery embolisation (eMMAE) has emerged as an alternative for those patients presenting health problems or multiple recurrences of CSDH. Despite several encouraging reports, the safety profile, indications, and limitations of the technique are not clearly established. DEVELOPMENT: This study aimed to evaluate the current evidence on eMMAE in patients with CSDH. We performed a systematic review of the literature, following the PRISMA guidelines. Our search yielded a total of 6 studies, in which a total of 164 patients with CSDH underwent eMMAE. The recurrence rate across all studies was 6.7%, and complications occurred in up to 6% of patients. CONCLUSIONS: eMMAE is a feasible technique for treating CSDH, with a relatively low recurrence rate and an acceptable rate of complications. Further prospective and randomised studies are needed to formally establish a clear profile of the safety and effectiveness of the technique.

Introducción: Dadas las altas tasas de recurrencia, el hematoma subdural crónico (HSDC) es una condición que supone un reto para el clínico. La embolización endovascular de la arteria meníngea media (EeAMM) ha surgido como una alternativa para aquellos pacientes con problemas de salud o con varias recurrencias de su HSDC. A pesar de la publicación de algunos artículos prometedores referentes a esta técnica, el perfil de seguridad, las indicaciones y las limitaciones no están claramente establecidas.DesarrolloNuestro objetivo es evaluar la evidencia actual sobre la EeAMM en el HSDC. Se realizó una revisión sistemática de la literatura según las guías PRISMA. Nuestra búsqueda arrojó un total de 6 estudios, en los que 164 pacientes con HSDC se sometieron a EeAMM. La tasa de recurrencia entre todos los estudios fue del 6,7% y se produjeron complicaciones en hasta el 6% de los pacientes.ConclusiónLa EeAMM es una técnica factible en el tratamiento del HSDC con una tasa de recurrencia relativamente baja y tasas aceptables de complicaciones. Se necesitan más estudios prospectivos y aleatorizados para establecer formalmente un perfil de seguridad claro y su efectividad. (AU)

El sangrado uterino anormal (SUA) es una de las patologías ginecológicas más comunes en mujeres en edad reproductiva. El sistema de clasificación de la Federación Internacional de Ginecología y Obstetricia (PALM-COEIN) armoniza las definiciones de los síntomas de hemorragia normal y anormal y clasifica las posibles causas subyacentes en causas estructurales y no estructurales. El objetivo de este manuscrito es revisar el diagnóstico radiológico de cada una de las causas estructurales de sangrado uterino anormal, examinar las indicaciones de las técnicas de radiología vascular intervencionista en el manejo del SUA y conocer el procedimiento de embolización de arterias uterinas, así como sus posibles complicaciones y contraindicaciones.(AU)


Abnormal uterine bleeding is one of the most common gynaecological pathologies in women of reproductive age. The classification system of the International Federation of Gynaecology and Obstetrics (PALM-COEIN) harmonizes the definitions of normal and abnormal bleeding symptoms and classifies possible underlying causes as structural and non-structural. The aim of this manuscript is to review the radiological diagnosis of each of the structural causes of abnormal uterine bleeding, to examine the indications for interventional vascular radiology techniques in the treatment of abnormal uterine bleeding, and to learn about the uterine artery embolization procedure, as well as its possible complications and contraindications.(AU)
