BACKGROUND AND OBJECTIVES: Chronic subdural hematoma (CSDH) is one of the most common pathologies in our daily practice. The standard treatment is the evacuation making a burr-hole and placement of a subdural drainage, which has shown to decrease its recurrence. However, this procedure can entail risks such as parenchymal damage, infection, or the onset of seizures, prompting the consideration of subgaleal drainage as an alternative. Our objective is to compare the use of subdural and subgaleal drainage in a cohort of patients undergoing intervention for CSDH, as well as to analyze the differences in complication rates and recurrence between the two groups. METHODOLOGY: A retrospective analytical observational study was conducted, analyzing 152 patients diagnosed with CSDH who underwent intervention at our center from January 2020 to April 2022. Patients in whom drainage was not placed were excluded. In all patients, a burr-hole was performed and the type of drainage was chosen by the neurosurgeon. RESULTS: Out of the 152 patients, subdural drainage was placed in 80 cases (52.63%), while subgaleal drainage was used in 72 cases (47.37%). There were no significant differences in the recurrence rate (30% in the subdural drainage group vs. 20.83% in the subgaleal drainage group; p = 0.134) or in the complication rate (7.5% in the subdural drainage group vs. 5.5% in the subgaleal drainage group; p = 0.749). CONCLUSIONS: Subgaleal drainage shows similar clinical outcomes with a recurrence and complication rate comparable to subdural drainage, suggesting it as a safe and effective alternative to subdural drainage in the treatment of CSDH.

Introducción: El trauma craneoencefálico (TCE) se define como una patología caracterizada por la alteración cerebral secundaria a una lesión traumática en la región de la cabeza, con la presencia de alteración de la consciencia y/o amnesia debido al trauma, cambios neurológicos, neurofisiológicos, con posibles fracturas de cráneo o lesiones intra craneanas atribuibles al trauma. Metodología: Es un estudio observacional, descriptivo, prospectivo en pacientes que sufrieron TCE internados en el servicio de neurocirugía en el Complejo Hospitalario Dr. Arnulfo Arias Madrid durante el periodo de marzo de 2022 a febrero de 2023. Resultados: Los resultados de este estudio mostraron que predomina el sexo masculino (78%) sobre el femenino (22%) en pacientes con TCE. Las causas de lesión más comunes fueron caídas de sus pies (27%), caídas de altura (25%) y colisión de moto/automóvil (20%). Las presentaciones clínicas más comunes fueron pérdida de consciencia (49%), amnesia/desorientación (19%) y cefalea (10%). Conclusión: El seguimiento promedio de los pacientes con TCE fue de 16 días. El género masculino fue el más frecuente, con mayor proporción en la edad media y adulta mayor. La etiología principalmente fue por caídas, manifestándose comúnmente por perdida del estado de alerta, y teniendo hallazgos múltiples en la tomografía cerebral. Introduction: Cranioencephalic trauma (TBI) is defined as a pathology characterized by cerebral alteration secondary to traumatic injury in the head region, with the presence of altered consciousness and/or amnesia due to trauma, neurological, neurophysiological changes, with possible skull fractures or intracranial lesions attributable to trauma. Methodology: This is an observational, descriptive, prospective study in patients who suffered TBI hospitalized in the neurosurgery service at the Complejo Hospitalario Dr. Arnulfo Arias Madrid during the period from March 2022 to February 2023. Results: The results of this study showed a predominance of male (78%) over female (22%) patients with TBI. The most common causes of injury were falls from their feet (27%), falls from height (25%) and motorcycle/automobile collision (20%). The most common clinical presentations were loss of consciousness (49%), amnesia/disorientation (19%) and headache (10%). Conclusion: The average follow-up of patients with TBI was 16 days. The male gender was the most frequent, with a higher proportion in middle age and older adults. The etiology was mainly due to falls, commonly manifested by loss of alertness, and multiple findings in brain tomography. Most patients were managed conservatively. OBJECTIVE: Chronic subdural hematoma (CSDH) patients using antithrombotic agents (AT) at high risk for cardiovascular disease are increasing. The authors aimed to analyze the factors influencing outcome by targeting patients using AT and to establish a desirable treatment strategy. METHODS: A retrospective analysis was performed on data from 462 patients who underwent burr hole trephination (BHT) surgery for CSDH at five hospitals from March 2010 to June 2021. Outcomes included incidence of postoperative acute bleeding, recurrence rate, and morbidity or mortality rate. Patients were divided into the following four groups based on their history of AT use : no AT. Only antiplatelet agents (AP), only anticoagulants (AC), both of AP and AC. In addition, a concurrent literature review was conducted alongside our cohort study. RESULTS: Of 462 patients, 119 (119/462, 25.76%) were using AT. AP prescription did not significantly delay surgery (p=0.318), but AC prescription led to a significant increase in the time interval from admission to operation (p=0.048). After BHT, AP or AC intake significantly increased the period required for an in-dwelling drain (p=0.026 and p=0.037). The use of AC was significantly related to acute bleeding (p=0.044), while the use of AP was not (p=0.808). Use of AP or AC had no significant effect on CSDH recurrence (p=0.517 and p=1.000) or reoperation (p=0.924 and p=1.000). Morbidity was not statistically correlated with use of either AP or AC (p=0.795 and p=0.557, respectively), and there was no significant correlation with mortality for use of these medications (p=0.470 and p=1.000). CONCLUSION: Elderly CSDH patients may benefit from maintenance of AT therapy during BHT due to reduced thromboembolic risk. However, the use of AC necessitates individualized due to potential postoperative bleeding. Careful post-operative monitoring could mitigate prognosis and recurrence impacts.

We report a fatal subdural empyema caused by Campylobacter rectus in a 66-year-old female who developed acute onset of confusion, dysarthria, and paresis in her left extremities. A CT scan showed hypodensity in a crescentic formation with a mild mid-line shift. She had a bruise on her forehead caused by a fall several days before admission, which initially raised subdural hematoma (SDH) diagnosis, and a burr hole procedure was planned. However, her condition deteriorated on the admission night, and she died before dawn. An autopsy revealed that she had subdural empyema (SDE) caused by Campylobacter rectus and Slackia exigua. Both microorganisms are oral microorganisms that rarely cause extra-oral infection. In our case, head trauma caused a skull bone fracture, and sinus infection might have expanded to the subdural space causing SDE. CT/MRI findings were not typical for either SDH or SDE. Early recognition of subdural empyema and prompt initiation of treatment with antibiotics and surgical drainage is essential for cases of SDE. We present our case and a review of four reported cases.

OBJECTIVE: A chronic subdural hematoma (CSDH) is a collection of bloody fluid located in the subdural space and encapsulated by neo-membranes. An inner subdural hygroma (ISH) is observed between the inner membrane of a CSDH and the brain surface. We present six cases of CSDH combined with ISH treated via endoscopy. METHODS: Between 2011 and 2022, among the 107 patients diagnosed with CSDH in our institute, six patients were identified as presenting with CSDH combined with ISH and were included in this study. Preoperative computerized tomography (CT) and magnetic resonance imaging (MRI) were performed simultaneously, and endoscopic surgery for aspiration of the hematoma was performed in all cases of CSDH combined with ISH. RESULTS: The mean age of patients was 71 years (range, 66 to 79). The patients were all male. In two cases, the ISH was not identified on CT, but was clearly seen on MRI in all patients. The inner membrane of the CSDH was tense and bulging after draining of the CSDH in endoscopic view due to the high pressure of the ISH. After fenestration of the inner membrane of the CSDH and aspiration of the ISH, the membrane was sunken down due to the decreasing pressure of the ISH. There was one recurrence in post-operative 2-month follow up. The symptoms improved in all patients after surgery, and there were no surgery-related complications. CONCLUSION: CSDH combined with ISH can be diagnosed on imaging, and endoscopic surgery facilitates safe and effective treatment.

Introduction: Chronic subdural hematoma is a frequent type of intracranial hemorrhage. Treatment for symptomatic cases is generally surgical. Burr-hole completion is the method of choice. Objective: To analyze the placement of a single versus double drain in patients with chronic subdural hematoma operated through burr-holes. Methods: An analytical correlational retrospective study was carried out in patients who underwent surgery for chronic subdural hematoma, between January 2018 and December 2020. The patients were divided into 2 groups based on the number of drains used: double or single. The variables were: age, morbidities, preoperative Glasgow Score, hematoma characteristics (laterality, internal architecture, maximum diameter, and midline shift), complications, and hospital stay. The probability value < 0.05 was considered statistically significant. Results: Of the 99 patients in the study, 39 were older than 75 years. The most frequent pattern was B with 25 cases (25,3 %). The diameter of the chronic subdural hematoma was more than 20 mm in 61 cases, and in 32 cases the displacement of the midline structures was greater than 10 mm. The average hospital stays 5,8 days. The most frequent complication was nosocomial pneumonia with eleven cases (11,11 %). The double drainage group showed fewer complications at 30 days. Conclusions: The placement of double subdural drainage in patients with chronic subdural hematoma operated through single or double burr-holes, and is related to fewer complications than those with single drainage.


Introducción: El hematoma subdural crónico es un tipo frecuente de hemorragia intracraneal. El tratamiento de los casos sintomáticos es generalmente quirúrgico. La terminación con agujeros de trépano es el método de elección. Objetivo: Analizar la colocación de drenaje simple versus doble en pacientes con hematoma subdural crónico operados mediante trepanación. Métodos: Se realizó un estudio retrospectivo correlacional analítico en pacientes operados de hematoma subdural crónico, entre enero de 2018 y diciembre de 2020. Los pacientes se dividieron en 2 grupos según el número de drenajes utilizados: doble o simple. Las variables fueron: edad, morbilidades, puntaje de Glasgow preoperatorio, características del hematoma (lateralidad, arquitectura interna, diámetro máximo y desviación de la línea media), complicaciones y estancia hospitalaria. El valor de probabilidad < 0,05 se consideró estadísticamente significativo. Resultados: De los 99 pacientes del estudio, 39 eran mayores de 75 años. El patrón más frecuente fue el B con 25 casos (25,3 %). El diámetro del hematoma subdural crónico fue mayor de 20 mm en 61 casos, y en 32 casos el desplazamiento de las estructuras de la línea media fue mayor de 10 mm. La estancia hospitalaria media fue de 5,8 días. La complicación más frecuente fue la neumonía nosocomial con once casos (11,11 %). El grupo de doble drenaje presentó menos complicaciones a los 30 días. Conclusiones: La colocación de drenaje subdural doble en pacientes con hematoma subdural crónico operados mediante trepanación simple o doble, se relaciona con menos complicaciones que aquellos con drenaje simple.

Introducción: Los traumatismos craneoencefalicos (TCE) son lesiones físicas producidas sobre el tejido cerebral cuando una fuerza cinética externa intercambia energía con este. Los hematomas subdurales agudos a menudo requieren intervención quirúrgica (craniectomía descompresiva9 con apertura amplia de la dura madre, por lo que es importante desarrollar nuevas técnicas que puedan reducir complicaciones inmediatas o tardías de este procedimiento. Conclusión: Esta técnica ha demostrado un mejor pronóstico funcional con grados de secuelas, diferencia en los días de estancia hospitalaria, y en la Unidad de Terapia Intensiva en nuestra población.

Endovascular middle meningeal artery embolisation (eMMAE) has emerged as an alternative for those patients presenting health problems or multiple recurrences of CSDH. Despite several encouraging reports, the safety profile, indications, and limitations of the technique are not clearly established. DEVELOPMENT: This study aimed to evaluate the current evidence on eMMAE in patients with CSDH. We performed a systematic review of the literature, following the PRISMA guidelines. Our search yielded a total of 6 studies, in which a total of 164 patients with CSDH underwent eMMAE. The recurrence rate across all studies was 6.7%, and complications occurred in up to 6% of patients. CONCLUSIONS: eMMAE is a feasible technique for treating CSDH, with a relatively low recurrence rate and an acceptable rate of complications. Further prospective and randomised studies are needed to formally establish a clear profile of the safety and effectiveness of the technique.

Introducción: Dadas las altas tasas de recurrencia, el hematoma subdural crónico (HSDC) es una condición que supone un reto para el clínico. La embolización endovascular de la arteria meníngea media (EeAMM) ha surgido como una alternativa para aquellos pacientes con problemas de salud o con varias recurrencias de su HSDC. A pesar de la publicación de algunos artículos prometedores referentes a esta técnica, el perfil de seguridad, las indicaciones y las limitaciones no están claramente establecidas.DesarrolloNuestro objetivo es evaluar la evidencia actual sobre la EeAMM en el HSDC. Se realizó una revisión sistemática de la literatura según las guías PRISMA. Nuestra búsqueda arrojó un total de 6 estudios, en los que 164 pacientes con HSDC se sometieron a EeAMM. La tasa de recurrencia entre todos los estudios fue del 6,7% y se produjeron complicaciones en hasta el 6% de los pacientes.ConclusiónLa EeAMM es una técnica factible en el tratamiento del HSDC con una tasa de recurrencia relativamente baja y tasas aceptables de complicaciones. Se necesitan más estudios prospectivos y aleatorizados para establecer formalmente un perfil de seguridad claro y su efectividad. (AU)

OBJECTIVE: Chronic subdural hematoma (CSDH) is a common neurosurgical disease and generally treated with burr-hole surgery alone. Tranexamic acid (TXA) is an antifibrinolytic agent that potentially reduces recurrence rates and the residual hematoma volume. However, the role of postoperative TXA medication remains unclear to date. This study aimed to verify the effectiveness of adjunctive TXA in the view of early hematoma resolution. METHODS: Between January 2018 and September 2021, patients with CSDH who underwent burr-hole trephination in a single tertiary institute were reviewed. The study population was divided into three groups, TXA, non-TXA, and antithrombotics (AT) groups, according to the medical history of cardio-cerebrovascular disease and TXA administration. The primary endpoint was CSDH recurrence, defined as re-appearance or re-accumulation of CSDH requiring neurosurgical interventions. The secondary outcome was CSDH resolution, defined as complete or near-complete resorption of the CSDH. The CSDH resolution time and serial changes of hematoma thickness were also investigated. RESULTS: A total of 240 patients was included in the analysis consisting of 185 male and 55 female, with a median age of 74 years. During the median imaging follow-up period of 75 days, 222 patients were reached to the primary or secondary endpoint. TXA was administered as an adjunctive therapy in 41 patients (TXA group, 16.9%) while 114 patients were included in the non-TXA group (47.9%) and 85 were in the AT group. The recurrence rate was the lowest in the TXA group (2.4%), followed by non-TXA (7.0%) and AT (8.2%) groups. However, there was no statistical significance due to the small number of patients with recurrence. CSDH resolution was achieved in 206 patients, and the median estimated time to resolution was significantly faster in the TXA group (p<0.001). Adjunctive TXA administration was a significant positive factor for achieving CSDH resolution (p<0.001). The hematoma thickness was comparable among the three groups at the initial time and after surgery. However, CSDH thickness in the TXA group decreased abruptly in a month and showed a significant difference from that in the other groups (p<0.001). There was no TXA-related adverse event. CONCLUSION: The adjunctive use of TXA after CSDH surgery significantly facilitated the resorption of residual CSDH and resulted in the early CSDH resolution. Adjunctive TXA may be an effective treatment option to reduce recurrence by enhancing CSDH resolution in the selective patients.

Introducción: la punción lumbar es una de las herramientas diagnósticas más importantes para diferentes patologías neurológicas. Caso clínico: hombre de 32 años, con epilepsia, toxoplasmosis cerebral y VIH que presentó un hematoma subdural posterior a una punción lumbar diagnóstica, sin sintomatología, siendo un hallazgo imagenológico incidental. Discusión y conclusiones: el mecanismo por el que se produce el hematoma subdural, se basa en una presión baja del LCR que da lugar a un deslizamiento de la aracnoides sobre la duramadre y como consecuencia se desgarran los vasos del espacio subdural, produciendo una extravasación de sangre en dicho espacio.


Introduction: lumbar puncture is one of the most important diagnostic tools for a variety of neurological disorders. Case presentation: a 32-year-old man with epilepsy, cerebral toxoplasmosis, and HIV, who presented an asymptomatic subdural hematoma following a diagnostic lumbar puncture, which was an incidental finding on imaging. Discussion and conclusions: the mechanism for post lumbar puncture subdural hematoma development is based on a low CSF pressure which allows the arachnoid matter to slide over the dura mater leading to subdural vessels tear, causing blood extravasation into the subdural space.

Introducción: El hematoma subdural crónico es la complicación tardía más frecuente del trauma craneoencefálico. Su diagnóstico precoz y el tratamiento oportuno permiten la evolución favorable y curación de los pacientes. Objetivo: Sistematizar los diferentes ejes de clasificación del hematoma subdural crónico, que permitan la evaluación multimodal con una orientación más precisa de la técnica quirúrgica. Métodos: Se realizó una revisión bibliográfica en las principales bases de datos disponibles, centrada en las diferentes clasificaciones clínicas y radiológicas del hematoma subdural crónico y se seleccionaron 26 artículos. Se escogieron las principales clasificaciones y escalas y se aplicaron en 22 pacientes, obteniéndose resultados preliminares. Resultados: Las escalas de evaluación clínica de Bender, Marckwalder y la clasificación de Gordon Firing son las más conocidas, sin embargo, existen clasificaciones tomográficas que tienen en cuenta la densidad, la homogeneidad del hematoma que son de gran importancia en la selección de la técnica quirúrgica. La presencia de membranas y tabiques aparecen solo incluidas dentro de la valoración de la densidad del hematoma. Existen otras con valor predictivo que identifican aquellos factores relacionados con las recurrencias, desde antes de la cirugía, las mismas se consideran un factor de mal pronóstico para la evolución final de los pacientes con este tipo de hematomas. Conclusiones: La evaluación preoperatoria exige de la aplicación de múltiples escalas, la identificación y caracterización de las membranas es importante para personalizar la técnica quirúrgica, en busca de disminuir la morbilidad y mortalidad posoperatoria.


Introduction: Chronic subdural hematoma is the most frequent late complication of cranioencephalic trauma. The early diagnosis and correct treatment allow the recovery of patients. Objective: To systematize the different criteria of classification of chronic subdural hematoma that leads the multimodal evaluation for more effective selection of surgical technique. Methods: A review of literature about chronic subdural hematoma was done in the main database focused on different clinical and radiological classifications and 26 articles were selected. The main classifications and scales were chosen and applied to 22 patients, obtaining preliminary results. Results : Benders, Marckwalder and Gordon Firing scales are the most common, nevertheless there are tomographic classifications that include density, homogeneity of hematoma that are of great importance in the selection of the surgical technique. The presence of membranes and septum are only included in the evaluation of hematoma density. There are some others with predictive value that identify those factors related to recurrences before surgery that are considered a bad prognosis to the final evolution of these patients with this type of hematoma. Conclusions: The previous evaluation to surgery requires the use of different scales, the identification and characterization of membranes are important to select the optimum surgical technique to decrease morbidity and mortality after surgery.

RESUMO

Traumatic intracranial hematoma progresses rapidly and may cause quick increase of intracranial pressure and even brain hernia, ultimately leading to coma or death. Therefore, traumatic intracranial hematoma needs prompt treatment, but the prerequisite of treatment is early examination and diagnosis. Due to the limited transportation and other reasons, the existing large-scale detection devices such as CT and MRI cannot be deployed on the rescue site or during patient transportation. Instead, the portable diagnosis devices have the characteristics of miniaturization and high flexibility, which is conducive to promoting early detection, assisting diagnosis and further guiding the formulation of treatment plans. At present, more and more attention has been paid to the portable diagnosis devices in the diagnosis of intracranial hematoma. The authors summarized the conventional diagnosis methods and application of portable diagnosis devices for traumatic intracranial hematoma, aiming to provide a reference for the diagnosis of traumatic intracranial hematoma.

Objective:To investigate the clinical efficacy of neuroendoscopic hematoma removal versus soft channel drainage in the treatment of chronic subdural hematoma. Methods:The clinical data of 102 patients with chronic subdural hematoma who received treatment in Jincheng People's Hospital from May 2018 to May 2020 were retrospectively analyzed. They were divided into the neuroendoscopy group ( n = 50) and the soft channel group ( n = 52) according to different surgical methods. Perioperative indexes, hematoma clearance rate, China Stroke Scale score, the activity of daily living score, and oxidative stress indexes were compared between the two groups. All patients were followed up for 3 months. The incidence of complications during the follow-up period was calculated. Results:The retention time of the drainage tube in the neuroendoscopy group was shorter than that in the soft channel group [(2.45 ± 0.63) days vs. (3.30 ± 0.78) days, t = 6.06, P < 0.001]. The length of hospital stay in the neuroendoscopy group was shorter than that in the soft channel group [(7.14 ± 1.65) days vs. (9.07 ± 2.11) days, t = 5.15, P < 0.001]. The hematoma clearance rate at postoperative 7 days in the neuroendoscopy group was higher than that in the soft channel group [(93.45 ± 5.50)% vs. (81.86 ± 7.24)%, χ2 = 9.12, P < 0.001]. There were no significant differences in operation time and intraoperative blood loss between the two groups (both P > 0.05). At postoperative 30 days, the China Stroke Scale score in the neuroendoscopy group was lower than that in the soft channel group [(12.74 ± 2.23) points vs. (18.67 ± 2.45) points, t = 12.79, P < 0.001]. The activity of daily life score in the neuroendoscopy group was significantly higher than that in the soft channel group [(77.69 ± 7.11) points vs. (91.35 ± 7.25) points, t = 9.60, P < 0.001]. At postoperative 7 days, glutathione peroxidase level in the neuroendoscopy group was significantly lower than that in the soft channel group [(130.75 ± 13.66) U/L vs. (148.60 ± 14.64) U/L, t = 6.37, P < 0.001]. Malondialdehyde level in the neuroendoscopy group was significantly lower than that in the soft channel group [(5.11 ± 0.65) nmol/L vs. (6.19 ± 0.74) nmol/L, t = 7.83, P < 0.001]. Superoxide dismutase level in the neuroendoscopy group was significantly higher than that in the soft channel group [(275.60 ± 22.33) U/L vs. (254.60 ± 18.55) U/L, t = 5.15, P < 0.001]. There was no significant difference in the incidence of complications between the two groups ( P > 0.05). Conclusion:Compared with soft channel drainage, neuroendoscopic hematoma removal can obtain better short-term curative effects and less oxidative stress response in the treatment of chronic subdural hematoma. Neuroendoscopic hematoma removal does not increase the incidence of postoperative complications and is highly safe.

Presentamos el tratamiento eficaz de una filtración espontánea de líquido cefalorraquídeo (LCR) asociada a un síndrome de hipoten-sión/hipovolumen de LCR a nivel cervical alto, caracterizado por delirio y hematomas subdurales secundarios, refractarios al drenaje quirúrgico, que se resolvió con dos parches de sangre epidurales cervicales consecutivos.


We present the case of a cerebrospinal fluid (CSF) hypotension/hypovolume syndrome due to a spontaneous CSF fistula at the upper cervical level characterized by loss of consciousness and bilateral subdural hematomas refractory to two drainage surgeries that resolved with two consecutive blood patches on the leak site.

This article reviews the case of a 65-year-old patient with unstable L1 fracture after trauma. The fracture was treated via balloon kyphoplasty, shortly after which the patient developed shortness of breath and severe headache. Subsequent computed tomography (CT) of the head revealed subarachnoid hemorrhage. CT angiography did not reveal any intracranial aneurysms or arteriovenous malformations. A massive spinal subdural hematoma, which caused the patient to develop right leg paresis and hip joint weakness with grade 2-3, was found during magnetic resonance imaging (MRI). The hematoma was removed using multi-stage laminectomy Th5-L3. A follow-up MRI showed no pathological findings. Due to the unusual findings, spinal angiography was performed, revealing the artery of Adamkiewicz (A. radicularis magna, AKA) on the L1 level on the right side. Control CT showed a suboptimal insertion of the needle into the right pedicle, which caused the injury of the artery. AKA is present in the majority of the population, and surgical attention should be paid to avoid injury. Surgeons operating on the thoracolumbar spinal cord should have a thorough understanding of the anatomical features and surgical implications of this artery.
