RESUMO

In August 2017, six international senior dental researchers published a critical declaration called 'La Cascada; Dentistry in Crisis: Time to Change'. This statement outlined major concerns with the current trajectory of the dental profession and proposed recommendations for change. This study piloted an online survey among an exploratory convenience sample of 138 Iranian dentists, ensuring a suitable gender mix and a representative mix of generalists and specialists. The questionnaire included two main questions about the respondents' agreement with the declaration and some of the recommendations. The majority (72.2%) of the 138 participants expressed agreement with the messages of the La Cascada Declaration by selecting 4 or 5 on a 5-point Likert scale. However, there was not a strong consensus among participants regarding all six selected recommendations. In conclusion, it appears that groups of dentists agree that the dental profession is facing a crisis. However, the profession has not yet reached a consensus on solutions.

PURPOSE: This study aimed to examine the health concerns of parents regarding their premature infants and to identify changes in these concerns during perinatal period and after discharge. METHODS: This was a retrospective study performed at a single tertiary center that enrolled 119 premature infants who were discharged from the neonatal intensive care unit (NICU) and visited the outpatient pediatrics department between December 2018 and October 2021. Data on the concerns of 176 parents regarding enrolled premature infants' health from before birth to 1 week after NICU discharge were obtained from outpatient records. The t test and with the chi-squared test were used to analyze the data for this study. RESULTS: The consistently greatest focus of parents' health concerns was the respiratory system. The second focus of parents' health concerns before discharge was the central nervous system. However, during the first week after NICU discharge, the gastrointestinal system was the second-most frequent focus of parents' health concerns among parents of infants without diseases related to prematurity and infants with older gestational ages. CONCLUSION: The results of this study offer insights into the health concerns among parents of premature infants. Parental health concerns about premature infants vary over time, from before birth to post-discharge, necessitating supportive interventions to enhance parental understanding of their child's health status.

Introducción: Durante la emergencia sanitaria, preocupan las repercusiones en la salud mental que pudieran estar experimentando los trabajadores sanitarios peruanos, quienes representan la primera línea de atención contra la COVID-19. Objetivo: Determinar si la preocupación por la COVID-19 y la carga laboral predicen el malestar psicológico en personal de salud. Métodos: Estudio predictivo en 367 trabajadores (enfermeros, médicos, técnicos en enfermería, obstetras, odontólogos, psicólogos, nutricionistas, entre otros) de ambos sexos en 12 redes de salud de la región de Puno, seleccionados a través de un muestreo no probabilístico intencional. Los datos se recogieron a través de la Escala de malestar psicológico de Kessler, la Escala de preocupación por el contagio de la COVID-19 y la Escala de carga de trabajo (ECT). Resultados: Se encontró que no hay diferencias significativas entre varones y mujeres en el malestar psicológico, la preocupación por el contagio de la COVID-19 y la carga laboral; además, se hallaron correlaciones muy significativas entre las variables de estudio (p < 0,01). El análisis de regresión múltiple demostró un ajuste adecuado para el modelo (F = 94,834; p < 0,001), en el que la preocupación por la COVID-19 (ß =-0,436; p < 0,01) y la carga laboral (ß = 0,239; p < 0,01) son variables que predicen significativamente el malestar psicológico (R2 ajustado = 0,33). Conclusiones: La preocupación por la COVID-19 y la sobrecarga de trabajo predicen el malestar psicológico en el personal sanitario de la región de Puno.

Introduction: The COVID-19 pandemic has brought a new normality, a scenario where different circumstances trigger emotions such as concern about returning to face-to-face classes, to which higher education students adapt. The objective was to identify how emotional intelligence and coping with stress explain students' concern about returning to face-to-face classes post COVID-19. Methods: This was an explanatory and cross-sectional study. The sample by intention was 588 students from the Faculty of Health Sciences. The Rotterdam Emotional Intelligence Scale (REIS), the Stress Coping Questionnaire (CAE), and the ACAD-COVID-19 scale were used. For data collection, the instruments were digitized. Bivariate analysis with Chi-square and multivariate logistic regression was performed. Results: The mean age of the participants was 19.72 years; most were single and without children (96.9% and 96.8%, respectively). 74.3% did not work, 80.8% were from the first years of study, and 52.7% belonged to nursing school. About 94.2% of participants indicated having adequate emotional intelligence, 91.3% expressed coping with adequate stress, and 67.2% indicated serious concern about returning to face-to-face classes. An association was found between gender (p=0.042), age (p=0.002), year of study (p=0.027), emotional intelligence (p=0.001), and coping with stress (p=0.001) with concern for return to face-to-face classes. Emotional intelligence identified as adequate (OR: 2.580; IC95%: 1.117-5.960) and coping with stress identified as adequate (OR: 2.008; IC95%: 1.018-3.960) are more likely to express serious concern about the return to face-to-face classes after the COVID-19 pandemic. Conclusion: According to the results, the need to safeguard the psychological aspects of students is highlighted, especially emotional intelligence, as well as to improve coping with stress so that they can better manage concerns about returning to face-to-face classes. The educational authorities should implement strategies to improve these aspects in order to ensure the adequate return to face-to-face classes in new scenarios.

In Mozambique, as in other parts of the world, COVID-19 has had consequences in many areas, especially in the health sector. This study aims to analyze the comments made and discussed on Facebook after the first death from the disease, verifying the main topics, concerns, and feelings that users most expressed on that social network. Using the content analysis method, IRaMuTeQ® generated a dendrogram focused on death, diagnostic circumstances, disease prevention, and restrictive government measures. Users also raised concerns about health care professionals, family and community, and feelings of sadness. Although the feelings presented reflect the lan-guage used in comments on a social network, and it is impossible to diagnose from them, this study creates paths for further research in the area. Therefore, for the first time, it demonstrates results from a mental health study with data analyzed from a social network in Mozambique. These results can guide and alert local health entities about health communications, strategies, and attention that should be given to the mental health of individuals during this pandemic and in the long term


En Mozambique, como en otras partes del mundo, el COVID-19 trajo consecuencias en muchas áreas, principalmente en el sector de la salud. Este estudio tiene como objetivo analizar los comentarios realizados y discutidos en Facebook después de la primera muerte por la enfermedad, con el fin de verificar los principales temas, preocupaciones y sentimientos que los usuarios más expresaron en esa red social. Utilizando el método de análisis de contenido, IRaMuTeQ® generó un dendrograma con temas enfocados en las circunstancias de la muerte, las circunstancias del diagnóstico, la prevención de enfermedades, y las medidas restrictivas del gobierno. También hubo preocupaciones con los profesionales de la salud, la familia y la comunidad, además de sentimientos de tristeza. Aunque los sentimientos presentados reflejan el lenguaje utilizado en los comentarios en una red social, y no es posible dar un diagnóstico basado en ellos, este estudio abre caminos para futuras investigaciones en el área. Por lo tanto, por primera vez, se demuestra un estudio de salud mental con datos analizados en una red social en Mozambique, y puede servir como ayuda y alerta a las entidades locales de salud sobre comunicaciones de salud, estrategias y atención que se debe dar a la salud mental de las personas durante esta pandemia y a largo plazo

Objective. The work sought to determine the effect of the risk factors, fear, and concern on self-care regarding COVID-19 in people in pre-elderly and elderly stages. Methods. Correlational-predictive study, gathered through convenience sampling. The study applied the scale of fear of COVID-19(Huarcaya et al.), the scale of concern about COVID-19 (Ruíz et al.,), and the scale of self-care during the COVID-19 confinement (Martínez et al.,). Descriptive and inferential statistics were applied as the mediation model based on regression. Results. The study had the participation of 333 people, with the majority being women (73.9%). Correlation was found between self-care with the scores from the scale of fear (r = -0.133, p<0.05) and of concern (r = -0.141, p<0.05) regarding COVID-19. The direct effect of the model was c' = 0.16, [95% BCa CI = -0.28, -0.09]. The standardized value for the indirect effect was estimated as c = -0.14, [95% BCa CI= -0.23,-0.09]), which shows existence of a 14.0% effect of the mediating variable on self-care conducts in the prediction model. Conclusion. A direct effect exists of risk factors for COVID-19 complication on self-care, mediated by concern and fear, besides explaining by 14% the self-care conducts for COVID-19. Recommendation is made to address other emotional variables to consider if these increase the prediction.


Objetivo. Determinar el efecto de los factores de riesgo, temor y preocupación sobre el autocuidado ante la COVID-19 en personas en etapa de prevejez y vejez. Métodos. Estudio correlacional-predictivo, recolectado mediante un muestreo a conveniencia. Se aplicó la escala de temor ante el COVID-19(Huarcaya et al.), la escala de preocupación ante el COVID-19 (Ruíz et al.) y la escala de autocuidado durante el confinamiento de COVID-19 (Martínez et al.). Se aplicó estadística descriptiva e inferencial como el modelo de mediación basado en regresión. Resultados. Participaron 333 personas, en su mayoría mujeres, 73.9%. Se halló correlación entre el autocuidado con los puntajes de la escala de temor (r=-0.133, p<0.05) y de preocupación (r=-0.141, p<0.05) ante la COVID-19. El efecto directo del modelo fue de c'=0.16, [95% BCa CI =-0.28,-0.09]. Por su parte, el valor estandarizado para el efecto indirecto se estimó en c=-0.14, [95% BCa CI= -0.23,-0.09]), lo que muestra la existencia de un efecto de 14.0% de la variable mediadora sobre las conductas de autocuidado en el modelo predictor. Conclusión. Existe un efecto directo de los factores de riesgo para complicación de la COVID-19 sobre el autocuidado, mediado por la preocupación y el temor, aunado a que explica en un 14% la conducta de autocuidado para la COVID-19. Se recomienda abordar otras variables emocionales para considerar si estas aumentan la predicción.

Estatísticas descritivas e inferenciais foram aplicadas como modelo de mediação baseado em regressão. Resultados.Participaram 333 pessoas, sendo a maioria mulheres 73,9%. Foi encontrada correlação entre o autocuidado e os escores da escala de medo (r=-0.133, p<0.05) e preocupação (r=-0.141, p<0.05) diante da COVID-19. O efeito direto do modelo foi c'=0.16, [95% BCa CI =-0.28,-0.09]. Por sua vez, o valor padronizado para o efeito indireto foi estimado em c=-0.14, [95% BCa CI= -0.23,-0.09]), o que mostra a existência de um efeito de 14.0% da variável mediadora sobre o self -comportamentos de cuidado no modelo preditor. Conclusão. Há efeito direto dos fatores de risco para complicações da COVID-19 no autocuidado, mediado pela preocupação e medo, além de explicar 14% do comportamento de autocuidado para a COVID-19. Recomenda-se abordar outras variáveis emocionais para considerar se elas aumentam a previsão.

Expressions of concern (EoC) can reduce the adverse effects of unreliable publications by alerting readers to concerns about publication integrity while assessment is undertaken. We investigated the use of EoC for 463 publications by two research groups for which we notified concerns about publication integrity to 142 journals and 44 publishers between March 2013 and February 2020. By December 2021, 95 papers had had an EoC, and 83 were retracted without an EoC. Median times from notification of concerns to EoC (10.4mo) or retraction without EoC (13.1mo) were similar. Among the 95 EoCs, 29 (30.5%) were followed by retraction after a median of 5.4mo, none was lifted, and 66 (69.5%) remained in place after a median of 18.1mo. Publishers with >10 notified publications issued EoCs for 0-81.8% of papers: for several publishers the proportions of notified papers for which EoCs were issued varied considerably between the 2 research groups. EoCs were issued for >30% of notified publications of randomized clinical trials and letters to the editor, and <20% of other types of research. These results demonstrate inconsistent application of EoCs between and within publishers, and prolonged times to issue and resolve EoCs.

Scientific publications with compromised integrity should be retracted. Papers citing retracted publications might need correction if findings depend on the retracted publication. While many studies have reported on post-retraction citations, few have focused on citations made before the retraction. We investigated the citation profile for a research group with 113 published concerns regarding publication integrity (CRPI). We identified 376 of their source publications that were cited by 5577 articles, and whether the source publication had a published CRPI. Of 6926 references to a source publication in these citing articles, for 3925 (57%) the source article had a published CRPI, while for 3001 (43%) it did not. Of these 3925 references, 3688 were in citing articles published before the source article CRPI was published. 166 citing articles containing 198 references to source publications were published after the corresponding source article CRPI was published (range 1-5 such references/article; 19/166 (11%) articles had >1 reference). In summary, many articles cite retracted publications, with the majority of these references occurring before the retraction. However, very few publications assess the impact of the retracted citations, even though the findings of many might be altered, at least in part, by removal of the retracted citation.

Publications of expressions of concern and retractions should be timely, accurate and comprehensive. We as sessed these characteristics for 292 publications by a research group about which we submitted concerns about publication integrity to 77 journals and 29 publishers between March 2013 and February 2020. By October 2020, 115 publications were corrected (3), had expressions of concern (18), or were retracted (94). The median (95% CI) time from submission of concerns to the first journal correction was 22.1 (18.2-26.9) months: this did not diminish by year of submission of concerns, varied between publishers, and was shorter for journals with higher impact factors. Eighty-four publications of original research were retracted. The median (range) proportion of concerns raised with the journal that were mentioned in the ensuing retraction notices was 9.5% (2-49). At least 75% of retraction notices included the suggested content for 7/9, 3/9 and 3/16 items in the Committee for Publication Ethics and Retraction Watch minimum and optimal recommended formats, respectively. Thus, assessment of concerns about publication integrity was delayed and incomplete. Adherence to recommended content of retraction notices was moderate at best. Strategies are needed to improve the efficiency, accuracy and transparency of processes for resolving concerns about publication integrity.

BACKGROUND: The COVID-19 pandemic gave rise to concerns about its potential impact on the mental health of pregnant and postpartum women as the general postpartum depression rates increased. METHODS: Three postpartum questionnaires (Edinburgh Postnatal Depression Scale - EPDS; Anxiety and Depression Scale - HAD; and a demographic questionnaire about isolation and information acquisition) were used to evaluate the mental health of women with and without COVID-19 and determine the factors associated with depressive symptoms (EPDS ≥ 12). RESULTS: Data from 184 participants with a mean of 56 postpartum days were analyzed. The rate of symptoms compatible with postpartum depression (PPD) was 38.8%, with a 14.3% positive response to item 10 on the EPDS (suicidal ideation - SI). Listening to the news about COVID-19 averaged 4.45 hours a day. Factors related to PPD were concerns about lack of hospital beds (OR = 2.45), absence of a partner (OR = 2.70), and anxiety symptoms (OR = 10). Factors related to SI were anxiety symptoms (OR = 1.56) and friends as a source of information (OR = 5.60). LIMITATIONS: Considering the rapidly changing epidemiological conditions of this pandemic, this study may only be the photograph of an instant. CONCLUSIONS: Higher rates of PPD in the Brazilian population are related not only to anxiety but also to an inadequate family environment, kind of information acquisition and concerns about the lack of hospital beds.

OBJECTIVE: A general practitioner's (GP's) suspicion of cancer is important to ensure early diagnosis of cancer. This study aimed to investigate the association between patients' cancer worry and GP's suspicion of cancer or serious illness. METHODS: This population-based study was based on Danish register and questionnaire data from 4,175 incident cancer patients diagnosed in 2010 or 2016 and their GPs. The association between the patient's worry about cancer and their GP's suspicion of cancer or serious illness was estimated by generalised linear models with log link for the Poisson family and presented in prevalence rate ratios (PRR). RESULTS: For 6 in 10 of the cases, the first consultation was categorised by absence of both or either patient worry or GP suspicion. When patients worried about cancer, the GPs were more likely to suspect cancer or serious illness in patients (PRR=1.26, (95%CI: 1.20-1.34)). For all levels of patient worry and most pronounced among very worried patients, GPs less often suspected cancer or serious illness in patients who presented with an ´intermediate´ or ´hard´ to diagnose cancer. CONCLUSION: GPs were more likely to suspect cancer or serious illness in patients who worried about cancer at the first presentation.

The current system for assessing and publicly notifying concerns about publication integrity is slow, inefficient, inconsistent, inadequate, and opaque. Readers are, therefore, left unaware of potential issues about publications or are given inadequate information to assess publication integrity. We propose a new process for dealing with publication integrity involving the establishment of independent panel(s) that assess publication integrity and transparently report the outcomes of those assessments, independent of the assessment of any misconduct.

OBJECTIVES: Nurse concerns documented in nursing notes are important predictors of patient risk of deterioration. Using a standard nursing terminology and inputs from subject-matter experts (SMEs), we aimed to identify and define nurse concern concepts and terms about patient deterioration, which can be used to support subsequent automated tasks, such as natural language processing and risk predication. METHODS: Group consensus meetings with nurse SMEs were held to identify nursing concerns by grading Clinical Care Classification (CCC) system concepts based on clinical knowledge. Next, a fundamental lexicon was built placing selected CCC concepts into a framework of entities and seed terms to extend CCC granularity. RESULTS: A total of 29 CCC concepts were selected as reflecting nurse concerns. From these, 111 entities and 586 seed terms were generated into a fundamental lexicon. Nursing concern concepts differed across settings (intensive care units versus non-intensive care units) and unit types (medicine versus surgery units). CONCLUSIONS: The CCC concepts were useful for representing nursing concern as they encompass a nursing-centric conceptual framework and are practical in lexicon construction. It enabled the codification of nursing concerns for deteriorating patients at a standardized conceptual level. The boundary of selected CCC concepts and lexicons were determined by the SMEs. The fundamental lexicon offers more granular terms that can be identified and processed in an automated fashion.

Objetivo: evidenciar as preocupações vivenciadas por pacientes com câncer em estágio avançado. Metodologia: estudo qualitativo desenvolvido com 11 pacientes oncológicos em cuidados paliativos. Os dados foram coletados em Maringá ­ PR, no período de 2016 a 2017, mediante entrevistas abertas, abordando as experiências e enfrentamentos vivenciados durante o processo de terminalidade da vida, das quais foram extraídos os discursos e submetidos à análise de conteúdo após aprovação pelo Comitê de Ética em Pesquisa. Resultados: o medo de tornar-se um incômodo e as incertezas de como a vida continuará após sua partida foram questões hegemônicas dos pacientes, que encontraram na aceitação da morte um momento de reflexão sobre a vida, despontando como uma probabilidade positiva de resiliência e esperança. Conclusão: as principais preocupações manifestadas foram: viver melhor o agora, aproximar-se mais daqueles que lhes fazem bem, preocupando-se com o bem-estar de seus familiares e como eles ficarão após sua partida, transcendendo assim o seu próprio sofrimento.


Objective: to highlight the concerns experienced by patients with advanced cancer. Methodology: qualitative study developed with 11 cancer patients in palliative care. The data were collected in Maringá - PR, from 2016 to 2017, through open interviews, addressing the experiences and confrontations experienced during the terminal life process, from which the speeches were extracted and submitted to content analysis after approval by the Committee Research Ethics. Results: the fear of becoming a nuisance and the uncertainties of how life will continue after his departure, were hegemonic issues of the patients, who found in the acceptance of death a moment of reflection on life, emerging as a positive probability of resilience and hope. Conclusion: the main concerns expressed were: to live better now, to get closer to those who do them well, worrying about the well-being of their families and how they will be after their departure, thus transcending their own suffering.


Objetivo: resaltar las preocupaciones experimentadas por los pacientes con cáncer avanzado. Metodología: estudio cualitativo desarrollado con 11 pacientes con cáncer en cuidados paliativos. Los datos se recopilaron en Maringá - PR, de 2016 a 2017, a través de entrevistas abiertas, abordando las experiencias y confrontaciones experimentadas durante el proceso de vida terminal, de donde se extrajeron los discursos y se sometieron a análisis de contenido después de la aprobación del Comité Ética de Investigación. Resultados: el miedo a convertirse en una molestia y las incertidumbres de cómo continuará la vida después de su partida, fueron problemas hegemónicos de los pacientes, quienes encontraron en la aceptación de la muerte un momento de reflexión sobre la vida, emergiendo como una probabilidad positiva de resiliencia y Espero Conclusión: las principales preocupaciones expresadas fueron: vivir mejor ahora, acercarse a quienes son buenos para ellos, preocuparse por el bienestar de su familia y cómo estarán después de su partida, trascendiendo así su propio sufrimiento.

BACKGROUND: Parents are likely to seek Web-based communities to verify their suspicions of autism spectrum disorder markers in their child. Automated tools support human decisions in many domains and could therefore potentially support concerned parents. OBJECTIVE: The objective of this study was to test the feasibility of assessing autism spectrum disorder risk in parental concerns from Web-based sources, using automated text analysis tools and minimal standard questioning. METHODS: Participants were 115 parents with concerns regarding their child's social-communication development. Children were 16- to 30-months old, and 57.4% (66/115) had a family history of autism spectrum disorder. Parents reported their concerns online, and completed an autism spectrum disorder-specific screener, the Modified Checklist for Autism in Toddlers-Revised, with Follow-up (M-CHAT-R/F), and a broad developmental screener, the Ages and Stages Questionnaire (ASQ). An algorithm predicted autism spectrum disorder risk using a combination of the parent's text and a single screening question, selected by the algorithm to enhance prediction accuracy. RESULTS: Screening measures identified 58% (67/115) to 88% (101/115) of children at risk for autism spectrum disorder. Children with a family history of autism spectrum disorder were 3 times more likely to show autism spectrum disorder risk on screening measures. The prediction of a child's risk on the ASQ or M-CHAT-R was significantly more accurate when predicted from text combined with an M-CHAT-R question selected (automatically) than from the text alone. The frequently automatically selected M-CHAT-R questions that predicted risk were: following a point, make-believe play, and concern about deafness. CONCLUSIONS: The internet can be harnessed to prescreen for autism spectrum disorder using parental concerns by administering a few standardized screening questions to augment this process.

BACKGROUND: An editorial expression of concern (EEoC) is issued by editors or publishers to draw attention to potential problems in a publication, without itself constituting a retraction or correction. METHODS: We searched PubMed, PubMed Central (PMC), and Google Scholar to identify EEoCs issued for publications in PubMed and PMC up to 22 August 2016. We also searched the archives of the Retraction Watch blog, some journal and publisher websites, and studies of EEoCs. In addition, we searched for retractions of EEoCs and affected articles in PubMed up to 8 December 2016. We analyzed overall historical trends, as well as reported reasons and subsequent editorial actions related to EEoCs issued between August 2014 and August 2016. RESULTS: After screening 5,076 records, we identified 230 EEoCs that affect 300 publications indexed in PubMed, the earliest issued in 1985. Half of the primary EEoCs were issued between 2014 and 2016 (52%). We found evidence of some EEoCs that had been removed by the publisher without leaving a record and some were not submitted for PubMed or PMC indexing. A minority of publications affected by EEoCs had been retracted by early December 2016 (25%). For the subset of 92 EEoCs issued between August 2014 and August 2016, affecting 99 publications, the rate of retraction was similar (29%). The majority of EEoCs were issued because of concerns with validity of data, methods, or interpretation of the publication (68%), and 31% of cases remained open. Issues with images were raised in 40% of affected publications. Ongoing monitoring after the study identified another 17 EEoCs to year's end in 2016, increasing the number of EEoCs to 247 and publications in PubMed known to be affected by EEoCs to 320 at the end of 2016. CONCLUSIONS: EEoCs have been rare publishing events in the biomedical literature, but their use has been increasing. Most have not led to retractions, and many remain unresolved. Lack of prominence and inconsistencies in management of EEoCs reduce the ability of these notices to alert the scientific community to potentially serious problems in publications. EEoCs will be made identifiable in PubMed in 2017.
